具体实施方式
综述
医疗探头诸如导管用于多种治疗和诊断医学规程中,例如心脏消融术中。导管远侧端部通常为多功能的并且包括很多元件诸如不同类型的电极、电极的电互连件、冲洗管和孔。所述远侧端部通常在尺寸上有所限制,这限制了可在其中结合的功能。
在下文描述的本发明的实施方案为医疗探头提供了改善的远侧端部组件构型,这克服了上述限制。在一些实施方案中,远侧端部组件由在小体积内提供各种功能的挠性基底的单个片材制成,诸如:(i)形成用于安装或嵌入电装置诸如电极或传感器的机械基底,(ii)包括将电信号传送至电装置或从电装置传送电信号的导电迹线,和/或(iii)包括用于冲洗组织区域的冲洗管和孔。本发明所公开的技术还描述了用于制作这些导管远侧端部的方法。
所述远侧端部组件通常包括在其顶部上具有盖的圆柱形插入管。在一些实施方案中,所述插入管包括内骨骼结构,并且该管的圆柱形形状通过围绕骨骼结构包裹挠性基底的片材诸如多层印刷电路板(PCB)形成。
在一些实施方案中,远侧端部的电线中的至少一些使用挠性PCB中或上的电路迹线来实施。所述迹线被配置成将联接到PCB(例如,电极或传感器)的电装置电连接至穿过导管的线。一些类型的电装置(例如,热电偶)可被嵌入在PCB层之间。在围绕插入管包裹PCB之前或之后,其他装置诸如微电极可安装在PCB表面上。在其他实施方案中,该PCB被穿孔,以便形成冲洗孔。
在一个实施方案中,该PCB包括具有两个边缘的片材,和穹顶状盖。该片材被配置成围绕插入管的骨骼结构包裹,并且通过焊接或使用任何其他合适的联接技术将两个边缘附接到彼此。在一些实施方案中,穹顶状盖可被胶合到远侧端部组件的顶点,并且可包括冲洗孔和安装在其上的一个或多个尖端电极。在另选的实施方案中,穹顶状盖由液晶聚合物(LCP)PCB制成,该穹顶状盖可被热成形为杯形形状并粘结到顶点。片材和穹顶状盖可被形成为PCB的单个连续件,或可在围绕远侧端部组件包裹之后彼此联接的单独部件。
在另选的实施方案中,插入管的远侧部分为连续的,并且上述导电迹线和电装置通过围绕插入管直接镀覆一个或多个膜来形成。至少最外侧的膜典型地具有生物相容性,以允许远侧端部组件与心脏之间的直接物理接触。在一些实施方案中,插入管和挠性基底可被穿孔,以便允许冲洗流经由冲洗孔离开插入管。
本公开的技术有助于增强医疗导管的功能性而不影响尺寸,并且为新的应用实现更小的导管。此外,使用非常大的集成电路(VLSI)技术为生产者提供具有高柔韧性的挠性基底以改善导管以及降低远侧端部的成本。例如,所公开的技术能够整合所有电装置和挠性基底中的电互连件围绕低成本的模制塑料组件包裹,而不是制作昂贵的微机械金属管、线和电极,并将它们安装到金属管上。
系统说明
图1为根据本发明的实施方案的导管插入术系统20的示意性图解。系统20包括探头22(在本示例中为心脏导管)和控制台24。在本文所述的实施方案中,导管22可用于任何合适的治疗的和/或诊断目的,诸如心脏26中的组织的消融。
控制台24包括处理器42(通常是通用计算机),该处理器42具有合适的前端和接口电路以从导管22接收信号并控制本文所述的系统20的其他部件。
操作者30(诸如介入心脏病专家)将导管22穿过躺在手术台29上的患者28的血管系统插入。导管22包括远侧端部组件40,其在图2-4中详细描述。操作者30通过用靠近导管的近侧端部的操纵器32来操纵导管22,而使组件40在心脏26中的目标区域附近移动,如图1的插图所示。导管22的近侧端部连接至处理器42中的接口电路。
在一些实施方案中,远侧端部组件在心腔中的位置通常通过导管跟踪系统20中的磁性位置感测来测量。在这种情况下,控制台24包括驱动电路34,驱动电路34驱动放置在躺在手术台29上的患者28体外已知位置处(例如,患者的躯干下方)的磁场发生器36。
图2为根据本发明实施方案的远侧端部组件40的示意性分解图解。在一些实施方案中,远侧端部组件40包括内部构件69,其可由塑料或任何其他合适的材料制成。内部构件在本文中也被称为插入管。该内部构件可具有如图2中所示的骨骼支撑结构,或将在下文其他实施方案中描述的任何其他合适的结构。
基部58位于构件69的近侧端部处,环状穹顶支撑件68位于构件69的顶点处,并且多个肋56连接基部和穹顶支撑件。在该实施方案中,构件69具有用于将冲洗引入由挠性印刷电路板(PCB)片材60形成的腔体的内部的内腔。
远侧端部组件40还包括挠性基底,诸如多层挠性印刷电路板(PCB)片材60,其被配置成围绕构件69包裹。在一个实施方案中,在包裹之后,片材60的左边缘和右边缘可通过焊接或使用任何其他合适的联接技术彼此联接。在一些实施方案中,一个或多个电装置可被安装在PCB片材60上,如将在下文图3中所描绘的。
PCB片材60通常包括电互连件,诸如导电迹线(未示出),其被配置成将联接到PCB的电装置电连接到穿过导管的合适的线,或到其他合适的电路。在一些实施方案中,PCB片材60可被穿孔,以便形成一个或多个冲洗孔64,该冲洗孔被配置成例如在消融规程期间冲洗心脏26的组织时允许冲洗流体流出冲洗管。
远侧端部组件40还包括穹顶状盖66,其由挠性PCB制成并被配置成围绕穹顶支撑件68包裹。在一些实施方案中,穹顶状盖66可胶合到穹顶支撑件68。在另选的实施方案中,盖66可由液晶聚合物(LCP)PCB制成,其可被成形(例如,热成形)为杯形形状并粘结到片材60。可使用本领域中已知的任何合适的粘结技术将该杯形形状粘结到穹顶支撑件68并将片材60粘结到基部58。
在一些实施方案中,片材60和盖66由单片连续PCB或任何其他合适的挠性基底制成。在另选的实施方案中,片材60和盖66可由单独的材料片形成,并使用焊接或任何其他合适的联接技术彼此联接。
图3为根据本发明实施方案的远侧端部组件40的示意性分解图解。图3示出了片材60和盖66的与图2中示出的那些相反的表面。在一些实施方案中,片材60可包括一个或多个微电极62,其被配置成读取信号和阻抗。盖66旨在在消融规程期间面对心脏26的内部组织。在一些实施方案中,一个或多个热电偶(未示出)可以被嵌入在PCB片材60的各层之间。热电偶可使用PCB制造技术诸如溅射来形成,或提前制备并嵌入在片材60的各层之间。
在一个实施方案中,盖66可包括尖端电极71,其被配置成消融心脏26的内部组织。在一些实施方案中,盖66可包括在尖端电极71的各部分之间的PCB上形成的非金属塑料部件70。在一个实施方案中,部件70可被穿孔,以便帮助塑料部件70粘结到PCB。
除此之外或另选地,远侧端部组件40可包括一个或多个位置传感器,该位置传感器使得磁性位置跟踪系统能够测量和跟踪其在体内的位置。
在本发明的上下文中,尖端电极71、微电极62和热电偶、热敏电阻器和位置传感器被视为联接到远侧端部组件的挠性基板的电装置的示例。在另选的实施方案中,可使用任何其他合适类型的电装置,例如压力传感器。
在一些实施方案中,微电极62和尖端电极71可被分别直接印刷在PCB片材60上和穹顶状盖66上。在另选的实施方案中,电极(例如微电极62)中的至少一些可例如被安装在片材60的内侧或外侧上,并被粘结到电互连件。电装置可经由包括在片材60和盖66中的电互连件以及穿过导管22的线被电连接到控制台24。
图4为根据本发明实施方案的远侧端部组件40的示意性图解。PCB片材60和穹顶状盖66围绕构件69包裹,使得冲洗孔64和电装置诸如微电极62和尖端电极71面向心脏26的内部组织。
图2-4所示的远侧端部装置40的构型是纯粹为了概念清楚的目的而选择的示例性构型。在替代实施方案中,可以使用任何其它合适的构型。例如,用于实施远侧端部装置的挠性基底可包括任何其他合适的基底,不一定是PCB。
在另选的实施方案中,远侧端部组件包括由塑料或任何其他合适材料制成的连续的中空插入管,并且该插入管在其远侧末端处具有顶盖而不是骨骼形状。该插入管被配置成形成用于经由导管22引导电线和/或冲洗管的内腔。在该实施方案中,可使用镀覆或任何其他合适的设置技术将上述电互连件和绝缘膜中的至少一些作为挠性基底直接设置到插入管上。
所设置的膜(例如,最外侧的膜)中的至少一些可具有生物相容性,以允许远侧端部组件与心脏组织之间的直接物理接触。此外,电极或其他电装置可直接镀覆在插入管上或在形成电互连件之后联接到该管。在一个实施方案中,插入管可使用本领域已知的超大规模集成(VLSI)工艺来穿孔以便形成冲洗孔,该工艺诸如沉积(例如,溅射、物理气相沉积(PVD)和镀覆)、图案化(例如,直接沉积在掩膜上,或光刻和蚀刻)、和/或用于在插入管中穿孔出冲洗孔的激光钻孔。
尽管本文所述的实施方案主要论述消融导管,但本文所述的方法和系统也可用于任何其他合适的医疗探头中,诸如鼻窦成形术和神经外科手术。
应当理解,上述实施方案以举例的方式引用,并且本发明不限于上文具体示出和描述的内容。相反,本发明的范围包括上文所述各种特征的组合和子组合,以及本领域技术人员在阅读上述说明时会想到且未在现有技术中公开的其变型形式和修改形式。以引用方式并入本专利申请的文献将视为本专利申请的整体部分,不同的是如果在这些并入的文献中定义的任何术语与在本说明书中明确或隐含地给出的定义相冲突,则应只考虑本说明书中的定义。

Claims (35)

1.一种导管,包括:
内部构件,所述内部构件用于插入患者身体中,所述内部构件具有基部,其通过从所述基部延伸的多个肋连接到环形穹顶支撑件,所述环形穹顶支撑件位于所述内部构件的远侧端部的顶点处;
成片材的第一挠性基底,其具有第一边缘和第二边缘,所述成片材的第一挠性基底被配置成围绕所述内部构件的远侧端部包裹使得所述成片材的第一挠性基底的第一边缘和第二边缘彼此连接而形成管,并且所述挠性基底包括电互连件;
成片材的第二挠性基底,其被配置成围绕所述环形穹顶支撑件包裹以限定穹顶状盖,其联接到由所述成片材的第一挠性基底形成的管;和
一个或多个电装置,所述一个或多个电装置被联接到所述成片材的第一挠性基底并被连接到所述电互连件。
2.根据权利要求1所述的导管,其中所述内部构件由塑料制成。
3.根据权利要求1所述的导管,其中所述内部构件为中空的,以便形成用于将一个或多个电线引导到所述远侧端部的内腔。
4.根据权利要求1所述的导管,其中所述内部构件为中空的,以便形成用于将一个或多个冲洗管引导到所述远侧端部的内腔。
5.根据权利要求1所述的导管,其中所述成片材的第一挠性基底包括挠性印刷电路板(PCB),并且其中所述电互连件包括导电迹线。
6.根据权利要求1所述的导管,其中所述成片材的第一挠性基底包括一个或多个膜,所述一个或多个膜镀覆所述远侧端部,以便形成所述电互连件和所述电装置中的至少一些。
7.根据权利要求1所述的导管,其中所述成片材的第一挠性基底被穿孔,以便形成冲洗孔。
8.根据权利要求1所述的导管,其中所述电装置包括一个或多个电极。
9.根据权利要求1所述的导管,其中所述电装置包括一个或多个热电偶。
10.根据权利要求1所述的导管,其中所述电装置包括一个或多个热敏电阻器。
11.根据权利要求1所述的导管,其中所述电装置包括一个或多个压力传感器。
12.根据权利要求1所述的导管,其中所述电装置包括一个或多个位置传感器。
13.根据权利要求1所述的导管,其中所述穹顶状盖被配置成覆盖所述远侧端部的顶点。
14.根据权利要求13所述的导管,其中所述成片材的第一挠性基底和所述穹顶状盖构成单个连续部分。
15.根据权利要求13所述的导管,其中在围绕所述远侧端部包裹时,所述成片材的第一挠性基底的所述第一边缘和所述第二边缘焊接到彼此。
16.根据权利要求13所述的导管,其中所述穹顶状盖被胶合到所述远侧端部的所述顶点。
17.根据权利要求13所述的导管,其中所述穹顶状盖由液晶聚合物(LCP)PCB制成,所述液晶聚合物PCB被配置成形成为杯形形状,并且其中所述杯形形状被配置成粘结到所述远侧端部的所述顶点。
18.一种用于制作导管的方法,包括:
提供内部构件,其具有基部,所述基部通过从所述基部延伸的多个肋连接到环形穹顶支撑件,所述环形穹顶支撑件位于所述内部构件的远侧端部的顶点处;
围绕所述内部构件的远侧端部包裹具有第一边缘和第二边缘的成片材的第一挠性基底,使得所述成片材的第一挠性基底的第一边缘和第二边缘彼此连接而形成管,其中所述挠性基底包括一个或多个电互连件;
将一个或多个电装置联接到所述成片材的第一挠性基底以及将所述电装置连接到所述电互连件;以及
将成片材的第二挠性基底形成为穹顶状盖,以围绕所述内部构件的环形穹顶支撑件包裹且联接到由所述成片材的第一挠性基底形成的管。
19.根据权利要求18所述的方法,其中提供所述内部构件包括提供塑料管。
20.根据权利要求18所述的方法,其中提供所述内部构件包括提供中空的内部构件,以便形成用于将一个或多个电线引导到所述远侧端部的内腔。
21.根据权利要求18所述的方法,其中提供所述内部构件包括提供中空的内部构件,以便形成用于将一个或多个冲洗管引导到所述远侧端部的内腔。
22.根据权利要求18所述的方法,其中包裹所述成片材的第一挠性基底包括包裹挠性印刷电路板(PCB),并且其中所述电互连件包括导电迹线。
23.根据权利要求18所述的方法,其中包裹所述成片材的第一挠性基底包括用一个或多个膜直接镀覆所述远侧端部,以便形成所述电互连件和所述电装置。
24.根据权利要求18所述的方法,并且包括将所述成片材的第一挠性基底穿孔,以便形成冲洗孔。

33.根据权利要求30所述的方法,其中覆盖所述顶点包括将所述穹顶状盖胶合到所述远侧端部的所述顶点。
34.根据权利要求30所述的方法,其中覆盖所述顶点包括将所述穹顶状盖形成为杯形形状以及将所述杯形形状的穹顶状盖粘结到所述远侧端部的所述顶点。
