- TECHNISCHES GEBIET
- Polyesteramide mit Gruppen, die von Aminosäuren stammen, die mit Gruppen, die von alpha-Hydroxy stammen, alternieren, und Verfahren zu deren Herstellung sind hierin beschrieben. Aus Polyesteramiden werden geformte chirurgische Vorrichtungen erzeugt.
- STAND DER TECHNIK
- Polyesteramide sind Polymere, die sowohl Ester- als auch Amidbindungen enthalten. Deren Bedeutung für die Technologie von chirurgischen Vorrichtungen stammt von der Tatsache, daß die Anfälligkeit ihrer Esterbindungen für Hydrolyse diesen die Fähigkeit zur möglichen Absorption oder Resorption durch einen Körper, in den sie implantiert worden sind, überträgt und ihre Amidbindungen diesen die wünschenswerten mechanischen Eigenschaft, die für Polyamide charakteristisch sind, überträgt.
- Faserbildende Polyesteramide, erhalten von der einstufigen Reaktion von ungefähr äquimolaren Mengen eines Monoalkanolamins und einer Dicarbonsäure sind aus dem US-Patent 2 386 454 bekannt. Polyesteramide, die zur Erzeugung von absorbierbaren Nähten und anderen chirurgischen Vorrichtungen angezeigt sind, sind in dem US-Patent 4 226 243 offenbart und werden durch Reaktion eines Bis-oxyamiddiols (das selbst von der Reaktion von Diethyloxalat mit einem Monoalkanolamin wie Ethanolamin stammt) mit einem Dicarbonsäureester erhalten. Vergleiche ebenfalls US-A-3 773 737 (und das erneut erteilte Patent 30170). US-Patent 4 343 931 offenbart absorbierbare chirurgische Vorrichtungen, hergestellt von Polyesteramiden, erhalten durch Reaktion eines Diamins mit Milch- oder Glykolsäure, zur Erzeugung eines Diamiddiols, das dann mit einem Bischlorformiat oder einer Verbindung reagiert wird, ausgewählt aus der Gruppe, bestehend aus Dicarbonsäuren, Dicarbonsäurechloriden und Dicarbonsäureanhydriden.
- Nylon betrifft eine Familie mit hoher Stärke, elastischen synthetischen Materialien, von denen die langkettigen Moleküle wiederholende Amid-Gruppen enthalten. Gegenstände, die aus Nylon hergestellt sind, wurden in großem Umfang für eine Vielzahl von Anwendungen akzeptiert. Bestimmte chirurgische Anwendungen erfordern jedoch eine bioabsorbierbare chirurgische Vorrichtung. Nylon ist nicht bioabsorbierbar und daher unter solchen Umständen nicht akzeptabel.
- Es ist gewünscht, eine geformte chirurgische Vorrichtung anzugeben, die eine Stärke und elastische Eigenschaften gleichermaßen wie Nylon hat, die aber bioabsorbierbar ist.
- ZUSAMMENFASSUNG
- Es wurde nun festgestellt, daß Polyesteramide mit Gruppen, die von Aminosäuren abstammen, die mit von Hydroxysäuren stammenden Gruppen alternieren, zur Erzeugung von Formgegenständen, insbesondere Formgegenständen nützlich sind, die zur Verwendung als chirurgische Vorrichtungen geeignet sind.

- Das Verfahren zur Erzeugung dieser Polyesteramide beinhaltet das Blockieren der Amino-Gruppe der Aminosäure, Reaktion mit einer alpha-Hydroxysäure, Entfernung des Blockiermittels und anschließende Polymerisation zur Bildung des Polyesteramides.
- Das resultierende Polyesteramid kann bioabsorbierbar sein und durch Formen zu Formgegenständen geformt werden. Besonders nützliche Formgegenstände umfassen chirurgische Implantate.
- DETAILLIERTE BESCHREIBUNG DER BEVORZUGTEN MERKMALE

- Der erste Schritt zur Erzeugung des Polyesteramides besteht in der Blockade der Amino-Gruppe der Aminosäure. Dies stellt sicher, daß die anschließende Chlorierung und Reaktion mit der alpha-Hydroxysäure nur an der Säuregruppe der Aminosäure auftritt. Die Einführung eines Blockiermittels in die Amino-Gruppe kann unter Anwendung irgendeiner Technik, die dem Fachmann bekannt ist, vollendet werden. Geeignete Blockiermittel umfassen Benzylchlorformiat. Benzylchlorformiat kann mit der Aminosäure in der Gegenwart eines Metalloxides (z.B. MgO) in einem Eisbad einer wäßrigen Ether-Lösung reagiert werden.
- Die geschützte Aminosäure wird dann in ein Säurehalogenid umgewandelt. Dies kann beispielsweise erreicht werden, indem die geschützte Aminosäure mit SOX2, worin X F, Cl, Br oder I ist, in Methylenchlorid unter Rückfluß gehalten wird.
- Das blockierte Säurehalogenid wird mit einer alpha-Hydroxysäure reagiert. Geeignete alpha-Hydroxysäuren umfassen solche der Formel: worin R gleich oder verschieden. bei jedem Auftreten sein kann und individuell ausgewählt wird aus der Gruppe, bestehend aus Wasserstoff und C1–4-Alkyl. Glykolsäure ist die bevorzugte Alkylhydroxysäure zur Erzeugung der hierin beschriebenen Polyesteramide. Die Reaktion des Säurechlorides mit der alpha-Hydroxysäure kann durch Rückfluß halten in der Gegenwart eines inerten Verdünnungsmittels wie z.B. DMF und eines tertiären Amins wie z.B. Triethylamin (TEA) durchgeführt werden.
- Das Blockiermittel wird dann von dem Säurechlorid unter Anwendung von dem Fachmann bekannten Techniken entfernt. Wenn beispielsweise Benzylchlorformiat als Blockiermittel verwendet wird, kann das blockierte Säurechlorid mit HBr in Essigsäure zur Entfernung des Blockiermittels reagiert werden.

- Das Monomer sollte gereinigt werden, bevorzugt auf wenigstens etwa 98 % Reinheit. Das Monomer kann unter Anwendung irgendeiner bekannten Technik wie vielfachen Destillationen und/oder Rekristallisierungen gereinigt werden.
- Das Monomer wird durch Erwärmen zur Erzeugung einer Selbstkondensationsreaktion polymerisiert. Zum Beispiel kann die Schmelzpolymerisation des von Glycin und Glykolsäure erzeugten Monomers erreicht werden, indem auf etwa 240°C erwärmt wird.
- Falls gewünscht, kann irgendein Anteil der beschriebenen Aminosäure/Estermonomers, die bei der Polymerisationsreaktion verwendet werden, zum Beispiel etwa 1 bis etwa 99 mol% mit einem ähnlichen mol%satz eines anderen difunktionellen, selbstkondensierenden Comonomers ersetzt werden. Solche Monomeren umfassen Verbindungen, die sowohl eine Säuregruppe als auch eine zweite aktive Gruppe wie eine Hydroxy-Gruppe, Amino-Gruppe oder zweite Säuregruppe umfassen. Somit umfassen als Beispiel geeignete Comonomere Hydroxycarbonsäuren, Dicarbonsäuren und Aminosäuren.
- Die Polyesteramide dieser Erfindung, die unten in Anspruch 1 definiert sind, können zu chirurgischen Gegenständen unter Anwendung irgendeiner bekannten Technik wie zum Beispiel Extrusion, Formen und/oder Lösungsmittelguß geformt werden. Die Polyesteramide können alleine, gemischt mit anderen absorbierbaren Zusammensetzungen oder in Kombination mit nicht-absorbierbaren Komponenten verwendet werden. Eine große Vielzahl von geformten chirurgischen Gegenständen kann von den hierin beschriebenen Polyesteramiden hergestellt werden. Diese umfassen Klammern und andere Befestigungsmittel, Klemmen, Stifte, Schrauben, prothetische Vorrichtungen, Verbandsmaterialien für Wunden, Arzneimittelabgabevorrichtungen, Anastomoseringe und andere implantierbare Geräte.
- Wahlweise Additive, die in Zusammensetzungen aus den hierin beschriebenen Polyesteramiden vorhanden sein können, umfassen Plastifizierer, Freisetzungsmittel und andere Verarbeitungshilfen. Wenn die Zusammensetzung zur Erzeugung einer chirurgischen Vorrichtung verwendet wird, sind Stearinsäure oder Calciumstearat besonders nützliche Additive aufgrund ihrer Biokompatibilität.
- Die Formgebungen, umfassend die hierin beschriebenen Polyesteramide, können verstärkte Verbundmaterialien sein. Zum Beispiel können die Polyesteramid-Zusammensetzungen die Matrix des Verbundes bilden und mit bioabsorbierbaren oder nicht-absorbierbaren Fasern oder Teilchen verstärkt sein. Alternativ kann eine Matrix aus irgendeiner bioabsorbierbaren oder nicht-bioabsorbierbaren Polymerzusammensetzung mit Fasern oder körnigem Material aus Zusammensetzungen mit den hierin beschriebenen Polyesteramiden verstärkt sein.
- In einem alternativen Merkmal werden in einer Formzusammensetzung die hierin beschriebenen Polyesteramide mit einem Füllstoff vermischt. Der Füllstoff kann in einer körnigen Form, einschließlich Granulat und Stapelfasern vorliegen. Während irgendein bekannter Füllstoff verwendet werden kann, sind Hydroxyapatit, Tricalciumphosphat, Bioglas oder andere Biokeramiken die bevorzugten Füllstoffe. Etwa 10 g bis etwa 400 g Füllstoff werden normalerweise mit 100 g Polymer gemischt.
- Es wird weiterhin in Erwägung gezogen, daß eine oder mehrere medico-chirurgisch nützliche Substanzen in Zusammensetzungen eingefügt werden können, die die hierin beschriebenen Polyesteramide enthalten. Beispiele solcher medico-chirurgisch nützlichen Substanzen umfassen beispielsweise solche, die den Heilprozeß beschleunigen oder grundsätzlich modifizieren, wenn Teilchen auf eine chirurgische Reparaturstelle aufgetragen werden. Somit können Formgegenstände entsprechend dieser Erfindung ein therapeutisches Mittel tragen, das an der Reparaturstelle niedergeschlagen wird. Das therapeutische Mittel kann wegen seiner antimikrobiellen Eigenschaften, der Fähigkeit zur Förderung der Wiederherstellung oder Rekonstruktion und/oder des neuen Gewebewachstums ausgewählt werden. Antimikrobielle Mittel wie Antibiotika mit breitem Spektrum (Gentamycinsulfat, Erythromycin oder derivatisierte Glycopeptide), die langsam in das Gewebe freigesetzt werden, können auf diese Weise angewandt werden, um auf diese Weise die Bekämpfung von klinischen und subklinischen Infektionen an einer Gewebe-Reparaturstelle zu unterstützen. Zur Förderung der Reparatur und/oder Gewebewachstums können ein oder mehrere Wachstumsförderfaktoren, Wachstumsfaktor, der von Plättchen stammt, Wachstumsfaktor, der von Makrophagen stammt, Wachstumsfaktor, der von Alveolar stammt, Wachstumsfaktor, der von Monozyten stammt, Magainin usw. eingeführt werden. Einige therapeutische Indikationen sind: Glycerin mit Gewebe- oder Nierenplasminogenaktivator zur Verursachung von Thrombose, Superoxiddimutase zum Einfangen von gewebeschädigenden freien Radikalen, Tumornekrosefaktor für die Krebstherapie oder koloniestimulierender Faktor und Interferon, Interleukin-2 oder andere Lymphokine zur Verstärkung des Immunsystems.
- Es wird in Erwägung gezogen, daß es wünschenswert sein kann, Gegenstände aus Zusammensetzungen, die die erfindungsgemäßen Polyesteramide enthalten, zu färben, um die Sichtbarkeit des Gegenstandes auf dem chirurgischen Gebiet zu erhöhen.
- Farbstoffe wie solche können verwendet werden, von denen bekannt ist, daß sie für die Einfügung in chirurgische Nähte geeignet sind. Solche Farbstoffe umfassen Ruß, Knochenschwarz, D&C Green No. 6 und D&C Viole No. 2, wie in dem Handbook von U.S. Colorants for Food, Drugs and Cosmetics von Daniel M. Marrion (1979) beschrieben ist, sind jedoch hierauf nicht beschränkt. Bevorzugt werden Gegenstände entsprechend dieser Offenbarung durch Zugabe von bis zu einigen wenigen Prozent und bevorzugt etwa 0,2 % Farbstoff zu der Harzzusammensetzung vor der Extrusion gefärbt.
Claims (16)
- Geformte chirurgische Vorrichtung, die ein Formling aus einer Zusammensetzung ist, umfassend: eine Esteramidverbindung, die ein Polymer ist, das Einheiten der allgemeinen Formel umfaßt: worin x eine ganze Zahl von 1 bis 6 ist und R bei jedem Auftreten gleich oder verschieden sein kann und unabhängig ausgewählt ist aus Wasserstoff und C1-C4-Alkyl.
- Formling entsprechend Anspruch 1, weiterhin umfassend zumindest eine mediko-chirurgisch nützliche Verbindung.
- Formling entsprechend Anspruch 1, weiterhin umfassend einen Plastifizierer.
- Formling entsprechend Anspruch 1, weiterhin umfassend einen Füllstoff.
- Formling entsprechend Anspruch 1, weiterhin umfassend ein oder mehrere bioabsorbierbare Polymere, ausgewählt aus Homopolymeren und Copolymeren von Glycolid, Lactid, Trimethylencarbonat, Dioxanon und Caprolacton, gemischt mit der Esteramidverbindung.
- Verfahren zur Erzeugung der geformten chirurgischen Vorrichtung nach Anspruch 1 aus einer Polyesteramid enthaltenden Zusammensetzung, wobei das Verfahren umfaßt: a) Blockieren der Aminogruppe einer Aminosäure; b) Halogenieren der blockierten Aminosäure zur Erzeugung einer chlorierten Säure; c) Reaktion der halogenierten Säure mit einer alpha-Hydroxysäure unter Erhalt eines Monomers; und d) Abspalten der Schutzgruppe beim Produkt von Schritt c); e) Polymerisieren des Monomers zur Erzeugung eines Polyesteramids; und f) Formen der Zusammensetzung zur Formgebung der chirurgischen Vorrichtung.
- Verfahren entsprechend Anspruch 9, worin Schritt a) die Reaktion einer Aminosäure mit Benzylchlorformiat umfaßt.
- Verfahren entsprechend Anspruch 9, worin Schritt b) die Reaktion der blockierten Aminosäure mit einer Verbindung der Formel SOX2 umfaßt, worin X ausgewählt ist aus F, Cl, Br und I.
- Verfahren entsprechend Anspruch 9, worin Schritt b) die Chlorierung der blockierten Aminosäure umfaßt.
- Verfahren entsprechend Anspruch 9, worin Schritt c) die Reaktion des Säurehalogenids mit Glycolsäure umfaßt.
- Verfahren entsprechend Anspruch 9, worin das Polyesteramid, erzeugt gemäß Schritt e), im wesentlichen aus Wiederholungseinheiten der allgemeinen Formel besteht: worin x eine ganze Zahl von 1 bis 6 ist und R bei jedem Auftreten gleich oder verschieden sein kann und unabhängig ausgewählt ist aus Wasserstoff und C1-C4-Alkyl.
