技术领域
本发明涉及药物释放系统;更具体而言,本发明涉及用于人或其他动物的小型可植入药物释放装置。
背景技术
在人体或动物体内会发生许多病变或疾病,这些病变或疾病对一种或多种药物治疗产生有效反应。对于多种此类疾病和病变,可通过口服给药进行治疗。当吞服后,药物通过胃肠系统最终到达病变或疾病部位。然而,在许多情况下,药物是通过血流到达病变或疾病部位。特别是在用注射器将药物注射入肌肉或软组织中,然后再通过血液流动到达病变或疾病部位的情况下。还有一些情况是(通常是在健康护理机构)采用静脉滴注的方式使药物直接进入血管。在另外一些情况下,常用一些外科手术在身体内的病变或疾病部位上或附近放置特定的药物。
申请人发现,通过采用集成电路技术可制造出即可装载又可释放药物到人体病变或疾病部位的小型药物释放系统。这种小型药物植入装置的示例公开在如下的美国专利中:美国专利号5,770,076;美国专利号5,797,898;美国专利号5,985,328;美国专利号6,123,861和美国专利号6,331,313。许多这种小型药物植入装置的结构非常复杂,制造起来既困难又昂贵。因此,在该领域需要一种简便、低成本、易于制造的可植入的小型药物释放装置,该装置可植入人体或动物体内从而将药物释放到各部位。
发明内容
本发明的简便、低成本、易于制造的可植入型药物释放装置使得该装置可被植入体内并将药物释放到各个部位。在此公开的释放系统包括至少一个盘(basin)、内孔(well)或空穴(open space)。所述盘、内孔或空穴被框在、附在、围在或塑形在核体(core body)内。核体内或盆壁部分内的盘、内孔或空穴要大到足够容纳长期体内治疗慢性病变或疾病所需的药物量。在眼睛内发生的这种已知的慢性病变或疾病具有典型性。
在用核体包围的盘、内孔或空穴上面、下面或者上下两面用一个滤网(screen)罩住。这个滤网用于控制药物或者置于盘、内孔或空穴内的片剂、粉剂或结晶浆液剂中的药物的释放或移动进入人或动物体内。滤网中孔的数量、大小、位置以及它们的排列随盘、内孔或空穴中药物溶解性、药物的溶出速率、药物浓度以及药物形式(为片剂、粉剂、结晶浆液或它们的组合)而发生改变。
当将一种或多种药物放置于盘、内孔或空穴中,且盘、内孔或空穴被滤网覆盖时,那么可将所述药物、含有盘的核体以及滤网整体植入体内。如对于发生在眼睛内的病变或疾病,一种方法是通过巩膜部分将所述的药物释放系统放入眼内。当将所述的药物装置恰当地放置于指定位置时,可以通过各种方法将该装置固定于该位置,包括用缝合线穿过核体上形成的孔。
当体内液体经滤网孔移至盘内时,盘、内孔或空穴中的药物随之发生扩散。
液体经滤网的流动使盘内的药物开始溶出。然后溶出的药物经过滤网上的孔缓慢地释放出去,只要所述药物释放装置中盘内的药物量足够,该装置便可持续释放药物从而治疗病变或疾病。

具体实施方式
在如下优选和备选的实施方案的说明中,采用附图标记以便于说明本发明。
在整个说明书中,个位和十位数指每个实施方案的同一部分。百位和千位数指备选实施方案。
如图1所示,本发明的实施方案为一小型可植入药物释放系统10,该系统用于治疗眼内感染性病变或疾病。这种疾病包括但不限于ARMD(年龄相关性黄斑变性)、PDR(增殖性糖尿病视网膜病)、新生血管性青光眼、局部缺血性及医源性视网膜病、眼后炎症(posterior ocular inflammation)及视网膜水肿。
尽管本发明的优选实施方案为眼内疾病的治疗,但是,本领域普通技术人员可以理解,本发明可以在动物体内的任何位置,使用本发明的小型药物释放系统中所含的药物来治疗疾病或病变。
除了眼睛疾病的治疗外,本发明的药物释放装置也可放置于男性前列腺附近,用于治疗前列腺癌或者良性前列腺增生。通过应用所述装置,通常的伴随前列腺癌治疗的副作用(如潮热、变声或乳腺增大)能够明显地减少或消除。另外,本领域普通技术人员还可以理解,治疗疾病或病变所需药物的量能明显地减少,因此减轻了患者的负担,并且因为有效地消除了频繁给药的需要,所以患者对治疗方案的依从性获得提高。从而药师的工作量以及接触危险或有毒药品的机会也将减少。药品-药品或药品-食品之间交互作用的机会将有效避免。并且将增加药物联合治疗的机会。
如果用所述装置代替植入的避孕用具植入女性体内,那么也能获得相同的益处。其他潜在的应用包括用抗真菌药物治疗阴道真菌感染。
也可将帕金森氏病患者作为候选者,将所述药物释放装置植入脑内缓慢地释放药物用于减少震颤。在溃疡性结肠炎或各种不同类型的胃肠疾病患者的GI肠道内植入所述装置也能使病情得到缓解。
如图2A所示,所述药物释放系统10包括盘14。在盘的围部分、框部分或套部分12中形成盘。为了便于简化说明,盘的围部分、框部分或套部分称为核体12。在优选的实施方案中,核体12由基本平的硬性材料形成。在图2A中所示的盘14具有一个顶16和一个底18。盘14的顶16横断核体12的上表面20。在优选的实施方案中,盘14的底18横断核体12的下表面22。所画的盘14恰恰为贯串核体12的孔。如果顶16大于底18,则盘14有一锥形壁17。
参考图2B,可以更好地理解核体12的构造。在此,所示的核体12有一个改进的弓形的外周。此处的盘14从上表面20到下表面22完全贯串且周边约与核体外周边平行。为了更好地理解小型药物释放系统,假定核体12长约为9.5mm且宽约为5.3mm。
如图2B所示,盘14位于核体12的中心或者靠近核体12的一端。盘14的大小要足够容纳各种不同的药物。这些药物包括直接或间接为神经保护剂、抗氧化剂、抗细胞凋亡剂、可溶性生长因子激动剂或拮抗剂、抗增生剂、抗血管生成剂、抗水肿剂、血管定位剂(vascular targeting agent)、抗炎药或抗生素的药物,无论它们是小的有机分子或生物制剂(如蛋白质、核酶、抗体、抗体片段、aptameters)还是低聚核苷酸。尤其,适当的药物包括但不限于信号转导抑制剂、蛋白激酶拮抗剂、酪氨酸激酶拮抗剂、VEGF受体拮抗剂、整联蛋白拮抗剂、基质金属蛋白酶抑制剂、糖皮质激素、NSAIDS、COX-1和/或-2抑制剂以及类固醇型血管抑制剂。每种这些药物均可为粉剂、结晶浆液形式或片剂形式。根据疾病的类型和严重程度,这些药物的量应足够在预定时期内治疗所述疾病。如果需要,可向药物内加入各种不同的添加剂以增加它的药效。如为了增加与水的亲合性,可向药物内加入如赋形剂-湿润剂以将水分子吸引到植入眼睛内的装置区以便于启动或增加药物的溶出或者将药物转运出盘14。
为了使盘14内小气泡的形成减少到最小,最好盘14的大小和形状与置于盘14内的药物的大小和形状基本相同以减少空余的空气量。
如图2C所示,所形成的第一滤网24与核体12的上表面20相连从而遮盖了盘14的顶16。第二滤网26任选与核体12的下表面22相连从而遮盖了盘14的底18。在优选的实施方案中,滤网24如图2C所示。具体而言,有改进的弓形轨迹的滤网24的周边36约平行于盘14的周边和核体12的周边。部分滤网23中所形成的孔28的厚度等于周边25的厚度或者根据装置10构造的不同而有不同的厚度。
如图2D所示,所形成的滤网24或者滤网24、26含有许多孔28。在滤网24或者滤网24、26中的孔28的数量、大小、位置以及它们的排列方式根据盘14内药物溶解性、盘14内药物溶出速率以及盘14内药物浓度等各种不同因素而定。但是,典型的孔28的大小大体上约为0.2微米-100微米之间。然而,因为本发明的多功能性,在实际应用中,也可用不同大小的孔。尽管可以理解在滤网24或者滤网24、26表面基本均匀地分布孔28能够最大限度地使药物溶出,但是,如下所说明的,不均匀分布孔28的情况也是可行的。另外,本领域普通技术人员应明白,当本发明药物释放系统在眼睛内应用时,所述孔必须小到能阻止任何不溶性的防碍视力的药物粒子的通过。
根据将要给予植入的靶点的药量的不同,每个滤网的适当厚度约为0.05mm-0.5mm,且核体12的适当厚度为约0.5mm-约3.0mm,优选约1.0mm-约2.0mm。
当所述药物释放装置应用金属或非金属材料制造时,可用各种不同的粘合剂(包括硅酮胶、氰基丙烯酸酯、或者通常可获得的的生物兼容性常温粘合剂、热粘合剂、环氧或紫外光熟成的粘合剂),将滤网24或滤网24、26连接于核体12上。在优选的实施方案中,所形成的滤网24或滤网24、26象核体12一样基本上是平的。
可用各种不同的材料制造核体12和滤网24、26。这些材料可选自各种不同具有生物兼容性的材料,包括硅、玻璃、红宝石、蓝宝石、钻石或陶瓷。如果需要,具有生物兼容性的金属也可用于制造核体12和滤网24、26。这些具有生物兼容性的金属包括金、银、铂、不锈钢、钨和钛。当应用具有生物兼容性的金属时,可用各种不同技术(包括如前述的激光焊、电热焊或者如上述的胶和粘合剂)将滤网24或滤网24、26焊接到核体12上。
本领域普通技术人员应明白,所述药物释放平台的效力由在预定时间内所释放的药物分子的适当数量来决定。因此,滤网中孔28的总面积必须能够满足盘14中药物释放率的要求。通常来讲,称为孔的密度。在本发明中,孔的密度为孔的总面积除以装置的总表面积,甚至包括滤网未覆盖的面积。
滤网表面的孔的数量、大小、位置以及整体外观形成孔的格局。调整孔的格局以确保所需量的药物以所需的速率释放。当核体含有多个盘时,可在滤网上用多种孔的格局控制药物的流动。例如,在盘的一端带有高密度孔的孔的格局最初使药物快速流动。但是当药物快用完后,药物将经过漫长的旅行才能离开盘14;因此,离开药物释放盘14的药物释放率将随着时间而递减。
在如图3所说的备选方案110中,盘114的底118本身为滤网,因此避免了应用和装配如图2A所示的第二滤网26的需要。在其他如图3所示的应用装置中,盘114也可有一个实心的底。当盘114有一个实心的底时,将唯一的滤网124用于盘114的顶116上。
当优选的实施方案为具有改进的周边轨迹36且盘14靠近远端15而缝合孔位于近端13时,本领域普通技术人员应明白,许多其他样式的可植入的药物释放平台也是可能的,并且包括在本发明范围内。例如,药物释放平台可具有多个缝合孔30或者具有如三角形310的直角边,如图4A所示。或者应用正方形、环形、马蹄形或者任何其他能够通过眼睛的小切口植入或者放置在药物最起效的身体部位的合适形状。
因为本发明可被成形为各种不同形状的适应性,尤其是在核体和滤网由生物兼容性金属制成的情况下,所以,所述装置可被制成环形或圆柱形,如图4B所示。当以该种方式成形时,所述装置可以置于肌腱、韧带、肌纤维、血管、神经束或者任何其他能够对局部给药有反应的身体部位。同样地,这种圆柱体380也可放置于体内的管道系统内,利用膨胀或缝合将其固定。
如果需要,可将不同类型的药物放置于有不同形状或不同颜色的不同核体内。在不同形状或不同颜色的核体内使用不同药物可减少药物的混淆,使得外科医生可通过装有药物的装置的形状或颜色区分药物。
在某些情况下,在治疗的病变或疾病时,可能需要将药物释放装置置于特定部位。在这种情况下,可如图5所示将药物释放装置410连接于支撑件432上使核体412位于理想的方位上。
在其他如图6A和6B所示的药物释放平台510的实施方案中,将核体512、上层滤网524及底盘525加工形成一个锋利的边缘534。如图2A、3和4A所示,尽管扁平的装置适于植入眼睛的玻璃体内,但是其他应用装置可能也能满足穿过软组织的要求。当形成核体512以使核体512的周边536的一部分形成锋利边缘534时,核体512本身可用于形成最初的切口或用于扩大放置药物释放装置510的切口。在这种实施方案中,上层滤网524的前缘作为锋利边缘的一部分用于形成植入所述药物释放装置的最初切口。
在另一个备选的实施方案610中,如图6C和6D所示,锋利边缘可处于盘614下方的实体件(solid piece)625的边缘部分。
虽然可在核体的盘内放置充足的药物来在最小或较长的时期内治疗病变或疾病,但是如果病变或疾病特别地顽固就可能需要替换药物。虽然整个装置可被替换,但是申请人发现,如图7所示,当患者体内的装置已用了一个较长的时期(如一年或更长)时,核体712内的盘714可通过从核体712的周边736到盘714的通道再填充。因为核体712的尺寸小,不可能从周边736钻一个通道到盘714。在这种情况下,可能需要由两个匹配构件712A、712B形成核体712,每个匹配的构件包括一部分通道。当将匹配件712A和712B连接时,这两部分通道水平或垂直连接到一起,形成一个从周边736到盘714的小孔738。
在另一个实施方案810中,通道838的尾端连接于突出部分840。突出部分840有利于用注射器(未显示)将药物从切口部位再填充入盘814中。另外,突出部分840的应用可防止或减少污染。突出部分840位于核体812侧面或者一小段距离内并且通过小管842连接。如图7A所示,通道838可形成“之”字形通道。
在复杂情况下,可能需要在病变或疾病位置附近放置多种药物。在这种情况下,在核体943、945内可形成多个盘。如图8所示,核体912包括至少一个近端的较小盘943和一个远端的较小盘945。因为药物不同,所以,可能需要用滤网覆盖近端盘943和远端盘945,所用的滤网在近端盘943和远端盘945上具有不同数量和不同大小的孔。
图9-23所示为所述药物释放平台的其他实施方案,在如下进行说明。
在图9所示的药物释放装置1010的实施方案中,药物释放装置包括一个近端盘1043、中间盘1045和一个远端盘1047。用第一隔板1041将近端盘1043与中间盘1045分开,且用第二隔板1049将中间盘1045与远端盘1047分开。如果需要,可在隔板1041、1049上形成通道1048以有利于药物在不同小盘间的转运或移动。也应注意到,所形成的核体1012上表面1020上的第一滤网1024和核体1012下表面1024上的第二滤网中的孔的格局是用于控制药物的释放。尤其是在远端1015有一个U形图案1050以及在近端1013有弓形图案1052。当在一个动物体内需要放置三种药物M1、M2和M3时,这种实施方案就特别有用。或者,隔板1041和1049为实心的或不能渗透的以保持药物M1、M2和M3的分离。
图10说明了用于体内的两种药物M1和M2分散的实施方案1110。因此,用实心或不能渗透的隔板1141将核体1112分隔为近端的较小盘1143和远端的较小盘1147。由位于远端隔板1147上的上层滤网1124和下层滤网1126中的U形图案的滤网孔1150控制药物M2的释放。由位于近端盘1143上的梯形图案滤网孔1154控制药物M1的释放。
在图11中,显示了另一个具有两个分割区的可植入的药物释放平台。注意:为了使药物M2和M1分离,要有一个覆盖整个远端盘1247的第一种图案的滤网孔1256及一个覆盖整个近端盘1243的第二种图案的滤网孔1258。
在图12中所示的实施方案1310仅有一个盛放药物M的盘1314。由两个核体构件1312A和1312B合起来形成这个唯一的盘1314。1312A、1312B中的每个构件均含有全图案(full pattern)的滤网孔1360来覆盖核体1312A、1312B两部分间形成的盘1314。
在图13中所示的另一个实施方案1410中,仅有一个盘1414。在该实施方案1410中,核体1412包括被上层滤网1424覆盖的底层滤网1426。注意:上层滤网1424完全覆盖核体1412并且由全图案的滤网孔1460覆盖整个盘1414。
在图14中显示了另一个具有一个盘的实施方案1510。在该实施方案中,上层滤网1524的周边1536从里面装配到由核体1512所形成的盘1514的里面周边上。
在图15中显示了另一个具有一个内孔1614的实施方案1610。在该实施方案中,上层滤网1634包括一个垂向下的突出部分1662,所设计的突出部分与核体1621内形成的盘1614的周边紧紧密合。或者,如图18所示,将突出部分置于核体1612上并向上伸展与滤网1624的凹进处吻合。
在图16中显示了另一个仅有一个盘1714的实施方案1710。唯一的盘1714由核体1712形成,核体1712安装在核体托1712C内。核体托1712C含有一个空穴1712C.1,其中可以牢固地固定核体1712。覆盖核体1712内的盘1714的是一个上层滤网1724。核体1712的底部形成一个滤网。
在图17中显示了一个与图16所示的实施方案1710类似的实施方案1810。然而,与有一个临近的周边的核体托1812D不同,所形成的核体托1812D在有两个尖端之间形成空穴1812D.1。所设计的空穴1812D.1用来容纳核体1812。凹槽1864承载核体托1812D的两个尖端。同样,核体1812还是由滤网1824覆盖并且核体1812的底部还是形成一个滤网。
在图18中公开了含有核体1912的实施方案1910,该核体包括一个向上伸展的突出部分1966,该突出部分安装在上层滤网1924内使上层滤网1924能够跨过盘1914固定在核体1912上。
在图19中显示了含有一个基本中空的核体2012的实施方案2010。位于基本中空的核体2012的下表面2022上的是下层滤网2026,且位于核体上表面2020的是上层滤网2026。
在图20中显示了含有近端较小盘2143和远端较小盘2147的实施方案2110。分割近端较小盘2143和远端较小盘2147的隔板2141有通道2148,所形成的通道便于药物在此处移动。由上层滤网2124和下层滤网2126上的弓形图案的孔2152控制药物M1、M2的释放。或者,隔板2141也可沿着盘2114的长轴放置(run)形成并排的小盘。
在图21中还显示了另一个含有近端较小盘2243和远端较小盘2247的实施方案2210。在核体2212内,隔板2241分割开近端较小盘2243和远端较小盘2247。在上层滤网2224和下层滤网2226的远端2215含有U形图案的孔2250且在近端2213含有全图案的孔2260。
在图22中显示了一个加厚的椭圆形实施方案2310,它包括位于核体2312上表面2320和下表面2322的圆形滤网2324、2326。
实施例
将类似于图15所描述的装置植入八个新西兰白兔中。用硅酮胶将滤网与核体相连。对侧眼睛作为对照。两天取走一只兔子,一个月后对三只兔子进行试验,三个月后对剩余的四只兔子进行试验。在一个月和三个月时进行组织病理学观察。在一个月时,三只动物的玻璃体内显示有少量的炎性细胞。这三只动物中的一只在锯齿缘处同样显示出轻微的炎症。最后的动物表现轻微的晶状体纤维肿胀。这组动物的毒理学观察不显著。三个月取样的所有动物的毒理学观察均不显著。
在体外研究中,将一种具有相当高水溶解性的物质-盐酸倍他洛尔与10%微晶纤维素和0.40%硬脂酸镁制成总量22mg的片剂。将盐酸倍他洛尔片剂植入类似于图15所述实施方案的药物释放装置的盘中。在盘的一侧采用8微米大小的1%密度的多孔滤网。所有孔的面积与滤网的面积的比约为0.18%。所有孔的面积与整个药物释放装置表面积的比约为0.02%。根据本发明负载药物的药物释放装置与磷酸盐/盐水缓冲液一起置于4mLHPLC小瓶内,然后用小搅拌棒搅拌。对小瓶进行周期性地取样并通过HPLC分析药物浓度。如图24所示的药物浓度随时间变化图说明了释放的大略情况。
在第二个体外研究中,将片剂形式的具有相对低水溶解性的物质奈帕芬胺作为第二种药物制剂,也在类似于图15所示实施方案的装置中进行研究。这些片剂同样包括10%微晶纤维素和0.40%硬脂酸镁。一个片剂放置于两侧均为14微米大小25%密度的多孔装置中。所有孔的面积与滤网面积的比约为4.5%。所有孔的面积与整个药物释放装置的表面积的比约为0.48%。另一个片剂放置于两侧均为14微米大小1%密度的装置中。所有孔的面积与滤网面积的比约为0.18%。所有孔的面积与整个药物释放装置的表面积的比约为0.02%。如上所述进行药物释放的研究。如图25所示,图形“A”显示了两侧均为14微米大小25%密度的多孔装置释放药物的大略情况。图形“B”显示了两侧均为14微米大小1%密度的多孔装置释放药物的大略情况。将两侧均为14微米大小1%密度的多孔装置中的药物用磷酸盐/盐水缓冲液替代进行第三个试验并从头开始试验。图形“C”说明了该试验。
操作
一旦体内的病变或疾病被确诊并定位,那么医生可以决定这种病变或疾病是否能够采用将药物放置于病变或疾病部位附近的治疗方法。如果决定采用将药物放置于病变或疾病部位附近的治疗方法时,就必须将药物放置于病变或疾病部位的附近。在其他的应用中,可能需要与病变或疾病部位有一段距离进行治疗。这种有一段距离的治疗方式需要药物释放装置中的药物流量水平稳定。
在如图1所示的实施方案10中,病变或疾病位于患者眼睛内。例如,外科医生可在巩膜上切开一个小切口,将可植入的装置植入患者的玻璃体腔内。然后通过切口将药物释放平台10植入并将缝合线穿过核体12上的缝合孔30使其固定且将缝合线的另一端接于眼睛上。植入平台的方位应使植入的平台保持远离不在从晶状体到视网膜的光线通道上。
为了防止细胞包裹植入的装置而阻止药物从盘内通过滤网上孔的移动,可在滤网和核体上进行抗增生包衣。同样地,如硅的物质可形成碎片从而使滤网碎裂,所以,可用物质包裹核体以防止这种碎裂。
如前所述,尽管在优选的实施方案中是将药物植入眼内以用于治疗眼内的病变或疾病,但是,本领域普通技术人员可以理解,所述的可植入的药物释放平台10可用于动物体内的任何部位以治疗可以用植入的药物治疗的病变或疾病。
前面已根据优选和备选的实施方案对本发明进行了说明,本领域普通技术人员现已了解了本发明。本领域普通技术人员可以理解,前述公开内容也包括许多其他的实施方案。这些其他的实施方案将被包括在附加权利要求的范围和意图内。
Claims (24)
1.一种可植入的药物释放装置,该装置包括:用于承载药物而设计的盘,所述盘被盘的围栏部分包围;和用于覆盖所述盘的上部而设计的滤网,它置于所述盘的围栏部分。
2.如权利要求1所定义的可植入的药物释放装置,该装置还包括用于覆盖所述盘底部的滤网。
3.如权利要求1所定义的可植入的药物释放装置,其中所述盘贯穿盘的围栏部分并且在盘的底部为设计的覆盖盘底部的第二滤网,该滤网置于上述盘的围栏部分上。
