- Hintergrund der Erfindung
- Wenn einem Patienten Wirkstoffe therapeutisch verabreicht werden, sind sie oft in einem Träger enthalten. Im Zusammenhang mit der Reparatur von Gewebe wird oft demineralisieries Knochenpulver als Wirkstoff verwendet, um neues Knochenwachstum an einer Stelle mit Knochendefekten auszulösen. Demineralisiertes Knochenpulver kann ein loses pulveriges Material sein, das sich nicht leicht an der Stelle eines Knochendefekts einschließen lässt, insbesondere in Gegenwart von Körperflüssigkeiten oder chirurgischer Spülungen. Daher kann demineralisiertes Knochenpulver mit einem Träger kombiniert werden, um eine Zusammensetzung zur Verfügung zu stellen, die sich besser handhaben lässt und ausreichend lange an der Stelle eines Knochendefekts verbleiben kann, um neues Knochenwachstum zu bewirken.
- Demineralisiertes Knochenpulver ist ein Material, das durch herkömmliche Verfahren hergestellt werden kann. BMPs sind eine Gruppe von Proteinen aus der Kategorie der Superfamilie von Proteinen, die den Wachstumsfaktor-β transformieren.
- Isolierte BMPs sind ein weiteres Material, das die Bildung neuen Knochens auslösen und durch herkömmliche Verfahren hergestellt werden kann. Bis heute sind verschiedene BMPs isoliert und mit dem Knochenheilungsprozess in Verbindung gebracht worden. BMPs können aus Knochen als Proteingemisch oder einzeln durch rekombinante Gentechnik hergestellt werden.
- Demineralisiertes Knochenpulver und BMP sind mit Trägern kombiniert worden, um Zusammensetzungen zur Knochenreparatur herzustellen. Jefferies (US-A-4,394,370) offenbart Gewebereparaturzusammensetzungen, die demineralisiertes Knochenpulver, BMPs oder beides in einer rekonstituierten Kollagenmatrix enthalten. Glowacki et al. (US-A-4,440,750) offenbart wässerige Zusammensetzungen aus demineralisierten Knochenteilchen und rekonstituierten Kollagenfasern.
- Clokie (US-A-6,309,659) beschreibt eine biokompatible Reparaturzusammensetzungen für Bindegewebe, die Knochenpulver und einen Träger aus Poloxamer 407 (auch als Pluronic® F127, hergestellt von der BASF Corporation, bekannt) und Wasser umfassen. Pluronic® F127 ist ein Polyoxyethylen-Polyoxypropylen-Polyoxyethylen-Triblockcopolymer der Formel in der a etwa 101 und b etwa 56 ist. Dieses Molekül hat zwei Hydroxylgruppen (-OH), eine an jedem Ende des langen Polymermoleküls. Bei speziellen Konzentrationen und Temperaturen weist eine Zusammensetzung aus Pluronic F127 und Wasser thermisch insofern ein umgekehrtes Phasenverhalten auf, als sie ein Gel bilden und zunehmend zähflüssig oder sogar fest werden kann, wenn ihre Temperatur ansteigt.
- Shimura et al. (Internationale Patentanmeldung Nr. WO 97/18829) beschreiben eine Zusammensetzung die ein Polyoxyethylen-Polyoxypropylenglycol (z.B. ADEKA® F127) und ein morphogenetisches Knochenprotein enthält und angeblich ein umgekehrtes Phasenverhalten hat.
- In der Technik besteht nach wie vor Bedarf an Trägern für Wirkstoffe und insbesondere Trägern, die in Zusammensetzungen zur Reparatur von Gewebe verwendet werden sollen.
- Zusammenfassung der Erfindung
- Diese Erfindung betrifft eine Zusammensetzung nach den Ansprüchen 1 bis 5.
- Bei einer bevorzugten Ausführungsform ist das mit Endkappen versehene Polymer die Trägerkomponente für eine Zusammensetzung zur Reparatur von Gewebe. Stärker bevorzugt umfasst die erfindungsgemäße Zusammensetzung zur Reparatur von Gewebe ein Polymer mit Endkappen und ein oder mehrere Knochengewebe, Kollagengewebe, Knochenprotein bzw. Kombinationen oder Derivate dieser Materialien. Solche Zusammensetzungen zur Reparatur von Gewebe können an die Stelle eines Knochendefekts, an die Stelle eines Knorpeldefekts oder an andere die Skelettmuskulatur betreffende Stellen aufgebracht werden. Die Zusammensetzung kann durch eine Spritze, einen Spatel oder eine andere chirurgische Abgabevorrichtung aufgebracht werden. Die Zusammensetzung kann auch als Beschichtung auf chirurgischen Implantaten verwendet werden, die an oder nahe der Stelle eines Knochendefekts, der Stelle eines Knorpeldefekts oder an andere die Skelettmuskulatur betreffende Stellen eingeführt werden soll.
- Beschreibung beispielhafter Ausführungsformen
- Polymere sind lange Moleküle, die in Form von Homopolymeren (die eine Monomereinheit eines einzigen Typs enthalten) oder Copolymeren (die zwei oder mehrere Typen von Monomereinheiten enthalten) vorliegen können. Viele Polymere haben Hydroxylendgruppen (-OH-Endgruppen). Die erfindungsgemäßen, mit Endkappen versehenen Polymere sind Polymere, aus denen die Hydroxylgruppen entfernt wurden und die von Polyoxyethylen-Polyoxypropylen-Polyoxyethylen-Triblockcopolymeren (POE-POP-POE-Triblockcopolymeren) abgeleitet wurden. Diese werden unter dem Handelsnamen Pluronic® von BASF vertrieben und haben die folgende allgemeine Struktur:
- Aus den erfindungsgemäßen mit Endkappen versehenen Polymeren sind beide Wasserstoffe der Hydroxylgruppen entfernt und durch eine weniger reaktive funktionelle Gruppe ersetzt worden. Die als Ersatz dienende funktionelle Gruppe ist eine Methylgruppe (-CH3).
- Das in den erfindungsgemäßen Zusammensetzungen demineralisierte Knochenpulver kann nach vielen verschiedenen Verfahren hergestellt werden. Einige herkömmliche Verfahren werden in Jefferies, supra, und Glowacki et al., supra, genannt. Solche herkömmlichen Verfahren zur Herstellung von DBM umfassen einen Entfettungsschritt und einen Demineralisierungsschritt. Unterschiedliche Verfahren zum Entfetten, z.B. Wäschen mit heißem Wasser oder Chloroform/Methanol, können zum Einsatz kommen. Im Allgemeinen verwendet man zur Demineralisierung mehrere unterschiedliche Verfahren mit verschiedenen Typen von Säurelösungen für wechselnde Zeiten und bei wechselnden Temperaturen. Der demineralisierte Knochen kann in verschiedenen Formen und Größen hergestellt werden. In einer bevorzugten Ausführungsform liegt der demineralisierte Knochen in Form eines Pulvers vor und hat stärker bevorzugt eine Größe im Bereich von etwa 100 bis 850 μm.
- Der erfindungsgemäßen Zusammensetzung zur Reparatur von Gewebe können weitere Materialien zugesetzt werden, und zwar sowohl aktive als auch inaktive Inhaltsstoffe. Diese zusätzlichen Materialien umfassen Kollagen, Gelatine, rückständige Feststoffe, die während des Extraktionsverfahrens erzeugt werden und rückständige BMPs enthalten können oder auch nicht, Knochenmineral, Hydroxyapatit, Tricalciumphosphat, biphasisches Calciumphosphat, Calciumsulfat, biologische Gläser sowie natürliche oder synthetische Polymere.
- Die biologischen und physikalisch-chemischen Eigenschaften sowie die Eigenschaften der Zusammensetzung zur Reparatur von Gewebe in Bezug auf biologische Abbaubarkeit können durch bekannte Vernetzungsmittel wie Chemikalien (z.B. Glutaraldehyd oder Formaldehyd) oder Bestrahlung (z.B. Gamma- oder Elektronenstrahlen) verändert werden. Vorzugsweise wird zur Vernetzung Strahlung, am meisten bevorzugt Elektronenstrahlung (E-Strahlung) eingesetzt, um die nassen oder trockenen Materialien zu bestrahlen.
- Bei einer anderen bevorzugten Ausführungsform hat die Zusammensetzung zur Reparatur von Gewebe die Konsistenz eines Gels oder von Kitt.
- Die folgenden Beispiele wurden aufgenommen, um bevorzugte Ausführungsformen der Erfindung vorzustellen. Fachleute werden sich darüber im klaren sein, dass die in den folgenden Beispielen offenbarten Techniken Techniken sind, die nach Feststellung der Erfinder bei der Durchführung der Erfindung gut funktionieren. Daher können sie als bevorzugte Wege zu ihrer Durchführung angesehen werden.
- Beispiele
- Beispiel 1
- Eine Probe von Pluronic® F127 (BASF) wird bereitgestellt. Die Pluronic® F127 Moleküle werden durch Dispergieren des Pluronic® F127 in einem aprotischen Lösungsmittel wie Tetrahydrofuran oder einem Alkan mit geeigneten Mengen von entweder n-Butyllithium, das im Handel in einer Hexanlösung erhältlich ist, oder Lithiumaluminiumhydrid, das im Handel in Lösungen von THF erhältlich ist, in ihre Dilithiumsalze umgewandelt.
- Die Einwirkung von Butyllithium oder Lithiumaluminiumhydrid auf das Pluronic® F127 führt zur Entwicklung einer Menge von entweder Butangas oder Wasserstoffgas proportional zur Anzahl der im Pluronic® F127 Molekül vorhandenen Hydroxylgruppen. Aus dem durchschnittlichen Molekulargewicht von Pluronic® F127 von 12.150 kann man berechnen, dass der Anteil der vorhandenen Hydroxylgruppen die Entwicklung von 4,48 ml Gas aus einer 1,25 Gramm Probe von Pluronic® F127 bei S. T. P. auslöst. Um ein genauer und leichter messbares Gasvolumen zu erzeugen, hat man berechnet, dass eine 12,5 g Probe von Pluronic® F127 zur Entwicklung von 44,8 ml Gas bei S. T. P. führt. Somit geben solche gasometrischen Messungen der Pluronic® F127-Ausgangsprobe und die Messungen nach der chemischen Modifikation der Pluronic® F127-Probe den Anteil der ursprünglich vorhandenen Hydroxygruppen an und bestätigen, dass in der chemisch modifizierten Probe keine Hydroxylgruppen mehr vorhanden sind.
- Wenn die Pluronic® F127 Probe quantitativ zu ihrem Dilithiumsalz umgewandelt wurde, wird ein geeigneter Anteil eines Alkylierungsmittels, nämlich Methyliodid, zugesetzt, um die Synthese des Dimethyletherderivats von Pluronic® F-127 zu vollenden.
- Um sicherzustellen, dass das ganze Lithium durch Methylgruppen ersetzt wurde, wird das Methyliodid in einem großzügigen Überschuss verwendet. Da Methyliodid ziemlich leicht flüchtig ist, ist solches überschüssiges Methyliodid nicht schädlich, weil es durch Verdampfen des Reaktionsgemischs ohne weiteres entfernt werden kann. Die resultierenden mit Dimethylkappen versehenen Polymermoleküle werden unter verringertem Druck verdampft, um alle Spuren von noch verbleibendem Methyliodid oder Lösungsmittel zu entfernen. Das mit Endkappen versehene Polymerprodukt kann ein oder zweimal aus einem geeigneten Lösungsmittel umkristallisiert werden, und somit kann das Derivat durch diese selektive Kristallisierung von allen kleineren Mengen noch verbleibender Verunreinigungen gereinigt werden. Wie bereits erwähnt, besteht der endgültige Test, ob auch wirklich alle Hydroxylgruppen entfernt wurden, darin, die gereinigten Produktproben der Einwirkung von Lithiumaluminiumhydridlösungen auszusetzen. Dabei wird erwartet, dass sich überhaupt kein Wasserstoffgas bildet, weil es keine Hydroxylgruppen mehr gibt. Zur physikalischen Bestätigung des Fehlens solcher endständiger Hydroxylgruppen kann das gereinigte Derivat in hochkonzentrierten Lösungen sowohl einer Infrarot- als auch einer spektroskopischen Protonen-NMR-Analyse unterzogen werden.
- Beispiel 2
- Eine Zusammensetzung zur Reparatur von Gewebe wird dadurch hergestellt, dass man etwa 25 Gew.-% des gemäß Beispiel 1 hergestellten, mit Endkappen versehenen Polymers mit 75% Wasser kombiniert. Möglicherweise muss das Gemisch mehrere Stunden bei heruntergekühlter Temperatur gerührt werden, um das mit Endkappen versehene Polymer vollständig zu dispergieren. Das Gemisch aus dem mit Endkappen versehenen Polymer und Wasser wird dann mit einer solchen Menge an demineralisiertem Knochenpulver kombiniert, dass man die gewünschte Konsistenz erhält. Beispielsweise kann die Zusammensetzung mindestens 20 bis 30 Gew.-% demineralisiertes Knochenpulver auf das Gewicht der Gesamtzusammensetzung enthalten. Die Zusammensetzung kann in einem sterilen Einwegpaket bereitgestellt werden.
- Beispiel 3
- Der in Beispiel 2 beschriebenen Zusammensetzung können zusätzliche Komponenten zugesetzt werden. Solche Komponenten können morphogenetische Knochenprotein(e), Kollagen, Gelatine, während des Extraktionsverfahrens erzeugte rückständige Feststoffe, die rückständige BMPs enthalten können oder auch nicht, Knochenmineral, Hydroxyapatit, Tricalciumphosphat, biphasisches Calciumphosphat, Calciumsulfat, biologische Gläser sowie natürliche und synthetische Polymere einschließen.
- Beispiel 4
- Anstelle des in Beispiel 2 beschriebenen Knochenpulvers können dem nach Beispiel 1 hergestellten, mit Endkappen versehenen Polymer ein oder mehrere extrahierte und gereinigte oder rekombinant hergestellte BMPs zugesetzt werden.
Claims (5)
- Zusammensetzung nach Anspruch 1, worin die in Teil (ii) definierte Verbindung in Wasser dispergiert ist.
- Zusammensetzung nach Anspruch 2, worin die Dispersion der in Teil (ii) definierten Verbindung in Wasser umgekehrtes Phasenverhalten aufweist.
- Zusammensetzung nach Anspruch 1, worin die Zusammensetzung eine Zusammensetzung zur Reparatur von Gewebe ist.
- Zusammensetzung nach Anspruch 1, worin die Zusammensetzung des Weiteren Collagen, Gelatine, BMPs, Knochenmineralien, Hydroxylapatit, Tricalciumphosphat, zweiphasiges Calciumphosphat, Calciumsulfat, biologische Gläser, ein natürliches Polymer oder synthetische Polymere umfasst.
