Abstract
Description
延出する取り付け構造体を有するインプラントを提供することが望ましい。
(a)それぞれがポリオレフィン材料の層を含む上面及び底面と、
(b)上面及び底面間に含まれる外科的グレードの金属メッシュと、
(c)インプラントを所望の表面に取り付けるようになっている1つ又は複数の取り付
け構造体とを備え、
インプラントは、手で操作することによって曲げることができるか又は変形することができ、変形したときに、変形後の形状を実質的に維持する、複合外科的インプラントに関する。外科的グレードの金属メッシュは、チタン、外科的グレードのステンレス鋼、チタンでコーティングされた鋼、窒化チタン、他の金属とのチタン合金、上述の材料のいずれかの複合材料、又はこれらの任意の組み合わせで構成されていてもよい。外科的グレードの金属メッシュは、ワイヤスクリーン、エキスパンドメタル、穿孔金属シート、穿孔棒、穿孔棒の相互接続された網目構造、格子、自由形態の固形物、穿孔か若しくは機械加工された形状のシート、又はこれらの任意の組み合わせをさらに含んでもよい。
(a)本体部と、取り付け構造部とを有する金属メッシュ材料を、取り付け構造部が棚状部に載るように金型の下側部分の空洞上に配置する工程、及び
(b)金型の前記空洞部に熱可塑性樹脂微粉を添加し、微粉によって金型の下側部分及びメッシュの本体部の空洞部を充填する工程を含む、外科的インプラントを作製する方法に関する。
(c)熱可塑性樹脂のシートを微粉及びメッシュ上に配置する工程、及び
(d)金型をシート上に配置すると共に金型に充填されている前記構成成分に熱及び圧力を加える工程であって、微粉を部分的に溶融し、且つ互いに融着させる工程をさらに含
んでもよく、
それによって、滑らかなバリア表面及び対向する多孔質表面を有するインプラントが作製される。

成長を促し、毛細管形成、骨形成を促進するのを助けること等のいずれかのために、生物学的に活性な分子、例えば成長因子、ホルモン、抗生物質、及び/又は任意の他の生物学的物質をインプラントに塗布することができる。細胞若しくはホルモン又は他の物質は、移植前にインプラントに局所的に塗布することができ、インプラントは、生物学的に活性な分子及び/又は細胞を含有する溶液に浸漬してもよく、この材料はシリンジによって噴霧又は塗布してもよく、また材料は、後にインプラントの孔に形成される徐放性の再吸収性ポリマーに溶解されるか、又は任意の他の適当な塗布方法を使用して塗布され得る。
Claims (35)
- 複合外科的インプラントであって、
(a)それぞれがポリオレフィン材料の層を含む上面及び底面と、
(b)前記上面及び前記底面間に含まれる外科的グレードの金属メッシュと、
(c)該インプラントを所望の表面に取り付けるようになっている1つ又は複数の取り付け構造体とを備え、
該インプラントは、手で操作することによって曲げることができるか又は変形することができ、変形したときに、変形後の形状を実質的に維持する、複合外科的インプラント。 - 前記外科的グレードの金属メッシュが、チタン、外科的グレードのステンレス鋼、チタンでコーティングされた鋼、窒化チタン、他の金属とのチタン合金、上述の材料のいずれかの複合材料、又はこれらの任意の組み合わせで構成される、請求項1に記載のインプラント。
- 前記ポリオレフィン材料が、ポリエチレン、高密度ポリエチレン、超高分子量ポリエチレン、ポリエーテルエーテルケトン、熱可塑性樹脂、ポリエチレンテレフタレート、ナイロン、1つ又は複数の二重結合を有する脂肪族炭化水素の任意のポリマー、上述の材料のいずれかの複合材料、又はこれらの任意の組み合わせを含む、請求項1又は2に記載のインプラント。
- 前記上面、前記底面又は両面が滑らかなバリア表面を有する、請求項1に記載のインプラント。
- 前記上面、前記底面又は両面が多孔質表面を有する、請求項1に記載のインプラント。
- 前記多孔質表面の孔が、線維血管の内方成長を可能にするようなサイズである、請求項5に記載のインプラント。
- 前記孔のサイズの範囲が約1μm〜約1000μmである請求項5に記載のインプラント。
- 前記上面又は前記底面のうちの一方が滑らかなバリア表面を有し、該上面又は該底面のうちの他方が多孔質表面を有する、請求項1に記載のインプラント。
- 前記1つ又は複数の取り付け構造体が、外科用ねじの頭部又は外科用骨アンカーを受け止め嵌合するための開口を前記メッシュ内に有する、請求項1〜8のいずれか1項に記載のインプラント。
- 前記1つ又は複数の取り付け構造体が、固定装置を受け止めるようになっている複数のリングから成る1つ又は複数のストランドを含む、請求項1〜9のいずれか1項に記載のインプラント。
- 前記1つ又は複数の取り付け構造体が、前記インプラントが骨に固定される1つ又は複数の位置で該インプラントに備え付けられている、請求項1に記載のインプラント。
- 前記1つ又は複数の取り付け構造体が1つ又は複数の角付きブラケットを含む、請求項1に記載のインプラント。
- 特定の手術部位に概ね適合する特定の形状で提供される、請求項1〜12のいずれか1
項に記載のインプラント。 - 細胞又は生物学的に活性な分子をさらに含む、請求項1〜13のいずれか1項に記載のインプラント。
- 使用中に前記インプラントの下に存在しうる神経又は血管を収容するようになっている開口、溝又はチャネルを含む、請求項1〜14のいずれか1項に記載のインプラント。
- さまざまな特徴を有するインプラントのキットで提供される、請求項1〜15のいずれか1項に記載のインプラント。
- 前記さまざまな特徴が、さまざまな厚さ、メッシュパターン、強度、サイズ、形状、柔軟性、播種の選択肢のうちの1つ又は複数、又はこれらの組み合わせを含む、請求項16に記載のインプラント。
- 前記金属メッシュが、下顎又は長骨の耐力のある復元を可能にするようなサイズであり、前記ポリオレフィン材料が、切除又は欠損した骨構造の体積充填能力を提供し、且つ前記インプラントが皮膚又は軟組織に触れる可能性を最小限に抑えるように金属構成要素上に滑らかな表面を提供する、請求項1〜17のいずれか1項に記載のインプラント。
- 前記外科的グレードの金属メッシュが、ワイヤスクリーン、エキスパンドメタル、穿孔金属シート、穿孔棒、穿孔棒の相互接続された網目構造、格子、自由形態の固形物、穿孔か若しくは機械加工された形状のシート、又はこれらの任意の組み合わせを含む、請求項1〜18のいずれか1項に記載のインプラント。
- 外科的インプラントを作製する方法であって、
(a)本体部と取り付け構造部とを有する金属メッシュ材料を、該取り付け構造部が棚状部に載るように金型の下側部分の空洞上に配置する工程、及び
(b)前記金型の前記空洞部に熱可塑性樹脂微粉を添加して、該微粉によって前記金型の前記下側部分及び前記メッシュの前記本体部の空洞部を充填する工程を含む、外科的インプラントを作製する方法。 - (c)熱可塑性樹脂のシートを前記微粉及び前記メッシュ上に配置する工程、及び
(d)金型を前記シート上に配置すると共に該金型に充填されている前記構成成分に熱及び圧力を加えて、前記微粉を部分的に溶融し、且つ互いに融着させる工程をさらに含み、
それによって、滑らかなバリア表面及び対向する多孔質表面を有するインプラントが組み立てられる、請求項20に記載の方法。 - (a)が、薄いバリアシートを前記金型の前記空洞の前記底面に配置する工程を含み、それによって、前記メッシュの対向する両側の面にバリアを含むインプラントが形成される請求項20に記載の方法。
- 前記微粉が、前記本体部の両方の側を被覆するように、第2の空洞を有する前記金型の上部に延びることができる、請求項21に記載の方法。
- 骨欠損を復元する方法であって、
(a)ポリオレフィン材料を含む上面及び底面と、前記欠損の輪郭に一致するように該材料に埋め込まれた金属メッシュと、インプラントから延出する1つ又は複数の取り付け構造体とを有する外科的インプラントを曲げる工程、及び
(b)前記インプラントを、該インプラントから延出する前記取り付け構造体を使用して、前記欠損近くの骨に機械的に取り付ける工程を含む、骨欠損を復元する方法。 - 前記欠損が頭蓋にある、請求項24に記載の復元する方法。
- 前記欠損が眼窩にある、請求項24に記載の復元する方法。
- 前記インプラントが、さらに滑らかな上部バリア表面及び底部多孔質表面を有し、且つ該滑らかな上部バリア表面を前記欠損とは反対側に向けて頭蓋欠損上に設置される、請求項24又は25に記載の復元する方法。
- 前記インプラントが、さらに滑らかな上部バリア表面及び底部多孔質表面を有し、且つ該滑らかな上部バリア表面を前記眼窩の欠損の方に向けて該眼窩に設置される、請求項24又は26に記載の復元する方法。
- 前記取り付ける工程が、前記取り付け構造体を通して前記骨内に機械的留め具を導入する工程を含む、請求項24〜26のいずれか1項に記載の復元する方法。
- 前記機械的留め具が外科用ねじを含む、請求項29に記載の復元する方法。
- 前記欠損の形状に一致するように前記インプラントを切断する工程をさらに含む、請求項24に記載の復元する方法。
- 骨欠損を修復するキットであって、
(a)1つ又は複数の外科的インプラントであって、それぞれがポリオレフィン材料の層を含む上面及び底面と、該上面及び該底面間に含まれる外科的グレードの金属メッシュとを含み、さらに該インプラントを所望の表面に取り付けるようになっている1つ又は複数の取り付け構造体を含み、手で操作することによって曲げることができるか又は変形することができる、1つ又は複数の外科的インプラントと、
(b)前記インプラントを成形する1つ又は複数の補助部品とを含む、骨欠損を修復するキット。 - 前記外科的インプラントが、1つ又は複数の異なるサイズ、形状若しくは厚さで提供され、且つ/又は1つ又は複数の異なる取り付け構造体を有する、請求項32に記載のキット。
- 前記インプラントを成形する前記1つ又は複数の補助部品が、前記インプラントの形状の透明な鋳型を含む、請求項32又は33に記載のキット。
- 以下の1つまたは複数を含む請求項32又は33に記載のキット。
(c)前記鋳型を切断するか又は前記インプラントの形を整えるハサミ
(d)前記インプラントの使用説明書
(e)(c)及び(d)の両方
