Description
Aparato para aplicar grapas quirúrgicas.
El aparato se refiere a un aparato de grapado
quirúrgico y, más particularmente, a un aparato para aplicar
secuencialmente una pluralidad de grapas quirúrgicas para tejido
corporal y, opcionalmente, cortar el tejido sujeto.
Los dispositivos quirúrgicos en los que el tejido
primero es agarrado o afianzado entre una estructura de mordazas
opuestas y después es unido por medio de sujetadores quirúrgicos
son bien conocidos en la tecnología. En algunos instrumentos se
dispone de un bisturí para cortar el tejido que ha sido unido por
los sujetadores. Los sujetadores tienen típicamente la forma de
grapas quirúrgicas, sin embargo, también se utilizan dos
sujetadores polímeros de dos partes.
Instrumentos con este propósito pueden comprender
dos miembros alargados que son utilizados respectivamente para
capturar o afianzar el tejido. Típicamente, uno de estos miembros
lleva un cartucho desechable que aloja una pluralidad de grapas
dispuestas en la menos dos filas laterales, mientras que el otro
miembro comprende un yunque que define una superficie para formar
las patas de grapa mientras que las grapas son impulsadas desde el
cartucho. Generalmente, la operación de grapado es efectuada por un
empujador que se desplaza longitudinalmente a través del miembro
portador de cartucho, actuando el empujador sobre las grapas para
eyectarlas secuencialmente desde el cartucho. Un cuchillo o bisturí
puede desplazarse con el empujador entre las filas de grapas para
cortar longitudinalmente y/o abrir el tejido grapado entre las filas
de grapas. Dichos instrumentos se describen en la patente
norteamericana No. 3.079.606, y en la patente norteamericana No.
3.490.675.
Una grapadora posterior descrita en la patente
norteamericana No. 3.499.591 aplica una fila doble de grapas sobre
cada lado de la incisión. Esto se lleva a cabo proporcionando un
conjunto de cartucho en el que un miembro de leva se mueve a través
de una trayectoria de guía alargada entre dos conjuntos de ranuras
portadoras de grapa escalonadas. Se sitúan miembros impulsores de
grapa dentro de las ranuras y se sitúan de manera que sean
contactadas por la leva que se mueve longitudinalmente para efectuar
la eyección de las grapas. Otros ejemplos de dichas grapadoras se
describen en la patente norteamericana No. 4.429.695.
Cada uno de los instrumentos descritos
anteriormente fueron diseñados para uso en procedimientos
quirúrgicos convencionales en los que los cirujanos tienen acceso
manual directo al lugar de operación. Sin embargo, en procedimientos
endoscópicos o laparoscópicos, la cirugía se lleva a cabo a través
de una pequeña incisión o a través de una cánula estrecha insertada
a través de pequeñas heridas de entrada en la piel. Para tratar las
necesidades específicas de procedimientos quirúrgicos endoscópicos
y/o laparoscópicos, se han desarrollado dispositivos de grapado
quirúrgico endoscópico y se describen en las patentes
norteamericanas Nos. 5.040.715; 5.312.023; 5.318.221; y 5.326.013.
En general, estos instrumentos están provistos de una estructura
afianzadora para efectuar la aproximación de un yunque y un
cartucho para asegurar tejido entre ellos, y una estructura de
disparo de grapas para efectuar la eyección secuencial de una
pluralidad de grapas desde el cartucho después de que se haya
asegurado el tejido. Sin embargo, sería beneficioso proporcionar un
dispositivo de grapado endoscópico que tuviera una estructura de
actuación configurada para efectuar una aproximación progresiva de
un yunque y un cartucho concomitante con la eyección secuencial de
una pluralidad de grapas desde el cartucho.
Los documentos
EP-A-593 920 y
US-A-5 170 925 describen grapadoras
quirúrgicas que tienen las características técnicas de la parte
precaracterizadora de la reivindicación 1 adjunta.
El presente invento se define en la
reivindicación 1 adjunta. Las reivindicaciones independientes están
dirigidas a características óptimas y preferidas.
Se proporciona un aparato quirúrgico para
efectuar la aplicación secuencial de una pluralidad de sujetadores
quirúrgicos a tejido corporal. El aparato incluye una porción de
mango, una porción de cuerpo que se extiende desde la porción de
mango, un alojamiento de cartucho soportado en una porción de
extremo alejada de la porción de cuerpo y que contiene una
pluralidad de sujetadores quirúrgicos y que define una superficie
de contacto con tejido, y un miembro de yunque que define una
superficie formadora de sujetador contra la que son impulsadas los
sujetadores eyectados desde el conjunto de cartucho.
El miembro de yunque está montado en un lugar
separado de la porción de mango y es movible entre una primera
posición, en la que la superficie formadora de sujetador o grapa
está separada de la superficie que contacta con tejido y una
posición cerrada en la que la superficie formadora de grapa y la
superficie que contacta con tejido están en alineamiento
cooperativo más próximo. El aparato incluye además un actuador que
está configurado para mover progresivamente el miembro de yunque
desde la posición abierta hasta la posición cerrada y eyectar
concomitantemente una pluralidad de grapas quirúrgicas desde el
cartucho para ser conformadas contra el yunque. Se dispone de un
mecanismo impulsor para impulsar el actuador. El mecanismo impulsor
es operable desde la porción de mango y se extiende a través de la
porción de cuerpo hasta el miembro de yunque.
En una realización preferida del aparato
quirúrgico, el alojamiento de cartucho y el miembro de yunque
tienen cada uno una ranura correspondiente que se extiende
longitudinalmente, formada en ellos para acomodar la traslación del
actuador. El actuador incluye preferiblemente un viga de leva
superior, una porción central transversal, y una viga de soporte
inferior paralela a la viga de leva superior. Una pluralidad de
placas de leva separadas están dispuestas sobre el actuador y están
configuradas para acoplarse a una pluralidad de empujadores de
grapa dispuestos dentro del alojamiento de cartucho para efectuar
la eyección de grapas quirúrgicas desde él.
En una realización, una cuchilla cortante está
montada sobre el actuador para cortar tejido grapado mientras que
el actuador se desplaza a través del conjunto de aplicación de
grapas. La cuchilla cortante está formada preferiblemente sobre un
borde delantero de la porción central del actuador.
En una realización, el mecanismo impulsor incluye
un sistema de impulsión neumática que tiene un recipiente de fluido
comprimido dispuesto dentro de la porción de mango, una cámara de
pistón que se extiende longitudinalmente, definida en la porción de
mango, un pistón configurado para desplazarse a través de la cámara
de pistón, y un eje impulsor que se extiende desde el pistón hasta
el actuador. Un mecanismo de gatillo se asocia preferiblemente al
sistema de impulsión neumática para efectuar una descarga de fluido
desde el recipiente hasta la cámara de pistón para impulsar el
pistón a través de la cámara. En una realización alternativa, se
puede proporcionar un sistema de impulsión manual que incluye un
mango de actuación pivotante asociado con dicha porción de mango y
conectado operativamente a un eje impulsor alargado que se extiende
hasta el actuador.
Características adicionales del aparato
quirúrgico serán evidentes más fácilmente para aquellos expertos en
la materia a partir de la siguiente descripción detallada, tomada
en relación con los dibujos.
Se describirán aquí a continuación realizaciones
preferidas del aparato quirúrgico con referencia a los dibujos, en
los que:
La figura 1 es una vista en perspectiva de un
aparato de grapado quirúrgico construido de acuerdo con una
realización preferida;
la figura 1A es una vista en perspectiva del
aparato quirúrgico de la figura 1 que se extiende a través de una
aguja de punción en la cavidad abdominal del paciente para grapar
tejido corporal;
la figura 2 es una vista en perspectiva, en
despiece ordenado, del conjunto aplicador de grapa del aparato
quirúrgico ilustrado en la figura 1;
la figura 3 es una vista en perspectiva del
conjunto aplicador de grapas del aparatado quirúrgico de la figura
1, con el cartucho retirado del alojamiento de cartucho para
ilustrar la estructura actuadora del instrumento;
la figura 4 es una vista en alzado lateral del
aparato quirúrgico de la figura 1 antes de una operación de
aplicación de grapa;
la figura 5 es una vista en alzado lateral, en
sección transversal parcial, del aparato de la figura 1 durante las
etapas iniciales de una operación de aplicación de grapa; y
La figura 6 es una vista en alzado lateral en
sección transversal parcial del aparato de la figura 1, en la
conclusión de la operación de aplicación de grapa.
En los dibujos y en la descripción que sigue, el
término "próximal", como es tradicional, se referirá al
extremo del aparato que está más próximo al operador, mientras que
el término "distal" se referirá al extremo del aparato que
está más alejado del operador.
El presente aparato se explicará en términos
tanto de procedimientos como de aparato endoscópicos. Sin embargo,
aquí el uso de términos tales como "endoscópico",
"endoscópicamente", y "porción endoscópica", entre otros,
no se ha interpretar para limitar el aparato para uso solamente en
relación con un tubo endoscópico. Al contrario, se cree que el
aparato puede encontrar uso en procedimientos en los que el acceso
está limitado a una pequeña incisión que incluye, pero no está
limitada a, procedimientos artroscópicos y/o laparoscópicos.
Haciendo ahora referencia a los dibujos, en los
que los mismos números de referencia identifican elementos
estructurales similares, se ilustra en la figura 1 una grapadora
quirúrgica construida de acuerdo con una realización preferida y
designada generalmente por el número de referencia 10. En resumen,
la grapadora quirúrgica 10 incluye una porción de mango 12, una
porción de cuerpo tubular alargada 14 que se extiende distalmente
de la porción de mango 12, y un conjunto de aplicación de grapa 16
asociado operativamente con un extremo distal de la porción de
cuerpo 14.
El conjunto de aplicación de grapa 16 incluye un
cartucho de grapa 18 que aloja una pluralidad de sujetadores o
grapas quirúrgicas y un miembro de yunque 20 asociado
pivotablemente con el cartucho de grapas y que define una superficie
de conformación contra la cual se impulsan grapas cuando son
eyectadas desde el cartucho 18. Como se ve mejor en la figura 1, se
proporciona un botón de control de rotación 22 en el extremo
proximal de la porción de cuerpo 14 y está montado de tal manera
que facilita la rotación de la porción de cuerpo 14 y del conjunto
de aplicación de grapas 16 alrededor de un eje longitudinal definido
por la porción de cuerpo 14. Tal como se ilustra en la figura 1A,
durante un procedimiento endoscópico, la porción de cuerpo 14 del
aparato quirúrgico 10 se extiende a través de una aguja de punción
o trocar 15 hasta la cavidad abdominal del paciente. Después de
ello, se captura tejido entre el cartucho 18 y el yunque 20, y se
aplican grapas al tejido. La operación del aparato quirúrgico 10 se
explicará en detalle aquí a continuación.
Haciendo referencia a la figura 2, el cartucho de
grapa 18 está soportado dentro de un alojamiento de cartucho
alargado 24 que tiene paredes laterales opuestas 26 y 28 y un suelo
30. Como se verá mejor en la figura 3, el alojamiento de cartucho
24 está soportado en el extremo distal de la porción de cuerpo 14.
Muescas 26a y 28a están formadas en las paredes laterales 26 y 28,
respectivamente, para soportar pivotablemente un par de aletas
correspondientes 36 y 38 que se extienden hacia fuera del miembro
de yunque 20. Tal como se muestra en las figuras 2 y 4, muelles 40
y 42 están asociados al miembro de yunque 20 para cargar el yunque
hacia una posición abierta con respecto al cartucho 18. En una
realización alternativa, el miembro de yunque 20 puede unirse
directamente a la porción de cuerpo 14. En cualquier realización, el
miembro de yunque 20 está situado distalmente de la porción de
mango, y el conjunto de disparo de grapa es actuado a distancia por
la porción de mango.
Alternativamente, se podría proporcionar un
muelle para cargar el yunque hasta una posición cerrada. El muelle
podría extenderse, por ejemplo, desde la porción proximal del
miembro de yunque 20 y conectarse a la porción de cuerpo 14 o
alojamiento 24. El tejido corporal, insertado entre el yunque y el
cartucho, movería el yunque alejándose del cartucho 18. El yunque
estaría entonces afianzado de manera suelta sobre el tejido hasta
que el mecanismo de actuación descrito más abajo sea actuado para
afianzar más firme y progresivamente el tejido mientras que las
grapas son disparadas.
Haciendo referencia a las figuras 2 y 3, se
asocia un mecanismo de actuación 50 al conjunto de aplicación de
grapas 16 para efectuar la aproximación progresiva del yunque 20 y
del cartucho 18 para afianzar tejido entre ellos, y efectuar
concomitantemente la eyección secuencial de una pluralidad de
grapas quirúrgicas 34 desde el cartucho 18. Más particularmente, el
mecanismo de actuación 50 está configurado para trasladarse a través
de conjunto de aplicación de grapas 16, esto es, a través del
miembro de yunque 20 y cartucho 18, en una dirección distal, para
mover gradualmente por medio de leva el miembro de yunque 20 hasta
una posición cerrada (o completamente afianzada) y, a la vez,
activar una pluralidad de impulsores de grapas 35 para impulsar
grapas 34 desde el cartucho 18. Un ejemplo de un impulsor de grapa
adecuado se describe en la patente norteamericana cedida en común
No. 4.978.049.
El mecanismo de actuación 50 incluye una viga de
soporte alargada 52 que tiene placas de leva superior e inferior 54
y 56 dispuestas en su extremo distal, y un bloque de montaje 58
dispuesto en su extremo proximal. Una ranura alargada 60 está
formada en el miembro de yunque 20 para acomodar la translación del
mecanismo de actuación 50 durante un procedimiento de grapado. Se
forma una ranura correspondiente 62 en el suelo 30 del alojamiento
de cartucho 24. En uso, cuando el mecanismo de actuación 50 se
traslada a través del conjunto de aplicación de grapas 16, la placa
de leva superior 54 se acopla a la superficie superior (exterior)
20a del miembro de yunque 20 para impulsarlo hacia el cartucho 18,
y la placa de leva inferior 56 se acopla a la superficie inferior
(exterior) 30a de suelo 30 para equilibrar las fuerzas compresivas
impartidas al yunque por la placa de leva superior. Tal como se
muestra, las placas de leva superior e inferior 54, 56 son
sustancialmente planas.
El mecanismo de actuación 50 incluye además
barras de disparo de grapas 70a-70d configuradas
para activar impulsores de grapas 35. Las barras de disparo se
acoplan a un bloque de montaje 58 en sus extremos proximales
respectivos por medio de un gancho de acoplamiento 74 que es
recibido en una ranura de acoplamiento correspondiente 76. Los
extremos distales de cada una de las barras de disparo incluyen una
superficie de leva acuñada 78 que se acopla con empujadores de
grapas 35 mientras el mecanismo de actuación 50 se traslada a
través del conjunto de aplicación de grapas 16. El borde delantero
de la viga de soporte 52 define una cuchilla de corte 80 que está
configurada para formar una incisión en el tejido situado entre las
filas paralelas de grapas aplicadas por el conjunto 16.
Haciendo referencia a la figura 5, se asocia
operativamente un sistema de impulsión neumática 90 a una porción
de mango 12 para controlar la traslación del mecanismo de actuación
50 a través del conjunto de aplicación de s sistema impulsor 90
sirve para efectuar el movimiento longitudinal de un eje impulsor 92
que se extiende desde la porción de mango 12, a través de la
porción de cuerpo 14, hasta el conjunto de aplicación de grapas 16.
El extremo distal del eje impulsor 92 incluye una porción de gancho
bifurcada 94, configurada para acoplar ranuras de recepción
correspondientes en el bloque de montaje 58 (véanse también las
figuras 2 y 3).
El extremo proximal del eje impulsor 92 se
extiende desde un pistón 98. El pistón 98 está configurado para
trasladarse a través de una cámara de pistón alargada 100 que está
definida dentro de la porción de mango 12. La cámara de pistón 100
está en comunicación con un conjunto de válvula 102 a través de un
conducto flexible 104. El conjunto de válvula 102 está asociado a un
recipiente cilíndrico 106 que contiene un suministro de gas
comprimido a presión relativamente baja. Un gatillo pivotante 108
está asociado a una porción de mango 112 y está configurado para
interactuar con el conjunto de válvula 102 para controlar la
emisión de gas desde el cartucho de suministro 106 a la cámara de
pistón 100. La estructura y la operación del sistema neumático se
describe en detalle en la patente norteamericana cedida en común
No. 5.312.023.
En operación, como se ilustra en la figura 5,
durante la manipulación del gatillo 108 en la dirección indicada
por la flecha "M", el conjunto de válvula 102 provoca que gas
comprimido fluya desde el recipiente 106, a través del conducto 104,
y hasta la cámara de pistón 100. Como resultado de la expansión del
gas comprimido dentro de la cámara 100, el pistón 98 es impulsado en
una dirección distal, como se indica por medio de la flecha de
referencia "B". En dicho momento, el eje impulsor 92 es
impulsado distalmente, impulsando el mecanismo de actuación 50
hacia el conjunto de aplicación de grapas 16. La traslación distal
del mecanismo de actuación 50 provoca que la placa de leva 54
impulse el miembro de yunque 20 hacia el cartucho 18 para llevar la
superficie de formación de grapa del miembro de yunque 20 hasta una
alineación cooperativa más cercana con la superficie de contacto
con tejido del cartucho 18, afianzando el tejido corporal 110
dispuesto entre ellos. Concomitantemente, impulsores de grapas 35
son activados por la superficie de leva acuñada 78 en cada una de
las barras de disparo de grapas 70a-70d, impulsando
grapas 34 a través del tejido corporal capturado 110. Además, la
cuchilla cortante 80 se desplaza detrás de las barras de disparo de
grapas para cortar el tejido grapado.
Tal como se ilustra en la figura 6, la expansión
del gas comprimido dentro de la cámara de pistón 100, provoca que
el pistón 98 y el mecanismo de actuación 50 se trasladen
distalmente hasta que cada una de las grapas es eyectada desde el
cartucho 18. Después de lo cual, un muelle de retorno helicoidal
112, dispuesto dentro de la cámara de pistón 100 y comprimido por
el movimiento distal del pistón 98, carga el pistón proximalmente,
forzando el eje impulsor 92 y el mecanismo de actuación 50 a
retornar a sus posiciones más distales. Tras ello, el cartucho de
grapa gastado puede ser retirado del alojamiento de cartucho 24 y
reemplazado con una cartucho completamente cargado, y el aparato 10
puede ser utilizado de nuevo.
Las reivindicaciones que siguen identifican
realizaciones del invento adicionales a las descritas en detalle
anteriormente.
Claims (15)
1. Un aparato (10) para aplicar sujetadores o
grapas quirúrgicas a tejido corporal, que comprende:
a) una porción de mango (12);
b) una porción de cuerpo alargada (14) que se
extiende distalmente de la porción de mango;
c) un alojamiento de cartucho (18) soportado en
una porción de extremo distal de la porción de cuerpo y que define
una superficie de contacto con tejido, conteniendo el alojamiento
de cartucho una pluralidad de grapas quirúrgicas;
d) un miembro de yunque (20) que define una
superficie de formación de grapa contra la cual son impulsadas
grapas (34) eyectadas desde el alojamiento de cartucho, estando el
miembro de yunque y el alojamiento de cartucho montados en un lugar

e) un mecanismo de actuación (50) configurado
para aproximar el miembro de yunque y el alojamiento de cartucho
desde la primera posición hasta la segunda posición;
f) el mecanismo de actuación que incluye barras
de disparo (70) para eyectar una pluralidad de grapas quirúrgicas
desde el cartucho para ser conformadas contra el yunque; y
g) un mecanismo impulsor (90) operable desde la
porción de mango y que se extiende a través de la porción de cuerpo
para impulsar el mecanismo de actuación;
estando el aparato caracterizado
porque:
el mecanismo de actuación comprende una viga de
leva superior (54) y una viga de soporte inferior (56) conectadas
por una porción central (52), que se trasladan conjuntamente a
través de un extremo del miembro de yunque y alojamiento de
cartucho hasta el otro extremo, impulsando la viga de leva superior
el miembro de yunque hacia el cartucho e impulsando la viga de
soporte inferior el cartucho hacia el yunque, para de ese modo
aproximar progresivamente el miembro de yunque y el alojamiento de
cartucho durante dicha traslación; y porque dicha viga de leva
superior, viga de soporte inferior y porción central están
conectadas con dichas barras de disparo para moverse conjuntamente
a través del alojamiento de cartucho.
2. Un aparato como el reivindicado en la
reivindicación 1, en el que el alojamiento de cartucho y el miembro
de yunque tienen cada uno una ranura correspondiente que se
extiende longitudinalmente (60, 62), formada en ellos para acomodar
la traslación del mecanismo de actuación.
3. Un aparato como el reivindicado en cualquiera
de las reivindicaciones precedentes, en el que la viga de leva
superior y la viga de soporte inferior son paralelas.
4. Un aparato como el reivindicado en cualquiera
de las reivindicaciones precedentes, en el que las barras de
disparo (70) están configuradas como una pluralidad de barras de
leva separadas configuradas para acoplarse a una pluralidad de
empujadores de grapas (35) dispuestos dentro del alojamiento de
cartucho para efectuar la eyección de grapas o sujetadores
quirúrgicos desde él concomitantemente con el movimiento del yunque
hasta la segunda posición.
5. Un aparato como el reivindicado en cualquiera
de las reivindicaciones precedentes, que comprende además un
sistema de impulsión neumático (90) asociado operativamente con el
conjunto de mango y operable para impulsar el actuador a través del
conjunto aplicador de grapas.
6. Un aparato como el reivindicado en la
reivindicación 5, en el que el sistema de impulsión neumático
incluye un recipiente (106) de fluido comprimido dispuesto dentro
de la porción de mango, una cámara de pistón que se extiende
longitudinalmente (100) definida en la porción de mango, un pistón
(98) configurado para trasladarse a través de la cámara de pistón,
y un eje impulsor (92) que se extiende desde el pistón hasta el
mecanismo de actuación.
7. Un aparato como el reivindicado en la
reivindicación 6, que comprende además un mecanismo de gatillo
(108) asociado con el sistema de impulsión neumática para efectuar
una descarga de fluido desde el recipiente hasta la cámara de
pistón para impulsar el pistón a través de la cámara, estando el
pistón configurado para trasladarse a través de la cámara de pistón
en una dirección generalmente distal, y un muelle de retorno
helicoidal (112) dispuesto dentro de la cámara de pistón para cargar
el pistón en una dirección proximal.
8. Un aparato como el reivindicado en cualquiera
de las reivindicaciones precedentes, que comprende además una
cuchilla cortante (80) montada sobre el mecanismo de actuación para
cortar tejido grapado mientras el mecanismo de actuación se desplaza
a través del miembro de yunque y del alojamiento de cartucho.
9. Un aparato como el reivindicado en la
reivindicación 8, en el que una cuchilla cortante está formada
sobre un borde delantero de la porción central del mecanismo de
actuación para cortar tejido grapado mientras el mecanismo de
actuación se desplaza a través del miembro de yunque y del
alojamiento de cartucho.
10. Un aparato como el reivindicado en cualquiera
de las reivindicaciones precedentes, en el que el miembro de yunque
está cargado por muelle hacia la segunda posición.
11. Un aparato como el reivindicado en cualquiera
de la reivindicaciones precedentes, en el que el mecanismo de
actuación incluye las vigas superior e inferior en un extremo
distal y un bloque de montaje (58) en un extremo proximal para
recibir las barras de disparo de grapas (70).
12. Un aparato como el reivindicado en cualquiera
de la reivindicaciones precedentes, en el que el miembro de yunque
tiene una porción de superficie exterior (20a) opuesta a la
superficie de conformación de grapas y la viga de leva superior se
acopla a la porción de superficie exterior.
13. Un aparato como el reivindicado en la
reivindicación 12, en el que el miembro de yunque incluye una
ranura que se extiende longitudinalmente y dicha porción central se
extiende a través de dicha ranura para que la viga de leva superior
se acople con dicha porción de superficie exterior.
14. Un aparato como el reivindicado en cualquiera
de la reivindicaciones precedentes, en el que dicho alojamiento de
cartucho incluye una porción de superficie exterior (30a) opuesta a
la superficie de contacto de tejido y una ranura que se extiende
longitudinalmente (62), extendiéndose dicho mecanismo de porción
central a través de dicha ranura para que la viga de soporte
inferior se acople con dicha porción de superficie exterior de
dicho alojamiento de cartucho.
15. Un aparato como el reivindicado en las
reivindicaciones 13 ó 14, en el que la viga de leva superior y la
viga de soporte inferior son placas de leva sustancialmente
planas.
