Description
Dispositivo para cirugía intracardiaca.
La invención se refiere en general a instrumentos
para realizar procedimientos quirúrgicos menos invasivos, y más
específicamente, a instrumentos para cirugía menos invasivos en el
corazón y en grandes vasos sanguíneos.
Actualmente se realizan varios tipos de
procedimientos quirúrgicos para investigar, diagnosticar y tratar
las enfermedades del corazón y de los grandes vasos sanguíneos del
tórax. Estos procedimientos incluyen la reparación y el reemplazo
de las válvulas mitral, aórtica y otras del corazón, la reparación
de defectos del tabique ventricular, trombectomía pulmonar,
tratamiento de aneurismas, trazado electrofisiológico y ablación del
miocardio, y otros procedimientos en los que se introducen
dispositivos de intervención en el interior del corazón o de un
gran vaso sanguíneo.
Usando las técnicas actuales, muchos de estos
procedimientos requieren una gran toracotomía, usualmente en forma
de una esternotomía media, para poder acceder a la cavidad torácica
del paciente. Se usa una sierra u otro instrumento de corte para
cortar el esternón longitudinalmente, permitiendo separar dos
mitades opuestas de la porción anterior o ventral del conjunto de
las costillas. De esta manera se crea una gran abertura en la
cavidad torácica, a través de la cual el equipo quirúrgico puede
visualizar directamente y operar en el corazón y en otros
contenidos torácicos.
Las intervenciones quirúrgicas en el interior del
corazón generalmente requieren el aislamiento del corazón y los
vasos sanguíneos coronarios del resto del sistema arterial, y la
detención de la función cardiaca. Usualmente, el corazón se aísla
del sistema arterial mediante la introducción de una abrazadera
transversal aórtica externa a través de una estereotomía y
aplicándola a la aorta entre la arteria branquiocefálica y los
ostia coronaria. El fluido cardioplégico se inyecta a continuación
en las arterias coronarias, directamente en los ostia coronaria o a
través de una punción en la raíz aórtica, para detener la función
cardiaca. En algunos casos, el fluido cardioplégico se inyecta en el
seno coronario para la perfusión retrógrada del miocardio. El
paciente se coloca en bypass cardiopulmonar para mantener la
circulación periférica de sangre oxigenada.
De particular interés para la presente invención
son los dispositivos intercardiacos para el tratamiento quirúrgico
de las válvulas del corazón, especialmente las válvulas mitral y
aórtica. Según estimaciones recientes, más de 79.000 pacientes son
diagnosticados con una dolencia de la válvula mitral y aórtica en
los hospitales de los Estados Unidos cada año. Más de 49.000
procedimientos de reemplazo la válvula mitral o de la válvula
aórtica se realizan anualmente en los Estados Unidos, junto con un
número significativo de procedimientos de reparación de las válvulas
del corazón.
Se pueden usar varias técnicas quirúrgicas para
reparar una dolencia o una válvula dañada, incluyendo la
anuloplastia (contracción del anillo de la válvula), resección
cuadrangular (estrechamiento de las hojuelas de la válvula),
comisurotomia (corte de las comisuras de la válvula para separar las
hojuelas de la válvula), acortamiento de las cuerdas tendinosas de
la válvula mitral o tricúspide, resujeción de las cuerdas
tendinosas de la válvula mitral o tricúspide severas o del tejido
muscular papilar, y descalcificación de la válvula y del tejido
anular. Alternativamente, la válvula se puede reemplazar, mediante
la extirpación de las hojuelas de la válvula de la válvula natural,
y colocando una válvula de reemplazo en la posición de la válvula,
usualmente suturando la válvula de reemplazo con el anillo de la
válvula natural. Actualmente se usan varios tipos de válvulas de
reemplazo, incluyendo prótesis mecánicas y biológicas, homoinjertos,
y aloinjertos, tal como se describe en Bodnar y Frater,
"Replacement Cardiac Valves" (Válvulas de Reemplazo Cardiaco),
1-357 (1991), que se incorpora aquí como referencia.
Una discusión comprensible de las dolencias de las válvulas del
corazón y el tratamiento quirúrgico de las mismas se encuentra en
Kirklin y Barratt-Boyes, "Cardiac Surgery"
(Cirugía Cardiaca) 323-459 (1986). El documento
WO-A-87/0589 describe una válvula de
corazón prostética y un instrumento para insertarla.
La válvula mitral, situada entre la aurícula
izquierda y el ventrículo izquierdo del corazón, es la que se
alcanza más fácilmente a través de la pared de la aurícula
izquierda, que normalmente reside en el lado posterior del corazón,
opuesto al lado del corazón que está expuesto mediante una
esternotomía media. De esta manera, para acceder a la válvula
mitral a través de una esternotomía, se gira el corazón para dejar
la aurícula izquierda en una posición anterior accesible a través
de la esternotomía. A continuación se realiza una abertura, o
atriotomía, en el lado derecho de la aurícula izquierda, anterior a
las venas pulmonares derechas. La atriotomía se retrae mediante
suturas o un dispositivo de retracción, exponiendo la válvula
mitral directamente posterior a la atriotomía. Una de las técnicas
citadas anteriormente se puede usar a continuación para reparar o
reemplazar la válvula.
Una técnica alternativa para el acceso a la
válvula mitral se puede usar cuando no son deseables una
esternotomía media y/o manipulación giratoria del corazón. En esta
técnica, se hace una larga incisión en el lado lateral derecho del
pecho, usualmente en una zona del quinto espacio intercostal. Se
pueden retirar una o más costillas del paciente, y otras costillas
cerca de la incisión se retraen hacia fuera para crear una gran
abertura en la cavidad torácica. La aurícula izquierda se expone
entonces sobre el lado posterior del corazón, y se forma una
atriotomía en la pared de la aurícula izquierda, a través de la cual
se puede acceder a la válvula mitral para su reparación o
reemplazo.
Usando estas técnicas de pecho abierto, la gran
abertura prevista mediante una esternotomía media o toracotomía
derecha permite al cirujano ver la válvula mitral directamente a
través de la atriotomía izquierda, y colocar sus manos en la
cavidad torácica en proximidad con el exterior del corazón para la
manipulación de instrumentos quirúrgicos, la retirada de tejido
extirpado, y/o la introducción de una válvula de reemplazo a través
de la atriotomía para colocarla en el corazón. Sin embargo, estos
procedimientos invasivos a pecho abierto producen un alto grado de
traumatismo, un riesgo significativo de complicaciones, una estancia
extendida en el hospital, y un periodo de recuperación doloroso
para el paciente. Además, aunque la cirugía de las válvulas del
corazón produce resultados beneficiosos para muchos pacientes,
otros numerosos que se podrían beneficiar de esta cirugía son pueden
o no están dispuestos a sufrir los traumatismos y los riesgos de
las técnicas actuales.
Lo que se necesita, por lo tanto, son
dispositivos para realizar la reparación y el reemplazo de las
válvulas del corazón, así como otros procedimientos dentro del
corazón y los grandes vasos sanguíneos, que reduzca los
traumatismos, los riesgos, el tiempo de recuperación y el dolor que
acompañan a las técnicas actuales. Los dispositivos han de
facilitar la intervención quirúrgica en el interior del corazón o
grandes vasos sanguíneos sin la necesidad de una gran toracotomía,
preferiblemente a través de pequeñas incisiones en los espacios
intercostales del conjunto de las costillas, sin cortar, retirar, o
desviar significativamente las costillas o el esternón del
paciente. En particular, los dispositivos han de permitir la
retirada del tejido de la cavidad torácica, así como la
introducción de instrumentos quirúrgicos, dispositivos de
visualización, válvulas de reemplazo y similares en la cavidad
torácica, para facilitar la reparación y el reemplazo de la válvula
del corazón. Preferiblemente, los dispositivos han de facilitar el
reemplazo de una válvula del corazón con varios tipos de prótesis,
incluyendo prótesis mecánicas y biológicas, homoinjertos y
aloinjertos.
La invención proporciona un dispositivo para
realizar procedimientos quirúrgicos menos invasivos en el interior
de un órgano o vaso sanguíneo, y particularmente en el interior del
corazón y los grandes vasos sanguíneos de la cavidad torácica. El
dispositivo de la invención facilita la intervención en el interior
del corazón o grandes vasos sanguíneos sin la necesidad de una
esternotomía media u otra forma de gran toracotomía, reduciendo
substancialmente los traumatismos, el riesgo de complicaciones, el
tiempo de recuperación, y el dolor al paciente. Usando el
dispositivo de la invención, pueden realizarse procedimientos
quirúrgicos a través de penetraciones percutáneas dentro de los
espacios intercostales del conjunto de costillas del paciente, sin
cortar, retirar o desplazar de manera significativa ninguna
costilla del paciente o su esternón. La invención facilita el
reemplazo de la válvula con cualquiera de una variedad de válvulas
de reemplazo comercialmente disponibles, incluyendo prótesis
mecánicas, bioprótesis, homoinjertos, y aloinjertos.
Según la presente invención, se proporciona un
dispositivo toracoscópico según la reivindicación 1 y un conjunto
de prótesis que incluye el dispositivo y una válvula de
reemplazo.
En una realización preferida, la válvula de
reemplazo y los medios de soporte juntos tienen un perfil con una
anchura que es menor que la anchura del espacio intercostal.
Preferiblemente, el espacio intercostal es menor de 20 mm de
anchura. La porción de fijación de la válvula de reemplazo tendrá
usualmente un diámetro externo que es mayor que la anchura
intercostal.
Los medios de soporte del dispositivo comprenden
un asa alargada que tiene un extremo distal montado en la válvula
de reemplazo y un extremo proximal opuesto al extremo distal. El
asa está configurada para introducir la válvula de reemplazo en el
interior del corazón del paciente a través del espacio intercostal.
Preferiblemente, el asa tiene una longitud de por lo menos unos 20
cm para permitir la colocación de la válvula de reemplazo en el
corazón desde una porción lateral derecha del pecho del paciente.
El asa también puede incluir medios para liberar la válvula de
reemplazo, estando configurados los medios de liberación para su
accionamiento desde el extremo proximal del asa.
El asa también incluye medios para pivotar la
válvula de reemplazo desde una primera orientación para su
introducción a través del espacio intercostal hasta una segunda
orientación para su fijación en el corazón del paciente. Los medios
de pivote están configurados para su accionamiento desde un extremo
proximal del asa. De esta manera, la válvula de reemplazo se puede
introducir primero por el borde a través del espacio intercostal, a
continuación pivotar alrededor de un eje generalmente perpendicular
al asa en una orientación adecuada para su fijación en el interior
del corazón del paciente.
Preferiblemente, la válvula de reemplazo está
montada previamente con los medios de soporte y los dos están
esterilizados y empaquetados juntos en un paquete estéril. De esta

La invención proporciona un conjunto de prótesis
para la colocación de una válvula de reemplazo en una posición de
la válvula del corazón de un paciente. El conjunto de prótesis
comprende un asa alargada configurada para colocarse a través de un
espacio intercostal en el pecho del paciente, tal como se ha
descrito anteriormente. El dispositivo incluye medios en un extremo
distal del asa para soportar de manera que se pueda liberar una
válvula de reemplazo en una orientación para su introducción a
través del espacio intercostal, y también incluye medios para
pivotar la válvula de reemplazo respecto al asa desde una primera
orientación para su introducción a través del espacio intercostal, a
una segunda orientación para su colocación en la posición de la
válvula. El conjunto de prótesis también incluye, en una
realización preferida, medios en el extremo proximal del asa para
liberar la válvula de reemplazo de los medios de soporte una vez la
prótesis se ha colocado y fijado en el corazón.

las ventajas de la invención se puede realizar con referencia a las

mitral, debe entenderse que la invención será útil para realizar una
gran variedad de procedimientos quirúrgicos, incluyendo la
reparación y el reemplazo de las válvulas aórtica, tricúspide, o
pulmonar. Ventajosamente, la invención facilita la realización de
estos procedimientos a través de penetraciones percutáneas en los
espacios intercostales del conjunto de costillas, evitando la
necesidad de una esternotomía media u otra forma de gran
toracotomía.
Los términos "penetración intercostal
percutánea" y "penetración intercostal" tal como se usan
aquí se refieren a una penetración, en forma de un pequeño corte,
incisión, orificio, cánula, manguito de punción, o similares, a
través de la pared del pecho entre dos costillas adyacentes, en los
que el conjunto de costillas del paciente y el esternón permanecen
substancialmente intactos, sin cortar, retirar, o desplazar de
manera significativa las costillas o el esternón. Estos términos
están pensados para distinguir una gran toractomía tal como una
esternotomía media, en la que el esternón y/o una o más costillas se
retraen de manera significativa, para crear una gran abertura en la
cavidad torácica. Una "penetración intercostal percutánea"
puede topar o solapar las costillas adyacentes entre las que se
forma, pero la anchura máxima de la penetración que está disponible
para la introducción de instrumentos, prótesis y similares en la
cavidad torácica será la anchura del espacio intercostal, rodeado
por dos costillas adyacentes en sus posiciones naturales,
substancialmente no flexionadas. Debe entenderse que se pueden
retraer o flexionar una o más costillas un poco sin apartarse del
ámbito de la invención; sin embargo, la invención busca
específicamente evitar el dolor, los traumatismos, y las
complicaciones que se producen por la gran deflexión o corte de las
costillas en las técnicas a pecho abierto convencionales.
La figura 1 muestra un sistema 20 para el
reemplazo de válvulas a pecho cerrado colocado en un paciente P en
una mesa de operaciones T. Preferiblemente, una cuña o bloque W que
tiene una superficie superior inclinada de 20º a 45º
aproximadamente se coloca bajo el lado derecho del paciente P, de
manera que el lado derecho del cuerpo del paciente está algo más
alto que el lado izquierdo. El brazo derecho A del paciente se
permite que gire hacia abajo para apoyarse sobre la mesa T,
exponiendo el lado lateral derecho del pecho del paciente.
El sistema de reemplazo de la válvula 20 incluye
una cánula de acceso 22 colocada de manera percutánea en un espacio
intercostal entre dos costillas (mostradas en líneas de trazos) en
un lado lateral derecho del pecho del paciente. Unos manguitos de
punción toracoscópicos 24 de construcción convencional están
colocados en espacios intercostales en el lateral derecho inferior
y superior del pecho para acceder a la cánula 22, asó como en la
porción anterior (o vental) derecho del pecho. Un endoscopio 25 de
construcción convencional se coloca a través de una penetración
intercostal percutánea en el interior del pecho del paciente,
usualmente a través de uno de los manguitos de punción 24. El
extremo distal del endoscopio 25 (mostrado en línea de trazos) está
preferiblemente configurado para ver en un ángulo entre unos 30º y
90º respecto al eje del endoscopio 25, para facilitar la
visualización del corazón desde la porción derecha de la cavidad
torácica. Una fuente de luz (no representada) está también prevista
en el endoscopio 25 para iluminar la cavidad torácica. Un primer
anillo de organización de sutura 30 está montado en un extremo
proximal de la cánula de acceso 22. Un segundo anillo de
organización 32 está montado en un pie de soporte 34 fijado a la
mesa T. Una válvula de reemplazo 36 está sujeta en el extremo
distal de un introductor 38 entre el primer anillo de organización
30 y el segundo anillo de organización 32. El introductor 38 se
extiende a través del segundo anillo de organización 32 y está
soportado mediante el pie de soporte 34. Los instrumentos
adicionales que se usan en un procedimiento tal como un retractor
40, así como dispositivos de corte, sutura, grapado, aspiración,
irrigación y otros, se pueden introducir a través de la cánula de
acceso 22, los manguitos de punción 24, y/o pequeñas incisiones
percutáneas en los espacios intercostales del conjunto de las
costillas.
Con referencia ahora a la figura 2, la cánula de
acceso 22 está colocada en el interior de un espacio intercostal I
en el lado lateral derecho del pecho, preferiblemente en el tercer,
cuarto, quinto o sexto espacio intercostal entre las costillas R
adyacentes. Unos manguitos de punción adicionales 24A, 24B están
colocados en el interior de espacios intercostales superior e
inferior para acceder a la cánula 22 en el lado lateral derecho del
pecho. La cánula de acceso 22 y los manguitos de punción 24A, 24B
están colocados de manera que los instrumentos 42 introducidos a
través de los mismos se pueden dirigir hacia el lado derecho de la
aurícula izquierda del corazón H. Un manguito de punción 24C está
colocado en un espacio intercostal en el lado anterior derecho del
pecho, de manera que el endoscopio 25 se puede introducir para ver
la cavidad torácica y el corazón H sin interferir con los
instrumentos introducidos a través de la cánula de acceso 22 o los
manguitos de punción 24A, 24B. Un manguito de punción adicional 24D
está colocado en un espacio intercostal en el lado anterior del
pecho justo a la derecha del esternón y anterior al lado lateral
derecho del corazón H.
Se entenderá por parte de los técnicos en la
materia que, en algunos casos, puede ser deseable eliminar algunos
o todos los manguitos de punción 24 y/o la cánula de acceso 22, e
introducir los instrumentos directamente a través de pequeñas
incisiones percutáneas intercostales en el pecho. Ventajosamente, a
diferencias de los procedimientos de laparoscopia, artroscopia y
otros procedimientos endoscópicos, no se requiere ninguna
distensión del pecho usando el procedimiento de la invención, de
manera que no hay que tener en cuenta el escape de fluido de
distensión a través de las penetraciones percutáneas. De esta
manera, se pueden utilizar manguitos de punción toracoscópicos sin
sellos de fluido o incisiones percutáneas para la introducción de
los instrumentos en la cavidad torácica. Se prefieren generalmente
los manguitos de punción, sin embargo, para proporcionar un paso
abierto en la cavidad torácica, para proteger el tejido adyacente
de las lesiones resultantes del contacto con los instrumentos, y
para evitar dañar los instrumentos, endoscopios, válvulas de
reemplazo, y similares cuando se introducen en la cavidad
torácica.
Con referencia ahora a las figuras
4A-4D, se describirá en mayor detalle la cánula de
acceso 22. La cánula de acceso 22 comprende un cuerpo 44 que tiene
un extremo proximal 46, un extremo distal 48, y un paso 50 que se
extiende entre los mismos. El cuerpo 44 está configurado para
ajustarse en un espacio intercostal I sin una deflexión
significativa de las costillas R adyacentes, teniendo usualmente
una anchura de menos de unos 20 mm. El paso 50 está configurado
para facilitar el paso de la válvula de reemplazo 36 a través del
mismo. La válvula de reemplazo 36 puede tener una serie de
configuraciones, pero ha de tener un diámetro por lo menos igual al
de la válvula del corazón natural del paciente, un diámetro que
comúnmente excede la anchura de los espacios intercostales en el
conjunto de las costillas. De esta manera, para evitar el corte o
la retracción de las costillas del paciente, la válvula de reemplazo
36 se introduce primero por el borde a través del paso 50 de la
cánula de acceso 22, tal como se describe de una manera más
completa a continuación. Para alojar esta introducción de la
válvula de reemplazo 36, el paso 50 usualmente tiene una anchura en
sección transversal w de unos 12 mm a 20 mm, y una altura en sección
transversal h que es algo mayor que la anchura en sección
transversal w, usualmente de 2 a 6 veces la anchura de la sección
transversal w, y preferiblemente en el rango de 25 mm a 50 mm. El
paso 50 puede tener varias formas en sección transversal,
incluyendo oval, rectangular, pista de carreras y similar. Esto
incluye una variedad de válvulas de corazón de reemplazo,
incluyendo prótesis mecánicas y biológicas, así como válvulas de
tejido de homoinjerto y aloinjerto. Se entenderá, sin embargo, que
ciertas válvulas de reemplazo pueden ser plegables o
suficientemente pequeñas de tamaño de manera que el paso 50 en la
cánula de acceso 22 puede tener una sección transversal redonda o
cuadrada y permita también el paso de la válvula de reemplazo a
través del mismo. Sin embargo, una forma en sección transversal en
la que la altura es mayor que la anchura también puede ser
ventajosa para permitir una mayor libertad de movimiento en la
manipulación de la válvula de reemplazo y otros instrumentos
introducidos a través del paso 50.
Con referencia a la figura 3, una vez todas las
suturas 198 se han retirado de la cavidad torácica y se han
colocado en las ranuras 62 del anillo de organización 30, las
suturas se aplican a la válvula de reemplazo 36, sujeta en posición
mediante el introductor 38. La válvula de reemplazo 36 puede ser
cualquiera de una variedad de prótesis comercialmente disponibles,
incluyendo las mecánicas y las bioprotésicas, en espiral y no en
espiral, tal como se describe en Bodnar y Frater, "Replacement
Cardiac Valves" (Válvulas cardiacas de reemplazo), pp.
4-7, y en Jamieson, "Modern Cardiac Valve Devices
- Bioprostheses and Mechanical Prostheses: State of the Art"

8:89-98 (1993). Las válvulas mecánicas pueden ser
del tipo de bola encajada, tal como la válvula

válvula Medtronic Hall (Medtronic, Inc., Minneapolis, MN), la
válvula Bjork-Shiley Monostrut (Shiley, Inc.,
Irving, CA), la válvula Omniscience® (Omnisciente Medical Inc.,
Grove Heights, MN), así como el tipo de doble hoja tal como la
válvula St. Jude Medical (St. Jude Medical, Inc., St. Paul, MN), la
válvula Baxter Duromedics (Baxter Healthcare Corp., Edwards CVS
Div., Irvine, CA), la válvula Carbomedics (Carbomedics, Inc.,
Austin, TX), o la válvula Sorin (Sorin Biomedica, Saluggia,
Italia). Las válvulas bioprotésicas pueden ser válvulas aórticas
porcinas, tal como la bioprótesis Hancock II (Medtronic, Inc.,
Minneapolis, MN), la bioprótesis supraanular
Carpentier-Edwards (Baxter Healthcare Corp., Edwards
CVS Div., Irvine, CA), la bioprótesis sin espiral
Carpentier-Edwards (Baxter Healthcare Corp., Edwards
CVS Div., Irvine, CA), la bioprótesis St.
Jude-Bioimplant (St. Jude Medical, Inc., St. Paul,
MN), o la bioprótesis Medtronic Intact® (Medtronic, Inc.,
Minneapolis, MN), así como las válvulas pericardiacas tales como la
bioprótesis Mitroflow (Mitroflow Internacional, Inc., Richmond,
British Columbia, Canadá), o las bioprótesis pericardiacas
Carpentier-Edwards (Baxter Healthcare Corp., Edwards
CVS Div., Irving, CA). La invención también facilita el reemplazo
de las válvulas con homoinjertos y aloinjertos, así como con una
variedad de válvulas de reemplazo no listadas aquí de manera
específica.
En una realización a modo de ejemplo, la
invención facilita el reemplazo de la válvula mitral de un paciente
con una válvula de reemplazo mecánica de doble hoja tal como la
válvula St. Jude Medical, representada en las figuras 5A a 5C. En
esta realización, la válvula de reemplazo 36 comprende un marco en
forma de anillo 218 y un par de hojuelas 220 montadas de manera
pivotante en el marco 218. En la configuración abierta mostrada en
las figuras 5A y 5B, las hojuelas 220 están casi paralelas entre sí,
proporcionando un paso de flujo 222 a través del cual la sangre
puede fluir en la dirección de las flechas 224. En el caso de
presión de fluido contra las caras internas 226 de las hojuelas 220,
las hojuelas 220 pivotan a una configuración cerrada, bloqueando el
paso del flujo 222. Un anillo de costura 228 está fijado al marco
218, al cual se pueden aplicar suturas 198 para fijar la válvula de
reemplazo 36 en el corazón.
Tal como se representa en las figuras 5B y 5C, la
válvula de reemplazo 36 se puede montar al introductor 38 para la
introducción en el corazón a través del paso 5 de la cánula de
acceso 22. La válvula de reemplazo 36 puede tener varios tamaños
según el tamaño de la válvula mitral que se reemplaza. Sin embargo,
el diámetro externo del anillo de costura 228 es usualmente entre 19
mm y 35 mm, que, para la mayoría de pacientes adultos, es mayor que
la anchura de los tercer, cuarto, quinto o sexto espacios
intercostales, que varían entre 15 mm y 20 mm de anchura. La altura
de la válvula de reemplazo 36, por otro lado, es menor que la
anchura de estos espacios intercostales, que es usualmente de unos 8
mm a 15 mm. De esta manera, el paso 50 se configura para permitir
que la válvula de reemplazo 36 pase a través del mismo en una
primera orientación de borde, tal como se representa en la figura
5C.
El introductor 38 se describirá ahora con
referencia a las figuras 6 a 8. El introductor 38 incluye un eje
230 que tiene un extremo distal 232, un extremo proximal 234, y un
lumen interno 236 entre los mismos. El eje 230 tiene una longitud
seleccionada para permitir la colocación de la válvula de reemplazo
36 en la posición de la válvula mitral en el interior del corazón
del paciente desde el exterior de la cavidad torácica del paciente,
y usualmente a una longitud de por lo menos unos 20 cm, y
preferiblemente de unos 25 cm a 35 cm de longitud. Un asa 238 está
fijada al extremo proximal 234, y un pomo giratorio 240 está
montado en el asa 238 para girar la válvula de reemplazo 36 respecto
al eje 230. Un anillo de tracción 242 se extiende de manera próxima
desde el pomo de pivote 240 para liberar la válvula de reemplazo 36
del introductor 38. Tal como se aprecia mejor en las figuras 8A y
8B, una varilla de empuje 244 se extiende a través del lumen
interno 236, y está acoplado en su extremo distal 248 a un pivote
250 que está montado de manera giratoria en el interior de una
ranura 252 en el extremo distal 232 del eje 230. Un vástago 254 se
extiende distalmente desde el pivote 250 y tiene roscas u otros
medios para su fijación a un soporte de válvula 255 para la válvula
de reemplazo 36. El pomo 240 está fijado a un eje roscado 256
alojado en el interior de un orificio roscado 258 en el asa 238,
con lo cual la rotación del pomo 240 mueve el eje roscado 256 de
manera distal o proximal, dependiendo de la dirección de rotación.
La varilla de empuje 244 tiene un extremo proximal 260 que se
acopla con un extremo distal 262 del eje roscado 256. Un muelle 264
empuja la varilla de empuje 244 en una dirección proximal contra el
extremo distal 262. De esta manera, la rotación del pomo 240 hace
estirar o empujar la varilla de empuje 244, haciendo girar de esta
manera el pivote 250 de manera que el vástago 254 se extiende distal
o lateralmente.
Con referencia a las figuras 7A a 7G, el soporte
de la válvula 255 incluye un brazo estacionario 266 fijado a un
vástago 254, y un brazo desplazable 268 montado de manera giratoria
en el brazo estacionario 266. Cada uno de los brazos 266, 268 tiene
un canal anular 270 configurado para acoplar el marco 218 de la
válvula de reemplazo 36 en el interior del canal de flujo 222
(figura 5A). Los brazos 266, 268 están también dimensionados y
configurados para introducirse a través del paso 50 de la cánula de
acceso 22 cuando la válvula de reemplazo 36 se sujeta en los
canales 270. Tal como se representa en la figura 7A, cuando está
sujeta al vástago 254 sobre el introductor 38, el soporte de la
válvula 255 se puede pivotar en la dirección de la flecha 272
mediante rotación del pomo 240. De esta manera, la válvula de
reemplazo 36 sujeta por el soporte 255 se introducir primero por el
borde a través del paso 50 en la cánula de acceso 22, a
continuación girarse aproximadamente 90º a una orientación adecuada
para su fijación en la posición de la válvula mitral en el interior
del corazón H.
Para facilitar la liberación de la válvula de
reemplazo 36 del soporte 55 desde una posición fuera del cuerpo del
paciente, un cable de tracción 274 está acoplado al brazo
desplazable 268 mediante, por ejemplo, una bola de anclaje 276
dispuesta en el interior de una abertura 278 (ver la figura 8A). El
cable de tracción 274 se extiende a través de un lumen interno (no
representado) en la varilla de empuje 244, y está fijado en su
extremo proximal 280 al anillo de tracción 242. Un muelle 282 en el
interior de una abertura 284 en el pomo 240 empuja el anillo de
tracción 242 en una dirección distal. De esta manera, estirando del
anillo de tracción 242 se gira el brazo desplazable 268 tal como se
muestra en la figura 7C, permitiendo que la válvula de reemplazo 36
se retire de los canales. La bola de anclaje 276 y/o el anillo de
tracción 242 se pueden configurar para que se puedan retirar del
cable de tracción 244, permitiendo que el soporte de la válvula 255
se retire del introductor 38 mediante el desacoplamiento del brazo
266 del vástago 254.
Para mantener la válvula de reemplazo 36 sobre el
soporte 255 cuando el soporte 255 no está fijado al introductor 38,
un par de orificios 286 están previstos en el brazo 266 en
alineación con un correspondiente par de orificios 288 en el brazo
268. Cuando la válvula de reemplazo 36 se ha colocado sobre el
soporte 255, se puede atar una sutura (no representada) a través de
los orificios 286, 288 para evitar el giro del brazo 268,
reteniendo así la válvula de reemplazo 36 sobre el soporte 255. Una
vez se ha fijado el soporte 255 al introductor 38, la sutura se
puede retirar, permitiendo que el brazo 268 gire en respuesta a la
rotación del pomo 240.
Será frecuentemente deseable que el soporte de la
válvula 255 y la válvula de reemplazo 36 estén montadas
previamente, esterilizadas, y empaquetadas juntas en un único
paquete estéril. De esta manera, al abrir el paquete estéril en el
quirófano, la válvula de reemplazo 36 y el soporte 255 están listos
para su uso quirúrgico inmediato. Además, puede ser deseable que el
introductor 38 se esterilice con la válvula de reemplazo 36 y que
esté incluido en el mismo paquete estéril. En estos casos, el
soporte 255 puede estar integrado con el introductor 38 y no sea
amovible del mismo, con la válvula de reemplazo 36 montada en los
brazos 266, 268 en el extremo distal del introductor 38 en el
interior de un paquete estéril. Alternativamente, el introductor 38
puede ser un dispositivo reutilizable que está fijado al soporte
255 y a la válvula de reemplazo 36 en el quirófano en el momento
del procedimiento.
Claims (9)
1. Conjunto de prótesis (38) para el reemplazo a
pecho cerrado de una válvula del corazón, comprendiendo el conjunto
de prótesis:
una válvula de reemplazo (36); y
medios de soporte (255) que comprenden un asa
alargada (230) que tiene un extremo distal (232) montado en la
válvula de reemplazo (36) y un extremo proximal (234) opuesto al
extremo distal (232), caracterizado por el hecho de que el
asa (230) comprende medios (250) para pivotar la válvula de
reemplazo (36) desde una primera orientación para la introducción
primero del borde a través de un espacio intercostal (I) en el
pecho del paciente a una segunda orientación para su fijación en el
interior del corazón (H) del paciente, estado configurados los
medios pivotantes (250) para su accionamiento desde el extremo
proximal (234) del asa (230).
2. Conjunto de prótesis según la reivindicación
1, en el que la prótesis de la válvula tiene un anillo de fijación
(228) que tiene un eje y el soporte (255) está configurado para
fijarse al asa (230) de manera que el eje longitudinal del asa está
aproximadamente perpendicular al eje del anillo de fijación cuando
la prótesis de la válvula está en su segunda orientación.
3. Conjunto de prótesis según la reivindicación 1
o la reivindicación 2, en el que el asa (230) tiene una longitud de
por lo menos 20 cm.
4. Conjunto de prótesis según una cualquiera de
las reivindicaciones 1 a 3, en el que la prótesis de la válvula (36)
es una prótesis de válvula mecánica.
5. Conjunto de prótesis según una cualquiera de
las reivindicaciones anteriores, que también comprende un mecanismo
de liberación configurado para liberar la prótesis de la válvula
(36) del soporte (255).
6. Conjunto de prótesis según una cualquiera de
las reivindicaciones 1 a 5, en el que la prótesis de la válvula (36)
puede pivotar según un ángulo de unos 90º.
7. Conjunto de prótesis según una cualquiera de
las reivindicaciones anteriores, en el que la prótesis de la válvula
(36) tiene un diámetro externo de por lo menos unos 20 mm.
8. Conjunto de prótesis según una cualquiera de
las reivindicaciones 1 a 7, en el que la válvula (36) y los medios
de soporte (255) están empaquetados juntos en un paquete
estéril.
9. Conjunto de prótesis según una cualquiera de
las reivindicaciones 1 a 8, en el que la válvula de reemplazo (36)
tiene una porción de fijación anular y una porción de válvula
desplazable acoplada a la porción de fijación.
