Abstract
本发明涉及一种治疗小儿厌食症的香苓开胃颗粒,其原料包括如下重量的组分:广藿香28g,茯苓28g,苍术15g,丁香15g,鸡内金15g,佩兰10g,法半夏20g,兰花参8g,山土瓜12g,薏苡仁5g,桂枝5g,白芍5g,莱菔子5g,佛手5g。该组合物从改变剂型,提高药效,增进口感等方面入手,研制成方便服用、安全有效的香苓开胃颗粒。

技术领域
本发明涉及中药领域,具体涉及一种治疗小儿厌食症的香苓开胃颗粒。
背景技术
小儿厌食症是指非疾病因素导致的较长时期见食不贪、食欲不振、食量减少、甚则拒食的一种常见病,多见于1岁-6岁小儿。病程迁延,常因患儿胃肠功能紊乱、营养缺乏和免疫功能低下致使诸症蜂起,导致反复呼吸道感染、佝偻病等疾病的发生,严重影响儿童的生长发育和身心健康。近年来,儿童厌食症的发病率日趋上升,城市儿童发病率较高,引起了广泛关注。
《素问·痹论》曰:“饮食自倍,肠胃乃伤。”钱乙《小儿药证直诀》中言:“脾胃不和,不能乳食,致肌瘦,亦因大病,或吐泻后脾胃尚弱,不能传化谷气也。”,李东垣云:“百病皆由脾胃衰而生也”,脾胃虚弱,后天之本生化无源,百病乃生”。近代医家认为:饮食不节,喂养不当或长期偏食,损伤脾胃正常运化功能,导致脾胃不和,受纳运化失健是本病发生的主要病因病机。
现代医学认为小儿厌食症除了不良的饮食习惯、喂养不当、心理因素外,还与微量元素锌缺乏、胃肠疾病、Hp感染等也有密切关系。治疗方面应进行喂养指导,配合心理疗法,多酶片能增加消化酶,补锌剂增强味蕾效应,促胃动力药可增强胃肠蠕动、增加有益菌而改善肠道菌群。这些治疗虽然可以增进患儿食欲,但难以整体调理患儿的体质。
临床报道治法众多,从外而治,可运用推拿、针灸、耳压、敷贴等疗法。从内而治,主要从脾胃辨证,以健脾开胃为主。然中医治疗讲究三因治宜,在选方用药上要依据当地气候特点,尽可能使用道地药材。由于全球极端天气频发(冷热无常)、贪凉饮冷、滥用抗生素等导致脾胃失和,运化失常,湿邪阻遏,气机失调的比为湿困型厌食,患儿表现为不思饮食,容易困倦,舌苔厚腻,尊儿科大家江育仁江老“脾健不在补,贵在运,还在醒”用芳香醒脾化湿开胃之法,创制“香苓开胃颗粒”。云南属立体交叉气候,早晚温差大,湿气较重,故治宜芳香化湿,香苓开胃颗粒正是基于本土特点,精选重要南药为君药,故临床疗效确切。但传统汤剂熬煮加工费时,味苦量多,不便服用,儿童依从性差。故发明从改变剂型,提高药效,增进口感等方面入手,研制成方便服用、安全有效的香苓开胃颗粒。

发明内容
本发明在前期研究和长期临床观察的基础上,提炼精选处方,研制成口感好、成本低、安全、服用方便、疗效可靠、适用范围广的医院制剂,以惠及广大患儿。该处方组成多为云南道地药材,又针对儿童常见病而设,且云南本土尚缺此类医院制剂,故香芩开胃汤具备较好的市场前景和医疗价值,并有助于开发利用我省天然药物资源,促进云南经济社会发展。
香苓开胃汤是云南省名中医熊磊用于治疗小儿厌食症的临床效验方,由藿香、茯苓、太子参、苍术、焦山楂、神曲、益智仁等组成(多为药食同源药,其中藿香、茯苓、益智仁等为重要滇产药及南药),具有醒脾健运,开胃和中的功效,用于治疗小儿厌食疗效确切,总有效率达91.4%。
本发明的目的即是提供一种效果显著的,用于治疗小儿厌食症的香苓开胃颗粒,其原料包括如下重量的组分:广藿香5-30g,茯苓5-30g,苍术5-30g,丁香5-30g,鸡内金5-30g,佩兰5-30g,法半夏5-30g,兰花参5-30g,山土瓜5-30g,薏苡仁5-30g,桂枝5-30g,白芍5-30g,莱菔子5-30g,佛手5-30g。
优选的,各组分的重量为:广藿香20-28g,茯苓20-28g,苍术10-25g,丁香10-25g,鸡内金5-15g,佩兰5-15g,法半夏10-25g,兰花参5-15g,山土瓜5-15g,薏苡仁5-10g,桂枝5-10g,白芍5-10g,莱菔子5-10g,佛手5-10g。
进一步优选的,各组分的重量为:广藿香28g,茯苓28g,苍术15g,丁香15g,鸡内金15g,佩兰10g,法半夏20g,兰花参8g,山土瓜12g,薏苡仁5g,桂枝5g,白芍5g,莱菔子5g,佛手5g。
“脾健贵运不贵补”,方中藿香性微温味辛,归脾胃肺经,芳香醒脾,化浊开胃,《本草正义》云“为湿困脾阳,倦怠无力,饮食不好,舌苔浊垢之最捷之药”。茯苓性平味甘淡,利湿而不损正气,免伤小儿脏腑之娇嫩。《本草崇原》云“有土位中央而枢机旋转之功”,二药共为君药,两药合用共奏芳香运脾之效。苍术性温味辛,可疏脾湿而通其滞,为运脾之要药;公丁香性温,味辛,温中降逆,散寒止痛,暖脾胃而行气滞,可止腹痛而消腹胀。法夏性温,味辛,功能燥湿化痰,降逆止呕,则痰鸣呕恶苔腻者得之可愈。其与公丁相配,彰温中降逆之效。三药共为臣药,温中健脾,燥湿化痰。佩兰性温味平,入脾胃经,与藿香配伍,增芳香化湿,醒脾和中之功。鸡内金性甘,味平,消食健脾,积滞得消则气机舒畅,运化有常。山土瓜性平味甘、淡,健脾消积,利湿;兰花参性平,味甘微苦,《滇南本草》云:"补虚损,止自汗、盗汗,调养元气,治五劳七伤,诸虚百损,益气滋阴。"四药共为佐药,小儿久病伤脾,气虚不固,配伍此补益之品,自汗盗汗者皆可获效。薏苡仁,利水渗透湿,健脾止泻;桂枝,散寒止痛、通阳化气;白芍,养血调经,敛阴止汗,柔肝止痛,平抑肝阳;莱菔子,用于饮食停滞;佛手疏肝理气,和胄止痛。以上五药共为使药。诸药共用,消补兼施,共奏醒脾健运,开胃和中的功效。
各原料药的功效如下:
藿香:为唇形科植物广藿香或藿香的全草。快气,和中,辟秽,祛湿。治感冒暑湿,寒热,头痛,胸脘痞闷,呕吐泄泻,疟疾,痢疾,口臭。
茯苓:多孔菌科真菌茯苓Poria cocos(Schw.)Wolf的干燥菌核。多于7~9月采挖,挖出后除去泥沙,堆置“发汗”后,摊开晾至表面干燥,再“发汗”,反复数次至现皱纹、内部水分大部散失后,阴干,称为“茯苓个”;或将鲜茯苓按不同部位切制,阴干,分别称为“茯苓皮”及“茯苓块”。利水渗湿,健脾宁心。用于水肿尿少,痰饮眩悸,脾虚食少,便溏泄泻,心神不安,惊悸失眠。
苍术:菊科植物茅苍术Atractylodes lancea(Thunb.)DC.或北苍术Atractylodeschinensis(DC.)Koidz.的干燥根茎。春、秋二季采挖,除去泥沙,晒干,撞去须根。燥湿健脾,祛风散寒,明目。用于脘腹胀满,泄泻,水肿,脚气痿躄,风湿痹痛,风寒感冒,夜盲。
丁香:桃金娘科植物丁香Eugenia caryophllata Thunb.的干燥花蕾。当花蕾由绿色转红时采摘,晒干。温中降逆,补肾助阳。用于脾胃虚寒,呃逆呕吐,食少吐泻,心腹冷痛,肾虚阳痿。
鸡内金:雉科动物家鸡Gallus gallus domesticus Brisson的干燥沙囊内壁。杀鸡后,取出鸡肫,趁热立即剥下内壁(不要先用水洗,否则难剥离且易破碎),洗净,干燥。健胃消食,涩精止遗。用于食积不消,呕吐泻痢,小儿疳积,遗尿,遗精。
佩兰:菊科植物佩兰Eupatorium fortunei Turcz.的干燥地上部分。夏、秋二季分两次采割,除去杂质,晒干。芳香化湿,醒脾开胃,发表解暑。用于湿浊中阻,脘痞呕恶,口中甜腻,口臭,多涎,暑湿表症,头胀胸闷。
法半夏:半夏的炮制加工品。取净半夏,大小分开,用水浸泡至内无干心,取出;另取甘草适量,加水煎煮二次,合并煎液,倒入用适量水制成的石灰液中,搅匀,加入上述已浸透的半夏,浸泡,每日搅拌1~2次,并保持浸液pH值12以上,至剖面黄色均匀,口尝微有麻舌感时,取出,洗净,阴干或烘干,即得。每100kg净半夏,用甘草15kg、生石灰10kg。燥湿化痰。用于痰多咳喘,痰饮眩悸,风痰眩晕,痰厥头痛。
兰花参:桔梗科植物兰花参的根或带根全草。夏季采收,晒干。补虚,解表。治虚损劳伤,咳血、衄血,自汗、盗汗,妇女白带,伤风咳嗽,胃痛,泻痢,刀伤。
山土瓜:旋花科番薯属植物山土瓜Ipomoea hungaiensis Lingelsh.et Borza,以块根入药。秋季采挖,洗净,切片,晒干。健脾消积,利湿。用于小儿疳积,乳汁缺乏,肝炎,白带;外用治烧烫伤。
薏苡仁,为禾本科植物薏苡Coix lacryma-jobi L.var.mayuen(Roman.)Stapf的干燥成熟种仁。有利水渗透湿,健脾止泻,除痹,排脓,解毒散结的作用。
桂枝,味辛、甘,性温。归肺、心、膀胱经。发汗解表、散寒止痛、通阳化气。
白芍,为毛茛科植物芍药Paeonia tacti lora Pall.的干燥根。味苦、酸,性微寒。养血调经,敛阴止汗,柔肝止痛,平抑肝阳。
莱菔子,为十字花科植物萝卜Raphanus sativus L.的干燥成熟种子。植物的干燥成熟种子。用于饮食停滞,脘腹胀痛,大便秘结,积滞泻痢,痰壅喘咳。
佛手,为芸香科植物佛手CitrusmedicaLvar.sarcodac-tylisSwingle的干燥果实。有疏肝理气,和胄止痛,燥湿化痰的功效。
本发明香苓开胃颗粒的制备方法包括以下步骤:
(1)粉碎各药物,过40目筛,取广藿香、苍术、丁香、佩兰,加6-10倍量的水蒸馏,提取挥发油;以获得挥发油6-10倍的β-CD 50℃包合2-5小时,自然冷却后置冰箱中冷藏12-36小时,抽滤并置干燥器中干燥24-72小时,得包合物,备用;
(2)取广藿香、苍术、丁香、佩兰药渣与其余药材采用加水煎煮两次,第一次加4-8倍量水,煎煮1-3小时;第二次加3-6倍量水,煎煮0.5-1.5小时,合并煎液,滤过,减压浓缩至膏状,加乙醇至药液含醇量达50-90%,搅拌后静置12-36小时,取上清液滤过,滤液回收乙醇至无醇味,真空干燥,得干浸膏,粉碎成细粉,加入步骤(1)获得的包合物,加糊精和蔗糖适量制成颗粒。
优选的,本发明香苓开胃颗粒的制备方法包括以下步骤:
(1)粉碎各药物,过40目筛,取广藿香、苍术、丁香、佩兰,加8倍量的水蒸馏,提取挥发油;以获得挥发油8倍的β-CD 50℃包合5小时,自然冷却后置冰箱中冷藏24小时,抽滤并置干燥器中干燥48小时,得包合物,备用;

所述的香苓开胃颗粒,还可以根据实际需要制备为口服液、丸、散、滴丸、胶囊等剂型。

实施例1香苓开胃颗粒的制备实施例
取广藿香28g,茯苓28g,苍术15g,丁香15g,鸡内金15g,佩兰10g,法半夏20g,兰花参8g,山土瓜12g,薏苡仁5g,桂枝5g,白芍5g,莱菔子5g,佛手5g。
(1)粉碎各药物,过40目筛,取广藿香、苍术、丁香、佩兰,加8倍量的水蒸馏,提取挥发油;以获得挥发油8倍的β-CD 50℃包合5小时,自然冷却后置冰箱中冷藏24小时,抽滤并置干燥器中干燥48小时,得包合物,备用;
(2)取广藿香药渣与其余药材采用加水煎煮两次,第一次加8倍量水,煎煮2小时;第二次加6倍量水,煎煮1小时,合并煎液,滤过,减压浓缩至膏状,加乙醇至药液含醇量达80%,搅拌后静置24小时,取上清液滤过,滤液回收乙醇至无醇味,真空干燥,得干浸膏,粉碎成细粉,加入步骤(1)获得的包合物,加糊精和蔗糖适量制成颗粒。
实施例2香苓开胃片剂的制备实施例
取广藿香26g,茯苓25g,苍术18g,丁香12g,鸡内金12g,佩兰8g,法半夏15g,兰花参10g,山土瓜12g,薏苡仁5g,桂枝5g,白芍5g,莱菔子5g,佛手5g。
(1)粉碎各药物,过40目筛,取广藿香、苍术、丁香、佩兰,加8倍量的水蒸馏,提取挥发油;以获得挥发油8倍的β-CD 50℃包合5小时,自然冷却后置冰箱中冷藏24小时,抽滤并置干燥器中干燥48小时,得包合物,备用;
(2)取广藿香药渣与其余药材采用加水煎煮两次,第一次加8倍量水,煎煮2小时;第二次加6倍量水,煎煮1小时,合并煎液,滤过,减压浓缩至膏状,加乙醇至药液含醇量达80%,搅拌后静置24小时,取上清液滤过,滤液回收乙醇至无醇味,真空干燥,得干浸膏,粉碎成细粉,加入步骤(1)获得的包合物,混合均匀后加入适量淀粉制粒、干燥,混匀,压制成片,包衣。
实施例3香苓开胃胶囊的制备实施例
取广藿香25g,茯苓20g,苍术25g,丁香8g,鸡内金15g,佩兰15g,法半夏12g,兰花参12g,山土瓜15g,薏苡仁5g,桂枝5g,白芍5g,莱菔子5g,佛手5g。
(1)粉碎各药物,过40目筛,取广藿香、苍术、丁香、佩兰,加8倍量的水蒸馏,提取挥发油;以获得挥发油8倍的β-CD 50℃包合5小时,自然冷却后置冰箱中冷藏24小时,抽滤并置干燥器中干燥48小时,得包合物,备用;
(2)取广藿香药渣与其余药材采用加水煎煮两次,第一次加8倍量水,煎煮2小时;第二次加6倍量水,煎煮1小时,合并煎液,滤过,减压浓缩至膏状,加乙醇至药液含醇量达80%,搅拌后静置24小时,取上清液滤过,滤液回收乙醇至无醇味,真空干燥,得干浸膏,粉碎成细粉,混合均匀后加入适量微粉硅胶和糊精,用95%乙醇制软材,过筛制粒,干燥,整粒,加入步骤(1)获得的包合物,混匀,装胶囊。
实施例4治疗小儿厌食症效果试验
将150例符合纳入标准的脾为湿困型厌食患儿随机分为治疗组和对照组各50例。治疗组口服香苓颗粒,对照1组用吗丁啉,对照2组口服下述颗粒(太子健颗粒):用太子参25g、白术25g、波蔻25g、枳壳25g、焦山楂20g、陈皮20g、肉豆蔻20g、鸡内金20g、佛手10g、白芍10g、神曲10g、益智仁10g、补骨脂10g;取上述药材加水煎煮两次,第一次加4-10倍量水,第二次加水4-10倍量,每次煎煮1-4小时,过滤,收集滤液,浓缩,干燥,加糊精和蔗糖适量制成颗粒。
三组均连续治疗2周。治疗组和对照组患儿在年龄、性别、病程等方面比较差异无统计学意义(P>0.05),具有可比性。治疗组总有效率98%,对照1组总有效率82%,对照2组总有效率80%,治疗组总疗效优于对照组,差异有统计学意义(P<0.05)。
另外,完成小鼠推进排空率和小肠推进率实验。与模型对照组比较,香苓开胃颗粒组能明显提高小鼠胃排空率和小肠推进率(P<0.01);吗丁啉组可明显降低小鼠胃排空率和小肠推进率(P<0.01),而太子健颗粒组能提高小鼠的小肠推进率(P<0.01),但对小鼠胃排空率无明显作用(P>0.05)。此外,香苓开胃颗粒组能提高正常大鼠的胃蛋白酶活力(P<0.01),促进大鼠胆汁分泌(P<0.01)。而吗丁啉组和太子健颗粒组均无上述作用(P>0.05)。
实施例5对大鼠厌食模型的效果试验
将Wistar大鼠随机分为4组,每组10只。实验前五周,每天均采用模拟病因的方法,用鱼松、奶粉、玉米粉、黄豆粉、白糖、鸡蛋、鲜肥猪肉等市售产品按一定比例制备特制饲料喂养。每日实验进行之前记录前日大鼠全天进食量和当日体重。以模型组大鼠出现进食量低于正常组40-60%,体重低于正常组10-15%,但无腹泻、拱背、蜷卧等显著脾虚症状,为造模成功的标志。
将各组大鼠每日灌胃给药,剂量为2.5mL/100g,至第22日处死,记录每组大鼠的每日进食量。
治疗组采用实施例1制备的香苓开胃颗粒灌胃给药;
阳性对照组采用吗丁啉,灌胃给药,剂量:100mg/100g;
正常组和模型组均采用生理盐水灌胃给药。
在灌胃之前记录每日大鼠饲料剩余量,当日饲料加入量,按照:每日大鼠摄食量=前日饲料加入量-当日饲料剩余量,计算每天每只大鼠的摄食量,并做前后和平行对比变化。结果如下表所示:

结果证实:实验开始时,四组的大鼠的摄食量均无统计学差异。而从第7天时,模型组摄食量明显下降,显示造模成功。同时,第7天时开始到第21天的治疗组合阳性对照组的摄食量均明显高于模型组,并且第14天和第21天,P<0.01。
实施例6典型病例举隅
病案一:
唐某,男,8岁,2016年4月10日
纳差2周,2周前无明显诱因出现纳差,未系统治疗,现纳差,无腹痛,偶有干呕,大便干,小便常,舌质淡苔白。
查:心肺(-),咽(-)
拟方:香苓开胃颗粒。
开水煎,6剂而愈。
病案二:
张某,女,11岁。2015年10月15日。
纳差3月余。挑食,眠可,大便干结,状如羊粪,1-2日一行,晨起口有异味,舌质红抬黄腻。
查:心肺(-),咽(+)
拟方:香苓开胃颗粒。
开水煎,6剂而愈。
病案三:
谭某,女,9岁。2014年7月3日。
纳差1月余,挑食,多食则腹胀,眠中易惊醒,会发出怪叫,大便调,1日1行,小便调,舌质红苔腻。
查:咽红(+),双侧扁桃体Ⅱ°肿大,心肺(-)
拟方:香苓开胃颗粒。
开水煎,6剂而愈。
病案四:
安某,女,3岁。2013年4月20日。
纳差流汗1周。1周前患儿因“疱疹性咽炎”出现发热,经口服抗炎药后症状好转。现证见盗汗,纳差,脸色偏黄,大便干,小便调,眠可,舌质红苔白。
拟方:香苓开胃颗粒。
开水煎,6剂而愈。
病案五:
陈某,男,5岁。2012年5月26.
纳少2年余。厌食油腻,有口臭,易感冒,夜间磨牙,汗多,偶有打鼾,偶有鼻出血,心烦易怒,眠欠安,大便臭秽,粘稠小便黄,舌质红,苔黄腻。
查:心肺(-),咽(-)
拟方:香苓开胃颗粒。
开水煎,6剂而愈。
Claims (10)
1.香苓开胃颗粒,其原料药由以下组分组成:广藿香5-30g,茯苓5-30g,苍术5-30g,丁香5-30g,鸡内金5-30g,佩兰5-30g,法半夏5-30g,兰花参5-30g,山土瓜5-30g,薏苡仁5-30g,桂枝5-30g,白芍5-30g,莱菔子5-30g,佛手5-30g。
2.如权利要求1所述的香苓开胃颗粒,各组分的重量为:广藿香20-28g,茯苓20-28g,苍术10-25g,丁香10-25g,鸡内金5-15g,佩兰5-15g,法半夏10-25g,兰花参5-15g,山土瓜5-15g,薏苡仁5-10g,桂枝5-10g,白芍5-10g,莱菔子5-10g,佛手5-10g。
3.如权利要求1所述的香苓开胃颗粒,各组分的重量为:广藿香28g,茯苓28g,苍术15g,丁香15g,鸡内金15g,佩兰10g,法半夏20g,兰花参8g,山土瓜12g,薏苡仁5g,桂枝5g,白芍5g,莱菔子5g,佛手5g。
4.一种权利要求1-3任一项所述的香苓开胃颗粒的制备方法,包括如下步骤:

(2)取广藿香药渣与其余药材采用加水煎煮两次,第一次加8倍量水,煎煮2小时;第二次加6倍量水,煎煮1小时,合并煎液,滤过,减压浓缩至膏状,加乙醇至药液含醇量达80%,搅拌后静置24小时,取上清液滤过,滤液回收乙醇至无醇味,真空干燥,得干浸膏,粉碎成细粉,加入步骤(1)获得的包合物,加糊精和蔗糖适量制成颗粒。
6.权利要求1-3任一项所述的香苓开胃颗粒,将颗粒剂替换为膏、丹、口服液、散、滴丸或胶囊剂型。
7.一种香苓开胃组合物,其原料药由以下组分组成:广藿香5-30g,茯苓5-30g,苍术5-30g,丁香5-30g,鸡内金5-30g,佩兰5-30g,法半夏5-30g,兰花参5-30g,山土瓜5-30g,薏苡仁5-30g,桂枝5-30g,白芍5-30g,莱菔子5-30g,佛手5-30g。
8.如权利要求7所述的香苓开胃组合物,各组分的重量为:广藿香20-28g,茯苓20-28g,苍术10-25g,丁香10-25g,鸡内金5-15g,佩兰5-15g,法半夏10-25g,兰花参5-15g,山土瓜5-15g,薏苡仁5-10g,桂枝5-10g,白芍5-10g,莱菔子5-10g,佛手5-10g。
9.如权利要求8所述的香苓开胃组合物,各组分的重量为:广藿香28g,茯苓28g,苍术15g,丁香15g,鸡内金15g,佩兰10g,法半夏20g,兰花参8g,山土瓜12g,薏苡仁5g,桂枝5g,白芍5g,莱菔子5g,佛手5g。
10.权利要求7-9任一项所述的香苓开胃组合物在制备治疗小儿厌食症药物中的用途。
