神经变性疾病是一组以中枢神经系统神经元逐渐发生变性所导致的神经系统疾病或神经病症,如帕金森病、阿尔茨海默病、亨廷顿病、肌萎缩侧索硬化症、橄榄桥脑小脑萎缩等。其病因大多不明,发病机制复杂,临床表现各异。该组疾病多见于中老年,呈慢性或亚急性起病,进行性加重,病程较长,且多数疾病和病症之间相互重叠,如10%左右的帕金森病合并痴呆。除少数疾病(如帕金森病)依靠药物或手术可暂时改善临床症状外,对大多数疾病目前尚无特效的治疗方法。随着我国人口逐渐趋向老年化,寻找合适的药物预防和治疗神经变性疾病迫在眉睫。
胰岛素样生长因子-1(insulin-like growth factor-1,IGF-1)是一种结构和功能都与胰岛素类似的多肽类神经营养因子,在体内具有胰岛素样作用,发挥着多种生理功能。它通过与中枢和周围神经系统广泛分布的IGF-1受体(IGF-1R)特异性结合,对多种神经元起着神经营养保护作用,在大脑发育和维持成年大脑功能方面起着重要作用。近年的研究证实,人类多种疾病的发生、发展与IGF-1信号转导通路密切相关,尤其在神经变性疾病的发生、发展过程中起着非常重要的作用。临床研究表明,随着机体的老化血清IGF-1水平将逐渐降低,阿尔茨海默病、肌萎缩侧索硬化症、小脑变性等多种神经变性疾病的发生都伴有机体IGF-1水平的降低。因此,以IGF-1为靶点的神经变性疾病的预防和治疗日益受到国内外学者的重视,与之相对应的临床研究相继开展。近十年来,国内外学者已在临床尝试用重组IGF-1治疗神经变性疾病,但由于价格昂贵、而且直接应用IGF-1还可能诱发肿瘤等原因,使IGF-1应用于神经变性疾病的治疗受到一定限制。为此,试图通过药物,尤其是通过中药或中药活性部位对机体分泌合成IGF-1的调控作用以干预神经变性疾病的发生、发展是一种新的治疗途径,并将有助于了解神经变性疾病的发病机制。
黄芪(astragalus membranaceus,AM),为豆科植物蒙古黄芪和膜荚黄芪的干燥根,性味甘、微温,归脾、肺经。黄芪是我国传统的补益类中药,已被证明可促进DNA、RNA、蛋白质的合成。
黄芪多糖(astragalus polysaccharides,APS)为黄芪的主要活性部位,为类白色无定形粉末,无臭,无味,其主要成分为α-1,4(1,6)葡聚糖、阿拉伯-半乳多糖、鼠李-半乳糖醛酸多糖和阿拉伯-半乳蛋白多糖等,平均分子量为2~6万。在临床应用中表现出促进物质代谢、免疫调节等作用,目前主要用于免疫调节和癌症病人放疗和化疗后的辅助治疗。
发明内容
本发明的目的是提供黄芪或黄芪多糖在制备抗神经变性疾病的药物中的应用。神经变性疾病包括老年性痴呆(阿尔茨海默病和血管性痴呆)、亨廷顿病、肌萎缩侧索硬化、小脑性共济失调等疾病。
本发明是基于黄芪或黄芪多糖具有调控机体IGF-1分泌与表达的作用,因此还可在制备其他与IGF-1信号转导障碍有关疾病(如糖尿病、骨质疏松症等)药物中的应用。
本发明制备的药物制剂形式包括液体制剂、固体制剂。所述制剂的给药形式包括口服给药或注射给药。
本发明的有益效果是:
(1)本发明对传统中药黄芪的药理作用进行二次开发,拓展了黄芪的药用范围。
(2)本发明证实黄芪和黄芪多糖皆具有调控机体IGF-1分泌与表达的作用,可以将两者作为治疗神经变性疾病的IGF-1信号转导障碍及其他IGF-1信号转导障碍有关疾病的药物。人类许多疾病与IGF-1信号转导障碍有关,黄芪和黄芪多糖将在这些疾病的预防和治疗中得到重要应用。
(3)本发明以IGF-1信号转导理论为背景,研究并阐明中药及其主要活性部位作用和机制,是发展中医药理论新的重要方向,为发现新的理论和药物作用新的靶点提供重要依据。

具体实施方式
本发明结合具体实施例和附图作进一步说明。应理解,这些实施例仅用于说明目的,而不用于限制本发明范围。
实施例1
黄芪和黄芪多糖对橄榄小脑变性大鼠的神经保护作用及其对机体IGF-1分泌与表达的调控作用
1.黄芪和黄芪多糖对橄榄小脑变性大鼠的神经保护作用
实验药物:
黄芪水煎液的制备:将黄芪(购自北京同仁堂杭州分店)切碎,加3倍量水,水浸1h,加热回流,微沸40min,过滤,取药渣再次加热回流,如此重复3次,合并滤液,旋转蒸发仪低温浓缩滤液,使药液终浓度达含生药2g.ml-1,4℃保存备用。
黄芪多糖粉针剂由天津赛诺制药公司生产。
实验方法:
(1)橄榄小脑变性大鼠模型的制备
健康SD系雄性大鼠,体重(160±10)g。在实验过程中大鼠均被饲养在12h光照、12h黑暗、温度21~22℃,湿度50%~70%的标准环境中、自由饮水摄食。经饲养3天后,依据文献所用方法[8],每只大鼠一次性腹腔注射3-acetylpyridine(Sigma公司)(50mg.kg-1),正常对照组以同样方法注射相等剂量的生理盐水。
(2)分组及给药方案
将大鼠随机分组,造模后立即给药:黄芪高剂量组18g.kg-1、中剂量组9g.kg-1、低剂量组3g.kg-1(分别相当于60kg成人每日用量的2倍、1倍、0.33倍),每组10只;黄芪多糖高剂量组50mg.kg-1、中剂量组25mg.kg-1、低剂量组12.5mg.kg-1(分别相当于60kg成人每日用量的2倍、1倍、0.5倍),每组8只;并行设立正常对照组,模型对照组。黄芪水煎液给药组灌胃给药,黄芪多糖给药组尾静脉注射给药,模型组和正常组给予等容量的生理盐水,每日1次,连续给药4周。末次给药24小时后,对黄芪水煎液和黄芪多糖各剂量组大鼠测试运动协调能力,断头取小脑,液氮保存备用。
(3)斜板试验测试橄榄小脑变性大鼠运动协调能力
连续给药4周,末次给药24h后采用斜板试验,将大鼠置于一块长90cm,宽80cm的有纹理橡胶皮覆盖的斜板上,斜板倾斜角度从0°缓慢上升,直至大鼠不能保持原有位置5s时,读取斜板度数。每例分别测试3个方向,头朝上身体与水平面垂直,头朝左身体与水平面平行,头朝右身体与水平面平行,取3次成绩之均值作为最终度数。
(4)大鼠下橄榄核神经元存活率检测
组织切片及HE染色
连续给药4周,末次给药24h后,每组4只动物用10%水合氯醛(350mg.kg-1)麻醉,迅速开胸暴露心脏,0.9%NaCl经心快速灌注约100ml,再缓慢灌注固定液(4%多聚甲醛/0.1M的PB,PH7.4)约400ml至肝脏褪色变硬,四肢、尾变硬,断头取脑干,按《大鼠脑立体定位图谱》[9]对下橄榄核定位:按前囟定位,即以前囟为嘴尾侧0点,由前囟往嘴侧为+,往尾侧为-,下橄榄核定位为-3.0~-5.2mm,即位于前囟往尾侧3.0~5.2mm处。取组织并置于前述固定液中,4℃后固定3h,再经常规石蜡包埋,连续冠状切片,制成8μm厚的石蜡切片,行HE染色(苏木素-伊红染色),常规光镜检查。
免疫组化法定性分析下橄榄核神经元Calbindin D 28K(CaB)蛋白表达
将上述石蜡切片用二甲苯脱蜡,水化,后续免疫组化步骤按北京中杉公司PV-6001免疫组化检测试剂盒中提供的操作说明进行,DAB显色,苏木素复染,梯度乙醇脱水,透明、中性树胶封片。一抗采用抗鼠CaB单克隆抗体(1∶1000稀释),阴性对照组用PBS代替特异性一抗,以细胞浆呈棕黄色为阳性,光镜下观察并拍照。
结果
(1)黄芪/黄芪多糖改善橄榄小脑变性大鼠的运动协调能力
斜板试验结果参见图1:(□)黄芪多糖各剂量组、(■)黄芪各剂量组,*P<0.05;***P<0.001vs.模型组(黄芪各剂量组,n=10;黄芪多糖各剂量组,n=8,Dunnett’s t test)。结果表明,与模型组相比,黄芪、黄芪多糖给药组斜板试验成绩明显提高,并呈剂量依赖性。黄芪低剂量组(3g.kg-1)和黄芪多糖低剂量组(12.5mg.kg-1)提高作用不明显(P>0.05);而黄芪中剂量组(9g.kg-1)和黄芪多糖中剂量组(25mg.kg-1)分别提高了20.5%和25.3%;黄芪高剂量组(18g.kg-1)和黄芪多糖高剂量组(50mg.kg-1),分别提高了52.9%和53.9%,用药组与模型组之间有显著差异(P<0.05)。表明黄芪和黄芪多糖对橄榄小脑变性大鼠运动协调能力皆有一定的改善作用。虽然黄芪多糖的提高能力比黄芪强,但两者无显著差异(P>0.05)。
(2)黄芪/黄芪多糖促进橄榄小脑变性大鼠下橄榄核神经元存活
黄芪/黄芪多糖促进下橄榄核神经元存活的组织形态学观察
结果参见图2.1:(A)为正常对照组;(B)为模型组;(C)为黄芪中剂量组(9g.kg-1);(D)黄芪多糖中剂量组(25mg.kg-1);(E)黄芪高剂量组(18g.kg-1);(F)为多糖高剂量组(50mg.kg-1)。箭头所示为空泡。正常组下橄榄核细胞密集排列,其神经元的胞体多为中、小型圆形或梨形;胞核呈圆形或椭圆形,核膜清晰,核基质均匀;核仁明显,清楚,浓染;细胞质分布均匀;细胞膜边界清楚。神经毒性物质3AP可造成下橄榄核损毁,由模型组可见下橄榄核神经元核膜完全解体,胞核、胞质严重空泡化,典型坏死的结构性改变。给药组较模型组下橄榄核细胞有明显改变:黄芪中剂量组(9g.kg-1)和黄芪多糖中剂量组(25mg.kg-1)空泡显著减少,但部分胞体呈三角形,大部分核基质不均匀,浓染,核仁不清楚;黄芪高剂量组(18g.kg-1)和黄芪多糖高剂量组(50mg.kg-1)未见空泡,胞体圆形或梨形,大部分核膜清晰,核基质均匀,可见核仁,且黄芪多糖的改善作用比黄芪强。
黄芪/黄芪多糖提高下橄榄核神经元Calbindin D 28K(CaB)蛋白的表达
CaB为目前广泛应用的下橄榄核神经元的特异性标记物[10],采用直观的免疫组化法,以细胞内显示棕褐色的强弱来判断细胞内CaB表达量的变化。结果显示,模型组无明显染色,提示下橄榄核神经元几乎没有存活,与模型组比较,黄芪中剂量组(9g.kg-1)和黄芪多糖中剂量组(25mg.kg-1)即表现为较强的染色,下橄榄核神经元存活率明显增加。正常组染色明显。免疫组化结果参见图2.2:(A)为正常对照组;(B)为模型组;(C)黄芪中剂量组(9g.kg-1);(D)黄芪多糖中剂量组(25mg.kg-1);(E)黄芪高剂量组(18g.kg-1);(F)多糖高剂量组(50mg.kg-1)。
2.黄芪提高橄榄小脑变性大鼠血清IGF-1含量的时效与量效
实验方法
(1)采用ELISA法定量分析血清IGF-1含量
将黄芪水煎液给药的大鼠于给药2周及4周后断尾取血,4℃5000rpm×15min离心分离血清。采用双抗体夹心ELISA法分析血清IGF-1浓度,严格按照试剂盒中说明操作,450nm处测定吸光度,定量分析血清中IGF-1的含量。
实验结果
与模型组比较,给药二周后,黄芪中(9g.kg-1)、高(18g.kg-1)剂量组血清IGF-1浓度分别上升了9.7%(P<0.05)和23.8%(P<0.001);给药四周后分别上升了15.1%(P<0.01)和28.6%(P<0.001),低剂量组皆无显著差异。以上结果显示,黄芪对橄榄小脑变性大鼠血清IGF-1含量具有明显的提高作用,且呈剂量依赖性和时间依赖性。参见表1。

3.黄芪多糖提高橄榄小脑变性大鼠IGF-1的表达和分泌
实验方法
(1)RT-PCR法分析小脑IGF-1 mRNA的表达
将黄芪多糖给药的大鼠于给药4周断头取小脑,各剂量组小脑按TrizolRNA提取试剂盒的说明,提取小脑总RNA,在M-MLV逆转录酶作用下,制备cDNA。在Taq酶的作用下,进行IGF-1基因片断的扩增。扩增程序为94℃变性50s,65℃退火50s,72℃延伸70s,30个循环。所得的扩增产物经1.5%的琼脂糖凝胶电泳,100V,30min。凝胶成像系统(Bio-Rad公司)成像,用Quantity One软件定量分析产物条带的光密度,以IGF-1与β-actin的相对光密度为观察指标。实验所用IGF-1的引物序列[11],上游引物:5’-AAG CCT ACA AAG TCA GCTCG-3’;下游引物:5’-GGT CTT GTT TCC TGC ACT TC-3’,产物114bp;内参β-actin的引物序列[51],上游引物:5’-TGA CGG GGT CAC CCA CAC TGT GCCCAT CTA-3’;下游引物:5’-CTA GAA GCA TTT GCG GTG GAC GAT GGA GGG-3’,产物660bp。
(2)Western Blot法定量分析小脑IGF-1蛋白的表达
用RIPA缓冲液[50mmol.L-1 Tris-HCl(PH8.0),150mmol.L-1 NaCl,0.1%SDS,1% sodium deoxycholate,1% Triton-X100,1mmol.L-1 EDTA,1mmol.L-1PMSF and 10μg.ml-1Leupeptin]裂解小脑组织,4℃静置30min,离心13000rpm×30min,提取总蛋白。DC Protein Assay试剂盒测定各组蛋白浓度。经18%SDS-PAGE电泳后,蛋白条带用180mA,40min转移至PVDF膜上(millipore)。膜分别用含5%脱脂奶粉的TPBS[0.01MPBS,0.05%Tween20,PH7.4]室温封闭1h;羊抗鼠IGF-1多克隆抗体(1∶500稀释),4℃孵育过夜;辣根过氧化物酶标记的二抗室温孵育1h(1∶5000稀释)。经TPBS洗膜后采用ECL法检测IGF-1的蛋白条带。采用同样的方法,使蛋白与抗Actin的抗体反应,检测Actin的表达量,作为内参。采用Quantity One软件对目标条带的光密度进行定量分析,以IGF-1与Actin的相对光密度为观察指标。
(3)ELISA法定量分析血清和小脑IGF-1含量
ELISA法定量分析血清IGF-1含量
黄芪多糖给药4周后大鼠断尾取血,ELISA法定量分析血清IGF-1含量,方法同2黄芪水煎液给药的大鼠。
ELISA法定量分析小脑IGF-1含量
取100mg小脑组织,按重量体积比1∶3加脑组织匀浆缓冲液[137mmol.L-1NaCl,20m mol.L-1Tris,1%NP40,10%glycerol,0.5mmol.L-1sodiumvanadate,1mmol.L-1PMSF,10μg.ml-1aprotinin,1μg.ml-1leupeptin],冰浴匀浆,4℃13000rpm×15min离心取上清,用DC Protein Assay试剂盒测定总蛋白含量,随即采用双抗体夹心ELISA法分析上清液中IGF-1浓度,严格按照试剂盒中说明操作,450nm处测定吸光度,定量分析小脑组织中IGF-1的含量。
实验结果
(1)黄芪多糖提高橄榄小脑变性大鼠小脑IGF-1 mRNA的表达采用Quantity One分析软件对IGF-1 mRNA表达量进行定量分析,结果显示:黄芪多糖低剂量(12.5mg.kg-1)无明显作用(P>0.05)。黄芪多糖中剂量(25mg.kg-1)组大鼠小脑IGF-1 mRNA的表达量上升为正常对照组的74.3%,高剂量组(50mg.kg-1)上升为正常对照组88.1%,呈剂量依赖性,与模型组比较均存在显著性差异(P<0.01)。结果参见图3:**,P<0.01,***,P<0.001vs.模型组(Dunnett’s t test)。
(2)黄芪多糖提高橄榄小脑变性大鼠小脑IGF-1蛋白的表达
采用Western blot方法检测小脑内IGF-1蛋白的表达量。结果表明,与模型组相比,黄芪多糖中(25mg.kg-1)、高(50mg.kg-1)剂量组小脑IGF-1表达有一定增强。结果参见图4。
(3)黄芪多糖提高橄榄小脑变性大鼠血清和小脑IGF-1水平
实验结果表明,与模型组比较,黄芪多糖低剂量(12.5mg.kg-1)组血清IGF-1含量上升了9.4%;小脑IGF-1含量上升了8.3%,皆有显著差异(P<0.05),随着药物剂量的增高血清和小脑内IGF-1含量依次上升,黄芪多糖中剂量(25mg.kg-1)组分别上升了18.9%(P<0.001)和14.7%(P<0.001);黄芪多糖高剂量(50mg.kg-1)组分别上升了32.6%(P<0.001)和22.1%(P<0.001)。提示,黄芪多糖对橄榄小脑变性大鼠血清和小脑IGF-1含量皆有明显的提高作用,并且该作用呈剂量依赖性。结果参见表2。

实施例2 黄芪水煎浓缩液纳米乳胶丸
处方:黄芪200g,加水1000ml
制备:将黄芪切碎,水浸1h后,加热回流,微沸40min,过滤,取药渣再次加热回流,如此重复3次,合并滤液,旋转蒸发仪低温浓缩滤液,使药液终浓度达含生药2g.ml-1,等量加入半干固体的复合辅料,按常规方法制备成软胶丸,每丸含生药1g。
这种复合辅料一旦与水或胃肠液相混合就会迅速和自发地形成纳米大小的水包油乳液,药物就溶解在这些极小的油滴中,并迅速分散到胃肠道中的吸收部位。在整个吸收过程中由于药物始终保持分子状态,而且这些极小的含药油滴能增加其在肠道上皮细胞的吸收,延长吸收时间,增加药物通过淋巴系统的转运和通过肠道Payer′s区M细胞吞噬进入体内循环,避免首过消除。从而提高药物生物利用度,和增加了药物的靶向性。
实施例3 黄芪多糖微球注射液
用缓释聚合物(Polycaprolactone,PCL)研磨黄芪多糖,制备成的熔融混合液用生物相容性和生物降解性聚合物制成微球注射液。每支注射液含黄芪多糖500mg。
所用的聚合物释药后水解为对人体无害的降解物(水和二氧化碳)。该微球注射液静脉注射后,在循环中缓慢释放药物,可以降低肝脏对黄芪多糖的摄取,使作用时间延长。
实施例4 制备黄芪水煎浓缩液纳米乳胶丸
将黄芪切碎,加3倍量水,水浸1h,加热回流,微沸40min,过滤,取药渣再次加热回流,如此重复3次,合并滤液,旋转蒸发仪低温浓缩滤液,使药液终浓度达含生药2g.ml-1,等量加入半干固体的复合辅料,按常规方法制备成软胶丸,每丸含生药1g。

实施例5 制备黄芪多糖微球注射液

5.根据权利要求1-4任一所述的黄芪或黄芪多糖在制备抗神经变性疾病的药物中的应用,其特征是:制备的药物制剂形式包括液体制剂、固体制剂。
