Description
- Es wird Bezug genommen auf die ebenfalls anhängigen Anmeldungen der Anmelderin EP-A-0 191 056 und EP-A-0 214 281, welche die Verwendung eines die Absorption von t-PA verbessernden Mittels wie z. B. Hydroxylamin oder eines nicht-toxischen Salzes davon, vorzugsweise Hydroxylaminhydrochlorid, zur Steigerung der Absorption von t-PA in das Blut beschreiben, wenn das t-PA intramuskulär verabreicht wird.
- Superoxiddismutase (SOD) ist ein Protein, in spezifischer Weise ein Enzym, welches eingesetzt worden ist, um die Nebenwirkungen einer Bestrahlungstherapie zu lindern und insbesondere, um die gesunden Zellen zu schützen.
- Superoxiddismutase ist ferner eingesetzt worden, um sauerstofffreie Radikale abzufangen, die sich bei einer Reperfusion gebildet haben, und die Schädigungen des Myocardiums zu verringern. Von Sauerstoff abgeleitete freie Radikale schließen das Superoxidradikal O&sub2;- und das reaktivere Hydroxylradikal OH, welches sich sekundär aus Superoxid bilden kann, ein.
- Im JACS Band 5, Nr. 2, Februar 1985, gibt es kurze Abhandlungen einer Reihe von Artikeln auf diesem Gebiet. So gibt es auf Seite 489 eine kurze Abhandlung eines Artikels von Ambrosio et al. mit der Überschrift "Reduction In Reperfusion Injury With Recombinant Human Superoxide Dismutase Following Global Ischemia" und ferner auf Seite 489 eine kurze Abhandlung eines Artikels von Werns et al. mit der Überschrift "Superoxide Dismutase But Not Catalase Protects Reperfused Ischemic Myocardium". Ferner gibt es auf Seite 541 eine kurze Abhandlung eines Artikels von Horneffer et al. mit der Überschrift "Reperfusion Injury Prevented By Oxygen Free Radical Scavenger: Functional and Morphological Evidence" und auf Seite 542 eine kurze Abhandlung eines Artikels von Myers et al. mit der Überschrift "Superoxide Dismutase And Catalase Enhance Recovery Of Myocardial Function Following Reversible Ischemia". Im Artikel von Horneffer et al. wird ausgeführt, daß die Bildung von cytotoxischen sauerstofffreien Radikalen mit einer Reperfusion eines ischämischen Myocardiums einhergeht. Horneffer et al. untersuchten die SOD, um zu sehen, ob sie die Erholung eines ischämischen Myocardiums verbessern könnte. Es wurde gefunden, daß durch Infusion von SOD in Schweine nach reversiblem Koronararterienverschluß eine Reperfusionsschädigung vermieden und die Wiederherstellung des Myocardiums verbessert wurden. Ähnliche Ergebnisse wurden von Myers et al. bei Hunden unter Verwendung von 5 mg/kg (3100 U/mg) SOD erhalten. Ambrosio et al. stellten fest, daß rekombinante humane Superoxiddismutase (SOD) die Reperfusionsschädigungen verringerte.
- Kürzlich wurde von McCord im New England Journal of Medicine, Band 312, Nr. 3, Seiten 159-163 ausgeführt, daß "ischämische Schädigung" in vielen Fällen eine falsche Bezeichnung ist und daß ein wesentlicher Teil der Schädigung besser als Reperfusionsschädigung bezeichnet wird und während der Wiedereinführung von molekularem Sauerstoff in das Gewebe auftritt. Ferner wurde von McCord ausgeführt, daß die Hauptquelle für Superoxid in wiederdurchbluteten und wieder mit Sauerstoff versorgten Geweben das Enzym Xanthinoxidase zu sein scheint, welches während einer Ischämie mittels eines durch Calcium ausgelösten proteolytischen Angriffs auf die Xanthindehydrogenase freigesetzt wird. Wiederdurchblutete Gewebe können durch Fängersubstanzen für Superoxidradikale oder Hydroxylradikale oder durch Allopurinol oder andere Inhibitoren der Xanthinoxidase wie z. B. Alloxanthin geschützt werden.
- Die Verbindung WR-2721 [H&sub2;N(CH&sub2;)&sub3;NHCH&sub2;CH&sub2;SPO&sub3;H&sub2;] ist zum Schutz gegen die cytotoxischen Effekte einer Bestrahlung eingesetzt worden. WR-2721 schützt ebenfalls die gesunden Zellen, wenn Krebs mit Chemotherapie behandelt wird.
- Es ist nun herausgefunden worden, daß WR-2721 anstelle von SOD eingesetzt werden kann, um die bei einer Reperfusion gebildeten sauerstofffreien Radikale abzufangen. WR-2721 kann entweder intravenös oder intramuskulär in derselben Weise wie SOD und in derselben Dosierung verabreicht werden.
- Zusätzlich ist nun gefunden worden, daß eine schnellere und länger andauernde Reperfusion und Thrombolyse erreicht werden kann durch die intramuskuläre Verabreichung einer Fängersubstanz für sauerstofffreie Radikale oder Hydroxylgruppen, z. B. Superoxiddismutase oder andere Proteine, z. B. Enzyme, die wie Fängersubstanzen wirken oder WR-2721 oder analoge Materialien zusammen mit einem die Absorption der Fängersubstanz verbessernden Mittel. Ebenfalls können Inhibitoren der Xanthinoxidase wie Allopyrinol und Alloxanthin intramuskulär verabreicht werden.
- Die vorliegende Erfindung betrifft eine Packung, die (1) eine Fängersubstanz für sauerstofffreie Radikale, welche sich bei einer Reperfusion in einem Säugetier gebildet haben, oder ein Mittel, welches gesunde Zellen vor cytotoxischen Effekten schützt, wenn ein Säugetier einer Bestrahlung oder Chemotherapie ausgesetzt wird, und (2) eine die Absorption der Fängersubstanz oder des Mittels verbessernde Substanz enthält, die aus Harnstoff, substituierten Harnstoffen, Hydantoin, 5-substituierten Hydantoinen, 5,5-Dialkylhydantoinen, 5,5-Diarylhydantoinen, Guanidin, Methylguanidin, Hydrazin, Alkylhydrazinen, Arylhydrazinen, Hydroxylamin, Alkylhydroxylaminen, Arylhydroxylaminen und nicht-toxischen Salzen davon ausgewählt ist.
- Ferner kann die intramuskuläre Verabreichung von SOD oder WR-2721 und verwandten Materialien vorgenommen werden, wenn diese Mittel als schützende Substanzen für gesunde Zellen bei der Behandlung von Krebs durch Bestrahlung oder Chemotherapie eingesetzt werden.
- Die intramuskuläre Verabreichung des vorliegenden Erfindungsgegenstands ist dort wichtig, wo ein Bedarf für eine kurze Ansprechzeit besteht, welche durch intravenöse Injektion nicht erreicht werden kann.
- Anstelle von WR-2721 können ebenfalls analoge Materialien der Formel
- H&sub2;N-R(NHR&sub2;)x-NHR&sub3;SPO&sub3;H&sub2; (I) eingesetzt werden, in der R&sub1;, R&sub2; und R&sub3; jeweils Alkylene mit mindestens zwei Kohlenstoffatomen, z. B. 2 bis 6 Kohlenstoffatomen, und x Null oder eine kleine Zahl, z. B. 1 bis 3, bedeuten. WR-2721 als eine der Verbindungen der Formel I ist S-aminotrimethylenaminoethylthiophosphat. Andere Verbindungen dieser Formel sind Saminoethylenaminoethylthiophosphat, Saminoethylenaminopropylthiophosphat und Saminoethylenaminoethylenaminoethylthiophosphat.
- Die Erfindung schließt das Verpacken der Fängersubstanz, des Inhibitors der Xanthinoxidase oder der zellschützenden Substanz mit dem die Absorption der Fängersubstanz, des Inhibitors oder der zellschützenden Substanz in das Blut verbessernden Mittel ein. Das Verbesserungsmittel ist vorzugsweise Hydroxylaminhydrochlorid. Es kann beispielsweise ein für Notfälle bekannter automatischer Injektionsapparat verwendet werden und das Verfahren umfaßt das Injizieren der beiden Medikamente in das Muskelgewebe.
- Erfindungsgemäß wird die Absorptionsrate von SOD, WR-2721 und anderen Fängersubstanzen und schützenden Substanzen in das Blut verbessert, indem mit der Dosierung der SOD, des WR-2721 oder anderer Fängersubstanzen oder Schutzsubstanzen eine Dosierung eines die Absorption von SOD, WR-2721 oder dergleichen verbessernden Mittels, vorzugsweise Hydroxylaminhydrochlorid vorgenommen wird. Vorzugsweise wird das die Absorption verbessernde Mittel wie Hydroxylaminhydrochlorid mit der Dosierung von SOD, WR-2721 oder anderen Fängersubstanzen oder Schutzsubstanzen vermischt, um eine einzige Dosierung zu bilden, die anschließend intramuskulär (i.m.) injiziert wird. Es liegt im Rahmen der Erfindung, daß das die Absorption verbessernde Mittel als eine separate Dosierung in dieselbe Stelle wie die separate Dosierung von SOD, WR-2721 oder anderen Fängersubstanzen oder Schutzsubstanzen (z. B. US-Patent 43 94 863) injiziert wird. Beispielsweise beträgt die Menge des die Absorption verbessernden Mittels, wie Hydroxylaminhydrochlorid, welches der Dosierung von SOD, WR-2721 oder anderen Fängersubstanzen oder Schutzsubstanzen wie zuvor beschrieben zugesetzt wird, um eine einzige vermischte Dosierung zu bilden, eine Menge von 0,25 bis 100 Milligramm pro Kilogramm Körpergewicht. Beispielsweise können bei Hunden und einer Dosierung von 5 mg/kg SOD 5 mg/kg Hydroxylaminhydrochlorid eingesetzt werden.
- Als das die Absorption verbessernde Mittel wird Hydroxylamin bevorzugt in der Form eines nicht-toxischen wasserlöslichen Salzes eingesetzt. Somit können beispielsweise anstelle von Hydroxylamin Salze wie Hydroxylaminhydrochlorid, Hydroxylaminhydrobromid, Hydroxylaminhydrojodid, Hydroxylaminsulfat, Hydroxylaminnitrat, Hydroxylaminacetat und Hydroxylaminpropionat eingesetzt werden. In besonders bevorzugter Weise wird Hydroxylaminhydrochlorid eingesetzt.
- Die erfindungsgemäßen die Absorption verbessernden Mittel werden ausgewählt aus Harnstoff, Mono- und Dialkylharnstoffen, z. B. Methylharnstoffen, Ethylharnstoff, Propylharnstoff, Butylharnstoff, N,N,-Dimethylharnstoff, N,N-Diethylharnstoff, N,N- Diisopropylharnstoff, Mono- und Dialkylharnstoffen, z. B. Phenylharnstoff, p-Tolylharnstoff, N,N,-Diphenylharnstoff, N,N-Di-ptolylharnstoff, Thioharnstoff, Hydantoin, 5-substituierten Hydantoinen, z. B. 5-Alkyl-, 5-Aralkyl- und 5-Arylhydantoinen und 5,5-Dialkyl- und 5,5-Diarylhydantoinen, z. B. 5-Methylhydantoin, 5-Ethylhydantoin, 5,5-Dimethylhydantoin, 1,5-Trimethylenhydantoin, 1,5-Tetramethylenhydantoin, 5-Phenylhydantoin, 5-p-Tolylhydantoin und 5,5-Diphenylhydantoin, Guanidin, Methylguanidin, Hydrazin, Alkyl- und Arylhydrazinen, z. B. Methylhydrazin, Ethylhydrazin, Butylhydrazin, Phenylhydrazin und Diphenylhydrazin, Alkyl- und Arylhydroxylaminen, z. B. Methylhydroxylamin, Ethylhydroxylamin und Phenylhydroxylamin. Die substituierten Harnstoffe, Hydrazine und Hydroxylamine können ebenfalls in der Form ihrer Salze wie z. B. als Hydrochloride eingesetzt werden.
- Das gleichzeitige Verabreichen von SOD, WR-2721 oder anderen Fängersubstanzen, Inhibitoren oder Schutzsubstanzen und eines die Absorption verbessernden Mittels ist nicht nur für die Anwendung beim Menschen beabsichtigt, sondern sie können erfindungsgemäß für tierärztliche Zwecke, z. B. anderen Säugetieren, z. B. Hunden, Katzen, Rindern, Kaninchen und Pferden verabreicht werden.
- Da es nicht notwendig ist, mit der SOD Katalase einzusetzen, kann gleichzeitig Katalase eingesetzt werden, beispielsweise um die Wirksamkeit von SOD noch länger auszudehnen, wie es von Werns et al. erwähnt wird.
- Ein weiterer Aspekt der Erfindung schließt ein Verfahren zur Behandlung eines Säugetiers wie z. B. eines der zuvor genannten mit einem flüssigen Medikament unter Bedingungen ein, wo intravenöses Injizieren nicht praktikabel ist, aber die kurze Ansprechzeit einer intravenösen Injektion wünschenswert ist. Dies wird erreicht durch intramuskuläres Verabreichen des Medikaments in flüssiger Form zusammen mit Hydroxylamin oder einem nichttoxischen Salz davon wie z. B. eines der zuvor genannten. Es können beispielsweise entweder eine einzige vermischte Dosierung oder separate Dosierungen in der oben beschriebenen Weise verwendet werden.
- Hydroxylamin oder das Salz davon kann in einer Menge von 0,25 bis 100 Milligramm pro Kilogramm Körpergewicht eingesetzt werden. Somit können beispielsweise zur Behandlung der Hyperglykämie einem 68 kg (150 Pfund) schweren Individium 1-2 mg Glucagon und 350 mg Hydroxylaminhydrochlorid intramuskulär injiziert werden. Ebenso kann einem 68 kg (150 Pfund) schweren Individium, welches Lidocain benötigt, 300 mg Lidocain und 350 mg Hydroxylaminhydrochlorid intramuskulär injiziert werden. Im Falle von Insulin können beispielsweise 21 Einheiten normalen Insulins zusammen mit 350 mg Hydroxylaminhydrochlorid injiziert werden.
- Das Allopurinol oder Alloxanthin kann intramuskulär, z. B. in einer Menge von 1-1000 mg, z. B. 250 mg, ohne Hydroxylamin oder einem Salz davon verabreicht werden. Jedoch wird es zur Verbesserung der Absorption des Inhibitors der Xanthinoxidase bevorzugt, das Hydroxylamin oder das Salz davon zuzugeben. Somit können 100 mg Allopurin zusammen mit 350 mg Hydroxylaminhydrochlorid intramuskulär injiziert werden.
Claims (11)
1. Packung, die (1) eine Fängersubstanz für sauerstofffreie
Radikale, welche sich bei einer Reperfusion in einem
Säugetier gebildet haben, oder ein Mittel, welches gesunde Zellen
vor cytotoxischen Effekten schützt, wenn ein Säugetier einer
Bestrahlung oder Chemotherapie ausgesetzt wird, und (2) eine
die Absorption der Fängersubstanz oder des Mittels
verbessernde Substanz enthält, die aus Harnstoff, substituierten
Harnstoffen, Hydantoin, 5-substituierten Hydantoinen, 5,5-
Dialkylhydantoinen, 5,5-Diarylhydantoinen, Guanidin,
Methylguanidin, Hydrazin, Alkylhydrazinen, Arylhydrazinen,
Hydroxylamin, Alkylhydroxylaminen, Arylhydroxylaminen und
nichttoxischen Salzen davon ausgewählt ist.
2. Packung nach Anspruch 1, in der die die Absorption
verbessernde Substanz Hydroxylamin oder ein nicht-toxisches Salz
davon ist.
3. Packung nach Anspruch 2, in der das nicht-toxische Salz von
Hydroxylamin Hydroxylaminhydrochlorid ist.
4. Packung nach den Ansprüchen 1 bis 3, in der (1) und (2)
jeweils in einer separaten Dosierungsform vorliegen.
5. Packung nach den Ansprüchen 1 bis 3, in der (1) und (2)
innerhalb einer einzigen Dosierungsform vermischt vorliegen.
6. Packung nach den Ansprüchen 1 bis 5, in der (1)
Superoxiddismutase ist.
7. Packung nach den Ansprüchen 1 bis 5, in der (1)
S-Aminotrimethylenaminoethylthiophosphat
ist.
8. Packung nach den Ansprüchen 1 bis 5, in der (1) ein
Inhibitor der Xanthinoxidase ist.
9. Packung nach Anspruch 8, in der (1) Allopurinol ist.
10. Packung nach Anspruch 8, in der (1) Alloxanthin ist.
11. Verwendung von Hydroxylamin oder einem nicht-toxischen Salz
davon zur Herstellung eines Medikaments zur intramuskulären
Verabreichung einer Fängersubstanz für sauerstofffreie
Radikale, welche sich bei einer Reperfusion gebildet haben, oder
eines Mittels, welches gesunde Zellen vor cytotoxischen
Effekten schützt, wenn ein Säugetier einer Bestrahlung oder
Chemotherapie ausgesetzt wird, und zur Verbesserung der
Absorption der Fängersubstanz oder des Mittels in das Blut.
