Es ist überraschend, daß durch eine in 3 Stufen abgestufte Dosierung die Hormonmengen und insbesondere
die östrogenmengen niedriger gehalten werden können als in den bekannten Präparaten, ohne daß
ίο die bei einer Mikrodosierung sonst üblichen Nachteile
einer schlechten Zykluskontrolle in Kauf genommen werden müssen. Bei der klinischen Prüfung des
Präparats für die neue 3stufige Verwendung zeigte es sich, daß neben der guten Toleranz und Akzeptabilität
auch in jedem Falle ein sicherer Konzeptionsschutz gewährleistet ist. Trotz monatelanger Anwendung
konnte ein physiologischer Aufbau des Endometriums und des Scheidenepithels beobachtet werden.
Hormonelle Methoden zur Kontrazeption sind bereits bekannt. Weit verbreitet ist die Verwendung von
Östrogenen und Gestagenen bei der oralen Kontrazeption. Außer Kombinationspräparate, die aus einer
gleichen Kombination von Östrogen und Gestagen für die tägliche Einnahme bestehen, gibt es auch schon
Zweistufenpräparate, die durchgehend die gleiche Östrogenmenge enthalten und von denen einige
Dosiseinheiten für die ersten Tage des Einnahmezyklus (1. Stufe) keine oder eine geringe Gestagenmenge und
einige Dosiseinheiten für die folgenden Tage des Einnahmezyklus (2. Stufe) eine gesteigerte Gestagenmenge
enthalten.
In der deutschen Offenlegungsschrift 22 18 831 werden Zweistufenpräparate beschrieben, die in 1. und
2. Stufe Gestagene und östrogene in niedriger Dosierung enthalten, wobei die Östrogene in beiden
Stufen in gleicher Höhe dosiert werden und die Dosierung der Gestagene von der 1. zur 2. Stufe
gesteigert wird. Zykluskontrolle und Verträglichkeit sind bei den Zweistufenpräparaten gemäß DE-OS
22 18 831 mit einem Östrogengehalt von täglich 0,05 mg Äthinylöstradiol sehr gut. Bei der Dosisreduzierung der
Östrogene bis auf 0,03 mg Äthinylöstradiol treten jedoch in verstärktem Maße Blutungsanomalien auf. Bei
längerer Einnahme kommt es zu störenden Oligo-Amenorrhoen.
Da nach wie vor ein starkes Interesse an sehr niedrig
dosierten Hormonpräparaten besteht, ist es die Aufgabe der Erfindung, eine kontrazeptive Verwendung aufzuzeigen
die weniger als 0,05 mg Äthinylöstradiol pro Tag (pro Dosiseinheit) erforderlich macht und gleichzeitig
eine gute Zykluskontrolle gewährleistet.
Die Aufgabe wird erfindungsgemäß gelöst durch Bereitstellung einer dreistufigen Hormonverwendung,
wobei von Stufe zu Stufe östrogene und Gestagene unterschiedlich dosiert werden. Völlig unerwartet wird
trotz Dosisreduzierung bis auf 0,03 mg Älhinylöstradiol eine bessere Zyklusstabilisierung erreicht als bei
entsprechenden Zweistufenpräparaten.
Die Erfindung betrifft somit die Verwendung von täglich
a) jeweils 0,03 mg 17<x-Äihinylöstradiol und
0,05 mg D-Norgestrel in 1. Stufe an
0,05 mg D-Norgestrel in 1. Stufe an
4-6 Tagen,
b) jeweils 0,04 mg 17cc-Äthinylöstradiolund
0,075 mg D-Norgestrel in 2. Stufe an
0,075 mg D-Norgestrel in 2. Stufe an
4 —6 Tagen und
für die Zusammensetzung eines Dragees je Stufe

bekannten Mehrstufenpräparaten aufgezeigt werden:

Frauen im fortpflanzungsfähigen Alter wurden in Zyklen mitjciem Prüfpräparat A,fc205. Frauen in
Zyklen mit dem Prüfpfäparat B und 187 Frauen in Zyklen mit dem Prüfpräparat C behandelt. Jede
Frau erhielt 21 Tage lang täglich 1 Dragee, die anschließenden 7 Tage, während der die Abbruchblutung
erfolgte, blieben einnahmefrei. Diese Verabreichungsform wurde über 12 Zyklen beibehalten. Die
meisten Frauen nahmen bis zum Ende an der Prüfung teil.
In der gesamten Behandlungszeit traten keine Schwangerschaften auf. Alle drei Präparate waren gut
verträglich. Bei dem Dreistufenpräparat A traten weniger Amenorrhoen, Schmier- und Durchbruchsblutungen
auf als bei den Zweistufenpräparaten B und C (siehe Tabelle).

Überraschenderweise ist die Zykluskontrolle bei A nicht nur besser als bei B mit 0,03 mg Äthinylöstradiol,
sondern audi besser als bei C mit 0,05 mg Äthinylöstradiol.
