Abstract
Description
生体組織心臓弁は、構造的な欠陥がなく、成人が生存している間、持ちこたえる弁に対する患者の継続的な要求を満足させるべく、幾つかの特別なデザインに発展してきた。そのような弁に関する新しいデザインの主たる焦点は、弁の生物学的な耐久性と同様に機械的な耐久性を有意に向上させてきた。前記に加えて、これらの弁は、ねじれを生じることなく、一貫した結果が得られるように、外科医による埋め込みを容易にするものでなければならず、大動脈弁の縫合リングのデザインは石灰性の環および二尖弁の環の両方と適合するようになっていることを要する。
本発明は、弾性を有する積層内側フレームを3つ支持する、薄くて堅い(または硬質の)外側フレームを含んで成る生体心臓弁を提供する。各内側フレームは、弾性を有するばね用金属(またはスプリングメタル)から成る薄い三日月形状のストリップが1つまたは複数の金属ピンで留められた形態の積層物を含む。薄い布が、宿主組織の過成長および弁葉の取付けのために、当該内側フレーム構造物を被覆する。内側フレーム同士が繋がれていることは、弁葉組織が摩耗する可能性を無くす。独立している内側フレームはそれぞれ、1つの弁葉に対して正確で且つ首尾一貫した幾何学的な位置合わせ(またはポジショニング)をもたらし、各々は、対称的かつ同期的な弁葉の運動のために、好ましくは非線形的なばね特性を有するように設計される。そのような積層構造は、弁の脱出をもたらすことなく、接合部(または合わせ目)および接合ゾーンにおいて応力を低下させることができる。
ビジョンのある血流学的な流れ効率の良いウシ心膜心臓弁が設計された。弁構造物は、自立性を有し、バルブが閉じている間に接合面の先端にて生じる局所的な高いストレスを低減させる積層された構造デザインを有する。構造物の軟らかい先端は、局所的にのみ反れて、接合部でのストレスを減少させる。加えて、自立性を有する弾性構造は、一定した、反復可能な弾性スプリングレートを与える(スプリングレートは、偏向により分割される力に等しい)。
Claims (19)
- 中央の通路を有する弁輪、
概して三日月形状である3つの支柱であって、弁輪に沿って角度間隔をあけた位置にて、略中央部付近で前記弁輪に接続されている支柱、および
3つの独立した生体適合性を有する可撓性のある弁葉であって、それぞれが前記通路を閉じるように互いに向かって収束し、それから分かれて当該通路を再び開くように配置されて前記支柱の各々に取り付けられている弁葉、および
前記弁輪を取り囲んでいる縫合リング
を含む置換組織心臓弁であって、
各弁葉が他の弁葉とは独立して動く自立性ユニットとして支持されており、
前記弁輪および前記支柱が生体適合性を有する布で覆われており、
置換組織心臓弁。 - 前記三日月形状の各支柱が複数のピースから成り、当該ピースはシートの形態である生体適合性を有する金属弾性材料から成り、個々のピースは積層物としてそれぞれ互いに並置されている、請求項1に記載の心臓弁。
- 前記積層物を構成する個々のピースがその中央領域内で物理的に互いに相互接続されている、請求項2に記載の心臓弁。
- 前記積層された三日月形状の支柱が、少なくとも3つの独立した三日月形状のピースであって、各々が異なる長さを有するピースを含んでなる、請求項3に記載の心臓弁。
- 前記積層された三日月形状の支柱が、対称的且つ同期的な弁葉の動きを生じさせる非線形のばね特性を有する請求項2に記載の心臓弁。
- 隣接する弁葉が、前記支柱の端部に近い位置にて互いにゆるく繋がれており、その他の部分が独立して自由に動く状態となっている、請求項2に記載の心臓弁。
- 前記積層物がアンカーピンによって前記弁輪に接続され、当該ピンは前記三日月形状の支柱に取り付けられた前記弁葉の周縁領域を通過している、請求項2に記載の心臓弁。
- 前記弁葉がウシ心膜組織の部分である、請求項7に記載の心臓弁。
- 前記組織が、周方向における応力および伸びに対する抵抗性が、半径方向におけるそれよりも少なくとも約10〜20%大きくなるように配向されている、請求項8に記載の心臓弁。
- 前記弁輪が平面方向において三葉形状であり、流入口にて波形である軸方向輪郭を有する、請求項2に記載の心臓弁。
- 前記弁輪が、円形に溶接されて、それからダイで引っ張り成形されて、前記三葉形状および波形を生成している、コバルト−クロム−ニッケル合金のストリップで形成されている、請求項10に記載の心臓弁。
- 前記弁輪が概して平面である入口端部および概して平面である出口端部を有し、当該平面が互いに対して約15度〜約25度の角度にて向けられている、僧帽弁を置換するために設計されている請求項1に記載の心臓弁。
- 前記弁輪の前記入口端部が概して円形であり、前記弁輪の前記出口端部が概して長円のD字形状を有する、請求項12に記載の心臓弁。
- 縫合リングとして作用し、且つ心臓の残部から弁を保護する弾性ポリマーリングが前記入口および出口端部のそれぞれにて前記弁輪の外側表面に取り付けられている、請求項12に記載の心臓弁。
- 左心房のキャビティで弁輪下に埋め込まれるように設計され、ウシの心膜組織の裾が前記入口端部を囲み、当該裾が左心房の壁に取り付けられるような形状および大きさにされて、埋め込まれた弁を囲む人工のフロアーを左心房内に形成する、請求項14に記載の心臓弁。
- 中央の通路を有する弁輪、
概して三日月形状である3つの支柱であって、弁輪に沿って角度間隔をあけた位置にて、その略中央部付近で前記弁輪に接続されている支柱、および
3つの独立した、生体適合性を有する可撓性のある弁葉であって、それぞれが前記通路を閉じるように互いに向かって収束し、それから分かれて当該通路を再び開くように配置されて前記支柱の各々に取り付けられている弁葉
を有する人工心臓弁であって、
前記三日月形状の各支柱が複数のピースから成り、当該ピースはシートの形態である生体適合性を有する金属弾性材料から成り、個々のピースは積層物としてそれぞれ互いに並置されており、
当該個々のピースは、中央領域においてのみ互いに接続されて、その端部領域は独立して動くものである、
人工心臓弁。 - 前記積層された三日月形状の支柱は少なくとも3つの独立した三日月形状のピースを含み、当該ピースの各々は異なる長さを有し、当該積層された三日月形状の支柱は対称的且つ同期的な弁葉の動きをもたらす非線形なばね特性を有する、請求項16に記載の心臓弁。
- 前記金属弾性材料のピースがすべて、所望のばね定数を与えるために形成された長円の開口部を有し、前記各支柱において最も長い長さを有する金属弾性材料から成る三日月形状のピースが他の2つのピースの内側に半径方向に配置されている、請求項17に記載の心臓弁。
- 最も長い長さを有する前記ピースのうち隣り合うものが先端付近の位置にて互いにゆるく繋がれている、請求項18に記載の心臓弁。
