Abstract
Description
放射線治療用の医療機器は、高エネルギー放射で腫瘍状組織を処置する。放射量およびその位置を正確に制御して、腫瘍状組織が破壊されるのに十分な放射を与え、周囲の隣接する非腫瘍状組織に対する損傷を最小限に抑えるようにしなければならない。
この方法に必要な入力は、1)部分CT画像、2)部分CT内の関連する解剖学的フィーチャの輪郭、3)部分CTについての線量分布であり、この線量分布は、部分CTについて直接計算するか、あるいは、間接的に計画CTについて計算し、画像融合とも称する重ね合わせにより部分CTに一致するように位置合わせする。
様々な重ね合わせ技術を用いることによって、本発明の様々な実施形態が実現される。基本的には、計画画像および部分画像を整列させて、処置前にどんな改変が必要とされるかを決定する。好ましくは、この重ね合わせは、自動化または半自動化された融合によって実施される。
本発明の方法では、オンラインCTまたは部分画像を取得する他の手段を用いて、放射線治療計画を完全に最適化し直さずに、標的構造への放射送達を改善する。より具体的には、患者に前処置を施すために体位固定する位置および規定部分についての内部の解剖学的構造の位置が、計画位置から剛体回転および並進移動分だけ異なるとき、既存の方法を用いて患者のこれらのオフセット量を決定し、同様に、この変位した位置を反映するように、患者の移動または送達の改変によってこれらのオフセット量を補正し得る。しかし、内部の解剖学的構造の変化の場合には、従来の方法が求める「正しい」患者の再位置決めでは、任意の標的および感受性構造に所望の線量が分配されないことがある。その代わりに、CADRを用いて線量分布に関して好ましい位置に患者を移動させ(または、処置を並進移動させ)、かつ/または、患者の現在の解剖学的構造が与えられる場合には、MMODSを用いて、所望の線量分布を実現するのにより適切な代替処置計画を選択し得る。
図11は、本発明の別の実施形態による改善された適応放射線治療法のブロック図である。
Claims (13)
- CADRにより、線量分布に対して部分画像の最適位置を見つけ、それによって、該部分の関連する構造が受け取る線量が改善される。
- CADRに用いる前記線量分布を、推定された部分画像について直接計算するか、あるいは、計画画像の線量分布に基づき、場合により、画像の重ね合わせと組み合わせて間接的に取得し得る。
- 最適化された物体関数が自由に変えられるものであり、適応放射線治療などを通じて、a)様々な構造に対して計画線量を模倣し、b)可能な場合には該線量を改善するか、あるいは解剖学的構造が変化する中で劣化を少なくし、c)所望の場合には(例えば、ある種の構造の保存に関して新たに見つかった懸念)、受け取る線量を改変するなどの目標を達成する項を含み得る。
- 単一の物体関数が可能であるが必要条件ではない。代わりに、複数の物体関数を用いることが可能であり、使用者は好ましい結果を選択し得る。
- 様々な場合にCADRの結果を微調整するために、使用者が物体関数および重み付けを調整し得る。
- 使用者のトレーニングに基づいて、システムが物体関数の重みを学習し得る。
- 患者を移動させるか、または送達を改変するか、あるいはこれら2つの組合せによって、結果を利用し得る。
- CADRに必要とされる輪郭は、手作業の輪郭処理、自動輪郭処理、変形可能な融合、テンプレート・ベースの自動輪郭処理、またはこれらの組合せを含めて任意の利用可能な方法によって生成し得るが、これらの方法に限定されるものではない。
- MMODSを用いて、異なるマージン(例えば、きつい、普通、緩い)、または異なる目的(例えば、攻撃的な処置、感受性構造の保存)を有する複数の計画をあらかじめ最適化し得る。それぞれの日に、その日の患者の解剖学的構造の最良推定値に基づいて、最適化された可能性から適切な計画を選択し得る。
- MMODSおよびCADRは独立に機能し得るが、合わせて用いることも可能である。
- MMODS法およびCADR法は、特定の画像化様式または画像取得手段に固有なものではなく、より最新の患者の位置または解剖学的な情報を取得する任意の形態に適用し得る。
- MMODS法およびCADR法は、最初に利用可能な画像および関連する処置計画をともに含む任意の利用可能な計画画像を用いて適用し得るだけでなく、最初の計画の後で(例えば、処置過程の最中に)取得された任意の画像によって生成された計画にも適用し得る。
- MMODSにおいて計画を選択する工程およびCADRにおいて再位置決めする工程では、画像情報、輪郭情報、線量−体積ヒストグラム、および線量測定情報を含む情報を用いることが可能であるが、これらの情報に限定されるものではない。
