Description
- Die Erfindung betrifft Anker für chirurgisches Nahtmaterial und spezieller betrifft sie kleine, langgestreckte Strukturen, die an einem Nahtmaterial befestigt werden und die der Länge nach durch ein Loch in Körpergewebe eingeführt werden und die dadurch eingesetzt werden, daß die Struktur nach dem Einführen im wesentlichen quer zum Nahtmaterial verdreht wird. Die Nahtmaterialanker können dazu verwendet werden, zwei oder mehr Teile von Körpergewebe zur Wiederverbindung fest zusammenzuhalten.
- Nahtmaterialanker wurden für verankernde Nahten während Arthroseoperationen mit Zugriff nur von einer Seite entwikkelt. Derartige Nahtmaterialanker werden im allgemeinen mit Spezialwerkzeugen zum Anbringen des Nahtmaterialankers eingesetzt, mit Handhabung von einer Seite des Körperteils her nach der Einführung.
- Ein Beispiel für eine solche Nahtmaterial-Verankerungseinrichtung ist im US-Patent Nr. 4,669,473 für Richards et al offenbart, die auf den Rechtsnachfolger der vorliegenden Erfindung übertragen ist. Das Patent für Richards offenbart eine chirurgische Befestigungseinrichtung mit einem Kopfteil und einem Faserteil, die normalerweise "T-förmig" angeordnet sind. Der Kopfteil verfügt über mindestens ein spitzes Ende, so daß dann, wenn der Kopfteil im Körpergewebe implantiert wird, dieser sich selbst fest am Körpergewebe hält. Die chirurgische Befestigungseinrichtung wird durch ein Werkzeug mit einem hohlen Schaft eingesetzt, durch den der Kopf der T-förmigen Befestigungseinrichtung durch einen Stößel in seine Position gedrückt wird. Die mit dem Kopf verbundene Faser wird mit dem Kopf durch einen Schlitz im Schaft eingezogen. Die Nachgiebigkeit der Befestigungseinrichtung bewirkt, daß diese zu ihrer normalen T-Form zurückkehrt, wobei der Kopf im allgemeinen quer oder unter einem spitzen Winkel zur Faser angeordnet wird. Bei dieser Ausrichtung plazieren die spitzen Enden des Kopfs der Befestigungseinrichtung sich selbst im umgebenden Gewebe. Dann wird das Werkzeug entfernt, wobei das freie Ende der Faser zurückbleibt, die sich aus dem Gewebe heraus erstreckt, um dann vom Chirurgen verwendet werden zu können.
- Eine weitere Vorgehensweise zum Schaffen eines Nahtmaterialankers, der hinter einem Knorpel verankert werden soll, ist im US-Patent Nr. 4,741,330 für Hayhurst offenbart. Der Nahtmaterialanker gemäß Hayhurst weist ebenfalls T-Form auf und er wird zur Implantation so eingesetzt, daß die Enden des Kopfs übereinander gelegt werden und von der Faser weg zeigen. Nach dem Einführen neigen die Enden des Kopfs aufgrund ihrer natürlichen Elastizität dazu, in ihre normale Ausrichtung rechtwinklig zur Faser zurückzukehren. Alternativ kann der T-förmige Anker so eingesetzt werden, daß die Faser parallel zum Kopf umgelegt ist und er durch ein langgestrecktes rohrförmiges Einführwerkzeug an seine Position gedrückt wird.
- Während es sich gezeigt hat, daß die vorstehend genannten chirurgischen Befestigungseinrichtungen zuverlässig und wirkungsvoll sind, ergaben sich verschiedene Nachteile, denen sich die Erfindung zuwendet. Z.B. muß der Durchmesser der Öffnung im Körper oder Gewebe, die zum Einführen bei jeder Einrichtung gemäß den vorstehend genannten Literaturstellen erforderlich ist, größer sein als der kombinierte Durchmesser aus dem Kopf des T-förmigen Ankers und der Faser. Auch ist der Anker gemäß Richards einstückig gegossen und beinhaltet eine eingegossene Faser, die schwerer abzubinden sein kann als herkömmliches Nahtmaterial. Beim Anker gemäß Hayhurst erfordert das Verbinden des Nahtmaterials mit dem Kunststoffanker manchmal verwickelte Prozesse bei der Herstellung und Qualitätskontrolle.
- Bei Abläufen in Verbindung mit Arthrose begrenzt der kleine zur Operation verfügbare Raum die Größe verwendbarer Instrumente. Im allgemeinen sind derartige Instrumente dann, wenn sie kleiner sind, praxisgerechter und sie können wirkungsvoller und leichter verwendet werden. Auf ähnliche Weise wird der Patient, je kleiner das zum Einführen chirurgischer Instrumente erforderliche Loch ist, durch die Operation umso weniger verletzt und umso weniger Zeit benötigt die Operationsstelle zum Verheilen.
- Der erfindungsgemäße Nahtmaterialanker minimiert diese und andere Schwierigkeiten, wenn er angebracht wird.
- Demgemäß ist es eine Hauptaufgabe der Erfindung, einen verbesserten, einseitigen Anker zu schaffen, der den Durchmesser der Einführöffnung minimiert. Eine andere Aufgabe ist es, einen verbesserten Anker zu schaffen, der einfach und billig hergestellt werden kann.
- Eine weitere Aufgabe der Erfindung ist es, einen verbesserten T-förmigen Nahtmaterialanker zu schaffen.
- Eine andere Aufgabe der Erfindung ist es, einen verbesserten T-förmigen Nahtmaterialanker zu schaffen, der eine festere Verbindung zwischen dem Nahtmaterial und dem Anker aufweist und der die volle Festigkeit des Nahtmaterials nutzt.
- Eine zusätzliche Aufgabe der Erfindung ist es, eine T-förmige chirurgische Nahtmaterial-Befestigungseinrichtung zu schaffen, die am Nahtmaterial eine Halteeinrichtung aufweist, die zu einem Einrastteil paßt, um dadurch dazu in der Lage zu sein, Gewebeteile sicher zusammenzuhalten.
- Diese und andere Aufgaben der Erfindung werden durch einen neuartigen Nahtmaterialanker gelöst, wie er unten beschrieben wird. Ein Nahtmaterialanker und Nahtmaterial in Kombination werden geschaffen, die ein Nahtmaterial und einen langgestreckten, rohrförmigen Anker mit einem Schlitz, der sich über ein Teilstück vom Hinterende zum Vorderende hin erstreckt, aufweisen. Das Nahtmaterial ist nahe seinem Abschlußende mit dem Knoten, der am Vorderende des Ankers angreift, verknotet oder auf andere Weise verbunden ist. Das Nahtmaterial ist ausgehend vom Vorderende zum Hinterende axial durch den Anker gezogen, um ein dünnes, langgestrecktes Paket zum Einführen zu bilden. Das Nahtmaterial verschiebt sich während des Einsetzens in eine Position, in der sich das Nahtmaterial ausgehend von einem mittleren Punkt am Anker erstreckt entlang des Schlitzes, um eine im wesentlichen T-förmige chirurgische Nahtmaterial-Befestigungseinrichtung zu bilden.
- Gemäß einer anderen Erscheinungsform der Erfindung weist ein Nahtmaterialanker zum Verankern von Nahtmaterial an Gewebe ein langgestrecktes Rohr mit einem Vorderende und einem Hinterende auf. Das Rohr verfügt über einen Schlitz, der sich linear vom Hinterende des Rohrs vorzugsweise bis ungefähr zur Hälfte zum Vorderende des Rohrs hin erstreckt. Das Nahtmaterial ist am Vorderende des Rohrs befestigt. Der Anker ist zwischen einer ersten Position, in der sich das Nahtmaterial vollständig durch das Rohr erstreckt, und einer zweiten Position verdrehbar, in der sich das Nahtmaterial aus dem Vorderende des Rohrs erstreckt und aus diesem durch den Schlitz austritt.
- Gemäß einer anderen Erscheinungsform der Erfindung legen die Enden des rohrförmigen Teils vorzugsweise ein Vorder- und ein Hinterende fest, die unter schiefen Winkeln zur Mittelachse des Rohrs angeordnet sind. Das Vorderende bildet eine zugespitzte Struktur, die beim Einführen hilft. Der schiefe Winkel des Hinterendes unterstützt das Verdrehen des Nahtmaterialankers in seine Kipposition, in der er eine T-förmige Verankerungsstruktur bildet.
- Der Schlitz erstreckt sich linear vom Hinterende des Kopfs aus und beginnt vorzugsweise am axial innersten Punkt des Hinterendes (d.h. dem Punkt des Hinterendes, der am dichtesten beim Mittelpunkt des Ankers liegt).
- Der Nahtmaterialanker und das Nahtmaterial können vor der Ausgabe in den Operationsraum zusammengesetzt werden, oder sie können vom Chirurgen vor dem Einführen in einen Patienten zusammengesetzt werden.
- Gemäß dem Verfahren zum Herstellen des Nahtmaterialankers wird ein T-förmiger Anker unter Verwendung eines kleinen, langgestreckten, rohrförmigen Teils mit einem Vorder- und einem Hinterende hergestellt. Im rohrförmigen Teil wird ein Schlitz ausgebildet, der sich vom Hinterende zu einem mittleren Punkt zwischen den zwei Enden erstreckt. Am Vorderende des rohrförmigen Teils wird Nahtmaterial befestigt und durch das rohrförmige Teil zum Hinterende durchgezogen.
- Das Verfahren zum Einsetzen des Nahtmaterialankers weist die Schritte des Einsetzens des Ankers unter Verwendung eines hohlen Werkzeugs so, daß das erste Ende des Ankers anfangs durch das Werkzeug in (oder durch) das Körpergewebe bis in eine gewünschte Tiefe, im allgemeinen hinter einem Knorpel, eingesetzt wird, auf. Im eingesetzten Zustand erstreckt sich das Nahtmaterial, das am ersten Ende bes rohrförmigen Teils befestigt ist, durch die Länge des Ankers hindurch. Nachdem der Anker durch das Ende des Werkzeugs ausgestoßen ist, wird auf das Nahtmaterial Zug ausgeübt, was bewirkt, daß der Anker am Gewebe angreift und sich in eine Kipposition verdreht, bei der das Nahtmaterial durch den Schlitz herausgezogen ist.
- Der erfindungsgemäße Nahtmaterialanker kann auch mit einer Nahtmaterial-Einrasteinrichtung am Nahtmaterial verwendet werden, um zwei oder mehr Körpergewebeteile sicher zusammenzuhalten.

- Fig. 6 veranschaulicht eine Verwendung des erfindungsgemässen Nahtmaterialankers mit Nahtmaterial mit einer daran vorhandenen Einrasteinrichtung;
- Fig. 7 zeigt die Verwendung der Erfindung von Fig. 6 beim sicheren Zusammenhalten zweier Körpergewebeteile;

- Es wird nun auf Fig. 1 Bezug genommen, in der ein Anker 10 gemäß einem Ausführungsbeispiel der Erfindung dargestellt ist. Der Anker 10 besteht aus einem Rohr 12 mit einem Vorderende 14 und einem Hinterende 16 an entgegengesetzten Enden des Rohrs 12. Ein Schlitz 18 erstreckt sich vom Hinterende 16 zu einem Punkt zwischen dem Vorderende 14 und dem Hinterende 16. Der Schlitz 18 erstreckt sich vorzugsweise bis ungefähr zum Mittelpunkt des Rohrs 12.
- Die Enden 14 und 16 des Nahtmaterialankers sind abgeschrägt, wie es dargestellt ist. Das Vorderende 14 ist so abgeschrägt, daß eine Spitze 15 das Einführen des Ankers durch Körpergewebe unterstützt. Die Hinterkante 16 ist so abgeschrägt, daß sie das Verdrehen des Ankers unterstützt, wenn dieser einmal positioniert ist, und sie dazu beiträgt, das Nahtmaterial in den Schlitz 18 zu führen und in diesem anzuordnen. Die Kante 17, die der axial äußerste Punkt des Hinterendes des Ankers ist (d.h. des Endes des Ankers, das weiter von seinem Mittelpunkt entfernt ist) ist so ausgebildet, daß sie sich in das Gewebe eingräbt, wodurch sie es ermöglicht, daß Zug auf das Nahtmaterial den Anker dazu veranlaßt, in T-Form zu verkippen oder sich dahin zu verdrehen. Der Schlitz 18 ist vorzugsweise so positioniert, daß er zum axial innersten Punkt 19 des Ankers geöffnet ist (d.h. der Kante des schrägen Endes 16, die dem Mittelpunkt des Ankers am nächsten liegt). Dies ermöglicht es, daß das schräge Ende 16 das Nahtmaterial zum und in den Schlitz 18 leitet, wenn der Anker verdreht wird.
- Obwohl es bevorzugt ist, daß sich der Schlitz 18 ungefähr über die Hälfte der Länge des Rohrs 12 erstreckt (um im wesentlichen gleiche oder entsprechende Verankerungsflächen an entgegengesetzten Seiten des Nahtmaterials zu schaffen), ist es innerhalb des Schutzbereichs der Erfindung möglich, daß sich der Schlitz über weniger oder mehr als einen solchen Weg erstreckt, solange er auf entsprechende Weise arbeitet und ausreichende Verankerungseffekte bereitstellt. Selbstverständlich kann es das Rohr auf unzufriedenstellende Weise schwächen, wenn sich der Schlitz zu weit zum Vorderende hin erstreckt, und ein Erstrecken des Schlitzes über keine ausreichende Länge kann verhindern, daß sich das Rohr in eine zufriedenstellende Verankerungsposition verdreht.
- In den Fig. 2 und 3 ist der Anker 10 mit einem an ihm befestigten Nahtmaterial 20 dargestellt. In Fig. 3 ist das Nahtmaterial 20 mit durchgezogenen Linien in Einführungsposition dargestellt, und es ist in seiner teilweise eingeführten Position mit gestrichelten Linien dargestellt und in seiner eingesetzten oder verkippten Position mit durchgezogenen Linien dargestellt. Das Nahtmaterial 20 ist durch einen Knoten 22 am Anker 10 befestigt. Der Knoten 22 ist ausreichend groß dafür, daß er ein Hindurchschlüpfen des Nahtmaterials 20 durch das Rohr 12 verhindert. Der Knoten 22 ist am Abschlußende 24 des Nahtmaterials 20 ausgebildet.
- Es ist zu beachten, daß andere Maßnahmen zum Befestigen des Nahtmaterials am Anker 10 möglich sind, wozu Verkerben durch Wärme, Verkleben, Ultraschallverschweißen, Anstecken oder andere ähnliche Maßnahmen zum Befestigen des Nahtmaterials 20 am Anker 10 gehören.
- In Fig. 2 ist das Einführen des Ankers 10 und des Nahtmaterials 20 in drei Phasen veranschaulicht. Das Einführen des Ankers wird durch ein (bruchteilhaft dargestelltes) rohrförmiges Einführwerkzeug 28 ausgeführt. Nach dem anfänglichen Einführen werden der Anker 10 und das Nahtmaterial 20 von einer hohlen Außennadel 30 des Einführwerkzeugs 28 eingeschlossen. Das Nahtmaterial 20 erstreckt sich vom am Vorderende 14 des Rohrs 12 ausgebildeten Knoten 22 aus durch das Hinterende 16 des Rohrs. Das Nahtmaterial erstreckt sich dann durch ein Ausstoßerrohr 32, das konzentrisch innerhalb der Außennadel 30 liegt. Das Einführen geht, wie in Fig. 2 dargestellt, bis zu einer Phase mittlerer Einführung weiter, in der der Anker 10 durch das Ausstoßerrohr 32 über das distale Ende 31 der Außennadel 30 herausgeschoben ist. Der Anker 10 ist dann aus der Ausrichtung zum Einführwerkzeug 28 herausgeschoben und beginnt sich zu einer Kipposition hin zu neigen, die im wesentlichen quer zur axialen Richtung des Nahtmaterials liegt. Dann wird das Nahtmaterial durch den Schlitz zum Grund 26 des Schlitzes hin durch Ausüben von Zug auf das Nahtmaterial 20 gezogen.
- Wie vorstehend ausgeführt, bildet das Vorderende 14 des Rohrs 12 vorzugsweise eine Vorderfläche 34, die schräg zur Mittelachse des Rohrs 12 abgewinkelt ist. Eine Hinterfläche 36 ist vorzugsweise am Hinterende 16 des Rohrs 12 ausgebildet und liegt vorzugsweise in einer Ebene parallel zu der der Vorderfläche 34, und auch sie ist schräg zur Mittelachse des Rohrs 12 abgewinkelt. Die Hinterfläche 36 arbeitet so, daß sie das Verkippen des Ankers 10 zur Kipposition unterstützt, nachdem dieser sich aus der Außennadel des Einführwerkzeugs 28 gelöst hat.
- Die winkelmäßige Ausrichtung der Vorderfläche erzeugt relativ scharfe Kanten, was die Verankerung des Ankers 10 in weichem Gewebe erleichtert. Alternativ könnten die Vorder- und Hinterfläche 34 und 36 im wesentlichen rechtwinklig zur Achse des Rohrs 12 stehen. Beim abschließenden Einführschritt (in den Fig.2 und 3 veranschaulicht), befindet sich der Anker 10 in einer Kipposition bezogen auf die Länge des Nahtmaterials 20, wobei das Nahtmaterial 20 das Rohr 12 am Grund 26 des Schlitzes 18 verläßt.
- Die Vorderfläche der Außennadel 30 des Einführwerkzeugs 28 ist vorzugsweise abgeschrägt, wie es in den Fig. 2 und 3 dargestellt ist. Auch die Vorderkante 33 dieser Fläche verfügt über ein zugespitztes Vorderende. Diese Struktur ermöglicht es der Nadel 30, in die Haut einzudringen und/oder leichter zur Anbringung des Ankers in ihre Position verschoben zu werden. Auch um die "Ausbildung eines Kerns" aus Gewebe oder Haut zu verhindern, wenn das Werkzeug 28 verwendet wird, ist der Anker 10 vorzugsweise so in der Nadel 30 positioniert, daß er während des Einführens im wesentlichen bündig mit dem distalen Ende oder der Vorderfläche ist. Zu diesem Zweck ist es erwünscht, daß der Winkel der Vorderfläche der Außennadel 30 derselbe wie der Winkel der Vorderfläche 34 des Ankerrohrs 12 ist.
- Als alternatives Ausführungsbeispiel des Einführwerkzeugs 28 kann ein (nicht dargestellter) dünner, fester Ausstoßerstab anstelle des hohlen Ausstoßerrohrs 32, wie es in Fig. 2 dargestellt ist, verwendet werden. Wenn der Anker und das Nahtmaterial 20 einmal im hohlen Außenrohr 30 positioniert sind, kann der Ausstoßerstab in das Rohr eingeführt werden und dazu verwendet werden, den Anker an seine Anbringungs- und Verankerungsposition zu verschieben. Die Verwendung eines Ausstoßerstabs beseitigt den Schritt des Durchfädelns des Nahtmaterials durch das Ausstoßerrohr. Mit Ausnahme der Verwendung eines Ausstoßerstabs anstelle eines Ausstoßerrohrs ist der Rest des Anbringungsablaufs für den Nahtmaterialanker derselbe.
- Es wird nun auf Fig. 4 Bezug genommen, in der eine bevorzugte Verwendung eines erfindungsgemäßen Ankers 10 dargestellt ist, wobei der Anker in Körpergewebe 38 eingesetzt ist und an einem Knorpel 40 verankert ist. Im Körpergewebe 38 und im Knorpel 40 werden unter Verwendung irgendeiner herkömmlichen chirurgischen Bohr- oder Lochausbildungseinrichtung Löcher 42 bzw. 44 ausgebildet. Wenn der erfindungsgemäße Anker 10 dazu verwendet wird, ein Nahtmaterial an einem Knorpel zu befestigen, wird das Einführungswerkzeug durch die Löcher 42 und 44 eingeführt, bis das Ende 31 durch den Knorpel 40 hindurchtritt. Dann wird der Anker 10 aus dem Einführwerkzeug herausgedrückt und in seine Kipposition verdreht, wie es zuvor unter Bezugnahme auf die Fig. 2 und 3 beschrieben wurde.
- Es wird nun auf Fig. 5 Bezug genommen, in der die Verwendung eines erfindungsgemäßen Ankers 10 in weichem Körpergewebe 38 dargestellt ist. Im weichen Gewebe wird entweder durch das Einführwerkzeug oder durch einen vorigen Einschnitt ein Loch 42 ausgebildet. Der Anker 10 und das Nahtmaterial 20 werden unter Verwendung des Einführwerkzeugs in das Loch 42 engeführt und der Anker wird dadurch verschoben, daß, nachdem der Anker 10 aus dem distalen Ende 31 der Außennadel 30 ausgetreten ist, Zug auf das Nahtmaterial 20 ausgeübt wird. Das Vorderende 14 und das Hinterende 16 des Ankers 10 verschieben sich ausgehend von der Einführposition, in die das Vorderende zunächst in das Gewebe eingeführt wurde, in eine Position, in der das Vorderende und das Hinterende eine im wesentlichen rechtwinklige oder verkippte Position bezogen auf das Stück Nahtmaterial 20 einnehmen.
- Wie angegeben, wird der Nahtmaterialanker vorzugsweise dazu verwendet, Nahtmaterial an Körpergewebe zu befestigen, im allgemeinen hinter einem Knorpel, obwohl er auch dazu verwendet werden kann, Nahtmaterial alleine in Körpergewebe zu befestigen. Darüber hinaus kann der Nahtmaterialanker ganz durch Körpergewebe oder einen Knorpel hindurchgeführt werden und an der entgegengesetzten Seite außerhalb des Körpers positioniert werden, oder dort, wo kein Körpergewebe vorhanden ist. Die Anbringung und der Betrieb der Erfindung sind derselbe.
- Der Anker 10 kann entweder aus einem biologisch absorbierbaren Material oder aus einem nicht absorbierbaren, beständigen Material bestehen. Zu bevorzugten absorbierbaren Materialien gehören Polyglycolsäure, Polymilchsäure oder Trimethylen-Carbonat-Copolymere. Zu bevorzugten nicht absorbierbaren Materialien gehören Acetal-Homopolymere oder -Copolymere, Polyethylen, Polypropylen, Polyester und Copolymere derselben. Das Nahtmaterial kann aus einem beliebigen herkömmlichen Typ von Nahtmaterial bestehen, wie den Markennahtmaterialen Ticron oder Dexon, wobei es sich um Handelsbezeichnungen von Davis & Geck handelt.
- Bei der Erfindung besteht keine Schwierigkeit, das Nahtmaterial sicher am Anker zu befestigen. Das Nahtmaterial muß nicht in den Anker eingegossen werden, was manchmal zu Festigkeitsschwierigkeiten führt, sondern statt dessen wird ein mechanisches Befestigungssystem verwendet, das starke und sichere Befestigung gewährleistet. Auch hält das Umbiegen des Nahtmaterials am Ende des Schlitzes, wenn der Anker vorort angebracht ist, die volle Festigkeit des Nahtmaterials an diesem Punkt aufrecht. Befestigungseinrichtungen mit gegossenem Kopf und Faserteil weisen oft Festigkeitsprobleme auf, die dadurch hervorgerufen werden, daß der Kopf während des Einführens parallel zur Faser umgebogen wird.
- Fig. 6 zeigt ein Ausführungsbeispiel eines erfindungsgemäßen Nahtmaterialankers 10 in Verbindung mit einem einrastenden Nahtmaterialteil. Der Nahtmaterialanker weist einen rohrförmigen Körper 12 mit einem darin vorhandenen Schlitz 18 auf. Das Nahtmaterial 20 paßt durch den Körper und den Schlitz und ist an einem Ende des Ankers mit einem Knoten 22 ausgebildet.
- Mehrere kleine Kugeln 50 sind am Stück Nahtmaterial 20 in regelmäßigen Abständen angebracht. Die Kugeln sind auf das Nahtmaterial gegossen oder von Anfang an mit ihm ausgebildet. Die Kugeln wirken als Halteteile für ein Einrastteil wie eine Scheibe 52. Die Einrastscheibe 52 weist eine Reihe von Schlitzen 54 auf, die in der Mitte der Scheibe zusammenlaufen. Die Schlitze 54 bilden eine Anzahl kleiner flexibler Klappenteile 56. Die Schlitze und die Klappenteile ermöglichen es dem Nahtmaterial 20, in einer Richtung durch die Scheibe hindurchzutreten, jedoch wirken sie als Verrastung oder Halteeinrichtung und hindern das Nahtmaterial daran, in der anderen Richtung durchzutreten.
- Wenn die Anordnung aus dem Nahtmaterial und dem Anker, wie in Fig. 6 dargestellt, verwendet wird, wird der Anker vorort wie oben beschrieben positioniert und eingesetzt und die Scheibe 52 wird über das Nahtmaterial gefädelt und über die Kugeln 50 geschoben oder über diese weggeführt, bis sie am Körpergewebe anliegt. Eine Verwendung einer solchen Anordnung ist in Fig. 7 dargestellt. Der Anker 10 wird an einer Seite eines Gewebes 60 positioniert und die Scheibe 52 wird in eine Position an der Gegenseite des Gewebes 62 geschoben. Das überschüssige Stück Nahtmaterial 20 mit den daran vorhandenen Kugeln 50 wird abgeschnitten und weggeworfen. Auf diese Weise werden Gewebestücke 60 und 62 zusammengedrückt und sicher aneinander gehalten, bis sie wieder miteinander verbunden oder zusammengewachsen sind.
- Ein anderes Ausführungsbeispiel einer Anordnung aus einem Nahtmaterial und einem geschlitzten Anker ist in Fig. 8 dargestellt. Das Nahtmaterial 20 verfügt über ein langgestrecktes, rohrförmiges Halteteil 70, das an ihm angegossen oder angeformt ist. Das Teil 70 verfügt über eine Reihe regelmässig beabstandeter Rippen oder Wülste 72. Die Rippen oder Wülste 72 wirken als Halteeinrichtungen für ein Einrastscheibenteil 74, wie es in Fig. 9 dargestellt ist. Die Scheibe 54 verfügt über eine konusförmige Aussparung 76 in der Mitte, unterteilt durch einen Kanal 78 mit drei Schlitzen für das Nahtmaterial. Bei der Verwendung wird die in Fig. 8 dargestellte Anordnung im wesentlichen auf dieselbe Weise wie die in den Fig. 6 und 7 dargestellte Anordnung verwendet.
- Fig. 10 zeigt ein Verfahren zum Herstellen der in Fig. 8 dargestellten Anordnung. Das Nahtmaterial 20 mit dem Anker 10 an einem Ende verfügt über ein rohrförmiges Teil 17, das entlang seiner Länge anschließend an den Anker angeordnet wird. Das Nahtmaterial mit dem daran befindlichen rohrförmigen Teil wird dann in einem Formwerkzeug 80 angeordnet. Die zwei Hälften des Formwerkzeugs verfügen über eine darin ausgebildete Aussparung die die Rippen oder Wülste 72 am rohrförmigen Teil ausbildet. Um die fertiggestellte Anordnung herzustellen, werden die Formhälften erwärmt und zusammengebracht, wobei sie das rohrförmige Teil und das Nahtmaterial zwischen sich einschließen und zusammendrücken. Wärme und Druck vom Formwerkzeug 80 formt das Teil 70 in die in Fig. 8 dargestellte Form und befestigt auch das Teil 70 sicher am Nahtmaterial 20. Der Formungsprozeß könnte alternativ Ultraschallschweißen, Hochfrequenzschweißen, Spritzgießen, Druckgießen oder ein anderes ähnliches oder herkömmliches Verfahren sein.
- Fig. 11 zeigt noch ein anderes Ausführungsbeispiel der Erfindung. Bei diesem Ausführungsbeispiel ist ein getrenntes und unabhängig angegossenes Halteteil 84 am Nahtmaterial 20 befestigt. Das Nahtmaterial 20 wird (z.B. durch Anknoten) an einem Ende 86 des Teils 84 befestigt. Ein anderes Nahtmaterial 20' wird am anderen Ende 88 des Halteteils befestigt. Um die Anpassung und Befestigung an den Rippen oder Wülsten 92 auf dem geformten Halteteil 84 herzustellen, wird eine Einrastscheibe 90 verwendet.
- Die Verwendung des Ausführungsbeispiels der Erfindung von Fig. 11 ist in den Fig. 12A und 12B dargestellt. Wie in Fig. 12A gezeigt, wird der geschlitzte Anker 10 auf einer Seite zweier zerrissener oder getrennter Körpergewebeteile 94 und 96 eingeführt und Vorort angebracht und die Scheibe 90 wird auf das gerippte Halteteil 84 aufgerastet und auf diesem verschoben. Die zwei Gewebestücke werden durch den Anker 10 und die Scheibe 90 zusammengezogen (wie in Fig. 12B dargestellt) und zusammengehalten. Der überschüssige Teil 98 des Teils 84 und das Nahtmaterial 20' werden abgeschnitten und weggeworfen.
- Die vorstehend beschriebenen bevorzugten Ausführungsbeispiele sollen für die Erfindung veranschaulichend sein, und sie können innerhalb des Schutzbereichs der beigefügten Ansprüche abgeändert werden.
Claims (10)
1. Anker zum Befestigen eines Strangs Nahtmaterial im
Körper, mit einem Rohr (12) mit einem Vorderende (14) und einem
Hinterende (16) und einem Schlitz (18), der sich vom
Hinterende (16) zu einem Punkt zwischen dem Vorder- und dem
Hinterende erstreckt, wobei das Hinterende (16) eine hintere
Fläche festlegt, die unter einem schrägen Winkel zur
Mittelachse des Rohrs (12) angeordnet ist.
2. Anker nach Anspruch 1, bei dem das Vorderende (14) eine
vordere Fläche festlegt, die unter einem schrägen Winkel zur
Mittelachse des Rohrs (12) angeordnet ist.
3. Anker nach Anspruch 1, bei dem sich der Schlitz (18)
vom axial inneren Bereich der hinteren Fläche (16) zu einem
mittleren Punkt zwischen dem Vorder- und Hinterende
erstreckt.
4. Anker nach Anspruch 1 und Strang eines Nahtmaterials in
Kombination, wobei der Anker (10) eine Einrichtung zum
Befestigen des Strangs Nahtmaterial (20) am Vorderende (14)
des Rohrs (12) aufweist, wobei sich der Strang vom
Vorderende aus zumindest teilweise durch das Rohr (12) erstreckt.
5. Kombination nach Anspruch 4, bei der die
Befestigungseinrichtung vom Strang Nahtmaterial (20) getragen wird.
6. Kombination nach Anspruch 5, bei der die
Befestigungseinrichtung ein Knoten ist, der im Strang am Abschlußende
des Strangs (20) geknotet ist, welcher Knoten am Vorderende
(14) des Rohrs (12) angreift, wenn Spannung auf das andere
Ende des Strangs ausgeübt wird.
7. Kombination nach Anspruch 5, ferner mit einem
Einrastteil (52; 74) zum Zusammenwirken mit der
Befestigungseinrichtung, um die Kombination zusammenzuhalten, um
zumindestens zwei Gewebestücke aneinander zu befestigen.
8. Kombination nach Anspruch 7, bei der die
Befestigungseinrichtung eine Reihe von Ausweitungen (50; 72) aufweist,
die mit dem Einrastteil (52; 74) zusammenpassen sollen.
9. Kombination nach Anspruch 7, bei der der Anker, der
Strang, die Befestigungseinrichtung und das Einrastteil aus
einem biologisch absorbierbaren Material bestehen.
10. Kombination nach Anspruch 4, bei der der Strang (20)
zwischen einer Einführposition, in der der Strang am
Hinterende (16) aus dem Rohr (12) austritt, und einer Kipposition
verschiebbar ist, in der der Strang durch den Schlitz (18)
aus dem Rohr austritt.
