Abstract
【解決手段】貫通腔を形成し、該貫通腔を拡大する貫通腔形成及び拡大装置であって、径方向へ拡張可能な管状編み紐20を備え、基端部と終端部と第一断面積を有する軸線方向の内腔とを有する長い拡張部材10と、該拡張部材の終端部に設けられ、該拡張部材が皮膚を貫通して進められると組織に腔を開ける組織腔開け手段と、終端部と基端部と第二断面積を有する軸線方向の内腔とを有する一定半径の管状要素を備えた長い拡大部材12,14と、該拡大部材の終端部に設けられ、前記管状要素を該拡張部材の軸線方向の内腔へ容易に挿入できるようにする挿入促進手段とを具備することを特徴とする貫通腔形成及び拡大装置。

Description
またトロカールの外周の周りでの外側シールは適切に取り組まれでこなかった。あるトロカールの設計では、その外周の周りにテーパ付きの螺旋ねじを採用している。この螺旋ねじは固定するには有用であるが、加圧された注入ガスの損失を防ぐのにシールするためにトロカールの穴を非常に大きくしてしまう。マルコー構造(malecot structure)のような他の固定具をトロカールの終端部で用いたものもあり、固定するには有用であるが、圧力損失に対しあまりシールできない。
896,669号、米国特許第4,479,497号、米国特許第3,902,492号には拡張可能な構造を有するカテーテルが開示されている。
2.前記長い拡張部材は前記拡張可能な管状編み紐を覆って形成された取り外し可能な鞘をさらに有し、該鞘は前記管状編み紐を覆いつつ前記拡張部材が皮膚を貫通して進められ、その後、取り外され、腔が形成された組織に前記管状編み紐を露出することを特徴とする項目1に記載の貫通腔形成及び拡大装置。

5.前記挿入促進手段は前記拡大部材の軸線方向の内腔に取り外し可能に収容されたテーパ付き終端部を有するロッドを具備し、該テーパ付き終端部は、前記ロッドが完全に挿入されたとき、前記拡大部材から終端部方向へ延びることを特徴とする項目1に記載の貫通腔形成及び拡大装置。
6.径方向へ拡張可能なトロカールシステムであって、 基端部と終端部とを有し、第一断面積を有する軸線方向の内腔を有する径方向へ拡張可能な管状編み紐を有する長い拡張部材と、 基端部と先端の尖った終端部とを有する長い貫通腔形成要素と、該貫通腔形成要素は前記管状編み紐の軸線方向の内腔内に取り外し可能に収容され、前記先端の尖った終先端は前記長い拡張部材の終端部から終端部方向へ延びており、 基端部とテーパ付き終端部とを有し、前記管状編み紐の第一断面積よりも大きい断面積を有する軸線方向の内腔を有する一定半径の管状要素を有する第一の長い拡大部材と、 該第一の長い拡大部材の基端部に取り外し可能に取り付けられたトロカール弁とを具備しており、該トロカール弁は前記一定半径の管状要素の前記軸線方向の内腔の終端部に配置されていることを特徴とするトロカールシステム。
7.前記径方向へ拡張可能な管状編み紐は非弾性フィラメントからなるメッシュを具備し、前記管状編み紐の長さはその半径が大きくなると短くなることを特徴とする項目6に記載のトロカールシステム。
8.前記長い拡張部材は前記管状編み紐を覆う取り外し可能な鞘を有し、前記拡張可能な管状編み紐はその基端部で第一取っ手に取り付けられ、前記取り外し可能な鞘はその基端部で第二取っ手に取り付けられ、前記第二取っ手は前記第一取っ手の終端部方向へ位置し、前記第二取っ手を用いて前記管状編み紐を覆う前記鞘を取り外すことが可能であることを特徴とする項目7に記載のトロカールシステム。
9.前記取り外し可能な鞘は両側部又は一方の側部のみに軸線方向へ切り目を付けられた円筒形の重合体チューブであり、前記第二取っ手は前記重合体チューブの軸線方向の切り目と整列した位置で弱くなっており、前記鞘を取り外し易いように前記鞘及び前記取っ手を割ることが可能である項目8に記載のトロカールシステム。
10.前記拡張部材の前記管状編み紐は弾性又は熱可塑的に変形可能なマトリックス層の編み紐フィラメントを具備する積層構造であることを特徴とする項目7に記載のトロカールシステム。
11.前記長い貫通腔形成要素はその終端部に後退可能なオブトラトールを有する針部材であることを特徴とする項目6に記載のトロカールシステム。
12.前記長い拡大部材は前記管状要素の軸線方向の内腔に取り外し可能に収容されたテーパ付き終端部を有するロッドをさらに有し、該テーパ付き終端部は、前記ロッドが前記管状要素に完全に収容されたとき、前記管状要素から終端部方向へ延びることを特徴とする項目6に記載のトロカールシステム。
13.前記一定半径の管状要素はの終端部付近に拡大可能な固定部をさらに有することを特徴とする項目12に記載のトロカールシステム。
14.前記長い拡大部材は前記拡大部材の前記内腔へ開放したコネクタをその基端部にさらに有し、該コネクタは前記トロカール弁を着脱可能に取り付けることを特徴とする項目6に記載のトロカールシステム。
15.前記コネクタは差込み式適合具であることを特徴とする項目14に記載のトロカールシステム。
16.前記トロカールシステムは軸線方向の通路を備えたボディを有する弁本体と、前記軸線方向の通路に設けられ、該通路を通る圧力損失を防ぐ弁手段と、前記ボディに設けられ、前記軸線方向の通路と整列し、前記ボディを前記長い拡大部材の基端部へ取り外し可能に取り付ける取付け手段とを具備することを特徴とする項目6に記載のトロカールシステム。
17.少なくとも1つの付加的な長い拡大部材と、基端部と、テーパ付き終端部とをさらに有し、前記管状編み紐の断面積よりも大きく且つ前記第一の長い拡大部材の断面積とは異なる断面積を有する軸線方向の内腔を有する一定半径の管状要素を有することを特徴とする項目6に記載のトロカールシステム。
18.径方向へ拡張可能な拡張部材と、該拡張部材の軸線方向の内腔を通って挿入可能な長い拡大部材とを有する改良型の径方向へ拡張可能な拡張器であって、前記改良は、非弾性のフィラメントのメッシュを具備する径方向へ拡張可能な管状編み紐を有する拡張可能な拡張器を具備し、該管状編み紐の長さは、径方向へ拡張すると短くなることを特徴とする拡張器。
19.前記拡張手段が前記管状編み紐を覆う取り外し可能な鞘をさらに具備することを特徴とする項目18に記載の改良型の径方向へ拡張可能な拡張器。
20.前記拡張部材の前記管状編み紐が、弾性又は熱可塑的に変形可能なマトリックス層の編み紐フィラメントを具備する積層構造であることを特徴とする項目18に記載の改良型の径方向へ拡張可能な拡張器。
21.基端部にトロカール弁を有する一定半径の管状要素を有するタイプの改良型のトロカールであって、該改良が、前記トロカール弁を着脱可能に取り付ける取付け手段を有する異なる一定半径の複数の管状要素と、該複数の管状要素を選択的に収容し且つ該管状要素に適合する径方向に拡張可能な軸線方向の内腔を形成する形成手段とを具備し、該形成手段が、管状部材を導入し且つ相互に交換するためのアクセス内腔を提供するよう皮膚を貫通して導入されると共に、前記トロカール弁が、導入されたあと各管状部材へ選択可能に取り付けられることを特徴とする改良型のトロカール。
22.前記形成手段は、径方向へ拡張可能な長い管状編み紐と、前記管状編み紐の終端部に設けられ、該管状編み紐が皮膚を貫通して進められると組織に腔を開ける組織腔開け手段とを具備することを特徴とする項目21に記載の改良型のトロカール。
23.前記組織腔開け手段は、先端の尖った終先端を有する貫通孔形成要素であって、径方向へ拡張可能な長い管状編み紐の軸線方向の内腔内に取り外し可能に収容される長い貫通腔形成要素を具備することを特徴とする項目22に記載の改良型のトロカール。
24.各管状要素は、その軸線方向の内腔に収容されるロッドを有し、該ロッドは前記管状要素の終端部から終端部方向へ延びるテーパ付き終端部であって、前記径方向へ拡張可能な長い管状編み紐の前記軸線方向の内腔への挿入を容易にするテーパ付き終端部を有することを特徴とする項目23に記載の改良型トロカール。
25.患者の体内の目標箇所へ皮膚を貫通した貫通腔を形成し、該貫通腔を拡大する貫通腔形成及び拡大方法であって、 組織を通って目標位置まで長い拡張部材を貫通する貫通工程であって、該拡張部材が非弾性フィラメントのメッシュで形成された管状編み紐を有し、前記非弾性フィラメントが径方向へ拡張するとその長さが軸線方向へ短くなる貫通工程と、 前記管状編み紐の内腔に第一の一定半径の管状要素を挿入する挿入工程であって、該管状要素の前記内腔が前記管状編み紐の前記内腔の断面積よりも大きい断面積を有し、前記メッシュが短くなることで前記拡張部材が組織に固定される挿入工程とを具備することを特徴とする貫通腔形成及び拡大方法。
26.径方向へつぶれるが概して軸線方向へは固定されたままである前記管状編み紐の前記内腔から前記第一の一定半径の管状要素を取り外す取外し工程と、前記管状編み紐の前記内腔に第二の一定半径の管状要素を挿入する挿入工程であって、該第二の一定半径の管状要素の断面積が、前記第一の一定半径の管状要素の断面積とは異なる挿入工程とを具備することを特徴とする項目25に記載の貫通腔形成及び拡大方法。
27.前記第二の管状要素の断面積は前記第一の管状要素の断面積よりも大きいことを特徴とする項目26に記載の貫通腔形成及び拡大方法。
28.前記第二の管状要素の断面積は前記第一の管状要素の断面積よりも小さいことを特徴とする項目26に記載の貫通腔形成及び拡大方法。
29.前記長い拡張部材は前記管状編み紐を覆って配置されたカバーを具備し、当該貫通腔形成及び拡大方法は、前記管状要素を挿入するまえに前記管状編み紐を露出するために前記カバーを取り外す取外し工程を具備することを特徴とする項目25に記載の貫通腔形成及び拡大方法。
30.前記カバーは、前記管状編み紐を割り、剥き、引き抜くことにより取り外される重合体材料からなる薄壁シートであることを特徴とする項目29に記載の貫通腔形成及び拡大方法。
31.前記長い拡張部材は、先端の尖った終先端を有する長い貫通腔形成要素を具備し、該終先端は前記拡張可能な管状編み紐の軸線方向の内腔内に取り外し可能に収容され、前記管状編み紐の軸線方向の内腔から終端部方向へ延び、当該貫通腔形成及び拡大方法は、前記拡張部材を貫通させたあとで且つ第一の半径一定の管状要素を挿入するまえに前記管状編み紐から前記長い貫通腔形成要素を取り外す取外し工程を具備することを特徴とする項目25に記載の貫通腔形成及び拡大方法。
32.前記第一の半径一定の管状要素は、その軸線方向の内腔に取り外し可能に収容されたテーパ付き終端部を有するロッドを有しており、該テーパ付き終端部は前記管状要素から終端部方向へ延び、前記管状編み紐の前記軸線方向の内腔への挿入を容易にし、当該貫通腔形成及び拡大方法は、アクセス路を残したまま前記管状要素から前記ロッドを取り外す取外し工程を具備することを特徴とする項目25に記載の貫通腔形成及び拡大方法。
33.前記管状編み紐の前記メッシュが弾性又は熱可塑的に変形するマトリックスで積層にされていることを特徴とする項目25に記載の貫通腔形成及び拡大方法。
本発明のさらなる好ましい局面においては、長い貫通孔形成手段は、その終端部に後退可能なオブトラトールを有する針部材である。オブトラトールは、針部材が皮膚を貫通すると後退させられ、針部材が患者の体内へ入ると直ぐに前方へ延び、したがって針部材による不意の外傷から患者を守るのに役立つ。例として挙げた針部材はガス注入装置で一般的に使用されるタイプのベレスニードル(Veress needle)である。
これら各基本構成要素と、本発明の方法におけるこれら基本構成要素の使用法を以下で詳細に説明する。
ここで図8を参照すると、第一の拡大部材12の取っ手70(及び第二の拡大部材14の取っ手70’)は、トロカール弁16を取り外し可能に収容するように構成されている。トロカール弁16は、このトロカール弁16がその終端部に取付け手段を有することを除いて、従来の構造と同じである。図8に示したように、取付け手段は、取っ手70の基端部半体70Bの雌差込み凹部92内に着脱可能に収容される雄差込み取付け部90を具備する。

面78及び80によって形成されたテーパ付き終端部が取っ手22の通路26へ挿入されて、管状編み紐20の軸線方向の内腔へ入る。選択可能には針部材40を引き抜き、金環24が鞘を通ることにより鞘30が割られる。選択可能には、拡大部材12を挿入することによって図9に示したように鞘30を軸線方向へ割る。明らかに、拡張部材12を終端部方向へ前進することによって管状編み紐20を径方向へ拡張し、結果として図10で示したような完全な管状編み紐へ拡張される(ここでは内部ロッド74は取り外されている)。
10に示したように、管状編み紐の終端部から終端部方向へ延びている。それから(拡大部材が通ることにより割られた)鞘30は、取っ手32を使用して引き抜かれ、内部ロッド74が取っ手76を使用して引き抜かれる。それから取っ手70の半体70A及び70Bが軸線方向へ引き離されて、マルコー構造72を展開する。それからトロカール弁16は取っ手70の基端部半体70Bに取り付けられ、概して図10に示したような構造になる。注意していただきたいのは、拡張された管状編み紐20内の構造的な支持を提供するのは一定半径の管状要素60であるということである。またトロカール弁16から構造の終端部まで所望のアクセス路を提供するのは一定半径の管状要素60内の軸線方向の内腔である。
した径方向へ拡張した形態への管状編み紐20の拡張を説明する。図12Aは、例としてエクスパンドピーティ(Expando PT)編み紐が約0.1インチから0.45インチヘ径方向へ拡張したときの短くなるパーセンテージを示している。
図13〜20を参照し、皮膚を貫通して腔を開ける本発明の例としてのトロカールシステムの使用の仕方を説明する。初めに長い拡張部材10が腔を開けるのに望ましい場所である患者の皮膚Sの位置に配置される。それから図14に示したように、拡張部材10は皮膚を貫通して針部材40の先の尖った終先端42(図1参照)を前進することにより皮膚Sを貫通する。腹腔鏡法の場合は、針部材40の先端の尖った終先端42が皮膚を通って腹部へ貫通すると直ぐに、オブトラトール48の先が丸くなった端部50が自動的に延び、患者の内蔵を不注意に傷つけてしまうことから保護する。所望であるならば、この時点で針部材40の内腔をガス注入に使用することもできる。
(針部材40と取り付けられた金環24とを引き抜いて割られた)鞘と、基端部に取っ手22を備えた管状編み紐とを残して、取っ手44を用いて引き抜かれる。
次に長い拡大部材12が取っ手22の通路26へ導入され、したがって図16に示したように管状編み紐20を拡張し、(まだ割られていないならば)鞘30を割る。鞘30と管状編み紐20とがあることで、図16に示したような、皮膚Sを貫通して形成された貫通腔を径方向へ拡大することは容易である。
内部共軸ロッド66が一定半径のチューブ60から引き抜かれ、拡張した管状編み紐の周りの鞘30が引き抜かれる。図17に示すように、マルコー構造72は半体70A及び70Bを引き離すことで拡大される。それからトロカール弁16が、図18に示したように、取っ手70の基端部半体70Bに取り付けられる。
