Abstract
【解決手段】胸部組織の生検サンプル等のような、組織サンプルを得るために生検装置およびその方法が提供されている。この生検装置は、先端側の穿孔端部と、カッター内孔部と、当該カッター内孔部に連通している側面の組織ポートと、当該側面の組織ポートの先端側に配置されている少なくとも1つの流体通路と、を有する外側カニューレ、を含むことができる。また、内側のカッターは組織サンプルを切断するために上記側面の組織ポートを通過して上記カッター内孔部の中において前進できる。組織サンプルが切断された後であって、上記内側のカッターが上記側面の組織ポートの基端側に後退する前に、上記カッターは上記側面の組織ポートの先端側に配置されている流体通路を交互に被覆および露出させるように使用できる。

Description
本特許出願は以下の共通に譲渡されている特許出願、すなわち、トンプソン(Thompson)他の名義で2004年2月24日に出願されている「バイオプシー・デバイス・ウィズ・バリアブル・スピード・カッター・アドバンス(Biopsy Device with Variable Speed Cutter Advance)」を発明の名称とする米国特許出願第10/785,755号、ヒブナー(Hibner)他の名義で2003年9月30日に出願されている「バイオプシー・インストルメント・ウィズ・インターナル・スペシミン・コレクション・メカニズム(Biopsy Instrument with Internal Specimen Collection Mechanism)」を発明の名称とする米国特許出願第10/676,944号、およびシセナス(Cicenas)他の名義で2003年12月10日に出願されている「バイオプシー・デバイス・ウィズ・サンプル・チューブ(Biopsy Device with Sample Tube)」を発明の名称とする米国特許出願第10/732,843号を、クロス・リファレンスし参照して含んでいる。
本発明は一般に生検装置に関連しており、特に、組織を切断するためのカッターを有する生検装置に関連している。
組織の診断および治療がこの研究の継続中の領域である。後のサンプリングおよび/または試験のために組織サンプルを得るための種々の医療装置が当業界において知られている。例えば、商品名をマンモトーム(MAMMOTOME)として現在販売されている生検器具が、胸(乳房)の生検サンプルを得ることにおいて使用するために、エシコン・エンド−サージェリー・インコーポレイテッド(Ethicon Endo-Surgery, Inc.)から市場において入手可能である。
一例の実施形態において、本発明は生検方法を提供している。この方法は先端側の穿孔用の先端部分、カッター内孔部、当該カッター内孔部に連通している側面の組織ポート、および当該側面の組織ポートの先端側に配置されている少なくとも1つの流体通路を有する外側カニューレを供給する工程と、組織サンプルを切断するために上記側面の組織ポートを通過して上記カッター内孔部の中に内側カッターを前進させる工程と、上記側面の組織ポートの基端側に上記カッターを後退させることなく当該カッターの基端側および先端側への移動により上記側面の組織ポートの先端側に配置されている流体通路を交互に被覆および曝露する工程と、を含むことができる。上記流体通路を交互に被覆および曝露する工程はその流体通路からあらゆる組織の存在を排除するために補助することができ、このような組織の存在は、排除されない場合に、上記通路の中の流れを妨げる可能性がある。
本発明は体内から所与の組織サンプルを得るための生検装置に関連している。この生検装置は市場において入手可能なマンモトーム(Mammotome)の銘柄の生検装置等のような装置に対して比べて、切断ストロークを短くすることができる。このように切断ストロークの長さを短くすることはそれぞれのサンプルを得るための時間、さらに、その生検装置の全体の大きさも減少させ、これにより、その装置の多用性および人間工学的な特性を高めることができる。また、カッターのストローク長を減少させることにより、同一のプローブ部品の多くが、マンモグラフィ、超音波およびMRIと言う、3種類の主な画像化環境の全てにおいて使用可能になる。加えて、本発明は組織サンプルの連続的な収集および貯蔵を可能にしている。組織サンプルは生検装置から取り外してリアルタイムで調べると共に、その生検処置の終了時に後で取り出すために連続的に貯蔵することも可能である。このように組織サンプルを連続的に貯蔵することは、サンプリングに続いて装置からそれぞれのサンプルを即時に取り外す必要性を排除し、これにより、そのサンプルの取得時間を減少させる。
(1)生検サンプルを供給する方法であって、
先端側の穿孔端部、内孔部、当該内孔部に連通している側面の組織ポート、および当該側面の組織ポートの先端側に配置されている少なくとも1つの流体通路、を有する、カニューレを供給する処理と、
組織サンプルを切断するために、前記側面の組織ポートを通過して、前記内孔部の中に、カッターの先端部を前進させる処理と、
前記側面の組織ポートの基端側に前記カッターの先端部を後退させることなく、そのカッターの基端側および先端側への移動により、前記側面の組織ポートの先端側に配置されている少なくとも1つの流体通路を交互に被覆および露出させる処理と、
を含む、方法。
(2)実施態様1に記載の方法であって、
前記カッターを回転させながら、前記流体通路を交互に被覆および露出させる処理を含む、方法。
(3)実施態様1に記載の方法であって、
組織を切断する前に、前記内孔部から前記流体通路の中を通して流体を引き込む工程をさらに含む、方法。
(4)実施態様3に記載の方法であって、
前記流体を引き込む工程が空気を引き込む処理を含む、方法。
(5)実施態様1に記載の方法であって、
組織を切断した後に、前記内孔部に前記流体通路の中を通して流体を供給する工程を含む、方法。
前記カッターの内孔部に流体を供給する工程が、そのカッターの内孔部に空気を供給する処理を含む、方法。
(7)実施態様5に記載の方法であって、
前記カッターの内孔部に流体を供給する工程が、そのカッターの内孔部に液体を供給する処理を含む、方法。
(8)実施態様1に記載の方法であって、
前記流体通路を交互に被覆および露出させる工程が、前記内側のカッターにより、第1の組織サンプルを切断した後であって、第2の組織サンプルを切断する前に行なわれる、方法。
(9)実施態様1に記載の方法であって、
前記カッターの基端側および先端側への移動を行なうために、基端側方向のバイアス力および先端側方向のバイアス力を交互に加える処理を含む、方法。
(10)実施態様1に記載の方法であって、
前記カッターの移動により、前記側面の組織ポートの先端側に配置されている少なくとも1つの流体通路から組織を除去する処理を含む、方法。
先端側の穿孔端部、内孔部、当該内孔部に連通している側面の組織ポート、および当該側面の組織ポートの先端側に配置されている少なくとも1つの流体通路、を有する、カニューレと、
カッターの先端部が組織サンプルを切断するために前記側面の組織ポートを通過して前進するように、前記内孔部の中において前進することに適合しているカッターと、
前記カッターの先端部が前記側面の組織ポートの先端側に配置されると、そのカッターの往復動を生じるための機構であって、そのカッターが前記側面の組織ポートの先端側に配置されている流体通路を交互に被覆および露出する、機構と、
を備えている、生検装置。
(12)実施態様11に記載の生検装置であって、
前記機構が、前記カッターの先端部が前記側面の組織ポートの先端側に配置されると、そのカッターの往復動を生じるためのバイアス部材を含む、生検装置。
(13)実施態様11に記載の生検装置であって、
前記カッターが回転することに適合しており、当該カッターが回転している間にそのカッターが往復動する、生検装置。
(14)実施態様11に記載の生検装置であって、
前記側面の組織ポートの先端側に配置されている流体通路が、液体のための通路を形成している、生検装置。
Claims (5)
- 生検サンプルを供給する方法であって、
先端側の穿孔端部、内孔部、当該内孔部に連通している側面の組織ポート、および当該側面の組織ポートの先端側に配置されている少なくとも1つの流体通路、を有するカニューレを供給する処理と、
組織サンプルを切断するために、前記側面の組織ポートを通過して、前記内孔部の中にカッターの先端部を前進させる処理と、
前記側面の組織ポートの基端側に前記カッターの先端部を後退させることなく、そのカッターの基端側および先端側への移動により、前記側面の組織ポートの先端側に配置されている少なくとも1つの流体通路を交互に被覆および露出させる処理と、
を含む、方法。 - 生検装置であって、
先端側の穿孔端部、内孔部、当該内孔部に連通している側面の組織ポート、および当該側面の組織ポートの先端側に配置されている少なくとも1つの流体通路、を有するカニューレと、
カッターの先端部が組織サンプルを切断するために前記側面の組織ポートを通過して前進するように、前記内孔部の中において前進することに適合しているカッターと、
前記カッターの先端部が前記側面の組織ポートの先端側に配置されると、そのカッターの往復動を生じるための機構であって、前記カッターが前記側面の組織ポートの先端側に配置されている流体通路を交互に被覆および露出する、機構と、
を備えている、生検装置。 - 請求項2に記載の生検装置であって、
前記機構が、前記カッターの先端部が前記側面の組織ポートの先端側に配置されると、そのカッターの往復動を生じるためのバイアス部材を含む、生検装置。 - 請求項2に記載の生検装置であって、
前記カッターが回転することに適合しており、当該カッターが回転している間にそのカッターが往復動する、生検装置。 - 請求項2に記載の生検装置であって、
前記側面の組織ポートの先端側に配置されている流体通路が、液体のための通路を形成している、生検装置。
