Abstract
本发明公开了含有盐酸R(-)-丙基去甲阿朴吗啡或盐酸S(+)-丙基去甲阿朴吗啡和/或其衍生物以及抗氧化剂、增溶剂和有助于活性成分透过皮肤的渗透激活剂的药物组合物。将该药物组合物用于透皮药贴(1、1'、1″)的基层(3、3'、3″)中,所述的药贴用于治疗中枢神经系统疾病且特别是用于治疗阳痿、偏头痛、帕金森病和精神疾病。可以通过改变所述增溶剂或所述渗透激活剂的浓度或通过设置一种透性膜(4)来改变所述活性成分的释放。
Description
题为:“用于治疗阳痿、偏头痛、帕金森病和精神疾病的含有盐酸R(-)-丙基去甲阿朴吗啡或盐酸S(+)-丙基去甲阿朴吗啡和/或其衍生物的透皮药贴和局部用组合物”的本发明的描述
技术领域
本发明涉及诸如盐酸R(-)-丙基去甲阿朴吗啡[下文缩写为R(-)-NPA-HCl或化合物(1)]和盐酸S(+)-丙基去甲阿朴吗啡[S(+)-NPA-HCl或化合物(2)]及其衍生物这样的物质用于治疗帕金森病、偏头痛、阳痿和精神疾病的用途。
更具体地说,本发明还涉及用于通过皮肤缓慢释放所述物质的一种“装置”(透皮药贴)。
现有技术
已经将阿朴吗啡(11B-13B)成功地用作药物且特别是在兽药领域用作催吐药。近来已经发现适当剂量的阿朴吗啡(缩写为APO)会对患帕金森病的患者具有有益的作用,而它在较低剂量下可用于治疗偏头痛、阳痿和精神疾病(4B、5B、6B、7B)。
在上述治疗适应症中,APO是一种强有力的多巴胺能兴奋剂药物,将它通过静脉(i.v.)、皮下(s.c.)、肌内(i.m.)、直肠、舌下或鼻内的途径给药。该物质的特征在于伴随快速发作(潜伏期约为15分钟)的快速吸收和同样快速消除(半衰期约为33分钟)(8B)。
阿朴吗啡的分子当暴露在空气中和暴露于光下时不稳定,它会快速氧化。实际上,当它暴露在空气中时,含有阿朴吗啡的制剂变色,变成了绿色,由此导致产品不能在临床上使用。
由于这一原因,为了保证患者在24小时内具有充足和恒定的治疗上有效的血药浓度所必需的药理作用,需要每日以1-5mg/小时的活性成分的限制剂量多次进行给药。如果超过这一剂量,甚至最轻微的过量也会导致严重的副作用诸如呕吐、中枢神经系统(CNS)抑制且在某些情况下甚至会导致患者死亡。
近来,已经提出了通过借助于以每小时低剂量输注确保药理作用而不会导致副作用的小型胰岛素泵的连续皮下输注给予阿朴吗啡的方式并将其付诸于医疗实践中。但即使是这种可供选择的给药方式也不是没有缺陷:由于已经使用了2-5mg/小时的剂量,所以存在输注部位周围形成皮下肉芽肿的倾向,需要暂停治疗。这是由于实际上药物集中在一点上:针眼周围所致的。
发明描述
本发明的一个目的是以连续方式使用含有盐酸R(-)-NPA-HCl(化合物(1))或S(+)-NPA-HCl(化合物(2))和/或其衍生物的药物组合物,可以将它们以活性成分的治疗有效剂量对患者进行给药而不会导致严重副作用诸如呕吐和中枢神经系统(CNS)抑制的发生,这是由于活性成分的剂量高,不然的话,就会如在使用APO中所发现的,在输注部位周围形成肉芽肿(当剂量较低时)。
化合物(1)和(2)的活性比APO的活性高10倍且另外它们的毒性却较低。此外,它们的半衰期高于APO的半衰期。由于这一原因,当在24小时内以合适的剂量给予所述的化合物时,获得了比APO更为有利的作用。
本发明的另一个目的是生产用于缓慢释放化合物(1)和(2)和/或其衍生物的透皮药贴,下文也称作PATCH-TDSS(TDSS=透皮药物转运系统)。本发明的透皮药贴会确保化合物(1)和(2)和/或其衍生物(活性成分)在24小时内的有效治疗浓度而不会显示传统技术的缺陷。
这是因为:由于所述药贴具有相对较大的尺寸(例如40cm2),所以所述的活性成分可以经整个40cm2透过表皮而无产生累积部位的任何可能性。
此外,较高的治疗活性使得剂量减少了约10倍。在水中较高的溶解度使得通过添加合适的渗透激活剂适当改变在体内的释放和渗透指数成为可能。

优选实施方案
附图1表示第一种类型的透皮药贴1,它包括不透性支持物薄膜2,在其上排列有基层3。将活性物质(1)、(2)和/或其衍生物溶解和/或分散在用作储存器的基层3中。在不透性支持物2对侧面上的基层3被可透过活性物质的膜4所覆盖,膜4可调节交叉通量。如果活性物质在皮肤中的渗透程度不超过可能导致副作用的值,那么可以不需要该膜(参见附图2和3,其中没有设置该膜)。
活性物质扩散的程度还取决于渗透激活剂、增溶剂等。在渗透膜4的自由面上存在一层被防粘条6保护的接触胶层5(粘合剂层)。在使用该透皮药贴的过程中,撕去防粘条6并将所述药贴固定在患者身体的所需部位,轻度施加压力。
在“起始”阶段后,通量达到恒定的“饱和”值。
活性物质的释放曲线(参见附图4、5)和以计算机化模型为基础的血浆浓度将在下面参照Ⅲ型透皮药贴来讨论。
就Ⅱ型药贴1′而言(附图2),将溶解和/或分散有所述活性物质的基层或“储存器”3施加在不透性支持物薄膜2′上。在这种情况下,用于改变交叉通量的透过活性物质的膜不存在。基层3′因此与表皮直接接触。胶层5’位于所述药贴的边缘周围,类似于一种粘合环。如同Ⅰ型,每一个药贴均在自由面上被可移去的单面防粘条6′所保护。
如下使用所述的“装置”:撕去防粘条6′并将所述装置固定在患者身体的所需部位,轻度施加压力。
特别地,如果活性成分以一种不需要的方式与粘合剂发生相互作用,那么可以采用这种解决方式(Ⅱ型),其结果是不可能使粘合层5′和活性成分在基层3′中混合。
Ⅲ型药贴是一种涉及“粘合剂基层中的药物”的药贴。
在这种“装置”中,直接将药理剂量置于胶层中(溶解或分散),该胶层由此也成为“储存器”基层3″且它在渗透性支持物薄膜2″上排列成一层。粘合剂基层3″在一面上被所述支持物2″(背衬)所保护且另一面上被防粘条6″所保护。如下使用所述的“装置”:撕去防粘条6″并将所述装置固定在患者身体的所需部位,轻度施加压力。
根据具体的应用来使用第一种、第二种或第三种类型的药贴。然而,本领域技术人员能够根据其需要来改变所述药贴的形状和/或结构,从而根据所选疗法和施用部位或根据其它因素来达到最佳结果,这是显而易见的。
因此,附图1、2和3中所示的三种结构是实施例而非起限定作用。
药贴结构和形状上的差异(矩形或剖面的)可能是由于:
-在活性成分、胶层(可以同时使用不同类型的粘合剂)、支持物和其它诸如赋形剂、稳定剂等这样的物质之间可能存在相互作用;
-在所选施用部位上较好的稳定性;
-剂量(为了较高的剂量还必须增加药贴的面积)。
为了说明的目的,在下述中将讨论Ⅲ型药贴的制备方法;且另外将表示出构成基层的药物组合物的成分。
然后,将说明用于形成Ⅲ型药贴基层的药物组合物的三个非限制性实施例(附图3)。接着将在豚鼠皮肤上“试验”含有由这三种类型的配方构成的基层的Ⅲ型药贴以便分析活性成分的渗透情况,即关于活性成分在48小时内的累积情况(附图4)和有关通量(也在48小时内,附图5)。Ⅲ型透皮药贴的制备方法
1.在热粘合剂溶液中将活性成分(1)或(2)或其衍生物同时与其它成分(稳定剂、渗透激活剂等)混合并通过搅拌进行匀化,直到获得液体粘合剂基质或“储存器”为止;
2.冷却所述的液体基质并获得“粘性”稠度;
3.在下列步骤中,使用连续与干燥机连接的成层机来进行在支持物上使粘合剂基质成层的过程:
-横跨其上安全固定有防粘条的成层机输送带的全宽度安放刀口;
-将“粘性”粘合剂基质倾倒在刀口前当输送带传送时,使粘合剂基质均匀层(成层)分布在防粘条上;
-所述层的厚度主要由刀口边缘与在其下方运转的防粘条之间的距离来决定;
-携带粘合剂基质的防粘条在干燥机内旋转,其中通过蒸发溶剂使所述的粘合剂基质固化,这一过程通过逐步升温和“通风”来实现,正如下列表Ⅰ中所示。表Ⅰ干燥步骤 时间(按分钟计) T℃ 通风(rpm)

所述过程除去了溶剂,从而防止它被快速形成的表面外壳所阻塞。
当粘合剂基质已经干燥时,施加支持物薄膜(背衬)。这个称作“叠压”的步骤结束了该工艺。
该工艺在文献中有描述(9B、10B、11B)并产生了PATCH-TDDS,其中粘合剂基质被“背衬”和可移去的防粘条所保护。
非常重要的是使用惰性和可渗透(1)或(2)及其衍生物的粘合剂,且该粘合剂的粘合特性(粘结力、粘合力和交织力)不会受到所述活性成分本身和/或赋形剂或所添加的任何其它物质的不利影响。Ⅲ型PATCH-TDDS的组成粘合剂基质:配方
-活性成分:(1)或(2)或其衍生物;
-抗氧化剂:焦亚硫酸钠、EDTA二钠盐;
-增溶剂:乙二醇;
-渗透激活剂:脂肪酸类;
-改善粘结强度的丙烯酸树脂:以二甲氨基乙基甲基丙烯酸酯和甲基丙烯酸酯类为基础的阳离子型共聚物;
-改善粘结强度的纤维素衍生物:乙基纤维素;
-表面活性剂:SDS(十二烷基硫酸钠);
-压合式粘合剂:两种粘合剂A和B的混合物,其中A是非自粘合性丙烯酸压合式粘合剂,它具有中等分子量、高交织指数、皮肤刺激指数为0.20、被分类为“最低刺激性”、使用100%乙酸乙酯作为溶剂;且B是自粘合性丙烯酸粘合剂,它具有高分子量、中等交织力、皮肤刺激指数为0、被分类为“非刺激性”、使用乙酸乙酯、异丙醇、己烷和甲苯的混合物作为溶剂。防粘条
防粘条是一种在一面上与硅氧烷叠压的聚酯薄膜(另一面是粘合剂基质)。厚度约为125μm。“背衬”
“背衬”是一种透明的且用热熔接层密封的叠压聚酯薄膜。总厚度约为51μm。活性成分的量
表示为(1)或(2)的(1)或(2)或其衍生物的用量按粘合剂基质的重量计为5%且在PATCH-TDDS中相当于5mg/cm2。将药物的大部分分散在基质中。将小部分溶解在基质中。分散在基质中的药物起“储存器”的作用,而供释放和渗透用的药物是溶解的药物。
现在给出在施用含有(1)或(2)和/或其衍生物的Ⅲ型透皮药贴中的功效的三个实施例。
为了这一目的制备了含有(1)或(2)和/或其衍生物的区别制剂的三批药贴。
使用由美国FDA推荐的体外细胞渗透技术(1B、2B、3B),例如,由下列给出的三种不同的配方而获得了下列结果,其中化合物(1)用作活性成分,即药理上最具活性的分子:

对于三个实施例A、B和C来说,在不同的时间处,按照所述步骤进行的使用豚鼠皮肤的体外渗透研究表明渗透率等于附图4中(总量)和附图5中(通量)所给出的值。
可以观察到:就实施例C而言,所述的值几乎为0,即所有的黑色方块实际上位于横轴上(时间,按小时计)。
这证明:在没有一定量的分散的活性成分存在的情况下,0.5%用量的溶解在基质中的活性成分不会产生使活性成分通过皮肤以便实现所预计应用的明显的渗透作用。
因此,有必要以这样一种方式在基质中分散一定量的活性成分,以便产生有助于活性成分扩散的浓度梯度并构成“储存器”以便获得缓慢释放的效果。
在使用用于透皮给予所述药物的最合适的药代动力学模型之一处理上述报导的体外渗透数据(10B、11B)并考虑到具有下列常数的40cm2(5×8cm)药贴时:盐酸丙基去甲阿朴吗啡的分子 331.8

分配体积 132获得了附图6中所示的近似曲线,就施用单一批量的NPA/B药贴而言,它代表指定时间处的人血浆浓度(纵坐标:单位ng/ml;横坐标:按小时计的时间)。
从附图4-6中可能得出结论:使用不同的渗透激活剂诸如脂肪酸类或醇类的化合物(1)的皮肤渗透性本身是充足的和可上下改变的。
所有体外渗透研究均使用应用豚鼠皮肤的模型来进行,我们知道它具有可与人皮肤相比拟的渗透性并给出比后者更可重复的结果。
化合物(1)在药贴制剂中的化学稳定性通过添加抗氧化剂(焦亚硫酸钠)来实现并通过40℃和75%RH(相对湿度)下的15天加速稳定性试验来证实。
为了增溶更大量的化合物(1)可添加表面活性剂(SDS),因为只有增溶的物质(1)才能供释放和渗透。药贴还表现出良好的物理化学特性,本领域技术人员可针对不同施用部位的粘合性和在不改变活性成分渗透率的治疗过程中治疗对象的耐受性使这些特性最佳化。
如上所述,透皮药贴的最合适的构造随情况的不同而改变。例如,可以从较大尺寸的带上切下Ⅲ型药贴以便获得用于施用部位的合适大小的透皮药贴。
另一方面,如果基质3′不是粘合剂且胶层仅限于环形区5′,那么Ⅱ型药贴已经具有最终的尺寸且不能切割。

Claims (17)
1.药物组合物,其特征在于它包括药物上可接受和有效剂量的盐酸R(-)-丙基去甲阿朴吗啡和/或盐酸S(+)-丙基去甲阿朴吗啡和/或其衍生物且进一步包括稳定剂、增溶剂和有助于活性成分透过皮肤的渗透激活剂。
2.根据权利要求1所述的药物组合物,其特征在于所述的盐酸R(-)-丙基去甲阿朴吗啡和盐酸S(+)-丙基去甲阿朴吗啡的衍生物是盐或有机衍生物。
3.根据权利要求1所述的药物组合物,其特征在于所述的稳定剂是抗氧化剂物质。
4.根据权利要求1所述的药物组合物,其特征在于将乙二醇用作增溶剂。
5.根据权利要求1所述的药物组合物,其特征在于将脂肪酸类或醇类用作渗透激活剂。
6.根据权利要求3所述的药物组合物,其特征在于所述的抗氧化剂是焦亚硫酸钠和EDTA。
7.根据上述权利要求中任意一项所述的药物组合物,它另外包括一种表面活性剂(SDS)。
8.根据上述权利要求中任意一项所述的药物组合物,它另外包括用于改善粘结强度的丙烯酸树脂、用于改善粘结强度的纤维素衍生物和压合式粘合剂的混合物。
9.透皮药贴(1;1′;1″),其特征在于它包括至少一种称作“背衬”的支持物薄膜(2;2′;2″)、含有权利要求1-8中所述的药物组合物的中间基层(3;3′;3″)和保护防粘条(6;6′;6″)。
10.根据权利要求9所述的透皮药贴(1),其特征在于所述的基层(3)不包括粘合剂且在其下面上带有由透性膜制成的层(4)。
11.根据权利要求10所述的透皮药贴,其特征在于改变活性成分释放的透性膜(4)本身由用于固定在皮肤上的粘合层构成。
12.根据权利要求10所述的透皮药贴,其特征在于透性膜(4)是不粘的且在下面带有用于固定在皮肤上的粘合层(5)。
13.根据权利要求9所述的透皮药贴(1′),其中在不含粘合剂的基层(3′)的侧面上施加有粘合层(5′)。
14.根据权利要求9所述的透皮药贴(1″),其特征在于它包括含有用于改善粘结强度的丙烯酸树脂和纤维素衍生物以及压合式粘合剂混合物的粘合基层(3″)。
15.权利要求1-8中所述的用于获得一种或多种活性成分透过皮肤缓慢释放的药物组合物在治疗帕金森病、偏头痛、阳痿和精神疾病中的用途。
16.一种权利要求9-14中所述的用于获得一种或多种活性成分透过皮肤缓慢释放的透皮药贴在治疗帕金森病、偏头痛、阳痿和精神疾病中的用途。
17.根据权利要求16所述的透皮药贴的用途,其中活性成分的一部分可立即通过皮肤得到渗透而另一部分则分散在起“储存器”作用的基层(3;3′;3″)中。
