Description
この問題に対処するために、内視鏡粘膜切除術に際し、切除前に、軟性針により液を粘膜下に局注する。針は粘膜下組織の的確な箇所に位置決めする必要がある。この粘膜下への局注により、粘膜は固有筋層から浮き上がり、粘膜の下に液のクッションが形成される。これは、固有筋層を安全に隔離し、遮熱層となる。そこで、軟性針のメスによる粘膜切除を行う。このような切除は非常に難しく、現状において、この種の介入に要する手術時間は2〜6時間である。
内視鏡的局注法による粘膜下への液注入に適した装置は、たとえば、特許文献1に記載されている。
また、粘膜切除は、液を使用せず高周波処置デバイスで行い、少なくとも1つの液を送り込む給液デバイスは局注のみに、すなわち粘膜の挙上にのみ使用することも本発明の手術器具で可能となる。
図1aに、本発明の実施形態に係る内視鏡手術器具または内視鏡11のプローブ3の先端部6を示す。図1の縦断面図に示すように、プローブ3には2つの作動チャンネル4、5がある。一方の作動チャンネル4にはノズル7が固設されている。この作動チャンネル4を介して少なくとも1つの液、特にNaCl溶液(生理食塩水)が送り込まれる。したがって、作動チャンネル4は少なくとも1つの液を送り込む給液デバイス1の一部である。他に給液デバイス1は液の供給容器と液を加圧して作動チャンネル4に送り込むポンプ機構を備えている。

Claims (7)
- 少なくとも1つの液を送り込み、具体的には局注および/または切開を行う給液デバイス(1)と、高周波処置デバイス(2)を備え、均一な処置のために、前記高周波処置デバイスと前記給液デバイスを一体化した、内視鏡的粘膜切除用の内視鏡手術器具。
- 前記給液デバイス(1)は、2つの作動チャンネル(4、5)を具備した軟性プローブ(3)を有し、前記軟性プローブ(3)の先端部(69において、一方の作動チャンネル(4)にはノズルが設けられ、他方の作動チャンネル(5)には高周波電極(8)が設けられていることを特徴とする請求項1に記載の内視鏡手術器具。
- 前記高周波処置デバイス(2)は、針(2a)、フック(2b)、絶縁先端部付針(2c)、ディスク、スネア(2d)、または絶縁先端部付スネア(2e)を備えることを特徴とする請求項1または2に記載の内視鏡手術器具。
- 前記高周波処置デバイス(2)をオンにするスイッチ(10)、具体的にはスライドスイッチと、前記高周波処置デバイス(2)を作動する別スイッチ、具体的にはフットスイッチを備えた操作部を設けたことを特徴とする請求項1乃至3のいずれか1項に記載の内視鏡手術器具。
- 前記別スイッチにより、前記高周波処置デバイス(2)と前記給液デバイス(1)の両方が作動されるようにしたことを特徴とする請求項4に記載の内視鏡手術器具。
- 前記プローブ(3)はプラスチックホースで構成されることを特徴とする請求項2乃至5のいずれか1項に記載の内視鏡手術器具。
- 前記プローブ(3)は外周部を絶縁した軟性金属チューブで構成されることを特徴とする請求項2乃至5のいずれか1項に記載の内視鏡手術器具。
