  • imaging from the radiation source 205 X-rays can provide targeting information to accurately direct the therapeutic X-rays to the target volume-from any angle while effectively excluding healthy tissue from injury.
  • the imaging radiation source 204 and detector 206 can be operated either in a continuous or pulsed manner to provide a real time or quasi-real time fluoroscopic image of moving internal anatomy.
  • This fluoroscopic image can also be used to provide information to track the motion of anatomy being treated. As is generally known, normal respiration or unwanted voluntary or involuntary patient movement may cause such motion.
  • This motion tracking information can in turn be used to adjust treatment parameters, track the target volume with the treatment beam, and/or gate the treatment beam off and on such that the anatomy intended to be treated is always in the intended position within the treatment beam.
  • each arm may have one or more of either or both of sources and/or image detectors.
  • the imaging sources and imaging detectors may be placed at any position on the arms.
  • both arms may have one of each, with the two imaging detectors placed in the position nearest the treatment source on both arms, and the two imaging sources placed in the lower position on each arm.
  • the number of image detectors and imaging sources to be used may be based on factors such as simplicity, cost, and versatility.
  • an arm attached to the gantry 202 may have two image detectors. These two detectors may capture radiation from one or more imaging sources.
  • the imaging source(s) and detectors may be off-set radially about the patient, to provide data to the computer to generate a stereoscopic representation of a target volume, as described above.
  • the imaging detector may be at a different distant from the gantry than the imaging source(s). Therefore, both the imaging detector and imaging source(s) are rotated at an angle, for the detector to capture the radiation of the imaging source.
  • one arm may have a single imaging detector that captures radiation from one or more imaging sources.
  • the imaging detector and/or imaging source might be repositioned to receive radiation from varying positions and angles. Thereby, providing target volume data to generate a stereoscopic representation of the target volume, as described.
  • a single imaging detector on one arm may also capture radiation from two spaced apart (possibly pulsing alternately) radiation sources on the other arm. Where the imaging detector is positioned and angled accordingly, to again, provide target volume data to generate a stereoscopic representation of the target volume.
  • an image from a single source/detector pair together with an image taken during treatment from e.g. imager 214 may be used to form a stereoscopic representation.
  • the image detector might be a multiple-energy imaging unit capable of receiving and displaying imaging information from multiple energy sources.
  • one of the energies can be a source of megavoltage x-ray energy and another a source of kilovoltage x-ray energy, both of which can alternately activate the multiple-energy imaging unit for high quality verification imaging and high quality target volume imaging, respectively.
  • the device 200 may also correct for mechanical flexure of the radiation source and imaging unit to improve image quality. Due to the weight of the radiation source and imager, the imaging beam may be misaligned causing error.
  • the imager-source stability is defined by projecting the x-ray beam past a set of markers of known fixed position, while rotating the gantry 360 degrees, and determining the difference from the ideal position by measuring the resulting, lateral and longitudinal shift in the image.
  • unwanted play or movement in any particular joint within the arm may be directly measured by a suitable sensor (for example encoder or potentiometer) incorporated in the mechanism associated to that joint. Either or both types of the foregoing information is used by a feedback control system to properly align the sagging radiation source and image detector in real-time during imaging.
  • a radiotherapy clinical treatment machine with a rotatable open gantry having articulating robotic arms
  • robotic arms provide variable positioning and clearance for e.g. effective administration of oncology radiation therapy to a patient.
  • the articulating robotic arms operating independently and/or in conjunction with the movement of the rotatable gantry.
  • the machine includes logic to detect and correct any mechanical flexure of the device in real-time before administrating the treatment beam. Thereby, providing better images for more accurate treatment and to minimize targeting of healthy tissue.
  • machine-readable medium shall be taken to include any medium that is capable of storing or encoding a sequence of instructions for execution by the machine and that cause the machine to perform any one of the methodologies of the present invention.
  • the term “machine-readable medium” shall accordingly be taken to included, but not be limited to, solid-state memories, optical and magnetic disks, and carrier wave signals.
  • one arm may have more pivot points than the other arm.

Claims (27)

  1. Appareil de radiothérapie comprenant : un montant d'entraînement (203) ; un portique rotatif (202) couplé en pivotement au montant d'entraînement (203) et comprenant une source de traitement (212) ; un premier bras (205) couplé au portique rotatif (202) ; une source d'imagerie (204) couplée à une extrémité de source (251) du premier bras (205), un second bras (207) couplé au portique rotatif (202) ; et un détecteur d'image (206) couplé à l'extrémité d'imagerie (252) du second bras (207), caractérisé en ce que le premier bras (205) est couplé en pivotement au niveau de son point de couplage.
  2. Appareil selon la revendication 1, dans lequel le premier bras (205) et le second bras (207) sont agencés pour manoeuvrer indépendamment du portique rotatif (202).
  3. Appareil selon la revendication 1 ou 2, dans lequel le premier bras (205) et le second bras (207) sont agencés pour s'étendre jusqu'à une position d'imagerie depuis une position rétractée.
  4. Appareil selon la revendication 3, dans lequel le premier bras (205) et le second bras (207) sont agencés pour basculer latéralement dans la position d'imagerie depuis la position rétractée.
  5. Appareil selon la revendication 3, dans lequel le premier bras (205) et le second bras (207) sont agencés pour basculer vers le bas jusqu'à la position d'imagerie depuis la position rétractée.
  6. Appareil selon l'une quelconque des revendications précédentes, dans lequel la source d'imagerie (204) se situe à une distance différente du portique (202) que le détecteur d'image (206), la source d'imagerie (204) est positionnée de façon pivotante pour cibler le détecteur d'image (206), et le détecteur d'image (206) est positionné de façon pivotante pour capturer un faisceau d'imagerie depuis la source d'imagerie (204).
  7. Appareil selon l'une quelconque des revendications précédentes, dans lequel le détecteur d'image (206) fournit des informations de volume cible.
  8. Appareil selon la revendication 7, comprenant un ordinateur adapté pour positionner un faisceau de traitement basé sur les informations de volume cible.
  9. Appareil selon l'une quelconque des revendications précédentes, comprenant un ordinateur ayant un composant de commande à rétroaction de flexion mécanique afin de corriger la flexion mécanique de la source d'imagerie (204) et la flexion mécanique de l'imageur (206).
  10. Appareil selon la revendication 1, permettant de réaliser un traitement médical, dans lequel un moyen de positionnement est fourni pour positionner le premier bras (205) et le second bras (207) sur le portique rotatif (202) afin de recevoir des données de volume cible, le second bras (207) étant mobile jusqu'à une position depuis une position de débattement ; un moyen est fourni pour déterminer une position de traitement pour un faisceau de traitement d'après les informations de volume cible ; et un moyen est fourni pour appliquer le faisceau de traitement lors de la manoeuvre jusqu'à la position de traitement.
  11. Appareil selon la revendication 10, dans lequel les données de volume cible sont basées sur un rayonnement reçu par le détecteur d'image (206).
  12. Appareil selon la revendication 10 ou 11, dans lequel le moyen de positionnement comprend un moyen permettant d'étendre les premier et second bras (205, 207) afin de fournir une résolution plus élevée.
  13. Appareil selon la revendication 10, 11 ou 12, dans lequel le moyen de positionnement est agencé pour étendre les premier et second bras (205, 207) pour une tomographie assistée par ordinateur à grand champ de vision.
  14. Appareil selon l'une quelconque des revendications 10 à 13, dans lequel le moyen de positionnement comprend un moyen permettant de corriger la flexion mécanique.
  15. Appareil selon l'une quelconque des revendications 10 à 14, dans lequel les informations de volume cible sont basées sur un mouvement intrafractionnel en éloignement d'un volume cible.
  16. Appareil selon l'une quelconque des revendications 10 à 15, dans lequel le moyen de détermination de la position de traitement est agencé pour utiliser des informations stéréoscopiques générées.
  17. Appareil selon la revendication 1, comprenant un moyen de positionnement permettant de positionner la source d'imagerie (204) et le détecteur d'imagerie (206), dans lequel le moyen de positionnement est couplé de façon pivotante à la source d'imagerie et au moyen d'imagerie au niveau de points de couplage, le positionnement comprenant une transition entre une position de débattement et une position d'imagerie, et dans lequel le moyen de positionnement comprend lesdits premier et second bras.
  18. Appareil selon la revendication 17, dans lequel le moyen de positionnement est agencé pour faire basculer la source d'imagerie (204) vers l'extérieur jusqu'à la position d'imagerie.
  19. Appareil selon la revendication 17 ou 18, lequel le moyen de positionnement est agencé pour contracter le moyen d'imagerie (206) afin de fournir une résolution plus élevée.
  20. Appareil selon l'une quelconque des revendications 17, 18 ou 19, dans lequel le moyen de positionnement est agencé pour étendre le moyen d'imagerie (206) afin de fournir un plus grand champ de vision d'image.
  21. Appareil selon la revendication 20, comprenant un moyen de calcul agencé pour corriger une flexion mécanique de la source d'imagerie (204).
  22. Appareil selon la revendication 1, comprenant une pluralité de sources d'imagerie (204) couplées au premier bras (205), dans lequel le détecteur d'image (206) couplé au second bras (207) est agencé pour recevoir un rayonnement de la pluralité de sources de rayonnement (204).
  23. Appareil selon la revendication 22, dans lequel une première source de la pluralité de sources d'imagerie (204) est une source de rayonnement en kilovolt.
  24. Appareil selon la revendication 22 ou 23, dans lequel le détecteur d'image (206) est un imageur multiénergie.
  25. Appareil selon la revendication 22, 23 ou 24, dans lequel le second bras (207) est agencé pour s'étendre jusqu'à une position d'imagerie depuis une position de débattement.
  26. Appareil selon la revendication 1, dans lequel le point de couplage pour le premier bras (205) est un premier point de pivot, et le premier bras (205) comprend un deuxième point de pivot.
  27. Appareil selon la revendication 26, dans lequel le point de couplage pour le second bras (207) est un troisième point de pivot.
