Description
- Die Erfindung betrifft transdermale Präparate, die als Wirkstoff Levosimendan oder ein pharmazeutisch verträgliches Salz davon enthalten, und die Verwendung von Levosimendan oder eines pharmazeutisch verträglichen Salzes davon zur Herstellung eines Medikaments zur Verwendung in einem Verfahren zur Behandlung von Herzversagen durch transdermale Verabreichung.
- Levosimendan, bei dem es sich um das (-)-Enantiomer von [[4-(1,4,5,6-Tetrahydro-4-methyl-6-oxo-3-pyridazinyl)phenyl]hydrazono]propandinitril (I) handelt, und das Verfahren zu seiner Herstellung sind z. B. in der EP 565546 B1 beschrieben. Levosimendan wirkt bei der Behandlung von Herzversagen und weist eine signifikante calciumabhängige Bindung an Troponin auf. Die Verwendung von Levosimendan bei der Behandlung von Myocardischämie ist in der WO 93/21921 beschrieben. Die Pharmakokinetik von Levosimendan beim Menschen nach i.v. und oraler Dosierung wird bei Sandell, E.-P. et al., J. Cardivasc. Pharmacol., 26 (Suppl. 1), S. 57-S. 62, 1995, beschrieben.
- Es wurde nun gefunden, daß sich therapeutisch wirksame Levosimendan-Serumspiegel durch transdermale Verabreichung von Levosimendan erreichen lassen. Transdermale Abgabe von Levosimendan stellt eine sichere Methode zur Verabreichung von Levosimendan an einen Patienten mit einer gewünschten konstanten Rate dar.
- Die vorliegende Erfindung stellt ein transdermales Präparat bereit, das (-)-[[4-(1,4,5,6-Tetrahydro-4-methyl-6- oxo-3-pyridazinyl)phenyl]hydrazono]propandinitril oder ein pharmazeutisch verträgliches Salz davon als therapeutischen Wirkstoff umfaßt.
- Die vorliegende Erfindung stellt ferner die Verwendung von (-)-[[4-(1,4,5,6-Tetrahydro-4-methyl-6-oxo-3-pyridazinyl)phenyl]hydrazono]propandinitril oder eines pharmazeutisch verträglichen Salzes davon zur Herstellung eines Medikaments zur transdermalen Verabreichung von (-)-[[4- (1,4,5,6-Tetrahydro-4-methyl-6-oxo-3-pyridazinyl)phenyl]- hydrazono]propandinitril oder eines pharmazeutisch verträglichen Salzes davon bereit.
- Die transdermale Verabreichung von Levosimendan oder eines pharmazeutisch verträglichen Salzes davon kann beispielsweise erfolgen (i) durch Vermischen der therapeutisch wirksamen Verbindung oder eines pharmazeutisch verträglichen Salzes davon mit geeigneten pharmazeutisch verträglichen Trägerstoffen und gegebenenfalls Penetrationsvermittlern für die Haut zu Präparaten wie Salben, Emulsionen, Lotionen, Lösungen, Cremes, Gelen o. ä., wobei vorzugsweise eine festgelegte Menge des Präparats, das gegebenenfalls mit einer undurchlässigen Abdeckschicht abgedeckt ist, auf eine festgelegte Fläche der Haut aufgebracht wird, oder (ii) durch Einarbeiten der therapeutisch wirksamen Substanz in Pflaster oder transdermale Abgabesysteme nach bekannten Verfahren.
- Die Herstellung geeigneter transdermaler Abgabesysteme wird zum Beispiel in der WO 92/21334, der WO 92/21338 und der EP 413487 beschrieben. Ein solches System kann (a) eine für den Arzneistoff undurchlässige Abdeckschicht und (b) eine Adhäsivschicht, die den Verband an der Haut fixiert, umfassen, wobei der Arzneistoff in der Adhäsivschicht dispergiert ist. Alternativ kann das System (a) eine für den Arzneistoff undurchlässige Abdeckschicht, (b) eine Adhäsivschicht und (c) eine Matrixschicht, vorzugsweise aus einem Polymermaterial, in der der Arzneistoff dispergiert ist, umfassen. Die Geschwindigkeit, mit der der Arzneistoff aus der Vorrichtung freigesetzt wird, wird hierbei durch die Polymermatrix kontrolliert. Das System kann auch (a) eine für den Arzneistoff undurchlässige Abdeckschicht, (b) eine Adhäsivschicht, (c) eine für den Arzneistoff durchlässige Membran, die so an einer Seite der Abdeckschicht befestigt ist, daß dazwischen wenigstens ein Arzneistoffreservoirkompartiment definiert wird, und (d) einen Arzneistoff oder eine Zusammensetzung davon in dem Arzneistoffreservoir umfassen. In diesem Fall liegt der Arzneistoff in dem Reservoir üblicherweise in flüssiger oder in Gelform vor. Die für den Arzneistoff durchlässige Membran kontrolliert die Geschwindigkeit, mit der der Arzneistoff an die Haut abgegeben wird.
- Technisch bekannte iontophoretische transdermale Abgabesysteme können ebenfalls zur transdermalen Abgabe von Levosimendan verwendet werden. Der Ausdruck "Iontophorese" bedeutet die Verwendung eines schwachen elektrischen Stroms, um die transdermale Permeation geladener Arzneistoffe zu steigern. Ein Überblick über diese Methode wird z. B. in Burnette, R., Iontophoresis. In Transdermal Drug Delivery, Seiten 247-292, Eds. Guy, R., und Hadgraft, J., Marcel Dekker Inc., New York und Basel, 1989, gegeben. Iontophoretische transdermale Abgabesysteme umfassen üblicherweise eine erste (Donor)Elektrode, die eine elektrolytisch zugängliche wirksame Verbindung in einem geeigneten Vehikel oder Träger enthält, eine zweite (passive) Elektrode und eine Stromquelle, wobei die erste und zweite Elektrode mit der Stromquelle jeweils elektrisch leitend verbunden sind. Der Abstand zwischen der ersten und zweiten Elektrode wird für den physischen Kontakt mit der Haut entsprechend eingestellt, wodurch in Reaktion auf einen Strom, der von der Stromquelle durch die Elektroden geliefert wird, eine therapeutische Menge der wirksamen Verbindung durch die Haut an einen Patienten verabreicht wird.
- Geeignete pharmazeutische Trägerstoffe umfassen die auf dem Gebiet der Pharmazie zur Herstellung topischer Formulierungen bekannten Stoffe wie Glycole und Glycolether, Polyethylenglycol, Propylenglycol, Glycerin und Glycerinether, niedrige Alkohole wie Ethanol oder Isopropanol, Wässer, nichtflüchtige Fettalkohole, beispielsweise Cetostearylalkohol und Cetylalkohol, N-Methylpyrrolidon, pflanzliche und tierische Öle, Sesamöl, Olivenöl, Holzspiritussalben, Vaseline und Paraffin, Cellulosederivate, beispielsweise Methylcellulose, Hydroxypropylmethylcellulose (HPMC) oder Carboxymethylcellulose, oder Mischungen davon.
- Geeignete Penetrationsvermittler für die Haut umfassen die in der Technik bekannten Penetrationsvermittler, beispielsweise C&sub2;-C&sub4;-Alkohole wie Ethanol und Isopropanol; Tenside, beispielsweise anionische Tenside wie Salze von Fettsäuren mit 5 bis 30 Kohlenstoffatomen, z. B. Natriumlaurylsulfat und andere Sulfatsalze von Fettsäuren, kationische Tenside wie Alkylamine mit 8 bis 22 Kohlenstoffatomen, z. B. Oleylamin, und nicht-ionische Tenside wie Polysorbate und Poloxamere; aliphatische einwertige Alkohole mit 8 bis 22 Kohlenstoffatomen wie Decanol, Laurylalkohol, Myristylalkohol, Palmitylalkohol, Linolenylalkohol und Oleylalkohol; Fettsäuren mit 5 bis 30 Kohlenstoffatomen wie Ölsäure, Stearinsäure, Linolsäure, Palmitinsäure, Myristinsäure, Laurinsäure und Caprinsäure und deren Ester, wie beispielsweise Ethylcaprylat, Isopropylmyristat, Methyllaurat, Hexamethylenpalmitat, Glycerylmonolaurat, Polypropylenglycolmonolaurat und Polyethylenglycolmonolaurat; Salicylsäure und deren Derivate; Alkylmethylsulfoxide wie Decylmethylsulfoxid und Dimethylsulfoxid; 1-substituierte Azacycloalkan-2-one wie 1-Dodecylazacycloheptan-2-on, vertrieben unter dem Handelsnamen AZONE; Amide wie Octylamid, Ölsäureamid, Hexamethylenlauramid, Lauryldiethanolamid, Polyethylenglycol-3-lauramid, N,N-Diethyl-m-toluamid und Crotamiton; und beliebige andere Verbindungen, die mit Levosimendan und den Verpackungen kompatibel sind und eine die transdermale Permeation fördernde Wirkung besitzen.
- Das erfindungsgemäße Präparat kann auch ein antimikrobielles Mittel, ein Konservierungsmittel, ein Antioxidans und ein pH-regulierendes Mittel und andere auf diesem Gebiet bekannte Zusatzstoffe enthalten.
- Bevorzugte Verabreichungsraten von Levosimendan bei der transdermalen Abgabe liegen im Bereich von ungefähr 1-1000 ug/h durch eine Hautfläche von ungefähr 2-100 cm². Die tägliche Dosis Levosimendan beim Menschen liegt im Bereich von ungefähr 0,1 bis 500 mg, vorzugsweise 0,5 bis 10 mg, abhängig von Alter, Körpergewicht und Allgemeinzustand des Patienten. Die Arzneistoffmenge, die in die Haut abgegeben wird, läßt sich durch eine Reihe von Faktoren steuern, einschließlich Größe des Hautflecks, Dichte der Arzneistoffbeladung, Verwendung geschwindigkeitskontrollierender Membranen, Permeationsvermittlern usw.
- Die transdermale Penetration von Levosimendan in vitro durch menschliche Haut wurde mit "Side-by-Side"- Diffusionskammern (DC-100, Crown Glass Co., Somerville, NJ) bei 25ºC untersucht. Die Akzeptorphase (3 ml) bestand aus bloßem Natriumphosphatpuffer (pH 5,0). Jede untersuchte gesättigte Levosimendanlösung (3 ml) wurde auf die Donorseite gegeben. Die Levosimendanpermeation aus der Gelformulierung wurde mit Franz'schen Diffusionskammern (DC-400, Crown Glass. Co., Somerville, NJ) bei 25ºC untersucht. Das Volumen von Donor- und Akzeptorphase betrug 1 ml bzw. 5 ml. Es wurden 72 Std. lang in festen Abständen Proben entnommen und die Levosimendankonzentration in den Proben wurde durch HPLC (Beckman System Gold, Beckman Instruments Inc., CA) bestimmt. Die verwendete HPLC-Säule war LiChrosorb RP-18 (7 um, 250 mm · 4 mm), und die mobile Phase bestand aus 55% Methanol und 45% Natriumphosphatpuffer mit pH 2,1. Die Detektionswellenlänge betrug 360 nm und die Durchflußgeschwindigkeit betrug 1,2 ml pro Min. Der transdermale Fluß von Levosimendan (ug/h pro cm²) durch die Haut wurde durch lineare Regression des geradlinigen Teils des Graphen von Arzneistoffpermeation gegen die Zeit und Dividieren durch die Hautoberfläche (0,64 cm²) berechnet.
- Die Gelformulierung von Levosimendan wurde aus Hydroxypropylmethylcellulose (HPMC) (2,5% m/V, mittleres MG 100.000; Aldrich Chemical Co., Steinheim, Deutschland) hergestellt, und zu dem Gel wurden Ethanol (10%) und Natriumlaurylsulfat (0,01%) gegeben. Die Permeationsresultate zeigen, daß die Permeation von Levosimendan in vitro durch die menschliche Haut aus der Lösung und aus dem aus derselben Lösung hergestellten Gel praktisch identisch war.

- Die Elektroden für die Iontophorese wurden aus Silberdraht und Silberchlorid (Aldrich-Chemie, Steinheim, Deutschland) hergestellt. Gleichstrom (0,5 mA/cm²) von den Elektroden zu den Diffusionskammern über Salzbrücken wurde während der lontophorese von einer HP 6181 DC- Stromquelle (Hewlett-Packard, CA) geliefert. Salzbrücken wurden durch Einspritzen eines 1 M NaCl-Gels (3% Agar) in Plastikröhrchen (Durchmesser 4 mm, Länge 15 cm) hergestellt. Die Salzbrücken verhinderten einen direkten Kontakt und mögliche Reaktionen von Levosimendan mit den Ag/AgCl-Elektroden. Als Akzeptorphase wurde HEPES-Puffer mit pH 7,4 verwendet. Auf die Donorseite wurde gesättigte Levosimendanlösung (3 ml) in HEPES-Puffer (pH 7,4) gegeben. Die AgCl-Kathode wurde über die Salzbrücke mit der Donorlösung verbunden. Die positive Silberanode wurde über die Salzbrücke mit der Akzeptorlösung verbunden. Die Kammern wurden in Reihe geschaltet, da Gleichstrom verwendet wurde. Die Ag/AgCl-Elektroden konnten kontinuierlich über 12 Std. verwendet werden. Die Resultate dieser Versuche sind in Tabelle 2 zusammengefaßt. TABELLE 2. Claims (9)
1. Transdermales
Präparat, umfassend als therapeutischen
Wirkstoff (-)-[[4-(1,4,5,6-Tetrahydro-4-methyl-6-oxo-
3-pyridazinyl)phenyl]hydrazono]propandinitril oder
ein pharmazeutisch verträgliches Salz davon.
2. Präparat nach Anspruch 1, das in Form einer Salbe,
einer Emulsion, einer Lotion, einer Lösung, eines
Gels, einer Creme oder eines Pflasters vorliegt.
3. Präparat nach Anspruch 1, worin das Präparat ein
transdermales Abgabesystem ist, das eine für den
Arzneistoff undurchlässige Abdeckschicht und eine
Adhäsivschicht umfaßt, wobei der Wirkstoff in der
Adhäsivschicht dispergiert ist.
4. Präparat nach Anspruch 1, worin das Präparat ein
transdermales Abgabesystem ist, das eine für den
Arzneistoff undurchlässige Abdeckschicht; eine
Adhäsivschicht und eine Matrixschicht umfaßt, worin der
Wirkstoff dispergiert ist.
5. Präparat nach Anspruch 4, worin die Matrixschicht aus
einem Polymermaterial hergestellt ist.
6. Präparat nach Anspruch 1, worin das Präparat ein
transdermales Abgabesystem ist, das eine für den
Arzneistoff undurchlässige Abdeckschicht; eine
Adhäsivschicht; , eine für den Arzneistoff durchlässige
Membran, die relativ zur Abdeckschicht so angeordnet
ist, daß zwischen der Membran und der Abdeckschicht
wenigstens ein Arzneistoffreservoirkompartiment
definiert wird, und den Wirkstoff oder eine
Zusammensetzung davon im Arzneistoffreservoir umfaßt.
7. Präparat nach Anspruch 1, worin das Präparat ein
iontophoretisches transdermales Abgabesystem ist.
8. Präparat nach irgendeinem der Ansprüche 1-7 zur
Verwendung bei der Behandlung von Herzversagen.
9. Verwendung von (-)-[[4-(1,4,5,6-Tetrahydro-4-methyl-
6-oxo-3-pyridazinyl)phenyl]hydrazono]propandinitril
oder einem pharmazeutisch verträglichen Salz davon
zur Herstellung eines Medikaments zur transdermalen
Verabreichung von (-)-[[4-(1,4,5,6-Tetrahydro-4-
methyl-6-oxo-3-pyridazinyl)phenyl]hydrazono]propandinitril oder eines pharmazeutisch verträglichen Salzes
davon.
