Abstract

本发明公开了一种复合材料食管补片,其特征在于:由补片本体和涂覆在补片本体的一个表面上的生物胶层构成,所述补片本体的材料为聚四氟乙烯纳米复合材料。本发明不需要使用支架结构即可实现气管食管瘘的瘘口修补,避免了现有技术中使用支架修补造成的各种问题;同时,采用补片结构,由于生物胶层的对组织缺损的封闭能力,可以对气管食管瘘进行放射治疗。通过在聚四氟乙烯中加入石墨烯和无机粉末,提升了材料的力学性能和耐磨性能,同时材料具备高度的生物相容性、弹性、韧性,具有抗拉伸、耐酸碱、耐辐射的特性。

Description

一种复合材料食管补片
技术领域
本发明涉及一种医用器械,具体涉及一种用于气管食管瘘治疗的复合材料食管补片。
背景技术
恶性气管食管瘘(malignant tracheoesophageal fistula, MTEF)是一种源于食管、等部位亚性肿瘤的严重并发症,其预后差,缺少有效根治性手段,迫切需要寻找新的有效治疗方法。
手术和食管带膜支架植入是目前最常用的治疗手段,但均不理想。手术治疗经选择的MTEF成功的患者,平均生存期达到13个月,但是术中死亡风险高达14.3%,且绝大多数患者由于局部炎症水肿,合并感染,肿瘤晚期无法手术。食管带膜支架植入也存在很多缺陷:1.支架需要覆盖瘘管近端及远端至少2cm的正常食管壁,要足够宽以便能够牢固固定。然而,上述条件有时并不易满足。2.支架刺激食管黏膜肉芽增生,放置一段时间后无法取出,同时改变了食管原有弹性、伸缩性等生物学特性,反流性食管炎、胸痛、出血等并发症较多。病灶位于食管上段者异物感明显,患者往往难以耐受。3.缺乏后续有效控制肿瘤的手段,对患者生存时间无明显延长,中位生存期仅8周左右。
另外,放疗在中晚期食管癌治疗中具有重要地位,但气管食管瘘一旦发生就是放疗的绝对禁忌证,即使在支架植入后,由于放疗有导致肿瘤坏死,瘘口进一步扩大,延长瘘口愈合时间的风险,也不适宜继续放疗。
发明内容
本发明的发明目的是提供一种复合材料食管补片,在治疗气管食管瘘时最在程度地保留食管原有生物学特性。
为达到上述发明目的,本发明采用的技术方案是:一种复合材料食管补片,包括补片本体,所述补片本体采用聚四氟乙烯纳米复合材料,补片本体的一个表面上涂覆有生物胶层。
上述技术方案中,通过补片对气管食管瘘的瘘口部位进行修补,采用聚四氟乙烯纳米复合材料制备补片本体,提供了高度的生物相容性、弹性、韧性,具有抗拉伸、耐酸碱、耐辐射的特性。在补片本体的一个表面上涂覆生物胶层,生物胶又称为纤维蛋白胶(fibringlue,FG),由生物组织中提取的多种可凝性蛋白质组成,模拟凝血过程的最后一步即纤维蛋白原转变成纤维蛋白的过程。采用生物胶一方面可实现补片的粘合固定,另一方面,能有效地封闭组织缺损,促进伤口愈合、防止组织粘连。同时,由于生物胶具有稳定的三维网状结构,根据需要可以由其承载药物并实现药物的稳定释放。
上述技术方案中,所述聚四氟乙烯纳米复合材料由聚四氟乙烯、石墨烯和无机粉末混合冷压成型后加热制备获得,所述聚四氟乙烯的质量为材料总质量的65%~90%,石墨烯的质量为材料质量的0.1%~10%,其余为无机粉末;原料中,所述聚四氟乙烯的粒径为20~100微米,所述石墨烯过200目筛,所述无机粉末为粒径1~40微米的碳酸钙、滑石粉、二硫化钼、石墨、玻璃纤维、碳纤维中的一种或多种的混合物。
通过上述配比制备获得聚四氟乙烯纳米复合材料,可以有效提升材料的综合性能,使材料具良优异的力学性能、耐磨性能。
进一步的技术方案,所述补片本体采用下列方法制备获得:
(1) 将聚四氟乙烯、石墨烯和超细无机粉末混合均匀,形成混合粉料;
(2) 将混合粉料置于模具内,在30~50MPa的压力下冷压成型,并保压2~5分钟,脱膜获得片状物;
(3) 将所得片状物以150~200℃/小时的升温速率,升温至370~380℃,保温2小时,冷却获得补片本体。
由于上述技术方案运用,本发明与现有技术相比具有下列优点:
1、本发明提供了一种食管补片,通过在补片本体上结合生物胶,实现补片的固定,因而不需要使用支架结构即可实现气管食管瘘的瘘口修补,避免了现有技术中使用支架修补造成的各种问题;同时,采用补片结构,由于生物胶层的对组织缺损的封闭能力,可以对气管食管瘘进行放射治疗。
2、本发明中制备补片本体的材料采用聚四氟乙烯纳米复合材料,通过在聚四氟乙烯中加入石墨烯和无机粉末,提升了材料的力学性能和耐磨性能,同时材料具备高度的生物相容性、弹性、韧性,具有抗拉伸、耐酸碱、耐辐射的特性。
具体实施方式
下面结合实施例对本发明作进一步描述:
实施例一:
一种复合材料食管补片,由补片本体和涂覆在补片本体的一个表面上的生物胶层构成,所述补片本体的材料为聚四氟乙烯纳米复合材料。
所述补片本体采用下列方法制备获得:
(1) 石墨烯过200目筛,与粒径约25微米的聚四氟乙烯粉末、超细碳酸钙粉末混合,以8000r/min转速间歇搅拌混合2~3min,形成混合粉料;其中聚四氟乙烯粉末的质量为85%,石墨烯的质量为6%,其余为碳酸钙粉末。
(2) 将混合粉料置于模具内,在34.5MPa的压力下冷压成型,并保压2~5分钟,脱膜获得片状物;所述模具形状设计使得的获得的片状物符合食管解剖生理特性。
(3) 将所得片状物以100℃/小时的升温速率,升温至375℃,保温2小时,自然冷却至室温,获得补片本体。
为说明本实施例获得的产品的性能,对本实施例的产品进行性能测定,经检测,食管补片的抗弯刚度、食管补片弹性回复率均匀符合补片应用的要求。
对食管补片进行抗辐射能力测试:
应用直线加速器,给予本实施例制备的复合材料食管补片50Gy,70Gy,90Gy放疗剂量,检测其放疗前后抗弯刚度和弹性回复率的变化情况。
结果表明,本发明获得的补片具有良好的抗辐射能力,可以适应防射治疗的要求。
对生物胶进行安全性检测:
将生物胶与20mM的醋酸1:30配成0.1mg/ml的液体,以50μl/cm2的量均匀涂于6cm2培养皿表面,放于4 ℃冰箱过夜,接种1小时前吸除液体,放于37℃晾干,无菌PBS液冲洗2遍,再以细胞培养液冲洗1遍。分离原代食管细胞,以将105/皿的密度接种于涂有或未涂有生物胶的培养皿表面,培养6天后小心移除培养基,20mM的醋酸溶解生物胶后行细胞计数,比较生物胶对正常食管细胞生长的影响。
结果表明,本发明使用的生物胶具备生物安全性。

1.一种复合材料食管补片,其特征在于:由补片本体和涂覆在补片本体的一个表面上的生物胶层构成,所述补片本体的材料为聚四氟乙烯纳米复合材料。
2.根据权利要求1所述的复合材料食管补片,其特征在于:所述聚四氟乙烯纳米复合材料由聚四氟乙烯、石墨烯和无机粉末混合冷压成型后加热制备获得,所述聚四氟乙烯的质量为材料总质量的65%~90%,石墨烯的质量为材料质量的0.1%~10%,其余为无机粉末;原料中,所述聚四氟乙烯的粒径为20~100微米,所述石墨烯过200目筛,所述无机粉末为粒径1~40微米的碳酸钙、滑石粉、二硫化钼、石墨、玻璃纤维、碳纤维中的一种或多种的混合物。
3.根据权利要求2所述的复合材料食管补片,其特征在于:所述补片本体采用下列方法制备获得:
(1) 将聚四氟乙烯、石墨烯和超细无机粉末混合均匀,形成混合粉料;
(2) 将混合粉料置于模具内,在30~50MPa的压力下冷压成型,并保压2~5分钟,脱膜获得片状物;
(3) 将所得片状物以100~200℃/小时的升温速率,升温至370~380℃,保温2小时,冷却获得补片本体。
