Description
Sistema de catéteres de cartografía y ablación
orientables.
La presente invención se refiere en líneas
generales al campo de la cartografía y ablación mediante catéteres
vasculares orientables. El objetivo específico de la invención es
suministrar un sistema de cartografía y ablación auriculares por
catéter para la creación de lesiones continuas lineales.
Se han ideado sistemas de catéter orientable de
diversos tipos. Dichos dispositivos se pueden insertar en vasos
sanguíneos o áreas corporales similares y se puede navegar hacia sus
extremos distales por los tortuosos caminos vasculares hasta
alcanzar áreas del cuerpo que normalmente resultan inaccesibles de
no emplearse la cirugía. Se conocen así mismo catéteres de tipo
orientable o autonavegante que tienen secciones distales con
electrodos para monitorizar partes del cuerpo, así como para
cartografiar eléctricamente el corazón recibiendo y emitiendo
señales eléctricas relativas a la operación de dicho órgano y a los
dispositivos de procesamiento y muestra de la grabación de señales.
La capacidad de grabar satisfactoriamente impulsos o señales y
cartografiar eléctricamente a partir de los mismos las cámaras y
válvulas cardiacas mediante catéteres flexibles con extremos
orientables provistos de electrodos ha llevado al uso de la técnica
del ablación por transcatéter de los tejidos cardiacos que han sido
identificados como causantes de las arritmias cardiacas. Esta
técnica se ha revelado como uno de los avances más importantes en la
electrofisiología cardiaca. Su meta es destruir el tejido
arritmogénico sin comprometer la integridad mecánica o muscular de
los tejidos y vasos sanguíneos.
No hace mucho, por ejemplo, muchos pacientes con
síndrome de Wolf-Parkinson-White o
con taquicardia ventricular fueron sometidos a la disección
quirúrgica del tejido arritmogénico, a la que siguió una dolorosa y
prolongada recuperación. La introducción del enfoque del
transcatéter ha reducido espectacularmente el sufrimiento y el coste
de este tratamiento definitivo en muchas de las causas de las
arritmias cardiacas.
Al principio, el enfoque general de este
procedimiento utilizaba de forma preferente corriente continua de
alta energía suministrada a los polos del catéter, por ejemplo, para
alterar la afección del nodo arterio-venoso e
incluso crear un bloque completo de corazón mediante la ablación del
haz de His. Más recientemente, no obstante, la radiofrecuencia ha
sustituido a la corriente continua como la fuente primaria de
energía preferida, mientras que el enfoque del transcatéter para la
ablación cardiaca se ha convertido en un procedimiento aceptado y
común que se ha ido usando cada vez más como el modo primario de
tratar arritmias cardiacas. La ablación por transcatéter de tejido
cardiaco se describe más detalladamente en Avitall y otros,
"Physics and Engineering of Transcatheter Tissue Ablation"
(Física e ingeniería de la ablación por transcatéter de
tejido), JACC, Volumen 22, nº 3:921-32. La
rápida aceptación clínica de este procedimiento y la proliferación
de médicos implicados en la ablación de tejido por transcatéter ha
exigido el desarrollo de dispositivos perfeccionados de catéteres
orientables.
Otras arritmias cardiacas comunes no tratables
excepto con medicación y, más recientemente, con cirugía, implican
la fibrilación auricular y el flúter. Estas afecciones, en realidad,
son las molestias rítmicas más comunes en los seres humanos. Por
ejemplo, aproximadamente el 1% de la población de los Estados
Unidos, o sea, más de 2’5 millones de personas, requieren
medicación para controlar esta afección. Estos ritmos irregulares
del corazón pueden alcanzar tasas de 180 latidos por minuto o más.
La consiguiente pérdida de flujo sanguíneo debida a las
contracciones auriculares incompletas junto con una elevada
velocidad del pulso pueden provocar dificultad al respirar, mareos,
resistencia física limitada, dolor de pecho, en pacientes con
enfermedades coronarias, y otros problemas de ese tipo.
Recientemente, el Dr. Cox y otros, de la Facultad
de Medicina de la Universidad Washington de Saint Louis, Missouri,
han ideado un procedimiento quirúrgico llamado la operación del
Laberinto y el Pasillo. Dicho procedimiento es un intento por
restablecer el ritmo normal del corazón segmentando los tejidos
auriculares para que un marcapasos normal pueda llegar hasta el nodo
arterio-venoso y para que los tejidos auriculares no
puedan sustentar la fibrilación auricular. Al cortar el tejido
auricular, no se puede transmitir actividad eléctrica de un segmento
a otro, con lo que los segmentos se hacen demasiado pequeños para
sustentar el proceso fibrilatorio. El enfoque, aunque es eficaz,
tiene los mismos inconvenientes que otros enfoques quirúrgicos
anteriores en lo que se refiere a la recuperación del paciente. Esto
representa otra área de tratamiento de la arritmia cardiaca en la
que un enfoque más benigno, o sea, sin cirugía invasiva, supondría
un avance definitivo.
A este respecto, como en el caso de otras
afecciones específicas de arritmia, el desacoplamiento eléctrico de
tejidos calentando los mismos con energía de radiofrecuencia (RF),
energía de microondas, energía de láser, congelación y sonicación,
suponen posibles enfoques alternativos. Se sabe que el calentamiento
de los tejidos por encima de 55ºC causa un daño irreparable a las
células, que quedan eléctricamente mudas. Se ha descubierto que la
segmentación de tejidos mediante la creación de lesiones continuas
lineales por medio de la ablación de las aurículas mimetiza algunos
aspectos del procedimiento del laberinto y el pasillo. El aspecto
más importante de estas lesiones es su carácter transmural continuo;
de lo contrario, no sería posible segmentar el corazón e impedir la
fibrilación auricular. Sin embargo, podría ser que la división
limitada de tejidos dentro de la aurícula derecha impidiera la
fibrilación auricular en algunos pacientes. Además, si se segmenta
un pasillo entre el nodo del seno y el nodo
arterio-venoso, se mantiene el control fisiológico
del ritmo del corazón a pesar de la fibrilación de los tejidos
auriculares.
Los actuales sistemas de catéter orientable,
aunque consiguen acceder a muchas zonas cardiacas internas, no han
sido tan eficaces en el tratamiento de la fibrilación auricular,
porque sólo con muchas dificultades han podido entrar en contacto
con ciertas zonas superficiales de la cámara auricular derecha. A
este respecto, los dispositivos anteriores no han sido capaces de
crear las lesiones lineales necesarias mediante ablación para
conseguir la deseada segmentación. El suministro de un sistema de
cartografía auricular y ablación por catéter que consiguiera tratar
satisfactoriamente la fibrilación auricular y el flúter creando con
facilidad lesiones lineales continuas en las aurículas supondría un
avance definitivo en el tratamiento de esta afección.
La patente de E.E.U.U. nº 4.699.147 describe una
sonda de cartografía intraventricular cardiaca que consta de una
serie de conjuntos de cables cada uno de los cuales lleva seis
electrodos espaciados a lo largo de sus respectivos tramos. Los
conjuntos de cables se acoplan todos juntos a un extremo distal y
van preformados, de modo que, cuando surgen del catéter que los
contiene adoptan una configuración envolvente elíptica. D1 también
sugiere que los conjuntos de cables pueden agruparse en torno a un
cable de mando que va conectado también al acoplamiento del extremo
distal, de forma que el cable de mando puede usarse para variar el
tamaño de la envoltura elíptica.
El documento DE 4025369 A1 describe una sonda de
cartografía intraventricular cardiaca en la que un conjunto de
cables que lleva cinco electrodos espaciados todo a lo largo va
enrollado en espiral alrededor de un cable de mando y conectado a
dicho cable por su extremo distal. El conjunto de cables enrollado
en espiral se halla preformado para que adopte la forma de muelle en
espiral una vez dentro del ventrículo.
Es objeto de la presente invención suministrar un
catéter perfeccionado, fácilmente desplegable y maniobrable, que
entre en contacto con las superficies interiores deseadas de la
cámara cardiaca auricular derecha y que mantenga dicho contacto para
que se produzcan las lesiones lineales en la forma deseada.
Otro objeto es suministrar formas de catéter
accionado por multielectrodos que se desplieguen fácilmente desde
vainas de catéteres principales una vez se acceda a la cámara
auricular deseada.
Es objeto adicional de la presente invención
proporcionar dichas formas de catéter capaces de modificarse
fácilmente para llegar hasta las superficies internas de contorno
variable de manera lineal.
El dispositivo de la invención puede usarse en un
método de cartografiado y ablación de la cámara auricular
derecha.
Otros objetos y ventajas de la invención
resultarán claras a los expertos en la materia a la vista de las
descripciones y figuras de esta especificación.
Por medio de la presente invención se suministra
un conjunto de formas de catéter operativo distal arqueado,
rápidamente controlables, que se despliegan fácilmente para entrar
en contacto con la superficie de la pared interna de la cámara
cardiaca auricular derecha de manera que puedan adaptarse a la
superficie intracardiaca de la aurícula derecha para grabar o
cartografiar fácilmente los impulsos, facilitando así un contacto
sostenido para que se produzcan lesiones lineales por medio de un
conjunto de dispositivos de electrodos para cartografía y ablación
espaciados en serie a lo largo de la forma del catéter operativo
mediante la energía de ablación por calentamiento eléctrico o por
radiofrecuencia. La sección de catéter operativo se despliega desde
un catéter principal o vaina usando cualquiera de las diversas
técnicas de posicionamiento y adopta diversas formas de
despliegue.
El catéter operativo de la invención consta de
uno o más dispositivos direccionales que se deslizan por un miembro
de alambre, que puede ser el hilo guía, y que coopera con un
recorrido limitador de topes de al menos uno de los miembros
direccionales, de forma que la sección intermedia de los miembros
direccionales adopta formas arqueadas ajustables al modularse su
distancia relativa de separación. En otra materialización
alternativa, el catéter operativo conformado a propósito adopta al
desplegarse una forma de bucle a mano derecha o izquierda.
En los dibujos, donde los números indican partes
semejantes en todos ellos:

auricular con el tramo extendido del segmento del tubo principal

La Figura 8 es la representación esquemática
ampliada de una alternativa a la materialización de las Figuras 2 a
4 de un catéter operativo de acuerdo con la invención, donde la
vaina aparece prolongada y separada;
La Figura 9 es la representación esquemática de
otro catéter ejemplar diferente:
La Figura 10 es una vista fragmentaria ampliada
del puerto de infusión utilizable con el sistema de catéteres de la
invención, y
Las Figuras 11 a 13 son vistas fragmentarias de
otra materialización diferente más del catéter de la invención que
al desplegarse toma la forma de bucle.
El sistema de cartografía eléctrica y ablación de
fibrilación auricular es portado por una porción, prolongación o
segmento distales de catéter operativo que, de acuerdo con la
invención, puede adoptar presentarse en una de diversas formas. La
porción o zona distal se despliega normalmente desde un catéter
principal o vaina en las proximidades de la aurícula derecha u otra
cámara de interés. La posición y forma del electrodo elegidas
dependerá de la superficie concreta a la que haya que acceder y del
modo de acceso a la cámara. Igualmente, la configuración del
electrodo no tiene por qué estar limitada en absoluto a los esquemas
de las ilustraciones, debiéndose entender además que se puede
emplear cualquier tamaño y esquema de electrodos coherente con la
cartografía y ablación de cualquier parte de la cámara en
cuestión.
Los sistemas de electrodos de acuerdo con la
sección distal del catéter operativo se hallan generalmente
diseñados de forma que cada electrodo individual va conectado
eléctricamente mediante un cable aislado y separado insertado por el
sistema de catéteres al extremo distal del mismo, donde cada cable
va conectado a un sistema de control que permite la cartografía o
grabación por separado de los impulsos recibidos de cada electrodo y
la conexión acoplada o por separado de los mismos electrodos para la
ablación. Esto permite la ablación usando el esquema de electrodos
múltiples que se desee en el conjunto serial para producir cualquier
configuración de lesiones que se desee. Dicha disposición del
control de electrodos aparece ilustrada y descrita en la solicitud
copendiente de este mismo solicitante con Número de Serie
08/161.916, registrada junto con la presente en la misma fecha,
actualmente patente de EEUU nº 5.454.370.
El catéter operativo de la invención está
diseñado para que un cirujano experimentado consiga un mayor grado
de control con respecto a la cartografía y colocación exacta de las
lesiones lineales en la superficie interna del tejido en las
proximidades de la cámara auricular derecha con mayor facilidad,
mediante ablación por radiofrecuencia o similar para lograr la
segmentación eléctrica. Ello se consigue por medio de una variedad
de materializaciones de catéter operativo único configuradas de modo
que se hallen en contacto con segmentos continuos de las superficies
de la cámara auricular. Aunque las materializaciones se describirán
con especial referencia a la cámara cardiaca auricular derecha, se
entenderá que los catéteres operativos de la invención pueden usarse
también en otras cámaras y órganos.
Un catéter, que se muestra en líneas generales en
20 de la Figura 1, consta de tres componentes principales que
cooperan entre sí, que son una sección o porción de vaina de catéter
operativo distal 22, que puede ser una prolongación de un miembro de
catéter tubular principal prolongado 24 que se muestra separado para
indicar la longitud relativamente considerable, y un mango de
control 26 con un botón de manipulación o control de orientación del
extremo operativo, como en 28. La sección de vaina del catéter
operativo va provista de una abertura en forma de ranura 30 de la
cual surge para desplegarse un segmento flexible o tramo distal
relativamente corto de catéter operativo 32, el cual se puede
desviar o doblar, y que lleva un conjunto de electrodos espaciados
en serie 34. El botón de control 28 puede dejarse fijo para
desplegar y manipular espacialmente (desviar y rotar) la sección de
catéter operativo 32 en cualquiera de las maneras habituales. Dicho
sistema de control aparece ilustrado y descrito en la solicitud
copendiente de este mismo solicitante con Número de Serie
08/156.284, registrada el 22 de noviembre de 1993 y titulada
Catheter Control Handle (Mango para control de catéter),
actualmente patente de EEUU nº 5.465.716. En cualquier caso, la
porción de catéter operativo 32 se despliega desde la abertura de la
vaina 30 y está diseñada para manipularse tanto en curvatura como en
postura a fin de situar los electrodos contra la superficie que se
va a cartografiar o someter a ablación.
El catéter 20 incluye también un corto miembro
guía vascular relativamente flexible 36 fijado al extremo distal del
mismo a fin de que el dispositivo sea esencialmente autonavegante.
Un dispositivo hermético de bloqueo de la vaina 37 con puerto de
infusión 38 va dispuesto cerca del punto del catéter que coopera con
un dispositivo introductor en una de las maneras habituales, para
que los controles del catéter y los dispositivos de entrada y salida
sean accesibles desde fuera o en las proximidades del punto de
introducción del catéter. En 39 se muestra una serie de
conductores.
La Figura 5 es la representación esquemática de
un corazón 40 seccionado por las cámaras; las que aquí aparecen son
la cámara auricular derecha 42 y el ventrículo derecho 44, separadas
por la válvula tricúspide 46. La válvula y la arteria pulmonares
aparecen respectivamente con los números 48 y 50. La vena cava
superior aparece con el número 52, mientras que la vena cava
inferior va numerada con el 54. La sección de catéter operativo
aparece en la aurícula derecha y se extiende por la vana cava,
mostrando así que se puede acceder a la cámara auricular derecha 42
o por la vena cava superior o por la vena cava inferior, y que el
segmento operativo de electrodos desplegado, en conjunción con el
movimiento de la vaina 22, posibilita la colocación de los
electrodos 34 en la forma deseada.
En las Figuras 2 a 4 se representa una
materialización funcional alternativa 120 del catéter de la
invención, en la que el hilo guía 122 sobresale de un extremo distal
124. La sección o porción de vaina 125 tiene una sección de ranura
alargada 126 por la que se despliega la sección de catéter operativo
127 que va provista de una serie de electrodos 128. Como se ve mejor
en la Figura 3, en esta materialización el miembro guía 122 se
extiende hasta dentro del lumen 129 de la vaina 125 y luego se
inserta de forma deslizante por un agujero 130 en un segmento
direccional 131 del extremo distal de la sección de catéter
operativo 127.
La sección de catéter operativo 127 lleva el
elemento direccional de su extremo distal insertado de forma
deslizante sobre el miembro guía vascular de forma que la porción
más próxima de la sección de catéter 127 produce una curva arqueada
en la sección de electrodos del catéter operativo. Un cable de
control fijado en la zona proximal de de la sección distal del
catéter operativo en una de las maneras habituales, como, por
ejemplo, la que se describe en las solicitudes copendientes
mencionadas anteriormente, cuando se manipula recíprocamente con el
mango 28, produce una curva arqueada de cierre variable, como se ve
en las Figuras 3 y 4. De esta manera, puede hacerse que el conjunto
de electrodos espaciados en serie 128 adopte un esquema ajustable
que puede colocarse en las superficies de las cámaras adyacentes en
diversas formas arqueadas; en la Figura 4 se ve una serie de
posibles configuraciones. La porción delantera 124 constituye un
tope distal que determina la posición distal más lejana del elemento
direccional de la punta 131 del segmento distal del catéter, para
que el desplazamiento longitudinal, dirigido de forma aún más
distal, de la porción proximal del catéter operativo hacia dentro de
la vaina produzca desviaciones arqueadas que formen configuraciones
como las que se ve en las ilustraciones.
La figura 8 es la vista esquemática ampliada de
una materialización montada sobre guía mediante un miembro de
navegación de alambre 140, de guía flexible y tipo deslizante, sobre
el que va insertada la sección de catéter operativo 142 con
electrodos 143 en las partes distal y proximal de la porción con
electrodos mediante segmentos direccionales, como se ve en 144 y
145, respectivamente, dejando suelta la porción central para formar
una configuración en "oruga". Un tope fijo 141 sujeto al
miembro guía 140 limita el recorrido distal de la punta del catéter.
La vaina del catéter principal se ve en 147, separada aquí para
mayor claridad, y provista opcionalmente de un puerto de infusión
148 con sistema de bloqueo 149.
El número, tamaño y espaciado de los electrodos
143 son opcionales. Una materialización usó 20 electrodos anulares
de unos 4 mm. de largo, espaciados a 4 mm. uno de otro. No obstante,
debe advertirse que la configuración de electrodos espaciados en
serie de acuerdo con la invención y sus diversas materializaciones
tiene como fin principal, aparte de la cartografía o grabación de
tejido arqueado, la creación de lesiones lineales mediante ablación
para conseguir vías de segmentación o conducción dentro del tejido
de la superficie de la cámara. Con esto en mente, se puede emplear
ciertas combinaciones de configuraciones y formas de electrodos. Los
electrodos de las materializaciones de las Figuras 2 a 4 y el
ejemplo de la Figura 9, de 2 mm. de largo y espaciados de 0,5 a 3
mm. uno de otro, se han usado también dispuestos en pares
espaciados, como se ve en las Figuras 11 a 13.
La ilustración de la materialización de la Figura
8 se complementa en lo referente a la colocación en la cámara
auricular derecha del corazón con las Figuras 6 y 7. En estas vistas
seccionales esquemáticas se ve que la forma relativamente arqueada
de la sección de catéter operativo de cartografía/ablación 142 puede
mantenerse en cualquier forma que se desee y que dichas formas
arqueadas guardan un gran parecido con las formas del contorno de
las superficies internas de las diversas paredes de la aurícula
derecha. En la figura 6, por ejemplo, la sección interior superior
150 es alcanzada fácilmente por la forma arqueada adoptada por la
sección de catéter operativo 142 como también el segmento inferior
152. En la Figura 7, la pared derecha de la cámara auricular es
alcanzada en 154. La sección de catéter operativo se ha girado
además con respecto al miembro guía 140. Estas posiciones pueden
mantenerse a pesar del continuo flujo de sangre y del movimiento de
las paredes internas.
Con respecto a la materialización de la Figura 8,
se construyó un sistema de catéter deslizante de 2,3 mm. (7 French)
similar al de la Figura 8, mediante el cual el catéter pudo curvarse
y adaptarse a la superficie intracardiaca de la aurícula derecha. El
catéter estaba equipado con 20 electrodos espaciados unos de otros a
sólo 4 mm., usados tanto para cartografía como para ablación. En 7
modelos se creó susceptibilidad a fibrilación auricular (AFIB)
mediante pericarditis estéril, estimulación vagal e infusión de
isuprel (3 \mugram/min.). Se insertó un hilo guía rígido con
extremo flexible en rabo de cerdo (como se en 140 de la Figura 8) en
la vena cava superior a través de la vena femoral. Se colocó una
vaina sobre el hilo guía con su extremo en la vena cava
inferior/articulación auricular derecha. Se insertó el catéter de
ablación en la vaina sobre el hilo guía y se colocó inicialmente en
la aurícula posterior lateral derecha con los electrodos en contacto
con los tejidos de la vena cava superior, la aurícula derecha y la
vena cava inferior. Se realizó la desviación de catéter empujando el
mango del mismo contra un tope situado a 10 cm. del extremo del hilo
guía. Se aplicó a cada electrodo energía de radiofrecuencia
graduada, empezando a 20 watios y siguiendo a 30, 40 y 50, durante
20 segundos en cada uno de estos niveles de energía. Tras la
ablación, se movió y curvó el catéter sobre la pared anterior de la
aurícula derecha y se repitió las ablaciones. Se indujo AFIB al
menos 10 veces consecutivas antes y después de la ablación mediante
corriente alterna de 60 Hz aplicada al apéndice auricular izquierdo
durante 5 segundos. Seis de los siete modelos tuvieron AFIB
sostenida (>3 min.) Tras la ablación no se pudo mantener AFIB y
duró sólo 20\pm48 segundos. El examen de cada corazón reveló
lesiones transmurales continuas que bisecaban la aurícula derecha de
forma posterior lateral y anterior.
En la Figura 9 se muestra un ejemplo en el que el
extremo distal de la punta 160 del segmento 161 con electrodos 162
del catéter operativo se despliega desde una abertura distal guiada
163 en el extremo distal de un lumen 165 en un catéter o vaina 166
equipado con un miembro guía vascular en punta hecho de alambre
flexible y suave. En esta materialización, como ocurre con la de la
Figura 1, la cantidad de despliegue, desviación y postura de la
sección de la punta del catéter operativo 160 puede controlarse por
medios de manipulación manuales en conjunción con uno o más hilos o
elementos de control (que no se muestran aquí).
En las Figuras 11 a 13 se ve otra configuración
más para suministrar una forma arqueada adecuada para cartografía y
ablación dentro de los límites de la cámara auricular derecha del
corazón. Como se ve en estas Figuras, el extremo distal 170 de la
sección distal de un catéter operativo 171 que surge de una vaina o
catéter principal 172 en 173 lleva un ánima que se inserta de forma
deslizante por un hilo guía flexible 174 provisto de un miembro de
bloqueo fijo 175 sujeto a una distancia predeterminada del extremo
distal de la punta de navegación del hilo guía 174. Un hilo de
control (que no se ve aquí) sujeto mediante el catéter operativo 172
se usa para ajustar axialmente la posición del extremo proximal de
la sección de catéter operativo 172 en relación al tope para formar
y ajustar así el tamaño relativo del bucle esencialmente circular
176. De esta manera, el bucle 176, 176A puede hacerse mayor o menor
en una cantidad determinada dada, permitiendo así que alcance las
cámaras auriculares derechas de diferentes tamaños y que se expanda
contra formas arqueadas de diferentes radios. También puede adoptar
una forma sustancialmente lineal antes o después del despliegue para
replegarse dentro del catéter o vaina. Mientras que los electrodos
177 aparecen representados en pares espaciados, puede usarse otras
configuraciones como las de la Figura 8.
Las Figuras 11 y 12 representan bucles circulares
a mano contraria uno de otro que pueden formarse desde el catéter
operativo, el cual se muestra truncado en la Figura 13. El
dispositivo puede predisponerse para formar un bucle a mano derecha
o izquierda con respecto a una determinada orientación del catéter
y, según la dirección de entrada en la aurícula derecha y/o la
superficie concreta que se vaya a someter a cartografía y/o
ablación, será preferible una u otra. En caso contrario, da igual
cualquiera de ellas.
Con respecto a las dimensiones de las diversas
materializaciones del catéter de la invención, los segmentos del
catéter operativo tienen típicamente de 5 French (1,7 mm.) a 8
French (2,7 mm.) de diámetro, mientras que el miembro de la vaina
tiene de 7 a 10 French (2,3 a 3,3 mm.) de diámetro. Los catéteres
que tienen aberturas en la vaina o laterales se prolongan unos 5 mm.
desde las aberturas 30, 126, etc. y unos 15 cm. desde la abertura de
la materialización de la Figura 9. Los segmentos del catéter
operativo tienen típicamente de 5 a 15 cm. de longitud en el caso de
los segmentos 127 y 146, y algo menos en el caso de los segmentos 32
y 160. Las configuraciones del bucle de las Figuras 11 y 12 pueden
tener la longitud que se desee, pero típicamente dicha longitud es
la adecuada para que el bucle se aproxime al tamaño del diseño de
oruga de las Figuras 3, 4 y 8.
La Figura 10 muestra un sistema de infusión
alternativo al de la Figura 8, o parecido, y consta de un puerto de
infusión 192 sobre una junta o bloqueo de catéter o vaina (que no se
ve aquí) y los hilos de conducción de los electrodos como en 192, y
tal vez pueda añadirse un hilo guía y/o miembro de control 194 con
un paso a través del sistema hacia los controles proximales.
La invención se ha descrito aquí muy
detalladamente en cumplimento de las Leyes sobre Patentes y para
suministrar a los expertos en la materia la información necesaria
para aplicar los nuevos principios y para construir y usar
materializaciones del ejemplo como haga falta. No obstante, debe
entenderse que la invención puede llevarse a cabo mediante otros
dispositivos específicamente diferentes y que pueden realizarse
diversas modificaciones sin apartarse del ámbito de la invención tal
como se define en relación con las reivindicaciones anexas.
Claims (14)
1. Sistema de catéteres de grabación y ablación
para catéter cardiaco vascular para cartografía y grabación
eléctricas y creación de lesiones lineales mediante ablación en una
cámara del corazón, que consta de:
- un catéter o vaina vascular alargado y hueco (125, 147, 172) que lleva un lumen para alojar un catéter interior desplegable;
- un miembro guía (122, 140, 174) para ayudar a dicho catéter o vaina en la navegación por el sistema vascular del paciente;
- un catéter flexible interior (127, 142, 176, 176A) que consta de una sección de catéter operativo provista de un extremo proximal y otro distal y dispuesta para que adopte una forma arqueada de curvatura controlable capaz de entrar en contacto con la superficie interna de una cámara cardiaca al desplegarse desde dicho catéter principal o vaina a través de
- un lumen del mismo para portar dicha sección de catéter operativo y un conjunto de electrodos espaciados conectados por separado en dicha sección de catéter operativo; en donde:
- el extremo distal del catéter flexible interior puede moverse a lo largo del miembro guía por medio de un elemento direccional distal del catéter (131, 144, 170) en el extremo distal de la sección de catéter operativo;
- dicho catéter o vaina tiene una abertura distal (126, 173), la sección de catéter operativo está adaptada para desplegarse haciéndola avanzar por dicha abertura distal, y
- el extremo proximal de la sección de catéter operativo que es mantenida en las proximidades del miembro guía (122, 140, 174) por un elemento direccional proximal (145) o por la abertura distal de la vaina del catéter (125, 172).
2. Aparato según al reivindicación 1
caracterizado por tener dicha sección de catéter operativo
(142) elementos direccionales de catéter espaciados, proximales y
distales (144, 145), con ánimas que se insertan de forma deslizable
sobre dicho miembro guía (140) y relativamente ajustables una con
otra de forma que la sección de catéter operativo (142) intermedia
entre dichos elementos direccionales proximal y distal del catéter
se suelte y se pueda flexionar de forma arqueada ajustable según la
separación relativa de dichas ánimas direccionales sobre dicho
miembro guía para que adopte la forma necesaria para llegar hasta al
superficie interna de una cámara, y caracterizado además por
un recorrido limitador o miembro de bloqueo (141) para limitar el
recorrido del extremo distal del catéter
operativo.

3. Aparato según la reivindicación 2,
caracterizado por medios para ajustar los electrodos de la
sección ajustable del catéter operativo para que adopte un esquema
de contacto sustancialmente lineal con respecto a la forma de la
superficie contactada de la cámara en la dirección deseada.
4. Aparato según cualquiera de las
reivindicaciones 2 ó 3, caracterizado porque dicha sección de
catéter operativo (142) es giratoria con respecto al miembro guía
(140).
5. Aparato según cualquiera de las
reivindicaciones 2 a 4, caracterizado porque la longitud de
dicha sección de catéter operativo (142) es de 5 cm. a 15 cm.
6. Aparato según cualquiera de las
reivindicaciones 2 a 4, caracterizado porque dichos
electrodos (143) tienen una longitud de unos 4 mm.
7. Aparato según cualquiera de las
reivindicaciones 2 a 5, caracterizado porque la distancia
entre electrodos es de unos 4 mm.
8. Aparato según la reivindicación 4,
caracterizado por la combinación de la sección ajustable del
catéter operativo (142) en un tramo y dicho miembro de bloqueo (141)
situado sobre dicho miembro guía para que el dispositivo pueda
acceder a una forma de superficie deseada en la cámara auricular
derecha del corazón humano o por la vena cava inferior o por la vena
cava superior.
9. Sistema de catéter de grabación y ablación
según la reivindicación 1, en el que el extremo distal (170) de la
sección de catéter operativo va sujeta de forma deslizante sobre el
miembro guía (174) y que comprende además un recorrido limitador o
miembro de bloqueo (175) para limitar el recorrido del extremo
distal (170) de la sección de catéter operativo a lo largo del
miembro guía, de forma que el posterior desplazamiento
relativamente axial de una porción más proximal de la sección de
catéter operativo mediante un hilo de control produce al desplegarse
una forma predeterminada de bucle central (176, 176A).
10. Aparato según la reivindicación 9, en donde
dicha forma de bucle es una seleccionada del grupo que consta de
bucles de mano izquierda y derecha
11. Aparato según la reivindicación 9 o la 10
caracterizado además por medios para ajustar el diámetro de
dicho bucle.
12. Aparato según cualquiera de las
reivindicaciones anteriores en donde dichos electrodos (128, 143,
177) van conectados por separado, por lo que se les puede aplicar
energía en cualquier combinación.
13. Aparato de cualquiera de las reivindicaciones
9 a 11 caracterizado porque los electrodos (177) van
dispuestos en pares espaciados con un espacio dentro de cada par y
un espacio entre cada par y en donde dicho espacio dentro de cada
par es menor que dicho espacio entre cada par.
14. Aparato según la reivindicación 1 en donde
dicha abertura distal de dicho catéter o vaina se caracteriza
además por una ranura lateral o abertura alargada (126) y por estar
adaptada la sección distal del catéter operativo para desplegarse
desde la ranura o abertura.
