Abstract
Implante (100) intragástrico de tratamiento de la obesidad, que comprende: un armazón (102) plegable y expansible formado por una pluralidad de hilos espaciados que se extienden cada uno entre polos (116, 120) de extremo, teniendo el armazón una configuración relajada generalmente esférica con un tamaño suficiente para entrar en contacto con las paredes interiores del estómago tras la contracción del mismo; y un material (104) flexible envuelto alrededor del, y conectado al, armazón para abarcar los espacios entre los hilos, siendo el armazón capaz de convertirse en una configuración de colocación sustancialmente lineal separando los polos uno de otro, y estando formado por un material que resistirá la degradación a lo largo de un periodo de al menos seis meses en el interior del estómago, caracterizado porque cada hilo de dicha pluralidad de hilos se extiende entre los polos de extremo a lo largo de una trayectoria generalmente helicoidal.

Tal como se ha mencionado, las figuras 6A-6F ilustran variaciones en el dispositivo intragástrico en forma de hilo envuelto de la presente solicitud. Las figuras 6A y 6B ilustran hilos enrollados alrededor del dispositivo en orientaciones tanto CW como CCW, con diez en total en la figura 6A y quince en total en la figura 6B. La figura 7 es una vista en perspectiva de un dispositivo 130 intragástrico alternativo que tiene un armazón 132 articulado expandido en una configuración esférica y que tiene un material 134 textil flexible envuelto sobre el mismo. En este contexto, “articulado” significa que el armazón 132 comprende una pluralidad de elementos relativamente rígidos conectados en bisagras. El componente central de este dispositivo es un conjunto mecanizado de montajes 136 unidos de manera independiente que comprenden dos elementos 138 de filamento y una bisagra 140, mostrándose ejemplos alternativos de los mismos en las figuras 8A y 8B. Todo el armazón 132 articulado consiste, de manera deseable, en entre 6-12 filamentos en total que convergen en polos 142 opuestos. Los polos 142 comprenden tapas de extremo con una pluralidad de bisagras unidas a cada elemento 138 de filamento convergente. En el ejemplo ilustrado, hay seis filamentos que comprenden cuatro elementos 138 de filamento cada uno, para un total de veinticuatro elementos de filamento unidos de manera independiente. La ventaja de esta estructura mecánica es la robustez inherente de los segmentos individuales, basándose en la mecánica de la interacción articulada en lugar de en las propiedades del material.
El dispositivo 130 intragástrico ilustrado presenta dos puntos de acceso principales para el ajuste endoscópico. Los elementos 138 de filamento son preferiblemente tubos huecos, y el dispositivo 130 incorpora, de manera deseable, capacidad de ajuste in vivo por medio de elementos que discurren a través de las luces de, o que son paralelos a, los filamentos. Se accedería entonces a estos elementos a través de puntos, tales como en los polos 142, en el armazón, utilizando un accesorio endoscópico (no mostrado). Por ejemplo, podrían discurrir cables flexibles a través de los elementos 138 de filamento huecos y a través de aberturas en cada bisagra 140, por lo que tensar los cables hace que los filamentos se pongan rectos, y viceversa.
La adaptabilidad y flexibilidad del armazón están relacionadas directamente con la longitud y el número de elementos 138 de filamento individuales, y con la configuración de las bisagras 140. La figura 7 muestra bisagras 140 que limitan la articulación de los elementos 138 de filamento adyacentes a dentro de un único plano. Obsérvese que las juntas articuladas limitan la flexibilidad en la forma plegada, pero aumentan la alineación dentro de su plano de pivote. Alternativamente, la figura 8A ilustra una junta 140’ esférica o de bolas, mientras que la figura 8B muestra juntas 140” flexibles de polímero blando, o bisagras sin partes móviles, si se desea. Estos dos últimos tipos de juntas 140 no están restringidas en ningún plano (son omnidireccionales) y, por tanto, proporcionan algo más de flexibilidad al dispositivo 130 expandido a partir de las presiones peristálticas del estómago. Es decir, el dispositivo con juntas 140 omnidireccionales se comporta más como un balón convencional. Las figuras 9A y 9B ilustran un dispositivo 150 intragástrico adicional que comprende un armazón 152 tubular de hilos que tiene capacidad de ajuste de tamaño. El armazón 152 tubular de hilos comprende una estructura de armazón no segmentada sin elementos de filamento segmentados y, en cambio, incluye una pluralidad de tubos 154 exteriores circulares resistentes al retorcimiento, adaptables, cada uno con un elemento 156 de capacidad de ajuste que discurre a través de las luces. Los materiales para el tubo 154 exterior podrían incluir silicona, PTFE enrollado, o equivalente. El elemento 156 con capacidad de ajuste podría ser de NiTi, PEEK, u otro material similar con alta flexibilidad, alta resistencia a la tracción y bajo alargamiento.
La principal diferencia con el dispositivo 150 de las figuras 9A-9B es que los filamentos mecánicos discurren longitudinalmente a lo largo de la longitud de la estructura, en contraposición a estar en una configuración helicoidal. Los esfuerzos tangenciales de los filamentos en circuito permitirán la adaptabilidad del armazón, pero garantizarán que los filamentos vuelvan a su posición por defecto (o ajustada). No se muestra en este documento un material opcional de envoltura que podría rodear el armazón para fines de ocupación de volumen, tal como se mencionó anteriormente. Como con los armazones de hilos comentados anteriormente, la flexibilidad y la adaptabilidad del dispositivo 150 se basan en las propiedades del material del tubo 154 exterior y del elemento 156 con capacidad de ajuste. Proporcionar una estructura tubular con una luz dentro de la cual puede colocarse un metal tal como Nitinol proporciona una mayor longevidad del implante, impidiendo la exposición del elemento 156 con capacidad de ajuste a disoluciones gástricas corrosivas.

Los términos también identifican intervalos de peso que se ha mostrado que aumentan la posibilidad de padecer determinadas enfermedades y otros problemas de salud. Los solicitantes proponen implantar los dispositivos tal como se describe en el presente documento en un grupo de estudio clínico de pacientes obesos, con el fin de monitorizar la pérdida de peso.
Los estudios clínicos utilizarán los dispositivos descritos anteriormente conjuntamente con los siguientes parámetros. Materiales: Los materiales de silicona utilizados incluyen silicona 3206 para cualquier cubierta, estructura inflable o estructura
hueca flexible de otro modo. Cualquier válvula de llenado puede estar compuesta por silicona 4850 con un 6% de BaSO4. Las estructuras tubulares u otros conductos flexibles estarán compuestos por caucho de silicona tal como define la Food and Drug Administration (FDA) en el Code of Federal Regulations (CFR), título 21, sección 177.2600.
Fines: los dispositivos son para implante en humanos; se pretende que los dispositivos ocupen espacio gástrico a la vez que también aplican presión intermitente a zonas

se pretende que los dispositivos estimulen sensaciones de saciedad, funcionando de ese modo como tratamiento para la obesidad. Procedimientos de implante generales: Se pretende que el dispositivo se implante por vía transoral mediante endoscopio en el interior del cuerpo del
estómago. La implantación de los dispositivos médicos se producirá mediante endoscopia. Puede utilizarse la administración nasal/respiratoria de oxígeno e isoflurano durante los procedimientos quirúrgicos
para mantener la anestesia, según sea necesario. A continuación, se enumera un procedimiento de implante a modo de ejemplo: a) Realizar una endoscopia preliminar en el paciente para examinar el tracto GI y para determinar si existe cualquier
anomalía anatómica que pueda afectar al procedimiento y/o al resultado del estudio. b) Insertar un elemento de introducción en el sobretubo. c) Insertar un gastroscopio a través de la entrada del elemento de introducción hasta que la parte flexible del
gastroscopio salga completamente del extremo distal del elemento de introducción.
d) Guiándose por visión endoscópica, hacer avanzar suavemente el gastroscopio, seguido por el elemento de introducción/sobretubo, al interior del estómago. e) Retirar el gastroscopio y el elemento de introducción mientras se mantiene el sobretubo en su sitio. f) OPCIONAL: Colocar la tapa de insuflación sobre la entrada de los sobretubos, insertar el gastroscopio y hacer
avanzar de nuevo hacia la cavidad del estómago.
g) OPCIONAL: Insuflar aire/gas inerte en el estómago para proporcionar mayor volumen de trabajo para la visión endoscópica. h) Plegar el implante gástrico e insertar el implante lubricado en el sobretubo, con catéter de inflado a continuación,
si es necesario.
i) Mediante la visión endoscópica, empujar el implante gástrico hacia abajo del sobretubo con gastroscopio hasta que puede determinarse la confirmación visual del despliegue del dispositivo en el interior del estómago. j) Retirar el hilo guía del catéter de inflado si se ha utilizado. k) Si se infla: Inflar el implante utilizando un kit de llenado de sistema de balón intragástrico BioEnterics (“sistema
BIB”) convencional.

l) Utilizar incrementos de 50-60 cm3, inflar el volumen hasta el volumen de llenado deseado.
m) Retirar el catéter de inflado a través del sobretubo.
n) Inspeccionar el implante gástrico mediante la visión endoscópica para determinar la fuga de válvulas y cualquier otra posible anomalía. Registrar todas las observaciones.
o) Retirar el gastroscopio del sobretubo.
p) Retirar el sobretubo del paciente.
