Definitions
- the invention relates to a device for stretching flexion-resistant fingers or toes, in particular for the treatment of Dupuytren's disease of the hand and foot, with a holding device which can be placed on the metacarpus area and optionally the forearm area or the metatarsus area and can be fixed on this area.
- Dupuytren's disease is a type of tendon shortening. Due to the growth of the connective tissue of the palm and fingers, coarse knots and strands appear. In the later stage, there is a slow "pulling" of individual fingers into the palm of the hand. Of course, this means a handicap when gripping. The flexor tendons themselves do not undergo any changes. Therefore, the term "tendon shortening" is actually wrong.
- Dupuytren's disease often occurs in families and mostly in men and is a common disease in local areas. It does not only occur on the hand but also on the feet. Knots then form on the sole of the foot, which cause discomfort when walking.
- AT 404 427 B discloses a device for extending flexion-contracting fingers and thus Dupuytren's contracture.
- the device described there has a one-piece shell to be fastened to the metacarpus by means of a Velcro fastener, buckle fastener or the like, to which is attached an inflatable tube, balloon or the like lying in the region of the inner surface of the flexing finger or fingers.
- the hose etc. is filled with air. The increase in pressure of the unfolding hose etc. leads to an expansion of only the finger or fingers and can be carried out and checked independently by the patient.
- a prosthesis for the conservative treatment of Dupuytren's contracture is known from German utility model G 88 04 564.1, in which the affected fingers can be brought into a variable stretching point with the aid of taut linen straps attached to stiff leather sleeves.
- This known prosthesis has the disadvantage, among other things, that it is extremely difficult to stretch curved fingers and to bind them into a tight leather sleeve.
- the object of the present invention is to provide an improved device of the type described, with which the Dupuytren's disease in the hand and foot can be treated more effectively.
- the device according to the invention is distinguished, inter alia, by the fact that it has an essentially plate-shaped support unit which is pivotally connected to the holding unit. It can be a support unit for one, two, three or four fingers. It is also possible to use several with the holding unit To connect support units pivotably, in which case one support unit is used for one finger. Mixed forms are also possible.
- the support unit can be fixed on the fingers or toes.
- the pivot axis between the holding unit and the support unit runs approximately transversely to the longitudinal direction of the hand in the area of the basic finger joints (joints between the metacarpals and the finger bones). If the device according to the invention is used to treat the foot, then this pivot axis runs in the area of the basic toe joints (joints between the metatarsal and the toe bones).
- the pivotable connection between the holding unit and the support unit can be designed in any way.
- it can be a film hinge. It is also possible to use plate hinges or the like.
- this connection must have sufficient mechanical strength so that the forces which occur, which are mentioned in more detail below, can be absorbed and primarily only lead to a pivoting of the support unit with respect to the holding unit.
- a swiveling device is connected to both the holding unit and the support unit, which can exert a force on the support unit in the direction of the direction of extension of the fingers or toes. Depending on how it is designed and arranged, this swivel device can exert a pressure or a pull. It is important that the pivoting device can pivot the support unit about the pivot axis in the direction described.
- the support unit is oriented at an angle to the main plane of the holding unit.
- the support unit is then pivoted in the course of the treatment in such a way that this angle is widened and increased, for example up to approximately 180 °, so that the holding unit and the support unit are approximately aligned. It is also possible to make the angle larger than 180 °.
- the support unit is always pivoted in the course of the treatment as far as the hand or fingers to be treated allow.
- the fingers exert a counter pressure which is absorbed by the device according to the invention.
- the support unit is equipped with means that press the fingers or toes down on the support surface. This can be accomplished, for example, with one or more strip-like bands which begin on one side of the support unit, are guided on the side of the fingers opposite the support unit, and are returned to the support unit on the other side, so that all fingers are pressed down practically together. It is also possible to provide a single band of this type for each finger that can also be combined with a band acting on all fingers together. These tapes are expediently designed as, in particular, elastic Velcro fasteners.
- the fingers are stretched as far as possible so that they are as parallel as possible to the plate-shaped support unit run (if possible straight position of the fingers).
- the force exerted by pivoting the support unit acts on the hand and fingers.
- the device according to the invention is described below with reference to a holding unit and support unit designed for the hand or arm. In this case, too, the explanations apply to a device for treating the feet.
- the holding unit is preferably designed such that it comes into contact with the palm of the hand or forearm, in particular approximately in a form-fitting manner. It is expediently made as a half-shell from a plastic material. This half-shell corresponds approximately to the shape of the palm or forearm. In the region of the thumb, an approximately tubular section having a longitudinal section is provided, into which the thumb can be pushed and then remains more or less stationary there.
- the holding unit can consist of a cuff which has a longitudinal slit, bands or the like being provided in the region of this longitudinal slit, with which the cuff can be tightened together and around the forearm or hand.
- Cuffs of this type are known.
- the swivel device exerts a compressive force on the support unit.
- the support unit is pushed away from the holding unit by the pivoting device.
- the swivel device is expediently arranged on the palm side. It is also possible to provide a swiveling device on the back of the hand, which exerts a pull on the support unit, so that it is “swiveled” outward from the holding unit.
- the pivoting device preferably exerts a resilient compressive or tensile force on the support unit.
- a resilient compressive or tensile force for example, it is possible to use springs or compression spring elements in the swivel device. Due to the fact that the support unit is connected to the holding unit in the manner described, the support unit is pivoted about the pivot axis described by the exercise of a pull or a pressure. This also swings the fingers and exerts a continuous pull on the tendons. In this way, new tissue is formed, so that the tendons lengthen. The overgrown trade is stretched; it comes to the pants. After a few months, the fingers can be stretched again as originally, and the function of the hand is restored.
- the device according to the invention is expediently applied in particular at night.
- FIG. 1 shows a perspective view obliquely from above of a device according to the invention in the open state for treating a hand
- FIG. 2 shows the device shown in FIG. 1 with the hand inserted and the forearm inserted in the closed state in a top view of the back of the hand
- Figure 4 is a cross-sectional view through a support unit of the device shown in Figures 1 to 4 with inserted fingers and
- Figure 5 is a perspective view obliquely from below of an inventive device for treating a foot with an inserted foot.
- the device 1 shown in a perspective top view in FIG. 1 has a holding unit 2 and a support unit 3, the support unit 3 serving to receive all four fingers of one hand.
- the support unit 3 is essentially plate-shaped and bent up at its two side edges 4, 4 '(see FIG. 4).
- Four flat, parallel grooves 5 are formed in the plate-shaped support unit, which correspond approximately to the contour of the fingers.
- the fingers are indicated in FIG. 4 by a dashed line.
- the support unit 3 is connected to the holding unit by means of a film hinge 6.
- This connection can be made in any way, for example by screwing or gluing.
- the support unit 3 can be pivoted about the pivot axis 7 with respect to the holding unit 2.
- Two slots 26 are thus provided for each finger or band 22.
- the two ends 23, 24 are connected to one another above the finger and fixed to one another under tension. In this way, each finger is virtually wrapped and can be fixed according to its degree of curvature and held down on the support unit 3.
- that section of the band 22 that extends between the two slots 26 runs on the outer surface of the support unit 3 opposite the finger (indicated by dashed lines in FIG. 1).
- the holding unit 2 represents a kind of half-shell, which is designed by the contour in such a way that it rests or clings to the hand to be treated in the area of the palm of the hand, to the metacarpus area and to the outer lower forearm area.
- For the thumb there is an approximately tubular area 8 with an elongated slot 26 into which the thumb is inserted and which almost completely surrounds the area of the thumb root, so that the thumb is fixed.
- the state shown in FIG. 2 results.
- the band 9 is attached to the support unit 3 at one end, runs around the back of the fingers at the top and is then attached to the support unit 3 at the other end, for example with the aid of a Velcro fastener.
- This band 3 is expediently arranged in the region of the finger joints, so that it can be regarded as a common means for depressing all fingers and, like the bands 22, presses the joints of the fingers onto the support unit 3 and stretches the fingers with it.
- the bands 10 and 1 1 serve to fix the metacarpal / wrist area or the forearm area. They can be configured in the same way as the band 9 of the support unit 3. However, they can also only run around the arm and the holding unit 2.
- Support unit 3 attached to the film hinge 6 during the Retaining bracket 14 is attached to the film hinge 6 in the region of the holding unit 2.
- a bushing 17, 18 is rotatably mounted about an axis extending between the two ends 15, 16, which have an internal thread and into which a threaded rod 19 engages, which on their free ends has a knob 20.
- the internal threads of the bushes 17, 18 are in opposite directions, so that when the threaded rod 19 is rotated, the bushes 17, 18 are either moved away from one another or are moved toward one another, depending on the thread. Of course, this also depends on the direction of rotation of the threaded rod 19.
- the holding bracket 13, 14, the threaded bushings 17, 18, the threaded rod 19 and the rotary knob 20 are preferably made of a metal.
- the film hinge 6 consists of a bendable material and is dimensioned such that it can be pivoted about the pivot axis 7, but at the same time the connection between the support unit 3 and the holding unit 2 is given sufficient rigidity and strength.
- the device shown in FIG. 5 corresponds essentially to the device described above, but of course it does the anatomical conditions of the foot are adjusted. Corresponding elements of this device are therefore provided with the same reference numerals.
- the swivel device which can be attached both outside and inside, is not shown for reasons of clarity.
