Abstract

Dispositivos médicos que tienen un recubrimiento
polimérico duradero y lubricado con un polímero de silicona y
polímeros hidrofóbicos que no contienen silicona.
Esta solicitud reivindica el beneficio de la
solicitud provisional con No. de serie 60/418054, presentada el 11
de Octubre del 2002.
Esta invención se refiere a dispositivos médicos
recubiertos. Más específicamente, se refiere a dispositivos médicos
que tienen una superficie de contacto expuesta repetidamente a un
tejido corporal, y en los que la superficie de contacto tiene un
recubrimiento para aumentar la lubricación, a la vez que
proporcionan la duración necesaria para proporcionar esa
lubricación durante la exposición repetida al tejido.
Se requiere que los dispositivos médicos que
entran en contacto repetidamente con un tejido corporal, por
ejemplo las agujas quirúrgicas, estén lubricados, a la vez que sean
duraderos para resistir los múltiples contactos con un tejido. Sin
embargo, la lubricación se sacrifica a menudo a expensas de hacer un
recubrimiento más duradero que se adhiera bien a los dispositivos
médicos. Existen muchos materiales de recubrimiento que son
extremadamente lubricantes, pero, o bien no se adhieren bien a los
sustratos deseados o bien se desprenden fácilmente de los sustratos
durante su uso. De la misma forma, existen muchos recubrimientos
extremadamente duraderos, pero estos recubrimientos no se
consideran lubricantes. Consecuentemente, los dispositivos médicos
recubiertos convencionales son incapaces de mantener la lubricación
a medida que se utilizan sucesivamente, ya que los recubrimientos
se desprenden, tal como una aguja quirúrgica que se pasa a través
del tejidos numerosas veces.
Un material lubricante altamente ventajoso, que
se usa como recubrimiento para muchos dispositivos médicos, tales
como agujas quirúrgicas y catéteres, es la silicona. Sin embargo,
como se discutió anteriormente en el contexto de los materiales de
recubrimiento lubricantes convencionales, el recubrimiento de
silicona se desprende generalmente del dispositivo médico con la
exposición repetida al tejido. Por ejemplo, cuando una aguja
recubierta con silicona se pasa sucesivamente a través del tejido
durante el cierre de una herida, el recubrimiento de silicona se
desprende apreciablemente con cada pase, resultando en un incremento
en la fuerza de penetración cada vez que la aguja se pasa a través
del tejido.
Abundan ejemplos en conexión con aumentar las
propiedades de los dispositivos médicos recubiertos con silicona
mediante la adición de ingredientes que modifiquen las propiedades
globales del recubrimiento de silicona. Por ejemplo, se han
mezclado tensioactivos hidrófilos, que incluyen los tensioactivos
poliméricos hidrófilos, con soluciones de recubrimiento de silicona
y se han aplicado a dispositivos médicos, tales como catéteres y
agujas. Desafortunadamente, la incorporación de estos polímeros
hidrófilos no aumenta significativamente la duración del
recubrimiento de silicona sobre estos dispositivos médicos.
A la vista de la incapacidad para desarrollar un
recubrimiento para un dispositivo médico que proporcione
lubricación sin sacrificar la duración del recubrimiento a lo largo
de usos sucesivos, lo que se necesita es un dispositivo médico
recubierto, en particular un dispositivo médico recubierto con una
silicona, que proporcione estas propiedades simultáneamente.
La invención proporciona un dispositivo médico
que tiene una superficie de contacto expuesta repetidamente a un
tejido corporal, estando la superficie de contacto recubierta con
una mezcla de recubrimiento que comprende de aproximadamente el 40
a aproximadamente el 86 por ciento en peso de un polímero de
silicona y de aproximadamente el 14 a aproximadamente el 60 por
ciento en peso de un polímero hidrofóbico que no contiene
silicona.
Sorprendentemente, la incorporación de un
polímero hidrófobo que no contienen silicona en el recubrimiento
polimérico con el que se recubre la superficie de contacto del
dispositivo médico proporciona a la superficie de contacto con el
requisito de lubricación, pero sin sacrificar la duración del
recubrimiento lubricante durante la exposición repetida al tejido
corporal. Además, las ventajas de incorporar el polímero hidrofóbico
que no contiene silicona en el recubrimiento contrastan
significativamente con los recubrimientos de silicona
convencionales, que incorporan algunas veces polímeros
hidrófilos.
El dispositivo médico de esta invención se puede
utilizar en cualquier aplicación sanitaria en la que sea deseable
proporcionar una superficie de contacto lubricada que presente
duración durante el uso sucesivo, por ejemplo, una aguja quirúrgica
que se pasa a través de un tejido corporal múltiples veces. El
dispositivo médico de esta invención puede estar compuesto de

El dispositivo médico de esta invención puede
ser cualquier dispositivo, adaptado para su uso en los campos

repetidamente a un tejido corporal. Alternativamente, un
dispositivo médico según esta invención puede ser cualquier
dispositivo adaptado para facilitar la extracción de fluido
intersticial, sangre u otros fluidos corporales para la medida de
analitos, que incluyen, por ejemplo agujas huecas que se colocan a
través de la superficie de la piel con el propósito de extraer
sangre o fluido intersticial continua o periódicamente con el
propósito de realizar medidas glucosa. Los ejemplos de dispositivos
médicos incluidos en esta clase son dispositivos médicos, que se
insertan o se implantan en el cuerpo en un sitio diana, o entran en
contacto de otra forma con un tejido sobre o en el cuerpo. Los
ejemplos específicos de dispositivos médicos en el ámbito de esta
invención incluyen agujas quirúrgicas, hojas de bisturí, stents y
catéteres. Los dispositivos médicos tienen preferentemente una
superficie de contacto que esta expuesta a una aplicación de
deslizamiento o corte durante su uso. En una forma de realización
preferida, el dispositivo médico tienen una superficie de contacto,
que incluye una región que penetra, o una superficie afilada, en la
que estas regiones se convierten en "romas" durante el uso
repetido cuando un recubrimiento lubricante convencional, tal como
un recubrimiento de silicona, se aplica a esta superficie. Hacerse
romo en esta aplicación se refiere a un incremento en la fuerza o
fricción con el uso repetido.
El dispositivo médico más preferido que tiene
una superficie de contacto expuesta repetidamente a un tejido
corporal es una aguja quirúrgica. Las agujas quirúrgicas preferidas
están fabricadas típicamente con una aleación metálica, en
particular, pero no limitado a, acero inoxidable tal como acero
inoxidable 420, acero inoxidable 455, acero inoxidable Ethalloy
(Patentes de EE.UU. Nos. 5.000.912 y 5.651.843) y acero inoxidable
302. Se pueden utilizar distintas geometrías de la aguja, por
ejemplo, agujas de punta cilíndricas, de corte sesgado, de extremo
afilado, con forma de bayoneta, y con configuraciones en I, de
cuerpo cuadrado y de cuerpo redondo.
El recubrimiento de la superficie de contacto
del dispositivo médico contiene preferentemente dos o más materiales
poliméricos. Uno de los polímeros es una silicona. Las siliconas
poliméricas son los polisiloxanos, en particular los
polialquilsiloxanos. Un polialquilsiloxano preferido es un
polidimetilsiloxano. El polímero de silicona más preferido es
polidimetilsiloxano que está terminado en hidroxi, tal como MED4 162
de NuSil Technology. El polímero de silicona puede contener
pequeñas cantidades (típicamente <5% en peso) de otros
ingredientes, que son compatibles con el polímero.
Por ejemplo, se puede incluir en el
recubrimiento metilhidrógeno siloxano. Los ingredientes menores son
típicamente catalizadores o compuestos que contienen silicona.
El segundo polímero par el recubrimiento sobre
la superficie de contacto es un polímero hidrofóbico que no
contiene silicona. Como se utiliza en este documento, un polímero
hidrofóbico es un polímero al que le falta afinidad por agua, y son
fácilmente conocidos y reconocidos por los expertos en la técnica
(contrastando, por ejemplo, con los tensioactivos poliméricos, y
los polialquilen glicoles, tales como polietilenglicol, que son de
naturaleza hidrófila). Más específicamente, los autores del
documento piensan que un polímero hidrofóbico como el referido por
esta invención es generalmente un polímero con una tensión crítica
de superficie menor de aproximadamente 50 mN/m. En una forma de
realización preferida de esta invención, el polímero hidrofóbico es
un polímero termoplástico. Preferentemente, el polímero hidrofóbico
tienen una temperatura de fusión, reblandecimiento o de transición
al vidrio que es suficientemente baja para permitir que el polímero
fluya a la temperatura necesaria para curar la porción de silicona
del recubrimiento. Consecuentemente, el polímero hidrofóbico tiene,
idealmente, un punto de fusión menor de aproximadamente 210ºC, en el
caso de polímero hidrofóbicos cristalinos, o una temperatura de
transición al vidrio menor de aproximadamente 100ºC, en el caso de
polímeros hidrofóbicos amorfos. Los ejemplos de polímeros
hidrofóbicos que no contienen silicona que se pueden utilizar
incluyen, pero no se limitan a, polipropileno, polietileno, ceras y
policaprolactona. El polímero hidrofóbico que no contienen silicona
más preferido es una mezcla de cera de polipropileno hidrocarbonada
tal como Micromatte 2000 producida por Micropowders, Inc. El
polímero hidrofóbico que no contiene silicona puede estar en la
forma de un sólido, y en particular puede estar en la forma de un
polvo micronizado. El recubrimiento preferido es una mezcla de
recubrimientos por la facilidad con la que se puede aplicar el
recubrimiento. La mezcla de recubrimientos se puede preparar por
cualquier medio convencional par la combinación de dos componentes
poliméricos primarios. Por ejemplo, los polímeros se pueden
combinar mediante las técnicas de mezclado, pulverización o
depósito. Aunque en muchos casos, el recubrimiento se puede aplicar
puro, los medios preferidos para la combinación de polímeros es
mezclar los polímeros con uno o más solventes. En una forma de
realización preferida, el solvente o mezcla de solventes que se
seleccionan disolverán el polímero de silicona. El solvente o mezcla
de solventes seleccionados puede o no disolver el polímero
hidrofóbico que no contiene silicona. Si el polímero hidrofóbico
que no contienen silicona no se disuelve en el solvente, entonces
los componentes forman una suspensión. Los solventes preferidos son
xileno y Exxon Isopar-k. Idealmente, el sistema
solvente será relativamente volátil en condiciones ambiente.
La mezcla de recubrimiento más preferida
contiene aproximadamente, en peso, el 74,86% de Exxon isopark, el
14,25% de xileno, el 5,7% de polidimetilsiloxano terminado en
hidroxi, el 5% de una mezcla de cera de propileno hidrocarobonada
(Micromatte 2000) y el 0,19% de metilhidrógeno siloxano. Los
intervalos aceptable aproximados, en peso, de los ingredientes en
la mezcla preferida son, en peso, del 1-20%
polidimetilsiloxano terminado en hidroxi, del
0,5-20% cera de propileno hidrocarobonada y del
0-0,4% de metilhidrógeno siloxano, siendo la
relación final entre Exxon isopark y xileno en la solución,
aproximadamente, pero no limitada a, 5,25:1. Los intervalos
aproximados preferidos son, en peso, del 2-12% de
polidimetilsiloxano terminado en hidroxi, del 1-9%
de la mezcla de cera de polipropileno hidrocarbonada, y del
0-0,4% de metilhidrógeno siloxano, siendo la
relación final entre Exxon isopark y xileno en la solución,
aproximadamente, pero no limitada a, 5,25:1.
Si el polímero de silicona seleccionado está en
estado líquido, entonces es posible que no sea necesaria la adición
de un solvente para mezclar adecuadamente el polímero de silicona
con el polímero(s) hidrofóbico que no contienen silicona.
La mezcla de recubrimiento se puede aplicar en
la superficie de contacto del dispositivo médico utilizando
diversos procedimientos tales como inmersión, pulverización y
barnizado con pincel. El procedimiento preferido es sumergir el
dispositivo médico en una solución o suspensión de mezcla de
recubrimiento. En esta forma de realización preferida, el
recubrimiento debe estar expuesto posteriormente a un ciclo térmico
para volatilizar el solvente o mezcla de solventes y curar el
polímero de silicona si se requiere. La temperatura puede oscilar
entre aproximadamente 140ºC y aproximadamente 210ºC, pero no se
limita a este intervalo. La temperatura preferida es típicamente
suficientemente alta para provocar que se fundan los polímeros
hidrofóbicos que no contienen silicona (materiales cristalinos o
semi-cristalinos) o para ablandar y fluidificar
(materiales amorfos), pero no suficientemente alta para tener un
efecto perjudicial en la capacidad de lubricación o en la duración
de los componentes poliméricos. Por ejemplo, la exposición a
temperaturas mayores de aproximadamente 210ºC durante periodos de
tiempo extensos provocan que algunas siliconas se oxiden y formen
sílice, que podría disminuir la capacidad de lubricación del
revestimiento. El tiempo de exposición a la temperatura variarán con
según los polímeros que se utilicen. En algunas formas de
realización, puede ser beneficioso fundir el polímero hidrofóbico
que no contiene silicona cuando el polímero seleccionado esta en una
forma sólida. Por tanto, puede ser necesario que la temperatura
utilizada durante el ciclo de secado/curación, sea más alta que el
punto de fusión del polímero hidrofóbico que no contiene silicona
seleccionado (para materiales cristalinos o
semi-cristalinos), pero no siempre es necesario que
esté completamente fundido. En el caso de materiales amorfos, la
temperatura preferida será tal que sea suficientemente por encima de
la temperatura de transición al vidrio para provocar que fluya el
polímero hidrofóbico que no contiene silicona. Por ejemplo, en el
caso de que el polímero hidrofóbico que no contiene silicona sea
poliestireno, que tiene una tensión de superficie crítica menor de
50 mN/m, pero un temperatura de transición al vidrio muy alta
(\sim100ºC), una temperatura de 200ºC es insuficiente para
producir un incremento en la duración cuando el poliestireno se
combina con silicona. Esto es debido probablemente a la incapacidad
para provocar que el poliestireno fluya a 200ºC. Además, la rigidez
muy alta asociada con el poliestireno, interfiere probablemente con
la capacidad para mejorar significativamente la lubricación. Otro
ejemplo se refiere al uso de politetrafluoroetileno (PTFE), que
tiene un punto de fusión de 325ºC aproximadamente. Cuando el PTFE
se añade a una solución de silicona, la lubricación, medida como la
fuerza de penetración del primer pase de agujas quirúrgicas, puede
disminuir como resultado de una lubricación mejorada, pero la
duración del recubrimiento puede ser más difícil de mejorar.
El uso de una temperatura suficiente para
permitir la fusión o un flujo suficiente de uno o más de los
polímeros seleccionados en la mezcla de recubrimiento, permite que
estos polímeros fluyan y produce una morfología de recubrimiento en
la que la silicona y los polímeros hidrofóbicos que no contienen
silicona se intercalen, de manera que el polímero de silicona
forma típicamente una fase continua y el polímero hidrofóbico que no
contiene silicona forma una fase discontinua. Sin embargo, esto
depende de las cantidades relativas de cada polímero. Si hay una
cantidad suficiente del polímero hidrofóbico que no contiene
silicona, entonces éste puede formar una fase continua.
Independientemente de si cualquiera de los polímeros seleccionados
se funde, el recubrimiento es un recubrimiento en múltiples fases
en la mayoría de las formas de realización preferidas. Un
recubrimiento en múltiples fases es aquel que contienen dos o más
fases en el recubrimiento después de la exposición a un ciclo
termal para volatilizar los solventes y/o curar el componente de
silicona del recubrimiento.
Una vez que se aplica el recubrimiento en la
superficie de contacto del dispositivo médico, y cualquiera de los
solventes o mezcla de solventes se ha eliminado, se aplica una
cantidad de recubrimiento efectiva para proporcionar lubricación a
la superficie de contacto sin sacrificar apreciablemente la duración
del recubrimiento y consecuentemente sus propiedades de lubricación
durante el uso sucesivo. Como se utiliza en este documento, una
cantidad efectiva es aquella cantidad que proporcionara las
propiedades aumentadas a la superficie de contacto del dispositivo
médico como se describió en la frase precedente. La cantidad
aproximada de recubrimiento en la superficie de contacto del
dispositivo médico (sin solventes) es del 40-86% en
peso del polímero de silicona y del 14-60% del
polímero hidrofóbico que no contiene silicona. El intervalo más
preferido oscila entre el 44 y el 66% en peso del polímero de
silicona y del 34 al 56% del polímero hidrofóbico que no contiene
silicona.
La eficacia del recubrimiento para dispositivos
médicos se puede analizar con diversos análisis de fricción y/o
adhesión. En el caso de agujas quirúrgicas, la eficacia del
recubrimiento y su integridad se evalúan utilizando un dispositivo
de punta de penetración. Una aguja quirúrgica recubierta se sujeta
utilizando unas pinzas con bloqueo automático o un dispositivo de
sujeción similar. La aguja recubierta se pasa a través de un medio
que es representativo de un tejido humano. La aguja entera no pasa a
través del medio; sólo se pasa a través del medio aproximadamente
la mitad de la longitud de la aguja y se retira posteriormente antes
del siguiente pase. El medio es típicamente un tipo de polímero o
piel sintética (Porvair o Permair en este ejemplo). La aguja es
posteriormente retirada del medio. Un análisis típico incluye la
utilización de 10 agujas que se pasan individualmente a través del
medio, 10 veces cada una. La fuerza máxima en cada pase se registra
y se utiliza como una medida de la eficacia del recubrimiento.
Típicamente, la fuerza de penetración aumenta con cada pase
sucesivo a medida que el recubrimiento se desprende de la aguja.
Detalles adicionales del equipo de análisis y del procedimiento se
pueden encontrar en la Patente de EE.UU. 5.181.416.
Las agujas quirúrgicas Ethicon
BV-1 (0,025 mm de diámetro, sin unir a una sutura,
ver más adelante para una descripción más detallada) se recubrieron
con una de dos mezclas de recubrimiento. La primera mezcla de
recubrimiento (Mezcla #1) contenía aproximadamente, en peso, 6% de
polidimetilsiloxano terminado en hidroxi, 0,2% de metilhidrógeno
siloxano, 15% de xileno y 78,8% del hidrocarburo isoparafínico
sintético, Exxon Isopar-k. Este es un recubrimiento
convencional de silicona de una fase (polidemetilsiloxano), después
de que se volatizan los solventes, que típicamente proporciona una
lubricación excelente pero sólo una moderada duración. Esta mezcla
de recubrimiento se consideró la línea base estándar y se aplicó
con propósitos comparativos. La segunda mezcla de recubrimiento
(Mezcla #2) contenía aproximadamente, en peso, 5,7% de
polidimetilsiloxano terminado en hidroxi, 0,19% de metilhidrógeno
siloxano, 14,25% de xileno, 74,86% del hidrocarburo isoparafínico
sintético, Exxon Isopar-k, y 5% de una mezcla en
polvo de cera de polipropileno hidrocarbonada, con una mediana del
tamaño de partícula de 6-9 micrómetros. Esta mezcla
de recubrimiento es un ejemplo del recubrimiento, que se utiliza en
los dispositivos médicos recubiertos de esta invención. Las agujas
BV-1 están fabricadas de un acero inoxidable
martensítico. Las agujas tienen una geometría de cuerpo cuadrado y
una punta cónica 12:1, y se describen en detalle en la Patente de
EE.UU. 5.477.604. Las agujas se recubrieron manualmente mediante un
proceso de inmersión y se colocaron en una bandeja magnética. La
bandeja tiene, realmente, bandas magnéticas elevadas, con el fin de
mantener seguros los extremos proximales de las agujas durante el
ciclo de curado y el transporte, mientras que los extremos distales
(puntas) de las agujas se suspenden sobre el extremo de las bandas
magnéticas. De esta forma, las puntas de las agujas no hacen
contacto con la bandeja. Posteriormente, las agujas se calentaron a
200ºC en un horno y se mantuvieron durante dos horas bajo atmósfera
ambiente. Se dejó que se enfriaran las agujas a temperatura
ambiente, fuera del horno. El análisis de penetración se realizó
como se describió anteriormente y los resultados se muestran a
continuación. Los resultados proceden del análisis de penetración
realizado utilizando dos grupos de 10 agujas, individualmente. Un
grupo de agujas se recubrió con la Mezcla #1 y el secundo grupo se
recubrió con la Mezcla #2. Se realizaron 10 penetraciones con cada
aguja recubierta. La media de la fuerza de penetración y la
desviación típica, para los dos grupos de agujas, para cada uno de
los pases se muestra a continuación.

Como se observa en la Tabla 1 y se ilustra
adicionalmente en la Gráfica 1, las agujas de esta invención
recubiertas con la Mezcla #2, que contiene la mezcla de cera de
polipropileno hidrocarbonada, presentan un recubrimiento que es
mucho más duradero y lubricante que el de las agujas de la técnica
anterior, recubiertas con la Mezcla #1. La fuerza de penetración
media del décimo pase decreció en un 47,5%. Típicamente, con el fin
de alcanzar un incremento en la duración de un dispositivo médico
de recubrimiento, se sacrifica la lubricación. Sin embargo, la
adición de una mezcla en polvo de cera de polipropileno
hidrocarbonada, en este ejemplo específico, para crear un
recubrimiento en dos fases, proporciona un incremento en la duración
sin sacrificar la lubricación. Se obtuvieron los mismos resultados
relativos utilizando procedimientos y recubrimientos idénticos sobre
muchos tipos de aguja diferentes, fabricados y comercializador por
Ethicon, tales como: CT-1 (acero inoxidable 420,
punta cónica 12:1), PC-1 (acero inoxidable
martensítico, agujas de punta afilada) y CTXB Ethiguard (acero
inoxidable 420, punta roma).
Se utilizó el mismo procedimiento que se
describió anteriormente, pero la mezcla de cera de polipropileno
hidrocarbonada se sustituyó con polietileno en polvo, cera sintética
en polvo, polipropileno en polvo y un polímero fluorocarbonado
modificado en polvo (que contiene polietileno). Los resultados en
todos los casos fueron agujas recubiertas, con una mejora
significativa en la duración y en la lubricación respecto de las de
las agujas recubiertas con la Mezcla #1, como se muestra en la Tabla
2 a continuación (comparar con los resultados de la Mezcla #1 de
recubrimiento en la Tabla 1).

Las agujas quirúrgicas Ethicon

de Recubrimiento #1 en el Ejemplo 1. Este es un recubrimiento

Ejemplo 1. Esta mezcla de recubrimiento es un ejemplo del
recubrimiento para los dispositivos médicos de esta invención. Las
agujas BV-175 están fabricadas de un acero
inoxidable martensítico. Estas agujas tienen una geometría de cuerpo
en I. Las agujas se recubrieron mientras estaban en un portador de
bandas con movimiento continuo, en un proceso de fabricación como el
descrito en la Patente de EE.UU. 5.477.604. Después del
recubrimiento, las agujas se sometieron a un secado rápido a 225ºC,
durante 30 segundos aproximadamente y se recogieron sobre un
rodillo. Posteriormente, las agujas se expusieron a una temperatura
de 158ºC durante 24 horas y se dejó que se enfriaran al aire
ambiente. El análisis de penetración se realizó como se describió
en el Ejemplo 1 anterior y los resultados se muestran a
continuación en la Tabla 3 y en la Grá-
fica 2.

En consecuencia con los resultados observados en
el Ejemplo1, las agujas recubiertas de esta invención, recubiertas
con la Mezcla #2, que contiene la mezcla de cera de polipropileno
hidrocarbonada, en un recubrimiento basado en silicona de dos
fases, presentan un incremento en la duración del recubrimiento, sin
sacrificar la lubricación, en comparación con las agujas
recubiertas con la Mezcla #1.
Las agujas quirúrgicas Ethicon
BV-1 (sin unir a una sutura, ver Ejemplo 1 para más
detalles) se recubrieron con una de las dos mezclas de
recubrimiento descritas en el Ejemplo 1 (Mezclas de Recubrimiento
No. 1 y 2), ambas aplicadas como recubrimientos de una única capa
de la forma descrita en ese ejemplo. Además, se aplicó sobre las
agujas un recubrimiento en dos capas. La capa interna se aplicó
manualmente mediante un proceso de inmersión de las agujas en una
mezcla de 80% en peso se xileno y 20% en peso de una mezcla en polvo
de cera de polipropileno hidrocarbonada. Posteriormente, las agujas
se calentaron a 150ºC y se mantuvieron durante 15 minutos bajo
atmósfera ambiente. Posteriormente, se dejó que las agujas se
enfriaran a temperatura ambiente. Estas agujas se recubrieron
posteriormente manualmente, para formar una capa de recubrimiento
externo, con la Mezcla de Recubrimiento #1 del Ejemplo #1.
Posteriormente, las agujas se calentaron a 200ºC y se mantuvieron
durante dos horas bajo atmósfera ambiente. Posteriormente, se dejó
que las agujas se enfriaran a temperatura ambiente. El análisis de
penetración se realizó como se describió en el Ejemplo #1 anterior,
con la excepción de que se utilizo Permair como medio de
penetración. Los resultados se muestran a continuación en la Tabla 4
y en la Grá-
fica 3.

la aplicación del recubrimiento en dos capas sobre las agujas
produce una eficacia de penetración similar a la del recubrimiento
en una única capa que contiene una mezcla de silicona y de un
polímero hidrofóbico que no contiene silicona.
Las agujas quirúrgicas Ethicon
BV-1 (sin unir a una sutura, ver Ejemplo 1 para más
detalles) se recubrieron con una de cuatro mezclas de
recubrimiento. La mezcla de recubrimiento #1 es equivalente a la
mezcla de recubrimiento #1 descrita en el Ejemplo 1. La mezcla de
recubrimiento #2 es equivalente a la mezcla de recubrimiento #2
descrita en el Ejemplo 1. La tercera mezcla de recubrimiento (Mezcla
#3) contiene aproximadamente, en peso, 5,7% de polidimetilsiloxano
terminado en hidroxi, 0,19% de metilhidrogeno siloxano, 14,25% de
xileno, 74,86% del hidrocarburo isoparafínico sintético, Exxon
Isopar-k y 5% de Triton-X. La cuarta
mezcla de recubrimiento (mezcla #4) contiene, aproximadamente, en
peso, 5,7% de polidimetilsiloxano terminado en hidroxi, 0,19% de
metilhidrogeno siloxano, 14,25% de xileno, 74,86% del hidrocarburo
isoparafínico sintético, Exxon Isopar-k y 5% de
IGEPAL CO-630. Tanto el Triton-X
como el IGEPAL CO-630 son octilfenoxi
polietoxietanoles que se utilizan comúnmente como tensioactivos y
agentes dispersantes en recubrimientos. Las agujas se recubrieron
como se describe en el Ejemplo 1. Posteriormente, las agujas se
calentaron a 200ºC y se mantuvieron durante dos horas bajo
atmósfera ambiente. Posteriormente, se dejó que las agujas se
enfriaran a temperatura ambiente. El análisis de penetración se
realizó como se describió en el Ejemplo #1 y los resultados se
muestran a continuación en la Tabla 5 y en la
Gráfica 4.

Como se muestra en la Tabla 5 y en la Gráfica 4,
el uso de la mezcla de recubrimiento #3 resultó en una mejora
marginal en la eficacia de penetración respecto de la mezcla de
recubrimiento #1 y la mezcla de recubrimiento #4 no supuso ninguna
mejora respecto de la mezcla de recubrimiento #1. Este ejemplo
muestra que el uso de tensioactivos poliméricos y/o agentes de
dispersión, tales como los octilfenoxi polietoxietanoles, combinados
con silicona no resultan en un incremento significativo en la
duración respecto a la utilización de silicona sola (mezcla de
recubrimiento #1), y exhiben apreciablemente menos duración, cuando
se comparan con la eficacia de penetración de las agujas
recubiertas en el ámbito de la invención (mezcla de recubrimiento
#2).

Las agujas quirúrgicas de punta afilada Ethicon
PC-1 (sin unir a una sutura) se recubrieron con una
de tres mezclas de recubrimiento. Las mezclas de recubrimiento #1 y
#2 son equivalentes a las mezclas de recubrimiento #1 y #2,
descritas en el Ejemplo 1. La mezcla de recubrimiento #3 contiene
aproximadamente, en peso, 5,7% de polidimetilsiloxano terminado en
hidroxi, 0,19% de metilhidrogeno siloxano, 14,25% de xileno, 74,86%
del hidrocarburo isoparafínico sintético, Exxon
Isopar-k y 5% de polietilenglicol. El
polietilenglicol es un polímero hidrófilo (PEG es soluble en agua)
muy conocido. Las agujas se recubrieron como se describe en el
Ejemplo 1 y posteriormente, se calentaron a 200ºC y se mantuvieron
durante dos horas bajo atmósfera ambiente. Posteriormente, se dejó
que las agujas se enfriaran a temperatura ambiente. El análisis de
penetración se realizó de forma similar a la descrita en el Ejemplo
#1 (con la excepción de que sólo se analizaron 5 agujas por grupo) y
los resultados se muestran a continuación en la Tabla 6.

La Muestra de Recubrimiento #3 proporciona sólo
un incremento marginal en la duración respecto a la Mezcla de
Recubrimiento #1, pero exhibe apreciablemente menos duración cuando
se compara con la eficacia de penetración de las agujas recubiertas
en el ámbito de la invención (mezcla de recubrimiento #2). Este
ejemplo demuestra una eficacia significativamente peor de las
agujas recubiertas, cuando se sustituye el polímero hidrofóbico que
no contiene silicona de la mezcla de recubrimiento, que es con el
que se recubren las agujas de esta invención, por un polímero
hidrófilo.

Se recubrieron hojas de bisturí cerámicas y
metálicas, utilizando las mezclas de recubrimiento #1 y #2 como las
descritas en el Ejemplo #1. La capacidad de lubricación y la
duración de los recubrimientos se analizó mediante la penetración
de las hojas en el material Porvair, utilizando una máquina para
análisis de tracción Instron, y registrando la carga máxima. Los
datos para las hojas de escalpelos cerámicos y metálicos, se
muestran en las Tablas 7 y 8, a continuación.

La mezcla de recubrimiento #2 proporciona una
mejora significativa en la duración, en comparación con el
recubrimiento #1, tanto para las hojas de escalpelos metálicos como
para las cerámicos.
Claims (13)
1. Un dispositivo médico que tiene una
superficie expuesta repetidamente a un tejido corporal, estando la
superficie de contacto recubierta con una cantidad efectiva de una
mezcla de recubrimiento que comprende del 40 al 86 por ciento en
peso de un polímero de silicona y del 14 al 60 por ciento en peso de
un polímero hidrofobo que no contiene silicona.
2. El dispositivo médico de la reivindicación 1,
en el que el polímero de silicona polisiloxano es un
polialquilsiloxano.
3. El dispositivo médico de la reivindicación 2,
en el que el polialquilsiloxano es un polidimetilsiloxano.
4. El dispositivo médico de la reivindicación 3,
en el que el polímero hidrofóbico que no contiene silicona tiene
una tensión superficial crítica menor de 50 mN/m.
5. El dispositivo médico de la reivindicación
4, en el que el polímero hidrofobo que no contiene silicona es un
polímero termoplástico.
6. El dispositivo médico de la reivindicación 5,
en el que el polímero termoplástico es fluido a menos de 210ºC.
7. El dispositivo médico de la reivindicación 6,
en el que el recubrimiento tienen del 44 al 66 por ciento en peso
de polidimetilsiloxano y del 34 a 56 por ciento en peso del polímero
termoplástico.
8. El dispositivo médico de la reivindicación 7,
en el que el dispositivo médico es una aguja quirúrgica.
9. El dispositivo médico de la reivindicación
8, en el que el polidimetilsiloxano está terminado en hidroxi.
10. El dispositivo médico de la reivindicación
9, en el que el polímero termoplástico es polietileno, polipropileno
o policaprolactona.
11. El dispositivo médico de la reivindicación
1, en el que el recubrimiento comprende una mezcla de un
polidimetilsiloxano terminado en hidroxi y un polímero hidrofobo
que no contiene silicona seleccionado entre el grupo constituido
por polietileno, polipropileno o policaprolactona.
12. El dispositivo médico de la reivindicación
11, en el que la mezcla de recubrimiento tiene del 44 al 66 por
ciento en peso de polidimetilsiloxano terminado en hidroxi y del 34
a 56 por ciento en peso de un polímero hidrofobo que no contiene
silicona.
13. El dispositivo médico de la reivindicación
12, en el que el dispositivo médico es una aguja quirúrgica.
