本発明は、幹細胞およびその分化に関する。特に、膵臓のランゲルハンス島のβ細胞およびネスチンポジティブな肝臓幹細胞、および幹細胞または前駆細胞からのそれらの分化、ならびに移植における膵臓の幹細胞、前駆細胞および分化したβ細胞、またはネスチンポジティブな肝臓の幹細胞または前駆細胞の使用に関する。

以前の研究者らは、島の内分泌細胞の新生に対する幹細胞の潜在的な供給源として、膵島の導管上皮細胞または外分泌組織に焦点を合わせていた。ネスチンは、R.401と命名されたモノクローナル抗体を用いて、E15ラット胚からcDNAライブラリをスクリーニングすることによってクローニングされた中間径フィラメントタンパク質である(HockfieldおよびMcKay、1985;Lendahlら、1990)。ネスチンは、神経上皮幹細胞において第一に見出され、そして発生中の中枢神経系において発現される。最大レベルがラット胚においてE16で到達された後、ネスチン発現は、成体の大脳皮質においてほとんど検出不可能なレベルまで低下し、これは初期のネスチン発現性前駆細胞の最終分化と一致する(Lendahlら、1990)。ネスチンは、胚の発生中の脳および骨格筋の幹細胞において独占的に当初見出された(Lendahlら、1990)。後の研究は、成体の哺乳動物前脳の上衣下層においてネスチンポジティブな神経幹細胞を同定した(Morsheadら、1994)。ネスチンポジティブな幹細胞は、成体のマウスまたはラットの脳から単離された場合でさえ多能性を示していた。例えば、ネスチンポジティブな幹細胞は、培養物中で神経細胞のあらゆる3つの主要なクラス(ニューロン、アストロサイトおよびオリゴデンドロサイト)を作製し得る(ReynoldsおよびWeiss、1996)。ネスチンポジティブな神経幹細胞は、アストロサイトに分化する遊走細胞の増殖および変性によって脊髄損傷に反応し、瘢痕形成に関与し(Johanssonら、1999)、そして照射を受けたマウスへの注入後に骨髄の造血細胞を回復させる(Bjornsonら、1999)。
本発明に従った幹細胞は、例えば、FACS、免疫細胞化学染色、RT−PCR、サザンノーザンブロット分析およびウェスタンブロット分析、ならびに当業者に公知のような細胞同定の他のこのような技術によって、ネスチンおよび/またはGLP−1R、ABCG2、Oct3/4、ラトロフィリン(2型)、Hes−1、インテグリンサブユニットα6およびβ1、C−kit、MDR−1、SST−R,2,3,4、SUR−1ならびにKir6.2のこれらの発現によって同定され得る。

他のこのような配列が可能であり、そしてこのような配列は、当該分野の範囲内にあることが考慮される。本発明は、以下からなる群より選択されるマーカーのうちのいずれかとして、PCRについてのプライマーまたはアンプライマーとして使用されるオリゴヌクレオチド、およびサザン分析についてのプローブとして使用されるオリゴヌクレオチドを有する:ABCG2、Oct3/4、GLP−1レセプター、ラトロフィリン(2型)、Hes−1、ネスチン、インテグリンサブユニットα6およびβ1、C−kit、MDR−1、SST−R,2,3,4、SUR−1、Kir6.2、CD34、CD45、CD133、MHCクラスIおよびMHCクラスII。一般的な指針として、プライマーは2つの異なるエキソンから選択され、そして少なくとも1つのイントロン配列を包含する。さらに、RTマイナスコントロールが、大半のサンプルについて行われる。PCR増幅は、94℃で1分間、その後の94℃で10秒間、58/56℃で10秒間、72℃で1分間を35サイクル、および72℃で2分間で達成される。このアニーリング温度は、ラットネスチンについては58℃であり、そして残存プライマー対については56℃である。
本発明者らは、成体哺乳動物(ヒトを含む)の膵臓がネスチンを発現する細胞を含むことを現在予期せず発見した。重要なことには、膵臓におけるネスチンポジティブな細胞の分布は、ホルモン産生細胞の分布と一致しない。例えば、インスリンまたはグルカゴンと特異的に反応性の蛍光標識された抗体は、それぞれ島のβ細胞およびα細胞を標識するが、一方本発明者らは、マウス、ラットおよびヒトにおいて、蛍光標識されたネスチン抗体は、小管上皮の特定の細胞のみに局在し、そして膵島におけるα細胞、β細胞、δ細胞および膵臓ペプチド産生細胞に局在しないことを観察した(図1)。本発明者らはまた、IV型コラーゲンに特異的な抗体(脈管内皮細胞に対するマーカー)、ガラニンに特異的な抗体(神経終末のマーカー)およびサイトケラチン19に特異的な抗体(導管細胞に対するマーカー)が、ネスチン抗体と共局在しないことを観察した。さらに本発明者らは、ネスチンポジティブな島細胞が、インスリン、グルカゴン、ソマトスタチンまたは膵臓ポリペプチドに対する抗体と共標識しないことを見出した(図1)。これは、これらのネスチン含有細胞が、内分泌細胞、導管細胞、神経細胞または脈管内皮細胞ではないが、以前に記載されていない島内部のまったく独特の細胞型を示し得ることを示唆する。本発明者らは、島において、ならびに膵管の局在化領域において、そして外分泌膵臓の房心領域内にネスチンポジティブな細胞を見出した。
ヒト島由来NIPは、ABCG2、MDR1およびネスチンを共発現するSP細胞の実質的分集団を含む(ABCG2発現は、染料ヘキスト33342の排除に基づいて、骨髄における多能性幹細胞のSP表現型を規定する)。MDR−1は、SP細胞に複製能を与える。SP細胞(CD34低/ネガティブ)は増殖せず、従ってインビトロでは拡張されることができない。CD34低ネガティブなSP細胞におけるMDR−1の強制発現は、このSP細胞の増殖を引き起こす(Buntingら、2000、Blood 96:902)。NIPはまた、CD34低/ネガティブである。従ってNIPは、そのABCG2発現に基づいてSP細胞としてここで規定され、そして髄由来SP細胞とは異なって、NIPはMDR−1を発現し、従って増殖し得、移植の目的についてエキソビボでNIP SP細胞の拡張を可能にする。
NIPはOct3/4を発現する。Oct3/4は、Pouホメオドメインタンパク質のファミリーに属する転写因子である(Niwaら、2002、Mol.Cell.Biol.22:1526;Niwaら、2000、Nat.Genet.24:328;Niwa、2001、Cell Struct Funct 26:137;Shimazakiら、1993、EMBO 12:4489;Wangら、1996、Biochem Cell Biol 74:579;Nicholsら、1998、Cell 95:379)。これは重要な発見である。なぜならOct3/4の発現は、厳密に幹/前駆細胞に限定されるからである(Niwaら、2002、上記;Niwaら、2000、上記;Niwa、2001、上記)。NIPは、強力にOct3/4を発現し、それによってそれ自身を幹/前駆細胞であると規定する。これは、Oct3/4発現が完全に幹/前駆細胞に限定され、そしてOct3/4がNIPにおいて発現される場合、NIPは幹/前駆細胞である。
NIPは、インテグリンサブユニットαおよびB1を発現する。α6/B1インテグリンはラミニンレセプターである(GiancottiおよびRuoslahti、1999、Science 285:1028)。ES細胞は、ラミニンでコーティングされた培養皿において増殖され得、そして支持細胞層上での成長を必要としないことが最近示されている(Xuら、Nature Biotechnology、2001年10月、19:971)。この細胞表面発現性インテグリンは、細胞成長および発達の維持に不可欠である(Giancottiら、上記を参照のこと)。このα6/B1レセプターは、ラミニンの認識に特異的であり、ラミニンは、胚盤胞において発現される本質的なタンパク質である。従ってα6/B1は、ES細胞(幹/前駆細胞)のマーカーである。NIPは、インテグリンサブユニットα6およびB1を発現する。従ってNIPは、この点ではES細胞に類似する。
側方抑制のδ/ノッチシグナリング経路は、胚発生に重大である。ノッチシグナリングの重要性について多く記載されている。ノッチシグナリングはまた、NIPにおいて重要である。ノッチシグナリングは、膵臓の発生に絶対的に必要とされる(Apelquistら、1999、Nature 400:6747;Lammertら、2000、Mech Dev.94:;Jensenら、2000、Nat.Genet.、24:36;Jansenら、Diabetes 2000、49:163)。Ngn−3は、内分泌膵臓系統の発生に必要とされる重要なbHLH転写因子決定基である。Hes−1は、Ngn−3発現の強力なサプレッサーである。NIPは、強力にHes−1を発現する。従ってNIPは、内分泌細胞への分化を妨げる前駆細胞である。従ってNIPにおいてHes−1発現をブロックする方法は、内分泌細胞(β細胞)へのその分化を導く。Hes−1発現をブロックするためのこのような方法は、siRNAオリゴヌクレオチドまたはアンチセンスHes−1 RNAを発現する発現プラスミドのいずれかを使用することによる、トランスフェクトされたアンチセンスRNAの使用であり得る。
本発明者らは、成体哺乳動物(ヒトを含む)の膵臓が、グルカゴン様ペプチド−1レセプター(GLP−1R)を発現する細胞を含むことをさらに発見した。本発明は、GLP−1Rポジティブな細胞が、ネスチンポジティブな細胞と共局在し得るという発見にさらに基づく。本発明はまた、ネスチンポジティブな膵臓幹細胞が、GLP−1Rポジティブな細胞と共局在し得るという逆の発見に基づく。グルカゴン遺伝子は、多機能性プログルカゴンをコードし、これは、膵臓および腸管におけるプロホルモン転換酵素による特異的なプロセスであって、グルカゴンおよびグルカゴン様ペプチド(GLP)を生じる。GLP−1は、膵β細胞に見出されるGタンパク質共役レセプターに特異的に結合して、cAMP依存性経路を介してインスリン分泌を刺激することが示されている(KieferおよびHabener、1999、Endocr.Rev.20:876;Drucker、1998 Diabetes、47:159)。本発明は、上記のように、膵島および膵管に見出されるネスチンポジティブな幹細胞がまたGLP−1Rポジティブであるという発見に一部基づく。
サイトケラチン−19(CK−19)は、別の中間径フィラメントタンパク質である。CK−19および関連するサイトケラチンは、膵管細胞において発現されることが以前に見出されていた(Bouwensら、1994)。しかし本発明者らは、CK−19発現は確かに小管に限定される一方で、CK−19に特異的な蛍光性抗体は、ネスチン特異的抗体で標識された細胞とは異なる導管細胞を標識することを発見した。これは、島におけるネスチンポジティブな細胞は、CK−19ポジティブな細胞とは異なる細胞型の導管細胞であり得ることを示唆する。
NIPのSP画分(図19Bの左下ゲート)は、低レベルからゼロのレベルのCD34を発現する。この発見は重要である。なぜならCD34ポジティブなSP細胞と対照してみると、CD34低ネガティブな髄由来のSP細胞はきわめて多能性であるからである(Goodell、1999、Blood、94:12545を参照のこと)。髄由来のCD34低/ネガティブなSP細胞は、たとえあったとしても増殖能が限定される(Goodellら、1996、J.Exp.Med.、183:1797;Goodellら、1997 Nature Medicine、3:1337)。CD34低/ネガティブなNIPはインビトロで増殖しない。なぜならこのNIPはMdr−1を発現するからである。
本発明はまた、以下からなる群より選択されるマーカーのうちの少なくとも1つに対してポジティブな膵臓幹細胞を提供する:ラトロフィリン(2型)(Sudhof、2001、Annu Rev Neurosci 24:933)、C−kit(CD117)(Gibsonら、2002、Adv Anat Pathol 9:65)、SSTR−2,3および4(ソマトスタチンレセプター)(Schulzら、2000、J Physiol Paris 94:259)、SUR−1(スルホニル尿素(sulonylurea)レセプター)(Winartoら、2001、Arch Histol Cytol 64:59;Landgraf、2000、Drugs Aging、17:411)、ならびにKir6.2(SUR−1を含む内向き整流K+チャネルサブユニット)(Winartoら、上記;Landgraf、上記)。
幹細胞は、膵臓組織の調製物(例えば糖尿病患者由来の組織の生検サンプルから得られた島)から単離され得る。次いでこの幹細胞はエキソビボで拡張され得、そしてこの得られる細胞は、同系移植片としてドナーに戻して移植され得る。ドナーの内部において、この細胞は分化して、インスリン分泌性細胞(例えばβ細胞)を提供し、糖尿病を引き起こした自己免疫攻撃に奪われたβ細胞の代わりとなり得る。このアプローチは、組織(例えばドナーとして役立ち得る別の個体由来の島)の移植から生じる免疫拒絶の問題を克服し得る。本発明の一実施態様において、同系移植された幹細胞の使用により、別の技術が免疫拒絶を防ごうと努力して行われ得る(すなわち、移植された細胞を免疫攻撃(例えば1型糖尿病を有する個体に存在する自己免疫)に対して抵抗性にする移植細胞の遺伝子治療)。島全体にわたる幹細胞の使用のさらなる利点は、移植された幹細胞がインサイチュで分化し得、そして宿主環境により良く適応し得る(例えば適切な微小循環および宿主の生理的要求に応答する異なる島細胞型の補体を提供する)ことである。本発明の別の実施態様は、例えば前駆細胞を形成するためにエキソビボで部分的に分化した幹細胞の使用を意図し、この前駆細胞は引き続いて宿主に移植され、さらなる分化が宿主内で必要に応じて生じる。幹細胞の同系移植片の使用、前駆細胞の同系移植片の使用または偽島の同系移植片の使用が好ましい。別の実施態様は、別の個体または別の種の哺乳動物から得られた幹細胞の同系移植片の使用、前駆細胞の同系移植片の使用または偽島の同系移植片の使用を意図する。
本発明の一実施態様は、幹細胞または前駆細胞に、不十分な島細胞集団を有する患者に移植され得る偽島様集団を形成させて、ホルモン療法を伴わずに生理学的制御を維持することによる幹細胞または前駆細胞の移植の代案を提供する。島由来の幹細胞は、上記で示されるように培養された島から調製され得るか、または増殖中の幹細胞系統から得られ得る。次いでこの幹細胞は、これらを種々の増殖因子に暴露することによって分化するように誘導され得る。このプロセスは、実施例2および3に例示される。
膵臓幹細胞の分化を誘導し得る増殖因子としては、EGF−2、塩基性FGF、高グルコース、KGF、HGF/SF、GLP−1、exendin−4、ベータセルリン、アクチビンA、TGF−βおよびそれらの組み合わせが挙げられるがこれらに限定されない。GLP−1は、グルカゴン様ペプチド−1をいう。高グルコースは、幹細胞の培養に通常使用される濃度よりも高いグルコース濃度をいう。例えば幹細胞は、およそ5.6mM グルコースにおいて一般に培養および増殖され得、そして高グルコース濃度は、5.6mMを超える別の濃度をいう。好ましい実施態様において、16.7mMの濃度が意図される。実施例2において、塩基性線維芽細胞増殖因子(bFGF)および上皮増殖因子(EGF)を使用する1つの可能な増殖因子処理が記載される。
本発明は、移植された物質の生存を評価するためのインビボ手順を提供する。実験的な移植拒絶は、免疫抑制された哺乳動物または免疫抑制されない哺乳動物に本発明に従った幹細胞または偽島様集団を移植することによって分析される。
本発明は、哺乳動物への移植の方法を提供する。幹細胞、前駆細胞または分化した細胞は、培養容器から患者へ移植される場合に哺乳動物へ「移植」または「導入」される。
幹細胞は、1型糖尿病患者由来の失われたβ細胞の置換、または2型糖尿病患者におけるβ細胞の総数の増大に有用である。糖尿病患者は、好ましくは幹細胞、前駆細胞または偽島様集団を産生するために使用される膵臓組織のドナーとして役立つ。幹細胞は、成体の膵島ならびに膵管内に存在する。糖尿病患者が膵臓生検を受けた後、島は生検組織から単離され、そしてエキソビボで好ましくは24時間の範囲で培養の準備をされる。幹細胞は、上記の方法によって2〜3週間の範囲で増殖および単離され得る。幹細胞は、単離後直接患者に戻して移植され得るか、または増殖因子によって誘導される分化の期間後に移植され得る。島は、実施例2に記載されるように継代培養によって産生され得る。外科的な膵臓生検および移植のプロセス全体は、およそ30日の期間以内に行われ得る。
膵臓幹細胞または膵臓前駆細胞が分化転換して肝細胞を形成する能力は周知である(BisgaardおよびThorgeirsson、1991)。本発明の膵臓幹細胞が使用されて、肝臓組織の機能的集団が減少している肝臓疾患(例えば肝硬変(cirrosis)、肝炎または肝臓癌)を患う患者に肝細胞を提供し得る。本発明の幹細胞はまた、遺伝子治療によって処置されて遺伝子欠陥を修正し得、そして患者に導入されて肝臓機能を回復させ得る。ネスチンポジティブな幹細胞は、1以上の増殖因子を加えることによって、または他の処理(例えば核酸分子を用いたトランスフェクション)によって培養物中またはインビボのいずれかで分化され得、これは幹細胞の肝細胞への分化をもたらす。一実施態様において、本発明は、例えばソニックヘッジホッグを抑制するためのサイクロパミンの使用を意図し、これは肝細胞形成をもたらす。本発明の別の実施態様において、この幹細胞は、任意のエキソビボ処理を受けずに移植され得、そして適切な増殖因子が、インサイチュで患者の体内に提供され得る。なお別の実施態様において、この幹細胞は、増殖因子または他の因子でエキソビボ処理され得、そして引き続いて部分的に分化した状態または最終分化した状態で患者に移植され得る。幹細胞または前駆細胞をトランスフェクトする方法、投与の投薬量および経路、薬学的組成物、ドナー−同系移植片プロトコル、ならびに免疫抑制方法を含む本発明の他の局面は、膵臓組織への分化の場合とまったく同様に肝細胞への分化転換によって行われ得る。
種々の方法が、膵臓幹細胞への遺伝子移植に利用可能である。リン酸カルシウム沈殿化DNAが使用されているが、特に非接着細胞について低い形質転換効率を提供する。加えて、リン酸カルシウム沈殿化DNA方法は、しばしば多数の縦列反復配列の挿入をもたらし、内因性DNAまたは外因性DNAのいずれかの遺伝子機能の混乱の可能性を増大する(Boggs、1990)。陽イオン脂質の使用(例えばリポソームの形態)がまた、真核生物細胞のトランスフェクションについてDNAをパッケージングする有効な方法であり、そして陽イオン脂質のいくつかの市販された調製物が入手可能である。エレクトロポレーションは、リン酸カルシウムプロトコルを超える改善された形質転換効率を提供する。これは、ゲノムの単一の部位で単一のコピーインサートを提供する利点を有する。細胞の核へのDNAの直接マイクロインジェクションは、遺伝子移植のなお別の方法である。これは、短期トランスフェクションについてほぼ100%の効率を提供し、そして安定なDNA組込みについては20%の効率を提供することが示されている。マイクロインジェクションは、細胞質への外因性DNAの時折問題の多い細胞輸送を回避する。このプロトコルは、少量の物質を必要とする。これは、細胞当たり公知の量のDNAの導入を可能にする。実質的に純粋な集団の幹細胞を得る能力は、膵臓幹細胞の標的化遺伝子修飾に対するマイクロインジェクションアプローチの実現可能性を改善する。しかしマイクロインジェクションは、長く極めて専門的なプロトコルである。このプロトコルの真の特質は、任意の所定の時に注入され得る細胞数を限定し、大規模産生におけるその使用を限定する。レトロウイルス方法を用いた膵臓幹細胞への遺伝子挿入が、好ましい方法である。レトロウイルスは、極めて高いトランスフェクション効率でランダムな単一コピーの単一部位のインサートを提供する。他のこのようなトランスフェクション方法が当業者に公知であり、そして本発明の範囲内にあるとみなされる。
膵臓細胞についての遺伝子移植プロトコルは、「ヘルパーウイルス」(すなわち、目的の外来遺伝子を保有するが、完全なウイルス粒子を形成することができない外殻被包(encapsidation)欠損ウイルスゲノム)としてレトロウイルスベクターを含み得る。他のキャリア(例えばDNA媒介性移植、アデノウイルスベクター、SV40ベクター、アデノ随伴ウイルスベクターおよび単純ヘルペスウイルスベクター)がまた利用され得る。いくつかの因子が、感染について適切なベクターを選択する際に考慮されるべきである。スプライシングされたサブゲノムRNAに頼るよりはむしろ外来遺伝子を発現するためにウイルスの末端反復配列または強力な内部プロモーターを使用することが時折好ましい。
哺乳動物の膵臓幹細胞のエキソビボ形質導入、ならびに有効な移植およびその子孫細胞を含む種々の組織における遺伝子発現を得るに十分な非切除レシピエントへの引き続く移植が、マウスにおいて示されている。この標的細胞は、レシピエントへ注入される前に、目的の遺伝子を含む適切なベクターの存在環境下で2〜4日間培養される。
本発明に従って有用な哺乳動物は、任意の哺乳動物(例えば、ヒト、マウス、ラット、ヒツジ、ウサギ、ヤギ、サル、ウマ、ハムスター、ブタまたはウシ)を有する。本発明に従った非ヒト哺乳動物は、ヒトではない任意の哺乳動物であり、マウス、ラット、ヒツジ、ウサギ、ヤギ、サル、ウマ、ハムスター、ブタまたはウシが挙げられるがこれらに限定されない。
例えば本明細書中で記載されるような膵臓幹細胞を必要とする患者は、以下のように処置され得る。本発明の細胞が、好ましくは生物学的に適合性の溶液または薬学的に受容可能な送達ビヒクル中で、経口摂取、注入、吸入または任意の数の他の方法によって患者に投与され得る。好ましい方法は、内視鏡的逆行性注入である。別の好ましい方法は、膵動脈への注入である。別の好ましい方法は、細胞または偽島様集団の腎被膜下空間への注入または配置である。投与される投薬量は、患者毎に変化する。「治療有効用量」は、例えば限定されないが、機能(例えばインスリン産生または血漿グルコースレベル)の増大のレベルによって決定され得る。幹細胞導入のレベルのモニタリング、このような移植によって影響を及ぼされる特定の遺伝子発現のレベルのモニタリング、および/またはコードされる産物の存在もしくはレベルのモニタリングによってまた、当業者は投与される投薬量を選択および調整し得る。一般的には、幹細胞を有する組成物は、体重1kg当たり105〜108細胞の範囲、好ましくは体重1kg当たり106〜107細胞の範囲で単回用量で投与される。この投薬量は、毎日、毎週、毎月、毎年または治療する医師によって適切であると考慮されるように繰り返され得る。本発明は、細胞集団がまた患者から取り出され得るか、または別の状況では提供され得、エキソビボで拡張され得、所望であれば治療遺伝子を含むプラスミドを用いて形質導入され得、次いで患者に再導入され得る。
本発明は、生理学的に適合性のキャリアと混合された本発明に従った幹細胞を含む組成物を提供する。本明細書中で使用される場合、「生理学的に適合性のキャリア」は、生理学的に受容可能な希釈剤(例えば水、リン酸緩衝化生理食塩水または生理食塩水)をいい、そしてさらにアジュバントを有し得る。アジュバント(例えば不完全フロイントアジュバント、リン酸アルミニウム、水酸化アルミニウムまたはミョウバン)は、当該分野で周知の物質である。
(ラット膵臓からのネスチンポジティブな幹細胞の単離)
ラット島を、LacyおよびKostianovskyによって記載されるコラゲナーゼ消化方法を用いて、2〜3月齢のSprague−Dawleyラットの膵臓から単離した。ヒト島を、コラゲナーゼ処理を用いてDiabetes Research Institute、Miami、FLによって提供した。この島を、コンカナバリンAでコーティングされている12ウェルプレート(Falcon3043プレート、Becton Dickinson、Lincoln Park、NJ)において37℃で96時間培養した。この培養培地は、10% ウシ胎仔血清、1mM ピルビン酸ナトリウム、10mM HEPES緩衝液、100μg/ml ストレプトマイシン、100単位/ml ペニシリン、0.25μg/ml アンホテリシンB(GIBCO BRL、Life Science Technology、Gaithersburg、MD)および71.5mM βメルカプトエタノール(Sigma、St.Louis、MO)を補充したRPMI 1640であった。
(島を形成するための膵臓幹細胞の分化)
ラット由来の膵島を、コンカナバリンAでコーティングされた12ウェルプレートを用いて、10% ウシ胎仔血清を含むRPMI培地中で第一に培養した。この島を、ウシ胎仔血清によって供給される増殖因子以外に添加された増殖因子の非存在環境下において3日間培地中で維持した。この期間後、島が接着しなかった間、この島をコンカナバリンAを含まない新しいプレートに移した。次いで幹細胞を、bFGF−2(20ng/ml)およびEGF(20ng/ml)に24日間暴露することによって、単層として島から出て増殖するように刺激した。24日間後、この単層はコンフルエントであった。それらの間で、細胞集団は島を囲んだ。この集団由来の細胞を取り出し、そして新しい12ウェルプレートにサブクローニングし、そして再びbFGFおよびEGFを含む培地中で培養した。このサブクローニングされた細胞は、クローン様式で単層に急速に増殖し、中心から周辺へと拡張した。この細胞は6日目でコンフルエントになり、次いで12日目に重なり合う細胞の波を形成し始めた。17日目までに、細胞はほとんど完全に小球体構造、ならびに島様構造に類似する管状構造(偽島様集団)および導管様構造(偽導管)に移動した(図4)。RT−PCR分析は、偽島様集団が、NCAM(内分泌細胞に対するマーカー、図5を参照のこと)、サイトケラチン19(導管細胞に対するマーカー、図5を参照のこと)および転写因子brain−4(β細胞のマーカー)を発現することを明らかにした。増殖因子での処理は、成熟島細胞への最終分化を達成するために必要とされる。
(ヒト膵島またはラット膵島の単離および培養)
ヒト膵島を単離および培養した。ヒト島組織を、Cell Transplant Center、Diabetes Research Institute、University of Miami School of MedicineおよびJuvenile Diabetes Foundation Center for Islet Transplantation、Harvard Medical School、Boston、MAの島分配プログラムから得た。十分に洗浄した島を手でつかみ、10% ウシ胎仔血清、10mM HEPES緩衝液、1mM ピルビン酸ナトリウム、100U/ml ペニシリンG溶液、100μg/ml ストレプトマイシン硫酸塩、0.25ng/ml アンホテリシンBおよび71.5μM βメルカプトエタノールを補充した改変RPMI 1640培地(11.1mM グルコース)中で懸濁し、そしてコンカナバリンA(ConA)でコーティングされているFalcon 3043 12ウェル組織培養プレートに添加した。この島調製物を、95% 空気および5% CO2とともに37℃で96時間インキュベートした。これらの条件において、多くの島は懸濁液中に残存した(浮遊した)が、一方線維芽細胞および他の非島細胞は基底に接着した。96時間のインキュベーション後、懸濁された島を含む培地を慎重に移し、この島を手動で取り出し、そしてここで各々20ng/mL 塩基性線維芽細胞増殖因子(bFGF)および上皮増殖因子(EGF)をさらに補充された改変RPMI 1640培地中に再懸濁した。この島懸濁液(1ウェル当たり20〜30の島を含む)を、ConAでコーティングされない12ウェル組織培養プレートに添加した。この島は、プレートの表面に直ちに接着した。数日以内に、外へ向かい、島を離れて成長する細胞の単層を観察した。特定の場合において、ヒト由来の細胞を、2.5mM グルコースおよびアクチビン−A(2nM)、肝細胞増殖因子(100pM)またはベータセルリン(500pM)を含むいくつかの増殖因子の組み合わせを含む改変RPMI培地中で培養した。細胞を10mM ニコチンアミドでチャレンジした実験において、この培地は血清も増殖因子も含まなかった。
(膵臓幹細胞の分化に対するグルコースおよびGLP−1の効果)
血漿グルコース濃度の上昇は、膵臓島サイズの増大を導いた。従って培養培地におけるグルコース濃度の効果を、単離された島を用いて研究し、この島はネスチンポジティブな幹細胞を含んだ。ラット膵島を、高(16.7mM)グルコースを含む培地または正常(5.6mM)グルコースでの培地中で培養した。4日後RT−PCRを行って、ネスチンmRNAのレベルを決定した。この結果は、正常グルコースで培養された島と比較して、高グルコースで培養された島においてネスチンmRNAレベルを3倍刺激したことを示した(図6)。
症状の緩和をもたらす真性糖尿病型に対する処置を、糖尿病の症状を示す動物において試験した。動物は、ヒトにおける糖尿病の処置に有用な因子および手順についてのモデルとして役立つことが意図された。従って糖尿病についての潜在的な処置は、潜在的な処置を動物に投与し、そしてその効果を観察し、そして処置された動物を未処置のコントロールと比較することによって、動物モデルにおいて第一に試験され得た。
(ネスチンポジティブなヒトおよびラットの膵臓幹細胞の免疫細胞化学同定)
膵島を、ネスチン発現について分析した。島および幹細胞を、上記のように単離した。ネスチン発現を、16日胚(E16)のラット膵臓の発達中の島集団内部の細胞の異なる集団において(図8A)、そして成体膵臓(生後60日)の島において(図8B)、免疫細胞化学染色によって観察した。免疫細胞化学染色を、以下のように行った。

(ATP依存性輸送体ABCG2は、膵島由来のネスチンポジティブな細胞において発現される)
ヒト島由来NIPは、ABCG2、MDR1およびネスチンを共発現するSP細胞の実質的な部分集団を含んだ。ネスチンは、神経および筋肉の幹/前駆細胞のマーカーとして第一に示された(Lendahlら、1990、Cell 60:585:Zimmermanら;1994、Neuron 12:11)。神経幹細胞は、ニューロンおよびグリア細胞の異なる型を生じさせる高程度の可塑性を特に示した。神経幹/前駆細胞はまた、造血細胞に分化し、これは神経幹/前駆細胞をまさに多能性幹細胞にする(Shihら、2001、Blood 98:2412)。これまでの多能性の成熟幹細胞の最良の例は、骨髄由来の幹細胞、いわゆるSP細胞であった(Goodellら、1996、J.Exp Med 183:1797)。SP細胞は、蛍光生体染料であるヘキスト33342を有効に排除し、そして骨髄再占有(repopulating)細胞の大多数を含むその特性によって同定されていた。SP細胞は、成体ヒトおよびマウスの骨髄全体の0.05%を表し、そしてあらゆる造血系統を生じさせることは別として、このSP細胞はまた、骨格筋および心筋、ならびに内皮細胞を生じさせ得た(Gussoniら、1999、Nature 401:390;Jacksonら、2001、J Clin Invest 107:1395)。ATP結合カセット輸送体ABCG2(BCRP1)は、SP表現型の主要な構成要素であることが実証されており、従って明確にSP細胞を同定する分子手段を提供した(Kimら、2002、Clin Cancer Res 8:22;Scharenbergら、Blood 99:507;Zhouら、2001、Nat Med 7:1028)。特に、骨髄由来のSP細胞は、たとえあったとしてもインビトロでの増殖能は限定されるが(Buntingら、2000、Blood 96:902;Stormsら、2000、Blood 96:2125)、骨髄細胞におけるP糖タンパク質ポンプ(MDR−1)機能の増強は、インビトロでのSP幹細胞の拡張およびインビボでの細胞の再占有をもたらした(Buntingら、2002、Stem Cells 20:11)。

NIP(そのネスチンの発現によって神経幹細胞に関連される)はまた、ヘキスト33342排除アッセイによって同定されるNIPの実質的な部分集団におけるそのABCG2の発現によって、骨髄SP幹細胞の特性を示した。このNIPのSPはまた、MDR−1を発現し、これはインビトロでのその持続拡張に寄与した。

SP細胞および非SPコントロール細胞を、FACSによって培養NIPから単離した(図20A;それぞれゲートR1およびR2)。ABCG2の発現は、RT−PCR(図20B)およびクローニングされたABCG2プローブとのサザンブロットハイブリダイゼーションによって示されるようにSP表現型と良好に相関した。RT−PCRおよびサザンブロットハイブリダイゼーションは、培養NIPのSP画分においてMDR−1((P糖タンパク質)、造血幹細胞に存在する別のATP結合カセット輸送体(Chaudharyら、1991、Cell 66:85)の発現を実証した(図20B)。このSP画分はまた、非SPコントロールよりも著しく高いレベルでネスチンを発現した(図20B)。
(GLP−1Rポジティブなヒト膵臓幹細胞の免疫細胞化学同定)
ネスチンポジティブな膵島幹細胞を、GLP−1R発現について分析した。ヒト島組織を、Juvenile Diabetes Research Center for Islet Transplantation、Harvard Medical Schoolから得た。NIPを以前に記載されるように単離した(Zulewskiら、2001、Diabetes 50:521)。手短に言えば、島を洗浄し、そして血清、11.1mM グルコース、抗生物質、ピルビン酸ナトリウム、βメルカプトエタノールおよび増殖因子を含むRPMI 1640培地中で培養した。数日以内に、ネスチンポジティブな細胞(上記のように免疫細胞化学的に同定された)は、島を離れて成長した。後にこれらの細胞をクローニングし、そして20ng/mlの塩基性線維芽細胞増殖因子および上皮増殖因子、または1000単位の組換えヒト白血病阻害因子を含む培地中で拡張した。血清の非存在環境下においてGLP−1とのインキュベーションを行い、そして新しいペプチドを、培地を変えずに48時間毎に添加した。
(RT−PCRによるGLP−R1ポジティブなヒト幹細胞の同定)
膵島(NIP)におけるGLP−1R発現の免疫細胞化学同定を確認するために、RT−PCRを、NIPから調製した総RNAを用いて行った。RT−PCRを、上記の実施例6のように行い、ネスチンmRNAの同定についての差異は、以下のGLP−1R特異的プライマーを使用したことであった:5’gtgtggcggccaattactac3’(フォワード);5’cttggcaagtctgcatttga3’(リバース)。NIP mRNAの増幅は、予測された346bpの産物を産生し(図22B)、GLP−1Rタンパク質の発現に加えて、NIPはGLP−1Rを産生する生合成能を有することを示した。従ってネスチンに加えてGLP−1Rは、本発明において膵臓幹細胞に対するマーカーとして有用であった。
(ネスチンポジティブな幹細胞のインビトロ増殖)
ネスチンポジティブな幹細胞がインビトロで増殖する能力を決定した。
(真性糖尿病を有するヒト被験体においてIDX−1を発現するように操作された膵臓幹細胞の移植)
ブタまたはヒトのドナー膵臓、あるいは起こり得るヒト移植レシピエントの膵臓生検から単離された島を、幹細胞の生長を刺激する条件においてエキソビボで培養した。次いで幹細胞を島から単離し(クローニング)、bFGF−2、EGFおよび11.1mM グルコースを含む増殖培地においてインビトロで拡張し、転写因子IDX−1をコードするDNAを含む発現ベクターを用いてトランスフェクト/注入し、そして糖尿病レシピエントに移植した。あるいはIDX−1をトランスフェクトされた幹細胞を、操作された幹細胞のβ細胞への分化のプロセスを開始するために、移植前にGLP−1あるいは他の分化モルフォゲンまたは増殖因子で1〜3日間処理した。一実施態様において、幹細胞を、レシピエントに投与する前に拡張も分化もしないか、あるいはレシピエントに投与する前に拡張または分化のみ行った。一実施態様において、GLP−1を、幹細胞の分化を刺激しそして首尾良い移植を促すために、移植の間および移植後数日間レシピエントに投与した。この方法に従って、異種移植(ブタ島)または同種移植(レシピエントではないヒトドナー由来のヒト島)、ならびに同系移植(レシピエント由来の島)を行った。宿主レシピエントへ移植された場合、幹細胞遺伝的レパートリーが、宿主が幹細胞を自己として認識する(異種移植または同種移植の場合において)ように再プログラムされて、その結果自己免疫(1型糖尿病)による免疫不寛容ならびに移植片拒絶および移植片破壊が生じないと仮定された。
(真性糖尿病を有するヒト被験体におけるIDX−1の発現を刺激するように培養された膵臓幹細胞の移植)
上記のように単離された島を、島内部に存在する幹細胞の拡張(増殖)を第一に刺激し、次いで転写因子IDX−1の発現を刺激する条件において、数日間エキソビボで培養した。幹細胞の増殖を、bFGF−2、EGFおよび11.1mM グルコースを含む培地において島を培養することによって達成した。IDX−1発現の誘導を、GLP−1および2mM グルコースの存在環境下でのインキュベーションによって達成した。記載される処理によってそのようにあらかじめ処理された島を、宿主レシピエントに移植した。さらに宿主レシピエントに、幹細胞をさらに拡張しそしてインスリン産生性細胞に分化させて移植の成功を増強するために、移植の間および移植後数日間GLP−1を投与し得た。
(腎臓への膵臓幹細胞の異種移植)
ヒトのネスチンポジティブな島前駆細胞(NIP)を記載されるように単離し、そして免疫抑制されていない8匹のC57B16マウスの腎被膜下に移植した。移植されたヒト細胞は、マウスレシピエントによって拒絶されなかった。現在の理解は、異種移植片(例えばヒト組織)は、5〜10日以内にマウスによって拒絶されるということであった。現在の理解と反対に、本発明者らは、今まで試験された8匹の非免疫抑制マウスのうちの8匹において、すべての移植片が首尾良く移植され、そしておよそ105〜106細胞の移植後1ヶ月(30〜38日)までに腎臓の極を巻き込む大集団の組織に増殖した。
(膵臓幹細胞の膵臓への異種移植)
ヒトのネスチンポジティブな島前駆細胞(NIP)を、記載されるように単離し、そして免疫抑制されないマウスの膵臓、および(a)ストレプトゾトシン(ストレプトゾトシン誘導性糖尿病を産生するため)処理によって傷つけられたマウスの膵臓または(b)炎症を伴う進行中の島が存在するNODマウスの膵臓へ移植した。
(膵臓幹細胞の異種移植による糖尿病の処置)

(NIPの肝臓表現型への分化)
肝臓幹細胞(卵円形細胞(oval cells))、肝臓星状細胞および膵臓の前駆細胞との間の明白な共通の性質の報告、ならびにいくらかの損傷後に再生中の膵臓が肝臓化生を受けるという観察の報告のために(Slack、1995;Reddyら、1991;Bisgaardら、1991;Raoら、1996)、本発明者らはRT−PCRを行って、幹細胞における肝臓発現性遺伝子を検出した。PCR産物を、XBP−1(肝細胞発生に必要とされる転写因子(Reimoldら、2000))、およびトランスチレチン(肝臓急性期タンパク質)について得た。いくつかの他の肝臓マーカー(例えばαフェトプロテイン(Dabevaら、2000)、E−カドヘリン(Stamatoglouら、1992)、c−MET(Ikedaら、1998)、HGF(Skrticら、1999)およびシナプトフィジン(Wangら、1998);図15を参照のこと)がまた発現された。膵臓および肝臓によって共有されるタンパク質(例えばHGFおよびシナプトフィジン)の発現は、胚前腸内胚葉に由来する共通の起源を反映し得、そして膵臓表現型または肝臓表現型のいずれかへの分化を示し得た。
(NIPにおけるGLP−1Rシグナリング)
GLP−1アミドの個々の単離されたNIPへの適用は、細胞内Ca2+濃度([Ca2+]i)のレベルを上昇させた。細胞を方眼のついたカバーグラス上にプレーティングして、ネスチン発現について試験するために、同じ細胞の引き続く免疫組織化学染色を可能にした。[Ca2+]i記録が行われたすべての細胞は、ネスチンを発現することを示した。成熟β細胞とは対照的に、GLP−1は、基礎的な(5.6mM)グルコースで[Ca2+]iレベルを刺激したが、高い(20mM)グルコースの存在環境下においては効果がなかった(図23、パネルA)。これらの効果を、フォルスコリンによって再現し(図23、パネルB)、これはGLP−1の効果が、GsおよびcAMP産生の活性化(正常なベータ細胞において使用される同じシグナリング経路)を通じて媒介されることを示唆した。ペプチドExtendin(9〜39)(GLP−1の特異的アンタゴニスト)での個々の単離されたNIPの前処理は、GLP−1によって媒介される[Ca2+]iの増大を妨げた(図23、パネルCおよびD)。GLP−1RアンタゴニストであるExtendin(9〜39)の効果は、GLP−1Rの同じアイソフォームが、β細胞のようにNIPにおいて発現されることを示唆した。GLP−1によって媒介される[Ca2+]iの増大は、細胞外(exprecellular)La3+(5μM)によって阻害され、これはGLP−1が、β細胞を脱分極するその公知の役割と一致して[Ca2+]i流入を活性化したことを示唆した(図23、パネルE)。本発明者らはさらに、トルブタミド(100μM)が、NIPにおける[Ca2+]i上昇を刺激することを実証し、これは細胞がATP感受性K+チャネルを発現することを示した(図23、パネルF)。これらのデータは、β細胞におけるその作用機構と一致して、GLP−1誘導性の膜脱分極、およびNIPにおける電圧依存性Ca2+チャネルの活性化と一致した。
(GLP−1はNIPのインスリン分泌性細胞への分化を誘導する)
以前の研究は、GLP−1のインスリン指向性作用、ならびに部分膵切除ラットにおいてβ細胞新生を刺激する能力を実証した(Xuら、1999、Diabetes 48:2270)。NIPを島培養物から選び、そしてbFGFおよびEGFを含む増殖培地において3〜10日間拡張した(継代01)(Zulewskiら、2001 Diabetes 50:521)。特定の場合において、3〜5日間拡張されたNIPOを固定し、そして図24Aに示されるようにネスチン(Cy−3)およびインスリン(Cy−2)の免疫細胞化学検出に供した。この段階で、NIPはネスチンポジティブでありそしてインスリンネガティブであることが見出された。NIP培養物を7〜10日間拡張し、次いでGLP−1またはその安定なアナログであるextendin−4のいずれかで処理した場合、細胞の部分集合は、インスリンポジティブ(Cy−2;図24B、パネル3、5および6)、およびIdx−1ポジティブ(Cy−3;図24B、パネル7)ならびにネスチンネガティブ(Cy−3;図23B、パネル4)になった。培養物をGLP−1で処理した場合、ネスチン発現は総体的に減少した(図24B、パネル2対4)。従ってExtendin−4での処理は、インスリン分泌の2〜3倍の増大を誘導した(図25)。しかしいくつかの培養ウェルにおいて、コンフルエントな状態だけで、少量のインスリン分泌を開始するのに十分であった。
(組織/器官移植片の有望な宿主レシピエントにおいて免疫寛容状態をあらかじめ誘導するためのNIPの使用のための方法)
NIPを、潜在的な器官ドナーに対して寛容状態をあらかじめ誘導するために使用した。最近公表された報告は、ドナーから移植レシピエントへの幹細胞の移植は、どの免疫抑制剤も必要とせずにドナー由来の移植片に対する寛容を誘導し得ることを示した(例えばFandrichら、Nat.Med.、Feb 2002、8:171;Quainiら、N.Engl.J.Med.、3 Jan 2002、346:5;Korblingら、N.Engl.J.Med.、7 Mar 2002、346:738を参照のこと)。本明細書中に記載される手順は、これらの報告を確証し、そしてドナーからレシピエントへの器官移植の前に、潜在的な器官ドナーに対して免疫寛容状態をあらかじめ誘導するというNIPの新規の使用を実証した。

- 肝細胞に分化するようにABCG2ポジティブな膵臓幹細胞を誘導する方法であって、
該ABCG2ポジティブな膵臓幹細胞を、肝細胞、または肝細胞に分化する前駆細胞に分化するように該幹細胞を誘導する有効量の因子で処理するステップ、
を含む方法。 - 上記因子がサイクロパミンであることを特徴とする請求項62または66に記載の方法。
- 肝臓疾患を有する患者を治療する方法であって、
(a)ネスチンポジティブな膵臓幹細胞をドナーの膵島から単離するステップと、
(b)該幹細胞が肝細胞に分化するような該幹細胞を該患者に移植するステップと、
を含む方法。 - 上記ネスチンポジティブな膵臓幹細胞がまたABCG2ポジティブであることを特徴とする請求項68に記載の方法。
- 上記ネスチンポジティブな膵臓幹細胞がまた、Oct3/4、GLP−1レセプター、ラトロフィリン(2型)、Hes−1、ネスチン、インテグリンサブユニットα6およびβ1、C−kit、MDR−1、SUR−1またはKir6.2からなる群より選択されるマーカーのうちの少なくとも1つについてポジティブであることを特徴とする請求項68に記載の方法。

(a)ABCG2ポジティブな膵臓幹細胞をドナーの膵島から単離するステップと、
(b)該幹細胞が肝細胞に分化するような該幹細胞を該患者に移植するステップと、
を含む方法。 - 上記患者が、上記のステップaの幹細胞に対する上記ドナーとして役立つことを特徴とする請求項68または72に記載の方法。
- 肝臓疾患を有する患者を治療する方法であって、

(c)該前駆細胞が肝細胞に分化するような該前駆細胞を該患者に移植するステップと、

- 上記ネスチンポジティブな膵臓幹細胞がまた、Oct3/4、GLP−1レセプター、ラトロフィリン(2型)、Hes−1、ネスチン、インテグリンサブユニットα6およびβ1、C−kit、MDR−1、SUR−1またはKir6.2からなる群より選択されるマーカーのうちの少なくとも1つについてポジティブであることを特徴とする請求項74に記載の方法。
- 上記ネスチンポジティブな膵臓幹細胞が、CD34、CD45、CD133、MHCクラスIおよびMHCクラスIIからなる群より選択されるマーカーのうちの少なくとも1つを発現しないことを特徴とする請求項74に記載の方法。
- 肝臓疾患を有する患者を治療する方法であって、
(a)ABCG2ポジティブな膵臓幹細胞をドナーの膵島から単離するステップと、
(b)該幹細胞をエキソビボで拡張して前駆細胞を産生するステップと、
(c)該前駆細胞が肝細胞に分化するような該前駆細胞を該患者に移植するステップと、
を含む方法。 - 上記患者が、上記のステップaの幹細胞に対する上記ドナーとして役立つことを特徴とする請求項74または78に記載の方法。
- 肝臓疾患を有する患者を治療する方法であって、
(a)ネスチンポジティブな膵臓幹細胞をドナーの膵島から単離するステップと、

(c)該ドナーの幹細胞に対して該レシピエントにおいて免疫寛容状態を誘導するステップと、
