Abstract

(57)【要約】 本発明は、脚が大腿部で切断された患者のエキソプロテーゼ治療用の大腿断端エンドプロテーゼに関するものであり、本発明によれば、大腿断端は、プロテーゼの下脚及び足部の模擬体が接続された幹中に引き込むことができる。本発明のプロテーゼは、大腿骨断端の内部に挿入することができる近位幹部分を具有し、該幹部分は粗目の立体網様構造によって少なくとも部分的に覆われ、かつ本発明のプロテーゼはその遠端側に円錐形アダプタを具有する。膝関節の自然関節丘の形を模した代用関節丘を該アダプタによって該幹部分に結合することができる。くさび形の支持体を脚の外反を再現するくさび角度αで該代用関節丘に結合することができる。

Description

【発明の詳細な説明】
【０００１】 本発明は、脚が大腿部で切断された患者のエキソプロテーゼ用の大腿断端エン
ドプロテーゼに関するものである。 大腿断端は、プロテーゼの膝、下脚及び足部の模擬体が接続された幹中に引き
込むことができる。エンドプロテーゼは、大腿断端中に据え付けることができる
近位側ポスト部を有し、該ポスト部は少なくとも部分的に粗目の立体網様構造に
よって覆われており、かつエンドプロテーゼの遠端側には円錐形アダプタが設け
られ、このアダプタによって膝関節の自然関節丘の形に似せた代用関節丘がポス
ト部に取り付けられる。
【０００２】 本発明の目的は、患者により強い知覚的感覚を与えることにある。
【０００３】 この目的は、特許請求の範囲の請求項１記載の発明を構成する手段によって達
成される。
【０００４】 本発明によれば、くさび形の支持体が代用関節丘に結合され、該支持体は自然
な外反、すなわち内側方向の大腿断端の角度を再現する。 これは、力の分布が本来の健全な脚の力の分布と同様に感じられるように作用
する。 このことは、くさびがくさび形の底辺と人工膝関節の滑らかな表面との間に平
行関係を作り出して、大いに自然なＣＣＤ角度を維持することができるというこ
とを意味する。これによって、プロテーゼの軸方向沿いの応力が均等化される。
【０００５】 例えば、外反角度の補正が可能なくさび形等化器として設計されたインプラン
トは、外科分野より周知である（米国特許第５，７６６，２５１号）。 このくさび形のインプラントはとりわけ脛骨の関節部で使用され、これによっ
て脛骨プラトーはくさび形のインプラントの傾斜角度に従って一方側が持ち上げ
られる。しかしながら、同米国特許公報には、インプラントを脚が切断された患
者に使用することを示唆する記載はない。
【０００６】 本発明のもう一つの実施例においては、くさび角は５°乃至と９°の角度範囲
内にある。５°の角度は好ましくは男性患者に使用され、９°は女性患者で最も
効果的に使用される。
【０００７】 支持体は、好ましくは大腿断端エンドプロテーゼの衝撃吸収層に結合される。
【０００８】 支持体は、応力が直接大腿断端に導入されないようにするために、衝撃吸収用
クッションで作ることが特に好ましい。このために、支持体は好ましくはシリコ
ーンで製造される。
【０００９】 以下、本発明を、実施例を用いて図面を参照しつつさらに詳細に説明する。
【００１０】 添付図面の図は、大腿断端エンドプロテーゼ５が埋め込まれた大腿断端１の概
略図である。
【００１１】 骨髄内ポスト４は、ポストの近位部を形成し、該近位部は大腿断端の髄腔中に
セットされている。ポスト部４の表面は粗目の立体網様構造６を有し、この網様
構造を通して骨小柱が成長することができ、その結果基質の流れに関連する一定
の癒合段階後に、ポスト部４が自然骨のほぼ一体の部分になるようになっている
。
【００１２】 ポスト部４の遠位側では、円錐形アダプタ７がポスト部４に取り付けられてい
る。この円錐形アダプタ７は、ポスト部４の遠位側に代用関節丘１１を取り付け
るために使用される。
【００１３】 代用関節丘部８は、膝関節の自然関節丘の形状１１に似せて設計される。
【００１４】 この実施例においては、代用関節丘８はシリコーンの衝撃吸収層１０で被覆さ
れている。
【００１５】 支持体１６は、代用関節丘８及びこれに被覆された衝撃吸収層に結合されてい
る。支持体１６は、くさび形をなすように設計され、水平に対して５°＜α＜９
°の範囲のくさび角を有する。
【００１６】 図示実施例においては、支持体１６は緩衝クッションで構成されており、その
結果膝、下脚及び足の部分３に作用する衝撃応力の吸収が促進される。 支持体１６のくさび角度αは個々の患者に合わせて調整されるので、支持体１
６の下面１７は、装着された人工膝関節の人工関節丘の摺動軌道の平面と基本的
に平行になる。
【００１７】 その結果、応力分布がプロテーゼ全体にわたって大いに均等化される。

Claims (5)

    【特許請求の範囲】
  1. 【請求項１】 脚が大腿部で切断された患者のエキソプロテーゼ用の大腿断
    端エンドプロテーゼにおいて、大腿断端（１）がプロテーゼの人工膝、下脚及び
    足の部分（３）が結合された幹（２）に引き込まれ、大腿骨断端（５）中にセッ
    トすることができる近位ステム部（４）を有し、該ステム部分（４）が粗目の立
    体状格子（６）によって少なくとも部分的に覆われ、かつ遠端側に円錐形アダプ
    タ（７）を有し、該アダプタによって、膝関節の自然関節丘の形（１１）を模擬
    した代用関節丘（８）が該代用関節丘（８）に結合されたくさび形底部（１６）
    を持つステム部（４）に結合されることを特徴とする大腿断端エンドプロテーゼ
    。
  2. 【請求項２】 上記底部（１６）のくさび角度αが５°＜α＜９°の範囲内
    であることを特徴とする請求項１記載の大腿断端エンドプロテーゼ。
  3. 【請求項３】 上記底部（１６）が衝撃吸収層（１０）と結合されているこ
    とを特徴とする請求項１または２のいずれか１項記載の大腿断端エンドプロテー
    ゼ。
  4. 【請求項４】 上記底部（１６）が衝撃吸収用クッションで形成されている
    ことを特徴とする請求項１乃至３のいずれか１項記載の大腿断端エンドプロテー
    ゼ。
  5. 【請求項５】 上記底部（１６）がシリコーンで形成したことを特徴とする
    請求項４記載の大腿断端エンドプロテーゼ。
