Description

Die Knieprothese umfasst auch eine femorale Komponente, die erste und zweite voneinander beabstandete, konvex gekrümmte Kondylenlageroberflächen aufweist und eine Nockenoberfläche, die sich zwischen den ersten und zweiten Lageroberflächen erstreckt. Wenn die Knieprothese bei einem Beugungswinkel von ungefähr 25° gebeugt wird, greift die posteriore femorale Nocke in die posteriore Seitenwand der destabilisierenden Stütze ein, um die Bewegung der femoralen Komponente relativ zu der tibialen Komponente zu steuern. - In Übereinstimmung mit einem ersten Aspekt der Erfindung wird eine Knieprothese bereitgestellt, umfassend: eine femorale Komponente zur Befestigung an einem Femur an dessen distalem Ende, aufweisend voneinander beabstandete Kondylenoberflächen zur Definition einer Aussparung zwischen diesen, eine anteriore femorale Nocke und eine posteriore femorale Nocke, die in der Aussparung angeordnet sind, eine tibiale Komponente zur Befestigung an einer Tibia an derem proximalen Ende, die eine Plattform für eine Befestigung an der Tibia und ein Lager mit einem anterioren Teil und einem posterioren Teil umfasst, wobei das Lager auf der Plattform für eine Drehbewegung um eine Achse, die sich im Allgemeinen in der Richtung der Tibia erstreckt, montiert ist, wobei das Lager Lageroberflächen zur Unterstützung der Kondylenoberflächen und einen Dorn für eine Erstreckung aufwärts in die Aussparung aufweist, der Dorn eine anteriore tibiale Nocke zum Eingreifen in die anteriore femorale Nocke, eine posteriore tibiale Nocke zum Eingreifen in die posteriore femorale Nocke und einen Scheitel zwischen der anterioren und posterioren tibialen Nocke aufweist, die anteriore tibiale Nocke aufwärts vom anterioren Teil des Lagers geneigt ist und die posteriore tibiale Nocke abwärts vom Scheitel geneigt ist, an einer Stelle neben der Drehachse des Lagers und im allgemeinen parallel zur Achse, bei der die posteriore femorale Nocke für einen Eingriff in die posteriore tibiale Nocke angeordnet ist, wenn das Knie sich in einer Beugung von ungefähr 40° bis 60° befindet, und bei der der Dom und die posteriore tibiale Nocke bemessen und positioniert sind, um eine Subluxationshöhe von ungefähr 15 mm bei einer Beugung von 90° zu gewährleisten.
- Vorzugsweise hat jeder von ihnen eine äußere Oberfläche, die vorzugsweise gleichmäßig konvex in der antero-posterioren Richtung gekrümmt ist und sich im Allgemeinen den Formen im lateralen Profil der kondylären Oberflächen des Femurs anpasst. Diese kondylären Oberflächen sind vorzugsweise bei allen Querschnitten entlang ihrer anteroposterioren Ausdehnung gleichmäßig konvex gekrümmt, und der zwischenkondyläre Teil, der die kondylären Teile verbindet, definiert eine zwischenkondyläre Aussparung mit voneinander beabstandeten lateralen Oberflächen oder Wänden. Die zwischenkondyläre Aussparung kann vorzugsweise ein Box-ähnliches Gehäuse sein mit voneinander beabstandeten lateralen Seitenwänden und einem offenen Dach. Die Plattform ist vorzugsweise bereitgestellt, um steif an das proximale Ende der Tibia befestigt zu sein, damit eine Oberfläche bereitgestellt ist, auf der das Lager sich um eine Achse dreht, die im Allgemeinen mit der Tibia ausgerichtet ist.
- In der dargestellten Ausführungsform sind die relativen Positionen und Formen des Dorns und der zwischenkondylären Aussparung der Prothese, sobald sie in das Kniegelenk implantiert worden ist, derart, dass, wenn das Bein in seiner vollständigen Ausstreckung oder in der Nähe davon ist, und wenn der Femur dazu tendiert, sich posterior relativ zu der Tibia zu verschieben, die anterioren femoralen und tibialen Nocken ineinander greifen, um eine posteriore Verschiebung der femoralen Komponente zu verhindern. Wenn das Bein teilweise gebeugt ist, sind die femoralen und tibialen Nocken voneinander beabstandet und erlauben eine relative freie antero-posteriore Translation der Komponenten, aber sind verfügbar, um übermäßige anteriore und posteriore Bewegungen zu verhindern. Bei einer Beugung von ungefähr 40° bis 60° bis zu einer Beugung von ungefähr 120° greifen die posterioren femoralen und tibialen Nocken ineinander ein und lagern aufeinander. Vorzugsweise greifen die posterioren Nocken bei einer Beugung von ungefähr 50° bis ungefähr 120° ein. Während dieser Beugung verhindert die tibiale Nocke, dass sich die femorale Komponente anterior bewegt, und der tibio-femorale Kontakt verschiebt sich posterior. Dieses femorale Zurückrollen gewährleistet theoretisch einen erhöhten Bewegungsbereich und eine verbesserte Effizienz des Quadrizeps bei tieferen Beugungswinkeln. Weiterhin sind die relativen Positionen und Formen der posterior femoralen und tibialen Nocken der Prothese, sobald sie in das Kniegelenk implantiert worden ist, derartig, dass, wenn sich das Bein einer vollen Beugung annähert, die tibiale Nocke in ausreichender Höhe ist, um eine anteriore Versetzung der femoralen Komponente zu verhindern.
- Nur als Hintergrundsinformation wird ein Verfahren zur Steuerung der anteroposterioren Translation einer Knieprothese beschrieben, das die Schritte der Bereitstellung einer femorale Komponente und der Befestigung der femoralen Komponente an einem Femur an dessen distalem Ende umfasst. Die femorale Komponente weist Kondylenoberflächen auf, die voneinander beabstandet sind, um eine Aussparung dazwischen und eine femorale anteriore Nocke und eine femorale posteriore Nocke, die in der Aussparung angeordnet sind, zu definieren. Eine tibiale Komponente ist zur Befestigung an der Tibia an ihrem proximalen Ende vorgesehen, und die tibiale Komponente umfasst eine Plattform zur Befestigung an der Plattform und ein Lager, das auf der Plattform für eine Drehbewegung um eine Achse, die sich im Allgemeinen in der Richtung der Tibia erstreckt, montiert ist. In dem Verfahren der vorliegenden Erfindung ist das Lager geformt, um Lageroberflächen für die bewegliche Unterstützung der Kondylenoberflächen der femoralen Komponente zur Verfügung zu stellen. Das Lager ist auch mit einem Dorn versehen, der sich höher (aufwärts) in die Aussparung zwischen den Kondylenoberflächen erstreckt. Dieser Dorn ist zum Eingreifen in die femoralen anterioren-posterioren Nocken vorgesehen, um eine anteriore und posteriore Stabilität und ein femorales Zurückrollen zu gewährleisten. Der Dorn weist auch eine ausreichende Größe auf, um eine adäquate Subluxationshöhe bereitzustellen. Die tibiale anteriore Nocke neigt sich aufwärts von dem anterioren Teil des Lagers und die tibiale posteriore Nocke neigt sich vorzugsweise abwärts von dem Scheitel an einer Stelle neben der Drehachse des Lagers und im Allgemeinen parallel zur Drehachse.
- Die dargestellte tibiale anteriore Nockenoberfläche neigt sich leicht aufwärts von dem am weitesten anterioren Teil des Lagers bis zu einem Punkt, an dem die Neigung ungefähr 40° bis 50° aufwärts zum Scheitel des Dorns beträgt. Die dargestellte femorale anteriore Nocke ist konvex gekrümmt, um einen Krümmungsteil zu haben, der im Allgemeine mit dem stärker geneigten Teil der tibialen anterioren Nocke von dem Punkt zu dem Scheitel ausgerichtet ist, wenn sich das Knie in voller Ausstreckung befindet. Die femorale posteriore Nocke ist auch konvex gekrümmt, und sie ist für einen Eingriff in die tibiale posteriore Nocke angeordnet, wenn das Knie sich in einer Beugung von ungefähr 40° bis 60° befindet, vorzugsweise in einer Beugung von ungefähr 50°. An diesem Punkt hindert die tibiale posteriore Nocke die femorale Komponente an einer weiteren anterioren Translation. Bei einer Beugung von 40° bis 60° bis zu einer Beugung von 120° fährt die femorale posteriore Nocke fort, in die tibiale posteriore Nocke einzugreifen und ein Zurückrollen geschieht während einer derartigen Beugung. Vorzugsweise ist ein Dorn mit einer ausreichenden Höhe vorgesehen, um die Möglichkeit der Lageveränderung bei einer Beugung von ungefähr 90° bis 120° zu reduzieren.

- Mit besonderem Bezug auf
1 wird deutlich, dass ein Kniesystem10 der vorliegenden Erfindung eine femorale Komponente12 und eine tibiale Komponente14 umfasst. Die tibiale Komponente14 umfasst eine tibiale Plattform16 , von der sich ein tibialer Schaft18 abwärts erstreckt, und ein Lager20 , das auf die Plattform16 montiert ist. Wie in den3 und4 angedeutet, ist die tibiale Plattform16 mit einer Gelenkpfanne26 vorgesehenen, die sich abwärts von einer Öffnung17 in die Plattform erstreckt, zum Aufnehmen eines Schaftes28 , der sich abwärts von dem Lager20 erstreckt. Wie auch in3 und4 gezeigt, ist der Schaft28 des Lagers20 mit einem im Allgemeinen zylindrischen Teil30 vorgesehen, der drehbar in einem im Allgemeinen zylindrischen Teil32 der Gelenkpfanne26 lagert, und der Schaft28 endet in einem distalen konischen Teil24 , der in einem damit zusammenpassenden distalen konischen Teil22 der Gelenkpfanne26 lagert. Es wird jedoch verständlich, dass der Schaft28 und die Gelenkpfanne26 nicht mit derartigen zylindrischen und konischen Teilen versehen werden müssen. Andere Halterungskonfigurationen, die eine relativ freie Drehbewegung ermöglichen, liegen innerhalb des Umfanges dieser Erfindung. - Wie man am besten in
1 sehen kann, ist der dargestellte tibiale Schaft18 mit radial voneinander beabstandeten, sich nach abwärts erstreckenden Erhöhungen34 , die sich in Richtung einer distalen Spitze36 des Schaftes erstrecken und die, wie angezeigt, bei38 enden, vorgesehen. Diese radial voneinander beabstandeten Erhöhungen34 dienen dazu, um die tibiale Komponente14 in der Tibia gegen eine Drehung um die Achse des Schaftes18 zu verankern. Es ist gut bekannt, dass der Schaft18 mit einer porösen Metallbeschichtung vorgesehen sein kann, in die der Knochen der Tibia hineinwachsen wird, um die tibiale Komponente in Position in der Tibia zu halten. Es ist auch gut bekannt, dass die tibiale Komponente14 in die Tibia zementiert sein kann. Andere Konfigurationen und Techniken zur Verankerung der tibialen Komponente an eine Tibia sind bekannt und sind innerhalb des Umfanges dieser Erfindung. Die tibiale Plattform16 stellt eine obere Plattformoberfläche40 zur Verfügung, die als eine Lageroberfläche dient, um eine Bodenoberfläche42 des Lagers20 zu unterstützen. Dem Lager20 ist es erlaubt, sich um die Achse seines Schaftes28 , der in der Gelenkpfanne26 aufgenommen ist, zu drehen. Solche Lager sind als Drehlager bekannt, dadurch, dass das Lager20 sich drehend relativ zu der tibialen Plattform16 , die an der Tibia verankert ist, bewegen kann. - In Bezug auf
6 , wird es anhand der perspektivischen Ansicht des oberen Teils des Lagers20 sichtbar, dass das Lager20 mit Lageroberflächen50 ,52 zur Unterstützung der Kondylenoberflächen84 ,86 der femoralen Komponente12 versehen ist. Die Lageroberflächen50 ,52 können in einer Anzahl von Arten konfiguriert werden, wie zum Beispiel jenen, die in denUS-Patentschriften Nrs. 4,309,778 ,4,340,978 und4,470,158 offenbart sind, die hierin durch Bezugnahme aufgenommen sind. Ein Dorn60 , der manchmal als ein Höcker oder eine Stütze bezeichnet wird, ist zwischen den Lageroberflächen50 ,52 angeordnet und ist mit Seiten62 ,64 versehen, die sich aufwärts erstrecken. Der Dorn60 ist auch mit einer anterioren tibialen Nocke66 , einer posterioren tibialen Nocke68 und einem Scheitel58 dazwischen versehen. Eine Rampe74 ist posterior von dem Dorn60 angeordnet, um sich aufwärts zu einem posterioren Ende56 zu neigen, wie am besten in4 zu sehen ist. Die posteriore tibiale Nocke68 des Dorns60 und die Rampe74 verbinden sich, um eine Aussparung oder um einen gleichmäßigen Übergang76 bereitzustellen, wie am besten in4 zu sehen ist. Mit weiterer Bezugnahme auf4 wird es deutlich, dass die anteriore tibiale Nocke66 eine Rampen-ähnliche Oberfläche aufweist, die der Nähe des anterioren Teiles des Lagers20 beginnt und aufwärts und zum Scheitel des Domes60 verläuft. Am Anfang neigt sich die anteriore tibiale Nocke66 in der Nähe eines anterioren Endes54 des Lagers20 leicht aufwärts, und weiter zurück umfasst die anteriore tibiale Nocke66 eine Oberfläche66a , die in einem Winkel α von ungefähr 40° bis 50° relativ zu der Fläche x geneigt ist. Die Fläche x kann als zusammenfallend mit der Oberfläche42 des Lagers20 , welches eine Plattform16 kontaktiert, gedacht werden oder kann wahlweise als eine Fläche gedacht werden, die im Allgemeinen senkrecht zu der Achse des Schaftes28 steht. Wahlweise, wenn die Prothese in einen Patienten implantiert wurde, dessen Bein in voller Streckung auf einer flachen Oberfläche positioniert ist, wie in7 gezeigt, kann die Fläche x als parallel zur Ebene der flachen Oberfläche gedacht sein. In der dargestellten Ausführungsform ist der Winkel α ungefähr 45°. - In der dargestellten Ausführungsform erstreckt sich die posteriore tibiale Nocke
6 im Wesentlichen linear in die proximale distale Richtung vom Scheitel58 zum Übergang76 . Die posteriore tibiale Nocke68 kann jedoch in einer beliebigen Anzahl von Wegen gekrümmt sein und dennoch eine geeignete Oberfläche bereitstellen, um ein Zurückrollen und eine posteriore Stabilität zu gewährleisten. Derartige alternative Dornkonfigurationen sind innerhalb des Umfanges und der Idee dieser Erfindung. - Wie am besten in
5 gesehen werden kann, ist die femorale Komponente12 konventionell geformt mit einem anterioren Teil80 und einem posterioren Teil82 , die Kondylen oder Kondylenlager84 ,86 bereitstellen, um beweglich in die Lageroberflächen50 ,52 des Lagers20 einzugreifen. Bevorzugte Ausführungsformen können in den '778- und '978-Patenten gefunden werden, die oben diskutiert wurden und durch Bezugnahme aufgenommen sind. Die femorale Komponente12 kann herkömmlich aus einem geeigneten Metall hergestellt werden, während das Lager20 aus einem Kunststoffmaterial, wie zum Beispiel Polyethylen (UHMWPE) mit ultrahohem Molekulargewicht, hergestellt werden kann. Die Referenzen im Stand der Technik, auf die sich oben bezogen wurde, beschreiben die Beschaffenheit der zulässigen Metalle und Kunststoffe für das Protheseimplantat. Die Kondylenlager84 ,86 werden Krümmungsradien haben, um einen entsprechenden Kontakt zwischen der femoralen Komponente12 und der tibialen Komponente14 bereitzustellen, wie in dem Stand der Technik diskutiert wird. Es ist zu beachten, dass die Kondylenlager84 ,86 bei vollständiger Ausstreckung, wie in7 dargestellt, mehr kongruent mit den Oberflächen50 ,52 des Lagers20 sind, als wenn das Knie in einer Beugung von 120° ist, wie in8 dargestellt. Bei einer Beugung von 120° wird nur der am weitesten posteriore Teil der Kondylenlager84 ,86 in Kontakt mit den Oberflächen50 ,52 des Lagers20 sein und der Kontaktpunkt wird posterior verschoben. - Es ist weiterhin zu beachten, dass bei einer typischen Kniebewegung eine beträchtliche anterior-posteriore Bewegung der femoralen Komponente
12 auf der tibialen Komponente14 auftritt, wobei in den Abbildungen die anteriore Richtung durch den Pfeil „A" und die posteriore Richtung durch den Pfeil „P" repräsentiert ist. Entsprechend dieser Erfindung wird die Translation der femoralen Komponente12 relativ zu der tibialen Komponente14 gesteuert, sobald sich das Knie zwischen vollständiger Ausstreckung und vollständiger Beugung bewegt. Diese Steuerung wird durch den Dorn60 mit seinen Nocken66 ,68 bereitgestellt. Der Dorn60 erstreckt sich besonders aufwärts in eine zwischenkondyläre Aussparung88 , die zwischen den Kondylenlagern84 ,86 in der femoralen Komponente12 angeordnet ist. Innerhalb dieser zwischenkondylären Aussparung88 befindet sich eine anteriore femorale Nocke100 und eine posteriore femorale Nocke102 zum Eingreifen jeweils in die anteriore tibiale Nocke66 und die posteriore tibiale Nocke68 des Domes60 . Diese Beziehung ist am besten sichtbar in3 und7 -9 . Wie am besten in3 gesehen wird, ist die anteriore femorale Nocke100 auf der femoralen Komponente12 konvex und erstreckt sich distal von dem anterioren Teil eines Daches110 der zwischenkondylären Aussparung88 , wie es am besten sichtbar in3 und7 -9 ist. Ebenso ist die posteriore femorale Nocke102 auf der femoralen Komponente12 ähnlich konvex und erstreckt sich distal von dem posterioren Teil des Daches110 der zwischenkondylären Aussparung88 . - Wie am besten in
5 zu sehen ist, sind innere Wände92 ,94 der zwischenkondylären Aussparung88 durch ein Nockengehäuse bereitgestellt, das durch die Seiten106 ,108 und das Dach110 , das veranschaulichend eine darin gebildete quadratische Öffnung112 hat, geformt ist. Die femorale Komponente12 kann an den herausgeschnittenen Femur zementiert werden oder die femorale Komponente kann mit einem porösen Material für den Knocheneinwuchs beschichtet werden. Alternativ kann sich eine Stamm-ähnliche Befestigungskomponente (nicht gezeigt) aufwärts durch die Öffnung112 erstrecken, um die femorale Komponente12 an dem Femur zu fixieren. In einer bevorzugten Ausführungsform, gezeigt in1 , kann das flache Dach weggelassen werden und ein proximales Ende130 der zwischenkondylären Aussparung88 kann nur durch eine proximale Oberfläche122 der anterioren femoralen Nocke100 , eine proximale Oberfläche124 der posterioren femoralen Nocke102 , eine proximale Kante126 der Seite106 und eine proximale Kante128 der Seite108 definiert werden. Es ist verständlich, dass diese bevorzugte Konfiguration einen zusätzlichen Raum für den Dorn60 ohne Herausschneiden von zusätzlichem Knochen bereitstellt. - Die Art, mit der die femorale Komponente
12 auf das digitale Ende eines Femur montiert werden kann, ist gut bekannt und braucht nicht im Detail für den Fachmann beschrieben werden. - Im Einsatz bilden die anteriore femorale Nocke
100 und die posteriore femorale Nocke102 der femoralen Komponente12 mit dem Dorn60 ein Gelenk auf dem tibialen Lager20 , um eine posteriore Stabilität zu gewährleisten. Alternativ formuliert, bildet der Dorn60 mit der anterioren femoralen Nocke100 und der posterioren femoralen Nocke102 der Femurkomponente12 ein Gelenk, um Stabilität zu gewährleisten. In der vollständig ausgestreckten Position der7 greift die anteriore tibiale Nocke66 des Dorns60 in die anteriore femorale Nocke100 ein, um zum Beispiel einen 3° Überdehnungsstop bereitzustellen. Die posteriore tibiale Nocke68 des Dorns60 greift in eine posteriore femorale Nocke102 bei einer Beugung von ungefähr 40° bis 60°, vorzugsweise bei einer Beugung von ungefähr 50°. Eine Gangart, die ein normales Gewicht trägt, umfasst Beugungswinkel von ungefähr 40° oder weniger. Daher greift der Dorn60 während der gewichtstragenden Phase des Gehens nicht in die posteriore femorale Nocke102 ein. Bei Aktivitäten, die Beugungswinkel von ungefähr 40° bis 60° einbeziehen, wie zum Beispiel Treppensteigen oder Aufstehen von einem Stuhl, greift die posteriore tibiale Nocke68 des Dorns60 in die posteriore femorale Nocke102 , um posteriore Stabilität und femorales Zurückrollen zu gewährleisten, wie in7 und8 dargestellt ist. Das femorale Zurückrollen ist die posteriore Verschiebung des tibio-femoralen Kontaktpunktes, während das Knie sich von einer vollständigen Ausstreckung (7 ) in eine tiefe Beugung beugt (8 ). Wie dargestellt, stellt das Kniesystem10 ungefähr 11 mm des femoralen Zurückrollens über eine Prothese von mittlerer Größe bereit. Vorzugsweise gewährleistet dieses Zurückrollen, kombiniert mit der Eigenschaft der Drehung, einen kontinuierlichen medialen und lateralen Kontakt zwischen den femoralen und tibialen Lageroberflächen. - Ein Element, das bei der Gestaltung der herkömmlichen posterioren, stabilisierten Kniee in Betracht gezogen wird, ist die Subluxationshöhe. Diese ist definiert, als der Abstand, den die femorale Komponente
12 angehoben werden muss, um während der Beugung (9 ) auszurenken oder über die Spitze des tibialen Dorns60 herüber zu springen, wobei die maximale Subluxationshöhe bei116 angezeigt ist. Wie in9 gezeigt, ist die maximale Subluxation116 die Distanz von einem am weitesten distalen Teil118 der femoralen Komponente12 bis zu einer korrespondierenden Stelle130 auf der Lageroberfläche50 , in die die femorale Komponente12 normalerweise bei dem vollständigen Zurückrollen eingreifen würde. Wenn natürlich der Zustand eintritt, der in9 dargestellt ist, kann eine Verlagerung stattfinden, wenn die Laxheit der kollateralen Bänder in der Beugung erlaubt, dass die Knieprothese die Subluxationshöhe der Konstruktion überschreitet. Zum Beispiel kann für eine Prothese in mittlerer Größe eine Subluxationshöhe116 von 15,4 mm bei einer Beugung (eine tiefe Beugung liegt vor, wenn die Komplikation der Verlagerung auftritt) von 90° und 14,5 mm bei einer Beugung von 120° geeignet sein. Es ist zu erkennen, dass die Subluxationshöhe116 von dem Punkt auf dem Kondylenlager84 aus, der bei118 angezeigt ist, gemessen wird, bis zu der Stelle120 auf der Lageroberfläche50 . - Die chirurgischen Techniken zum Installieren des Kniesystems
10 der vorliegenden Erfindung können ähnlich sein wie bekannte chirurgische Techniken, die für das Installieren von Knien mit einer drehenden Plattform oder für Lagerkomponenten einer tiefen Schale verwendet werden. Es ist verständlich, dass eine Ausnahme dazu vorliegt, wenn der vertiefende Schnitt, der die Trochlea betrifft, der mit der Fertigstellung der Führung für den Femur gemacht wird, tiefer geschnitten werden kann, um Platz in dem distalen femoralen Knochen für die zwischenkondyläre Aussparung88 der femoralen Komponente12 zur Verfügung zu stellen. Claims (8)
- Knieprothese umfassend: eine femorale Komponente (
12 ) zur Befestigung an einem Femur an dessen distalem Ende, aufweisend voneinander beabstandete Kondylenoberflächen (84 ,86 ) zur Definition einer Aussparung (88 ) zwischen diesen, eine anteriore femorale Nocke (100 ) und eine posteriore femorale Nocke (102 ), die in der Aussparung (88 ) angeordnet sind; eine tibiale Komponente (14 ) zur Befestigung an einer Tibia an deren proximalem Ende, die eine Plattform (16 ) für eine Befestigung an der Tibia und ein Lager (20 ) mit einem anterioren Teil und einem posterioren Teil umfasst, wobei das Lager auf der Plattform für eine Drehbewegung um eine Achse, die sich im Allgemeinen in der Richtung der Tibia erstreckt, montiert ist; wobei das Lager (20 ) Lageroberflächen (50 ,52 ) zur Unterstützung der Kondylenoberflächen (84 ,86 ) und einen Dorn (60 ) für eine Erstreckung aufwärts in die Aussparung (88 ) aufweist, der Dorn eine anteriore tibiale Nocke (66 ) zum Eingreifen in die anteriore femorale Nocke (100 ), eine posteriore tibiale Nocke (68 ) zum Eingreifen in die posteriore femorale Nocke (102 ) und einen Scheitel (58 ) zwischen der anterioren (66 ) und posterioren (68 ) tibialen Nocke aufweist, die anteriore tibiale Nocke (66 ) aufwärts vom anterioren Teil des Lagers (20 ) geneigt ist und die posteriore tibiale Nocke (68 ) abwärts vom Scheitel (58 ) geneigt ist, an einer Stelle neben der Drehachse des Lagers und im Allgemeinen parallel zur Achse; bei der die posteriore femorale Nocke (102 ) für einen Eingriff in die posteriore tibiale Nocke (68 ) angeordnet ist, wenn das Knie sich in einer Beugung von ungefähr 40° bis 60° befindet; und bei der der Dorn (60 ) und die posteriore tibiale Nocke (102 ) bemessen und positioniert sind, um eine Subluxationshöhe von ungefähr 15 mm bei einer Beugung von 90° zu gewährleisten. - Prothese nach Anspruch 1, bei der das Lager (
20 ) ein Oberfläche umfasst, die die Plattform (16 ) berührt, und die anteriore tibiale Nocke (66 ) eine Oberfläche umfasst, die unter einem Winkel von ungefähr 40° bis 50° gegenüber der Oberfläche des Lagers, das die Plattform berührt, geneigt ist. - Prothese nach Anspruch 2, bei der ein Teil der anterioren femoralen Nocke (
100 ) neben der Oberfläche der anterioren tibialen Nocke (66 ) ist. - Prothese nach Anspruch 1, bei der die posteriore tibiale Nocke (
68 ) im Wesentlichen planar ist. - Prothese nach Anspruch 1, bei der die posteriore femorale Nocke (
102 ) konvex gekrümmt ist und die femorale Komponente (12 ) derart geformt ist, dass ein femorales Zurückrollen gewährleistet ist, wenn die Beugung zunimmt. - Prothese nach Anspruch 1, bei der das tibiale Lager Polyethylen mit ultrahohem Molekulargewicht umfasst.
- Knieprothese nach Anspruch 1, wobei der Dorn (
60 ) in die posteriore femorale Nocke (102 ) bei einer Beugung von ungefähr 50° eingreift. - Knieprothese nach Anspruch 1, bei der die Kondylenoberflächen (
84 ,86 ) eine mediale Kondylusoberfläche und eine laterale Kondylusoberfläche umfassen und die Kondylenoberflächen und die Lageroberflächen (50 ,52 ) derart positioniert und entworfen sind, dass von einer etwa vollständigen Streckung bis zu einer Beugung von ungefähr 120° die mediale Kondylusoberfläche im Allgemeinen in Kontakt mit der medialen Lageroberfläche bleibt und die laterale Kondylusoberfläche im Allgemeinen in Kontakt mit der lateralen Lageroberfläche bleibt.
