Description
A találmány tárgya eljárás új, előnyösen a protein-kináz C enzim aktiválásának gátlására alkalmazható, idegrendszeri zavarok gyógykezelésére alkalmas gyógyászati készítmények előállítására. Ezek az új gyógyászati készítmények hatóanyagként ismert módon előállított N-(diklór-acetil)-szfingozint vagy N-(diklóracetilj-dihidroszfmgozint tartalmaznak, a gyógyszerkészítésben szokásos vivőanyag, hígítószer és/vagy egyéb gyógyszerészeti segédanyag kíséretében.
A találmány szerinti gyógyászati készítményekben hatóanyagként alkalmazott N-(diklór-acetiI)-szfingozin vagy N-(diklór-acetil)-dihidroszfingozin önmagukban ismert vegyületek, amelyeket és előállítási módjukat a Biochemistry 21, 928 (1982) és a J. Org. Chem. 28, 2157 (1963) közlemények ismertették; ezek a közlemények azonban nem tartalmaztak utalást e vegyületek farmakológiai tulajdonságaira vagy gyógyászati alkalmazásuk lehetőségére, és tudomásunk szerint azóta sem merült fel adat a két említett vegyület gyógyászati alkalmazhatóságára vonatkozólag.
A találmány szerinti új gyógyászati készítmények önmagukban ismert hatóanyagai, az N-(diklór-acetil)szfingozin és N-(diklór-acetil)-dihidroszfingozin az idézett korábbi irodalomban leírt módon, úgy állíthatók elő, hogy az ismert szfingozint vagy dihidroszfingozint diklór-ecetsavval vagy annak valamely reakcióképes származékával előnyösen diklór-acetil-kloriddal N-acilezik; az eljárás kiviteli módját az alábbiakban egy példában is bemutatjuk.
A találmány értelmében a protein-kináz C enzim aktiválását gátló hatású, idegrendszeri zavarok gyógykezelésére alkalmas gyógyászati készítményeket oly módon állítjuk elő, hogy az ismert módon előállított N-(diklór-acetil-) szfingozint vagy N-diklór-acetil-dihidroszfingozint a gyógyszerkészítésnél szokásos vivőanyaggal, hígítószerrel és/vagy egyéb gyógyszerészeti segédanyaggal összekeverve, gyógyászati felhasználásra alkalmas készítménnyé alakítjuk. Az ilyen gyógyászati készítményeket különösen orális vagy parenterális (injekciós) beadásra alkalmas alakokban állítjuk elő, de előállíthatok egyéb úton, például transzdermálisan (bőrön át) történő beadásra alkalmas készítmények is.
A találmány szerint előállított új gyógyászati készítmények terápiás szempontból értékes hatásokat mutatnak; elsősorban gátolják a protein-kináz C enzim aktiválását, amely utóbbi a normális idegingerület-átviteli (neutrotranszmissziós) mechamizmusok egyensúlyára bizonyos körülmények között nemkívánt negatív hatást fejthet ki. A proteint-kináz C aktiválódása a serkentő (ingerlő) hatású aminosavak - így a glutaminsav és/vagy aszparginsav - megnövekedett koncetrációjának a következménye. Ilyen abnormális körülmények között e savak közvetlen toxikus hatást gyakorolnak az idegsejtekre (neuronokra). A jelen találmány szerinti gyógyászati készítmények egyik nagy előnye amely más protein-kináz C gátló hatóanyagoktól, például a szfingozinoktól és gangliozidoktól megkülönbözteti ezeket a készítményeket -, abban áll, hogy ezt a neurotoxikus hatást képesek megelőzni vagy kivédeni.
A találmány szerinti új készítményekben hatóanyagként alkalmazott vegyületek továbbá csak sokkal nagyobb dózisokban neurotoxikusak, mint amilyen dózisokban a protein-kináz C enzimet gátolják. Ezek a félszintetikus szfingolipidek különböző kóroktani eredetű idegrendszeri zavarok, például agyvérzés, trauma, vitustánc, aggkori idegrendszeri zavarok, epilepszia, az idegrendszer neurodegeneratív zavarai, valamint szívizom-infarktus és koszorúérszűkület kezelésére alkalmazhatók. Farmakológiai hatásaikat az alábbiakban az N-(diklór-acetil)-szfingozinnal végzett vizsgálatokkal szemléltetjük.
Az alábbiakban szemléltetjük a serkentő (ingerlő) hatású aminosavak neuronotrop hatásai elleni védőhatást.
Pirmer sejttenyészetekben a protein-kináz C (az alábbiakban rövidítve: PKC) aktiválása és transzlokációja (áthelyeződése) a serkentő aminosavak receptorainak stimulálásával kiváltható. A PKC transzlokációja úgy mérhető, hogy épp sejtekben megállapítjuk a 4-B[3H]-forbol-12,13-dibutirát kötési helyeit. Közelebbről: glutamát kiváltja a 4-B-[3H]-forbol-12,13-dibutirát kötési helyeinek dózisfüggő transzlokációját (áthelyeződését) a citoszolból a neuron membránjára. Ismert, hogy ha glutamátot adnak ezekhez a tenyészetekhez, akkor az sejtkárosodást idéz elő, amelynek közvetítője feltehetően a glutamát által okozott Ca2+-beáramlás növekvése, és ennek következményeként a protein-kináz C áthelyeződése a citoszolból a neuron (idesejt) membránjára.
Újabban Vaccarino és munkatársai (Proc. Natl. Acad. Sci. USA 84, 8707 (1987) közölték, hogy ha granulasejteket triszialozil-N-tetraglükozil-ceramid vagy monoszialozil-N-tetraglükozil-ceramid hatása alá helyeznek, akkor ez gátolja a PKC-nek glutamát által előidézett transzlokációját és aktiválódását. Ezek a glükozil-szfingolipidek nem befolyásolják nagy affinitású felismerő helyhez kötődő glutamát vagy a 4-B-[3H]forbol-12,13-dibutirát kötődését. Továbbá ezek a gangliozidok - különösen a triszialozil-N-tetraglükozil-ceramid védőhatást fejtenek ki a glutamát-előidézte sejtkárosodással szemben. Újabban közölték [Bioi. Chem. 261, 12 604 (1986); ugyanott 261, 12 610 (1986); ugyanott 261, 12 616 (1986); Science 235, 670 (1987)], hogy szfingozinok és rokon bázisok a PKC aktivitását és transzlokációját gátolják.
Az alábbiakban írjuk le azokat a vizsgálatokat, amelyek során az N-(diklór-acetil)-szfingozint szfingozinnal hasonlítottuk össze. E vizsgálatok során megfigyeltük egyrészt a PKC transzlokációjára, másrészt a primer granulasejt-tenyészetekben glutamát-előidézte a neutrotoxicitásra kifejtett hatásaikat. Az anyagok esetleges citotoxikus hatását agysejteken kifejtett neurotoxikus hatásaikkal, valamint a hemolízis alapján értékeltük ki.
Sejttenyészetek:
napos Sprague-Dawley patkányokból (Zivic Miller) vett granulasejtekből elsődleges tenyészeteket készítettünk. E tenyészetek granulasejttartalma 90%-nál nagyobb, 5% GABA-erg neuront [Proc. Natl. Acad.
HU 209 561 B
Sci. USA 79, 7919 (1982)] és 5%-nél kevesebb gliasejtet [J. Neurosci. 7, 65 (1987)] tartalmaznak. Ezeket a tenyészeteket in vitro vizsgálatainkban a 8. és 9. napon alkalmaztuk.
A tenyészetekhez adott vegyületek, valamint az adagolás módja és paraméterei:
A tenyészetekhez a 8. és 9. napon in vitro különböző' koncentrációkban (1-100 μΜ) szfingozinokat és N(diklór-acetil)-szfingozinokat adagoltunk. Közelebbről a granulasejtekből álló egyréteget (monoréteget) e vegyületekkel Locke-oldatban (1 ml) 2 órán át 37 °C-on előinkubáltuk. A szfingozint etanolban oldottuk, és szarvasmarha-szérumalbuminnal alkotott ekvimoláris komplex alakjában (zsírsavak nélkül) tároltuk, majd közvetlenül a sejtek előinkubálásához alkalmazott Locke-oldattal hígítottuk. Az N-(diklór-acetil)-szfingozint dimetil-szulfoxidban oldottuk, és közvetlenül hígítottuk Locke-oldattal. A hígított oldat alikvot részeit nitrogénáramban szárítottuk, majd a kívánt koncentráció eléréséhez szükséges mennyiségben Locke-oldatban oldottuk.
A PKC glutamát-előidézte transzlokációja: a [3H]forbol-észter-kötés megállapítása és sejtekben:
Miután a sejteket a vegyületekkel együtt vagy vegyületek nélkül (lásd fentebb), 2 órán át 37 öC-on inkubáltuk, a monorétegeket egyszer mostuk, és megállapítottuk a 4-B-[3]-forbol-12,13-dibutirát kötődését ΙμΜ glutamát jelenlétében, Mg2+ nélkül. A granulasejteket 35 mm méretű lemezeken tenyésztettük, majd egyszer mostuk Locke-oldattal (154 mM nátrium-klorid, 5,6 mM kálium-klorid, 3,6 mM nátrium-hidrogén-karbonát, 2,3 mM kalcium-klorid, 1 mM magnézium-klorid, 5,6 mM glükóz és 5 mM Hepes, pH 7,4), majd ml 4-B-[3H]-forbol-12,13-dibutirátot tartalmazó Locke-oldattal inkubáltuk (a triciált vegyület aktivitása 12,5 Ci/mmol; 1 Ci = 37 GBq; beszerzési helye a New England Nuclear) 0,1%-os szarvasmarha-szérumalbumin-oldatban zsírsavak nélkül. Előzetes kísérletek arra utaltak, hogy az egyensúly 10 percen belül elérhető, ezért a monorétegeket a 4-B-[3H]-forbol-12,13-dibutiráttal 15 percig 22 °C-on inkubáltuk. Ezt követően a kötés legfeljebb 1 órán át változatlanul fennállt. Az inkubálás után a sejteket háromszor mostuk hideg Locke-oldattal, majd 0,1 n nátronlúg oldatban szuszpendáltuk. A szuszpenzió alikvot részeit használtuk fehérjemeghatározásra és folyadékszcintillációs méréshez. A nem specifikus kötést 2 μΜ fórból-12-tetradekanoát13-acetát jelenlétében határoztuk meg.
A glutamáttal előidézett neurotoxicitás vizsgálata:
Ép granulasejtekből álló monorétegeket különböző vegyületek különböző koncentrációinak a jelenlétében órán át 37 °C-on előinkubáltuk. A vegyületek feleslegének az eltávolítása és ismételt kimosás után a sejteket 50 μΜ glutamáttal Mg2+ nélkül 15 percig szobahőmérsékleten inkubáltuk. Ezután három mosást alkalmaztuk, majd a monorétegeket ismét a tenyésztőközegbe helyeztük. A sejtek túlélését 24 óra elmúltával hisztokémiai eljárással határoztuk meg. Közelebbről a sejt életképességét úgy állapítottuk meg, hogy a sejteket fluoreszceinnel festettük, amely zöld fluoreszcenciát eredményez az élő sejteken és vörös fluoroszcenciát ad az elpusztult sejtekkel.
A citotoxicitás megállapítása a neurotoxicitás és hemolízis vizsgálata útján:
A vegyületek esetleges citotoxicitását agysejt-tenyészetekre glutamát távollétében kifejtett toxicitásuk alapján értékeltük ki. E célra a fentiekben leírt vizsgálati módszereket alkalmaztuk azzal a különbséggel, hogy glutamátot nem adtunk a vizsgálati rendszerhez. Kiértékeltük továbbá a vegyületek hemolitikus hatását is. E célra 4 önkéntes egyéntől vérmintát vettünk, minden egyes mintát 3 kémcsőben osztottuk el, és a következőképpen jártunk el:
1. 1 ml vér + 1 ml N-(diklór-acetil)-szfingozin oldat
2. 1 ml vér + 1 ml D-szfingozin oldat
3. 1 ml vér + 1 ml plazma ugyanazon önkéntes egyéntől (negatív kontroll).
Az N-(diklór-acetil)-szfingozint és a D-szfingozint különböző koncentrációkban adtuk a mintákhoz (lásd a fentebb leírt kísérleteket). Ezután a kémcsöveket 45 percig 37 °C-on inkubáltuk, majd centrifugáltuk. A hemoglobint a felülúszóban mennyiségileg határoztuk meg a ciano-methemoglobin módszerrel. Ebben a próbában az inkubált hemoglobin mennyiségének azonosnak vagy kisebbnek kell lennie, mint a negatív kontrolion mért érték.
A kapott eredményeket az alábbiakban ismertetjük.
A vizsgált vegyület gátolta a PKC glutamát-előidézte transzlokációját, és jelentős mértékben csökkentette a glutamát által okozott sejttoxicitást. A vegyületek hatását jellemző ED50-értékeket az 1. táblázat 1. és 2. oszlopában foglaltuk össze. Az 1. táblázat 3. oszlopában találhatók a vegyületek esetleges neurotoxicitásának in vitro vizsgálati eredményei. Megjegyezzük, hogy az N-(diklór-acetil)-szfingozin egyáltalán nem bizonyult toxikusnak, illetve toxicitása csak lényegesen magasabb koncentrációban nyilvánult meg, mint amelyek a glutamát-előidézte transzlokáció antagonizálásához (1. oszlop) és a glutamát által okozott neurotoxicitás kivédéséhez (2. oszlop) szükségesek. Továbbá, amint az 1. ábrán látható, azN-(diklóracetil)-szfingozin - a szfingozinnal ellentétben - nem idéz elő hemolízist.
Szfingozin hatása a PKC glutamát-előidézte transzlokációjára, a glutamát által okozott neurotoxicitásra (védőhatás glutamáttal szemben) és a vegyület neurotoxicitása; az adatok a μΜ-ban kifejezett ED50-értékre vonatkoznak.

A találmány szerinti új gyógyászati készítmények amint fentebb már említettük - orális, rektális, parenterális, helyi (lokális) vagy transzdermális (bőrön át történő) adagolásra alkalmas alakban állíthatók elő. Ezek a készítmények lehetnek szilárdak, félszilárdak vagy folyékonyak; orális beadás céljaira például tabletták, pilulák, zselatinkapszulák vagy lágy kapszulák, reaktális beadásra végbélkúpok állíthatók elő. A parenterálisan alkalmazható készítmények intramuszkuláris, szubkután vagy transzdermális adagolásra alkalmasak; vagy infúziók vagy intravénás injekciók, amelyeket vagy a hatóanyagok oldatának alakjában vagy hatóanyagok liofilizálással kapott poralakjában szerelünk ki; utóbbi esetben a liofilizált hatóanyagot felhasználás előtt egy vagy több gyógyászati szempontból elfogadható vivő- vagy hígítószenei elegyítjük, ügyelvén arra, hogy a készítmény ozmózisnyomása a testfolyadékokkal összeegyeztethető legyen. Helyi (lokális) alkalmazás céljára permetet, például orrpermetet, vagy krémeket vagy kenőcsöket állíthatunk elő; bőrön át történő adagolásra megfelelő módon kezelt tapaszokat alkalmazhatunk.
A találmány szerinti készítmények embereknek vagy állatoknak adagolhatók; a hatóanyagot oldatok, permetek, kenőcsök és krémek esetében 0,01 tömeg%tól (az alábbiakban röviden: t%) 10 t%-ig teijedő mennyiségben, szilárd készítmények esetében 1 t%-tól 100 t%-ig, előnyösen 5 t%-tól 50 t%-ig terjedő menynyiségben tartalmazzák. A szükséges adag mennyisége egyedi feltételektől, a kívánt hatástól és az adagolás választott módjától függ. A találmány szerinti hatóanyagok napi adagja általában 0,05 mg/testtömeg kg és 6 mg/kg között lehet. Az alábbi 1. példa a hatóanyagként alkalmazható N-(diklór-acetil)-szfingozin előállítását [analóg módon állítható elő az N-(diklór-acetil)dihidroszfingozin is], a további példák a gyógyszerkészítmények összetételét szemléltetik.
1. példa
N-(Diklór-acetilf-szfingozin előállítása
5,0 g (16,70 mmol) 4. példa szerint előállított szfingozin és 100 ml kloroform oldatában 3,5 ml (25,11 mmol) trietil-amint és 3,2 g (21,45 mmol) diklóracetil-kloridot adunk, és a reakcióelegyet szobahőmérsékleten 24 órán át állni hagyjuk. Ekkor a kloroformos oldatot vízzel többször mossuk, vízmentes nátrium-szulfáton szárítjuk, majd vákuumban szárazra pároljuk. A lepárlási maradékot preparatív kromatográfiával tisztítjuk, eluálószerként kloroform és metanol 90:10 arányú elegyét alkalmazzuk. Az így kapott terméket acetonitrilből készer átkristályosítjuk, majd vákuumban szárítjuk. így kristályos alakban kapjuk a cím szerinti vegyületet 7,0 g hozammal, op.: 79 °C; VRK során szilikagélen diklórmetán/etil-acetát-metanol 70:30:10 arányú elegyével kifejlesztve egyetlen foltot ad, amelynek Rrértéke 0,48.
A következő 2-5. példa a hatóanyagként N-(diklóracetil)-szfingozint vagy N-(diklór-acetil)-dihidroszfingozint tartalmazó különféle gyógyszerkészítmények összetételét szemlélteti. A készítmények előállítása ismert gyógyszerészeti eljárásokkal történik.

reknek parenterálisan adagolható. A fenti készítmények a hatásspektrum kiszélesítésére egyéb, önmagukban ismert gyógyászati hatású anyagokat is tartalmazhatnak.
5. példa
Kettős flakonban kiszerelt gyógyászati készítmények előállítása
Az ezen példában bemutatott készítményeket kettős flakonban szereljük ki. Az egyik flakon tartalmazza a hatóanyagot liofilizált por alakjában, amelynek hatóanyagmennyisége 10 és 90 tömeg% között váltakozik, a fennmaradó rész gyógyászati szempontból elfogadható vivőanyag, például glicin vagy mannit. A másik flakon oldószert - például nátrium-klorid oldatot és citrátpuffert -tartalmaz.
A két flakon tartalmát közvetlenül felhasználás előtt összekeverve a hatóanyag liofilizált pora gyorsan feloldódik, s így befecskendezhető (injekciós) oldatot kapunk. A hatóanyagot liofilizált por alakjában tartalmazó flakon a találmány szerint előállított gyógyászati készítmények egyik előnyös kiviteli alakja.

1. Eljárás N-(diklór-acetil)-szfmgozint vagy N-(diklór-acetil)-dihidroszfingozint tartalmazó gyógyászati készítmények előállítására, azzal jellemezve, hogy az ismert módon előállított N-(diklór-acetil)-szfingozin vagy N-(diklór-acetil)-dihidroszfmgozin vegyület gyógyászati szempontból elfogadható vivőanyaggal hígítószerrel és/vagy egyéb gyógyszerészeti segédanyaggal összekeverve előnyösen idegrendszeri zavarok gyógykezelésére alkalmas gyógyászati készítménnyé alakítjuk.
2. Az 1. igénypont szerinti eljárás, azzal jellemezve, hogy hatóanyagként N-(diklór-acetil)-szfingozint alkalmazunk.
