Abstract
【課題】 公知技術の欠点を克服した、血液の光学的酸素飽和度測定用の新規センサを提供すること。
【解決手段】 新規の光学的血液酸素飽和度測定センサについて、新規センサを用いた測定方法と装置とともに記載する。新規センサは、装置の中央に相互に近傍する位置にある2つのポイント状発光器と、発光器の周囲に同心的に設けられた少なくとも1つ、好ましくは2つの環状検出ターミナルとを含む。光源は、例えば、670〜940nmの範囲内の単色光をそれぞれ発する2つのレーザーダイオードにすることができる。検出装置は、例えば光ダイオードである。

Description
(a)2つの波長それぞれに於ける酸化ヘモグロビンの光吸収は、還元ヘモグロビンの光吸収と異なる。
(b)血液が分布する組織での各波長の光吸収には、光源とセンサの間にある組織中を流れる動脈血の体積が変動するために、拍動的な要素が見られる。
(a)第1の光源92をONにして、第2の光源93をOFFにして、第1の波長での第1信号を第1と第2の検出器94と95のそれぞれによって記録する。
(b)第2の光源93をONにして、第1の光源92をOFFにして、第2の波長での第2信号を第1と第2の検出器94と95のそれぞれによって記録する。
(c)光源を両方ともOFFにして、第1と第2の検出器94と95によって周囲光を記録する。
(d)アナログ処理ユニット96によって、検出信号を連続的にフィルタにかけて増幅し、ノイズや周囲光成分を取り除く。
(e)アナログ−デジタル(A/D)変換器97で一連の信号をアナログ処理ユニット96から受信し、デジタル化して、その結果生じるデジタル信号をマイクロプロセッサ98に送信する。
(f)マイクロプロセッサ98がDCとAC信号成分のデジタル抽出を行い、以下のアルゴリズムにしたがってSaO2を計算する。
(i)比R1とR2を各波長で計算する。
SaO2=K1xγ+K2
式中K1とK2は校正定数である。
a)ゼロ点における第1の測定
1)第1の光源92をONにして、第2の光源93をOFFにして、第1の検出器94によって第1の信号I11(1)を検出し、増幅し、マイクロプロセッサ98の記憶部へ送信する。I11(1)=1000
2)第1の光源92をONにして、第2の光源93をOFFにして、第2の検出器95によって第1の信号I12(1)を検出し、増幅し、マイクロプロセッサ98の記憶部へ送信する。I12(1)=2500
3)ゼロ点における第1の波長での強度比N1を次式にしたがって計算する。:DC1(1)=I11(1)/I12(1)=1000/2500=0.4
4)第2の光源93をONにして、第1の光源92をOFFにして、第1の検出器94によって第2の信号I21(1)を検出し、増幅し、マイクロプロセッサ98の記憶部へ送信する。I21(1)=800
5)第2の光源93をONにして、第1の光源92をOFFにして、第2の検出器によって第2の信号I22(1)を検出し、増幅し、マイクロプロセッサ98の記憶部へ送信する。I22(1)=2300
6)ゼロ点における第2の波長での強度比N2を次式にしたがって計算する。:DC2(1)=I21(1)/I22(1)=800/2300=0.348
b)頂点における第2の測定
7)第1の光源92をONにして、第2の光源93をOFFにして、第1の検出器94によって第1の信号I11(2)を検出し、増幅し、マイクロプロセッサ98の記憶部へ送信する。I11(2)=990
8)第1の光源92をONにして、第2の光源93をOFFにして、第2の検出器95によって第1の信号I12(2)を検出し、増幅し、マイクロプロセッサ98の記憶部へ送信する。I12(2)=2460
9)頂点における第1の波長での強度比M1を次式にしたがって計算する。:DC1(2)=I11(2)/I12(2)=980/2490=0.394
10)第2の光源93をONにして、第1の光源92をOFFにして、第1の検出器94によって第2の信号I21(2)を検出し、増幅し、マイクロプロセッサ98の記憧部へ送信する。I21(2)=780
11)第2の光源93をONにして、第1の光源92をOFFにして、第2の検出器95によって第2の信号I22(2)を検出し、増幅し、マイクロプロセッサ98の記憶部へ送信する。I22(2)=2400
12)頂点における第2の波長での強度比M2を次式にしたがって計算する。:DC2(2)=I21(2)/I22(2)=780/2400=0.325
c)計算
13)第1の波長での拍動的要素の特徴となるAC1値は、M1−N1=0.349−0.4=−0.006と計算される。
18)R2=AC2/DC2=(−0.023/0.337)=−0.068
最後に
19)SaO2=K1(R1/R2)+K2=2×0.221+0.5=0.942
を計算する。
Claims (6)
- 血液分布組織における、血液の無侵襲的な光学的酸素飽和度測定方法であって、
少なくとも2つの光発光ターミナルから少なくとも2つの異なる波長の光を連続して発する工程と、
鏡面反射と逸脱光を除く一方で、検査中の前記組織から到達し且つ前記2つの異なる波長のうち1つの波長で発せられた光をコリメートする工程と、
前記コリメートされた光を少なくとも2つの環状光検出ターミナルによって完全に捕捉することで、該光信号の強度を検出する工程と、
前記少なくとも2つの環状光検出ターミナルによって、少なくとも2つの異なる波長のそれぞれで検出される光強度間の比を決定する工程と、
前記比に基づいて前記組織に分布する血液の酸素飽和値を決定する工程と、
を含むことを特徴とする方法。 - 前記検出する工程は、前記少なくとも2つの光検出ターミナルによって前記光信号の強度を同時に検出することを含むことを特徴とする請求項1に記載の方法。
- 前記発光ターミナルはそれぞれ、非接触の発光端部を有し且つ別の端部を通じて光源に連結される発光ターミナルであることを特徴とする請求項1に記載の方法。
- 前記光源は、670〜940nmの範囲内の単色光をそれぞれ発するレーザーダイオードであることを特徴とする請求項3に記載の方法。
- 第1の光源は、750〜760nmの光を発し、第2の光源は780〜800nmの光を発することを特徴とする請求項4に記載の方法。
- 前記少なくとも2つの光検出ターミナルは、第1と第2の光検出器にそれぞれ接続され、さらに前記方法は、
測定を実施するために2点の時刻、つまり拍動性動脈血成分のゼロ点である第1点と、上記の頂点である第2点とを選択する工程と、
第1の波長光で照射すると同時に、組織から到達する信号を前記第1及び第2の光検出器によって記録し、続いて、第2の波長光で照射すると同時に、組織から到達する信号を前記第1及び第2の光検出器によって記録することを含む前記第1点及び第2点での測定をそれぞれ実施する工程と、
前記2点の時刻それぞれにおける2つの強度比を決定する工程であって、第1の強度比は、前記第1の波長での前記第1と第2の光検出器によって記録された光信号強度間の比であって、第2の強度比は、前記第2の波長での前記第1と第2の光検出器によって記録された光信号強度間の比である工程と、
前記第1と第2の波長それぞれでの光信号の第1と第2の拍動的要素AC1とAC2、つまり各波長における頂点とゼロ点で計算された強度比間の差を計算する工程と、
前記第1と第2の波長それぞれでの光信号の第1と第2の一定成分DC1とDC2、つまり2つの波長におけるゼロ点と頂点で計算された2つの強度比の平均をそれぞれ計算する工程と、
動脈血の酸素飽和度SaO2を以下の方程式にしたがって計算する工程と:
SaO2=K1[(AC1×DC2)/(DC1×AC2)]+K2
式中K1とK2は校正定数:
