Abstract
Description
心房細動は、最も一般的な不整脈であり、高い罹患率および死亡率と関連している。その発生率および頻度はますます増加し、それは臨床的および経済的負担の増大となる(現在の頻度は全人口の2%である)。動悸、眩暈、呼吸困難などの重篤な臨床症状に加え、心房細動は、75歳以上の人々における虚血性脳梗塞において、単一の最も重要な因子である。全体的にみれば、心房細動は、心血管疾患による入院の5%以上を占める。心房細動は、約90%のケースにおいて、他の心疾患、例えば高血圧性心疾患、うっ血性心不全、または心臓弁疾患を伴い発生している。わずか10%のケースにおいて、心房細動は、他の心臓異常を伴わずに発生している(心房細動“単独”)。心房細動は三つの形態に分類される:(1)数秒〜数日にわたる期間の自己終結型心房細動の発症(episode)により特徴づけられる発作性心房細動、(2)医療介入により、終結されるまで無期限に続く持続性心房細動、そして(3)薬理的または電気的除細動により、終結されない恒久型心房細動である。
アデノシンは、体内において全ての代謝活性細胞により産生されるプリンヌクレオシドである。アデノシンは、細胞表面レセプターの四種の異なるサブタイプ:Gタンパク質結合レセプタースーパーファミリーに属するA1、A2A、A2B、およびA3を介して、その効果を発揮する。A1およびA3は阻害性Gタンパク質と結合するが、A2AおよびA2Bは刺激性Gタンパク質と結合する。A2Aレセプターは、脳において、ニューロンおよびグリア細胞の双方において主に見られる(線条および側坐核において最高レベル、嗅球と視床下部および海馬領域において中〜高レベル)。線条において、A2Aレセプターは、神経伝達物質の放出および機能を調節する(cf.H.Kase et al.,”Progress in pursuit of therapeutic A2A antagonists:the adenosine A2A receptor selective antagonist KW6002:research and development toward a novel nondopaminergic therapy for Parkinson’s disease”,Neurology,2003,vol.61,pp.97-100)。末梢組織において、A2Aレセプターは、血小板、好中球、脾臓、胸腺、内皮、および血管平滑筋細胞に存在することが知られ、そこでは冠状動脈心疾患の患者において、心筋灌流の評価をなしうる強い冠状血管拡張を誘導する(cf.Z.Gao et al.,”Novel short-acting A2A adenosine receptor agonists for coronary vasodilatation:inverse relationship between affinity and duration od action of A2A agonists”,J.Pharmacol.Exp.Ther.,2001,vol.298,pp.209-18)。しかしながら、A2Aレセプターは、これまでにヒト心房筋細胞に存在していないとされてきた。
一般的に、アンタゴニストは、レセプターを活性化することなしに、レセプターと結合する分子である。それはレセプターの結合部位における内在性リガンドまたは基質と競合し、よって内在性リガンド結合に応答して細胞内シグナルを伝達しうるレセプターの能力を阻害する。アンタゴニストは、いくつかの様式により機能している。それは、A2Aレセプターへのアデノシンの結合を実質的に妨害、阻止、または防止し、それによりアデノシン媒介生物活性を阻害、抑制するか、またはその停止を引き起こすために十分な親和性および特異性により、アデノシンと結合またはそれを隔離しうる。
治療に関して、有効量とは、有益または望ましい臨床結果に影響を与えるために(即ち、疾患または障害の進行を緩和、改善、安定、逆転、もしくは遅延させるか、または疾患もしくは障害の病的結果を和らげるために)十分である、活性成分の量に関する。有効量は通常、個別に医師により決定される。
図1Aは、抗A2Aレセプター(パネル1、4、7、および10)と抗ミオシン(2)、抗α‐アクチニン(5)、抗コネキシン‐43(8)、または抗RyR(11)により二重染色された、摘出直後のヒト右心房筋細胞を示す。二重免疫蛍光像の二重焼付け(パネル3、6、9、および12)においては、A2Aレセプターとα‐アクチニンとの共存およびA2Aレセプターとリアノジンレセプター(RyR)との重複分布を現した。図1Bは、SDS‐PAGEにより分離され、かつウサギ抗A2Aレセプター抗体を用いてイムノブロットされた、ヒトA2Aレセプターを有する一過性トランスフェクトHEK細胞からの細胞膜(陽性制御、C+)と、洞調律(SINUS)または心房細動(AF)を有する個体から得られたヒト心房膜とを示す。“Di”はダイマーを意味し、“Mo”はモノマーを意味する。図1Cは、図1Bのイムノブロットの相対濃度走査(Relative Densitometric Scan(RDS))を示す。A2Aレセプター(モノマー+ダイマー)の全量が黒棒により示される。A2Aレセプターモノマー(白棒)およびダイマー(灰色棒)の相対量は、各列において、A2Aレセプター(ダイマー+モノマー)の全合計を用いて標準化される。
ヒト右心房筋細胞におけるA2Aレセプターの存在をイムノブロッティングにより調べた。その結果、A2Aレセプターが右心房に存在し、該レセプターのモノマーおよびダイマー種双方がこの組織において発現され、洞調律のある心房の試料においてダイマー型が少数であることを示した。心房筋におけるA2Aレセプター局在性が、レセプターA2Aとミオシン、α‐アクチニン、コネキシン‐43、またはRyRに対する抗体により二重標識された試料によって、共焦点顕微鏡観察により研究された。A2Aレセプターは心筋線維に沿い横縞状パターンで発現され、筋節においてZ線のレベルで細胞骨格関連タンパク質α‐アクチニンと共存した。A2Aレセプターの分布は、心房細動を有するおよび有しない患者の試料において質的に類似した。
摘出された心房筋細胞は、A2Aレセプター縞状パターンと、心房組織サンプルとにおいて見られるα‐アクチニンとのコロニー化およびRyRとの重複分布を保持していた(図1A参照)。心房細動を有するおよび有しない患者の心筋細胞において、レセプター局在性に差異が観察されなかった(データ示さず)。ウエスタンブロッティングにより測定されたA2Aレセプタータンパク質レベルは心房細動のある患者において著しく増加し(図1B‐C参照)、興味深いことに、細胞表面に存在する機能型であるダイマーA2Aレセプター種が心房細動において著しく上昇した(図1C参照)。有意差は、血管において観察されなかった。したがって、これらの結果は、ヒト心房筋細胞におけるA2Aレセプターの発現のみならず、心房細動のある患者において、主にホモダイマー型でのそれらの上方調節も初めて証明した。
自発的SRカルシウム放出に及ぼすA2Aレセプターアゴニストの効果を、洞調律のある患者および心房細動のある患者から摘出された心房筋細胞により分析した。局所的な非伝播カルシウム放出(カルシウムスパーク)とSRからの再現的カルシウム放出(カルシウム波)との双方を分析した。A2AレセプターアゴニストCGS21680(200nM)とのプレインキュベーションは、心房細動を有する患者においてカルシウム波の数を増加させたが、不整脈を有しないものではそうではなかった。
ヒト心房筋細胞における基礎cAMPトーン(tone)の存在が、L型カルシウム電流の神経ホルモン調節の研究において主張された。PKA遮断剤H‐89が、カルシウム波振動数を基準レベル以下に減少させるという本発明者らの観察は、ヒト心房筋細胞で基礎A2Aレセプター依存性cAMPトーンがあることをさらに示唆している。この考えと一致して、基礎SRカルシウム放出の上昇を示す心房細動患者からの細胞に適用されたA2Aレセプター選択的アンタゴニストSCH58261またはZM241385は、共焦点顕微鏡観察またはパッチ‐クランプ技術により測定されるカルシウム波振動数を有意に減少させた(図3参照)。
心臓手術を受ける合計54例の患者からの心臓組織試料をこの研究において用いた。心房組織試料は手術時に通常捨てられる組織からなるが、この研究において用いられる許諾が各患者から得られた。該研究は、Sant Pau病院(Barcelona,Spain)の倫理委員会により承認された。
次の一次抗体を用いた:A2Aレセプターの第二細胞外ループに相当するペプチドに対して作製されたウサギアフィニティー精製抗A2Aレセプターポリクローナル抗体VC21‐Ab、ウサギ抗A2Aレセプター抗体(クローンPA1‐042,Affinity BioReagents,Golden,CO,U.S.A.)、マウス抗遅筋ミオシン抗体(クローンNOQ7.5.4D,Chemicon International,Temecula,California,U.S.A.)、マウス抗α‐アクチニン抗体(クローンEA‐53,SigmaAldrich Chemical Co.,St.Louis,MO,U.S.A.)、マウス抗コネキシン‐43抗体(クローン2,Transduction Laboratories,Lexington,KY,U.S.A.)、マウス抗リアノジンレセプター抗体(クローンC3‐33,Calbiochem,San Diego,California,U.S.A.)。用いられた二次抗体は、ALEXAFLUOR 488複合ヤギ抗マウスIgG(1/1000)、TEXAS RED複合ヤギ抗ウサギIgG(1/2000)(Molecular Probes,Eugene,OR,U.S.A.)、西洋ワサビ(horseradish)ペルオキシダーゼ複合ヤギ抗ウサギIgG(1/60000)(Pierce Chemical Co.,Rockford,IL,U.S.A.)、およびウサギ抗マウスIgG(1:2000)(Dako,Glostrup,Denmark)であった。
前記ようにヒト心房組織試料から筋細胞を摘出した(cf.L.Hove-Madsen et al.,”Atrial fibrillation is associated with increased spontaneous calcium release from the sarcoplasmic reticulum in human atrial myocytes”,Circulation,2004,vol.110,pp.1358-63)。簡単に言えば、試料をすすぎ、30mMブタンジオンモノオキシム(BDM)を含有した無カルシウム溶液中において細断し、0.5mg/mLコラゲナーゼ(Worthington2型,318u/mg)および0.5mg/mLプロテイナーゼ(Sigma XXIV型,11u/mg固体)を含有した無Ca2+溶液中、35℃においてインキュベートした。45分間後、組織を酵素溶液から取り出し、細胞を無Ca2+溶液中パスツールピペットによりほぐした。残りの組織は0.4mg/mLコラゲナーゼを含有した新鮮無カルシウム溶液中において、3×15分間かけて消化した。ほぐした細胞を含有する溶液を600rpmで1分間遠心し、無カルシウム溶液に再懸濁し、次いでカルシウムを1mMまで徐々に増やした。透明な横縞のある粒状でない細長い細胞のみを実験に用いた。
HEK‐293細胞をダルベッコ変法イーグル培地(DMEM,Sigma Aldrich Chemical Co.)により増殖させた。リン酸カルシウム沈降法により、ヒトA2AレセプターについてコードするDNA(cf.M.Canals et al.,”Adenosine A2A-dopamine D2 receptor-receptor heteromerization:qualitative and quantitative assessment by fluorescence and bioluminescence energy tranfer”,J.Biol.Chem.,2003,vol.278,pp.46741-9)により細胞を一過性にトランスフェクトした。細胞をトランスフェクションから24時間後または48時間後に収集した。
レーザー走査共焦点顕微鏡(Leica TCS SP2 AOBS,Germany)を用いて、fluo‐3負荷細胞によりカルシウムスパークおよびカルシウム波を検出した。実験溶液はNaCl 136(mM)、KCl 4、NaH2PO4 0.33、NaHCO3 4、CaCl2 2、MgCl2 1.6、HEPES10、グルコース5、ピルビン酸5(pH=7.4)を含有していた。1〜5%に減衰された488nmの励起により500〜650nmにおいて蛍光を集めた。カルシウムスパークおよびカルシウム波は1kHzの走査速度で2×10.24sにわたり静止条件下において測定した。カルシウムスパークは、隣接3μmセクションには検出可能な増加がなく、カルシウムスパークの中心を含む3μmセクションでのシグナル量の増加として検出した。2以上の隣接3μmセクションにおけるシグナル量の増加をカルシウム波として計測した。各カルシウムスパークの振幅とその半減期とをカルシウムスパーク過渡現象の減衰期の指数的激変から求めた。カルシウムスパーク振動数を各細胞において求め、走査された細胞の長さに対して標準化した。
カルシウム波に伴う一過性内向きNa‐Ca交換電流を、ソフトウェア制御パッチ‐クランプ増幅器(EPC10,HEKA,Germany)を用いて、穿孔パッチ形状により記録した。ピペット抵抗は、1.5〜4MΩであった。実験は、室温において行い、アクセス抵抗が安定して、ピペット抵抗の5倍未満に減少したときに始めた。細胞外溶液は、NaCl 127(mM)、テトラエチルアンモニウム(TEA)5、HEPES 10、NaHCO3 4、NaH2PO4 0.33、グルコース10、ピルビン酸5、CaCl2 2、MgCl2 1.8(pH=7.4)を含有していた。ピペット溶液は、アスパラギン酸 109(mM)、CsCl 47、Mg2ATP3、MgCl2 1、Na2ホスホクレアチン5、Li2GTP0.42、HEPES 10、および250μg/mLアンホテリシンB(pH=7.2)を含有していた。A2AレセプターアゴニストCGS21680、アンタゴニストZM241385およびSCH58261とプロテインキナーゼAインヒビターH‐89をDMSOにすべて溶解し、10mMストック溶液として保管した。細胞を10mMカフェインへ一時的に暴露することにより、カフェイン放出性SRカルシウム含有量を測定した。得られたNa‐Ca交換電流の時間積分量を、Na‐Ca交換器により3Na+:1Ca2+の化学量論と仮定して、SRから放出されたカルシウムのpmole(10−18モル)に変換した。
電気生理学的および共焦点カルシウム画像法実験を患者の臨床データに関する知識無しに実施した。同一患者の細胞において記録されたCa2+スパークおよびCa2+波を平均化した。別記されない限り、各患者の平均値を統計分析に用い、平均±S.E.M.として表示した。一連のデータを正規性について検定した。具体的効果を検定する際に、有意差を評価する上でスチューデントt検定を用いた。ANOVAは複数効果の比較に用い、スチューデント‐ニューマン‐クールス事後検定は具体的効果の有意性を評価するために用いた。
0.32Mスクロース含有5mMトリスHCl(pH7.4:緩衝液A)の氷冷溶液中、Polytronホモゲナイザー(Kinematica,PTA 20TSローター,セッティング5,3回の15s期)による細胞の破壊後に、遠心により膜を得た。4℃において30分間にわたる1000×gの遠心(タイプ75Tiローターで3500rpm)により細胞残屑(cell debris)を分離した。上澄みを4℃において、30分間にわたり26000×gにより遠心し(タイプ75Tiローターで20000rpm)、ペレットを緩衝液Aに再懸濁し、37℃において30分間インキュベートし、同条件により遠心した。ペレットを0.3M KClに再懸濁し、4℃において一晩攪拌し、4℃において30分間にわたり26000×gにより遠心し(タイプ75Tiローターで20000rpm)、上記のような緩衝液Aにより一度洗浄した。最終ペレット(膜フラクション)を50mMトリスHClに再懸濁し、−80℃において凍結した。トランスフェクトされたHEK細胞からの膜懸濁物は上述のとおり、得た(C.Herrera et al.,“Adenosine A2B receptors behave as an alternative anchoring protein for cell surface adenosine deaminase in lymphocytes and cultured cells”,Mol.Pharmacol.,2001,vol.59,pp.127-34参照)。タンパク質をビシンコニン酸法(Pierce Chemical Co.)により定量した。
ヒト細胞または一過性トランスフェクトHEK細胞からの膜を37℃において、2時間加熱することによりSDS‐PAGE試料緩衝液(8M尿素,2%SDS,100mM DTT,375mMトリス,pH6.8)により処理し、10%ゲルでSDS‐ポリアクリルアミドゲル電気泳動により分離させた。セミドライ・トランスファー・システムを用いてタンパク質をPVDF膜へ移し、指定された抗体、次いで西洋ワサビ(horseradish)ペルオキシダーゼ(HRP)複合ヤギ抗ウサギIgG(1/60000)によりイムノブロットした。化学発光検出キット(SuperSignal,West Pico Chemiluminiscent基質,Pierce)を用いて、免疫反応バンドを展開させた。
免疫組織化学法では、ヒト心房組織をOCTに包埋し、液体窒素冷却イソペンタンにより凍結させた。18℃に冷却されたクリオスタットにより、8μm断片に切断した。各断片をSUPERFROST PLUS(BDH Chemicals Ltd.,Poole,United Kingdom)スライド上に集め、風乾し、−70℃において貯蔵した。免疫蛍光法においては、トリス緩衝液(TBS)(150mM NaCl/50mMトリスHCl,pH7.5)中10%ロバ血清で30分間にわたり各断片を遮断した。スライドをアフィニティー精製抗A2Aレセプター(VC21,25μg/mL)および抗α‐アクチニン(1:500希釈)、抗ミオシン(2μg/mL)、抗コネキシン‐43(2.5μg/mL)、および抗リアノジンレセプター(6.5μg/mL)を室温において1時間インキュベートし、次いで10分間にわたりTBSによって3回洗浄した。ALEXA FLUOR 488複合ヤギ抗マウスIgGおよびTEXAS RED複合ヤギ抗ウサギIgGを1:2000の希釈で遮断溶液に適用した。断片をすすぎ、VECTASHIELD免疫蛍光培地(Vector Laboratories Inc.,Burlingame,CA,U.S.A.)によりマウントした。免疫組織化学法においては、ポリ‐D‐リジンで被覆された24mmガラスカバースリップに摘出直後のヒト筋細胞を置いた。付着後、摘出溶液細胞をリン酸緩衝液ですすぎ、室温において20分間かけてPBS中2%パラホルムアルデヒドにより固定した。細胞をPBSにより洗浄し、アルデヒド基を封止するために0.1Mグリシンと5分間インキュベートした。PBSによる洗浄後、PBS中0.2%トリトン‐X‐100で15分間かけて筋細胞の透過性を上げた。次いで細胞をPBSにより洗浄し、室温において10%ウマ血清で30分間遮断し、次の一次抗体:ウサギポリクローナル抗A2Aレセプター(10μg/mL)、抗α‐アクチニン(1:500希釈)、抗ミオシン(1:500希釈)、および抗コネキシン‐43(1:150希釈)と室温において1時間インキュベートし、PBSにより3回洗浄し、ALEXA FLUOR 488と複合化された抗マウス抗体および/またはTEXAS REDと複合化された抗ウサギ抗体により染色した。最後に、細胞を再び3回すすぎ、VECTASHIELDでマウントした。Leica TCS4D共焦点レーザー走査顕微鏡(Leica Lasertechnik GmbH,Heidelberg,Germany)で共焦点顕微鏡観察を行なった。
