- BEREICH DER ERFINDUNG
- Die Erfindung betrifft die Behandlung von Schizophrenie mit Fettsäure.
- STAND DER TECHNIK
- Die essentiellen Fettsäuren und ihre Umwandlungen im Körper werden in der folgenden Tabelle dargestellt.
- Die Säuren, die von Natur aus die all-cis-Konfiguration haben, werden systematisch als Derivate der entsprechenden Octadecan-, Eicosan- oder Docosansäuren bezeichnet, z. B. LA z,z-Octadeca-9,12-Diensäure oder DHA z,z,z,z,z,z-Docosa-4,7,10,13,16,19-Hexaensäure, allerdings sind numerische Kennzeichnungen auf der Basis der Zahl der Kohlenstoffatome, der Zahl der Ungesättigtheitsschwerpunkte und der Zahl der Kohlenstoffatome vom Ende der Kette, an dem die Ungesättigtheit beginnt, wie z. B. entsprechend 18 : 2 n-6 oder 22 : 6 n-3, geeignet. In einigen Fällen werden die Initialien, wie z. B. EPA, und Abkürzungen des Namens, wie z. B. Eicosapentaensäure, als Trivialnamen verwendet.
- In den vorherigen Patentanmeldungen EPA-0347856 und EPA-0599576 haben wir die Aufmerksamkeit auf die Wirkung verschiedener essentieller Fettsäuren bei Schizophrenie gelenkt und die Verwendung dieser Fettsäuren in der Behandlung beansprucht. Wir haben vor allem auf den niedrigen Erythrozytengehalt von Arachidonsäure (AA) und Docosahexaensäure (DHA) und die Verwendung dieser Fettsäuren aufmerksam gemacht.
- Vorliegende Arbeit
- Nun haben wir unerwarteterweise festgestellt, dass eine spezielle essentielle Fettsäure, der bisher nur eine kleine Rolle zuerkannt wurde, in der Behandlung in der Tat besonders wirksam ist. Hierbei handelt es sich um Eicosapentaensäure (EPA; 20 : 5n-3), die im Gehirn nur in geringen Mengen im Vergleich zu AA und DHA vorhanden ist. Im Rahmen eines Behandlungsversuchs haben wir allerdings festgestellt, dass EPA Schizophrenie außergewöhnlich wirksam behandelt. Besonders wirksam sind Präparate, die EPA in einer Menge von mehr als 20% der gesamten vorliegenden Fettsäuren (vorzugsweise der gesamten vorliegenden ungesättigten Fettsäuren), vorzugsweise von mehr als 40 umfassen, wobei mehr als 70% am meisten bevorzugt werden.
- Studien über die Verwendung von essentiellen Omega-3-Fettsäuren in der Behandlung, unter Verwendung eines Gemischs aus DHA und EPA, haben bisher mäßig vorteilhafte Wirkungen gezeigt (Mellor et al, Human Psychopharmacology 11: 39–46, 1996). Das verwendete Präparat enthielt 18% EPA und 12% DHA, und soweit es diese Offenbarung betrifft, könnten die therapeutischen Wirkungen durch eine der Komponenten oder beide verursacht worden sein. Eine Analyse der Beziehung zwischen Fettsäureveränderung und Schizophreniesymptomen hat ergeben, dass ein Anstieg des Omega-3-Gesamtgehalts an Erythrozytenmembranen mit einem Rückgang der Schizophreniesymptome verbunden war.
- Wir beschlossen daher zu versuchen, die relative Bedeutung von EPA und DHA bei Schizophrenie zu bestimmen. Es wurde eine Studie mit 30 Schizophreniepatienten durchgeführt, von denen die meisten sowohl positive als auch negative Symptome hatten, wie anhand der Skala positiver und negativer Symptome (PANSS) gezeigt wird, und außerdem gewisse Anzeichen für eine tardive Dyskinesie aufwiesen, wie anhand der Skala abnormaler unwillkürlicher Bewegungen (AIMS) gezeigt wird. Die Patienten wurden auf Doppelblindbasis zufällig den folgenden Behandlungen zugeordnet: 20 ml Plazeboemulsion, 20 ml einer 40%igen Emulsion, die 8 g Öl mit etwa 2,0 g EPA und 0,4 g DHA pro Tag bereitstellte ('EPA-Gruppe'), und 20 ml einer Emulsion, die etwa 2,3 g DHA und 0,5 g EPA pro Tag ('DHA'-Gruppe) in 8 g Öl bereitstellte. Die Patienten wurden auf Basislinie und nach 12 Behandlungswochen untersucht. Bei der Placebogruppe wiesen 9 Patienten in Bezug auf die PANSS- und AIMS-Werte keine Veränderungen auf oder verschlechterten sich; bei einem Patienten wurde eine Verbesserung verzeichnet. Bei der DHA-Gruppe wiesen sieben Patienten in Bezug auf die PANSS- und AIMS-Werte keine Veränderungen auf oder verschlechterten sich; drei Patienten zeigten Verbesserungen. Im Gegensatz dazu wies ein Patient der EPA-Gruppe keine Veränderung auf, allerdings wurden bei neun Patienten Verbesserungen verzeichnet. Die Verbesserungen traten sowohl bei den negativen als auch positiven Symptomen sowie hinsichtlich der AIMS-Werte auf. Es gab daher ein breites Verbesserungsspektrum in allen Aspekten des Schizophreniesyndroms. Die DHA-Gruppe unterschied sich nicht wesentlich von der Placebogruppe, wohingegen die EPA-Gruppe bedeutend besser als die DHA-Gruppe und die Placebogruppe abschnitt (p<0,02 in beiden Fällen).
- Man kann daher zu dem Schluss gelangen, dass der hauptsächliche therapeutische Behandlungseffekt mit EFAs dem Effet der EPA zugeschrieben werden kann. Die anderen EFAs tragen möglicherweise in gewissem Maße dazu bei, doch ist EPA zweifellos hauptsächlich für die positiven Wirkungen der Behandlung mit Omega-3-EFA-Präparaten verantwortlich. Wir folgerten, dass EPA auch bei anderen Geistesstörungen wie der Alzheimerschen Krankheit und Depression wirksam sein müsste, da im Blut und/oder Gehirn von Patienten mit diesen Krankheiten ebenfalls ein niedriges n-3-Fettsäure-Niveau festgestellt wird. Zwei Patienten mit stärker Depression, die nicht auf die üblichen Antidepressiva ansprachen, wurden daher mit der im Schizophrenieversuch verwendeten EPA-Formulierung behandelt. Innerhalb von 4 Wochen zeigte sich bei beiden eine bemerkenswerte Verbesserung der Symptome.
- Wir sind uns zwar bezüglich des Funktionsmechanismus von EPA nicht sicher, doch besteht eine Möglichkeit darin, dass sie das Enzym Phospholipase A2 hemmt. Es gibt beträchtliche Hinweise darauf, dass die Phospholipase-(PL)-A2-Aktivität bei Schizophrenie erhöht ist (Horrobin et al, Schizophrenia Research 1994; 13: 195–207). Es ist daher zu erwarten, dass Verbindungen, die PLA2 sicher hemmen, eine therapeutische Wirkung haben. In In-vitro-Studien der Wirkungen von Fettsäuren auf die PLA2-Aktivität haben wir festgestellt, dass EPA ein starker Inhibitor ist, was auf die relativ ähnliche Fettsäure DHA nicht zutrifft. Dies ist möglicherweise eine Erklärung dafür, dass sich DHA in den klinischen Studien nicht als wirksam erwies.
- Wir haben ferner gefunden, dass eine weitere n-3-Fettsäure, Stearidonsäure (18 : 4 n-3), genauso wirksam PLA2 hemmt wie EPA. EPA und SA sind 20- und 18-Kohlenstoff-Fettsäuren, die in der Tat nachweislich die Aktivität von Phospholipase A2 hemmen (Finnen, Biochem Soc. Trans 1991; 19 : 915). Im Gegensatz dazu ist DHA eine 22-Kohlenstoff-Fettsäure, die wie andere 22-Kohlenstoff-Säuren Phospholipase nicht hemmt. Dies ist möglicherweise eine Erklärung für die Unterschiede zwischen EPA und DHA, da es Hinweise auf eine Überaktivität von Phospholipase A2 bei Schizophrenie gibt. Wir schlagen daher vor, dass Stearidonsäure wirksam Schizophrenie und die anderen Krankheiten behandelt, und schließen ihre Verwendung für diesen Zweck alleine oder mit EPA ein.
- Die n-6 EFAs wie Linolsäure, Gammalinolensäure (GLA), Dihomogammalinolensäure (DGLA) und Arachidonsäure (AA) sind in der Gehirnstruktur von Bedeutung. GLA kann zu DGLA und AA metabolisiert werden. wir versuchten daher festzustellen, ob die Verabreichung eines Öls, das Linolsäure und GLA enthält, an zwei Personen, die auf EPA angesprochen hatten, von Vorteil sein würde. Tatsächlich schien ihre Verfassung mit der Verabreichung der n-6 EFAs weniger gut zu sein. Eine Behandlung mit EPA-Präparaten, in denen der Gehalt von n-6 EFAs im Vergleich zur EPA-Konzentratian gering bleibt, ist daher besser. n-6 EFAs sollten entweder nicht vorhanden sein, oder wenn doch, in einem Verhältnis zur EPA von nicht mehr als 1 : 3, vorzugsweise 1 : 4 oder weniger.
- Zur Gewährleistung, dass den erzielten Effekten nicht entgegengewirkt wird, liegt das Gewichtsverhältnis zwischen SA/EPA und eventuell vorliegender DHA in jedem Fall vorteilhafterweise nicht unter 3 : 1 nach Gewicht und vorteilhafterweise bei 4 : 1 oder darüber.
- Die Aussage der Erfindung
- Die Erfindung ist in den beiliegenden Ansprüchen dargelegt, stellt jedoch inter alia ein pharmazeutisches Präparat mit einem Öl bereit, umfassend Stearidonsäure (SA) oder eine Kombination aus Eicosapentaensäure (EPA) und Stearidonsäure (SA) in einer Menge von mehr als 20%, vorzugsweise mehr als 40% und am bevorzugtesten von mehr als 70 Gew.-% der gesamten (vorzugsweise gesamten ungesättigten) vorhandenen Fettsäuren, wobei das Gewichtsverhältnis zwischen SA (SA + EPA) und vorhandenen n-6 EFAS nicht unter 3 : 1 liegt und vorzugsweise 4 : 1 oder mehr beträgt, oder n-6 EFAs nicht vorliegen. Verfahren zur Herstellung von Medikamenten, in denen solche Öle verwendet werden, liegen ebenfalls innerhalb der Erfindung, die zur Behandlung oder Prophylaxe von Schizophrenie, tardiver Dyskinesie, Depression oder der Alzheimerschen Krankheit oder anderer Demenzen eingesetzt werden.
- Die Verwendung von EPA-reichem Öl, wie in den Ansprüchen dargelegt, zur Herstellung eines Medikamentes zur Behandlung oder Prophylaxe dieser Erkrankungen ist eine Ausgestaltung der Erfindung.
- Die EPA kann in jeder angemessenen Art und Weise bereitgestellt werden, die die EPA-Werte im Blut erhöht. Mono-, Di- und Tri-Glyceride, Mono- oder Diester, Salze, Cholesterolester, Amide, Phospholipide, freie Säuren oder. jede andere geeignete Form kann zur Zuführung der EPA verwendet werden. Mono- oder Diester von EPA, die in vorherigen Anmeldungen aufgeführt sind, bei denen der Autor. der vorliegenden Erfindung ein Miterfinder ist (PCT/GB96/01052 und 01053), die jeweils als WO96/34855 und W096/34846 veröffentlicht wurden, sind besonders praktische Formen, in denen die EPA verabreicht werden kann. Die EPA kann von Fisch- oder Merressäugerrölen, mikrobiellen Ölen oder sogar von der chemischen Totalsynthese abstammen. Die verwendete EPA-Dosis kann von 10 mg bis 100 g/Tag, vorzugsweise 100 mg bis 20 g/Tag und am bevorzugtesten von 500 mg bis 10 g/Tag reichen. Es kann der orale, enterale, parenterale, topische oder irgendein anderer geeigneter Verabreichungsweg genutzt werden. Stearidonsäure kann in ähnlichen Dosen und in ähnlicher Weise alleine oder mit der EPA bereitgestellt werden.
- Beispiele
- Die oben beschriebene Studie illustriert eine erfindungsgemäße Behandlung; als Beispiele für geeignete Formulierungen können jedoch die Folgenden angeführt werden:

- 1. weiche Gelatinekapseln, die jeweils 200 mg EPA in einer der folgenden Formen enthalten:
- a) Ein Öl mit 22% EPA von marinen oder mikrobiellen Quellen.
- b) Ein Öl mit 56% EPA von marinen oder mikrobiellen Quellen.
- c) Ein Öl mit 75% EPA von marinen oder mikrobiellen Quellen.
- d) Ein Öl mit 95% EPA von marinen, mikrobiellen oder synthetischen Quellen.
- e) EPA 1–3 Propandiol-Diester.
- 2. Öle wie in den Beispielen 1a bis 1e, die jedoch als Emulsion zur oralen Verabreichung formuliert sind. Die Emulsion kann 5 bis 50% des Öls enthalten; das emulgiert wird mit Emulgatoren, die der fachkundigen Person bekannt sind, einschließlich natürlicher, synthetischer und halbsynthetischer Mittel wie Phospholipide und Galactolipide, wobei Letztere zum Beispiel in der PCT SE95/00115, veröffentlicht als W095/20943, beschrieben sind.
- 3. Emulsionen wie unter 2., die jedoch sterilisiert und für eine intravenöse Verabreichung angemessen formuliert sind.
- 4. Öle wie in den Beispielen 1a bis 1e, die sterilisiert und zur intramuskulären oder subkutanen Injektion formuliert sind.
- 5. Öle wie in den Beispielen 1a bis 1e, die zur topischen Verabreichung unter Anwendung von Patch- oder anderen Technologien, die der fachkundigen Person bekannt sind, formuliert sind. 6–10. Wie die Beispiele 1–5, mit der Ausnahme, dass Stearidonsäure anstelle von EPA oder in Beimengung mit ihr verwendet wird, z. B. jeweils zur Hälfte.
Claims (26)
- Verwendung eines Öls, umfassend Eicosapentaensäure (EPA) und/oder Stearidonsäure (SA) in einer Menge von mehr als 20 Gew.-% der gesamten Fettsäuren, die in der Herstellung eines Medikamentes zur Behandlung oder Prophylaxe von Schizophrenie, tardiver Dyskinesie, Depression, der Alzheimerschen Krankheit oder anderen Demenzen vorliegen, wobei das Gewichtsverhältnis zwischen SA/EPA und irgendeiner vorhandenen n-6 EFA nicht unter 3 : 1 liegt oder n-6 EFAs nicht vorliegen.
- Verwendung nach Anspruch 1, wobei die EPA und/oder SA in einer Menge. von mehr als 20 Gew.-% der gesamten vorhandenen ungesättigten Fettsäuren vorliegen.
- Verwendung nach Anspruch 1, wobei die EPA und/oder SA in einer Menge von mehr als 40 Gew.-% der gesamten vorhandenen Fettsäuren vorliegen.
- Verwendung nach Anspruch 1, wobei die EPA und/oder SA in einer Menge von mehr als 40 Gew.-% der gesamten vorhandenen ungesättigten Fettsäuren vorliegen.

- Pharmazeutisches Präparat, umfassend ein Öl, das Stearidonsäure (SA) oder Stearidonsäure in Kombination mit Eicosapentaensäure (EPA) in einer Menge von mehr als 20 Gew.-% der gesamten vorhandenen Fettsäuren umfasst, wobei das Gewichtsverhältnis zwischen SA oder SA in Kombination mit EPA und einer vorhandenen n-6 EFA nicht unter 3 : 1 liegt oder n-6 EFAs nicht vorliegen.
- Pharmazeutisches Präparat nach Anspruch 14, wobei SA oder SA in Kombination mit EPA in einer Menge von mehr als 20 Gew.-% der gesamten vorhandenen ungesättigten Fettsäuren vorliegt.
