Abstract

本发明公开了一种一体式支架置入器,包括前手柄和后手柄,所述前手柄前端设有外管,外管顶端活动连接有切割头,外管内设有中管以及支架;后手柄包括用于支撑的不锈钢管以及位于不锈钢管内的内管,内管顶端与切割头固定连接;中管的一端与支架的一端相互抵接,中管的另一端与所述不锈钢管相互连接,支架的一端与切割头靠近,留有一定间隙;当前手柄沿不锈钢管后撤时,外管同时后撤且与切割头分离,中管和后手柄保持位置不变,支架自动释放。本发明在病灶器官切开后可直接置入支架,建立了病灶器官与外界的通道,可以继续使用其他器械进行感染物或坏死组织清除等其他手术操作,增加了手术的综合性,使病人可以避免承受多次手术的痛苦。

Description

一种一体式支架置入器
技术领域
本发明涉及一种医疗器械,尤其是一种可以穿透管腔壁的置入器,具体地说是一种结合穿刺造口及置入功能为一体的支架置入器。
背景技术
支架植入术是利用穿刺、导管、球囊导管扩张形成和金属内支架置入等技术,使狭窄、闭塞的血管或腔道扩张、再通,解决传统手术盲区的一种技术。目前,支架置入术已被国内医院普遍用于临床,其有效性已经得到普遍的认可,支架植入术有手术风险低,微创性,病人痛苦小,手术时间短,费用低等优点。在恶性良性梗阻、造瘘、吻合术等手术中都有使用。
在人体官腔通道发生梗阻时,医师往往需要以手术方式去除梗阻,或重建生理通道,在条件允许的情况下还可进行坏死或感染物清除等操作,该方法成功率较高而且术后并发症较少。但现在通常使用的置入器只有将支架输送至人体目标位置的作用。在假性胰腺囊肿、消化道胆囊吻合和胃肠吻合术等手术的人体通道再建过程中,在放置支架前,就必须在内镜超声引导下,首先用穿刺针、切开刀或激光穿通病灶器官壁或人体通道重建,并扩张通道,将上述器械退出后,再将相应器械放入穿透的位置进行持续引流。而在上述手术过程中一旦扩张与重建通道的器械推出,就可能会造成器官内容物的迅速外流、器官缩小,使得后续操作变得困难,甚至有可能污染腹腔。如果不能顺利将引流器械植入囊内,不仅达不到治疗效果,还会造成腹腔感染等严重并发症。
发明内容
发明目的:本发明所要解决的技术问题是针对现有技术的不足,提供一种一体式支架置入器。
为了解决上述技术问题,本发明公开了一种一体式支架置入器,包括前手柄和后手柄,所述前手柄前端设有外管,外管顶端抵接一切割头,外管内设有中管以及支架;后手柄包括用于支撑的不锈钢管以及位于不锈钢管内的内管,内管顶端与切割头固定连接;
中管的一端与支架的一端相互抵接,中管的另一端与所述不锈钢管相互嵌套连接,支架的一端与切割头靠近,留有一定间隙;
当前手柄沿不锈钢管后撤时,外管同时后撤且与切割头分离,中管和后手柄保持位置不变,支架自动释放。
本发明中,所述后手柄设有导电插座,导电插座电连接内管,内管为金属导电材质的内管,切割头为金属导电材质的切割头。从而可以对切割部位实时进行电疗。
本发明中,中管与支架抵接的位置以及切割头与支架抵接的位置各设有一个显影环。从而可以在影像仪器下显示出支架的位置,用于更加精确地定位。
本发明中,所述前手柄后端设有锁紧螺母,用于将不锈钢管与前手柄锁定。锁紧螺母可以由一弹性材料制成的密封圈与前后夹紧用的塑料件组成,中间通道用于穿过不锈钢管,一端的塑料件与前手柄后端螺纹配合,在锁紧螺母锁紧过程中,塑料件压缩中间的弹性材料,至其变形,压紧不锈钢管,从而达到锁紧定位的目的。
本发明中,所述导电插座外围设有绝缘垫块。
本发明中,所述切割头的外表面设有螺纹。
本发明中,所述内管外表面设有绝缘涂层或者绝缘层。绝缘涂层为直接涂覆,绝缘层为类似塑料或其他绝缘的高分子材料形成的层状结构。
本发明使用时,首先锁紧螺母,将电源插在导电插头上,切割头带电,扩开孔道后,将电源拔去,打开锁紧螺母,保持后手柄不动,回撤前手柄,释放出支架。
本发明可以实现扩张、再建瘘道和置入内引流支架一步完成。
有益效果:本发明的有益效果:
a.本发明采用螺旋状金属切割头,有利于切开病灶器官,便于器械插入;
b.采用导电插头使切割头通电,便于在切开病灶器官过程中使组织快速凝结,起到止血作用;
c.本发明在病灶器官切开后可直接置入支架,建立了病灶器官与外界的通道,可以继续使用其他器械进行感染物或坏死组织清除等其他手术操作,增加了手术的综合性,使病人可以避免再次承受二次或三次的手术痛苦。

具体实施方式

本发明将穿刺造口及置入支架操作结合为一体的输送装置。置入器的切割头为用金属材料制成的螺旋状结构,该金属材料可以为任意种类的医用金属,后手柄处增加了通电所需的导电插座。置入器通过切割头带电进行穿刺操作,接着进行释放支架,无需更换器械;所述的置入器的切割头其形状可以是圆柱、圆锥、圆台等多种螺旋结构;所述的置入器的导电插座与内管相连,通过垫块保证导电插座与内管接触完好;所述的置入器的内管为由医用金属材料制成的柔韧性较好的弹性管,在放置支架部位其内管外表面具有绝缘涂层。
实施例
本实施例以经内镜胰腺假性囊肿的支架置入器为具体的例子,但本发明一体式支架置入器在结构不变的情况下,可以应用于多种病灶器官,不仅仅限定为胰腺假性囊肿。

如图1~图4所示,本实施例中,中管6套在与不锈钢管12胶接,不锈钢管套和中管6在内管3外,外管7套在不锈钢管外,外管与螺帽8相连,螺帽8与前手柄10的连接处设有连接件9。前手柄10的尾端安装有锁紧螺母11,可以固定前手柄10与中管6以及不锈钢管12的相对位置。前手柄10的前端设有旁路管道18。内管远端与切割头1连接,切割头1外部设有螺纹结构1a。携带支架4时,中管6远端与支架相抵,不锈钢管穿过前手柄与后手柄14,不锈钢管的近端抵住垫块16,而内管的近端穿过垫块,从上端被导电插头15压住。后手柄14末端设有鲁尔接口17。远端显影环2与近端显影环5可在X光照射下显影。螺帽8与连接件9与前手柄相连接,起到保护管材、方便手持的作用.装饰帽13与后手柄相连接,起到方便手持的作用。
本实施例使用时,首先依次安装好各个部件,将锁紧螺母11锁紧,固定前手柄、不锈钢管和内管位置,在导丝引导,X线或内窥镜的辅助监视作用下将置入器输送到病变指定位置。将置入器后手柄处的导电插座进行通电,通过带电的螺旋切割头穿透囊肿壁。断开电源,将置入器插入囊肿。到达理想位置后,松开锁紧螺母11,将前手柄10后撤,后手柄14保持位置不变,此时外管7随前手柄10后撤,内管以及中管6位置不变,此时有弹性的支架4没有了外管7的包覆,自由扩张伸展开释放支架,支架4扩张伸展开后,内管以及切割头可从支架中心的空间回撤,从而实现撤出置入器保证引流通畅,若有需要可进行后续清除坏死组织等其他微创手术。
本发明提供了一种一体式支架置入器,具体实现该技术方案的方法和途径很多,以上所述仅是本发明的优选实施方式,应当指出,对于本技术领域的普通技术人员来说,在不脱离本发明原理的前提下,还可以做出若干改进和润饰,这些改进和润饰也应视为本发明的保护范围。本实施例中未明确的各组成部分均可用现有技术加以实现。

Claims (7)

1.一种一体式支架置入器,包括前手柄和后手柄,其特征在于,所述前手柄前端设有外管,外管顶端抵接切割头,外管内设有中管以及支架;后手柄包括用于支撑的不锈钢管以及位于不锈钢管内的内管,内管顶端与切割头固定连接;
中管的一端与支架的一端相互抵接,中管的另一端与所述不锈钢管相互连接,支架的一端与切割头靠近,留有一定间隙;
当前手柄沿不锈钢管后撤时,外管同时后撤且与切割头分离,中管和后手柄保持位置不变,支架自动释放。
2.根据权利要求1所述的一种一体式支架置入器,其特征在于,所述后手柄设有导电插座,导电插座电连接内管,内管为金属导电材质的内管,切割头为金属导电材质的切割头。
3.根据权利要求1或2所述的一种一体式支架置入器,其特征在于,中管与支架抵接的位置以及切割头与支架端部靠近的位置各设有一个显影环。
4.根据权利要求1或2所述的一种一体式支架置入器,其特征在于,所述前手柄后端设有锁紧螺母,用于将不锈钢管与前手柄锁定。
