Abstract
式(I)のアニリノピリミジン誘導体並びに、ポリエチレングリコール、カプリン酸のグリセリルエステル、又はそれらの混合物からなる群から選ばれる医薬として許容される賦形剤を含む液体充填組成物を含むカプセル剤形を含む経口医薬組成物に関する。当該液体充填組成物は、ハードゼラチンカプセルに剤形され、そして、癌、特にAMLの治療用の医薬の製造のために使用できる。
Description
PCT WO02/46171は、一連のアニリノピリミジン誘導体を記載し、そしてその中で、以下の式:1-(4{4-[4-(4-クロロ-フェニル)-ピリミジン-2-イルアミノ]-ベンゾイル}-ピペラジン-1-イル)-エタノン(上記式(I)を参照のこと)を有するアニリノピリミジン誘導体を記載する。
Frelinら(Blood、2005年1月15日、第105巻、第2号、804-811頁)は、AS602868、式Iの化合物に対応し、そして特許出願PCT WO 02/46171によりカバーされるアニリノピリミジン誘導体、が急性骨髄性白血病(AML)患者から得た新鮮な初代細胞培養の多くにおいて細胞死を誘導するということを示した。
急性骨髄白血病(AML)は、様々な成熟ステップで停止された白血病骨髄性芽細胞(leukemic myeloid blast)による骨髄の浸潤により特徴付けられる。AMLの異なるサブタイプの診断の進歩、及び治療アプローチの進歩にもかかわらず、現在の化学療法は、初回寛解しかもたらさない。再発がしばしば生じ、そしてAMLは70%を超える症例において最終的に致命的となる。悪性造血幹細胞/前駆細胞(白血病性幹細胞)の調節異常は、分化に不具合を有する芽細胞をもたらす。休眠しているか、又はゆっくりサイクルし、そうして化学療法に対して感受性の低いLSCが、疾患の再発の原因であり、そして将来の革新的な治療の標的となる。
LSCは、表現型においても、生物学的にも通常の造血幹細胞(HSC)に類似しており、識別及び標的化を難しくしている。1の興味深い差異は、LSC及びAMLブラストにおける核因子κB(NF-κB)転写因子の構成的活性化であり、HSCにおいては活性化されていない。このような異常な構成的NF-κB活性化は、他の造血系の2種の癌、ホジキンリンパ腫及び急性リンパ性白血病、並びに様々な固形癌において検出されてきた。NF-κBファミリーの転写因子は、細胞増殖及び生存、並びに腫瘍形成プロセスに関連する幾つかの遺伝子の制御発現のレギュレーターである。
例えば、NF-κBは、抗アポトーシスタンパク質(アポトーシスタンパク質の細胞性阻害因子1(c-IAP1)、cIAP2、bfl-1及びbcl-x1)をコードする遺伝子の発現を通して細胞の生存を促進する。NF-κBは、増殖因子(インターロイキン-2(IL-2)、顆粒球-マクロファージコロニー刺激因子(GM-CSF))、または細胞周期レギュレーター(サイクリンD1及びc-myc)の誘導を介して細胞増殖を刺激することが知られている。NF-κBは、多剤耐性(MDR)タンパク質の発現後の治療に対する腫瘍細胞の抵抗性に寄与する。NF-κBは、細胞外マトリクス変性酵素マトリクスメタロプロテイナーゼ9(MMP9)及びウロキナーゼ型プラスミノーゲンアクチベーター(uPA)の誘導を介して転移を促進することができる。さらに、多くの実際の新生物治療薬は、NF-κBも活性化してしまい、これは治療に干渉する事象である。
その結果、NF-κBの阻害が、癌のアジュバント治療となりうるということが提案されてきた。NF-κBダイマーは、IκBファミリーの阻害サブユニットにより細胞質中で不活性に維持されている。広範囲の刺激により細胞が誘発されると、IκB分子は、骨格タンパク質IKKγ/NF-κB必須モジュレーター(NEMO)と一緒に、NF-κB活性化についてのシグナルを統合するIKK複合体を形成する2つの関連生の高い特異的キナーゼ、IKK1、つまりIKKα、及びIKK2、つまりIKKβ、によって、2つの決定的なセリン残基上でリン酸化される。セリンリン酸化は、IκBのポリユビキチン化及びそれに続くプロテオソームによる分解をもたらす。IκB-α阻害分子のスーパーリプレッサフォームのトランスフェクションがNF-κBを阻害するのに特異性が高いならば、その使用は細胞株に制限されている。
特許出願PCT WO 02/46171によりカバーされるアニリノピリミジン誘導体を用いたIKK2キナーゼの薬物による阻害が、Jurkat白血病細胞においてTNF-α誘導性NF-κB活性化を抑制したということが示され(Frelinら、Oncogene 2003 (22) pages 8187-8194)、その結果これらの誘導体が、癌(好ましくは当該癌は、大腸癌、AML、直腸癌、前立腺癌、肝臓癌、肺癌、気管支癌、膵臓癌、脳癌、頭部癌、頸部癌、胃癌、皮膚癌、腎臓癌、頸部癌、血液癌、喉頭癌、食道癌、口腔癌、咽頭癌、精巣癌、膀胱癌、卵巣癌又は子宮癌である)の治療においてアジュバントとして使用できるということが示された。
本発明は、ハードカプセル中に封入するのに適している活性成分として式(I)のアニリノピリミジン誘導体を含む医薬組成物、並びにその経口投与により癌を治療する方法に関する。従って、本発明はカプセル剤形用の式(I)のアニリノピリミジン誘導体の液体充填組成物を提供する。好ましい実施態様では、当該カプセルはハードカプセルである。当該医薬組成物は、患者に良好なコンプライアンスを提供し、そしてin vivoにおいて高い生物利用能を可能にする。
液体充填組成物は、式(I)のアニリノピリミジン誘導体、及びポリエチレングリコール、カプリン酸のグリセリルエステル、又はそれらの混合物からなる群から選ばれる医薬として許容される賦形剤を含む。好ましい実施態様では、最終混合物、つまり式(I)のアニリノピリミジン誘導体及び賦形剤が液体形態となるように医薬として許容される賦形剤が選択される。液体充填組成物を封入するためのカプセル組成物は、ゼラチンでありうる。好ましい実施態様では、カプセルはハードカプセルであり、より好ましくはハードゼラチンカプセルサイズ00である。
好ましくは液体充填組成物は、熱可塑性物質であり、そして充填後に固体となる。
好ましい医薬として許容される賦形剤は、400〜800、より好ましくは570〜630から構成される平均分子量を有するポリエチレングリコール、例えばLutrol E600(登録商標)である。
液体充填組成物は、好ましくは3000〜5000、好ましくは3500〜4500から構成される平均分子量を有する第二ポリエチレングリコール、例えばPEG4000(商標)をさらに含んでもよい。
本発明の好ましい実施態様では、液体充填組成物は、5%〜15%の式(I)のアニリノピリミジン誘導体、65%〜85%の平均分子量3500〜4500を有するポリエチレングリコール、及び10%〜20%の平均分子量570〜630を有するポリエチレングリコールを含む。
より好ましくは、液体充填組成物は、11%の式(I)のアニリノピリミジン誘導体、79%の平均分子量3500〜4500を有するポリエチレングリコール、及び10%の平均分子量570〜630を有するポリエチレングリコールを含む。
或いは、医薬として許容される賦形剤は、カプリン酸のグリセリルエステル、より好ましくはモノカプリン酸グリセリル、例えばCapmul MCM(商標)から選ばれる。
さらに、本発明に記載の経口投与可能な医薬組成物は、式(I)のアニリニノピリミジン誘導体、及びポリエチレングリコールとカプリン酸のグリセリルエステルとの等モル混合物、好ましくはモノカプリン酸グリセリルと400〜800から構成される平均分子量を有するポリエチレングリコール、より好ましくは570〜630から構成される平均分子量を有するポリエチレングリコールとの等モル混合物を含んでもよい。
好ましくは、医薬組成物は、経口投与用の液体充填カプセルの形態に剤形される。
本発明の更なる態様によれば、癌、好ましくは急性骨髄性白血病の治療用の医薬の製造のための、本発明に記載の医薬組成物の使用が提供される。
本発明の更なる特徴は、制限することなく、純粋に説明を目的として提供される幾つかの実施例についての以下の記載から明らかとなるであろう。
実施例1
式(I)のアニリノピリミジン誘導体の可溶性試験
粉砕された薬剤物質の異なるバッチを製造した。式(I)のアニリノピリミジン誘導体の異なるサンプルを、異なるノズル圧力及び送り込み速度を用いて、ジェットミルマイクロナイザーPilotMill-3で微粉化した。窒素を処理ガスとして用いた。表1は、各製造バッチについて使用された全ての処理条件、及び得られた粒子サイズ分布(LLSにより計測される)を要約する。
式(I)のアニリノピリミジン誘導体の可溶性試験
粉砕された薬剤物質の異なるバッチを製造した。式(I)のアニリノピリミジン誘導体の異なるサンプルを、異なるノズル圧力及び送り込み速度を用いて、ジェットミルマイクロナイザーPilotMill-3で微粉化した。窒素を処理ガスとして用いた。表1は、各製造バッチについて使用された全ての処理条件、及び得られた粒子サイズ分布(LLSにより計測される)を要約する。
続いて、式(I)のアニリノピリミジンの可溶性を分析し、そして好ましい賦形剤を同定するために、バッチFD050007の10の溶液、及び開始バッチ61569-02の10の溶液を調製し、そして視覚可溶性試験で分析した。
式(I)のアニリノピリミジン誘導体の、異なる賦形剤中の見かけの可溶性についてのデーターサマリーを表2に与える。
表2に示される様に、式(I)のアニリノピリミジンは、粉砕したときに、特に粉砕し次にカプマルMCM C10(Capmul MCM C10)、ルトロールE600(Lutrol E600)中、及びカプマルMCM-ルトロールE600(1:1)中に溶解したときに、より優れた可溶性を示した。
実施例2
式(I)の微紛化アニリノピリミジン誘導体を含む液体充填カプセルの製造
式(I)の微紛化アニリノピリミジン誘導体と、実施例1に開示される賦形剤の中から選択された賦形剤(つまり、カプマルMCM C10、ルトロールE600及びカプマル MCM-ルトロールE600(1:1))の熱可塑性懸濁物を、ホモジェナイズにより調製した。ハードゼラチンカプセルサイズ00を充填するために、熱可塑性懸濁物を用いた。
式(I)の微紛化アニリノピリミジン誘導体を含む液体充填カプセルの製造
式(I)の微紛化アニリノピリミジン誘導体と、実施例1に開示される賦形剤の中から選択された賦形剤(つまり、カプマルMCM C10、ルトロールE600及びカプマル MCM-ルトロールE600(1:1))の熱可塑性懸濁物を、ホモジェナイズにより調製した。ハードゼラチンカプセルサイズ00を充填するために、熱可塑性懸濁物を用いた。
カプセルの製造後、熱可塑性懸濁物は固体となった。
異なる混合物を調製し、その組成を表3に示す(以下を参照のこと)。
実施例3
生物利用能の比較試験
実施例2に記載されるように、液体充填ハードゼラチンカプセル中のFD050124組成物を、生物利用能について、トップスプレー顆粒形態及びスプレー乾燥CD複合体形態(これらの製法は以下に記載される)に剤形された式(I)のアニリノピリミジン誘導体と比較をした。
生物利用能の比較試験
実施例2に記載されるように、液体充填ハードゼラチンカプセル中のFD050124組成物を、生物利用能について、トップスプレー顆粒形態及びスプレー乾燥CD複合体形態(これらの製法は以下に記載される)に剤形された式(I)のアニリノピリミジン誘導体と比較をした。
式(I)のアニリノピリミジン誘導体のトップスプレー顆粒の製法:
実施例1に得られるように、式(I)の微紛化アニリノピリミジン誘導体、バッチFE050007を用いて、流動層トップスプレー顆粒化により顆粒を製造した。トップスプレー顆粒プロセスにおいて、粉末粒子を気流中に流動化させ、そしてさらに、流動顆粒をノズルから粉体層にスプレーする。加熱しそしてフィルターを通した気体を管を通して噴きつけ粒子を流動化させ、そして粉末を混合させた。顆粒流体をリザーバーから、粒子層の上に位置するスプレーノズルへと送る。流体は、液滴及び粒子が衝突した際に、一次粉体粒子の接着を引き起こす。十分な液体がスプレーされて、必要とされるサイズの顆粒を生成し;次に当該スプレーをオフにするが、湿った顆粒を乾かすために加熱流動化気流はとどめておいた。ラクトース及び潤滑剤で最終希釈することは、良好な流動性を有する粉末を製造し、そしてサチェットの充填に進めるために必須であった。

スプレー乾燥によりさらに処理された液体CD-複合体を産生するため、実施例1で得られる式(I)の微紛化アニリノピリジン誘導体、バッチFD050007を用いた。

スプレー乾燥は、液体を加熱チャンバーに噴霧することにより、溶液又はスラリーを迅速に微粒子フォームへと乾燥させる。スプレー乾燥は、制御された条件の下で、水溶液系又は溶媒系を用いて行うことができる。原則的に、液体を蒸発させ、そして乾燥生成物を得るために、薬剤の溶液を高温及び高圧で密封容器にスプレーし、サイクロンを用いて乾燥生成物を回収した。サイクロンにおいて、乾燥生成物は処理ガスから分離される。当該技術の利点は、当該技術は、有効成分自体よりも可溶性の高い非結晶物質を得ることが可能であるという点である。
20.25mMの式(I)のアニリノピリミジン誘導体及び50mMのHP-β-CD(ヒドロキシ-プロピル-β-シクロデキストリン)(モル比1:2.5)を含む75:25のEtOH:H2O溶液を、有効成分をEtOHに、そしてHP-β-CDを精製水中に溶解することにより調製した。
次に、2つの溶液を所望の比で混合して、75:25のEtOH:H2Oの溶液を得、そして250rpmで48時間一定の速度で攪拌して、40℃の恒温槽に配置した。全ての場合、沈殿物は観測されなかった。Buchi Mini スプレー乾燥機B-191中において、溶液をスプレー乾燥することによって当該バッチを製造した。使用される作動条件は以下のとおりである:

サチェット充填に使用される物理的混合物を生成するために、さらなる混合ステップが必須であった。この更なる混合ステップから得られる混合物の組成を表5に要約する。
次にこの混合物を、一般的な医薬品製造慣習と同様に、例えばサチェット充填において使用される最終混合物を得る前に、充填し、続いて粉砕することによる2ステップの乾燥圧縮充填にかけた。
その結果、まずCD複合体をタブレトース80(Tablettose 80)の半量と混合し、そして11mmの錠剤に圧縮した。次に錠剤を0.5mmスクリーンFitzpatrickミル(FS75)に通して粉末化し、そして再び圧縮し、そしてスクリーニングした。最終的に、このようにして生成された乾燥顆粒を残りの半分のタブレトース80及び潤滑剤としてステアリン酸マグネシウムと混合した。
生物利用能の比較:
ICH条件化において貯蔵されたスプレー乾燥されたCD-複合体、液体充填カプセル、及び顆粒は、最大40℃±75%RHで6ヵ月後に安定であることが示された。

3つの製剤をラットにおいて経口試験した:

製剤(c):液体充填カプセル中の式(I)のアニリノピリミジン誘導体。

表7に示されるように、液体充填カプセルはin vivoで最も高い暴露を与え;薬剤の相対生物利用能は他の2つの製剤、つまりトップスプレー顆粒及びシクロデキストリン複合体で得られる相対生物利用能よりも約2倍高く、そして微紛化された式(I)のアニリノピリミジン誘導体で得られた生物利用能よりも約20%高いということが発見された。

- 前記医薬組成物が、ポリエチレングリコールとカプリン酸のグリセリルエステルとの等モル量の混合物を含むことを特徴とする、請求項1〜4のいずれか一項に記載の医薬組成物。
- 前記医薬組成物が、モノカプリン酸グリセリルと400〜800から構成される平均分子量を有するポリエチレングリコールとの等モル量の混合物を含むことを特徴とする、請求項10に記載の医薬組成物。
- 前記医薬組成物が、モノカプリン酸グリセリルと570〜630から構成される平均分子量を有するポリエチレングリコールとの等モル量の混合物を含むことを特徴とする、請求項10又は11に記載の医薬組成物。
- 前記カプセル剤形が、液体充填カプセルであることを特徴とする、請求項1〜12のいずれか一項に記載の医薬組成物。
- 前記液体充填組成物が、熱可塑性懸濁物である、請求項1〜13のいずれか一項に記載の医薬組成物。
- 前記熱可塑懸濁物が、充填後に固体となる、請求項14に記載の医薬組成物。
- 前記カプセル剤形が、ハードカプセルである、請求項13〜15のいずれか一向に記載の医薬組成物。
- 前記カプセル剤形が、ハードゼラチンカプセルサイズ00である、請求項16に記載の医薬組成物。
- 癌治療用の医薬の製造のための、請求項1〜17のいずれか一項に記載の経口医薬組成物の使用。
- 前記癌が、急性骨髄性白血病であることを特徴とする、請求項18に記載の使用。
