Description
Foreliggende oppfinnelse vedrører protesesett for personer med amputerte lemmer/stumper hvor protesesettet består av en forankringsdel anordnet innvendig i et gjenværende margbein og en utvendig protese, idet forankringsdelen har en boltlignende utforming med en stamme og et hode i sin ene ende, idet stammen er innførbar og forankringsbar i det gjenværende margbein og hodet danner en subkutan avslutning av margbeinet.
Tradisjonelt blir eksogene proteser for eksempel for overarmsamputerte festet til en person ved hjelp av vakuum eller seletøy innbefattende stropper etc. som festes rundt personens overkropp d.v.s. nakke og skuldre. Slike proteser er således meget klumpete og tungvinte i bruk, ubekvemme og gir begrensede be-vegelsesmuligheter for personen som anvender disse. Dårlig forankring av protesehylsen reduserer også bevegeligheten og følelsen med protesen. Protesebrukeren opplever ofte slitasjeproblemer, rygg- og nakkeproblemer, problemer med svette, strangulering og lignende. Et annet problem med slike proteser er at trykk-kreftene fra protesen mot den amputerte beinenden kan gi store smerter, og i verste fall kan beinstumpen trenge gjennom huden i berøringspunktet med protesen.
DE-Ai 3 125 268 løser problemet med å oppta trykkreftene som beskrevet ovenfor idet en bolt settes inn i enden av beinet for å kunne oppta trykkreftene og fungerer som en "støtpute". Denne publikasjonen bidrar imidlertid ikke til å bedre forankringen av en eksogen protese.
DE- hi 4 338 746 beskriver et benimplantat beregnet på å bli innoperert i den gjenværende ende av en amputert kroppsdel, og består av en forankringsdel som har en boltningene utforming med en stamme som beregnet for å anordnes innvendig i et gjenværende margben. I den distale enden er forankringsdelen utstyrt med et utvidet hode som danner avslutningen av den amputerte stumpen. Den utvidede del er utstyrt med et spor for opptak av sener, og hull for innvekst av kroppsvev. Implantatet er beregnet for å benyttes sammen med en tradisjonell hylsetilpasning, hvilket medfører at man er avhengig av å foreta gipsavstøpning og modellere gipsen for at man skal kunne produsere protesehylsen.
Et mål med den foreliggende oppfinnelse er å tilveiebringe et protesesett for personer med amputerte lemmer/stumper hvor protesehylsen haren bedre forankring til det amputerte lem eller stump slik at protesefunksjonen forbedres.
Et annet mål er at protesen skal ha en enkel innfesting lokalt til den amputerte stumpen, og at protesen kan forankres distalt på stumpen. En slik forankring av protesen vil øke antall frihetsgrader d.v.s. at bevegeligheten vil øke.
Et tredje mål erat protesebrukeren skal kunne endebelaste protesen samt overføre torsjonskrefter fra stumpen til protesen.
Et fjerde mål er at protesesettet skal kunne benyttes for alle amputerte lemmer for eksempel humurus, femur, tibia, fibula, ulna, radius etc.
Et femte mål med oppfinnelsen er at det skal være mulig å forlenge den avkuttede stumpen for å oppnå en mest mulig ideell stumplengde for feste av en utvendig protese.
Målene med oppfinnelsen oppnås ved et protesesett for personer med amputerte lemmer/stumper ifølge innledningen av beskrivelsen og som er kjenneteg-net ved at forankringsdelens hode har en bredde som overstiger bredden av stumpen slik at det oppstår en lokal utvidelse/vulst ved det amputerte lemmets avslutning og at den utvendige protese har en innvendig utføring tilpasset den lokale utvidelse hvorved en lokal forankring, opphenging og endebæring av protesehylsen er tilveiebrakt.
Foretrukne utførelser av protesesettet er videre utdypet i kravene 2-9.

Med henvisning til figurene ovenfor skal det først bemerkes at de samme referansenummer er benyttet for å angi tilsvarende deler ut gjennom de forskjellige utførelsesformer av oppfinnelsen. Fig. 1 viser en person med en amputert overarm 10, og fig. 2 viser personen med en tradisjonell eksogen protese. Som fig. 2 viser omslutter protesen hele overarmen, deler av skulderen og er videre festet ved hjelp av seletøy rundt nak-ken og skuldre. Protesen er stor og klumpete og med en festeanordning som reduserer personens bevegelighet og følelse med protesen. I tillegg er det åpenbart at brukeren vil kunne få problemer med slitasje, svetting o.s.v. Fig. 3 viser et første trinn i en fremgangsmåte for å implantere en forankringsdel 2 tilhørende et protesesett 1 ifølge foreliggende oppfinnelse. Et kirurgisk inngrep 13 blir gjort i enden av den amputerte overarm 10 og den amputerte enden av margbeinet (overarmsbeinet) 11 blir avdekket. Forankringsdelen 2 med en stamme 3 og et utvidet hode 4 ifølge fig. 4 blir innsatt i overarmsbeinet eller margbeinet 11. Før innsetting av forankringsdelen 4 vil det innvendige av margbeinet 11 bores opp og tilpasses forankringsdelen 4 som igjen blir sementert innvendig i margbeinet 11. Forankringsdelens stamme 3 vil være anordnet med ett eller flere langsgående spor 6 fortrinnsvis for innføring av sementmasse. Fig. 6 viser den amputerte overarmen 10 med innsatt forankringsdel 2. Som det fremgår av denne figuren vil det utvidede hodet 4 til forankringsdelen 2 medføre en lokal utvidelse 12 ved det amputerte lemmets avslutning 10. Denne utvidelse 12 vil tilveiebringe en meget god forankring for en utvendig protese 7 ifølge den foreliggende oppfinnelse. Fig. 7 viser skjematisk forankringsdelen 2 innsatt og sementert innvendig i margbeinet 11 og illustrerer på en tydelig måte at det utvidede hodet 4 danner en subkutan avslutning av margbeinet 11 og en lokal utvidelse 12 ved det amputerte lemmets avslutning. Forankringsdelens hode 4 er her plateformet med en svak krumning. Fig. 8 viser en andre utførelse av forankringsdelen 2 nå med en lokal utvidelse 5 mot det utvidede hodet 4. Denne andre utførelse av forankringsdelen 2 anvendes i de tilfeller hvor det er nødvendig med en forlengelse av margbeinet 11 for å oppnå en bedre fiksering av den utvendige protesen 7. Dette vil for eksempel være i tilfelle hvor overarmen 10 er amputert høyt oppe mot armhulen. Fig. 9 viser den utvendige protesen 7 som en del av protesesettet 1. Den utvendige protesen 7 er tilpasset den amputerte overarmen 10 og spesielt den lokale utvidelse 12 som er et resultat av den implanterte forankringsdelen 2. Fig. 10 viser det komplette protesesettet 1 forfra og fig. 11 viser det komplette protesesettet bakfra. Som det fremgår av fig. 10 og 11 er den utvendige protese 7 vesentlig mindre og således lettere og mer brukervennlig enn den tradisjonelle protesen vist i fig. 2. Den lokale innfesting av den utvendige protesen 7 bidrar også til økt bevegelsesfrihet og også økt komfort for brukeren. Fig. 12 viser protesesettet 1 med påmontert opererbar kunstig underarm 9. Fig. 13 viser en tredje utførelse av forankringsdelen 2 anordnet i et amputert lårbein. Som det fremgår av figuren har spesielt hodet 4 en helt annen utforming og er konstruert for å oppta større trykkrefter på grunn av den store støtbe-lastningen som en slik beinprotese medfører. Fig. 14 viseren utvendig protese 7 for anvendelse i forbindelse med det amputerte lårbeinet innbefattende implantatet ifølge foreliggende oppfinnelse. Fig. 15 viser det komplette protesesettet 1 for en lårbeinsamputert person. Som det fremgår av fig. 15 oppnås det en god forankring gjennom den lokale utvidelse 12 og den tilpassede innvendige utformingen 8 i den utvendige protesen 7. En god "støtpute" oppnås videre gjennom utformingen av forankringsdelens utvidede hode 4. Fig. 16 viser en fjerde utførelse av forankringsdelen 2. Forankringsdelens hode har nå en T-form med svak krumning. Fig. 17 viser en femte utførelse av forankringsdelen 2. Forankringsdelens stamme 3 har nå en utvidelse 5 nærmest hodet 4. Denne utvidelsen 5 fungerer i dette tilfelle som en forlengelse av margbeinet 11 som omtalt i forbindelse med fig. 8, og ville i dette tilfelle egne seg for en overarmsamputert person med kort armstump. Forlengelsen av overarmsbeinet 11 vil gi bedre feste for den utvendige protese 7 og således øke protesefunksjon for personen.
Det skal nevnes at det kirurgiske inngrepet vil bli foretatt av en ortoped eller kirurg. Implantatet eller forankringsdelen 2 festes til margbeinet (rørknokkelen) 11 ved sementering, og dette er en teknologi som er kjent i applikasjoner som hofte-leddsproteser, kneleddsproteser og lignende. Implanteringen av forankringsdelen (kondylen) skjer i et inngrep, og således vil restitueres i løpet av 3-4 uker. Dette medfører at pasienten kan få full uttelling av protesesettet 1 allerede etter 4-6 uker etter inngrepet. Forankringskondylen vil bli produsert i titan, et materiale som kroppen ikke frastøter. Kirurgisk stål er også et annet materiale som kan benyttes. Implantatet vil befinne seg i et lukket miljø, subkutan, noe som i stor grad reduserer faren for infeksjoner og kuldegjennomslag.
Claims (9)
1. Protesesett (1) for personer med amputerte lemmer/stumper (10) hvor protesesettet består av en forankringsdel (2) anordnet innvendig i et gjenværende margbein (11) og en utvendig protese (7), idet forankringsdelen (2) har en boltlignende utforming med en stamme (3) og et hode (4) i sin ene ende, idet stammen (3) er innførbar og forankringsbar i det gjenværende margbein (11) og hodet (4) danner en subkutan avslutning av margbeinet (11),
karakterisert ved at forankringsdelens hode (4) har en bredde som overstiger bredden av stumpen (10) slik at det oppstår en lokal utvidelse/vulst (12) ved det amputerte lemmets avslutning (10) og at den utvendige protese (7) har en innvendig utføring (8) tilpasset den lokale utvidelse (12) hvorved en lokal forankring, opphenging og endebæring av protesehylsen er tilveiebrakt.
