可能とし、これは損傷した動脈内皮の治療と補修を改良することができる。

剛性でなければならない。超弾性NiTi合金を使用する場合、圧縮応力が除去
された時に拡張が生じて、オーステナイトからマルテンサイトへの相転移が生じ
、その結果ステントの拡張を生じる。

法を含む。

ステントは比較的可撓性である。
構造20は、ステントの展開中に拡張力が加えられた時に周方向および縦方向に
可撓的に変形する湾曲したバネ状セグメントを生じるように相互に協働する。U
型構造の周方向変形が各円筒形要素18の放射方向拡張を生じ、ステントの各部
分を第1直径から拡大された第2直径まで拡張させる。

分遠位端部分14、16および中央部分12を協働させる事により、対応の末端
部分が完全に拡張する前にステントの中央部分を所望の最終直径まで拡張させて
血管の壁体の内側と接触させることができる。

い弾性係数を有する素材から作られたU型構造は、より低い弾性係数を有する素
材から作られた類似のU型構造よりも周方向変形抵抗が大であり、同様により大
きな放射方向抵抗を有する円筒形要素を形成するために使用することができる。

央部分分53の円筒形要素54のU型構造56よりも実質的に幅広い断面のU型
構造56’によって形成された少なくとも1つの円筒形要素54’によって形成
されている。前述のように、幅広い断面を有するU型構造はこれより狭い断面を
有するU型構造よりも周方向変形抵抗が大である。従ってステントの展開に際し
て放射方向拡張力が加えられる際に、ステントの近位端部分と遠位端部分の中に
含まれる円筒形要素は中央部分に含まれる円筒形要素よりも大きな放射方向拡張
抵抗を示す。その結果ステントの中央部分は、それぞれ近位端部分と遠位端部分
が完全に展開する前に、その圧縮された第1直径から拡大された第2直径まで拡
張して血管の壁体の内側面と接触するであろう。
遠位端部分は、中央部分の円筒形要素のU型構造より短い軸方向長さと幅広い断
面とを有するU型構造によって形成された少なくとも1つの円筒形要素によって
形成される。前記の各実施態様の場合と同様に、ステントの展開中に放射方向拡
張力を加えられる際に、近位端部分と遠位端部分の中の各円筒形要素は中央部分
の中の円筒形要素よりも大きな放射方向拡張抵抗を有するので、中央部分は末端
部分より先に拡張する。
より低い弾性係数を有する素材を有する同様形状の円筒形要素よりも高い弾性係
数を有する素材によって形成された円筒形要素からなる場合も考えられる。これ
らの相異なる素材からなる隣接の各ステント部分の円筒形要素は統一的ステント
本体をなすように相互に機械的に連結される。従って、より高い弾性係数を有す
る円筒形要素からなるステント部分は、これより低い弾性係数の素材からなる円
筒形要素によって構成されたそれぞれの部分よりも放射方向拡張抵抗が大である
ことが理解されよう。その結果ステント展開中に拡張力を加える際に、ステント
の中央部分は、それぞれ隣接の近位端部分と遠位端部分が完全に展開する前に、
圧縮された第1直径から拡大した第2直径まで拡張して血管の壁体の内側と接触
する。
ステントの軸方向長さにそって配列されたそれぞれのステント部分の放射方向拡
張抵抗を示す。大きなブロックは大きな放射方向拡張抵抗を有するステント部分
を示す。同様に小さなブロックは比較的小さい放射方向拡張抵抗を有するステン
ト部分を示す。図8について述べれば、このような実施態様の1つのステント構
造においては、それぞれの円筒形要素をなすU型構造の幅および/または軸方向
長さがステントの中央部分からステントの両端部分に向かって放射方向に変化さ
せられている。すなわち放射方向拡張抵抗は図8に図示のようにステントの近位
端部分と遠位端部分60と62において中央部分64と比較して実質的に増大し
ている。他の実施態様において、図9に図示のように、U型構造の幅および/ま
たは軸方向長さが中央部分74の隣接の円筒形要素から近位端部分および遠位端
部分70、72の円筒形要素に向かって徐々に変更され、従って放射方向拡張抵
抗もステント76の中央部分から各末端部分に向かって徐々に増大する。

れるようなアイオノマーなどの適当材料で形成することができる。他のポリマー
も使用することができる。動脈15内部の損傷箇所に対してステントの送入中に
ステントをバルーンの上に定置するため、ステントはバルーンの上に強く締め付
けられる。ステントが送入カテーテルの拡張性部分の上に確実に留まるようにし
、また所望動脈部位の中にステントを送入する際にその開放面による内腔の磨耗
を防止するため、引き込み式の送入スリーブ46を備えることができる。またバ
ルーンの作動部分、すなわち円筒形部分の両端にカラーまたはリッジを備えるな
ど、ステントをバルーンの上に保持する他の手段を使用することもできる。
まずステントが送入カテーテル38の遠位端において拡張性バルーンの上に取り
付けられる。低輪郭を保証するためステントをバルーン上に「締め付ける」こと
ができる。このカテーテル−ステント組立体が通常のセルディンガー技術によっ
て案内カテーテル(図示されていない)を患者の血管の中に導入される。次に案
内ワイヤ48が離脱または切断された血管内皮44を有する損傷動脈部分を横切
って配置され、次にステントが離脱した内皮のすぐ下に来るまでカテーテル−ス
テント組立体が案内ワイヤにそって前進させられる。カテーテルのバルーンが公
知のようにして拡張させられて、ステントを動脈に当接させて拡張させる。この
状態は図2に図示されている。図示されてはいないが、ステントが移植後に運動
することを防止するため、ステントの拡張によって動脈が少し拡張させられて、
ステントを動脈壁体の中に埋め込ませる。いくつかの場合には、動脈の狭窄部分
の治療中に、血流を容易にするために動脈を著しく拡張させなければならない。
央部分の回りにステント10によって加えられる放射方向拡張抵抗の増大の故に
、送入カテーテル38のバルーン40がその長さにそって不均等に拡張する傾向
がある。この場合、バルーンはまずその近位端39と遠位端41の最小抵抗路に
そって拡張して、ステントの両端に円環形ローブ43をなし「ドッグボーン」型
を成す。バルーンの両端が過度に拡張して「ドッグボーン」型をなすに従って、
バルーンから放射方向外側に作用する力がステント構造と干渉する。U型構造2
0’によって形成される各ステント末端部分14、16の円筒形要素18’は、
ステントの対応の中央部分12の円筒形要素18の中のU型構造20よりも軸方
向長さに関してはるかに短く、これらの円筒形要素18’には中央部分における
放射方向拡張抵抗よりも大きな放射方向拡張抵抗を有する末端部分が生じる。従
って、その下にあるバルーン40が拡張する事によってステントの中央部分を対
応の末端部分の少し前にまたは同時に拡張させる。その結果、ステントの中央部
分はステントの両端部分が完全に拡張する前にその第1直径から拡大した第2直
径まで拡張して血管の壁体と接触し、このようにしてステントに対する損害を防
止しまたは血管の壁体に対する損傷を防止しまたステントの適正な移植を保証す
る。

則的な間隔で密接に配置された円筒形要素は動脈の壁体を均等に支承し、従って
図2と図3に示すように動脈の壁体の小さなフラップまたは切断片を留めて定位
置に保持するのに役立つ。
各円筒形要素のピーク26と谷間28との間に均等に分布される。図5に図示の
ように、ステント10の円筒形要素18の一部は拡張力の均等な分布を助長する
ための複数のピークと谷間を有する不規則な蛇行模様を示す。前述のように、連
結部材30が円筒形要素の隣接する谷間を連結するために使用される。拡張中に
、連結部材の連結された谷間区域中の部分29が変形に際して最も強固な構造で
あり、ピーク部分が最も強固でない部分である。すなわち、部分29の曲率半径
をより大ききくすることにより、ピーク部分26の拡張と比較して、部分29が
より早期にかつより均等な速度で拡張し始める。
最も低い剛性の区域であり、これは拡張中に加えられる種々の応力を考慮するも
のである。また最も低い剛性の構造のピーク部分26は、この部分より剛性の高
い構造である部分29と谷間部分28との間に配置される。応力の均等化のため
、ピーク部分26はその区域23と25において相異なる曲率半径を有する。区
域23は区域25より大きな曲率半径を有し、一層容易に拡張する。区域25は
部分29の剛性の高い区域に隣接しているので、区域25と部分29は共に均等
に拡張し、拡張応力をより均等に分布させる。さらに、谷間部分28と29はピ
ーク部分26よりも均等に拡張し、また拡張応力をより均等に分布させる。前記
の新規な構造の故に、金属の面外ねじれを含む先行技術の問題点が解決される。
ピーク部分26にそったこのような相異なる曲率半径が円筒形要素18の全体と
しての均等な拡張を可能とする。さらに拡張中の相異なる応力レベルを補正する
ために谷間部分28は相異なる曲率の度合いを有することができる。拡張後に、
各要素のそれぞれの部分が反転して小さな突起を生じ、これらの突起が血管の壁
体中に埋込まれる。例えば、拡張に際して、ピーク部分26の先端が、血管の壁
体中に埋込まれて移植されたステントを固着するのに十分な程度に外側に突出す
る。拡張に際して、突起ピーク26が形成するステントの外側壁面は平滑でなく
、この外側壁面全体に沿って配置された複数の突出ピーク26を備える。

もはるかに大きな直径まで拡張することができ、しかもなおこの拡張状態におい
て構造一体性を保持することにある。本発明のステントはその構造の故に、使用
されるステンレス鋼の組成に対応して1乃至約4.0の範囲内の拡張比を有する
。例えば本発明の316Lステンレス鋼からなるステントは1単位の直径から約
4.0単位の直径まで放射方向に拡張することができ、このようにして構造部材
をその弾性限度を越えて変形させる。このステントはその拡張状態においてもそ
の構造一体性を保持し、このステントの移植された血管部分を開放状態に保持す
ることができる。ステンレス鋼(316L)以外の材料はそれぞれ構造一体性を
損なう事なくこれより高いまたは低い拡張比を与えることができる。
合金チューブから形成されるが、本発明のステントは例えばタンタルNiTiな
どの他の生体適合性および金属合金、または熱可塑性ポリマーから製造すること
ができる。さらに、本発明のステント構造は生体適合性コーティングによって被
覆することができる。現在のステントの好ましい製造法は例えばMETHOD AND APP
ARATUS FOR DIRECT LASER CUTTING OF METAL SHTENT と題する同一譲渡米国特許
第号第5,759,192号に記載ののようにステンレス鋼管ををレーザ切断する方法で
ある。本発明のステントの他の製造方法も考慮されまた業界公知である。
たのであるが当技術者には明らかなように、ステントは人体のすべての輸送管ま
たは需要器官においても使用することができる。本発明のステント構造はバルー
ンカテーテル装置による展開中にステントの軸方向長さにそって放射方向拡張度
を制御する新規な特徴的構成を有するのであるから、このステントはこの装置の
使用されるほとんどすべての器官の中の移植に好適である。また本発明のステン
ト構造はその構造一体性を保持しながら非常に大きな直径まで拡張することがで
きる。この特徴的構成は、ステントが放射方向に拡張した時の縦方向収縮が限定
されている特徴と相まって、人体のすべての器官に対する非常に望ましい支承シ
ステムを提供する。本発明の主旨の範囲内においてその他の変更および改良を加
えることができる。

の可撓性ステントにおいて、 前記ステントは、 少なくとも1つの末端部分を持つ中央部分を有する細長い円筒形本体であって
、前記中央部分および少なくとも1つの末端部分はそれぞれ列を成して隣接配置
された複数の円筒形要素からなり、前記円筒形要素は複数のU型構造によって画
成され、これらのU型構造は、相互に連結されてステントの縦方向軸線に対して
横方向の全体として波形の蛇行パタンをなし、かつ、放射方向外向きに作用する

ステントの中央部分の円筒形要素のU型構造よりも、軸方向長さまたは周方向寸
法に関して十分に短くかつ幅広い横断面を有するU型構造によって形成され、こ
れにより、前記近位端部分および前記遠位端部分に前記中央部分より大きな放射
