Description
- Die Erfindung betrifft ein flüssiges Augenpräparat, das einen Flüssig-Gel-Phasenübergang durchläuft.
- Bei vielen bekannten Augenpräparaten wird ein großer Teil des an das Auge verabreichten Wirkprinzips über die Tränendrainage entfernt. Die effektive therapeutische Wirkung solcher Präparate ist demnach auf eine sehr kurze Zeit begrenzt. Zur Beseitigung dieses Nachteils wurde die Verwendung von Präparaten vom Typ Gel, festes Implantat oder Salbe vorgeschlagen. Jedoch stoßen diese Dosierungsformen beim Patienten auf Widerstand, die flüssigen Formen werden besser vertragen.
- Somit wurden zur Lösung dieses Problems flüssige Dosierungsformen vorgeschlagen, die in situ durch die Wirkung einer pH-Änderung (US 4 136 173) oder eines Temperaturanstiegs (US 4 188 373, US 4 474 751) gelieren.
- Der Hauptnachteil der obigen Formulierungen ist im ersten Fall ein nicht-physiologischer und daher schlecht vertragener pH- Wert und in zweiten Fall die Gefahr, daß die Formulierung vor ihrer Verabreichung geliert, d.h. sozusagen während der Aufbewahrung durch die Wirkung eines Temperaturanstiegs.
- Aufgabe der Erfindung ist es, ein Augenpräparat bereitzustellen, welches eine über einen Zeitraum anhaltende Aktivität aufweist und diese Nachteile nicht besitzt.
- Insbesondere betrifft die Erfindung ein flüssiges Augenpräparat, welches einen Flüssig-Gel-Phasenübergang durchläuft, wenn es an das Auge verabreicht wird und eine wäßrige Lösung mit mindestens einem Wirkprinzip enthält, das dadurch gekennzeichnet ist, daß das Präparat 0,1 bis 5 Gew.-% eines sulfathaltigen Polysaccharidderivates enthält, ausgewählt aus kappa-Carrageenan, iota-Carrageenan und Gemischen davon, wobei der Flüssig-Gel-Phasenübergang durch die Wechselwirkung des genannten sulfathaltigen Polysaccharidderivates mit den Proteinen der Tränenflüssigkeit ausgelöst wird.
- Erfindungsgemäß ist der Flüssig-Gel-Phasenübergang, den das Augenpräparat durchläuft, eine Folge der Wechselwirkung der sulfathaltigen Polysaccharide in wäßriger Lösung mit den Tränenproteinen.
- Ferner verbessert die Anwesenheit von Elektrolyten die rheologischen Eigenschaften der erhaltenen Gele.
- Dieser Phasenübergang, den das Augenpräparat durchläuft, vergrößert die Kontaktzeit des genannten Präparates mit dem Auge und ermöglicht demgemäß eine anhaltende oder verzögerte Diffusion des Wirkprinzips, das in diesem zu erhaltenen Präparat enthalten ist.
- Darüber hinaus wurde gefunden, daß das erfindungsgemäße Präparat zu größeren Konzentrationen des Wirkprinzips im Auge führt, sobald das Präparat verabreicht wird.
- Ein solches Augenpräparat besitzt weiterhin die Vorteile, daß es durch die Wirkung eines Temperaturanstiegs oder dadurch, daß es einen pH-Wert in der Nähe des physiologischen pH-Wertes besitzt, d.h. in der Größenordnung von 7, nicht spontan geliert.
- Die erfindungsgemäß verwendeten sulfathaltigen Polysaccharide werden ausgewählt aus kappa-Carrageenan, iota-Carrageenan und Gemischen davon.
- Gemäß einer bevorzugten Ausführungsform der Erfindung wird iota-Carrageenan verwendet.
- Gemäß einer wichtigen Eigenschaft der Erfindung wird das sulfathaltige Polysaccharidderivat in einer Konzentration in der Größenordnung von 0,1 bis 5 Gew.-% verwendet.
- Das genannte Präparat enthält mindestens ein aktives Wirkprinzip in Lösung oder Suspension in einem wäßrigen Medium.
- Es ist möglich, eine breite Vielzahl von Wirkprinzipien zu verwenden. Letztere können insbesondere aus den folgenden Pharmazeutischen Verbindungen ausgewählt werden:
- - antibakterielle Substanzen, wie beta-Lactam Antibiotika, beipielsweise Cefoxitin, n-Formamindoylthienamycin und weitere Thienamicinderivate, Tetracycline, Chloramphenicol, Neomycin, Carbenicillin, Colistin, Penicillin G, Polymyxin B, Vancomycin, Cefazolin, Cefaloridin, Chibrorifamycin, Gramicidin, Bacitracin und Sulphonamide; Aminoglycosidantibiotika, wie Gentamicin, Kanamycin, Amikacin, Sisomicin und Tobramycin; Nalidixinsäure und ihre Analoga, wie Norfloxacin und die antimikrobielle Kombination aus Fluoralanin/Pentizidon, Nitrofurazonen und ihren Analoga;
- - anti-inflammatorische Verbindungen, wie Cortison, Hydrocortison, Hydrocortisonacetat, Betamethason, Dexamethason, Dexamethasonnatriumphospat, Prednison, Methylprednisolon, Medryson, Fluormetholon, Prednisolon, Triamcinolon, Indomethacin, Sulindac, ihre Salze und entsprechenden Sulfide und ihre Analoga;
- - Miotika und Anticholinergika, wie Echothiopat, Pilocarpin, Physostigminsalicylat, Diisopropylfluorphosphat, Epinephrin, Dipivalylepinephrin, Neostigmin, Echothiopatiodid, Demecarciumbromid, Carbamoylcholinchlorid, Methacholin, Bethanechol und ihre Analoga;
- - Mydriatika, wie Atropin, Homatropin, Scopolamin, Hydroxyamphetamin, Ephedrin, Cocain, Tropicamid, Phenylephrin, Cyclopentolat, Oxyphenonium, Eucatropin und ihre Analoga;
- - weitere, zur Behandlung von Augenzuständen und Läsionen verwendete Arzneimittel, wie: Antiglaucom-Arnzeimittel, beispielsweise Timolol und R-Timolol und eine Kombination von Timolol oder R-Timolol mit Pilocarpin und auch viele andere adrenerge Agonisten und/oder Antagonisten; Epinephrine und ein Komplex aus Epinephrin oder Arzneimittelvorstufen und Dipivefrinderivate und hyperosmotische Mittel, wie Glycerin, Mannit und Harnstoff;
- - antiparasitäre Verbindungen und/oder antiprotozoale Verbindungen, wie Ivermectin, Pyrimethamin, Trisulphapyrimidin, Clindamycin und Corticosteroid- Präparationen:
- - Verbindungen, die antivirale Aktivität besitzen, wie Acyclovir, 5-Iod-2'-deoxyuridin (IDU), Adenosinarabinosid (Ara-A), Trifuorthymidin und Interferon und Interferoninduzierende Mittel, wie PolyI.PolyC;
- - Kohlenstoffanhydrase-Inhibitoren, wie Acetazolamid, Dichlorphenamid, 2-(p-Hydroxyphenyl)-thio-5-thiophensulphonamid, 6-Hydroxy-2-benzothiazol-sulphonamid, 6-Pivaloyloxy-2-benzothiazolsulphonamid, MK 927 und MK 417;
- - Antimykotika, wie Amphotericin B, Nystatin, Flucytosin, Natamycin und Miconazol;
- - Anaesthetika, wie Etidocain, Cocain, Benoxinat, Dibucain-Hydrochlorid, Dyclonin-Hydrochlorid, Naepain, Phenacain-Hydrochlorid, Piperocain, Proparacain- Hydrochlorid, Tetracain-Hydrochlorid, Hexylcain, Bupivacain, Lidocain, Mepivacain und Prilocain;
- - diagnostische Augenmittel, wie:
- a.) die, die zur Prüfung der Retina verwendet werden, wie Fluoresceinnatrium;
- b.) die, die zur Prüfung von Bindehaut, Cornea oder des Tränenapparates verwendet werden, wie Fluorescein und Bengalrosa; und
- c.) die, die zur Prüfung abnormer Reaktionen der Pupille verwendet werden, wie Metacholin, Cocain, Adrenalin, Atropin, Hydroxyamphetamin und Pilocarpin;
- - Augenmittel, wie Ethylendiamintetraessigsäure (EDTA) und Deferoxamin;
- - Immunosuppressiva und Antimetaboliten, wie Methotrexat, Cyclophosphamid, 6-Mercaptopurin und Azathioprin; und antibiotische/antiinflammatorische Kombinationen, wie die Kombination Neomycinsulphat/Dexamethasonnatriumphosphat und die Kombinationen, mit denen gleichzeitig das Glaucom behandelt wird, beispielsweise eine Timolomaleat-/Aceclidin- Kombination.
- Das erfindungsgemäße Augenpräparat muß natürlich die Kriterien zur Verwendung auf diesem Gebiet erfüllen, insbesondere sollte es vorzugsweise isotonisch sein und einen pH-Wert aufweisen, der zwischen 5,0 und 8,0 liegt. Somit kann es weitere Komponenten einschließen, wie Tonizitätsregler, Konservierungsstoffe und Puffersysteme.
- Die Erfindung betrifft ferner ein Verfahren zur Herstellung des genannten Präparates, das dadurch gekennzeichnet ist, daß das Wirkprinzip und gegebenenfalls die anderen Komponenten unter Rühren in einer wäßrigen Lösung in Gegenwart von 0,1 bis 5 Gew.-% eines sulfathaltigen Polysaccharidderivates vermischt werden, welches ausgewählt wird aus kappa-Carrageenan, iota- Carrageenan und Gemischen davon.
- Die unten angegebenen Beispiele ermöglichen es, daß weitere Vorteile und Eigenschaften der Erfindung gezeigt werden.

- Wasser auf 100,00 g
- Timololmaleat ist das Wirkprinzip, Aubygel X52TA (Warenzeichen), vertrieben von Meso-Rousselot-Satia, ist iota- Carrageenan, die Kombination aus Tromethamin/ Maleinsäure stellt ein Puffersystem dar, Mannit ist ein Tonizitätsregler und Benzalkoniumchlorid ein Konservierungsstoff.
- Das Präparat aus Beispiel 1 wird in vitro gemäß einem Gelierungstest getestet.
- In diesem Test wird die Gelierung hervorgerufen durch Zugabe von 0,5 ml des genannten Präparates zu 1 ml künstlicher Tränen, deren Formulierung in Tabelle I angegeben ist. Die Gelbildung wird nach 30 sec geprüft. Auf die Inkubation bei 34 ºC folgte das Filtrieren und Wiegen des auf dem Filter zurückgebliebenen Rückstandes.
- Die Ergebnisse sind in Tabelle II angegeben.
- In den proteinfreien künstlichen Tränen ist die Gelbildung gering und kann der Wechselwirkung zwischen Carrageenan und den in den künstlichen Tränen vorhandenen Elektrolyten zugeschrieben werden. Derselbe Test, der mit künstlichen proteinhaltigen Tränen (Lysozyin/Fraktion V-Rinderalbumin) durchgeführt worden ist, ergibt andererseits gute Ergebnisse. Es wird die Bildung eines opaleszierenden Gels beobachtet, das einen geringen Zusammenhalt aufweist. Die Bildung dieses Gels wird darüberhinaus durch leichtes Rühren verstärkt.
- Zur Demonstration der verlängerten Anwesenheit des in dem erfindungsgemäßen Augenpräparat enthaltenen Wirkprinzips im Auge wurde eine in vivo-Studie durchgeführt.
- Ein erfindungsgemäßes Augenpräparat und Timolol (Warenzeichen), welches zu Vergleichszwecken verwendet wurde, wurden einem Test unterzogen, bei dem das Eindringen von Timolol in das Auge an Albinokaninchen geprüft wurde.
- Zu diesem Zweck wurden Instillationen von jeweils 50 µl der Testlösungen im Inneren des Bindehautsacks lebender Albinokaninchen durchgeführt. Nach der Verabreichung wurden dann sechs Tiere zu den Zeiten 10 min., 30 min., 1 Std., 2 Std. und 4 Std. getötet und Timolol in der Cornea, der wäßrigen Flüssigkeit und der Iris getestet.
- Die so getesteten Timolol-Konzentrationen sind in Tabelle III für die Cornea bzw. in Tabelle IV für die wäßrige Flüssigkeit und in Tabelle V für die Iris und die Ciliarkörper angegeben.
- Die Ergebnisse zeigen, daß ein erfindungsgemäßes Augenpräparat höhere Konzentrationen des Wirkprinzips in den verschiedenen Geweben ergibt als das Vergleichsprodukt Timolol (Warenzeichen) in einer Konzentration von 0,25 %, wobei dies der Fall ist sowohl zu Beginn der Anwendung als auch nach 4 Stunden. Claims (7)
1. Flüssiges Augenpräparat, das bei Anwendung am Auge einen Flüssigkeit-Gel-
Phasenübergang durchmacht und eine wäßrige Lösung aus mindestens einem
Wirkprinzip enthält, dadurch gekennzeichnet, daß das Präparat 0,1 bis 5 Gew.-%
eines sulfathaltigen Polysaccharid-Derivates enthält, ausgewählt aus kappa-
Carrageenan, iota-Carrageenan und Gemischen davon, wobei der Flüssigkeit-Gel-
Phasenübergang durch eine Wechselwirkung des genannten sulfathaltigen
Polysaccarid-Derivates mit den Proteinen der Tränenflüssigkeit ausgelöst wird.
2. Augenpräparat nach Anspruch 1, dadurch gekennzeichnet, daß es einen pH-Wert in
der Nähe des physiologischen pH-Wertes aufweist.
3. Augenpräparat nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß das sulfathaltige
Polysaccharid iota-Carrageenan ist.
4. Augenpräparat nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet,
daß es weitere Verbindungen, ausgewählt aus Tonus-Regulatoren,
Konservierungsstoffen und Puffersystemen, enthält.
5. Augenpräparat nach einem der vorausgegangenen Ansprüche, dadurch
gekennzeichnet, daß das Wirkprinzip aus Antiglaucom-Mitteln, Antibiotika und
antiviralen Mitteln ausgewählt ist.
6. Augenpräparat nach Anspruch 5, dadurch gekennzeichnet, daß das Wirkprinzip
Timolol ist.
7. Verfahren zur Herstellung eines Augenpräparates nach einem der vorausgegangenen
Ansprüche, dadurch gekennzeichnet, daß das Wirkprinzip und gegebenenfalls die
anderen Komponenten unter Rühren in einer wäßrigen Lösung in Gegenwart von 0,1
bis 5 Gew.-% eines sulfathaltigen Polysaccharid-Derivates, ausgewählt aus Kappa-
Carrageenan, iota-Carrageenan und Gemischen davon, vermischt werden.
