Description
Prótesis de ensayo modular de cadera para
vástagos largos.
La presente invención se refiere a un
dispositivo usado en artroplastia, tal como un dispositivo de ensayo
para determinar las dimensiones necesarias de un componente de
prótesis femoral.
Una sustitución de cadera o procedimiento
artroplástico exitoso es el resultado, en parte, de la selección de
los componentes de la prótesis de la articulación que estén
dimensionados y que se sitúen de forma que se aproximen o repliquen
estrechamente la geometría y características funcionales de la
articulación de la cadera sana. Normalmente, el procedimiento de
selección de componentes incluye un análisis preoperativo de
imágenes de la articulación. Sin embargo, se ha descubierto que un
valioso adjunto a análisis de imagen es la fijación temporal de uno
o más componentes provisionales al hueso o a los huesos de interés
en una etapa del procedimiento artroplástico antes de la fijación
permanente de la prótesis de la articulación. Se pretende que los
componentes provisionales imiten algunos aspectos de la prótesis de
la articulación permanente con el fin de que el cirujano valide las
medidas y pruebe o "pruebe el molde" con diferentes tamaños y
configuraciones de los componentes. Así, los componentes
provisionales se denominan apropiadamente como "ensayos".
En un procedimiento conocido, se usa un ensayo
de componente femoral de la siguiente manera. Se reseca el extremo
proximal de un fémur, y el canal medular del fémur se escaria. Se
inserta un taladro en el extremo proximal del fémur resecado para
conseguir una cavidad en el interior del hueso dimensionado y
contorneado para recibir un vástago. Sin embargo, antes de retirar
el taladro, se pueden unir un cuello de ensayo o pivote, y una
cabeza de prueba para simular un vástago femoral completo.
Normalmente, se unen sucesivamente al taladro diferentes cuellos y
cabezas de ensayo de diferentes longitudes y geometrías con el fin
de determinar la longitud correcta del cuello y la longitud total
del vástago femoral. Una vez se han determinado dichas longitudes,
se separan del taladro el cuello y cabeza de ensayo, y el taladro se
retira del fémur. Posteriormente, se selecciona un vástago femoral
de longitud adecuada para su inserción en la cavidad definida por el
taladro usando técnicas conocidas de las personas expertas en la
técnica.
Otras técnicas requieren que el taladro sea
retirado del canal medular para permitir un ensayo que se use una
parte del vástago, además del cuello y cabeza de ensayo. Por
ejemplo, la Patente de los Estados Unidos nº 5.100.407 describe un
vástago que incluye un grupo de porciones de ensayo cuello/cuerpo, y
un grupo de porciones de vástago de diferentes longitudes que se
mezclan y emparejan para crear un ensayo aceptable. Sin embargo, la
repetida inserción y extracción de sucesiones de vástagos de ensayo
junto con sucesivos ensamblajes y desensamblajes con respecto al
cuerpo pueden consumir un largo y costoso intervalo de tiempo.
Otro ensayo conocido incluye un vástago al cual
se asegura un collar en puntos sucesivos a lo largo de la longitud
del ensayo hasta que se ha obtenido la longitud de cuello y longitud
de vástago apropiadas. De forma no deseable, este tipo de ensayo
induce imprecisiones en la medida que dan como resultado el
movimiento del vástago, a medida que el collar se encaja con y se
desencaja repetidamente del vástago. Adicionalmente, a medida que
el collar se mueve hacia el extremo distal del vástago, cada vez
está insertado menos vástago en el canal medular, haciendo que el
vástago se vuelva cada vez más inestable y haciendo que sea muy
difícil de conseguir una medida precisa.
Otras consideraciones anatómicas pueden además,
y de manera indeseada, incrementar el número de componentes de
ensayo en un kit. Por ejemplo, los ensayos de vástagos largos de
cadera deben ser diferentes para el fémur izquierdo y derecho
debido a la curvatura o arco de los fémures respectivos. En otras
palabras, un vástago largo izquierdo no se puede usar en el fémur
derecho y viceversa. Se cree que un vástago de ensayo que está
constituido por numerosas partes que se deben seleccionar y
emparejar en diferentes combinaciones, posiblemente varias veces,
es embarazoso, innecesariamente complejo y malgasta tiempo
quirúrgico, entre otras deficiencias.
El Documento
US-A-5002578 describe una prótesis
modular de vástago de cadera con un hombro que se puede sustituir
fabricado como un componente separado.
El Documento
EP-A-0428303 describe una prótesis
modular de cadera con un medio de conexión ajustable entre la parte
de vástago y una porción de módulo.
La presente invención supera las desventajas de
los ensayos conocidos proporcionando un conjunto unificado que
facilita medidas muy precisas con un manejo conveniente y sencillo.
El ensayo no requiere el encaje y desencaje, y es únicamente capaz
de proporcionar una geometría que se aproxima estrechamente a una
cavidad horadada, sin tener en cuenta la altura del ensayo.
La presente invención se define en las
reivindicaciones adjuntas.
Una prótesis de ensayo incluye un vástago que
tiene un extremo proximal y un extremo distal y un cuerpo. El
cuerpo se encaja con el vástago cerca del extremo proximal del
vástago y se puede deslizar con respecto al vástago. Se puede
proporcionar un mecanismo de bloqueo para inhibir el movimiento del
cuerpo con respecto al vástago. Se puede proporcionar un mecanismo
de bloqueo para inhibir el movimiento del cuerpo con respecto al
vástago. Se puede extender radialmente un collar hacia el exterior
desde el vástago para rodear una parte del cuerpo. Cuerpo y vástago
se pueden configurar de manera que el diámetro de la prótesis de
ensayo en un punto entre el collar y el extremo distal del vástago
aumente a medida que el cuerpo se mueve hacia el extremo distal del
vástago y disminuya a medida que el cuerpo se aleje del extremo
distal del vástago. Adicionalmente, el extremo proximal del vástago
puede incluir estructuras de encaje tales como entalladuras opuestas
que se pueden encajar con una herramienta en forma de
horquilla.
En otra forma de realización de la invención, la
prótesis de ensayo incluye una porción de vástago proximal que se
puede encajar con una porción de vástago distal. Una única porción
de vástago distal puede curvarse y ajustarse de forma reversible a
la porción de vástago proximal para proporcionar un vástago de
ensayo largo adecuado para procedimientos tanto del fémur derecho
como del izquierdo. El ensayo puede incluir una porción de cuerpo
deslizable para permitir el ajuste de la altura del ensayo.
En otra forma de realización de la invención
adicional, una prótesis de ensayo incluye un vástago que tiene un
extremo proximal y un extremo distal, y un cuerpo definido por un
pivote. El cuerpo está encajado con el vástago de forma que puede
girar para proporcionar un ensayo único adecuado para uso en un
hueso largo tanto izquierdo como derecho, tal como el fémur.

Haciendo referencia a la Fig. 1, se muestra un
ensayo de acuerdo con la invención en una vista en despiece para
mostrar un vástago 10, un cuerpo 12, un pasador 14, un resorte 16, y
un pulsador 18. El vástago 10 tiene un extremo proximal 20 y un
extremo distal 22, y el cuerpo 12 tiene un extremo proximal que
define un pivote 24 y un extremo distal 26. El cuerpo 12 se puede
encajar en el vástago 10 cerca del extremo proximal del vástago de
forma que se pueda deslizar con respecto al vástago una distancia
predeterminada entre el extremo proximal y el extremo distal del
vástago.
Se puede asociar una guía o rail con cada uno o
ambos del cuerpo 12 y el vástago 10 para guiar el movimiento del
cuerpo con respecto al vástago. Por ejemplo, tal como se muestra en
la Fig. 2, el vástago 10 incluye un rail 28 que se extiende hacia
el exterior desde una primera cara 30 del vástago y se extiende una
distancia predeterminada entre el extremo proximal 20 y el extremo
distal 22 del vástago. Tal como se muestra en la Fig. 1, el cuerpo
10 incluye una muesca 32 para acoger el rail 28. Sin embargo, en
otras formas de realización, el cuerpo 12 incluye una porción
elevada que se puede encajar en un rail que está tallado en el
interior del vástago. Sin tener en cuenta esta configuración, la
complementariedad guía/rail/muesca del cuerpo y vástago se usan
para limitar el movimiento del cuerpo 12 a lo largo de un camino
predeterminado, así como para inhibir movimientos no deseados tales
como el alabeo o el desplazamiento lateral.
Adicionalmente, se puede proporcionar un
mecanismo de cierre para impedir el movimiento del cuerpo 12 con
respecto al vástago 10. Tal como se muestra en las Figs. 1 y 3, el
cuerpo 12 puede incluir un canal 34 perpendicular a la muesca 32 y
que está adaptada para recibir en su interior una porción alargada
del pulsador 18. El pulsador 18 es móvil en el interior del canal
desde una primera posición, en la que una parte del pulsador entra
en contacto y encaja una parte del rail 28 del vástago, hasta una
segunda posición en la que el pulsador se desencaja del rail. Tal
como se muestra en la Fig. 4, el pulsador 18 está desviado respecto
de la primera posición mediante el resorte 16. La Fig. 5 muestra el
pulsador 18 en la segunda posición. El pulsador 18 incluye un
cabezal extendido 36 que encaja con el cuerpo 12 para limitar la
profundidad de inserción del pulsador 18 en el interior del cuerpo.
El pulsador 18 incluye también una porción de corte 38 en el que se
ajustan el pasador 14 y una porción del rail 28. Se verá más
claramente en la Fig. 2 que el rail incluye muescas perpendiculares
40 y 42. Cuando el cuerpo 12 y el vástago 10 se hacen deslizar
entre sí, la porción de corte 38 remonta sobre / a lo largo del
rail 28 hasta alcanzar una muesca 40, 42, momento en el que un
extremo final 44 del pulsador se desvía hacia el interior del
muesca 40, 42. El extremo final 44 se libera de la muesca 40, 42
pulsando el pulsador 18 en el interior del cuerpo 12 con fuerza
suficiente para superar la resistencia del resorte 16.
Haciendo referencia de nuevo a las Figs. 1 y 2,
se extiende un collar 46 radialmente hacia el exterior del vástago.
El collar 16 rodea una porción del cuerpo 12, tal como se muestra en
la Fig. 6, y está dimensionado para colocarse sobre una porción de
hueso resecada tal como se muestra en las Figs. 7 y 8. El collar 46
y el extremo distal del vástago 22 se fijan con cierta distancia de
separación. El collar define una abertura 48 que tiene curvas para
ajustarse a la forma del cuerpo 12 y en el interior de la cual se
aloja el cuerpo.
Volviendo ahora a las Figs. 7 y 8, se ilustra el
uso de la prótesis de ensayo con respecto a los fémures resecados
50 y 52 respectivamente. Una vez se ha preparado el fémur para
recibir el ensayo, el extremo distal 22 del ensayo se inserta en el
interior del canal medular 54, 56. Debe tenerse en cuenta que en
ambas Figs. 7 y 8 se inserta la longitud completa del vástago 10
desde el collar 46 hasta el extremo distal del vástago 22 en el
interior del canal medular. Esto permite que el cirujano verifique
que se ha perforado el canal medular hasta una profundidad
suficiente para acomodar el vástago de sustitución de la cadera. A
continuación, el cirujano desliza el cuerpo 12 con respecto al
vástago según sea necesario para ajustar la altura de cabeza o
distancia entre un punto de referencia 58 sobre un cabezal 60 fijado
al cuerpo 12 y el collar 46 para determinar la longitud del vástago
de la prótesis de cadera. En la Fig. 7, el cuerpo 12 se coloca con
respecto al vástago 10 a una altura de cabeza de 45mm, mientras que
en la Fig. 8 la altura de cabeza es de 55 mm. El ensayo se retira a
continuación del canal medular y se selecciona un vástago de
prótesis de cadera entre un grupo de vástagos de cadera. El vástago
de cadera seleccionado se cementa en el interior del canal
medular.
Debe tenerse en cuenta en estas ilustraciones
que la porción proximal del ensayo por debajo del collar es mas
amplia que una porción intermedia del vástago o su extremo distal 22
para asegurar un ajuste hermético del ensayo en el interior del
fémur. El ensayo es ligeramente más ancho que un vástago de
sustitución real en la sección proximal por debajo del collar para
permitir que el vástago que llena el canal medular que se ha
perforado sea ligeramente más largo que el vástago de sustitución
real (para dejar sitio al cemento óseo que rodea el vástago de
sustitución). Igualmente, el vástago 10 y el cuerpo 12 están
configurados de forma que el diámetro del ensayo en un punto entre
el collar y el extremo distal del vástago aumente a medida que el
cuerpo se mueve hacia el extremo distal del vástago y disminuya a
medida que el cuerpo se aleje del extremo distal del vástago. De
esta forma, sin tener en cuenta dónde se realiza el corte de
resección, las dimensiones y la forma del ensayo se corresponden
con las dimensiones y la forma del canal medular perforado.
Puesto que el ensayo se asienta normalmente en
el interior del canal medular de manera muy ajustada, el ensayo de
acuerdo con la invención incluye además características que son
útiles cuando el ensayo se inserta en o se extrae del canal
medular. Por ejemplo, haciendo referencia a la Fig. 2, el extremo
proximal del vástago incluye una estructura de acople de
herramienta, tal como un par de muescas opuestas 62 y 64 sobre el
vástago. Una herramienta 66, mostrada en la Fig. 10, incluye una
primera porción de horquilla o ahorquillado 68 y un segundo
ahorquillado 70 para encajar las muescas opuestas 62 y 63. La
separación entre los ahorquillados se corresponde con la forma del
cuerpo 12 para permitir que la herramienta se ajuste estrechamente
con el cuerpo. Esto asegura que la herramienta permanezca alineada
axialmente con el cuerpo 12 y el vástago 10. Cada ahorquillado 68 y
70 puede incluir porciones finales en ángulo o brazos 72 y 74,
respectivamente. Un mango 76 proporciona una estructura fácilmente
asible para estirar o empujar la herramienta 66, así como una
superficie adecuada para amartillado. Puesto que las muescas 62, 64
son similares a las muescas del vástago de sustitución, usadas para
orientar los cables de sujeción, se puede usar la misma herramienta
66 para insertar y extraer tanto el ensayo como el vástago de
sustitución.
La Fig. 10 muestra la herramienta 66 colocada
con respecto al ensayo para la inserción del ensayo en el interior
del canal medular, en la que los brazos 72, 74 no están encajados
con las muescas 62, 64, sino que descansan directamente sobre el
collar 46 sobre los lados opuestos del cuerpo 12.
La Fig. 11 muestra la herramienta 66 colocada
con respecto al ensayo para la extracción del ensayo del canal
medular, en la que los brazos 72, 74 están encajados con las muescas
62, 64. La herramienta ahorquillada permite entonces aplicar una
fuerza distribuida y completa al vástago 10 tanto durante la
inserción como durante la extracción.
Otro problema más con los ensayos, en particular
con los ensayos para vástagos largos de cadera, es que el ensayo
debe tener en cuenta la curvatura o desviación del fémur. Un vástago
modular puede, dependiendo de la configuración de la porción de
vástago distal, aumentar la longitud del vástago de ensayo y
proporcionar la curvatura necesaria para una apropiada implantación
femoral.
Por ejemplo, la Fig. 12 es una vista en despiece
de una forma de realización de un ensayo que tiene una porción de
vástago proximal 78, una porción de vástago distal 80, y un elemento
de conexión 82. La porción de vástago proximal 78 puede incluir una
o más de las propiedades anteriormente descritas con respecto al
enganche de un cuerpo 12 o con respecto a una herramienta 66.
Alternativamente, la porción de vástago proximal 78 puede no
incluir ninguna de las anteriores propiedades inventivas, sino en su
lugar otras propiedades que aparecen en los ensayos conocidos en la
técnica.
En la forma de realización mostrada, la porción
de vástago proximal 78 es aproximadamente de la misma longitud que
la porción de vástago 10 que se muestra en las Figs.
1-11 y por tanto se puede usar como se ha descrito
más arriba sin la adición del elemento de conexión 82 o la porción
de vástago distal 82. Sin embargo, cuando la porción de vástago
distal 80 se junta con la porción de vástago proximal 78, la
longitud completa del ensayo es equivalente a la del ensayo
tradicional de pieza única de vástago largo. En otras formas de
realización, la porción de vástago proximal 78 es más larga o más
corta que la porción de vástago 10, y la porción de vástago distal
80 es correspondientemente más larga o más corta que la que se
muestra en La Fig. 12.
Aunque la porción de vástago distal 80 puede ser
paralela o coaxial a la porción de vástago proximal 78, en la forma
de realización que se muestra está curvada o incluye una angulación
de forma que parte o toda de la porción de vástago distal no es
coaxial a la porción de vástago proximal. Se puede conseguir la
curvatura o la angulación mediante la forma de la porción de
vástago distal y/o juntando las porciones de vástago formando un
ángulo. De forma colectiva, la curvatura o angulación del vástago o
vástagos se denomina como "curvado".
Continuando con referencia a la Fig. 12, la
porción de vástago distal 82 se aplica de forma selectiva y
reversible a la porción de vástago proximal 78 para hacer que el
ensayo tenga una curva en una primera o segunda dirección opuesta.
De esta forma, el ensayo se puede usar tanto para el fémur izquierdo
como para el derecho sin necesidad de una parte adicional. En la
forma de realización que se muestra, las estructuras que facilitan
la reversibilidad incluyen una cola 84 sobre la porción de vástago
distal 80 que se ajusta en el interior de una ranura 86 definida
mediante la porción de vástago proximal 78. La cola 84 y la ranura
86 aseguran que las porciones del vástago proximal y distal se
emparejan en una orientación precisa, y que la orientación no puede
cambiarse de manera inadvertida. Por ejemplo, cuando las porciones
de vástago se unen simplemente por fricción ajustando un elemento
cilíndrico en el interior de un orificio perforado, o mediante una
conexión roscada, no puede asegurarse la orientación precisa.
La ranura 86 y la cola 84 se pueden localizar
sobre las porciones de diámetro reducido 88 y 90 de la respectiva
porción del vástago proximal 78 y la porción de vástago distal 80.
Las porciones de diámetro reducido 88 y 90 se pueden roscar y
encajarse en el interior de un orificio roscado 92 del elemento de
conexión 82. El elemento de conexión 82 puede incluir texturas,
tales como aristas alargadas alternativas 94 y depresiones 96 para
permitir que el elemento de conexión se pueda coger con la mano o
con una herramienta y hacerlo girar con respecto a la porción o
porciones de vástago para crear una ensambladura entre las porciones
de vástago. Sin embargo, se contemplan otras formas de realización
que permiten que las porciones de vástago se unan en una o dos o
más orientaciones precisas y se mantengan juntas sin un elemento de
conexión diferenciado.
La Fig. 13 representa el ensayo de la Fig. 12 en
estado ensamblado. Debe tenerse en cuenta que el elemento de
conexión 82 tiene un diámetro máximo que se aproxima al de las
porciones de vástago dónde se unen al elemento de conexión.
La Fig. 14 es una vista en corte parcial de una
conexión entre la porción de vástago proximal 78 y la porción de
vástago distal 80 que muestra claramente el ensamblado de la cola 84
con la ranura 86. Aunque la cola y ranura que se muestran
proporcionan una conexión reversible excelente, se contemplan otras
estructuras para unir las porciones de vástago, y la conexión
concreta no es una limitación de la invención.
La Fig. 15 es una vista en despiece de otra
forma adicional de realización de un ensayo, en la que un vástago
98 puede rotar con respecto a un cuerpo 100 que define o incluye un
pivote 102. El vástago 98 incluye un extremo proximal 104 y un
extremo distal 106. El vástago 98 se curva para corrección anatómica
según requiera su longitud.
Las porciones de diámetro reducido 108, 110, 112
del vástago 98 son capaces de insertarse en el interior del
orificio 114 en el interior del cuerpo 100. Un primer mecanismo de
cierre 116 se puede encajar en el cuerpo para impedir la retirada
del extremo proximal 104 del vástago 98 del orificio 114 y para
proporcionar al extremo proximal un intervalo limitado de
desplazamiento en el interior del orificio. Se usa un pasador 118,
que se puede insertar a través de una abertura 120 en el cuerpo
100, para limitar el movimiento del primer mecanismo de cierre 116
con respecto al cuerpo.
La Fig. 16 muestra el pasador 118 con mayor
detalle. El pasador 118 es un cuerpo esencialmente cilíndrico 122
que tiene una protuberancia 124 que se extiende desde un extremo. Un
elemento de desviación 126, tal como un resorte o cuerpo elástico,
se coloca en el interior el cuerpo 122 de forma que ejerza una
desviación hacia fuera sobre la protuberancia 124. El exterior del
cuerpo puede incluir estrías 128 que complementan otras estrías (no
mostradas) en la abertura de recepción 120 del cuerpo para asegurar
el ensamblaje del pasador 118 en el interior el cuerpo 110. Se
puede colocar una ranura 130, manguito, u otro punto de ajuste para
una herramienta, tal como un destornillador o una llave Allen, en
un extremo del pasador para facilitar el emparejamiento de pasador y
cuerpo.
La Fig. 17 es una vista en planta del primer
mecanismo de cierre 116. El mecanismo de cierre 116 incluye una
región 132a que tiene un primer diámetro y región 132b que tiene un
segundo diámetro, en el que el primer diámetro es más grande que el
segundo diámetro. Cuando el primer mecanismo de cierre se ensambla
con el cuerpo 100, la región 132 define una porción del orificio
114 (o una extensión del anterior) a través del cuerpo. El
movimiento del primer mecanismo de cierre 116 con respecto al cuerpo
100, tal como se muestra y describe con respecto a las Figs.
18-21 siguientes, aumentan o disminuyen el diámetro
local del orificio 114.
Se proporcionan dactileras 134 y 136 para

116 con respecto al cuerpo 100. Un brazo 138 se extiende
perpendicularmente desde el plano definido por las dactileras 134,
136 en el interior de o alrededor de una porción del cuerpo 100. Se
extiende un nudo 140 desde el brazo 138 para proporcionar una
primera superficie de agarre 142 sobre el lado opuesto del orificio
114 desde las dactileras. Una segunda superficie de agarre 144 se
opone a la primera superficie de agarre 142.
Cuando el primer dispositivo de cierre 116 y
pasador 118 se ensamblan con el cuerpo 100, la primera y segunda
superficies de agarre 142 y 144, respectivamente, se encuentran en
los lados opuestos de la protuberancia 124. La protuberancia 124 se
desvía contra una cara 146 del brazo 138, y la separación entre las
superficies de agarre 140, 144 define el limite de desplazamiento
del primer dispositivo de cierre. Se dimensiona un rebaje, ranura o
lengüeta 148 en la cara 146 para alojar al menos una parte de la
protuberancia 124. Cuando el primer dispositivo de cierre 116 se
coloca de manera que se aloje la protuberancia 124 en el interior de
la lengüeta 148, se inhibe el movimiento del primer dispositivo de
cierre hasta que se aplica fuerza suficiente al primer dispositivo
de cierre (estirando) para superar la resistencia aplicada a la
protuberancia por el elemento de resorte 126. Un pasador de
retención 150, que se ajusta en el interior a orificio (no mostrado)
en el cuerpo 100, se proporciona sobre el primer dispositivo de
cierre 116 para evitar que el primer dispositivo de cierre se
desplace desde el cuerpo en la dirección de la resistencia del
resorte.
Haciendo referencia ahora a la Fig. 18, se
ilustra un ensayo completamente ensamblado en el estado cerrado y
configurado para su implantación. En esta vista, se muestra un
segundo dispositivo de cierre que incluye una cola 152 asociada con
el vástago 98, que se asienta en el interior una muesca 154 asociada
con el cuerpo 100. Se proporciona una segunda muesca (no mostrada)
en el cuerpo 100 directamente opuesta (a 180 grados) a la primera
muesca 154. El segundo mecanismo de cierre inhibe la rotación del
cuerpo 100 con respecto al vástago cuando la muesca y cola están
emparejadas En otras formas de realización, la cola está asociada
con el cuerpo y las muescas están asociadas con el vástago.
Se puede proporcionar una ranura 156 que da
acceso al orificio 114 en el cuerpo 100 para permitir retirar
tejido o materia extraña del orificio, o simplemente para limpieza,
sin el desensamblaje completo del ensayo.
Debe tenerse en cuenta que, en el estado
cerrado, el primer dispositivo de cierre 116 está sustancialmente
encajado con una porción del cuerpo 100 o en el interior del perfil
geométrico del cuerpo. En la forma de realización que se muestra,
el primer dispositivo de cierre está tapado con un elemento de aleta
158. Sin embargo, la existencia de una aleta no es importante para
la invención, ni tampoco lo es la forma particular del cuerpo.

Tal como se muestra en la Fig. 19, en el estado
cerrado, la porción de diámetro reducido 110 del vástago está
rodeada por la región 132b del dispositivo de cierre 116 que tiene
un diámetro más pequeño que la porción 112 del vástago. De esta
forma, la porción 112 queda sujeta o soportada por el primer
mecanismo de cierre 116 a su mayor distancia insertada en el
interior del orificio 114 y el vástago 98 no se puede sacar del
cuerpo 100 lo suficiente para dislocar la cola 152 de la muesca
154. La protuberancia 124 del pasador 118 se asienta en la lengüeta
148 (ver la Fig. 17). De esta forma, el pasador 118 actúa como un
mecanismo de cierre para el primer mecanismo de cierre 116.
La Fig. 19 representa también un orificio
pasante 160 a través del pivote. No solo el orificio 160 identifica
rápidamente el dispositivo como un ensayo, sino que también permite
la inserción de otra herramienta (no mostrada) que se va a insertar
en el interior o a través del orificio 160 para facilitar la
extracción del ensayo de un canal taladrado en el hueso en el que
se haya implantado temporalmente. Una ranura 162 permite limpiar el
ensayo sin el desensamblaje completo del ensayo, o para permitir
retirar tejido corporal o materia extraña del orificio 114.
Haciendo referencia ahora a la Fig. 20, se
ilustra un ensayo completamente ensamblado en el estado no cerrado,
en el que el primer dispositivo de cierre 116 se extrae del cuerpo
100. El vástago 98 se hace girar en la dirección de la flecha para
alinear la segunda muesca (colocada en oposición a la muesca 154)
con la cola 152 (ver la Fig. 18) para configurar el vástago largo
de ensayo para un fémur izquierdo. En el estado no cerrado, la cola
152 no queda asentada en el interior de la muesca y el vástago 98
gira libremente con respecto al cuerpo 100.
Tal como se muestra en la Fig. 21, en el estado
no cerrado, el primer dispositivo de cierre 116 se extrae del
cuerpo 100 lo suficiente para permitir que la porción de diámetro
reducido 112 se mueva en el interior del área del orificio 114
definido en parte por la región 132a del dispositivo de cierre 116.
A medida que el vástago desciende (alejándose del cuerpo), el
segundo dispositivo de cierre (cola y muesca) se desensambla. Sin
embargo, puesto que el vástago 98 finaliza con una cabeza 164 que es
más ancha que el diámetro del orificio definido en parte por la
región 132a, el vástago no se separa completamente del cuerpo. Tal
como se muestra en la Fig. 21, la protuberancia 124 del pasador 118
se desensambla de la lengüeta 148 y la protuberancia y pasador
confrontan una superficie de sujeción 142.
Se derivan muchas ventajas funcionales de las
propiedades descritas más arriba. Por ejemplo, el cierre de muesca
y cola anti-rotacional únicamente permite dos
configuraciones ensambladas, en las que cada configuración
proporciona la geometría adecuada. No es posible una posición
inadvertida o "desalineada". Aunque se describen una muesca y
una cola, se contemplan otras geometrías y estructuras que realicen
una función similar. Igualmente, debe tenerse en cuenta que, como
los primero y segundo dispositivos de cierre quedan completamente en
el interior de la geometría del implante cuando están cerrados, el
ensayo replica estrechamente el perfil de un implante real. Además,
puesto que el ensayo puede configurarse de forma que una porción de
diámetro reducido de la porción de vástago penetre en el interior
de la porción del cuerpo cerca del extremo proximal del ensayo, la
porción de vástago se recupera fácilmente si el cuerpo y vástago se
separan, puesto que el extremo proximal del vástago está cercano, o
sobresale de la entrada del canal de hueso perforado.
Aunque la invención se ha demostrado y descrito
con respecto a las formas de realización ejemplares de la misma, se
pueden realizar otros cambios, omisiones y adiciones diferentes de
la misma sin separarse del ámbito de la invención.
Claims (3)
1. Un ensayo protésico que comprende:
- un vástago (10, 98) que tiene un extremo proximal (104) y un extremo distal (106);
- un cuerpo (12, 100) que incluye un pivote (24, 102), de forma que el cuerpo (12, 100) pueda ensamblarse de forma giratoria con el vástago (10, 98); y
- un primer mecanismo de cierre (116) adaptado para inhibir el movimiento longitudinal del vástago (10, 98) con respecto al cuerpo (12, 100),
en el que el extremo proximal (104) del vástago
(10, 98) incluye una porción de diámetro reducido (108, 110, 112);
y en el que el cuerpo (12, 100) incluye un orificio (114)
dimensionado para recibir la porción de diámetro reducido del
vástago (10, 98), y el primer mecanismo de cierre (116) está
adaptado para inhibir el movimiento de la porción de diámetro
reducido (108, 110, 112) del vástago (10, 98); en el interior del
orificio (114) del cuerpo (12, 100),
caracterizado en que el primer mecanismo
de cierre (116) define una porción (132) del orificio (114) y en el
que el primer mecanismo de cierre (116) está adaptado para ser
movido desde una primera posición a una segunda posición para
aumentar el diámetro del orificio (114).
2. El ensayo protésico de la reivindicación 1,
que comprende además un segundo mecanismo de cierre (118) adaptado
para retener el primer mecanismo de cierre (116) en la primera
posición.
3. El ensayo protésico de la reivindicación 2,
en el que el segundo mecanismo de cierre (118) incluye un elemento
(124) desviado contra el primer mecanismo de cierre (116) y que se
ensambla con el anterior cuando el primer mecanismo de cierre (116)
está en la primera posición.
