本発明は、例えばポリマーに基づく製剤の粒子状懸濁液のための注射媒体、関連医薬製剤及び方法に関する。特に、本発明は、注射媒体、医薬製剤、及び粒子状懸濁液を注射するためにより小さな内径の針の使用を可能ならしめる方法に関する。

(発明の背景)
治療薬のための徐放性供給システムは、近年、かなりの注目を集めてきた。その例として、放出調節注射及び経口製剤、経皮貼布、及び移植可能な貯蔵製剤が含まれる。このようなシステムは、治療用タンパク質を供給する手段として特に興味深いものである。
治療用タンパク質製剤を製造する場合、物理的、化学的及びタンパク質の生物学的性質を保つことが重要である。より低分子量の薬剤と異なり、典型的に、タンパク質はより大きな球状構造を有し生物学的活性にとって重要な、二次構造、三次構造、場合によっては四次構造的特徴を包含する。さらに、タンパク質は、酸化(メチオニン、トリプトファン、ヒスチジン、チロシン)、脱アミド化(アルギニン、グルタミン)又はジスルフィド還元もしくは交換(システイン)を受けやすい側鎖上に不安定な結合及び化学的反応基を有する。生物学的活性を保つことに加え、タンパク質の免疫原性を増加させるタンパク質変性を減少させ、取り除くことが特に重要である。望ましくない免疫反応は、身体の安全関連事象を誘導し、中和抗体反応は続いて行われる処置の有効性を制限しうる。従って、生物学的環境において長期間わたり治療用タンパク質を安定化する必要性が、徐放性タンパク質供給システムの発達にとって障害となっていた。

薬剤を安定化する一つの方法は、生物分解可能な重合体の微小粒子中にそれらを包埋させることである(Maulding(1987), J. Controlled Release 6:167−176;Smith等、(1990), Advanced Drug Delivery Reviews 4:343−357;Holland等、(1986), J. Controlled Release 4:155−180;Lewis等、(1990), Biodegradable Polymers as Drug Delivery Systems, pp. 1−41, Dekker, New York.)。乳酸及びグリコール酸のホモ重合体及び共重合体(PLGAポリマー)から作られた微小粒子を用いた研究により、これらのポリマーが酸単体に加水分解し(Maulding(1987), J. Controlled Release 6:167−176;Smith等、(1990), Advanced Drug Delivery Reviews 4:343−357;Cower等、(1985), Methods in Enzymology 112:101−116)、微小粒子を調製するために用いられた条件下では化学的に不活性であることが示された。このようなポリマーは、特定の処理に応じた薬剤放出速度の調整を可能にする分子量及びモノマー比の範囲において製造することができる。PLGAポリマーは、非免疫原性及び無毒性である。これらの特徴により、黄体形成ホルモン放出ホルモン(LHRH)アゴニストであるリュプロリド(luprolide)の貯留製剤についての使用に対してPLGAポリマーの選択に至った(Sanders等、(1986), J. Pharm. Sci. 75:356−360;Ogawa等、(1988), Chem. Pharm. Bull. 5:1095−1103;Ogawa等、(1988), Chem. Pharm. Bull. 36:2576−2581)。Johnson等、(1997)(Pharmaceutical Research 14:730−735)は、組換体ヒト成長ホルモンを亜鉛−タンパク質複合体を形成させることにより安定化させ、その複合体を固体状態でPLGA微小粒子中にカプセル化させた(PCT出願、PCT/US95/05511、国際公開第WO95/29664号も参照のこと)。

タンパク質を安定化することに対して利点を有していたにも拘わらず、ポリマーに基づく薬物製剤の投与には問題がある。例えば、容易に懸濁し注射し得る製剤の量により、投与量が制限される。粒子を含む製剤中で、凝集体又は膨張体が注射針に目詰まりし、意図する投与量を投与することを困難にしている。凝集作用を減少させるために、カルボキシメチルセルロース(CMC)、デキストラン、又はソルビトールのような賦形剤が、注射媒体中に含められた。また、界面活性剤及び塩が粒子状流動性を変えるために添加された。CMC、ツイーンTM、及びリン酸塩緩衝液が、リュプロン貯留製剤を供給するための媒体中に使用された。しかし、この製剤の投与量は、比較的少量である:微小粒子あたり30−60 mgである。通常、21−又は23−ゲージの注射針を通して、微小粒子当たり200 mg/mL以上の投与量を注射することは困難である。1 mLの最大皮下投与量を想定すると、最大微小粒子投与量は200 mgとなる。このように大内径の注射針の使用は、注射の痛みを増大させるが、より小さな内径の注射針の使用は一回の注射で供給し得る投与量をさらに制限する。

通常、粒子状懸濁液及び/又はポリマーに基づく薬物製剤の注射性能を促進する注射媒体は、薬物の高投与量の供給を可能にし、及び/又はより小さな内径の注射針の使用を可能ならしめる。このような利点により、より広範にわたる治療適用に対して、ポリマーに基づく製剤の可能性を増加することになるであろう。

(発明の概要)
本発明は、注射により粒子状懸濁液を投与することにおいて使用に適した液体を含む。当該注射媒体には、食塩水のような生理的なバッファーに溶解されたヒアルロン酸又はその誘導体のような可動性に富む分子が含まれる。本発明の注射媒体は、通常の注射媒体を用いる場合に可能である内径より小さい内径の針を使用し、ポリマーに基づく製剤などの粒子状懸濁液のより高投与量の注射を可能にする。
また、本発明は、当該発明の注射媒体中に生物学的活性薬剤の有効量を含む医薬製剤を提供する。生物学的活性薬剤は、注射媒体中において、粒子状の形態で、粒子上にコートされ、粒子内に分散され、又は粒子に付着した形態をとり得る。このような製剤は、予防的、治療的、又は診断的な処置に対する生物学的活性薬剤を投与するために用いられ得る。

好まし実施態様において、生物学的活性薬剤はポリペプチドの持続放出を提供する粒子内に分散されたポリペプチドである。好ましくは、粒子は、ポリ(ラクチド−グリコリド共重合)基質などの生体適合性ポリマー基質から構成される。好ましい製剤には、生物学的活性薬剤として、例えば、成長ホルモン、肝細胞成長因子(HGF)、血管内皮成長因子(VEGF)、抗VEGF Fab,グルカゴン様ペプチドI(GLP−I)、神経成長因子、又はインシュリン様成長因子が含まれる。
また、本発明は、ここで記述される医薬製剤を製造し投与する方法を含む。好ましい実施態様において、本発明による医薬製剤が23−ゲージ又はより小さな内径の注射針を通した注射により投与される。

(発明の詳細な説明)
定義
明細書及び特許請求の範囲において用いられる用語は、他に明記されない限り下記に示すように定義される。
「注射媒体」という用語は、注射(例えば、皮下、筋肉、静脈など)により薬剤を投与するのに使用するために適する液体を意味する。
「偽塑性組成物」という用語は、剪断速度の増加に伴い減少する粘性を有する組成物を意味する。剪断とは、あるプレートが他のプレートに相対してプレート間の溶液と共に動かされる場合に生じる摩擦を意味する。剪断速度とは、プレートが動かされる速度のことである。また、剪断は、粘性物質が注射針を通って流動するときに生じ、従って、偽塑性は、このような物質が注射され得る容易性に影響を与える。

「ヒアルロン酸」は、交互性のあるβ(1−3)グルクロニド及びβ(1−4)グルコサミド結合を持つ大きな分岐ムコ多糖類分子である。ここで用いられるように、「ヒアルロン酸」という用語は、脊椎動物の結合組織の細胞外マトリックス中に見い出される天然に生じる分子と同じ構造を有する形態を意味する。
広範にわたるヒアルロン酸の化学修飾が可能である。「ヒアルロン酸誘導体」という用語は、ヒアルロン酸「背骨」(即ち、交換性のグルクロニド及びグルコサミド結合のこと)及び天然に生じるヒアルロン酸中には存在しない一又は複数の化学基を有する分子のことを意味する。議論を簡略にするため、ヒアルロン酸及びその多様な誘導体は、「ヒアルロン酸」と総称される。ヒアルロン酸の例には、エステル、アミド及びラクチドの誘導体が含まれる。ヒアルロン酸に特異的なアシル誘導体の形成は、米国特許第5,527,893号中に記載されている。さらに、水溶性ヒアルロン酸誘導体は、米国特許第5,017,229号及び4,937,270号に記載されている。ヒアルロン酸は、ポリエチレングリコールに結合され、このような「ペグ化(pegylated)」形態も、この用語をここで用いるときには、「ヒアルロン酸」である。エステル又は塩の形態のヒアルロン酸、「ヒアルロン酸塩」と称されるが、本発明おいて都合に合わせて用いられる。

ヒアルロン酸の分子量は変動し、従って、ヒアルロン酸の分子量に対するここでの言及は、平均分子量を意味する。
「パーセント バイ ボリューム」又は「%(v/v)」で表現される濃度とは、組成物の総容量で構成成分の容量を除することにより計算される値である。
「パーセント バイ ウェイト」又は「%(w/w)」で表現される濃度とは、組成物の総重量で構成成分の重量を除することにより計算される値である。
「パーセント ウェイト パー ボリューム」又は「%(w/v)」で表現される濃度とは、重量(例えば、グラム)を容量(例えば、リットル)で除することにより計算される値である。
「粘性」とは、流れに対する溶液の抵抗力の目安であり、典型的にはセンチストーク(cSt又はcs)の単位である。
「生理食塩水」とは、0.9 %(重量/容量)の水中の塩化ナトリウムとして定義される。
「ポリマーに基づく製剤」とは、生物学的に活性のある薬剤が重合マトリックス中に分散されている製剤として定義される。
「重合マトリックス」とは、一又は複数の他の物質及び/又は全体に分散された空間を持つ不連続な重合体構造を意味する。
「生体適合性」とは、毒性又は免疫原性のような重大かつ有害な影響を及ぼすことなく、ここで論じられる量でヒト生体内に導入され得る物質を意味する。また、この用語は、その物質がインヴィボで分解される場合、インヴィボでの分解産物が重大かつ有害な影響を引き起こさない物質も示す。
「生物分解性」とは、より小さな単位を形成するために、インヴィボにおいて分解又は腐食する組成物を意味する。分解は、酵素学的、化学的及び物理的なプロセスを含むインヴィボで生じる何れかの過程より生じ得る。

「ブロック化ポリマー」は、ブロックされたカルボキシル末端基を有する。通常、ブロック化基は重合開始因子に由来し、典型的にはアルキルラジカルである。「非ブロック化ポリマー」は、フリーのカルボキシル末端基を有する。
「微小粒子」とは、最大約1mmの以下の寸法(即ち、長さ又は直径)を持つ粒子を意味するのに用いられる。実質的に球状及び/又は楕円体である微小粒子は「微粒子」と名付ける。
「生物学的活性薬剤」とは、インヴィボでの活性であって、典型的には治療的、予防的、及び/又は診断上の利用を与える活性を有する薬剤として定義される。
「タンパク質」及び「ポリペプチド」はここでは相互に変更し得るものとして用いられる。
「有効量」という用語は、インヴィボで活性のある活性薬剤の生物学的量を意味する。
「無菌的処理工程」という用語は、工程や製品に使用される個々の構成成分の滅菌に関連する滅菌工程を意味し、その後、無菌的な条件下においてカード記入される活性を処理し包装する。多くの通常の方法は、無菌的過程において使用され、製品、その容器、及び閉止法の物理的特徴に依存する。実施例には、ろ過滅菌(液体)、乾熱滅菌(ガラス製品)、高圧滅菌(ラバー製品)、及び放射線及び/又はエチレンオキシド(プラスチック製品)処理を含む。
「最終滅菌」という用語は、予備滅菌されるか又はされていない薬物が、容器に充填され密封され、その後最終滅菌にかけられる過程を意味する。多くの技術が末端滅菌において使用可能であり、例えば、放射線及びオートクレーブが使用可能である。

注射媒体
本発明は、偽塑性組成物を含む注射媒体を提供する。偽塑性組成物は、通常の注射媒体と比較して改善された注射可能性を提供し、例えば、ポリマーに基づく組成物のような粒子の懸濁液を注射するのに特に有用である。本発明に従う偽塑性組成物は、通常、剪断力に曝した場合に楕円体高次構造へ変形する可動性分子の溶液であり、剪断速度の増加とともに流動方向に整列される傾向にある。本発明において使用するのに適した可動性分子は典型的には大きな分岐した重合分子である。例示的な可動性分子には、ヒアルロン酸、ヒアルロン酸誘導体及びそれらの組合せが含まれる。ヒアルロン酸が好ましく、ヒアルロン酸ナトリウムの形態で都合よく用いられる。

偽塑性組成物の選択に影響を及ぼすことにおいて考慮すべき事には、粘性、分子量及びpHが含まれる。通常、本発明の使用に適した偽塑性組成物は、十分粘性があるため、注射の間、ポリマーに基づく製剤のポリマー成分を注射媒体中に懸濁させた状態にできる。動力学的粘度は、25℃、1/秒の剪断速度において約1,000から約500,000センチストーク(cSt)の範囲であり得、好ましくは、約1,000から約100,000cStの範囲、及びより好ましくは、約2,000から約65,000cStの範囲であり得る。
可動性分子を含む溶液の粘性は、温度及び用いられ溶媒のみならず分子量及び分子濃度の複合機能である。特に、粘性は分子量及び濃度が増大するにつれて増加する。本発明の偽塑性組成物への使用に適する可動性分子は、通常、約0.5 x 105から約5 x 106ダルトンの範囲に平均分子量を持つが、より高い平均分子量又はより低い平均分子量の分子を用いることが可能である。本発明において有用なヒアルロン酸に対するより好ましい平均分子量は、約2 x 105と約5 x 106ダルトンの間である。

可動性分子の濃度は、通常、約0.01から約10.0%(w/v)の間であるが、当業者は、平均分子量が増加するにつれて、特定の粘性を達成するのに必要な濃度は減少することを理解することができる。上述の好ましいサイズのヒアルロン酸は、約0.01%から約3%(w/v)の濃度で都合よく用いられ、好ましくは約0.1%から約1%(w/v)の間であり、より好ましくは、0.8%(w/v)以下である。
偽塑性組成物のpHは、通常、おおよそ7.0の生理的基準に近い値である。好ましくは、偽塑性組成物は約5.0から7.8の範囲のpHである。
偽塑性組成物は、上述したような可動性分子を溶解させることが可能ないずれかの溶媒を含み、調製された溶液は本発明に適した粘性及びpHを持つ。当業者は、特定の可動性分子に適した溶媒を容易に選択できる。水性溶媒が好ましく、その例として、生理的緩衝液(生理的リン酸緩衝液などの)及び生理食塩水が含まれる。

当該発明の医薬製剤は、本発明の注射媒体、粒子、及び生物学的活性薬剤の注射媒体を含む。粒子及び生物学的活性薬剤は、単一の構成成分(即ち、生物学的活性薬剤が粒子状形態であってよい)であってもよく、又は2つの異なる構成成分であってもよい。後者の例には、生物学的活性薬剤が粒子上にコートされ、粒子内に分散される実施態様が含まれる。好ましい実施態様では、分散された生物学的活性薬剤を有する重合性マトリックスから調製される微小粒子を用いる。微粒子/生物学的活性薬剤構成成分の濃度は、注射され得る構成成分の所望の投与量及び最大量に依存する。例えば、分散された生物学的活性薬剤を含む重合性の微小粒子は、通常約1 mg/mLと約500 mg/mLの間の濃度、より好ましくは約50 mg/mLと150 mg/mLの間の濃度が用いられる。

ポリマーに基づく製剤において、重合性マトリックスを形成するポリマーは、生物分解性又は非生物分解性、生物分解性及び非生物分解性ポリマーの混合物、又は生物分解性及び非生物分解性単位を含む共重合体である生体適合性ポリマーである。適当な生体適合性、生物分解性ポリマーには、例えば、ポリ(ラクチド)、ポリ(グリコリド)、ポリ(ラクチド−グリコリド共重合)、ポリ(乳酸)、ポリ(グリコール酸)、ポリ(乳酸−グリコール酸共重合)、重合無水物、ポリオルトエステル、ポリエーテルエステル、ポリカプロラクトン、ポリエステルアミド、ポリエチレングリコール及びラクチド又はグリコリドのブロック共重合体、及びこれらの混合物及び共重合体が含まれる。

本発明の医薬製剤には生物学的活性薬剤の有効量が含まれる。生物学的活性薬剤の有効量は、治療上、予防上又は診断上有効な量であり、投与経路;体重;年齢;身体的状態;治療上、予防上又は診断上の目標;及び生物学的活性薬剤のタイプを考慮して当業者により決定される。
ポリマーに基づく製剤の場合、使用されるポリマーのタイプ、最初の破裂及び続いて生じる所望の放出レベル、及び放出速度も、所望の効果を産み出す生物学的活性製剤の量に影響を与える。最初及び続く放出レベル並びに放出速度は多くのポリマーマトリックスに対して知られている。また、これらの変数は、一又は複数の異なる濃度の生物学的活性製剤を含むポリマーマトリックスのインヴィトロにおける比較テストにより、経験的に決定することができる。典型的には、生物学的活性製剤の放出を調節することを目的とされるポリマーマトリックスは、約0.01%(w/w)から約50%(w/w)の生物学的活性製剤を含む。

本発明に係る医薬製剤は、微粒子/生物学的活性薬剤成分を本発明に係る注射媒体へ加えることにより製造される。ポリマーに基づく製剤のため、生物学的活性薬剤を含むポリマー状微小粒子が注射媒体へ添加される。
このようなポリマー状微小粒子を形成するために、適するポリマーがポリマー溶液を形成するために溶媒中に溶解される。適する溶媒の例には、例えば、塩化メチレン、ジクロロメタン、クロロホルム、テトラヒドロフラン、ジメチルスルホキシド、及びヘキサフルオロイソプロパノールなどの極性有機溶媒が含まれる。通常、適するポリマー溶液は約5%から約30%のポリマー(w/v)を包含する。さらに好ましい実施態様においては、ポリマー溶液は約5から約15%のポリマー(w/v)を包含する。

本発明の重合マトリックスは、微小粒子など注射に適するいずれかの形に成形することができる。微小粒子は、球状(即ちほぼ丸い)、非球状、又は不規則な形を持ち得る。しかし、好ましい微小粒子の形は球状である。好ましい微小粒子は平均約5から約200ミクロンの平均直径を持つ。

ポリマー状製剤の投与
本発明の製剤は、注射により動物に投与することが可能で、好ましくは哺乳動物、より好ましくはヒトである。投与に適する経路には、例えば、静脈内、腹腔内、脳内、筋肉内、眼内、硝子体内、動脈内、皮下、又は病巣内経路を含む。本発明の医薬製剤は、輸液又は大量静脈内投与又は特別な調製に適する何れかの注射方法により連続的に投与することができる。
注射に使用される針は、医薬製剤の所望投与量の注射を可能にする内径サイズを有していなければならない。通常、23ゲージ注射針はポリマーに基づく製剤の良好な注射可能性を提供するが、それより小さな(例えば、24、25、26、27、及び28ゲージ)注射針は、注射時の痛みを軽減するのに好ましい。
本発明に係る医薬製剤の投与量は、治療目的、投与経路、受容者の状態及び注射により投与し得る最大投与量などの様々な考慮すべき事項に依存する。従って、臨床医にとって、投与量を滴定し、最適な治療効果を得るのに必要とされる投与経路の修正を行うことが必要である。典型的な毎日の投与量は、体重あたり約1μg/kgから約100 mg/kgまでの範囲内又は一日にそれ以上が可能であるが、典型的には約10μg/kg/dayから約10 mg/kg/dayの間である。通常、臨床医は低投与量の医薬製剤から始めて、所望の治療効果が得られるまで投与量を増加させる。

実施例1
組換体ヒト成長ホルモンを含むポリマー状微粒子の調製
組換体ヒト成長ホルモン(rhGH)は、1991年発行のGombotz等、米国特許第5,019,400号に記述された極低温性、不含水処理により、PLGA微粒子中へカプセル化された。rhGHを安定化するために、当該タンパク質は、やや溶けにくいZn:rhGH複合体を生成させるように、酢酸亜鉛と共に製剤化された。組織学的証拠により、hGHは亜鉛との複合体として下垂体に貯蔵されることが示されたため、亜鉛が選択された。さらに、亜鉛とrhGHとの複合体は、複合体を形成しないrhGHよりもグアニジン塩酸による変性に対して耐性を示すことが知られていた。
11の異なる微粒子製剤が、初期の選定評価のために作成された。詳細に調べられる可変量は、ポリマー分子量(10kD、8kD、及び31kD)、ポリマーの末端基(キャップされているか又はされていないか)、及び賦形剤として添加される炭酸亜鉛の量(0,1,3,及び6%)であった。炭酸亜鉛賦形剤は、rhGHを亜鉛と複合体化させたままにするために亜鉛イオンに対する補給所として作用するために包含された。全ポリマー中のグリコリドに対するラクチドの分子比率は、50:50の一定値に保たれ、全微粒子製剤中の調べられたZn:rhGHの分子比率は、6:1であった。全ての微粒子製剤は、Birmingham Polymers (Birmingham, AL;#115−56−1;0.2dL/g, 10kD, ドデカニル末端基[即ちキャップされた状態])又はBoehringer Ingelheim (Ingelheim, Germany; RG502H;0.2dL/g, 8kD;及びRG503H;0.4dL/g, 31kD, 両方ともにカルボン酸及び酸末端基を持つ)から得られたD,L−PLAGを用いて調製された。

ポリマー懸濁液は、超音波処理ノズルを通して液体窒素が重層された凍結エタノールを含む容器中に分散された。その後、容器は−80℃へ移され、エタノールは溶解し微粒子は、ジクロロメタンがエタノールによって抽出されるにつれて硬化した。3日後、微粒子は濾過により回収され、減圧下で乾燥され、106μmのメッシュのスクリーンを通して濾過された。

実施例2
rhGH微粒子の分析
実施例1における11の微粒子製剤は、種々の技術を用いることにより分析された。微粒子は、アルミニウムのスタッブ(stub)又は炭素もしくは金の層でコートされたスパッタ(sputter)上に静置され、JEOL model 6400走査電子顕微鏡を用いて画像化された。微粒子の平均粒子直径分布は、Coulter Multisizerを用いて決定された。微粒子のrhGH積載量は窒素分析により決定された。
およそ95%のタンパク質は、微粒子中へカプセル化され、約50ミクロンの平均容量直径を有していた。
さらに、微粒子から回復されるrhGHの物理的及び生物的整合性は、サイズ排除(SEC)、逆相、及びアニオン交換クロマトグラフィー分析された。これらの技術は、凝集され、酸化され、脱アミド化されたrhGHを各々検出するために用いられた。これらの分析のために、2つの異なる方法をを用いて微粒子からrhGHが回復された。最初に、微粒子は、塩化メチレン及びアセトン中に溶解された後、タンパク質の沈殿物が回収された。次に、当該タンパク質は、微粒子をHEPESバッファー中でインキュベートすることにより回復された。

Peptide Protein Res. 35:17−24)。概略すると、SECは移動相としてリン酸バッファーを用い、G2000SW XL TSK Gel Columnを用いて実施した。rhGHは214 nmの紫外線吸収により検出された。逆相クロマトグラフィーは、ポリマー性逆相カラムにより、50℃にてアセトニトリル濃度勾配溶出を用いて行った。イオン交換クロマトグラフィーは、DEAE−SPW TSK Gel Columnによりリン酸及びアセトニトリル濃度勾配溶出を用いて実施した。逆相及びイオン交換法については、rhGHは、286 nmの励起波長及び335 nmの吸収波長の蛍光により検出された。最後に、rhGHの生物活性は、hGHに対するレセプターを発現し、hGHの存在下で増殖する株化細胞を用いた細胞増殖アッセイにおいて検出された(Roswall, EC.等、(1996)Biologicals 24:25−39を参照)。細胞増殖は、細胞内の還元酵素によるAlamar Blue色素から蛍光色素産物への変換により測定された。蛍光は、細胞数に比例し、rhGHを含む試料は37℃において72時間細胞とインキュベートされ、その後蛍光は消光させた。

注射媒体は、0.9%塩化ナトリウムUSP中でAmviscR Plus(Chiron Vision Corporation, Claremont, CA, USA)により希釈して調製された。
AmviscR Plusは、生理食塩水中で0.4mL又は0.8mLの1.6%ヒアルロン酸ナトリウム(w/v)を供する使い捨てのガラス注射筒中にて供給される。
各mLのAmviscR Plusは、注射用の滅菌水(SWFI USP)に溶解された16mgのヒアルロン酸ナトリウムと、9mgのNaClを含む。AmviscR Plusは、0.05%、0.1%、0.2%、0.4%、0.6%、及び0.8%(即ち、0.5、1、2、4、6及び8mg/ml)に希釈された。
100mg/mLの抗VEGF Fab PLGA微粒子(lot 96−22−195−1)を含む製剤を生産するために、注射媒体は、実施例1において記載されたように調製された抗VEGF Fab PLGA微粒子と混合された。
これらの製剤は、23,24,25,27及び28ゲージの注射針を用いて各1mLの注射媒体を管へ注入することにより、注射性能についてテストされた。各1mLの注射媒体は、テストされた全ての注射針を通過して容易に注射できた。
注射媒体として、3.0%(w/v)の直鎖状糖質カルボキシメチルセルロース(CMC)溶液を用いた同様のポリマーに基づく製剤は、100mg/mLの投与量を注射するのに21ゲージの注射針が必要であった。

実施例4
ヒアルロン酸ナトリウム(AmviscR Plus)を含む種々のポリマーに基づく製剤の注射性能研究
注射媒体は、0.9%塩化ナトリウムUSP中で、AmviscR Plus(Chiron Vision Corporation, Claremont, CA, USA)を0.05%、0.1%、0.15%及び0.2%(w/v)に希釈することにより調製された。
ポリマーに基づく製剤を生産するために、注射媒体はNutropin DepotR、VEGF微粒子、VEGF/ヘパリン微粒子、NGF微粒子と混合された。これらの微粒子調製品は、実施例1で記述されたように調製された。微粒子濃度は、表1に示される。
これらの製剤は、23,24,25,27及び28ゲージの注射針を用いて1mLの各製剤を管へ注入することにより注射性能に関してテストされた。
本研究結果は、表1に示されており、各製剤1mLを注射するのに使用される最小ゲージの注射針を示す。

実施例5
ヒアルロン酸ナトリウム(HylumedR)を含む種々のポリマーに基づく製剤の注射性能研究
ヒアルロン酸ナトリウムを含む注射媒体は、0.9%塩化ナトリウムUSP中で、HylumedR(Genzyme pharmaceuticals, Cambridge, MA, USA)を0.01%及び0.10%に希釈することにより調製された。これに比して、0.9%塩化ナトリウムUSP中で、3%(w/v)のカルボキシメチルセルロース(CMC)を含む第二の製剤も調製された。
注射媒体は、表2に示される微粒子調製品と混合され、実施例1に記述されているように調製された。微粒子濃度は、表2に示される。
これらの製剤は、21,22,23,24,25,26及び27ゲージの注射針を用いて1mLの各製剤を管へ注入することにより注射性能に関してテストされた。本研究結果は表2及び表3に示され、これは各製剤を注射するために使用される最小ゲージの針を示す。表3は、微粒子濃度が0.10%HylumedRを含む製剤において2倍にになると、小(24G)ゲージの注射針を注射に用いることができる。従って、より小さいゲージの注射針は、通常のCMCに基づく注射媒体と同量の1回当たりの投与量を、2回注射することが可能である。

実施例6
ポリマーに基づく微粒子のための注射媒体の使用に対する種々の高分子の適正
0.9%塩化ナトリウム USP中で以下に示す高分子溶液が、ポリマーに基づく微粒子のための注射媒体として調製された:アルギン酸ナトリウム(Kelco Co., San Diego, CA, USA)、アグリカン(aggrecan)(Sigma Chemical Co., St. Louis, MO, USA)、デキストラン 70(Sigma Chemical Co., St. Louis, MO, USA)、ジェファミンM−600(Hampton Res., Laguna Niguel, CA, USA)、ジェファミン ED−2001(Hampton Res., Laguna Niguel, CA, USA)、ケレタン硫酸塩(Sigma Chemical Co., St. Louis, MO, USA)、ラミニン(Sigma Chemical Co., St. Louis, MO, USA)、ポリ−L−オルニチン(Sigma Chemical Co., St. Louis, MO, USA)、キサンタンガム(Sigma Chemical Co., St. Louis, MO, USA)及びゲランガム(Sigma Chemical Co., St. Louis, MO, USA)。
テストされた濃度は、0.01%−0.1%(w/v)の範囲であった。Nutropin DepotR微粒子は、実施例1に記述されたように調製され、最終濃度125mg/mLになるように添加された。
これらの製剤は、21,22,23,24,25,26及び27ゲージの注射針を用いて1mLの各製剤を管へ注入することにより注射性能に関してテストされた。本研究結果は、21G注射針が、1mLの各製剤を注射するのに使用され得る最小ゲージの注射針であることを示すものであった。従って、テストされた媒体の中には、従来のCMCに基づく媒体よりも優れたものは存在しなかった。

実施例7
抗−VEGF Fabをヒアルロン酸ナトリウム中に含むポリマーに基づく製剤の硝子体内への注射性能研究
2つの注射媒体が、第一にAmviscR Plus(Chiron Vision Corporation, Claremont, CA, USA)、第二にシグマケミカル社から(St. Louis, MO)より入手したヒアルロン酸ナトリウムから調製された。両製剤は、0.9%塩化ナトリウム USP中0.1%(w/v)のヒアルロン酸ナトリウムに希釈された。
ポリマーに基づく製剤を生産するために、各注射媒体は抗−VEGF Fab微粒子(100mg/mL)と混合された。微粒子は実施例1に記述されたように調製された。
これらの製剤は、ウサギの眼の硝子体内への注射性能についてテストされた。50μLの各製剤は、30ゲージの注射針を用いて容易に注射可能であった。注射24時間後においてウサギの眼には重大な炎症は観察されなかった。

Claims (20)
- ヒアルロン酸又はその誘導体を約0.01から約0.8パーセントの容量濃度で含む注射媒体。
- ヒアルロン酸を含んでなる請求項1に記載の注射媒体。
- ヒアルロン酸又はその誘導体が生理食塩中に溶解されている請求項1に記載の注射媒体。
- (a)粒状形態の、又は粒子上にコートされ、粒子内部に分散され、又は粒子に付着された有効量の生物学的活性薬剤と;
(b)ヒアルロン酸又はその誘導体を含む注射媒体とを含む医薬製剤。 - 生物学的活性薬剤が、生体適合性重合マトリックスを含む粒子内に分散された請求項4に記載の医薬製剤。
- ヒアルロン酸を含んでなる請求項5に記載の医薬製剤。
- ヒアルロン酸濃度が約0.01から約0.8パーセント(w/v)の間である請求項6に記載の医薬製剤。
- ヒアルロン酸が生理食塩水に溶解されている請求項6に記載の医薬製剤。
- 重合マトリックスが、生物分解性ポリマー、非−生物分解性ポリマー、生物分解性ポリマーと非−生物分解性ポリマーの混合物、及び生物分解性及び非−生物分解性単位を含む共重合体からなる群から選択されるポリマーを含む、請求項6に記載の医薬製剤。
- 重合マトリックスがブロック化ポリマー、非ブロック化ポリマー及びその混合物からなる群から選択されるポリマーを含む、請求項6の医薬製剤。
- 重合マトリックスが、ポリ(グリコリド);ポリ(ラクチド−グリコリド共重合);ポリ(乳酸);ポリ(グリコール酸);ポリ(乳酸−グリコール酸共重合);重合無水物;ポリオルトエステル;ポリエーテルエステル;ポリカプロラクトン;ポリエステルアミド;ポリエチレングリコール及びラクチド又はグリコリドのブロック共重合体;及びこれらの混合物又は共重合体からなる群から選択されるポリマーを含む、請求項6に記載の医薬製剤。
- ポリマーがポリ(ラクチド−グリコリド共重合)ポリマーである請求項11に記載の医薬製剤。
- 生物学的活性薬剤がポリペプチドである請求項6に記載の医薬製剤。
- ポリペプチドが成長ホルモン、肝細胞成長因子(HGF)、血管内皮成長因子(VEGF)、抗−血管内皮成長因子Fab(抗−VEGF Fab)、グルカゴン様ペプチドI(GLP−I)、神経成長因子、及びインシュリン様成長因子からなる群から選択される、請求項11に記載の医薬製剤。
- 生物学的活性薬剤を含む重合マトリックス濃度が製剤の少なくとも約1mg/mLから約500mg/mLの間である請求項6に記載の医薬製剤。
- 生物学的活性薬剤を含む重合マトリックス濃度が製剤の少なくとも約1mg/mLから約300mg/mLの間である請求項15に記載の医薬製剤。
- (a)ポリ(ラクチド−グリコリド共重合)ポリマーを含む生体適合性重合マトリックスを包含する粒子と;
(b)重合マトリックス内に分散された有効量の生物学的活性ポリペプチドと;
(c)ヒアルロン酸又はその誘導体を含む注射媒体とを含有する医薬製剤。 - 粒状形態の、又は粒子上にコートされ、粒子内部に分散され、又は粒子に付着された有効量の生物学的活性薬剤を、ヒアルロン酸又はその誘導体を含む注射媒体に添加することを含んでなる医薬製剤を製造する方法。
- 注射媒体がヒアルロン酸を含む請求項18に記載の方法。
- 23−ゲージ又はそれより小さい注射針を通して医薬製剤を注射することを含む、請求項17の医薬製剤を投与する方法。
