Claims (1)
- 【特許請求の範囲】 1.心臓の右心房内に直線状の障害部を作り出すための記録、切除用のカテー テル装置において、 (a) 患者の血管系内にカテーテルを案内するための案内部材と、 (b) 主カテーテルあるいはシースに関連した可撓性のある末端作動カテー テル領域であって、該作動カテーテル領域が隔置された末端カテーテ ルライダーと基部カテーテルライダーとを有し、該ライダーがワイヤ 部材の上へ相互にかぶせるための、滑動的にねじ込まれ、比較的調節 可能になるような孔を有し、従って、前記末端カテーテルライダーと 基部カテーテルライダーとの中間における作動カテーテル領域が固定 されず、また前記案内ワイヤにおける前記ライダー孔を相対的に分離 させることによって、調節可能的に弓形にたわみ、心房の内部表面に 位置付けるような望みの形状になることができる、その末端作動カテ ーテル領域と、 (c) 前記作動カテーテルによって支持された複数個の連続的な電極と、 を具備するカテーテル装置。 2.請求の範囲第1項に記載した装置において、調節可能な作動カテーテル領 域の電極を調節して、望みの方向において、接触している形状の右心房の表面に 関してほぼ直線状の接触パターンを形成するための装置を有しているカテーテル 装置。 3.請求の範囲第1項に記載した装置において、前記シースが案内部材に関し て回転可能であるカテーテル装置。 4.請求の範囲第1項に記載した装置において、前記調節可能な作動カテーテ ルが下大静脈あるいは上大静脈のいずれかから右心房に近接することができるカ テーテル装置。 5.請求の範囲第1項に記載した装置において、さらに作動カテーテルの末端 の動きを制限するためのストッパを有しているカテーテル装置。 6.右心房内でセグメント化するために、直線状の障害部を繰り出す記録、切 除用の心臓血管カテーテルにおいて、 (a) 主カテーテルあるいはシースの末端から突出するように設計された血 管案内用の案内ワイヤと、 (b) 基部と末端部とを有する可撓性のある末端作動カテーテル部分であっ て、該作動カテーテル部分を収容することのできる内腔を有する、主 たる細長いカテーテルあるいはシースから展開するようになっており 、又該作動カテーテル部分が複数個の離隔的に連結された連続的な電 極をも有している、その末端作動カテーテル部分と、 を具備し、 (c) 前記作動カテーテル部分の末端が右心房の内面と接触することのでき る、調節可能な曲率の弓形形状になることができ、また複数個の離隔 された電極を用いて右心房の表面に沿って、ほぼ直線状の切除障害部 を作り出すことのできるような位置関係になっている、 ことを特徴とするカテーテル装置。 7.請求の範囲第6項に記載した装置において、前記作動カテーテル部分の末 端はさらに前記案内ワイヤに滑動可能的に取り付けられており、作動カテーテル 部分の末端の末端方向への移動を制限するための移動制限装置が設けられ、従っ て、末端作動カテーテル部分のより基部的な部分をさらに相対的に軸線方向末端 へ移動させることにより、カテーテルの弓形突出部が得られるカテーテル装置。 8.請求の範囲第7項に記載した装置において、前記末端作動カテーテル部分 が、展開された時に所定の中央ループ形状を形成するカテーテル装置。 9.請求の範囲第8項に記載した装置において、前記ループは右向きのものと 左向きのものとから選択されるカテーテル装置。 10.請求の範囲第6項に記載した装置において、前記末端作動カテーテルが、 シースの末端部における孔を通って前進されることによって展開されるカテーテ ル装置。 11.請求の範囲第8項に記載した装置において、前記ループの寸法が調節可能 であるカテーテル装置。 12.請求の範囲第6項に記載した装置において、前記電極が隔置された対状に なっており、対内の間隔の方が、対間の間隔より短かくなっているカテーテル装 置。 13.右心房内の表面組織をマッピングし、切除するための方法において、 (a) 展開可能な、可撓性のある末端カテーテル部分を支持している主カテ ーテルあるいはシースを、関心のある患者の血管系内へ案内する段階 と、 (b) 前記カテーテルの末端部を上大静脈あるいは下大静脈のいずれかを通 して、選択的に右心房内へ挿入する段階と、 (c) ここで、 (1) 前記可撓性のある末端作動カテーテル部分は基部と末端部を有し 、該作動カテーテル部分を収容することのできる内腔を有する、 主たる細長いカテーテルあるいはシースから展開するようになっ ており、又該作動カテーテル部分は複数個の離隔的に連結された 連続的な電極をも有しており、 (2) 前記作動カテーテル部分の末端は右心房の内面と接触することの できる、調節可能な曲率の弓形形状になることができ、また複数 個の離隔された電極を用いて右心房の表面に沿って、ほぼ直線状 の切除障害部を作り出すことのできるような位置関係になってお り、 さらに、 (d) 前記作動カテーテル部分の末端領域を望みの右心房内表面に対して、 曲率を調節しながら接触させ、複数個の前記離隔した連続的な電極 を励磁することによって、比較的直線状の切除障害部を形成させる 段階と、 (e) インディケーションのあった直線状の障害部の組織を切除する段階と 、 (f) 前記段階(b)と(a)とを逆転させる段階と からなる右心房組織表面のマッピングおよび切除方法。 14.請求の範囲第15項に記載した方法において、さらに、正確な切除位置を 確定するために、切除の前に、電気的な活性度をマッピングするために、前記末 端作動カテーテルを用いる段階を含む右心房組織表面のマッピングおよび切除方 法。
