Denne opfindelse angår en række kvaternære 8-(alkyldi-thio)-1-(2-pyrimidyl)quinoliniumsalte, der overraskende er hæmmere af H+/K+ ATPase-enzymet og dermed anvendelige som ulcusbekæmpende midler.
5 Kroniske mavesår og duodenalsår, tilsammen kendt som mavesår, er en almindelig lidelse, for hvilken der anvendes en række behandlinger, herunder diætetiske forholdsregler, lægemiddelterapi og kirurgi, hvilket afhænger af tilstandens alvor. Specielt værdifulde terapeutiske mid-10 ler til brug ved behandling af forøget syreindhold i maven og mavesår er histamin-H2-receptorantagonisterne, der blokerer virkningen af den fysiologisk aktive forbindelse, histamin, ved H2-receptorstederne i dyrets krop og derved hæmmer sekretionen af mavesyre.
15 Forskellige 2-(pyridylmethylsulfinyl)benzimidazoler, såsom omeprazol, og beslægtede forbindelser (Drugs, 32, 15 (1986) vides at være ulcusbekæmpende midler, der virker ved en mekanisme, som involverer hæmningen af H+/K+ ATPa-se-enzymsystemet. For nylig er sådanne forbindelser ble-20 vet omsat med alkylmercaptaner i nærværelse af en syre til frembringelse af kvaternære 2-(alkyldithio)-1-(2-benzimidazolyl)pyridiniumsalte, der også er ulcusbekæmpende midler med syreundertrykkende egenskab og cytobe-skyttende egenskab; EP 214 479 A. Endvidere omtales 8-(2-25 benzimidazolylsulfinylalkyl)-1,2-dihydroquinolinderivater i EP ansøgning nr. 239 129 A som havende en sådan aktivitet .
I US patentskrift nr. 4 808 591 (og det tilsvarende EP patentskrift nr. 330 329) omtales ulcusbekæmpende 8-(2-30 pyrimidylsulfinyl)quinoliner. Disse forbindelser anvendes som udgangsmaterialer ved fremstilling af forbindelserne ifølge den foreliggende opfindelse.

Den farmaceutisk acceptable anion, W®, kan være, men er ikke begrænset til sådanne anioner som Cle, p-CH3C6H4S03®, 10 CH3S03®, HS04®, (S04)H®, H2P04®, (HP04)^ og (cis- HOOCCH=CHCOO)®.
For at lette fremstillingen og få en god aktivitet er den foretrukne betydning af W® Cl®/ de foretrukne betydninger af R er ethyl, isopropyl eller t-butyl, og den foretrukne 15 betydning af Z er ethoxy.

0 hvori Z er som defineret ovenfor, med mindst et molært ækvivalent af hver af en ligekædet eller forgrene T (Ci— C5)alkylmercaptan og en syre HW (hvor W danner en farmaceutisk acceptabel anion som defineret ovenfor) i et 25 reaktionsinert opløsningsmiddel (hensigtsmæssigt et delvis vandigt opløsningsmiddel, fortrinsvis vand og en lavere alkohol, såsom ethanol). Temperaturen er ikke kritisk, f.eks. er temperaturer i området fra -10 °C til 60 3 DK 171988 B1 °C almindeligvis tilfredsstillende. Eftersom reaktionen er hurtig, er omgivelsestemperatur sædvanligvis det mest bekvemme med afkøling, om ønsket, for at undgå enhver mulighed for en uhensigtsmæssig exotherm. Enzymet (1-2 pg) blev forinkuberet ved 37 °C i 45 minutter med et medium, som indeholdt 2 x 10“3 M MgCl2, 0,05 M Tris-Cl-puffer (pH 7,5) med eller uden 0,01 M KC1, og det syreaktiverede testlægemiddel i et slutvolumen på 0,590 25 ml. Reaktionen blev startet ved at tilsætte 0,010 mmol ATP (slutkoncentration 3 x 10”3 M). Reaktionen blev standset ved at tilsætte trichloreddikesyre til en koncentration på 4,2%. Frigjort, uorganisk phosphat blev bestemt under anvendelse af "Fiske and Subbarow Reducer", 30 der kan fås kommercielt (f.eks. fra Sigma Chemical CO., P.O. Box 14508, St. Louis, MO 63178, USA). In vivo anvendeligheden af de foreliggende forbindelser som ulcusbekæmpende midler vises også specielt ved deres cytobeskyttende aktivitet. En sådan aktivitet vises ved 15 hæmningen af ethanol-induceret maveulceration i rotter under anvendelse af metoden fra eksempel 18 i US patentskrift nr. 4 560 690.
Til brug ved behandling eller forebyggelse af ulcus i et pattedyr, herunder mennesket, indgives en forbindelse med 20 formlen (I) i en parietalcelle-H*/K+ ATPase-hæmmende mængde på ca. 0,25-50 mg/kg/dag, i en enkelt eller delte daglige doser. I specielle tilfælde kan doser uden for dette område blive ordineret af den tilsynsførende læge.
Den foretrukne indgivningsvej er sædvanligvis oral, men 25 parenteral indgivning (f.eks. intramuskulær, intravenøs, intradermal) vil være at foretrække i specielle tilfælde, f.eks. hvor oral absorption er formindsket ved sygdom, eller patienten er ude af stand til at synke.
Forbindelserne ifølge den foreliggende opfindelse indgi-30 ves almindeligvis i form af farmaceutiske præparater, som indeholder mindst én forbindelse med formlen (I) sammen med et farmaceutisk acceptabelt bærestof eller fortyndingsmiddel. Sådanne præparater sammensættes almindelig- 5 DK 171988 B1 vis på traditionel måde under anvendelse af faste eller flydende bærestoffer eller fortyndingsmidler, alt efter den ønskede indgivningsmåde: til oral indgivning i form af tabletter, hårde og bløde gelatinekapsler, suspensio-5 ner, granula, pulvere og lignende, og til parenteral indgivning i form af injicerbare opløsninger eller suspensioner og lignende.
Opfindelsen belyses ved de efterfølgende eksempler.
EKSEMPEL 1 10 8-(t-butyldithio)-1-(4-ethoxy-2-pyrimidyl)guinolinium- chlorid t-Butylmercaptan (8 ml) blev opløst i 400 ml 7:3 ethanol: vand, og pH-værdien blev indstillet til 2 med 1 N HC1.
Der blev tilsat 8-(4-ethoxy-2-pyrimidylsulfinyl)quinolin 15 (600 mg), og blandingen blev omrørt i 20 minutter, hvor efter opløsningsmidlet blev strippet. EKSEMPEL 3 15 8-(ethyldithio)-1-(4-ethoxy-2-pyrimidyl)quinolinium- chlorid
Ethylmercaptan (10 ml) blev opløst i 500 ml 7:3 etha-nol:H20 ved 0 °C, og pH-værdien blev indstillet til 2,0 med 1 N HC1. Claims (7)
1. Kvaternære 8-(alkyldithio)-1-(2-pyrimidyl)quinolinium-salte med den almene formel 5 r^| RS \ JsN JJL (1 > Cf z hvori W® er en farmaceutisk acceptabel anion, R er (Ci-10 C5) alkyl, og Z er (C1-C5) alkoxy.
2. Forbindelse ifølge krav 1, hvori er Cl®.
3. Forbindelse ifølge krav 1 eller 2, hvori R er ethyl, isopropyl eller t-butyl.
4. Forbindelse ifølge krav 1, 2 eller 3, hvori Z er 15 ethoxy.
