背景技术
纤维化

肝纤维化

治疗

潜在的治疗药物

美国专利公开No.2015/359805涉及Farnesoid X受体(FXR)调节剂,其可用于治疗胆汁淤积性疾病,特别是其中C6含有乙基并且C24羧基被转化为硫酸盐基团的胆汁酸衍生物。建议治疗的疾病包括酒精性肝病、活体供肝移植肝再生、先天性肝纤维化、胆总管结石和肉芽肿性肝病。美国专利2014/187633涉及治疗和/或预防非酒精性肝炎(NASH)和/或原发性胆汁性肝硬化的方法,包括向有需要的受试者施用包含二十碳五烯酸或其衍生物的药物组合物。FXR激动剂,即改良胆汁酸的奥贝胆酸,正处于原发性胆汁性肝硬化的III期临床试验中。据报道,这种药物的使用通常与瘙痒等副作用有关。
熊去氧胆酸(UDCA,Ursodiol)是最常用于原发性胆汁性肝硬化的治疗剂。它是次级胆汁酸之一,是肠道细菌的代谢副产物。该药被认为有助于减少胆汁淤积并改善血液检查结果(肝功能检查)。然而,它对症状的影响很小,是否可以改善预后是有争议的。为了缓解通常由肝脏除去的循环中的胆汁酸引起的瘙痒,可以对原发性胆汁性肝硬化患者开具消胆胺(胆汁酸螯合剂)。该药剂可以帮助吸收肠道中的胆汁酸而防止其进入血流。替代药物包括康力龙、纳曲酮和利福平。
奥贝胆酸(OCA,Ocaliva)是一种半合成胆汁酸类似物,在特定肝脏和胃肠道疾病的2期和3期研究中正在开发。美国食品和药物管理局于2016年5月27日加速批准Ocaliva与熊去氧胆酸(UDCA)组合用于在对UDCA响应不足的成人中治疗原发性胆汁性胆管炎(PBC),或作为无法耐受UDCA的成人的单一治疗剂。此外,在NASH患者中进行的2期试验表明,施用OCA减少了肝脏炎症和纤维化的标志物并增加了胰岛素敏感性。
WO2014/197738和WO2016/094570涉及公开为肌成纤维细胞转分化和活化的抑制剂的小分子化合物。建议用于治疗尤其是脂肪肝的药物和组合公开于例如WO 2016/112305和EP2632925(乙酰辅酶A羧化酶抑制剂)以及WO 2016/154258(双重PPARδ/γ激动剂)。建议将一些公开的化合物与各种其他药物组合使用。
许多患者对纤维化疾病的可用治疗不响应,并且长期治疗受到毒性和副作用的限制。因此,仍然需要开发旨在减少纤维化,特别是肝纤维化的治疗方式。对已确认的肝硬化和门静脉高压以及减轻纤维化的安全有效治疗的开发将是非常有益的。
发明内容
脂肪酸胆汁酸缀合物
脂肪酸胆汁盐缀合物,也称为脂肪酸胆汁酸缀合物(FABAC),是可用于改善与胆汁酸或胆固醇代谢相关的病症的合成分子家族。据信FABAC降低血液胆固醇浓度/降低肝脏脂肪水平并溶解胆结石(Gilat等,Hepatology 2003;38:436-442;和Gilat等,Hepatology2002;35:597-600)。FABAC包括3β-花生酰氨基-7α,12α-二羟基-5β-胆甾烷-24-酸,也称为Aramchol。

适应症
本发明部分基于以下令人惊讶的发现:Aramchol不依赖于其报道的对脂肪肝和脂肪变性的活性发挥有效的抗纤维化作用,并且在各种实验模型中减少纤维化。具体地,用Aramchol(5mg/kg)治疗显著抑制体内硫代乙酰胺(TAA)模型中毒素诱导的肝硬化、坏死和肝纤维化的发展。还发现Aramchol出乎意料地优于奥贝胆酸(OCA),其在测试的实验条件下没有引起这些参数的统计学显著降低。此外,Aramchol通过PPARγ上调显著降低LX-2人肝星状细胞中的C0L1A1表达。令人惊讶地发现,Aramchol可有效地逆转已确认的纤维化,并在特异性减少星状细胞中胶原的产生方面具有效果。
因此,独立于其报道的NAFLD受试者的肝脏代谢活性,本文令人惊讶地发现Aramchol在治疗新患者群体和患者亚群方面是有效的,例如治疗非肝纤维化和以环境和/或免疫病因纤维化为特征的各种病症、NAFLD或NASH患者的肝纤维化,治疗NAFLD或NASH患者的肝纤维化和晚期纤维化(即2期或3期纤维化),治疗患有肝硬化(即4期纤维化)的NAFLD或NASH患者的肝纤维化,治疗因接触药物、毒素或手术引起的肝纤维化,特别是减轻肝硬化。本发明有利地提供了这些新患者群体的治疗,其具有增强的功效和/或安全性以及最小化的副作用。

例如,心内膜心肌纤维化是特发性类型的心肌病,其在非洲的各个地区流行,在其他地区很少,以心脏肥大为特征,心内膜明显增厚,致密的白色纤维组织可延伸至涉及内心肌,和充血性心力衰竭。
特发性肺纤维化(例如弥漫性特发性间质纤维化、弥漫性间质性肺纤维化)是肺泡壁的慢性炎性进行性纤维化,具有稳定的进行性呼吸困难,导致缺氧或右心衰竭导致的死亡。大多数病例来源不明,但有些病例被认为是由尘肺病、过敏性肺炎、硬皮病和其他疾病引起的。
纵隔纤维化的特征在于在纵隔的上部形成硬白色纤维组织,有时阻塞空气通道和大血管;也称为纤维化或纤维性纵隔炎。

施用两种药物治疗特定疾病,如非酒精性脂肪性肝病(NAFLD),会引发许多潜在的问题。两种药物之间的体内相互作用是复杂的。任何单一药物的作用与其吸收、分布和清除有关。当两种药物被引入体内时,每种药物都会影响另一种药物的吸收、分布和清除,从而改变另一种药物的作用。例如,一种药物可以抑制、激活或诱导参与清除另一种药物的代谢途径的酶的产生。(工业指南,1999)因此,当给予两种药物来治疗相同的病症时,每种药物是否会补充、影响或干扰其他药物在人类受试者中的治疗活性是不可预测的。

另外,难以准确地预测两种药物之间相互作用的影响何时变得明显。例如,药物之间的代谢相互作用可以在第二种药物的初始施用后,在两种药物达到稳态浓度之后或在药物之一停止时变得明显。(Guidance for Industry,1999)

实施例1-硫代乙酰胺(TAA)诱导的肝纤维化——用于肝硬化的模型
通过在10周内每周两次腹膜内注射TAA(20mg/100g体重)在Wistar大鼠中诱导肝纤维化。腹腔TAA的应用导致肝脏中心性坏死、转氨酶活性升高和肝纤维化强烈。治疗组还包括共同给予Aramchol(1或5mg/kg口服)或奥贝胆酸(OCA,5mg/kg)。还包括对照组的盐水处理的大鼠(没有施用TAA)。然后处死大鼠,并在Masson Goldner染色后肉眼观察肝脏的肝硬化和坏死病变的迹象,并在显微镜下观察。对每个样品测定以0-4的等级计算的纤维评分,其中0表示无纤维化,4表示晚期纤维化和肝硬化。

来自TAA中毒的肝硬化和门静脉高压可能最终导致急性肝衰竭和肝性脑病等相关病症的发展,并且TAA模型也用于评估这些现象。因此,如本文所公开的,在一些实施方案中,Aramchol也可用于预防急性或致命的肝衰竭和/或肝或门体系统脑病,例如毒素诱导的肝衰竭和/或肝性脑病。
实施例2-抑制星状细胞中胶原蛋白的合成
将LX2细胞(每孔150,000个细胞)接种在含有抗生素、谷氨酰胺和牛胎儿血清的DMEM培养基中。孵育24小时后,将培养基更换为0%血清并再培养16小时。然后,加入Aramchol(10mM),24小时后用Trizol提取RNA。
令人惊讶的是,如图3和4中可见,通过PPARγ上调,Aramchol降低了LX-2人肝星状细胞中的COL1A1表达。

实施例3-Aranchole在MCD饮食动物模型中减少确认的纤维化
下面描述的研究使用NASH的0.1%蛋氨酸和胆碱缺乏(0.1MCD)的饮食小鼠模型研究Aramchol的作用机制及其对纤维化的潜在影响。

讨论
基于本文所述的研究,令人惊讶地发现Aramchol是有效的抗纤维化和抗肝硬化剂。还发现Aramchol出乎意料地优于OCA,并且为肝纤维化提供了改进的有效治疗。因此,Aramchol可用于预防急性或致命的肝衰竭和/或肝或门体系统脑病,例如毒素诱导的肝衰竭和/或肝性脑病。
此外,还令人惊讶地发现Aramchol可有效逆转已确认的纤维化。Aramchol治疗通过减少脂质积累(苏丹红染色)、纤维化(天狼星红和SMA染色)和炎症(F4/80和CD64染色)来确定改善肝脏组织学。实际上,除了NASH的主要病理学外,Aramchol还对纤维化有影响,即脂肪变性和炎症。

Claims (40)
