Abstract
本发明公开了一种PNN抑制剂在制备逆转转移性肾癌细胞对舒尼替尼耐药性药物中的应用;通过采用PNN抑制剂减低PNN基因的拷贝数进而逆转肿瘤细胞对sunitinib耐药的一种生物治疗策略,从而为揭示靶向治疗更加有效地对抗sunitinib耐药提供科学依据。本发明首次发现PNN在促进ccRCC细胞转移的过程中发挥了重要作用;其次在体外细胞培养条件下,对ccRCC细胞采用逐步增加剂量法诱导构建耐药细胞株;通过不同浓度的sunitinib处理耐药细胞和亲本细胞,观察二者对药物的耐受性;再用PNN抑制剂处理后、耐药细胞的转移能力显著性被抑制。
Description
技术领域
本发明涉及PNN(桥粒相关蛋白,pinin)高表达诱发转移性肾癌细胞对舒尼替尼(sunitinib)耐药,PNN抑制剂通过降低细胞中PI3K/AKT活性和PNN基因的拷贝数进 而逆转转移性肾癌细胞对耐药性的方法,为揭示新的治疗靶点、有效对抗sunitinib耐药 提供科学的借鉴和途径,属于肿瘤生物治疗技术领域。
背景技术
手术和化疗是肾透明细胞癌(clear cell renal cell carcinoma,ccRCC)的主要治疗手 段。对于I-III期的ccRCC患者,虽然手术是其首选的治疗方案,但手术切除后肿瘤的 复发比例较高,总体治疗效果并不理想。而对于m-ccRCC(metastatic ccRCC,m-ccRCC)患者,5年生存率更是极低。对于中晚期的ccRCC患者,在2000年以前,化疗通常作 为一线推荐的治疗方案。但是,随着对肿瘤发生发展和转移机制的认识深入,针对肿瘤 相关基因的分子靶向药物逐渐成为目前研究的热点。
从2005年底开始,六个靶向治疗药物被美国FDA批准用于进展期RCC。其中四个 靶向药为抗血管生成,另外两个为靶向雷帕霉素靶蛋白(mTOR)。靶向治疗进入临床以 后,大大地延长了RCC患者的生存期。舒尼替尼(sunitinib)是一种口服、多靶点的酪 氨酸激酶受体抑制剂,因其可以同时多靶点抑制血管内皮生长因子受体家族(VEGFR-1, VEGFR-2和VEGFR-3),血小板源性生长因子受体(PDGFRα和PDGFRβ),干细胞生长 因子受体(KIT),FMS样酪氨酸激酶3(FLT3)和胶质细胞衍生的神经营养因子受体(RET) 等钟酪氨酸激酶受体的活性,所以具有抑制肿瘤血管的生成以及诱导肿瘤细胞凋亡的双 重抗肿瘤作用。
国内外Ⅲ期~Ⅳ期临床研究结果显示,sunitinib一线治疗m-ccRCC的客观有效率为 25%~50%,中位OS为26~30个月。虽然,sunitinib的临床应用改善了RCC患者的预后;但临床上有26%的患者在开始使用时就表现出先天性耐药而对sunitinib无效;另外,初治有效的患者也可能在服药6-12个月后,出现获得性耐药。由此可见,虽然对于晚 期的ccRCC患者,sunitinib能有效延长患者的生存时间,提高生活质量;但是,化疗耐 药现象的出现是ccRCC患者化疗失败的主要原因。因此,研究ccRCC产生耐药的机制, 寻找化疗耐药的靶向基因并探索其作用机制,具有重要的理论意义和应用价值。
发明内容
针对ccRCC在治疗过程中常产生耐药而导致化疗失败甚至复发的现象,本发明的目的在于提供PNN抑制剂在制备逆转转移性肾癌细胞对舒尼替尼耐药性药物中的应用。
具体的,通过采用PNN抑制剂减低PNN基因的拷贝数进而逆转肿瘤细胞对sunitinib耐药的一种生物治疗策略,从而为揭示靶向治疗更加有效地对抗sunitinib耐药提供科学依据。
上述PNN抑制剂在逆转转移性肾癌细胞对sunitinib耐药中的应用,具体的:
(1)分析ccRCC病人组织中PNN的表达分析其与转移、病理分级和病人愈后等 之间的关系:对从医院收集的ccRCC组织和癌旁组织样本,进行采用免疫组化、 qRT-PCR和Western blot方法检测乳腺癌组织和癌旁组织中PNN的表达水平;统 计分析PNN的表达量与临床病理资料的关系。
(2)研究ccRCC细胞中PNN的表达,及其对细胞迁移和侵袭的影响:对培养人 RCC细胞系和上皮细胞HK-2,用慢病毒系统构建了稳定低表达PNN的细胞株,划 痕和transwell实验研究PNN低表达与ccRCC细胞转移和侵袭的影响。
(3)体内研究PNN表达对ccRCC细胞转移的影响:采用尾静脉注射的方法,将 稳定低表达PNN的Caki-1/shPNN细胞及其对照Caki-1/Control细胞接种至裸鼠体内; 50天后统计裸鼠肺组织表面的结节数量,体内实验分析PNN对ccRCC细胞转移的影响。
(4)研究PNN表达与sunitinib耐药的关系:在体外细胞培养条件下,对Caki-1 和786-O细胞采用逐步增加剂量法诱导构建耐药细胞株;通过不同浓度的舒尼替尼处理 耐药细胞和亲本细胞,观察二者对药物的耐受性。
(5)研究耐药对ccRCC细胞转移的影响:用划痕实验比较耐药细胞和亲本细胞之间转移能力的差异;用transwell实验比较耐药细胞和亲本细胞之间转移能力的差异。
(6)研究PNN表达与ccRCC对舒尼替尼耐药之间的关系:用基因芯片方法分析 PNN在耐药细胞中的表达;再用PNN抑制剂降低耐药细胞中PNN的表达后,探讨其对 细胞转移的影响。
(7)研究PNN促进ccRCC对sunitinib耐药的分子机制:用PNN抑制剂降低耐药 细胞中PNN的表达后,检测EMT相关蛋白在PNN高低表达的耐药和亲本细胞之间的 表达、耐药细胞中PI3K和AKT的磷酸化水平、反之升高shPNN处理的耐药细胞中PI3K 的水平,研究对细胞迁移和EMT蛋白表达的影响。
(8)分析临床舒尼替尼治疗病人耐药的潜在因素,以及和PNN表达的关系:用 Cox回归模型分析开始治疗时间、器官转移个数以及PNN表达强度对sunitinib治疗病 人的影响;分析PNN表达对m-ccRCC患者的生存影响。
通过上述研究,本发明的发明人针对sunitinib耐药且PNN高度扩增的ccRCC细胞,特异性的抑制PI3K和AKT通路,能够降低EMT相关蛋白的表达,进而减少PNN诱导 的肿瘤耐药和细胞转移。因此,在上述研究的基础上,本发明提出了PNN抑制剂在制 备逆转ccRCC对sunitinib耐药性药物中的应用。
本发明中,优选的所述的PNN抑制剂为干扰PNN的慢病毒载体。
更优选的,所述的干扰PNN的慢病毒载体为pCDH-CMV-MCS-EF1-Puro,购自深 圳市宝珠生物科技有限公司。
本发明的优点和有益效果:
本发明首次发现PNN在促进ccRCC细胞转移的过程中发挥了重要作用;其次在体外细胞培养条件下,对ccRCC细胞采用逐步增加剂量法诱导构建耐药细胞株;通过不 同浓度的sunitinib处理耐药细胞和亲本细胞,观察二者对药物的耐受性;再用PNN抑 制剂处理后、耐药细胞的转移能力显著性被抑制。最后结合临床病例,确定该基因的表 达与ccRCC转移之间存在正相关的关联。研究结果揭示了PNN作为新的治疗靶点将有 效对抗sunitinib的耐药,通过采用PNN抑制剂减低PNN基因的拷贝数、达到逆转肿瘤 细胞对sunitinib的耐药性。
附图说明
图1是PNN在m-ccRCC组织和细胞中高表达图。qRT-PCR和Western blot实验方 法显示,在80例m-ccRCC病例中有76(95%)(P<0.001)PNN的mRNA水平显 著性高于癌旁组织(图1A)。检测PNN在RCC不同亚型细胞(表1)中的表达时发现 (图1B),与HK-2细胞相比,PNN在ccRCC细胞(Caki-1和ACHN)中的mRNA水平 分别高达3.2和2.8倍,蛋白表达结果相似(图1C);由于Caki-1细胞属于ccRCC细胞 中具备高转移特性(表1)。
图2是生长曲线图,表示耐药细胞Caki-1/sunitinib(A)和786-O/sunitinib(B)对舒 尼替尼的耐受性都强于亲本细胞。
图3是Western Blot图,表示舒尼替尼诱导亲本细胞Caki-1内caspase-3活化增强。
图4是降低PNN的表达后抑制Caki-1细胞的体外转移能力图。shPNN显著性抑制Caki-1细胞中PNN的表达(A);Caki-1细胞转染shPNN后,细胞的迁移能力(B)和 穿过transwell小室的能力(C)都显著性低于对照组。结果用三次独立重复实验的均值 ±标准差表示。**P<0.01,与对照组比较。
图5是降低PNN的表达后抑制ccRCC细胞体内转移,裸鼠肺组织照。降低Caki-1 细胞内PNN的表达,裸鼠体内转移的肺表面转移结节数显著性降低(A,B);小鼠肺 组织表面的转移结节HE染色图(C)。**P<0.01和对照组相比。
图6是耐药和对照细胞基因芯片检测结果,和降低Caki-1/sunitinib细胞中PNN的表达,抑制细胞的转移和侵袭能力。基因芯片结果示,PNN在Caki-1/sunitinib和 786-O/sunitinib中分别升高8.12和6.35倍(A)。shPNN显著性抑制Caki-1/sunitinib细 胞中PNN的表达(B)。降低耐药细胞Caki-1/sunitinib中PNN的表达后,细胞的转移 (C)和侵袭能力(D)被显著性抑制。**P<0.01,***P<0.001和scramble组对照。
图7是PI3K/AKT在PNN促进耐药细胞迁移中的作用检测结果。在耐药细胞中,pPI3K和pAKT的表达显著性升高(A),降低耐药细胞中PNN的表达,伴随pPI3K和 pAKT的表达水平下降(B),AKT激活剂(SC79,10μM)能拮抗siPNN抑制的耐药 细胞迁移(C)、侵袭(D)能力和EMT发生。*P<0.05,**P<0.01,***P<0.001和 scramble组对比。
图8是免疫组化图,表示PNN的表达与临床分期间的关系。III和IV期病人组织 中细胞膜PNN的信号强度明显高于I和II期,×200。
图9是生存曲线图,PNN高表达患者的总体生存率显著性低于低表达的患者。
图10组织中PNN表达水平与转移性肾透明细胞癌对舒尼替尼反应性呈正相关。
具体实施方式
下面结合具体实施例来进一步描述本发明,以便更明确的阐述其优点和特点。但实 施例仅为范例,并不对本方面的范围构成任何限制。本领域技术人员在不偏离本发明的精神和范围下可对本发明的技术方案细节和形式进行修改或替换,但均属本发明的保护范围。
实施例1
1.细胞培养
HK-2(人肾皮质近曲小管上皮细胞)和ACHN(人肾癌细胞)均培养在含有10% 胎牛血清的DMEM培养基中,OS-RC-2(人肾癌细胞)和786-O细胞(人肾透明细胞 癌细胞)均培养在含有10%胎牛血清的RPMI培养基中;耐药细胞培养基中添加相应浓 度的sunitinib。所有细胞均在37℃、5%CO2、饱和湿度细胞恒温培养箱中培养。

2.QRT-PCR
2.1RNA提取
PBS清洗后加入1ml的Trizol(Trizol试剂),室温静置5min,移至RNAse-free 的EP管中;加200μl三氯甲烷震荡混匀后静置5min;4℃ 12000g离心15min;取上 清加等体积的异丙醇,静置10min;4℃ 12000g离心150min;白色沉淀加1ml 75% 乙醇重悬,离心5min,重复一遍;吸干乙醇,通风橱中开盖静置至白色沉淀为透明后 加30-50μl 65℃ RNAse-free水溶解RNA;测RNA浓度;-80℃长时间保存。
2.2RT-PCR
a.在RNAse-free EP管中配制下列混合液,具体见表2:

b.在PCR仪上进行变性、退火反应。PCR仪温度65℃反应5min,4℃保存。

2.4数据处理
采用2-ΔΔCT方法计算。CT值:每个反应管内的荧光信号到达设定的域值时所 经历的循环数。

求出2-ΔΔCT值,根据求得的值判断基因表达量。
3.慢病毒系统包装实验
3.1酶切:用EcolR I酶和Not I酶双酶切空载体质粒pCDH-CMV-MCS-EF1-Puro 和质粒。酶切体系:DNA 2μl,EcolR I酶1μl,Not I酶1μl,10×H buffer 2μl,0.1% BSA 2μl,0.1%TritonX-100 2μl,ddH2O 10μl。混匀后,37℃水浴酶切3小时,20μl 体系进行琼脂糖凝胶电泳,100V跑30min。
3.2割胶回收:(1)在紫外灯下用锋利的刀片切下含DNCREB DNA片段的凝胶块或含空载体DNA片段的凝胶块,放入干净的1.5ml离心管中;称取凝胶块的重量;(2)每 100mg凝胶块加400μl Binding Solution B,于50~60℃放置5~10min,每2-3min 间断混合一次,直至凝胶块融化;(3)将上述混合液转移到GenClean柱,室温放2min, 6000rpm离心1min,取出GenClean柱,倒弃收集管中的废液;(4)将GenClean柱放 回收集管中,加500μlWash Solution,室温12,000rpm离心1min,倒弃收集管中的废 液;(5)将GenClean柱放回收集管中,加500μl Wash Solution,室温12,000rpm离心1 min,倒弃收集管中的废液;(6)将GenClean柱放回收集管中,室温12,000rpm离心1 min,彻底去除多余的Wash Solution。(7)将GenClean柱放入干净的1.5ml离心管中, 在GenClean柱膜的中央加30-50μl ElutionBuffer,37℃放置2min。室温12,000rpm 离心1min,离心管中的液体则为含DNCREB DNA片段的溶液或含有空载体DNA片段 的溶液。
3.3空载体DNA片段与目的DNA片段连接
按照用量在微量离心管中加入下列反应液,具体见下表6:

混匀后,室温连接3h。此时的重组载体质粒为pCDH-DNCREB-Puro。
3.4转化:(1)将感受态细胞置于冰上溶化,然后将10μl连接产物(3.3空载体 DNA片段与目的DNA片段连接)加入到100μl感受态细胞中,混匀,在冰浴上放置 30min;(2)42℃热激90s,立即转移至冰上放置2min,注意不要摇动试管;(3)加 入1ml不含Amp的LB液体培养基,37℃、100rpm摇床过夜培养1h,混匀菌液,用 涂布器均匀涂布于含Amp的LB固体培养基,于37℃培养箱中过夜培养。
3.4提取重组载体质粒:(1)挑取单菌落,将其置于LB液体培养基(含有Amp)中, 吹打混匀后于37℃,200rpm过夜摇床培养;(2)取30-50ml菌液到干净的离心管中, 室温3500-5000×g离心10min,收集适量的菌体;(3)倒弃LB培养基,加2.5ml Solution I/RNase混和液,漩涡振荡,完全悬浮菌体;(4)在重悬液中加2.5ml SolutionⅡ,轻轻 缓慢颠倒混匀10次,然后将混合液在室温放2min。注意:切记避免剧烈混和,裂解 反应不超过5min;(5)提前将Buffer N3冰浴,然后加入1.25ml,温和颠倒数次直至形 成白色絮状沉淀;(6)准备好过滤器。在HiBind中量柱中加2ml Buffer GPS,室温放置 3-10min。3000-5000×g离心5min。倒弃滤液,柱子重新放回收集管中。(7)将第(5) 步的裂解液倒入针筒过滤器中(过滤器下面的出口接收集管),室温放置2min。轻推活塞 使裂解液缓慢流入收集管中。(8)在第(7)步收集的裂解液中加0.1倍体积的ETR,颠倒10次左右后发现溶液会变浑浊,放置冰上10-20min。42℃水浴5min后,室温条件下, 3000-5000×g离心5min,ETR溶液在管底会形成蓝色分层。(9)转移上清液于干净的 离心管中,加入1/2倍体积的无水乙醇(室温),混匀后,室温静放2min。(10)转移20ml 混合液至(6)步中准备好的HiBind中量柱內,室温下3000-5000×g离心3-5min,弃掉 滤液。(11)重复该步骤,直至所有混合液都从柱子滤出。(12)把柱子装回至收集管内, 加入3ml HB Buffer后,3000-5000×g离心3-5min,丢弃滤液。(13)把柱子装回收 集管内,加3.5ml DNA Wash Buffer,3000-5000×g离心3-5min,丢弃滤液。重复该步骤一次。(14)丢弃滤液,把柱子放回收集管中,最大速度(<6000×g)离心10-15min, 甩干柱子,弃去多余的液体。(15)将柱子装入干净的15ml离心管上,加70℃预热的 500μlEndotoxin free Elution Buffer到柱子中央上,室温静置2min后,以最大速度离心 5min洗脱DNA,吸出这500μl的Endotoxin free Elution Buffer再次到柱子中央上,以 最大速度再次离心5min洗脱DNA,该DNA即为重组质粒,放于-20℃。
3.6酶切验证:用EcolRⅠ酶和NotⅠ酶双酶切重组载体质粒,酶切体系:重组DNA 2μl,EcolRⅠ酶0.25μl,NotⅠ酶0.25μl,10×H buffer 2μl,0.1%BSA,2μl 0.1% TritonX-1002μl,ddH2O 11.5μl。混匀后37℃水浴酶切3h。20μl体系进行琼脂糖凝胶 电泳,100V跑30min。
3.7提取包装质粒和空载体质粒:方法步骤与提取重组载体质粒一致。
3.8包装慢病毒:在EP管中加入下列成分吹打混匀,静置2min后,加入到含10mlDMEM培养基的293T细胞中。6h后更换10ml新的培养基,培养60h后用0.45μm 的过滤器收集慢病毒。

4.慢病毒转染OS-RC-2和HK-2细胞:
将对数生长期的细胞消化成细胞悬液,4ml细胞悬液加2ml慢病毒液(3.8包装慢病毒收集的慢病毒)混匀,37℃、5%CO2、饱和湿度培养24h后更换新的培养基,60h 后更换成含0.1‰puromycin(嘌呤霉素)的培养基进行筛选、传代、冻存。此时,慢病 毒转染pCDH-CMV-MCS-EF1-Puro空载体质粒的细胞系被命名为vector,转染 pCDH-DNCREB-Puro重组载体质粒的细胞。
5.划痕实验和Transwell实验鉴定PNN抑制剂对细胞体外迁移的影响:为了研究PNN表 达与ccRCC细胞转移的关系,我们首先用慢病毒系统构建了稳定低表达PNN的细胞株(),western blot验证shPNN能显著性降低Caki-1细胞内PNN的表达(图4A)。随 后进行的细胞划痕和侵袭实验结果发现,shPNN处理组细胞的迁移能力(图4B)和穿 过transwell小室(图4C)的能力都明显下降。
6.裸鼠尾静脉注射实验鉴定PNN抑制剂对细胞体内迁移的影响:选用4周大 的裸鼠进行尾静脉注射实验,根据实验需要,设置2组,每组5只裸鼠;用于裸鼠皮下 成瘤实验实验组和对照组细胞消化、离心后,用PBS漂洗2次,然后用PBS重悬,细 胞计数后用PBS调整细胞浓度至1×107个细胞/ml;消毒皮肤,将单细胞悬液注入裸鼠 尾静脉,接种量为1×106个细胞/100μl悬液;接种细胞后置于动物实验中心饲养,每 日观察裸鼠生长状况,50天后用颈椎脱臼法处死裸鼠,取出裸鼠肺组织观察、统计肺表 面的转移结节数量,并拍照。结果发现,Caki-1/shPNN组小鼠肺组织表面的转移结节数 目显著性低于Caki-1/Control组(图5A,B)。肺表面结节进行HE染色后可见(图5C), 结蹄组织包围肿瘤细胞(T)形成结节,结蹄组织外为正常的肺间质组织(N)。由此 可见,PNN表达可以促进ccRCC细胞的体内转移。
7.降低PNN的表达抑制舒尼替尼耐药细胞的转移能力
为了研究舒尼替尼耐药的分子机制,我们将两种耐药和对照细胞进行了基因芯片检 测。结果发现,PNN在两种耐药细胞中的表达分别升高8.12和6.35倍(图6A)。由 于PNN升高能促进ccRCC的迁移,我们用慢病毒系统包装稳定低表达PNN的舒尼替 尼耐药细胞后,Western blot验证PNN的表达显著性低于对照(图6B)。随后进行划 痕实验和transwell实验,计算在Caki-1/sunitinib中,降低PNN后细胞转移能力和穿透 膜的细胞数。结果显示,用shPNN降低耐药细胞中PNN的表达24h后,细胞的转移能 力低于对照0.32倍(图6C),穿透膜的细胞数也低于对照0.48倍(图6D)。由此可 见,降低sunitinib耐药细胞的PNN表达,能显著性抑制ccRCC的转移。
8.PI3K/AKT通路在PNN促进耐药细胞迁移中的作用
为了研究PI3K/AKT在PNN促进耐药细胞迁移中的作用,我们首先研究了耐药细 胞中PI3K/AKT的磷酸化蛋白表达情况。结果发现,在两种耐药细胞中,pPI3K和pAKT 的表达都显著性高于亲本细胞(图7A);此外,降低耐药细胞中的PNN表达后,pPI3K 和pAKT的表达也显著性下降(图7B);当用PI3K/AKT激活剂SC79(10μM)处理耐 药细胞后发现,由PNN降低诱导的细胞迁移(图7C)、侵袭(图7D)能力和EMT 发生(E)被完全逆转。由此可见,PI3K/AKT通路在PNN促进耐药细胞的迁移中扮 演重要角色。
由此可见,PNN抑制剂能够降低sunitinib耐药细胞的迁移能力。
实施例2:PNN高表达与m-ccRCC临床病人病理参数之间和患者预后的关系
1.PNN在临床病人标本中的表达
为了研究PNN与临床病例资料的关系,我们将160例ccRCC石蜡组织进行 免疫组化染色。结果显示,PNN在ccRCC不同临床分期中的表达水平具有显著 性差异(P<0.05);此外,其表达水平在淋巴结转移组与无淋巴结转移组之间也有 显著性差异(P<0.001);PNN的表达水平与患者的年龄、肿瘤大小、组织学分级、 病理类型和分子分型等临床病理指标无关(P>0.05)(表8)。棕褐色的PNN颗 粒主要集中在RCC细胞的膜中,Ⅲ期+IV期患者PNN的阳性表达率显著性高 于Ⅰ+Ⅱ期患者(图8)。参照Hadi等[21]的免疫组织化学反应结果的判定标准, 综合考虑切片中阳性细胞占所观察同类细胞数的百分比和阳性细胞着色强度两 项指标,半定量判断结果。由此可见,PNN升高与ccRCC的转移密切相关。

2.PNN高表达的m-ccRCC患者预后较差
通过生存分析研究PNN表达与m-ccRCC患者预后的关系时发现,PNN高表达 患者的总体生存率显著性低于低表达的患者(P=0.010,HR:3.595;95%CI:1.361to 9.497)(图9)。上述结果提示,PNN高表达显著性影响了m-ccRCC的发展和 预后。
实施例3PNN在舒尼替尼治疗转移性肾透明细胞癌中的作用及其预后因素分析
1临床资料
选择2011年6月至2016年12月在医院就诊77例晚期m-ccRCC患者。检测所有入组患者肿瘤组织中PNN的表达水平,将患者分为PNN高表达组(high)和PNN低表达组(low)。 其中41例患者接受了至少2个疗程的sunitinib治疗,定义为sunitinib治疗组,另外36例患 者为同期收治但因其他非疾病因素未接受sunitinib治疗治疗,定义为对照组(control)。sunitinib组初治转移16例(39%),根治性肾切除术后继发转移25例(61%)。男性25 例(61%),女性16例(39%)。年龄46-79岁,中位年龄57岁。ECOG评分0-2分27例(65.9%), >2分14例(34.1%)。对照组初治转移14例(38.9%),根治性肾切除术后继发转移22 例(61.1%)。男性22例(61.1%),女性14例(38.9%)。年龄45-79岁,中位年龄57 岁。ECOG评分0-2分23例(63.9%),>2分13例(36.1%)。
纪念斯隆一凯特琳癌症中心(MSKCC)风险模型[3]包括5个危险因素:(1)确诊至全身治疗的间隔时间<1年;(2)乳酸脱氢酶(LDH)>1.5倍正常上限值;(3)血清校正钙>100mg/L;(4)血红蛋白低于正常下限值;(5)Karnofsky体能状态评分<80分。根据预后 因子的数目分为低危(0个)、中危(2个)和高危(≥3个)。
2治疗方案
接受肾切除术及减瘤术的患者待术后全身情况好转、伤口拆线并排除手术并发症等 影响因素后开始使用sunitinib靶向治疗。余患者经穿刺活检明确病理后开始使用sunitinib 靶向治疗。所有患者接受4/2给药方案,即口服sunitinib50mg/d,给药4周,停用2周,每 6周为1周期。治疗持续期间根据不良反应严重程度及与sunitinib的相关性进行剂量调整 或暂停用药,直至疾病进展或出现不可耐受的不良反应。每个周期进行1次安全性评价, 每2个周期进行1次疗效评价。
3疗效与不良反应评价
疗效根据实体瘤疗效评价标准(RECIST1.0)进行评价,分为完全缓解(completeremission,CR)、部分缓解(partial remission,PR)、疾病稳定(stable disease,SD) 以及疾病进展(progress disease,PD),疗效需要维持4周以上。肿瘤客观缓解率(ORR) =CR率+PR率,疾病控制率(DCR)=PR率+CR率+SD率。总生存期(OS)是指从 患者接受治疗至死亡的时间。药物毒副反应根据实体肿瘤疗效评价标准(CTCAE3.0) 进行评价,每4周进行安全性评估。所有患者治疗前及治疗后均记录完善的基本资料: 包括详细的病史、全面的体格检查、实验室检查(血常规、血生化和甲状腺功能等)、 影像学检查(盆腔MRI、胸腹部CT和全身核素骨扫描等,其中有5位患者接受全身PETCT 检查)。靶向药物治疗首月相对剂量密度(relative dose intensity in first month,1M-RDI) 的计算方法为实际剂量密度/标注推荐剂量的百分比。
4统计学方法
采用SPSS 22.0统计软件包进行分析。Log-rank法对生存率进行单因素分析,Cox回归 模型进行多因素分析。P<0.05为差异具有统计学意义。
5结果
5.1患者一般资料
41例sunitinib组转移性肾透明细胞癌患者中,PNN高表达21例,低表达20例,确诊至sunitinib开始治疗时间≥1年21例,占51.1%。最常见的转移部位为肺24例(58.5%)、骨21例(51%)、肝10例(24.4%)、胰腺2例(4.9%)、后腹膜淋巴结转移14例(34.1%)、 肾上腺转移6例(14.6%)、其他(脑、软组织、腹壁、胸壁等)转移4例(9.8%);单 器官转移20例(48.7%);转移灶数目>4个37例,占90.2%。53.7%(22/41)患者的 血红蛋白<120g/L;36.6%(15/41)患者的乳酸脱氢酶≥227U/L,其中大于1.5倍乳酸脱 氢酶正常值上限的6例(14.6%);24.4%(10/41)患者的碱性磷酸酶≥125U/L;MSKCC 模型评分高危6例,中危35例;73.1%(30/41)患者的sunitinib1M-RDI≥50%。全部患者 至少接受2个周期的sunitinib靶向治疗。
36例对照组m-ccRCC患者中,PNN高表达18例,低表达18例。按照PNN高表达和低 表达将所有患者进行分组,对各组的临床病理特征进行统计分析未发现两组之间有统计 学差异(结果见表9)。
5.2多因素生存分析结果
对sunitinib治疗组病例进行Cox回归模型分析。结果显示确诊至开始治疗时间、器官 转移个数以及PNN表达强度是影响sunitinib治疗后转移性肾癌总生存的独立预后危险因 素(结果见表10)。
5.3组织中PNN的表达水平与m-ccRCC对舒尼替尼治疗反应性之间的关系
针对不同PNN表达水平情况下,分别对sunitinib和对照组(control)的m-ccRCC患者 进行生存分析。在PNN高表达组,与对照组相比,sunitinib治疗并不能延长m-ccRCC患者的总生存期(中位生存期sunitinib组17.5月,对照组14.5月,p=0.369;图10A);而 在PNN低表达组,sunitinib治疗对比对照组明显延长了m-ccRCC患者的总生存(中位生 存期sunitinib组33月,对照组17月,p=0.019;图10B)。

通过上述实施例证实:PNN在促进ccRCC细胞转移的过程中发挥了重要作用;其 次在体外细胞培养条件下,对ccRCC细胞采用逐步增加剂量法诱导构建耐药细胞株; 通过不同浓度的sunitinib处理耐药细胞和亲本细胞,观察二者对药物的耐受性;再用 PNN抑制剂处理后耐药细胞的转移能力显著性被抑制。最后结合临床病例,确定该基因 的表达与ccRCC转移之间存在正相关的关联;研究结果揭示PNN作为新的治疗靶点将 有效对抗sunitinib的耐药。

2.根据权利要求1所述的PNN抑制剂在制备逆转转移性肾癌细胞对舒尼替尼耐药性药物中的应用,其特征在于:所述的PNN抑制剂为干扰PNN的慢病毒载体。
3.根据权利要求2所述的PNN抑制剂在制备逆转转移性肾癌细胞对舒尼替尼耐药性药物中的应用,其特征在于:所述的干扰PNN的慢病毒载体为pCDH-CMV-MCS-EF1-Puro。
4.根据权利要求2所述的P PNN抑制剂在制备逆转转移性肾癌细胞对舒尼替尼耐药性药物中的应用,其特征在于:采用PNN抑制剂减低基PNN因的拷贝数进而逆转肿瘤细胞对sunitinib的耐药。
5.根据权利要求1所述的PNN抑制剂在制备逆转转移性肾癌细胞对舒尼替尼耐药性药物中的应用,其特征在于:具体包括:
(1)分析ccRCC病人组织中PNN的表达,分析其与转移、病理分级和病人愈后之间的关系;对从医院收集的ccRCC组织和癌旁组织样本,进行采用免疫组化、qRT-PCR和Westernblot方法检测乳腺癌组织和癌旁组织中PNN的表达水平;统计分析PNN的表达量与临床病理资料的关系;
(2)研究ccRCC细胞中PNN的表达,及其对细胞迁移和侵袭的影响;
(3)体内研究PNN表达对ccRCC细胞转移的影响;
(4)研究PNN表达与sunitinib耐药的关系;
(5)研究耐药对ccRCC细胞转移的影响;
(6)研究PNN表达与ccRCC对舒尼替尼耐药之间的关系;
(7)研究PNN促进ccRCC对sunitinib耐药的分子机制;
(8)分析临床舒尼替尼治疗病人耐药的潜在因素,以及和PNN表达的关系。
6.根据权利要求5所述的PNN抑制剂在制备逆转转移性肾癌细胞对舒尼替尼耐药性药物中的应用,其特征在于:所述的(2)研究ccRCC细胞中PNN的表达,及其对细胞迁移和侵袭的影响,具体为:对培养人RCC细胞系和上皮细胞HK-2,用慢病毒系统构建了稳定低表达PNN的细胞株,划痕和transwell实验研究PNN低表达与ccRCC细胞转移和侵袭的影响。
7.根据权利要求5所述的PNN抑制剂在制备逆转转移性肾癌细胞对舒尼替尼耐药性药物中的应用,其特征在于:所述的(3)体内研究PNN表达对ccRCC细胞转移的影响,具体为:采用尾静脉注射的方法,将稳定低表达PNN的Caki-1/shPNN细胞及其对照Caki-1/Control细胞接种至裸鼠体内;50天后统计裸鼠肺组织表面的结节数量,体内实验分析PNN对ccRCC细胞转移的影响。
8.根据权利要求5所述的PNN抑制剂在制备逆转转移性肾癌细胞对舒尼替尼耐药性药物中的应用,其特征在于:所述的(4)研究PNN表达与sunitinib耐药的关系,具体为:在体外细胞培养条件下,对Caki-1和786-O细胞采用逐步增加剂量法诱导构建耐药细胞株;通过不同浓度的舒尼替尼处理耐药细胞和亲本细胞,观察二者对药物的耐受性。
9.根据权利要求5所述的PNN抑制剂在制备逆转转移性肾癌细胞对舒尼替尼耐药性药物中的应用,其特征在于:所述的(5)研究耐药对ccRCC细胞转移的影响,具体为:用划痕实验比较耐药细胞和亲本细胞之间转移能力的差异;用transwell实验比较耐药细胞和亲本细胞之间转移能力的差异。
10.根据权利要求5所述的PNN抑制剂在制备逆转转移性肾癌细胞对舒尼替尼耐药性药物中的应用,其特征在于:所述的(6)研究PNN表达与ccRCC对舒尼替尼耐药之间的关系,具体为:用基因芯片方法分析PNN在耐药细胞中的表达;再用PNN抑制剂降低耐药细胞中PNN的表达后,得出其对细胞转移的影响;所述的(7)研究PNN促进ccRCC对sunitinib耐药的分子机制,具体的:用PNN抑制剂降低耐药细胞中PNN的表达后,检测EMT相关蛋白在PNN高低表达的耐药和亲本细胞之间的表达、耐药细胞中PI3K和AKT的磷酸化水平、反之升高shPNN处理的耐药细胞中PI3K的水平,得到对细胞迁移和EMT蛋白表达的影响;所述的(8)分析临床舒尼替尼治疗病人耐药的潜在因素,以及和PNN表达的关系;具体为:用Cox回归模型分析开始治疗时间、器官转移个数以及PNN表达强度对sunitinib治疗病人的影响;分析PNN表达对m-ccRCC患者的生存影响。
