Abstract
Die Erfindung betrifft die Verwendung ausgewählter CGRP-Antagonisten, deren physiologisch verträglicher Salze oder den Hydraten der Hydrate der Salze zur Bekämpfung menopausaler Hitzewallungen sowie entsprechende Arzneimittel, enthaltend als Wirkstoff einen oder mehrere der ausgewählten CGRP-Antagonisten, und deren Herstellung.

Verwendung ausgewählter CGRP-Antagonisten zur Bekämpfung menopausaler Hitzewallungen
Hintergrund der Erfindung Hitzewallungen sind ein verbreitetes Symptom des pβri/post-menopausalen Syndroms, dessen Physiologie bis heute nicht vollständig verstanden ist. Außer der Hormonsubstitution (hormone replacement therapy), die eine komplexe Intervention darstellt und aufgrund der damit verbundenen Nebenwirkungen häufig nicht dauerhaft angewandt werden kann, existiert bislang keine einfach durchzuführende, nebenwirkungsarme Therapie für diese im allgemeinen als lästig empfundene Erscheinung.
Hitzewallungen werden durch Gefäßerweiterung und erhöhten Blutfluss verursacht.
Bereits mehrfach wurde die Vermutung publiziert, dass CGRP (calcitonin gene- related peptide) aufgrund der vasodilatorischen Eigenschaften dieses Neuropeptids eine Rolle bei der Entstehung menopausaler Hitzewallungen östrogendefizi enter
Frauen spielt ([1]: J. Endocrinol. (1995), 146(3), 431-437; [2]: Acta Physiol. Scand.
(1998), 162(4), 517-522; [3]: Am. J. Obstet. Gynecol. (1996), 175(3, Pt. 1), 638-642).
Ein therapeutischer Einsatz von CGRP-Antagonisten zur Behandlung des menopausalen Syndroms wurde in der Literatur noch nicht vorgeschlagen.
Es wurde nun gefunden, dass die Symptomatik menopausaler Hitzewallungen durch Substanzen, die die Wirkungen von CGRP antagonisieren (CGRP-Antagonisten), effektiv verhindert oder in ihrer beeinträchtigenden Wirkung erheblich abgeschwächt werden kann, wobei sich dieser Therapieansatz insbesondere durch die Nebenwirkungsarmut vor der Hormonsubstitution auszeichnet.
Gegenstand der vorliegenden Erfindung ist somit die Verwendung ausgewählter CGRP-Antagonisten, deren physiologisch verträglicher Salze oder der Hydrate der Salze zur Bekämpfung menopausaler Hitzewallungen, wobei sowohl die Prävention als auch die Akut-Behandlung eingeschlossen sind. Die erfindungsgemäße Verwendung betrifft bevorzugt die Monotherapie mit einer Einzelsubstanz, schließt jedoch auch die Kombinationstherapie mit mehreren Substanzen der genannten Wirkstoff-
gruppe ein. Ferner kann die erfindungsgemäße Verwendung in Ergänzung zu einer üblicherweise durchgeführten Hormonsubstitution erfolgen.
Weitere Gegenstände der Erfindung sind die Verwendung ausgewählter CGRP- Antagonisten, deren physiologisch verträglicher Salze oder der Hydrate der Salze zur Herstellung eines Arzneimittels zur Bekämpfung menopausaler Hitzewallungen sowie die entsprechenden Arzneimittel, enthaltend als Wirkstoff einen oder mehrere der ausgewählten CGRP-Antagonisten, deren physiologisch verträglicher Salze oder der Hydrate der Salze.
Als CGRP-Antagonisten kommen zur Bekämpfung menopausaler Hitzewallungen, zur Herstellung eines entsprechenden Arzneimittels sowie als Bestandteil eines entsprechenden Arzneimittels vorzugsweise folgende Verbindungen in Frage:

Die zur Erzielung einer entsprechenden Wirkung erforderliche Dosierung beträgt bei intravenöser oder subkutaner Gabe zweckmäßig 0.0001 bis 3 mg/kg Körpergewicht, vorzugsweise 0.01 bis 1 mg/kg Körpergewicht, bei oraler Gabe 0.01 bis 20 mg/kg Körpergewicht, vorzugsweise 0.1 bis 20 mg/kg Körpergewicht, und bei nasaler oder inhalativer Gabe 0.01 bis 10 mg/kg Körpergewicht, vorzugsweise 0.1 bis 10 mg/kg Körpergewicht, jeweils ein- bis dreimal täglich.
Sofern die Behandlung mit den ausgewählten CGRP-Antagonisten in Ergänzung zu einer üblichen Hormonsubstitution erfolgt, empfiehlt sich eine Verringerung der vorstehend angegebenen Dosierungen, wobei die Dosierung dann 1/5 der vorstehend angegebenen Untergrenzen bis zu 1/1 der vorstehend angegebenen Obergrenzen betragen kann.
Hierzu lassen sich die ausgewählten CGRP-Antagonisten, deren physiologisch verträgliche Salze oder die Hydrate der Salze zusammen mit einem oder mehreren inerten üblichen Trägerstoffen und/oder Verdünnungsmitteln, z.B. mit Maisstärke, Milchzucker, Rohrzucker, mikrokristalliner Zellulose, Magnesiumstearat, Polyvinyl- pyrrolidon, Zitronensäure, Weinsäure, Wasser, Wasser/Ethanol, Wasser/Glycerin, Wasser/Sorbit, Wasser/Polyethylenglykol, Propylenglykol, Cetylstearylalkohol, Carboxymethylcellulose oder fetthaltigen Substanzen wie Hartfett oder deren geeigneten Gemischen, in übliche galenische Zubereitungen wie Tabletten, Dragees,
Kapseln, Pulver, Suspensionen, Lösungen, Dosieraerosole oder Zäpfchen einarbeiten.
Mögliche pharmazeutische Darreichungsformen sind im folgenden dargestellt:
Kapseln zur Pulverinhalation mit 1 mg Wirkstoff,
Inhalationslösung für Vemebler mit 1 mg Wirkstoff,
Treibgas-Dosieraerosol mit 1 mg Wirkstoff,
Nasalspray mit 1 mg Wirkstoff,
Tabletten mit 20 mg Wirkstoff,
Kapseln mit 20 mg Wirkstoff,
wässrige Lösung für die nasale Applikation mit 10 mg Wirkstoff,
wässrige Lösung für die nasale Applikation mit 5 mg Wirkstoff, oder
Suspension für die nasale Applikation mit 20 mg Wirkstoff.
CGRP wird von sensorischen Nerven freigesetzt, beispielsweise vom Nervus Trigeminus, der einen Teil der Gesichtshaut innerviert. Es wurde bereits gezeigt, dass die Reizung des Trigeminalganglions bei Menschen zu einer Erhöhung des CGRP Plasmaspiegels führt und Gesichtsröte hervorruft ([4]: P.J. Goadsby et al., Annais of Neurology, Vol. 23, No. 2, 1988, 193-196).
Die nachfolgenden Beispiele beschreiben pharmazeutische Anwβndungsformen, die als Wirkstoff einen der ausgewählten CGRP-Antagonisten für die erfindungsgemäße Verwendung enthalten,.
Die ausgewählten CGRP-Antagonisten (A) können z.B. unter Verwendung einer der folgenden pharmazeutischen Formulierungen verabreicht werden:
Kapseln für die Pulverinhalation, enthaltend 0.1 bis 50 mg, vorzugsweise 0.3 bis 30 mg, (A);
Nasenspray, enthaltend 2 bis 50 mg, vorzugsweise 5 bis 40 mg, (A);
Tabletten, enthaltend 10 bis 600 mg, vorzugsweise 30 bis 400 mg, (A);
Pellets für Kapseln, enthaltend variierende Gewichtsteile (A);
Extrudate für Kapseln oder Tabletten, enthaltend variierende Gewichtsteile (A);
Suppositorien, enthaltend 50 bis 600 mg, vorzugsweise 30 bis 400 mg, (A);
Injektionslösungen, enthaltend 0.2 bis 30 mg, vorzugsweise 0.5 bis 15 mg, (A);
Die folgenden Beispiele beschreiben pharmazeutische Zubereitungen, enthaltend als aktive Substanz einen der erfindungsgemäß ausgewählten CGRP-Antagonisten, ein physiologisch verträgliches Salz davon oder ein Hydrat des Salzes. Vorangestellt ist zunächst eine Tabelle, in der den Arzneistoffkomponenten Nummern zugewiesen werde, die in folgenden Beispieltabellen zur Identifikation der Wirkstoffe dienen.

Im Anschluss wird das Granulat in einer geeigneten Mühle, z.B. einer Comill bei 3000 U/Min mit Maschenweite 1.1 mm gesiebt. Daraufhin wird das Granulat 5 Minuten mit Crospovidon gemischt und
anschließend noch 1 Minute mit Magnesiumstearat. Die so erhaltene Mischung wird in einer Tablettenpresse zu Tabletten mit einem geeigneten Durchmesser gepresst.

Pellets
Die erfindungsgemäßen Arzneistoffe können auch in Form kleiner Partikeln wie z.B. Pellets hergestellt werden. Hierbei kann der Wirkstoff auf Neutralpellets, die aus Saccharose und Stärke oder mikrokristalliner Cellulose bestehen, aufgetragen werden.
Falls aufgrund pH-abhängiger Löslichkeit eines Wirkstoffs saure oder basische Hilfsstoffe die Auflösung des Wirkstoffs verbessern, können auch saure oder basische Starterkerne anstelle von Neutralpellets verwendet werden. Die Herstellung erfolgt in folgenden Schritten:
1. Auswahl bzw. Herstellung von Starterpellets
2. Aufbau der Wirkstoff Schicht
Fakultativ: Überziehen von Pellets zur Stabilitätsverbesserung oder Geschmackskorrektur oder - falls gewünscht - zur Retardierung einer oder mehrerer Wirkstoffe.
Beispiel 4a
Herstellung basischer Starterkerne:
Zusammensetzung: Povidone K25 3 Gewichtsteile Mikrokrist. Cellulose 20 Gewichtsteile Meglumin 77 Gewichtsteile
77 Gewichtsteile Meglumin, 20 Gewichtsteile Mikrokrist. Cellulose und 3 Gewichtsteile Povidone K25 werden in einem Rhönradmischer 15 Minuten lang gemischt. Dann wird die Pulvermischung mit einer Geschwindigkeit von ca. 1 kg/h zusammen mit Wasser, das mit einer Dosierpumpe zugegeben wird, in einen Doppelschnecken-
Extruder eingebracht. Die Wasserdosierung wird automatisch so geregelt, dass im Extruder ein Solldrehmoment von ca. 19% erzeugt wird. Die Extrusion erfolgt über eine Düsenplatte mit Bohrungen von 0.8 mm Durchmesser. Die Ausrundung der Extrusionsstränge zu Pellets erfolgt in einem Spheronizer, wobei ca. 3 Minuten mit ca. 850 RPM gerundet wird.
Trocknung der Pellets bei 800C ca. 1.5 Stunden in einem Wirbelschichttrockner.
Das Kernmaterial wird über eine Taumelsiebmaschine mit unterschiedlichen Siebböden mit nominalen Maschenweiten von 0.71 bis 1.25 mm fraktioniert. Die jeweils geeigneten Gutfraktionen zwischen 0.71 und 0.90 bzw. 0.90 und 1.12 mm werden in den weiteren Prozessen verwendet.
Beispiel 4b
Herstellung einer Wirkstoffauftraαunα mit 100 mg CGRP-Antaαonist
Zusammensetzung:
Kernmaterial 200 Gewichtsteile
Hydroxypropylcellulose 38 Gewichtsteile Talkum 20 Gewichtsteile
CGRP-Antagonist 100 Gewichtsteile
In 250 Gewichtsteilen 2-Propanol wird Hydroxypropylcellulose unter Rühren gelöst und anschließend werden in dieser Lösung der Wirkstoff und Talkum unter Rühren dispergiert.
In einer Wirbelschichtprozessanlage werden 200 Gewichtsteile Kernmaterial bei einer Zulufttemperatur von 20° bis 30° C mit der wirkstoffhaltigen Dispersion im Unterbettsprühverfahren besprüht. Die wirkstoffhaltigen Pellets werden anschließend im Umlufttrockenschrank bei 35°C über 8 Stunden nachgetrocknet.
Zur Entfernung von Agglomeraten werden die wirkstoffhaltigen Pellets mit einem Sieb mit der nominalen Maschenweite 1.25 mm gesiebt. Die Gutfraktion (Korngröße < 1.25 mm) wird weiterverarbeitet.
Der Aufbau der Wirkstoffschicht erfolgt generell in immer gleicher Weise, wobei aber Wirkstoffart und Menge, Bindemittelart und Menge, Talkummenge und Wasser, Isopropanol bzw. Ethanolmenge variiert werden.

In 250 Gewichtsteilen 2-Propanol wird Hydroxypropylcellulose unter Rühren gelöst und anschließend werden in dieser Lösung der Wirkstoff und Talkum unter Rühren dispergiert.
In einer Wirbelschichtprozessanlage werden 200 Gewichtsteile Kernmaterial bei einer Zulufttemperatur von 20° bis 30° C mit der wirkstoffhaltigen Dispersion im Unterbettsprühverfahren besprüht. Die wirkstoffhaltigen Pellets werden anschließend im Umtufttrockenschrank bei 35° C über 8 Stunden nachgetrocknet.
Zur Entfernung von Agglomeraten werden die wirkstoffhaltigen Pellets mit einem Sieb mit der nominalen Maschenweite 1.25 mm gesiebt. Die Gutfraktion (Korngröße < 1.25 mm) wird weiten/erarbeitet.

Herstellung einer Wirkstoffauftraqunα mit 400 Gewichtsteilen CGRP-Antaαonist

Sieb mit der nominalen Maschenweite 1.25 mm gesiebt. Die Gutfraktion (Korngröße
< 1.25 mm) wird weiterverarbeitet. Der Aufbau der Wirkstoff schicht erfolgt generell in immer gleicher Weise, wobei aber
Wirkstoffart und Menge, Bindemittelart und Menge, Talkummenge und Wasser,
Isopropanol bzw. Ethanolmenge variiert werden.
Die jeweiligen Mengen können variieren und werden im folgenden in Tabellenform dargestellt. Die Beispiele enthalten 10 bis 380 Gewichtsteile CGRP-Antagonist entweder als aktive Form, in Form eine physiologisch verträglichen Salzes oder in Form des
Hydrats eines Salzes, die übrige Zusammensetzung ist in der folgenden Tabelle enthalten.

Das isolierte Kernmaterial wird anschließend im Umlufttrockenschrank bei 400C über
8 Stunden nachgetrocknet.
Zur Entfernung von Agglomeraten werden die getrockneten retardierten Pellets mit einem Sieb mit der nominalen Maschenweite 1.5 mm gesiebt. Die Gutfraktion
(Korngröße < 1.5 mm) wird weiterverarbeitet.

Die Wasserdosierung wird automatisch so geregelt, dass im Extruder ein Solldrehmoment von ca. 19% erzeugt wird. Die Extrusion erfolgt über eine Düsenplatte mit Bohrungen von 0.8 mm Durchmesser.
Das Ausrunden der Extrusionsstränge zu Pellets erfolgt in einem Spheronizer, wobei ca. 3 Minuten mit ca. 850 RPM gerundet wird.
Die Pellets werden bei 8O0C ca. 1.5 Stunden in einem Wirbelschichttrockner getrocknet.
Das Kernmaterial wird über eine Taumelsiebmaschine mit unterschiedlichen Sieb- böden mit nominalen Maschenweiten von 0.71 bis 1.25 mm fraktioniert. Die jeweils geeigneten Gutfraktionen zwischen 0.71 und 0.90 bzw. 0.90 und 1.12 mm werden in den weiteren Prozessen verwendet.

Herstellung sphärisch nanostrukturierter Mikropartikel der Wirkstoffe zur Herstellung eines Pulverinhalativums
Die Wirkstoffe werden zur Herstellung einer Wirkstofflösung von 4 Gew.-% entsprechend in einem Ethanol/Wasser (4:1)-Gemisch gelöst und die Wirkstofflösung
so versprüht, dass ein Sprühnebel mit einer Tropfengröße mit dem Kennwert X50 (Medianwert = Teilchengröße/Tropfengröße, unterhalb derer 50% der Teilchenmenge liegt bezüglich der Volumenverteilung der einzelnen Teilchen/Tropfen) im Bereich von 1.5 bis 8 μm und Q(5.8) (Entspricht der Teilchenmenge, die bezogen auf die Volumenverteilung der Tröpfchen unterhalb von 5.8 μm liegt) zwischen 30% und 100% erzielt wird. Der so erhaltene Sprühnebel wird mit Hilfe eines Trocknungsgases mit einer Eingangstemperatur zwischen 13O0C und 2000C und einer Ausgangstemperatur von 4O0C bis 1200C getrocknet Dabei beträgt der Volumenstrom des Sprühgases 1 Nm3Zh bis 15 Nm3Zh und der Volumenstrom des Trocknungsgases 15 Nm3Zh bis 150 Nm3Zh. Der getrocknete Feststoffanteil wird mittels Massekraftabscheider undZoder Filtereinheit gesammelt.

(20) 4-(2-Oxo-1 ,2,4,5-tetrahydro-i ,3-benzdiazepin-3-yl)-piperidin-1 -carbonsäure- (f?)-1-(4-hydroxy-3,5-dimethyl-benzyl)-2-oxo-2-(4-piperazin-1-yl-piperidin-1-yl)- ethylester,

deren physiologisch verträglichen Salzen und den Hydraten der Salze zur Bekämpfung menopausaler Hitzewallungen.
2. Verwendung gemäß Anspruch 1 , dadurch gekennzeichnet, dass diese in Monotherapie mit einem Einzelwirkstoff erfolgt.
3. Verwendung gemäß Anspruch 1 , dadurch gekennzeichnet, dass diese in Ergänzung zu einer Hormonsubstitutionstherapie erfolgt.
4. Verwendung eines Wirkstoffs, der ausgewählt ist aus der Gruppe bestehend 5 aus
(1) 4-(2-Oxo-1 ,2,4,5-tetrahydro-1 ,3-benzdiazepin-3-yl)-piperidin-1-carbonsäure-{(fl)- 1 -(4-amino-3-chlor-5-ethyl-benzyl)-2-[4-(4-methyl-piperazin-1 -yl)-piperidin-1 -yl]-2- oxo-ethylj-amid,

(2) [1 '-((H)-3-(4-Amino-3-chlor-5-trifluormethyl-phenyl)-2-{[4-(2-oxo-1 ,2,4,5-tetra- hydro-I .S-benzdiazepin-S-yO-piperidin-i -carbonylJ-aminoJ-propionyl^^'-bi- piperidinyl-1 -yl]-essigsäure,

5. Verwendung gemäß Anspruch 4, dadurch gekennzeichnet, dass das Arzneimittel nur einen Wirkstoff enthält.
6. Arzneimittel zur Bekämpfung menopausaler Hitzewallungen, enthaltend als Wirkstoff einen oder mehrere CGRP-Antagonisten ausgewählt aus der Gruppe gemäß Anspruch 1 neben gegebenenfalls einem oder mehreren inerten Trägerstoffen und/oder Verdünnungsmitteln.
