B) una parte superior del implante (5) con un mango (6) esencialmente en forma cilíndrica circular con rosca exterior (7) y eje de rotación (11), que se extiende a la cavidad (3) de la parte inferior del implante (2), y una sección de aposición (8) destinada a ser colocada en la superficie de cubierta de la vértebra superior contigua; donde C) ambas partes del implante (2, 5) están aseguradas contra la rotación en torno a su eje de rotación (11); así como D) un anillo (9) colocado entre ambas partes del implante (2, 5) con rosca interior (10), la cual coopera con la rosca exterior (7) de la parte superior del implante 5, donde E) ambas partes del implante (2, 5) y el anillo (9) se colocan coaxialmente a lo largo de su eje de rotación común (11), de manera que F) al girar el anillo (9) en la rosca exterior (7) del mango (6), la distancia entre ambas secciones de aposición (4, 8) puede cambiar continuamente; caracterizado porque G) el anillo (9) se une axialmente de forma firme, pero con movimiento rotativo, a la parte inferior del implante (2); y H) el anillo (9) posee una superficie de anillo circular inferior (12) situada en relación con la parte inferior del implante (2), y una superficie de anillo circular superior (13) situada en relación con la parte superior del implante, donde I) la superficie de anillo circular inferior (12) posee una corona dentada (23), que es apropiada para recibir un piñón correspondiente; y J) la rosca exterior (7) del mango cilíndrico circular (6) y la rosca interior (10) del anillo (9) son autobloqueables.
Description
Implante intervertebral.
La invención se refiere a un implante
intervertebral de acuerdo con el concepto que aparece en la
reivindicación 1.
A partir de la DE-A 196 22 827
se conoce un implante intervertebral de ese tipo. En el mismo, la
parte del implante que tiene forma de anillo con rosca interior se
une mediante rosca solo a la parte superior terminal del implante.
El anillo se mueve libremente de forma rotativa y axial con respecto
a la parte inferior terminal del implante. La parte inferior
terminal del implante, sirve al anillo sólo como tope axial, es
decir como superficie de apoyo contra la cual puede hacerse girar el
anillo, de manera que la parte superior terminal del implante pueda
moverse axialmente (mecanismo de acción tornillo/tuerca). De esta
forma, ambas partes terminales del implante se montan libremente una
contra otra, lo cual también se demuestra por el hecho de que para
su seguridad axial dispone de un orificio roscado radial en una de
las dos partes del implante, a través del cual se pasa un tornillo
prisionero de retención. Con esto, este conocido implante
intervertebral presenta la desventaja de que es poco seguro y puede
zafarse antes y durante la operación. Un desprendimiento de una
parte del implante en la sala de operaciones podría originar un
considerable retraso de tiempo por razones de esterilización.
A partir de la EP-A 1 219 266
ULRICH se conoce una prótesis intervertebral telescópica, que posee
un engranaje de rueda cónica el cual puede hacerse girar de manera
radial mediante un piñón de rueda cónica. La desventaja de este
conocido dispositivo es el hecho de que el anillo de rosca no está
fijado axialmente a una de las partes del casquillo. Por ello, el
implante tiene una estructura poca estable y además se necesitaría
un instrumento especial para provocar la distracción del implante.
Otra desventaja de este conocido implante consiste en la utilización
del engranaje de rueda cónica, que es poco "aceptable" y por
ende desventajoso. En el caso del engranaje de rueda cónica, el
instrumento de accionamiento ya no podría realizar su función al
producirse un pequeño desplazamiento axial y el cirujano tendría que
colocar de nuevo el instrumento in-situ.
En este caso, la invención servirá de ayuda. La
invención tiene la tarea de crear un implante intervertebral, que,
conservando una altura constructiva lo más pequeña posible, cree un
todo asegurado contra el desprendimiento de las diferentes partes y
que pueda ser puesto a disposición del cirujano como un conjunto
preensamblado.
La invención soluciona la tarea propuesta con un
implante intervertebral, el cual posee las características de la
reivindicación 1.
Otras ventajas de la invención se describen en
las reivindicaciones secundarias.
Las ventajas alcanzadas mediante la invención se
pueden apreciar esencialmente en que, gracias al implante
intervertebral de acuerdo con la invención, es posible una
manipulación más sencilla, lo cual incrementa por una parte la
seguridad, y por otra, reduce el tiempo necesario para realizar el
implante.
En una forma de realización especial, el anillo
del implante intervertebral se fija axialmente de manera firme,
pero con movimiento rotativo, mediante una unión con grapa en la
sección inferior del implante. De esta forma, el anillo permanece
seguro en su posición para el accionamiento, es decir, el anillo no
puede apartarse axialmente. Ventajosamente, la unión con grapa está
compuesta por un corte trasero en forma de cuerno en el anillo y
una correspondiente protuberancia en forma de anillo en el extremo
libre de la cubierta de la parte inferior del implante. Con ello se
garantiza la rotación del anillo de manera tal que la rotación
alrededor del mismo centro se produce como ambas partes del implante
en forma de casquillo.
De acuerdo con la invención, el anillo posee una
superficie circular inferior en relación con la parte inferior del
implante, y una superficie de anillo circular superior en relación
con la parte superior del implante, donde la superficie de anillo
circular inferior posee una corona dentada apropiada para recibir
un piñón correspondiente. Esta realización permite una rotación
axial del anillo mediante la rotación radial del instrumento
portador del piñón. En comparación con los implantes conocidos, en
los que el anillo tiene que moverse tangencialmente, éste tiene la
ventaja de que requiere un espacio más pequeño para el instrumento
giratorio, así como permite una alta velocidad, ya que se puede
girar de forma continua sin que sea necesario insertar nuevamente el
instrumento giratorio en nuevos orificios del anillo.
En otra forma de realización, la cubierta de la
sección inferior del implante posee una hendidura radial,
preferentemente continua, la cual es apropiada para colocar y apoyar
un piñón contra la corona dentada. Con ello surge la ventajosa
posibilidad de poder introducir virutas de hueso en la cavidad de la
sección inferior del implante a través de la hendidura continua, una
vez que se produzca la distracción de la vértebra.
En otra forma de realización, el mango
cilíndrico circular (6) de la parte superior del implante -para
asegurar contra la rotación de la parte superior del implante con
respecto a la parte inferior del implante- posee una ranura guía
paralela al eje de rotación y la cubierta de la parte inferior del
implante posee en su lado interior un elemento guía (17) que
sobresale hacia adentro y que penetra en la ranura guía. Dicho
elemento guía se puede insertar preferentemente en la cubierta a
través de una perforación radial. Con ello se evita la rotación
recíproca de ambas partes del implante. Una rotación recíproca
afectaría el contacto del implante con las placas terminales y los
ángulos de Lordose.
En otra forma de realización, el mango
cilíndrico circular (6) de la parte superior del implante (5) posee
un volumen más pequeño que la cavidad cilíndrica hueca (3) de la
parte inferior (2) del implante, de manera que ambos componentes no
entran en contacto.
De acuerdo con la invención, la rosca exterior
del mango cilíndrico circular y la rosca interior del anillo se
autobloquean. Mediante el autobloqueo de la rosca, el implante puede
se distraer hasta una altura deseada, sin que para ello sea
necesario un retén adicional.
En otra forma de realización, la inclinación de
la rosca exterior y de la rosca interior se encuentra en el
intervalo de 0,5 hasta 1,0 mm, preferentemente de
0,6-0,8 mm. Mayores inclinaciones, por ejemplo, en
el intervalo de 2 mm, mostraron desventajas dentro de la operación,
ya que la distracción fue demasiado brusca, e inclinaciones más
pequeñas trajeron como consecuencia una rotación que tomó más
tiempo.
En otra forma de realización, tanto la rosca
exterior como la rosca interior son concebidas como rosca a derecha.
Ello tiene la ventaja de que el instrumento portador del piñón,
usualmente puede ser girado hacia la derecha como un destornillador
normal para distraer el implante.
En otra forma de realización, tanto la rosca
exterior como la rosca interior son concebidas como rosca de paso
múltiple, preferentemente como rosca de dos pasos. Con una rosca de
dos pasos, por cada vuelta del anillo se logra un recorrido doble.
Con ello se logra una manipulación más rápida y más sencilla del
implante, lo que permite reducir el tiempo de operación y se causan
menos molestias al paciente.
En otra forma de realización, ambas secciones de
aposición son concebidas como elementos en forma de placas
transversales al eje de rotación, con una superficie de aposición
destinada a apoyarse en la superficie de cubierta de la vertebra
contigua.
En otra forma de realización, las superficies de
aposición no son colocadas de forma ortogonal con respecto al eje
de rotación, sino que forman preferentemente un ángulo de 83º hasta
85º con el eje de rotación. Con ello se logra una mejor adaptación a
las condiciones anatómicas. Preferentemente ambas superficies de
aposición forman entre sí un ángulo de 10º hasta 14º, pudiéndose así
garantizar el ángulo anatómico de Lordose.
En otra forma de realización, al menos una de
las dos superficies de aposición, preferentemente la de la parte
superior del implante se arquea hacia fuera. Con ello se logra una
adaptación óptima a la vértebra. La superficie de aposición de la
parte inferior del implante tiene una forma preferentemente
plana.
En otra forma de realización, la corona dentada
tiene dientes rectos. La corona dentada de dentadura recta es más
"aceptada" que la de engranaje cónico/oblicuo.
La invención y las modificaciones de la
invención se explicarán a continuación de forma más detallada a
partir de las representaciones parcialmente esquemáticas de varios
ejemplos de realización.
Se muestran:
- en la Fig. 1 una vista en perspectiva del
implante intervertebral en estado ensamblado;
- en la Fig. 2 una vista en perspectiva del
implante intervertebral en estado desmontado;
- en la Fig. 3 un corte longitudinal a través
del eje de rotación del implante intervertebral de la Fig. 1;
- en la Fig. 4 una vista frontal del implante
intervertebral de la Fig. 1;
- en la Fig. 5 un corte longitudinal a través
del eje de rotación del implante intervertebral de la Fig. 1 en
estado desmontado; y
- en la Fig. 6 una vista frontal del implante
intervertebral de la Fig. 1 en estado desmontado.
El implante intervertebral 1 representado en las
figuras 1-6 consiste esencialmente en una parte
inferior 2 del implante, una parte superior 5 del implante y un
anillo 9 colocado entre ambas partes del implante 2, 5. La parte
inferior 2 del implante posee una cavidad cilíndrica hueca 3 con una
cubierta 14 y un eje de rotación 11 y una sección de aposición 4
destinada a ser colocada en la superficie de cubierta de la vértebra
inferior contigua. La parte superior 5 del implante posee un mango
esencialmente cilíndrico circular 6 con rosca exterior 7 y eje de
rotación 11, que se extiende a la cavidad 3 de la parte inferior 2
del implante, y una sección de aposición 8 destinada a ser colocada
en la superficie de cubierta de la vértebra superior contigua. Ambas
partes del implante están aseguradas contra la rotación en torno a
su eje de rotación 11.
El anillo 9 posee una rosca interior 10, la cual
coopera con la rosca exterior 7 de la parte superior 5 del
implante, donde ambas partes 2, 5 del implante y el anillo 9 se
colocan coaxialmente a lo largo de su eje de rotación común 11, de
manera que al girar el anillo 9 en la rosca exterior 7 del mango 6,
la distancia entre ambas secciones de aposición 4, 8 puede cambiar
continuamente. El anillo 9 se une axialmente de forma firme, pero
con movimiento rotativo, a la parte inferior 2 del implante mediante
una unión con grapa.
Como se representa en la figura 3, la unión con
grapa está compuesta por un corte trasero 21 en forma de cuerno en
el anillo 9 y una correspondiente protuberancia 22 en forma de
anillo en el extremo libre de la cubierta 14 de la parte inferior 2
del implante.
El anillo 9 posee una superficie 12 de anillo
circular inferior situada en relación con la parte inferior 2 del
implante, y una superficie de anillo circular superior 13 situada en
relación con la parte superior del implante, donde la superficie de
anillo circular inferior 12 posee una corona dentada 23 con
dentadura recta, que es apropiada para recibir un piñón
correspondiente.
La cubierta 14 de la parte inferior 2 del
implante posee una hendidura radial 15, la cual es apropiada para
colocar y soportar un piñón (no representado en la figura) en
relación con la corona dentada 23.
Para asegurar contra la rotación la parte
superior del implante 5 con respecto a la parte inferior 2 del
implante, el mango cilíndrico circular 6 de la parte superior 5 del
implante posee una ranura guía 16 paralela al eje de rotación 11 y
la cubierta 14 de la parte inferior 2 del implante posee en su lado
interior un elemento guía 17 (Fig. 2), que sobresale hacia adentro y
que penetra en la ranura guía 16. Dicho elemento guía se puede
insertar preferentemente en la cubierta 14 a través de una
perforación radial 18.
La rosca exterior 7 del mango cilíndrico
circular 6 y la rosca interior 10 del anillo 9 son autobloqueables.
La inclinación de la rosca exterior 7 y de la rosca interior 10
alcanza 0,7 mm. Tanto la rosca exterior 7 como la rosca interior 10
están concebidas como rosca a derecha de dos pasos.
Como se representa en la figura 3, ambas
secciones de aposición 4, 8 están concebidas como elementos en
forma de placa que se encuentran transversales al eje de rotación 11
con una superficie de aposición 19, 20 destinada a ser colocada en
la superficie de cubierta de la vértebra contigua, donde las
superficies de aposición 19, 20 no están colocadas ortogonalmente al
eje de rotación 11, sino que forman un ángulo de 85º con el eje de
rotación 11. De esta forma, ambas superficies de aposición 19, 20
forman un ángulo de 10º entre sí. La superficie de aposición
superior 20 -como se representa en la figura 3- se arquea hacia
fuera, mientras la superficie de aposición 19 de la parte inferior
del implante es esencialmente plana.
El implante intervertebral puede fabricarse a
partir de materiales de implante tradicionales. Preferentemente se
utiliza PEEK conductor de rayos X, para permitir una valoración de
la fusión mediante rayos X.
El implante intervertebral es apropiado
especialmente como sustituto vertebral en el área cervical y
torácica superior de la columna vertebral.

Claims (16)
1. Implante intervertebral (1) con
- A)
- una parte inferior del implante (2), la cual muestra una cavidad cilíndrica hueca (3) con una cubierta (14) y un eje de rotación (11) y una sección de aposición (4) destinada a ser colocada en la superficie de cubierta de la vértebra inferior contigua.
- B)
- una parte superior del implante (5) con un mango (6) esencialmente en forma cilíndrica circular con rosca exterior (7) y eje de rotación (11), que se extiende a la cavidad (3) de la parte inferior del implante (2), y una sección de aposición (8) destinada a ser colocada en la superficie de cubierta de la vértebra superior contigua; donde
- C)
- ambas partes del implante (2, 5) están aseguradas contra la rotación en torno a su eje de rotación (11); así como
- D)
- un anillo (9) colocado entre ambas partes del implante (2, 5) con rosca interior (10), la cual coopera con la rosca exterior (7) de la parte superior del implante 5, donde
- E)
- ambas partes del implante (2, 5) y el anillo (9) se colocan coaxialmente a lo largo de su eje de rotación común (11), de manera que
- F)
- al girar el anillo (9) en la rosca exterior (7) del mango (6), la distancia entre ambas secciones de aposición (4, 8) puede cambiar continuamente;
caracterizado porque

- el anillo (9) se une axialmente de forma firme, pero con movimiento rotativo, a la parte inferior del implante (2); y
- H)
- el anillo (9) posee una superficie de anillo circular inferior (12) situada en relación con la parte inferior del implante (2), y una superficie de anillo circular superior (13) situada en relación con la parte superior del implante, donde

- la superficie de anillo circular inferior (12) posee una corona dentada (23), que es apropiada para recibir un piñón correspondiente; y

2. Implante intervertebral (1) de acuerdo con la
reivindicación 1, caracterizado porque el anillo (9) se fija
axialmente de manera firme, pero con movimiento rotativo, mediante
una unión con grapa en la sección inferior del implante (2).
3. Implante intervertebral (1) de acuerdo con la
reivindicación 2, caracterizado porque la unión con grapa
está compuesta por un corte trasero (21) en forma de cuerno en el
anillo (9) y una correspondiente protuberancia (22) en forma de
anillo en el extremo libre de la cubierta (14) de la parte inferior
del implante (2).
4. Implante intervertebral (1) de acuerdo con
cualquiera de las reivindicaciones 1 a la 3, caracterizado
porque la cubierta (14) de la parte inferior del implante (2) está
provista de una hendidura radial (15), la cual es apropiada para
colocar y soportar un piñón en relación con la corona dentada
(23).
5. Implante intervertebral (1) de acuerdo con la
reivindicación 4, caracterizado porque la hendidura (15) es
continua.
6. Implante intervertebral (1) de acuerdo con
cualquiera de las reivindicaciones 1 a la 5, caracterizado
porque para asegurar contra la rotación (6) de la parte superior (5)
del implante con respecto a la parte inferior (2) del implante el
mango cilíndrico circular (6) de la parte superior (5) del implante
posee una ranura guía (16) paralela al eje de rotación (11) y la
cubierta (14) de la parte inferior (2) del implante posee en su lado
interior un elemento guía (17) que sobresale hacia adentro y que
penetra en la ranura guía (16). Dicho elemento guía (17) se puede
insertar preferentemente en la cubierta (14) a través de una
perforación radial (18).
7. Implante intervertebral (1) de acuerdo con
cualquiera de las reivindicaciones 1 a la 6, caracterizado
porque el mango cilíndrico circular (6) de la parte superior (5) del
implante posee un volumen más pequeño que la cavidad cilíndrica
hueca (3) de la parte inferior (2) del implante, de manera que ambos
componentes no entren en contacto.

8. Implante intervertebral (1) de acuerdo con
cualquiera de las reivindicaciones 1 a la 7, caracterizado
porque la inclinación de la rosca exterior (7) y de la rosca
interior (10) se encuentran en el intervalo de 0,5 hasta 1,0 mm,
preferentemente de 0,6-0,8 mm.
9. Implante intervertebral (1) de acuerdo con
cualquiera de las reivindicaciones 1 a la 8, caracterizado
porque, tanto la rosca exterior (7) como la rosca interior (10) son
concebidas como rosca a derecha.
10. Implante intervertebral (1) de acuerdo con
cualquiera de las reivindicaciones 1 a la 9, caracterizado
porque, tanto la rosca exterior (7) como la rosca interior (10) son
concebidas como rosca de paso múltiple, preferentemente como rosca
de dos pasos.
11. Implante intervertebral (1) de acuerdo con
cualquiera de las reivindicaciones 1 a la 10, caracterizado
porque ambas secciones de aposición (4, 8) son concebidas como
elementos en forma de placas transversales al eje de rotación (11),
con una superficie de aposición (19, 20) destinada a apoyarse en la
superficie de cubierta de la vertebra contigua.
12. Implante intervertebral (1) de acuerdo con
la reivindicación 11, caracterizado porque las superficies de
aposición (19, 20) no son colocadas de forma ortogonal con respecto
al eje de rotación (11) y forman preferentemente un ángulo de 83º
hasta 85º con el eje de rotación (11).
13. Implante intervertebral (1) de acuerdo con
la reivindicación 12, caracterizado porque ambas superficies
de aposición (19, 20) forman entre sí un ángulo de 10º hasta
14º.
14. Implante intervertebral (1) de acuerdo con
cualquiera de las reivindicaciones 11 a la 13, caracterizado
porque, al menos una de las dos superficies de aposición (19, 20),
preferentemente la de la parte superior del implante (5) se arquea
hacia fuera.
15. Implante intervertebral (1) de acuerdo con
cualquiera de las reivindicaciones 11 a la 13, caracterizado
porque la superficie de aposición (19) de la parte inferior del
implante, es esencialmente plana.
16. Implante intervertebral (1) de acuerdo con
cualquiera de las reivindicaciones 4 a la 15, caracterizado
porque la corona dentada (23) tiene una dentadura recta.
