- HINTERGRUND DER ERFINDUNG
- Es ist bekannt, dass gewisse biologisch aktive Verbindungen besser durch die orale Schleimhaut absorbiert werden als über andere Routen der Verabreichung, wie z.B. durch den Magen oder Darm. Formulierungen, die für die Verabreichung über diese letzteren Routen geeignet sind, zeigen jedoch ihre eigenen Probleme. Z.B. muss die biologisch aktive Verbindung mit den anderen Bestandteilen der Zusammensetzung kompatibel sein, wie z.B. Treibmitteln, Lösungsmitteln, etc. Viele solche Formulierungen wurden vorgeschlagen. Z.B. beschreibt
U.S.P. 4 689 233 , Dvorsky et al., eine weiche Gelatinekapsel für die Verabreichung des Antikoronar-Arzneimittels Nifedipin, gelöst in einer Mischung aus Polyetheralkoholen.U.S.P. 4 755 389 , Jones et al., beschreibt eine kaubare harte Gelatinekapsel, enthaltend Nifedipin. Eine kaubare Gelatinekapsel, enthaltend eine Lösung oder Dispersion eines Arzneimittels, wird inU.S.P. 4 935 243 , Borkan et al., beschrieben.U.S.P. 4 919 919 , Aouda et al., undU.S.P. 5 370 862 , Klokkers-Bethke, beschreiben ein Nitroglycerinspray, aufweisend Nitroglycerin, Ethanol und andere Bestandteile, zur Verabreichung auf die orale Schleimhaut. Ein oral verabreichtes Pumpspray wird von Cholcha inU.S.P. 5 186 925 beschrieben. Aerosolzusammensetzungen, enthaltend ein Kohlenwasserstofftreibmittel und ein Arzneimittel zur Verabreichung auf eine Schleimhautoberfläche, werden inU.K. 2 082 457 U.S.P. 3 155 574 , Silson et al.,U.S.P. 5 011 678 , Wang et al. und von Parnell inU.S.P. 5 128 132 beschrieben. Es soll bemerkt werden, dass diese Referenzen die Bioverfügbarkeit von Lösungen eher durch Inhalation als durch die Membranen diskutieren, über welche sie verabreicht werden. - ZUSAMMENFASSUNG DER ERFINDUNG
- Es wurde nun ein bukkales Aerosolspray entwickelt, welches ein nicht-polares Lösungsmittel verwendet, welches biologisch aktive Verbindungen für schnelle Absorption durch die orale Schleimhaut zur Verfügung stellt, was in schnellem Beginn der Wirkung resultiert.
- Die erfindungsgemäßen bukkalen Aerosolspray-Zusammensetzungen für die transmukosale Verabreichung einer pharmakologisch aktiven Verbindung, die in einem pharmakologisch akzeptablen, nicht-polaren Lösungsmittel löslich ist, weist in Gewichts-% der Gesamtzusammensetzung auf: Pharmazeutisch akzeptables Treibmittel, welches ein C3-C8-Kohlenwasserstoff einer linearen oder verzeigten Konfiguration ist, 50 bis 95%, nicht-polares Lösungsmittel 5-50%, aktive Verbindung 0,0025-40%, geeigneterweise zusätzlich aufweisend bezüglich Gewicht der Gesamtzusammensetzung einen Aromastoff mit 0,05-5%. Vorzugsweise weist die Zusammensetzung auf: Treibmittel 55 bis 85%, nicht-polares Lösungsmittel 15-45%, aktive Verbindung 0,025-20%, Aromastoff 0,1-2,5%, am geeignetsten Treibmittel 60-80%, nicht-polares Lösungsmittel 19-32%, aktive Verbindung 0,125-12,5%, Aromastoff 0,1-3,0%.
- Es ist eine Aufgabe der Erfindung, die Schleimhautmembranen mit extrem feinen Tröpfchen eines Sprays zu überziehen, welches die aktiven Verbindungen enthält.
- Es ist auch eine weitere Aufgabe der Erfindung, einem Säugetier, welches diese benötigt, vorzugsweise Menschen, eine vorherbestimmte Menge einer biologisch aktiven Verbindung durch dieses Verfahren zu verabreichen.
- Ein weiterer Gegenstand ist ein verschlossener Aerosolspraybehälter, enthaltend eine Zusammensetzung der Sprayformulierung und ein Messventil, geeignet zum Freisetzen einer vorherbestimmten Menge der genannten Zusammensetzung aus dem genannten Behälter.
- Wenn das Treibmittel nach der Aktivierung des Aerosolventils verdampft, wird ein Nebel aus feinen Tröpfchen gebildet, der Lösungsmittel und aktive Verbindungen enthält.
- Das Treibmittel ist ein Nicht-Freon-Material, ein C3-C8-Kohlenwasserstoff einer linearen oder verzweigten Konfiguration. Das Treibmittel sollte geeigneterweise nicht-wässrig sein. Das Treibmittel erzeugt einen Druck in dem Aerosolbehälter, so dass es unter normal erwarteter Verwendung ausreichenden Druck erzeugt, um das Lösungsmittel aus dem Behälter auszutreiben, wenn das Ventil aktiviert wird, aber nicht übermäßigen Druck, so dass der Behälter oder die Ventildichtungen zerstört werden.
- Das Lösungsmittel ist ein nicht-polarer Kohlenwasserstoff, vorzugsweise ein C7-18-Kohlenwasserstoff einer linearen oder verzweigten Konfiguration, seine Alkohole, Fettsäureester und Triglyceride, wie z.B. Miglyol. Das Lösungsmittel muss die aktive Verbindung lösen und mit dem Treibmittel mischbar sein, d.h. Lösungsmittel und Treibmittel müssen bei 0 bis 40°C in einem Druckbereich von 1-3 atm eine einzelne Phase bilden.
- Die erfindungsgemäßen Sprayzusammensetzungen sind dazu gedacht, aus einem verschlossenen, unter Druck befindlichen Behälter verabreicht zu werden. Im Gegensatz zu einem Pumpspray, welches das Eintreten von Luft in den Behälter nach jeder Aktivierung ermöglicht, wird der erfindungsgemäße Aerosolbehälter zur Zeit der Herstellung verschlossen. Die Inhalte des Behälters werden durch Aktivieren eines Messventils freigesetzt, werden aber atmosphärischen Gasen mit jeder Aktivierung keinen Eintritt ermöglichen. Solche Behälter sind kommerziell erhältlich.
- KURZE BESCHREIBUNG DER ZEICHNUNG
- Die Figur ist ein schematisches Diagramm, welches die Routen der Absorption und Verarbeitung von pharmakologisch aktiven Substanzen in einem Säugetiersystem zeigt.
- BESCHREIBUNG DER BEVORZUGTEN AUSFÜHRUNGSFORMEN
- Die bevorzugten aktiven Verbindungen der vorliegenden Erfindung sind Nikotin und Piroxicam in ihrer nicht-ionisierten Form oder die freie Base der pharmazeutisch akzeptablen Salze davon (vorausgesetzt, dass diese bei den Aerosol- oder Sprayzusammensetzungen in dem Spraylösungsmittel löslich sind), sowie, wo angebracht, die Ester oder Triglyceride davon. Diese Verbindungen sind in den erfindungsgemäßen, nicht-polarem Lösungsmitteln in geeigneten Konzentrationen löslich. Diese Konzentrationen können weniger als die standardmäßig akzeptierte Dosis für diese Verbindungen sein, da es sich um eine erhöhte Absorption der Verbindungen durch die orale Schleimhaut handelt. Dieser Gesichtspunkt der Erfindung ist insbesondere wichtig, wenn ein großer (40-99,99%) First-pass-Effekt vorliegt.
- Als Treibmittel für die Sprays können Propan, n-Butan, Isobutan, n-Pentan, Isopentan und Neopentan, sowie Mischungen daraus verwendet werden. N-Butan und Isobutan als Einzelgase sind die bevorzugten Treibmittel. Es ist zulässig, dass das Treibmittel einen Wassergehalt von nicht mehr als 0,2%, typischerweise 0,1-0,2%, hat (alle Prozentangaben sind hier in Gewicht, sofern nicht anders angegeben). Es ist ebenfalls bevorzugt, dass das Treibmittel synthetisch hergestellt wird, um die Gegenwart von Verunreinigungen zu minimieren, die schädlich für die aktiven Verbindungen sind. Diese Verunreinigungen beinhalten Oxidationsmittel, Reduktionsmittel, Lewissäuren oder -basen und Wasser. Die Konzentration von jedem von diesen sollte weniger als 0,1% sein, mit Ausnahme von Wasser, dass so hoch wie 0,2% sein kann.
- Die bevorzugten Aromastoffe sind synthetische oder natürliche Öle von Pfefferminze, Öl von Spearmint, Zitrusöl, Fruchtaromen, Süßstoffe (Zucker, Aspartam, Saccharin, etc.), sowie Kombinationen davon.
- Die aktiven Substanzen beinhalten die aktiven Verbindungen, ausgewählt aus der Gruppe bestehend aus Alkaloiden und Analgetika.
- Nikotin ist ein Bestandteil von Tabakprodukten, der als süchtig machend angesehen wird. Raucher, die wünschen, mit dem Rauchen aufzuhören, haben ein doppeltes Problem. Das Erste sind die süchtig machenden Eigenschaften des Nikotins selbst. Das Zweite ist, dass die Abhängigkeit mit Rauchaktivitäten assoziiert ist, d.h. Paffen, Inhalieren, etc. Das erfindungsgemäße Spray trennt diese beiden Probleme. Indem Nikotin in einer Form dargereicht wird, die leicht absorbiert werden kann, ermöglicht das Spray dem Raucher, temporär mit der Nikotinverwendung fortzufahren, das Rauchen aber zu beenden. Wenn die Abhängigkeit des Rauchens einmal gestoppt ist, kann der vormalige Raucher dann von der Nikotinverwendung entwöhnt werden, wie z.B. durch weniger häufiger Verwendung und/oder durch Verwendung eines Sprays mit einer geringeren Konzentration. Vorteilhafterweise wird während dieser Kur der Nutzer keinem der Karzinogene, die im Tabakrauch vorhanden sind, ausgesetzt.
- Puroxicam (Feldene®, Pfizer) ist ein bekannter Entzündungshemmer.
- Die erfindungsgemäßen Formulierungen weisen eine aktive Verbindung oder ein pharmazeutisch akzeptables Salz davon auf. Die Bezeichnung „pharmazeutisch akzeptable Salze" betrifft Salze, die aus pharmazeutisch akzeptablen, nicht-toxischen Säuren oder Basen, einschließlich organischen und anorganischen Säuren oder Basen, hergestellt werden.
- Wenn eine erfindungsgemäße aktive Verbindung sauer ist, können Salze aus pharmazeutisch akzeptablen nicht-toxischen Basen hergestellt werden. Salze, die aus vollständig stabilen Formen von anorganischen Basen erhalten werden, beinhalten Aluminium, Ammonium, Kalzium, Kupfer, Eisen, Lithium, Magnesium, Mangan, Kalium, Natrium, Zink, etc. Besonders bevorzugt sind die Ammonium-, Kalzium-, Magnesium-, Kalium-, und Natriumsalze. Salze, die aus pharmazeutisch akzeptablen organischen, nicht-toxischen Basen erhalten werden, beinhalten Salze von primären, sekundären und tertiären Aminen, substituierten Aminen, einschließlich natürlich auftretenden substituierten Aminen, cyclischen Aminen und basischen Ionenaustauscherharzen, wie z.B. Arginin, Betain, Koffein, Cholin, N,N'-Dibenzylethylendiamin, Diethylamin, 2-Diethylaminoethanol, 2-Dimethylaminoethanol, Ethanolamin, Ethylendiamin, N-Ethylmorpholin, N-Ethylpiperidin, Glutamin, Glucosamin, Histidin, Isopropylamin, Lysin, Methylglucosamin, Morpholin, Piperazin, Piperidin, Polyaminharze, Procain, Purin, Theobromin, Triethylamin, Trimethylamin, Tripropylamin, etc.
- Wenn eine erfindungsgemäße aktive Verbindung basisch ist, können Salze aus pharmazeutisch akzeptablen, nicht-toxischen Säuren hergestellt werden. Solche Säuren beinhalten Essigsäure, Benzolsulfonsäure, Benzoesäure, Camphersulfonsäure, Zitronensäure, Ethansulfonsäure, Fumarsäure, Gluconsäure, Glutaminsäure, Bromwasserstoffsäure, Salzsäure, Isethionsäure, Milchsäure, Maleinsäure, Mandelsäure, Methansulfonsäure, Mucinsäure, Salpetersäure, Pamoasäure, Pantothensäure, Phosphorsäure, Bernsteinsäure, Schwefelsäure, Weinsäure, p-Toluolsulfonsäure, etc.
- Besonders bevorzugt sind Zitronensäure, Bromwasserstoffsäure, Maleinsäure, Phosphorsäure, Schwefelsäure und Weinsäure.
- Bei der Diskussion von Verfahren zur Behandlung wird hier Bezugnahme auf aktive Verbindungen so verstanden, dass auch die pharmazeutisch akzeptablen Salze davon eingeschlossen sind. Während gewisse Formulierungen hier ausgeführt sind, müssen die tatsächlichen Mengen, die dem dies benötigenden Säugetiere oder Mensch verabreicht werden, durch den behandelnden Mediziner bestimmt werden.
- Die Erfindung wird weiterhin durch Bezugnahme auf die folgenden Beispiele definiert, die dazu gedacht sind, zu veranschaulichen und nicht einzuschränken.
- BEISPIEL 2
- Nikotin-Spray
- Ein erfindungsgemäßes Spray weist die folgende Formulierung auf:

Claims (9)
- Bukkale Aerosol-Sprayzusammensetzung zur Verabreichung über die Schleimhaut einer in einem pharmakologisch brauchbaren, nicht-polaren Lösungsmittel löslichen pharmakologisch wirksamen Verbindung, umfassend in Gew.-% der Gesamtzusammensetzung: 50 bis 95% eines pharmakologisch brauchbaren Treibmittels, 5 bis 50% eines nicht-polaren Lösungsmittels und 0,0025 bis 40% eines Wirkstoffs, wobei das Treibmittel ein C3-C8-Kohlenwasserstoff einer gerad- oder verzweigtkettigen Anordnung ist, wobei der Wirkstoff ein Alkaloid oder ein Analgetikum ist.
- Zusammensetzung gemäß Anspruch 1, zusätzlich enthaltend 0,05 bis 5% eines Geschmackstoffs, bezogen auf das Gewicht der Zusammensetzung.
- Zusammensetzungen gemäß Anspruch 1, bei der das Treibmittel Propan, n-Butan, n-Pentan, Isopentan, neo-Pentan oder ein Gemisch derselben ist.
- Zusammensetzung gemäß Anspruch 1, bei das Treibmittel n-Butan oder Isobutan ist, bei der der Wassergehalt nicht mehr als 0,2%, bezogen auf die Zusammensetzung, ist und bei der der Gehalt an Oxidationsmitteln, Reduktionsmitteln und Lewis-Säuren oder -Basen jeweils kleiner 0,1%, bezogen auf die Zusammensetzung, ist.
- Zusammensetzung gemäß Anspruch 1, bei der das Lösungsmittel ein (C2-C24)-Fettsäure-(C2-C8)-Ester, ein C7-C18-Kohlenwasserstoff geradkettiger oder verzweigter Anordnung, ein Alkohol davon, ein C2-C6-Alkanoylester oder ein Triglycerid dieser Alkohole ist.
- Zusammensetzung gemäß Anspruch 2, bei der die Geschmacksmittel synthetisches oder natürliches Pfefferminzöl, Spearmintöl, Zitrusöl, Fruchtgeschmackstoffe und/oder Süßungsmittel sind.
- Zusammensetzung gemäß Anspruch 2, umfassend 55 bis 85% pharmakologisch brauchbares Treibmittel, 15 bis 40% nicht-polares Lösungsmittel und 0,025 bis 20% Wirkstoff und 0,1 bis 2,5% Geschmacksmittel.

- Verschlossener Aerosol-Spraybehälter, enthaltend eine Zusammensetzung gemäß Anspruch 1 und ein zur Freisetzung einer zuvor festgelegten Menge der Zusammensetzung aus dem Behälter geeignetes Dosierungsventil.
