Denne opfindelse angår hidtil ukendte 1,4-dihydropyridin-forbindelser med en nitrogenholdig gruppe i en sidekæde knyttet til 2-stillingen, hvilke forbindelser er nyttige som antiiskæmiske og antihypertensive midler, samt farma-5 ceutiske præparater indeholdende sådanne forbindelser.
1,4-Dihydropyridin-forbindelserne ifølge opfindelsen reducerer bevægelsen af calcium ind i cellen, og de er således i stand til at forsinke eller forhindre den hjerte-10 kontraktur, som formodes at være forårsaget af en akkumulering af intracellulært calcium under iskæmiske tilstande. Under iskæmi kan alt for kraftig influx af calcium udøve et antal yderligere ugunstige virkninger, som yderligere vil kompromittere det iskæmiske myocardium. Disse 15 virkninger inkluderer en mindre effektiv brug af oxygen til dannelse af ATP, aktivering af fedtsyreoxidation i mitochondrierne, og muligvis fremkaldelse af cellenekrose. De omhandlede forbindelser er således nyttige ved behandling og forebyggelse af et antal hjertetilstande, så-20 som angina pectoris, hjertearytmier, hjerteanfald og hjertehypertrofi. Forbindelserne udviser ligeledes vaso-dilatorisk aktivitet, eftersom de kan inhibere calcium-influx i celler af vasculært væv, og de er derfor således også nyttige som antihypertensive midler og til behand-25 ling af coronare vasospasmer.
Fra dansk patentansøgning nr. 1099/82, som har ført til DK-B-155 601, kendes lignende 1,4-dihydropyridin-forbindelser, som er karakteristiske ved, at de i 2-stillingen 30 er substitueret med en sekundær eller tertiær aminogruppe forbundet via en alkoxymethylkæde, og som er angivet at være meget aktive calciumantagonister. Imidlertid repræsenterede disse forbindelser i sig selv en nyudvikling i forhold til fagfolks daværende opfattelse af struktur-35 aktivitets-relationer for 1,4-dihydropyridiner, og man måtte på denne baggrund antage, at det var nødvendigt at bevare en basisk sidekæde i 2-stillingen. Det er derfor 2 DK 168378 B1 overraskende, at det ifølge opfindelsen har vist sig, at den basiske nitrogenholdige gruppe i sidekæden kan erstattes af neutrale polære nitrogenholdige grupper under bevarelse af høj calciumantagonist-aktivitet.

Eftersom forbindelserne med formlen (I) indeholder et el-10 ler flere asymmetriske centre, vil de forekomme som et eller flere par af enantiomere, og sådanne par eller de individuelle isomere kan adskilles ved fysiske metoder, f.eks. ved fraktioneret krystallisation af de frie baser eller af passende salte, eller ved chromatografering af 15 de frie baser. Opfindelsen inkluderer de adskilte par såvel som blandinger deraf, som racemiske blandinger eller som adskilte d- og 1-optisk aktive isomere former.
Farmaceutisk acceptable syreadditionssalte af forbindel-20 serne med formlen (I) er sådanne, som dannes ud fra syrer, som danner ikke-toxiske syreadditionssalte, f.eks. hydrochlorid-, hydrobromid-, sulfat- eller hydrogensulfat-, phosphat- eller hydrogenphosphat-, acetat-, citrat-, fumarat-, gluconat-, lactat-, maleat-, succinat-25 og tartrat-saltene.
Særligt egnede eksempler på heterocycliske grupper i for- 5 7 bindelse med substituenteme R og R inkluderer pyridyl, pyrazinyl, hydroxypyridyl, dihydroxypyrimidinyl, pipe-30 ridinyl, piperazinyl, 4-methyl-l-piperazinyl, morfolinyl, l-imidazolidin-2-on, 2-furyl, thienyl, thiazolyl og qui-nolyl.

Forbindelser med formlen (I), hvori R4 er -C(=X)R5, hvor X er O eller S, og R5 er NHCH2C0NH2 eller NHCH2C02H, fremstilles let ud fra de forbindelser, hvori R5 er NHCI^CO^C^-C^ alkyl) ved omsætning med koncentreret am-15 moniumhydroxid til dannelse af de tilsvarende amider, eller ved hydrolyse, f.eks. med fortyndet natriumhydroxid, til dannelse af de tilsvarende syrer.

Urinstof-derivaterne, hvori R er C(=X)R , hvor X er O, 5 20 og R er NH2, fremstilles på tilsvarende måde som beskrevet ovenfor ud fra aminen (II), idet man dog anvender kaliumcyanat.
4 (b) Urinstof- eller thiourinstof-derivaterne, hvori R er 25 -C(=X)R8, hvori X er O eller S, og R8 er NH2, NH(C^-C^ alkyl), NH(C2-Cg cycloalkyl) eller N(C^-C^ alkyl)2, fremstilles ved, at man først omsætter aminerne med formlen (II) med henholdsvis phosgen eller thiophosgen, og derpå omsætter de derved opnåede isocyanat- eller isothiocya-30 nat-mellemprodukter med ammoniak eller med en (C^-C^al-kylamin, (C^-Cg)cycloalkylamin eller di(-C^)alkylamin.
Reaktionen mellem aminen (II) og thiophosgen gennemføres bekvemt ved, at man sætter thiophosgen til en omrørt op-35 løsning af aminen i en blanding af vand og methylenchlo-rid i nærvær af pulveriseret calciumcarbonat. Efter nogle timer ved stuetemperatur frasepareres det organiske lag ----- 7 DK 168378 B1 indeholdende isothiocyanat-mellemproduktet, og reaktionsproduktet isoleres. Det opvarmes derpå med ethanolisk ammoniakopløsning eller med den passende amin til dannelse af thiourinstof-reaktionsproduktet.
5 (c) Ved en alternativ fremgangsmåde fremstilles urinstof- 4 5 derivaterne med formlen (X), hvori R er (C=X)R , hvor X er 0, og R1 er NHCH2C0NH2, NHCH2CH2NH2, NH-heterocyclisk, NHNH2 eller NHNHCO^^CH^, ud fra en amin med formlen 10 (II) ved, at man først omsætter med N,N^-carbonyldiimida-zol og derpå omsætter det resulterende imidazolylcarbo-nyl-derivat med et amin- eller hydrazin-derivat med formlen NH2CH2C0NH2, NH2CH2CH2NH2, en NH2-heterocyclisk gruppe, NH2NH2 eller NH2NHC02CH2CH3.

4 Således fremstilles forbindelser, hvori R er H (R er 25 formyl), ved en konventionel formylerings-reaktion, idet man f.eks. anvender en blanding af myresyre og eddikesy- 5 reanhydrid. De forbindelser, hvori R er CFg eller (C^-C4)alkyl, fremstilles bekvemt på tilsvarende måde, idet man anvender det passende syreanhydrid i pyridin. De for- 5 30 bindeiser, hvori R er en heterocyclisk gruppe, fremstil les mere bekvemt ud fra den passende carboxyl-substitue-rede heterocycliske gruppe ved hjælp af en koblingsreaktion, idet man f.eks. anvender et diimid-kondensations-middel, såsom 1-(3-dimethylaminopropyl)-3-ethylcarbodi-35 imid.

Alternativt, i passende tilfælde (f.eks. når den hetero-cycliske gruppe er en 1-piperidyl- eller en 1-piperazi-nyl-gruppe), omsættes den heterocycliske gruppe med phosgen til dannelse af N-C0C1-mellemproduktet, som derpå 5 omsættes med aminen med formlen (II). 4 10 (e) Forbindelser med formlen (I), hvori R er -C(=N- 6 7 R°)R, fremstilles ud fra aminerne med formlen (II) ved omsætning med en forbindelse med formlen: 15 ch3s-c — (ϊν) XR10 6 X0 20 hvori R har den tidligere anførte betydning, og Rx er SCHg, NH^ eller NHC0(C1~C^ alkyl) og, i det tilfælde hvor R^ er SCHg, omsætning af produktet med en (C^-C^) alkyl- amin, dialkylamino-alkylamin eller heterocyclisk amin.
25 Reaktionen mellem aminen (II) og forbindelsen med formlen (IV) gennemføres i almindelighed ved opvarmning af reaktanterne sammen i mere eller mindre ækvimolære forhold i et reaktionsinert organisk opløsningsmiddel. En opvarmningsperiode på flere timer under tilbagesvaling i etha-30 nol findes i almindelighed at være tilstrækkelig, og produktet isoleres derpå simpelthen ved fjernelse af opløsningsmidlet og rensning ved konventionelle procedurer.
Når R^ er H, og R1^ er NH2, er forbindelsen med formlen 35 (IV) S-methyl-isothiourinstof, som bekvemt omsættes i form af sit sulfat til dannelse af de ikke-substituerede 4 guanidiner med formlen (I), hvori r er C(=NH)NH2· Denne 9 DK 168378 B1 forbindelse kan yderligere omsættes med et sulfonylha-logenid til dannelse af de forbindelser, hvori er ®^2^1-C4 a1^1) eHer S02~phenyl.
5 Når R^ er SCHg, omsættes det resulterende methylisothio-urinstof yderligere med en passende amin, sædvanligvis ved, at man sætter reaktionskomponenterne til ethanol ved stuetemperatur, typisk i en periode natten over, hvorpå produktet isoleres ved afdampning af opløsningsmidlet og 10 rensning ved konventionelle metoder, f.eks. ved omkrystallisation. Methylisothiourinstof-mellemproduktet kan ligeledes skabes ud fra thiourinstof-forbindelsen med 3 4 formlen (I) (hvori R er H, og R er C(=S)NH2 ved omsætning med methyliodid. Det resulterende S-methylisothio-15 urinsalt kan derpå omsættes med aminkomponenten som beskrevet ovenfor.
4 5 (f) Forbindelser med formlen (I), hvori R er SC^-R , fremstilles ligeledes ud fra aminerne med formlen (li) 20 ved omsætning med sulfamid eller med et sulfonylchlorid med formlen: CISO^11 — (V) 25 hvori R11 er (^-^)alkyl, NH(C^-C^ alkyl), NH(C3-Cg cyc-loalkyl), NfC-^-C^ alkyl)^, NH-phenyl, NHCO-phenyl eller en heterocyclisk, NH-heterocyclisk eller NHCO-heterocyc-lisk gruppe.
30 Reaktionen med sulfamid gennemføres typisk ved opvarmning af aminen og sulfamidet i overskud under tilbagesvaling i et reaktionsinert organisk opløsningsmiddel, f.eks. di-oxan. Efter 1 til 2 timer isoleres produktet og renses på konventionel måde. Omsætningen med et sulfonylchlorid med 35 formlen (V) gennemføres på i og for sig kendt måde ved tilsætning af sulfonylchloridet til aminen i et inert organisk opløsningsmiddel, f.eks. dichlormethan, i nærvær 10 DK 168378 B1 af en organisk base, f.eks. triethylamin. Reaktionen er i almindelighed fuldført efter nogle timer ved stuetemperatur, og produktet isoleres og renses under anvendelse af konventionelle metoder.

Fremstillingen af de som udgangsmaterialer anvendte aminer med formlen (II) er beskrevet i EP offentliggørelsesskrift nr. 89167. De forskellige reaktanter med formlerne g III, IV, V og R C02H etc. er almindeligt kendte forbin-10 delser, som enten er kommercielt tilgængelige eller kan fremstilles ved konventionelle metoder i overensstemmelse med litteraturforskrifter.
Evnen hos forbindelserne ifølge opfindelsen til at inhi-15 bere bevægelsen af calcium ind i cellen, påvises ved deres effektivitet med hensyn til at reducere isoleret hjertevævs respons på en forøgelse af calciumionkoncen-trationen in vitro. Afprøvningen gennemføres ved, at man monterer spiralt udskårne strimler af rotteaorta med den 20 ene ende fikseret og den anden fastgjort til en krafttransducer. Vævet neddykkes i et bad bestående af fysiologisk saltopløsning indeholdende kaliumioner i en koncentration på 45 millimolær og intet calcium. Derpå sættes calciumchlorid til badet med en pipette, således at 25 der fås en slutkoncentration af calciumioner på 2 millimolær. Ændringen i spænding forårsaget af den resulterende kontraktion af vævet noteres. Badet udtømmes og erstattes med frisk saltopløsning, og efter 45 minutters forløb gentages prøvningen med den bestemte forbindelse, 30 som skal afprøves, til stede i saltopløsningen. Den koncentration af forbindelsen, som kræves for at reducere responset med 50 % (IC^q), noteres.

Til indgivning til mennesker ved kurativ eller profylaktisk behandling af hjertetilstande og hypertension, vil orale doseringer af forbindelserne almindeligvis være i området 2-100 mg dagligt for en voksen gennemsnitspatient 5 (70 kg). Til en typisk voksen patient vil enkelte tablet ter eller kapsler således indeholde 1-10 mg af den aktive forbindelse i et egnet farmaceutisk acceptabelt medium eller bærestof. Doseringer til intravenøs indgivning vil typisk være i området 1-10 mg pr. enkeltdosis efter be-10 hov. I praksis vil lægen bestemme den faktiske dosering, som vil være mest egnet for en individuel patient, og denne vil variere med den bestemte patients alder, vægt og respons. De ovennævnte doseringer er eksempler på gennemsnitst ilf ælde, men der kan selvfølgelig være indivi-15 duelle tilfælde, hvor højere eller lavere doseringsområder vil være af værdi.
Til human brug kan forbindelserne med formlen (I) indgives alene; men de vil i almindelighed blive indgivet i 20 sammenblanding med et farmaceutisk bærestof, som er valgt under hensyntagen til den påtænkte indgivningsvej og farmaceutisk standardpraksis. De kan f.eks. indgives peroralt i form af tabletter indeholdende sådanne excipien-ter som stivelse eller lactose, eller i kapsler eller æg 25 enten alene eller i sammenblanding med excipienter, eller i form af eliksirer eller suspensioner indeholdende smagsgivende eller farvende midler. De kan injiceres pa-renteralt, f.eks. intravenøst, intramuskulært eller sub-cutant. Til parenteral indgivning anvendes de bedst i 30 form af en steril vandig opløsning, som kan indeholde andre stoffer, f.eks. tilstrækkeligt salte eller glucose til at gøre opløsningen isotonisk med blod.
Den foreliggende opfindelse angår således også et farma-35 ceutisk præparat indeholdende en forbindelse med formlen (I) eller et farmaceutisk acceptabelt syreadditionssalt deraf sammen med et farmaceutisk acceptabelt fortyndings- 13 DK 168378 B1 middel eller bærestof.
Forbindelserne og de farmaceutiske præparater ifølge opfindelsen kan anvendes ved en fremgangsmåde til beskyt-5 telse af hjertet mod de skadelige virkninger af iskæmi, hvorved der indgives en effektiv mængde af en forbindelse med formlen (I) eller et farmaceutisk acceptabelt syreadditionssalt deraf eller et farmaceutisk præparat som defineret ovenfor.

Forbindelserne og de farmaceutiske præparater ifølge opfindelsen kan ligeledes anvendes ved en fremgangsmåde til behandling af hypertension, hvorved der indgives en anti-hypertensiv mængde af en forbindelse med formlen (I) el-15 ler et farmaceutisk acceptabelt syreadditionssalt deraf eller et farmaceutisk præparat som defineret ovenfor.

Man satte 0,5 ml methylisocyanat til en opløsning af 0,41 g 2-(2-aminoethoxymethyl)-4-(2-chlorphenyl)-3-ethoxycar-bonyl-5-methoxycarbonyl-6-methyl-l,4-dihydropyridin i 50 ml dichlormethan, og blandingen blev omrørt ved stuetem-30 peratur i 2 timer og derpå inddampet. Inddampningsresten blev tri tureret med diethylether, og det faste stof blev opsamlet, vasket med diethylether og tørret, hvilket gav den i overskriften nævnte forbindelse (0,40 g), smp. 75-90 °C dekomponering.

Blandingen blev omrørt ved stuetemperatur i 30 minutter og derpå behandlet med en 35 opløsning af 9,7 g ethyl-4-chloracetoacetat i 150 ml tetrahydrofuran tilsat dråbevis i løbet af 30 minutter. Blandingen blev omrørt ved stuetemperatur i 16 timer, 20 DK 168378 B1 hældt ud i vand, og pH blev reguleret til 3-4 med 2M saltsyre. Den vandige opløsning blev ekstraheret med ethylacetat (3 x 200 ml), og det organiske lag blev tørret (MgS04) og inddampet til en olie, som blev opløst i 5 acetonitril og vasket med benzin. Reaktionsblandingen blev filtreret og koncentreret. Inddampningsresten blev behandlet med en opløsning af 1,0 g fumarsyre i methanol.
Det resulterende bundfald blev opsamlet, behandlet med 2M 25 ammoniumhydroxidopløsning og ekstraheret med 2 x 25 ml methylenchlorid. Blandingen blev rystet kraftigt i 15 minutter og derpå omrørt i yderligere 1 time ved stuetemperatur. En opløsning af 409 mg 2-(2-aminoethoxymethyl)-4-(2-chlorphenyl)-3-ethoxycarbonyl-5-methoxycarbonyl-6-methyl- 1,4-dihydropyridin og 202 mg triethylamin i 20 ml tørt 10 dichlormethan blev omrørt under nitrogen og afkølet i et isbad, medens man dråbevis tilsatte en opløsning af 212 mg 2-chlor-5-pyridinsulfonylchlorid i 10 ml tørt dichlormethan. Reaktionsblandingen fik lov til at opvarmes til stuetemperatur, og omrøringen blev fortsat i yderligere 3 15 timer. Det organiske lag blev tørret (MgSO^), filtreret og inddampet, hvilket gav 0,3 g af en farveløs olie, som blev opløst i diisopropylether (15 ml) og holdt i et køleskab i 14 dage.
