farmaceutisk acceptable salte heraf, der er lægemidler H
med en knogleresorptionshæmmende virkning såvel som en H
5 anti-inflammatorisk virkning og en anti-rheumatisk virk- I
ning og angår også præparater indeholdende forbindelser- H
ne som en aktiv bestanddel. H
Visse derivater af (cycloalkylamino)methylenbis (phos- H
phonsyre) er kendte. Japansk offentliggjort patent nr. H
10 37,829/79 omhandler forbindelser med en usubstitueret H
cyclopentylgruppe eller en cyclohexylgruppe, og japansk H
patentskrift nr. 12,319/80 omhandler en forbindelse med H
en cyclohexylgruppe som cycloalkylgruppen. Disse ja- H
panske patentskrifter nævner, at forbindelserne kan an- H
15 vendes som kemikalier inden for landbruget, specielt som H
herbicider, og at de kan anvendes ved en metode til at
forhindre udfældning i vand eller i vandig opløsning, I
men omtaler overhovedet ikke anvendelsen af forbindel- H
serne som lægemidler. H
20 Den foreliggende opfindelse angår således (cycloalkyl- H
amino)methylenbix(phosphonsyre) derivater med en usub- I
stitueret eller substitueret cycloalkylgruppe med 3 til I
10 carbonatomer, der kan anvendes som en knogl-resorp- H
tionsinhibitor og et anti-arthritis-middel. I

5 Den foreliggende opfindelse tilvejebringer også et knog-le-resorptionshæmmende og anti-arthritis-holdigt lægemiddel, der som den aktive bestanddel indeholder en forbindelse med den almene formel (I), en lavere alkylester heraf eller et farmaceutisk acceptabelt salt heraf.
10 Herudover tilvejebringer opfindelsen også en hidtil ukendt forbindelse med den almene formel (I), hvori R, 12 3 4 R , R , R og R betegner et hydrogenatom eller en lavere alkylgruppe, n er et helt tal fra 3 til 10, men R betegner en lavere alkylgruppe, når n er 5 eller 6.eller 15 et farmaceutisk acceptabelt salt af forbindelsen, når denne er en syre.
Den lavere alkylgruppe i den almene formel (I) er en lige eller forgrenet carbonhydridgruppe med 1 til 5 carbon-atomer. Typiske lavere alkylgrupper er methylgruppen, 20 ethylgruppen, propylgruppen eller isopropylgruppen.

tionsprodukt hældes reaktionsblandingen for eksempel på I
I DK 174744 B1 I 4 I en silicagelsøjle og eluéres med et blandet opløsnings- I middel af methanol-chloroform. Ved ovennævnte omsætning I kan den tilsvarende bisphosphonsyre eller bisphosphonat I fås henholdsvis ud fra phosphorsyrlingen eller dens I 5 ester (IV).
I Bisphosphonaterne kan omdannes til de tilsvarende bis- I phosphonsyrer ved hydrolyse. Hydrolysen udføres sædvan- I ligvis ved opvarmning af bisphosphonaterne under tilbage- I svaling i koncentreret saltsyre. Alternativt kan bis- I 10 phosphonaterne behandles med en stærk syre eller et tri- I methylsilylhalogenid i et vandfrit opløsningsmiddel.
I Til fremgangsmåden er almindeligvis en kommerciel vandfri I hydrogenbromidsyre i eddikesyre direkte anendelig eller I i form af en forsigtigt fortyndet opløsning eller en op- I 15 løsning af trimethylsilyliodid opløst i et opløsnings- I middel, som for eksempel carbontetrachlorid, dimethyl- I formamid, chloroform eller toluen. I Forbindelserne med formel (I) og deres salte har en I knogleresorptionshæmmende virkning og også en evne til I at hæmme hypercalcemi forårsaget af knogleresorption.
I Herudover ses de at have glimrende anti-inflammatorisk I 30 virkning og antipyretisk og analgesisk virkning. 1
Eksperimentelle forsøgsmetoder og resultater er omtalt I nedenfor for at påvise den hæmmende virkning på hyper- I calcemi udøvet af forbindelserne med formel (I) ifølge DK 174744 B1 5 opfindelsen eller deres salte.
(1) Hæmmende virkning på hypercalcemi hos rotter
Rotter med hypercalcemi induceret ved administrering af parathyroidea-hormon blev anvendt, og formindskelsen af 5 serumcalciummængden ved administrering af forbindelsen blev målt.
Forsøgsmetode 30 yg/kg humant 1-34 parathyroidea-hormin (PTH, fremstillet af Peptide Laboratory), der var opløst i en 0,1% 10 BSA {bovint serum-albumin)-holdig fysiologisk saltopløsning (indholdet af PTH er 6 yg/ml) blev intravenøst injiceret i en mængde på 30 yg/kg (5 ml/kg som opløsning) til 5 uger gamle Wistar-rotter, der havde fastet i 20 timer. Kun 0,1% BSA-holdigt fysiologisk saltvand blev 15 injiceret i de normale kontrolgrupperpå samme måde. 45 minutter efter PTH-injektion blev rotterne etherbedøvet og underkastet celiotomi, hvorved blodet blev opsamlet fra bughulen med en vakuum-blodopsamlingsanordning. Det opsamlede blod blev straks herefter centrifugeret ved 20 3.000 omdrejninger pr. minut ved 4°C i 10 minutter for ++ at isolere serum. Den ioniserede calcium (Ca )-koncentration i serum blev straks målt med et Ca++-meter (Sera 250, fabrikeret af Horiba Manufacturing Co.).
De omhandlede forbindelser blev opløst under anvendelse 25 af natriumhydroxid og saltsyre i fysiologisk saltvand (pH 7,4) til subcutan administrering i sådanne mængder, at dosen androg 2 ml/kg. De blev administreret 72 timer inden PTH-injektionen. Fysiologisk saltvand eller destilleret vand blev administreret til den normale kon-30 trolgruppe og kontrolgruppen på samme måde. Som en referenceforbindelse blev laksecalciton (SCT, fremstillet af Armour Co.) anvendt til målingerne. SCT blev administreret subcutant i en dosis på 2 mg/kg 30 minutter inden PTH-injektionen.

Adjuvans-induceret arthritis er en af de populære model- I
ler for afprøvning af rheumatoid arthritis hos mennesker. H
5 Arthritiske rotter induceret ved injektion af adjuvanset I
viste ikke alene opsvulmning af foden# men også nedgang H
af BMC, hvilket antages at være forårsaget af stigning H
af knogleresorption og/eller inaktivering af baglemmer- H
ne.
10 Indomethacin 1 mg/kg hæmmede markant opsvulmningen af H
baglemmerne. Den hæmmede også nedgangen af BMC som et H
resultat af hæmning af udvikling af arthritis. YM-21175- H
1 hæmmede opsvulmningen kun ved høje doser, men nedgang H
af BMC i 1 mg/kg. Der er derfor sandsynligvis forskel H
15 mellem den hæmmende virkning af indimethacin og YM- H
21175-1. YM-21175-1 var mere effektiv end APD. I

I Den hæmmende virkning af YM-21175-1 på inogletab blev I 10 sammenlignet med APD. Som følge af de tre slags forsøg I antages YM-21175-1 at være af værdi ved behandling af I osteoporosis, rheumatoid arthritis og andre sygdomme, I hvor knogleresorptionen er accelereret, og herudover kan I ses, at YM-21175-1 er kraftigere end APD.
I 15 Ud fra forsøgsresultaterne påvistes, at de omhandlede I forbindelser udviste en glimrende evne til at reducere I mængden af serumcalcium. Det er derfor blevet bekræf- I tet, at de omhandlede forbindelser udviser en knogle- I resorptionshæmmende virkning. Som sygdomme, der kan an- I 20 tages at være forårsaget af for stor knogleresorption, I kan nævnes Paget's syge, hypercalcemi, metastatisk oste- I ocarcinoma og osteopsathrosis. Yderligere er sthenisk I knogleresorption efter inflammatorisk arthritis, som fx I kronisk rheumatoid arthritis, et vigtigt problem set ud I 25 fra et klinisk synspunkt. De omhandlede forbindelser I kan anvendes som helbredende lægemidler for disse syg- I domme ved at hæmme knogleresorptionen og forhindre ned- I gang af knoglemængden eller forhindre forøgelse af se- I rumcalciumværdier forårsaget af den stheniske knoglere- 30 sorption eller sænke værdien.
De omhandlede forbindelser og deres salte kan blandes I med et hvilket som helst farmaceutisk acceptabelt bære-

Det opnåede olieagtige produkt blev størknet ved omrøring med methanol og acetone og fil-10 treret. Det faste materiale blev vasket med acetone til opnåelse af 0,42 g (cyclopropylamino)methylenbis-(phosphonsyre) som et hvidt fast stof.
