Abstract
Description
昨今、被膜を有するステントが、頭蓋内の動脈瘤の治療を含む様々な目的のために提案されてきた。被覆されたステントが、そのような目的に使用される場合、極めて正確に配置されなければならない。通常、ステントの被覆された部分は、動脈瘤の付け根を越えて配置されなければならないが、動脈の分岐部即ち側枝動脈を覆ってはならない。
本発明の範囲を限定することなく、請求される本発明の実施形態のうちいくつかの要約を以下に記載する。要約された実施形態のさらなる詳細及び/又は本発明のさらなる実施形態については、「発明の詳細な説明」に記載されている。
他の実施例においては、本発明は、外面、内面及び複数の互いに連結されたストラットを有する管状構造からなるステントに関する。PTFEからなる外側被膜は、拡張可能な構造の外面の少なくとも一部に沿って延び、PTFEからなる内側被膜は同構造の内面の少なくとも一部に沿って延びる。内側被膜及び外側被膜の少なくとも一部は、互いに連続している。内側被膜及び外側被膜は、互いに同一の広がりを有することが望ましい。また、ステントは、放射線不透過性を有する、或いは、磁気共鳴映像法を使用して視覚化され得る、少なくとも1つのマーカを備える。マーカは、内側被膜と外側被膜との間に配置される。PTFEは、延伸加工を施されたPTFEであることが望ましい。
一実施例において、本発明は、外面と内面と複数の互いに連結されたストラットとを備える管状構造体からなるステントに関する。そのようなステントの限定されない例は、図la,lbにおいて符号100により概略的に示される。ステント100は、複数の互いに連結されたストラット108からなる構造体104を備える。本発明は、図laに示される構造体に限定されない。他の構造体には、米国特許出願公開第2002−0055770号明細書に開示されている任意の構造体が使用されてもよい。一般的には、構造体は、図1cに示す態様のように、複数の位置において互いに連結される複数の蛇行帯の形態をなしてもよい。より一般的には、構造体は、そこを貫通する任意の形状からなる開口部を有する管材の形態であってもよい。
非遺伝物質からなる治療薬の例としては、抗血栓剤(ヘパリン、ヘパリン誘導体、ウロキナーゼ、PPack(デキストロフェニルアラニンプロリンアルギニンクロロメチルケトン)等);増殖抑制剤(エノキサプリン、アンジオペプチン、平滑筋細胞の増殖を阻害するモノクローナル抗体、ヒルジン、アセチルサリチル酸等);抗炎症剤(デキサメタゾン、プレドニソロン、コルチコステロン、ブデソニド、エストロゲン、サルファサラジン、メサラミン等);抗腫瘍剤/増殖抑制剤/抗有糸分裂剤(パクリタキセル、5−フルオロウラシル、シスプラチン、ビンブラスチン、ビンクリスチン、エポシロン類、エンドスタチン、アンジオスタチン、チミジンキナーゼ阻害剤等);麻酔薬(リドカイン、ブピバカイン、ロピバカイン等);抗血液凝固剤(D−Phe−Pro−Argクロロメチルケトン、RGDペプチド含有化合物、ヘパリン、アンチトロンビン化合物、血小板受容体拮抗剤、アンチトロンビン抗体、抗血小板受容体抗体、アスピリン、プロスタグランジン阻害剤、血小板阻害剤、ダニ由来の抗血小板ペプチド等);血管細胞増殖促進剤(成長因子阻害剤、成長因子受容体拮抗剤、転写活性化因子、翻訳促進因子等);血管細胞増殖阻害剤(成長因子阻害剤、成長因子受容体拮抗剤、転写抑制剤、翻訳抑制剤、複製阻害剤、阻害抗体、成長因子に対する抗体、成長因子及び細胞毒素で構成される二官能性分子、抗体及び細胞毒素で構成される二官能性分子等);コレステロール低下薬;血管拡張剤;体内における血管作用機構を阻害する薬剤等がある。
本発明のステントに使用されるステント構造体は、レーザ切断を含む任意の好適な公知技術、即ちシート状の材料からパターンを機械的に切り取ってシート材を巻いたり、機械的切断、エッチング、管状の材料からステントパターンを化学的切断や他の切断方法やレーザにより切り取ったり、或いは、シート状の材料や管状の材料からステントのパターンを切り取るためにEDM(放電加工)技術を使用して製造されてもよい。
Claims (32)
- 外面と内面と互いに連結された複数のストラットとを有する管状構造体と、PTFEからなる外側被膜と、PTFEからなる内側被膜とからなるステントであって、該外側被膜は該拡張可能な構造体の外面の少なくとも一部に沿って延びることと、該内側被膜は該拡張可能な構造体の内面の少なくとも一部に沿って延びることと、該内側被膜及び外側被膜の少なくとも一部は連続していることと、該内側被膜と外側被膜との間に配置された放射線不透過性マーカをさらに備えることとを特徴とするステント。
- 前記PTFEは延伸加工を施されたPTFEである請求項1に記載のステント。
- 前記放射線不透過性マーカは、放射線不透過性マーカバンドである請求項1に記載のステント。
- 前記マーカバンドはステントの一部の周囲に巻き付けられる請求項3に記載のステント。
- 前記マーカバンドは前記構造体に圧着される請求項3に記載のステント。
- 前記放射線不透過性マーカは、前記構造体の一部に埋め込まれる請求項1に記載のステント。
- 前記放射線不透過性マーカは、ステントの被覆されていない領域に近接して配置される請求項1に記載のステント。
- 前記放射線不透過性マーカは、前記構造体の開口部に挿入された放射線不透過性プラグの形態を取る請求項1に記載のステント。
- 複数の放射線不透過性マーカを備える請求項1に記載のステント。
- 前記PTFEは延伸加工を施されたPTFEである請求項9に記載のステント。
- 前記構造体の外面上のPTFE及び内面上のPTFEは、互いに同一の広がりを有する請求項10に記載のステント。
- 前記放射線不透過性マーカの少なくとも幾つかは、前記内面及び外面上のPTFEの少なくとも一端を示す請求項11に記載のステント。
- 前記放射線不透過性マーカの少なくとも幾つかは、前記内面及び外面上のPTFEの第1端部を示し、他の放射線不透過性マーカは、前記内面及び外面上のPTFEの第2端部を示す請求項11に記載のステント。
- 頭蓋内の血管に使用されるように寸法が設定された請求項13に記載のステント。

- 外面と内面と互いに連結された複数のストラットとを有する管状構造体と、PTFEからなる外側被膜と、PTFEからなる内側被膜とからなるステントであって、該外側被膜は該拡張可能な構造体の外面の少なくとも一部に沿って延びることと、該内側被膜は該拡張可能な構造体の内面の少なくとも一部に沿って延びることと、該内側被膜及び外側被膜の少なくとも一部は連続していることと、該ステントは、放射線不透過性を有する、或いは、磁気共鳴映像法を使用して視覚化される、少なくとも1つのマーカを備え、該マーカは該内側被膜と外側被膜との間に配置されることとを特徴とするステント。
- ステントの製造方法であって、
複数の互いに連結されたストラットからなり、且つ、内面と外面とを有するステント構造体を提供する工程と、
該構造体の所望の領域において該構造体に放射線不透過性を付与する工程と、
該構造体の所望の領域において該内面をPTFEで覆う工程と、
該構造体の所望の領域において該外面をPTFEで覆う工程とからなる方法。 - 複数の所望の領域において前記構造体に放射線不透過性を付与する工程と、
該所望の領域の各々において前記構造体の外面と内面をPTFEで覆う工程とをさらに備える請求項21に記載の方法。 - 前記放射線不透過性は、前記構造体に装着された放射線不透過性マーカにより提供される請求項22に記載の方法。
- 各放射線不透過性マーカは、前記構造体の一部の周囲に巻き付けられた放射線不透過性材料の形態を取る請求項23に記載の方法。
- 各放射線不透過性マーカは、前記構造体の開口部に挿入された放射線不透過性プラグの形態を取る請求項23に記載の方法。
- 前記放射線不透過性は、前記ステントの内面及び外面上のPTFEの一端を示すマーカにより付与される請求項21に記載の方法。
- 前記放射線不透過性は、前記ステントの内面及び外面上のPTFEの少なくとも一端を示すマーカにより付与される請求項22に記載の方法。
- 前記ステントの内面及び外面上のPTFEは、互いに同一の広がりを有する請求項27に記載の方法。
- 前記ステントの内面及び外面上のPTFEは、互いに同一の広がりを有する請求項21に記載の方法。
- 前記内面上のPTFEは、延伸加工を施されたPTFEからなる第1の押出成形された管材の形態をなし、前記外面上のPTFEは延伸加工を施されたPTFEからなる第2の押出成形された管材の形態をなす請求項28に記載の方法。
- 前記ステントは頭蓋内の血管に使用されるように寸法が設定される請求項21に記載の方法。
- 内面と外面とマーカ領域とを有するステント構造体と、
該構造体のマーカ領域内に配置された少なくとも1つの放射線不透過性マーカと、
該マーカ領域内の構造体の内面と外面とを覆う延伸加工を施されたPTFEからなる被膜とを備える被覆されたステント。
