Abstract

Prótesis total de rodilla posteroestabilizada que consta de una parte tibial que tiene una plataforma tibial, una parte femoral y un inserto tibial interpuesto entre las partes femoral y tibial, fijada mediante unos medios de fijación a la plataforma tibial y constando de una base (5) sobre la parte superior de la cual están formadas una o varías superficies cóncavas de contacto con uno o varios cóndilos de una parte femoral y, un pasador (6) tibial que sobresale, en particular, perpendicularmente desde la base, montándose el pasador tibial de manera amovible con respecto a la base, constando el pasador tibial de un saliente (12) procedente de su superficie inferior de longitud mayor que el espesor de la base y, la base consta de un orificio (9) pasante por el que pasa el pasador tibial para sobresalir por fuera de la superficie inferior de la base, opuesta a la superficie superior, caracterizada porque la disposición se realiza de modo que el pasador no pueda pivotar con respecto ala base con relación al eje del saliente.

Pasador modular para prótesis de rodilla posteroestabilizada
La presente invención se refiere a un pasador tibial de una prótesis de rodilla, denominada posteroestabilizada, en particular, de una prótesis de rodilla de prueba, así como a una prótesis de rodilla que consta de un pasador de este tipo, en particular, una prótesis de prueba.
Este tipo de prótesis consta de un implante tibial y de un implante femoral. El implante tibial consta de una plataforma tibial sobre la cual descansa un inserto tibial (también denominado menisco), generalmente de polietileno. El inserto tibial consta, por una parte, de una base sobre la parte superior de la cual se forman las superficies superiores, en general cóncavas, destinadas a cooperar con las superficies exteriores de los cóndilos, generalmente convexas, del implante femoral y, por otra parte, de un pasador tibial que sobresale de la base, en particular, en perpendicular y, destinada a cooperar con un pasador femoral asociado. El pasador femoral es sustancialmente transversal y se extiende de un cóndilo al otro del implante femoral y hace tope contra el pasador tibial para impedir un deslizamiento anteroposterior del implante femoral en un área de flexión de rodilla, en particular, a partir de una flexión de 30°.
En función de la anatomía del fémur y de la tibia sobre los que debe instalarse la prótesis, en particular, su orientación mutua y el volumen de resección ósea que se quiere obtener, es necesario seleccionar un inserto tibial de entre varios tamaños de inserto tibial posibles.
Cuando un cirujano instala este tipo de prótesis, debe verificar, por una parte, el tamaño del implante y, también, la correcta posición del pasador tibial. Por lo tanto, debe tener disponible el conjunto de los diferentes insertos tibiales posibles, en forma de prótesis de prueba, lo que implica tener muchas existencias. Además, es necesario tener una prótesis de prueba estable y resistente a los esfuerzos como una prótesis real, con el fin de poder verificar que el tamaño seleccionado se adapta a las necesidades.
El documento EP 1108403 describe una prótesis de acuerdo con el preámbulo de la reivindicación 1.
La presente invención tiene como objetivo una prótesis total de rodilla posteroestabilizada, en particular, una prótesis de prueba de acuerdo con la reivindicación 1.
De este modo, el pasador, después de atravesar la base, puede penetrar también en un orificio formado en la plataforma tibial, incluso la quilla de anclaje en la tibia. De este modo, también es posible tener disponibles varias bases de diferentes tamaños, en particular, de diferentes espesores, teniendo todas el mismo orificio pasante para recibir un mismo saliente. Se obtiene con un único pasador insertos de diferentes tamaños para prótesis de prueba que, no obstante, son robustas con respecto a los esfuerzos y restricciones y, por lo tanto, que permite realizar pruebas reales. En particular, el saliente que atraviesa la base mantiene firmemente en su posición al pasador fijado con respecto a la base, sin que este pueda bascular debido a un impacto de la parte femoral sobre el pasador.
De acuerdo con un perfeccionamiento de la invención, el pasador tibial consta de un soporte de base sustancialmente plano y de forma no circular, en particular, sustancialmente rectangular o cuadrada, de la que sobresale el saliente, mientras que el inserto tibial consta de un rebaje en su superficie superior de dimensiones y forma complementarias a las del soporte con el fin de recibirlo para evitar la rotación con respecto al eje del saliente.
De acuerdo con un modo de realización preferente, el saliente tiene una forma cilíndrica, en particular, circular y, el orificio tiene una forma complementaria a la del saliente.
De acuerdo con un modo de realización preferente, una junta tórica de sujeción se ensarta en el saliente, con el fin de permitir un bloqueo del saliente en el orificio.
La presente invención también se refiere a un conjunto de insertos tibiales, en concreto para prótesis de prueba, constituido por una pluralidad de bases que tienen tamaños diferentes y de un pasador tibial de acuerdo con la invención, teniendo todas las bases el mismo orificio, incluso el mismo rebaje.
A continuación, se describe una prótesis de rodilla de prueba de acuerdo con un modo de realización de la invención, únicamente a modo de ejemplo, haciendo referencia a los dibujos, en los que:
la figura 1 representa un inserto tibial montado sobre una parte tibial de una prótesis de rodilla, en un estado no ensamblado, y
la figura 2 representa la parte tibial de la figura 1, en un estado ensamblado.
En la figura 1, se puede ver una parte tibial de una prótesis de prueba de rodilla, que consta de una quilla 1 de anclaje en el canal medular de la tibia, una plataforma 2 tibial, un espaciador 3 y un inserto 4, de polímero.
El inserto 4 consta de una base 5 y de un pasador 6. La superficie superior de la base 5 consta de superficies 7 cóncavas de contacto con cóndilos de una parte femoral.

La base 5 consta de un rebaje 8 de fondo sustancialmente cuadrado en el que se forma un orificio 9 pasante de la cara superior a la cara inferior de la base. El orificio 9 tiene una sección circular. La sección transversal del orificio es más pequeña que el fondo del rebaje 8.
Dos pasadores de fijación, derecho e izquierdo, sobresalen de la cara inferior de la base 5 y penetran en los agujeros 15, derecho e izquierdo, complementarios formados en la plataforma 2 tibial y el espaciador 3, para permitir una fijación del inserto 4 tibial con respecto a la plataforma 2. En particular, se impide una rotación del inserto 4 con respecto a la plataforma 2 con relación al eje de la quilla 1 mediante estos pasadores y agujeros 15 de forma complementaria. Por supuesto, son posibles otros medios de fijación del inserto a la plataforma tibial.
El pasador 6 consta de un soporte 10 de forma complementaria a la del rebaje desde el cual se eleva hacia arriba una especie de prominencia 11 destinada a entrar en contacto con el pasador femoral que se extiende entre los dos cóndilos de la parte femoral, en dirección mediolateral.
El soporte 10 y el rebaje 8 no tienen una forma circular, de modo que el soporte no puede girar en el interior del rebaje con respecto al eje de la quilla 1.
Desde la cara plana inferior del pasador 6, procede un saliente 12 cilíndrico circular oblongo de forma complementaria a la del orificio 9.
En la posición ensamblada de la prótesis (figura 2), el pasador penetra en el orificio con ajuste apretado y el soporte 10 se coloca en el fondo del rebaje 8, en particular, con ajuste a presión.
La longitud del saliente es superior al espesor de la base al nivel del orificio pasante, de modo que el saliente "vuelve a salir" por el otro lado.
La longitud del saliente es superior a la suma del espesor de la base, del espesor del posible espaciador y del espesor de la plataforma tibial, para que penetre en un rebaje 13 de quilla formado en la quilla de la prótesis.
En el caso, por ejemplo, donde existen cuatro espesores diferentes posibles para la base, el saliente se dimensiona en longitud teniendo en cuenta el espesor más grande.
Se embute una junta 14 tórica alrededor del saliente, de modo que, una vez introducida en el orificio, la junta tenderá a bloquearlo.
El hecho de que el saliente atraviese la base 5 y penetre en la quilla permite obtener una prótesis de prueba particularmente rígida y resistente a los esfuerzos, de modo que las pruebas que se realizan con ella puedan considerarse tan fiables como con una prótesis de prueba con un pasador fijo. La ventaja es, por supuesto, que esta fiabilidad de la prueba puede obtenerse sin necesitar de tener disponible toda la gama de dimensiones de las partes tibiales.

    REIVINDICACIONES
    1. Prótesis total de rodilla posteroestabilizada que consta de una parte tibial que tiene una plataforma tibial, una parte femoral y un inserto tibial interpuesto entre las partes femoral y tibial, fijada mediante unos medios de fijación a la plataforma tibial y constando de una base (5) sobre la parte superior de la cual están formadas una o varías superficies cóncavas de contacto con uno o varios cóndilos de una parte femoral y, un pasador (6) tibial que sobresale, en particular, perpendicularmente desde la base, montándose el pasador tibial de manera amovible con respecto a la base, constando el pasador tibial de un saliente (12) procedente de su superficie inferior de longitud mayor que el espesor de la base y, la base consta de un orificio (9) pasante por el que pasa el pasador tibial para sobresalir por fuera de la superficie inferior de la base, opuesta a la superficie superior, caracterizada porque la disposición se realiza de modo que el pasador no pueda pivotar con respecto ala base con relación al eje del saliente.
  2. 2. Prótesis de acuerdo con la reivindicación 1, caracterizada porque el pasador tibial consta de un soporte de base sustancialmente plana y de forma no circular, en particular, sustancialmente rectangular o cuadrada, de la que sobresale el saliente, mientras que el inserto tibial consta de un rebaje en su superficie superior de dimensiones y forma complementarias a las del soporte con el fin de recibirlo para evitar la rotación con respecto al eje del saliente.
  3. 3. Prótesis de acuerdo con la reivindicación 1 o 2, caracterizada porque el saliente tiene una forma cilíndrica, en particular, circular y, el orificio tiene una forma complementaria a la del saliente.
  4. 4. Prótesis de acuerdo con la reivindicación 1, 2 o 3, caracterizada porque una junta tórica de sujeción se ensarta en el saliente, con el fin de permitir un bloqueo del saliente en el orificio.
  5. 5. Prótesis de acuerdo con una de las reivindicaciones anteriores, caracterizada porque se trata de una prótesis de prueba.
  6. 6. Conjunto de insertos tibiales para prótesis de prueba de acuerdo con la reivindicación 5, constituido
    - por una pluralidad de bases que tienen tamaños diferentes y teniendo cada base (5) sobre su parte superior una o varias superficies cóncavas de contacto con uno o varios cóndilos de una parte femoral; y
    - por un pasador (6) tibial que consta de un saliente (12) proveniente de su superficie inferior de longitud mayor que el espesor de cada base y cada base consta de un orificio (9) pasante por el que puede pasar el pasador tibial para sobresalir por fuera de la superficie inferior de cada base, opuesta a la superficie superior, realizándose la disposición de modo que el pasador no pueda pivotar con respecto a cada base con relación al el eje del saliente.
  7. 7. Conjunto de insertos tibiales de acuerdo con la reivindicación 6, caracterizado porque el pasador tibial consta de un soporte de base sustancialmente plana y de forma no circular, en particular, sustancialmente rectangular o cuadrada, de la que sobresale el saliente, mientras que cada base consta de un rebaje en su superficie superior de dimensión y forma complementaria a las del soporte con el fin de recibirlo para evitar la rotación con respecto al eje del saliente.
  8. 8. Conjunto de insertos tibiales de acuerdo con la reivindicación 6 o 7, caracterizado porque el saliente tiene una forma cilindrica, en particular, circular y, el orificio tiene una forma complementaria a la del saliente.
